Purpose: This study aim to determine the occurrence of gestational diabetes mellitus (GDM) inpregnancies after IVF treatment and to evaluate the factors that reduce this risk.Material and Methods: This retrospective cohort study was conducted using the medical records ofpregnant women who conceived following IVF at the in-vitro fertilization center between 2002–2019. Thedata were obtained from medical records and phone interviews. Univariate and multivariate logisticregression analyses were performed.Results: The incidence of GDM was found to be 16.7%. The regression model indicated that the riskof GDM was 4.57 times higher in the age group 36–40 at conception during the IVF cycle than the agegroup 31–35 (95% CI = 1.18–17.73, p = .028). Furthermore, women who conceived after the secondIVF trial had a risk of GDM 3.464 times higher than those that conceived after their first IVF trial (95%CI = 1.07–11.23, p= .038).Conclusion: As age and number of IVF trials increase in infertile women, the risk of GDM increases afterIVF treatment.Keywords: Gestational diabetes mellitus, in vitro fertilization, risk factors

Yöntem:Leflunomid kan düzeyinin düşürülmesi için gebelikte kolestiramin şelasyon protokolünün uygulanmasıBulgular:IVF yöntemi ile oluşan ilk gebeliği sezaryen doğum ile sonlanan, bilinen hipotiroidi ve romatoid artrit hastalıkları nedeniyle tedavi almakta olan hasta leflunomid kullanırken spontan gebe kalması sonrası başvurdu. İlk muayenede 7 hafta 4 gün ile uyumlu gebelik izlendi. X grubu ilaç kullanımına rağmen gebelik terminasyonu istemeyen hasta farmakoloji ve romatoloji bölümlerine danışıldı. Farmakoloji raporunda, leflunomid kullanımının gebelikte kontrendike olduğu, gebelik gelişirse ilacın kesilmesi, fetal zarar açısından hasta değerlendirilmesi ve plazma leflunomid konsantrasyonunu 0.02 mg/L altına indirmek için hızlandırılmış ilaç eliminasyon prosedürü başlatılması gerektiği belirtildi. Hastanın bakılan ilk serum leflunomid düzeyi 9 mg/L olarak ölçüldü. Hastaya 3x8 gram kolestiramin şelasyonu 11 gün süre ile verildi. Sonrasında serum leflunomid düzeyi 1,21 mg/L olarak ölçüldü. Hastaya şelasyon tedavisinin devamı planlandı. Hastanın ikinci küre devam etmek istememesi üzerine tekrar serum leflunomid düzeyi ölçülemedi ve rutin gebelik takibine devam edildi. Fetal büyüme takibi miada kadar olağanseyreden hasta, 24. Haftada gestasyonel diyabet tanısı aldı ve insülin tedavisi başlandı. 32. Hafta sonrası trombositopeni gelişen hastada etyoloji araştırması sonrası gestasyonel trombositopeni tanısı teyit edildi. 37. Gebelik haftasında sezaryen ile 2960 gram canlı kız bebek doğdu. Yenidoğan muayenesi ve takibi sonrası bebekte patolojik bir durum saptanmadı. Maternal trombositopeni doğum sonrası süreçte dramatik olarak düzeldi.Sonuç:Otoimmun hastalıkların tedavisinde yer alan ilaçların pek çoğu gebelik sürecinde kontraendikedir. Tedavi altında iken oluşan gebeliklerin terme sağlıkla ulaşabilmesi için ilaç eliminasyon prosedürü kullanılabilmektedir. Ancak yine de olası riskleri azaltmak için gebelik planlayan hastaların tedavi düzenlemesi ilaç özelliklerine göre uygun zamanda ve dozda yapılmalıdır.Anahtar Kelimeler:kolestiramin, leflunomid, riskli gebelik yönetimi, romatoid artrit, şelasyon Methods: Application of a cholestyramine chelation protocol in pregnancy to reduce leflunomide blood levelsResults: A patient whose first IVF pregnancy ended with cesarean delivery and who was receiving treatment for known hypothyroidism and rheumatoid arthritis diseases presented after spontaneous pregnancy while taking leflunomide. At the first examination, a pregnancy compatible with 7 weeks and 4 days was observed. The patient who did not want termination of pregnancy despite the use of group X drugs was consulted to pharmacology and rheumatology departments. The pharmacology report stated that the use of leflunomide was contraindicated in pregnancy, the drug should be discontinued if pregnancy developed, the patient should be evaluated for fetal harm and an accelerated drug elimination procedure should be initiated to reduce the plasma leflunomide concentration below 0.02 mg/L. The patient's first serum leflunomide level was measured as 9 mg/L. The patient was given 3x8 gram cholestyramine chelation for 11 days. Afterwards, serum leflunomide level was measured as 1.21 mg/L. The patient was planned to continue chelation treatment. Since the patient did not want to continue with the second cycle, serum leflunomide level could not be measured again and routine pregnancy follow-up was continued. Fetal growth was normal until mida, but gestational diabetes was diagnosed at 24 weeks and insulin treatment was started. After 32 weeks of gestation, thrombocytopenia developed and the diagnosis of gestational thrombocytopenia was confirmed after etiology investigation. At 37 weeks of gestation, a 2960 gram live baby girl was born by cesarean section. After neonatal examination and follow-up, no pathologic condition was found in the baby. Maternal thrombocytopenia improved dramatically in the postpartum period.Conclusion: Many of the drugs used in the treatment of autoimmune diseases are contraindicated during pregnancy. Drug elimination procedures can be used to ensure that pregnancies that occur during treatment reach term in good health. However, in order to reduce the possible risks, the treatment arrangement of patients planning pregnancy should be made at the appropriate time and dose according to the drug properties.Keywords: cholestyramine, leflunomide, risky pregnancy management, rheumatoid arthritis, chelation&nbsp

Ovaryan hiperstimülasyon sendromu (OHSS)polikistik over sendromu (PCOS) hastalarında görülen ve gonadotropinlere overinaşırı yanıt vermesiyle oluşan, ciddi morbiditesi olabilen iyatrojenik birdurumdur. Sendrom hCG enjeksiyonu sonrasında VEGF'nin tetiklediği vaskülergeçirgenlik artışına bağlı 3. boşluklarda (periton, plevra gibi) proteindenzengin sıvı birikimi ve sıvı-elektrolit dengesizlikleri ile karakterizedir.Endojen hCG'nin yükseldiği erken gebelik döneminde daha da şiddetlenebilir. Patofizyolojisitam olarak anlaşılamadığı için tedavisi de destekleyici ve semptomatiktir. Yüksekriskli hastalarda OHSS'yi önlemek için uzun yıllardır kullanılan bir çok tedaviyöntemi bulunmaktadır. Ancak antagonist sikluslarında son oosit matürasyonuiçin hCG yerine GnRH agonisti ile tetiklemeyapılması ve ardından oluşan tüm embriyoların dondurularak daha sonraki birsiklusda transfer edilmesine dayalı "hepsinidondur" yaklaşımı ile ciddi OHSS neredeyse elimine edilmiştir. Yine deOHSS'nin geliştiği durumlarda hastalardaki ciddi morbiditeleri önlemek amacıylaparasentez, sıvı elektrolit tedavisi ve tromboemboli profilaksisi gibitedaviler yapılmalıdır Ovarian hyperstimulation syndrome (OHSS) is an iatrogenic condition,which occurs in polycystic ovarian syndrome patients as an excessive responseto gonadotropin stimulation in assisted reproduction cycles. It ischaracterized by protein rich fluid accumulation in the 3rd space (peritoneumand pleura) due to increased vascular permeability triggered by VEGF after hCGinjection and may cause severe morbidity. The symptoms may aggravate afterendogenous hCG rise during early pregnancy. The pathophysiology is not very clear;therefore treatment is supportive and symptomatic. Over the years, manytreatment strategies are developed to prevent OHSS. However, "freezeall" strategy, that is triggering with GnRH agonists for final oocytematuration in replace of hCG in antagonist IVF cycles, and then cryopreservingall the embryos to transfer in subsequent cycles has almost eliminated thesyndrome. However, when severe OHSS develops treatment plan should include paracentesis,correction of fluid and electrolyte imbalances and prophylaxis fortromboembolic events.&nbsp

A 36 year old woman with 2 pregnancies which one ofthem is delivered by cesarean section 7 year ago and the other one was abortusapplied for periodic hematuria (menuria) and chronic pelvic pain. Cystoscopywas performed and bladder mucosal biopsy was obtained because of suspiciousbladder mass in abdominopelvic ultrasonography. Pathological diagnosis wasbladder endometriosis. Gonadotrophin releasing hormon analog ( leuprolideasetat) treatment was used for 6 months. 2 months after therapy she appliedagain with pelvic pain. Diagnostic laparoscopy and second look cystoscopy wasperformed and regression of the endometriotic foci were seen. Pain and palpablemass out of pelvis may be a symptom of extrapelvic endometriosis although it isfrequently asymptomatic. Urinary endometriosis is an unrecoverabledisease, and collective approach of the gynecologist and urologist is necessaryfor treatment options.Theseverity of symptoms, spread of disease, probability of recurrences and cost oftreatment shoud be considered and accurate modality of the treatment shoud beoffered to the patient., Otuz altı yaşında, gebelik 2 doğum 1 abortus 1 ve 7yıl önce sezeryan öyküsü bulunan hasta siklik hematüri (menüri) ve kronikpelvik ağrı yakınması ile başvurdu. Abdominopelvik ultrasonografide mesanedekitle şüphesiyle sistoskopi yapılarak biopsi alındı. Patolojik tanı mesanedeendometriozis idi. Gonadotropin releasing hormon analoğu yakınması (leuprolideasetat) tedavisi 6 ay uygulandı. Tedaviden 2 ay sonra kasık ağrısı şikayeti iletekrar başvurdu. Tanısal laparoskopi ve ikinci bakış sistoskopisi yapıldı veendometriozis odaklarının regrese olduğu saptandı. Sıklıkla asemptomatik olsada, ağrı ve pelvis dışında ele gelen bir kitlenin varlığı ekstrapelvikendometriozisin semptomları olabilir. Üriner endometriozis gibi sağaltımı zorbir hastalıkta tedavi basamaklarına kadın doğum ve üroloji uzmanları birliktekarar vermelidir. Yakınmaların şiddeti, hastalığın yaygınlığı, tekrarlamaolasılığı ve tedavi maliyeti göz önüne alınarak doğru tedavi yöntemi hastayaönerilmelidir.

Amaç: Bu çalışmada 39 yaş ve üzeri infertil kadın hastalarda yaşın, prognostik faktörler ile üreme potansiyeli üzerindeki sonuçlarının değerlendirilmesi amaçlandı. Gereç ve Yöntemler: İleri yaşta (≥39 yıl) 107 infertil kadın hasta retrospektif olarak değerlendirildi. Hastaların insan koryonik gonadotropin (hCG) uygulanan günde ölçülen endometrium kalınlığı, toplam kullanılan gonadotropin dozu, stimülasyon süreleri, 3. gün bazal follikül stimüle edici hormon (FSH) ile bazal estradiol (E2) düzeyleri, toplanan oosit ile fertilize oosit sayıları ve transfer edilen embriyo sayıları kaydedildi. Bu parametrelerin gebelik başarısı üzerine etkileri değerlendirildi. Hastalar 39 yaş (n=47), 40-41 yaş (n=37) ve 42 ve üzeri yaş (n=23) olmak üzere üç yaş grubu karşılaştırıldı. Bulgular: Canlı doğum oranı 39 yaş grubunda %23,2 ve 40-41 yaş grubunda %14,7 olarak gözlendi. Kırk iki ve üzeri yaş grubunda ise canlı doğum görülmedi. Bu çalışmanın sonuçları yaşın, toplanan oosit ve fertilize oosit sayısının gebelik gelişimi üzerine etkisinin olduğunu gösterdi. Sonuç: Gebelik oranı 39 yaşından sonra hızlı bir şekilde düşmektedir; intrasitoplazmik sperm enjeksiyonu sonrası, hastalarda 39 yaşından itibaren gebelik başarısı 3 yıl sonra 9 kat azalmıştır. İleri maternal yaşın, gebelik ve canlı doğum oranları üzerindeki olumsuz etkisi klinik çalışmamızda da gözlenmiştir. Objective: The objective of this study is to investigate the effect of age on the prognostic factors and fertilization potential of infertile women who are 39 years or older. Material and Methods: 107 patients with advanced maternal age (≥39 years) were included in this retrospective study. The endometrial thickness on the day of human chorionic gonadotropin (hCG), total dose of gonadotropin used, duration of stimulation, levels of day-3 basal follicule stimulating hormone (FSH) and basal estradiol (E2), number of retrieved oocytes and fertilized oocytes, and number of embryos transferred were recorded. These parameters were used to evaluate their effects on the success of pregnancy. Patients were categorized into three age groups: 39 years old (n=47), 40-41 years old (n=37), and ≥ 42 years old (n=23) and age groups were compared. Results: The rate of live birth was %23.2 in 39 years and %14.7 in 40-41 years. No live birth in 42 years or older patients was recorded. The results revealed that that the age, number of retrieved oocytes and fertilized oocytes predict the success rate of pregnancy. Conclusion: Pregnancy rate dramatically decreased after the age of 39; the success of pregnancy after the intracytoplasmic sperm injection decreases 9 times, three years later starting age of 39. The negative effect of advanced maternal age on pregnancy and live birth rates was observed in our clinical study, as well.

We report a case of adenomyoma of endocervical type that occurred in a 28-year-old-woman. The patient presented with a mass protruding into vagina which was treated by polypectomy. Histologically, tumor was composed of a mixture of proliferating glands of endocervical type and fascicles of smooth muscle cells. There were no architectural abnormalities. Both the epithelium and smooth muscle were uniformly bland. No mitotic activity was observed in the epithelial and stromal component. Polypectomized specimen was diagnosed as adenomyoma of endocervical type. This tumor type should be added to the list of a differential diagnosis of uterine endocervical tumor.
