BACKGROUND: Severe acetabular bone loss in revision total hip arthroplasty (RTHA), both with or without pelvic discontinuity, remains a great challenge in orthopaedic surgery. OBJECTIVE: The aim of this study was to evaluate risk factors for failure of custom-made acetabular implants in RTHA. METHODS: Seventy patients with severe acetabular bone loss (Paprosky Type III) and pelvic discontinuity, who required RTHA, were included in our study. All prostheses were constructed based on a thin-layer computed-tomography (CT) scan of the pelvis. The treatment was considered unsuccessful in the event of periprosthetic joint infection (PJI) or aseptic loosening (AL) with need for explantation of the custom-made acetabular implant. RESULTS: The average follow-up was 41.9 ± 34.8 months (range 1.5–120). Implant survival at last follow-up was 75.7% (53 of 70). Explantation was necessary in 17 cases (15 PJI; 2 AL). Previous PJI as reason for RTHA (p= 0.025; OR 3.56 (95% CI: 1.14; 11.21)), additional revision of femoral components (p= 0.003; OR 8.4 (95% CI: 1.75; 40.42)), rheumatoid disease (p= 0.039; OR 3.43 (95% CI: 1.01; 11.40)), elevated preoperative CRP > 15.2 mg/l (p= 0.015; AUC: 0.7) and preoperative haemoglobin < 10.05 (p= 0.022; AUC: 0.69) were statistically significant risk factors associated with treatment failure. Age and BMI were not statistically significant contributing to implant failure. CONCLUSION: Risk factors for treatment failure were a previous PJI, additional revision of femoral component, rheumatoid disease, elevated preoperative CRP and low preoperative haemoglobin. Awareness of these risk factors will help to improve future treatment standards.

Purpose Epileptic seizures can cause multiple shoulder injuries, the most common of which are dislocations, recurrent instability, fractures, and isolated lesions of the rotator cuff. Currently, only limited literature exists which describes the frequency and types of lesions in cohorts of epileptic patients and the corresponding treatment outcome. This study aims to document the occurrence of shoulder lesions in patients affected by seizures and to provide detailed information on trauma dynamics, specific lesion characteristics and treatment complications. Methods All patients referring to a tertiary epilepsy center were screened for shoulder injuries and the clinical records of those sustaining them during a seizure were reviewed. Demographic information, lesions’ characteristics and trauma dynamics were analysed, as wells as—when carried out—the type of surgical intervention and any postoperative complications. Results The average age at the time of injury of 106 included patients was 39.7 ± 17.5 years and a male predominance was recorded (65%). Bilateral injuries occurred in 29 patients, simultaneously in 17 cases. A younger age, bilateral shoulder injuries and shoulder dislocations were significantly associated with the occurrence of a shoulder injury solely by muscular activation (p = 0.0054, p = 0.011, p Conclusions Uncontrolled muscle activation during a seizure is a distinctive but not exclusive dynamic of injury in epileptic patients, accounting for more than the half of all shoulder lesions, especially in the younger. This can lead both to anterior and posterior dislocations or fracture-dislocations and is frequently cause of bilateral lesions and of instability recurrence after surgery. The high complication rates after surgical treatment in this selected subgroup of patients require that appropriate preventative measures are taken to increase the probability of treatment success. Purpose In view of our aging society, co-management with a geriatrician is becoming increasingly important. While such collaborations have been working successfully in trauma surgery for years, it is still unclear whether they are also helpful for non-trauma patients in orthopedics. The aim of this study was to investigate the effect of such a cooperation in orthopedic non-trauma patients with native and periprosthetic joint infections on the basis of five key areas. Results Analysis was carried out with 59 patients “with” and 63 “without” geriatric co-management. In the co-management group, delirium was detected significantly more often (p Conclusion Orthogeriatric co-management in orthopedic patients with native and periprosthetic joint infections and nontraumatic surgery appears to have positive impacts on recognition and treatment of delirium, pain management, transfer performance, and attention to renal function. Further studies should follow in order to conclusively assess the value of such co-management in orthopedic nontraumatic surgery patients.

ZusammenfassungDie Vorteile von Titanlegierungen gegenüber den herkömmlichen Stahl-, Kobalt-Chrom- und Chrom-Nickel-Legierungen als Material für Endoprothesen sind eine gute Dauerschwingfestigkeit, eine exzellente Biokompatibilität, ein niedrigeres Elastizitätsmodul und eine hohe Korrosionsbeständigkeit. Teilweise schlechte klinische Ergebnisse, vermutlich ausgelöst durch Spaltkorrosion, haben jedoch zu einer verminderten Benutzung geführt. Um einen sicheren Einsatz zu gewährleisten, sollte das Design optimiert werden (ausreichender proximaler Durchmesser, proximaler Kragen), um übermäßige Verformungen besonders im proximalen Anteil zu reduzieren, und auf eine raue Oberfläche (Ra > 2,5 µm) verzichtet werden. Eine Weiterführung der Forschung über Oberflächenbearbeitung, wie z. B. eine Silikat-/Silan-Beschichtung, könnte weitere Verbesserungen bewirken.

Introduction Acetabular defect recognition and classification remains a challenging field of practice for orthopedic surgeons. Recently, the Acetabular Defect Classification (ADC) has been introduced to provide a reliable, reproducible and intuitive classification system. In order to improve ease of use and efficiency of the ADC, a browser-based application has been created. We hypothesized that the ADC application can improve rating performance of non-specialists (medical students) to achieve good inter- and intra-rater agreement and will compare favorable to the results of specialists (experienced surgeons) without the help of the application. Materials and methods The ADC is based on the integrity of the acetabular rim and the supporting structures. It consists of four main types of defects ascending in severity. These defects are further subdivided in A–C, narrowing down defect location. 80 randomized radiographs were graded according to ADC by three non-specialists (medical students) with help of the ADC application and by three specialists (orthopedic surgeons) without help of the application to evaluate the difference in inter-rater agreement between groups. To account for intra-rater agreement, the rating process was repeated after a reasonable wash-out period. Results Inter-rater and intra-rater agreement within the non-specialist group rated lower when compared to the specialist group while still falling into the good agreement range. The student group presented with k values of 0.61 for inter-rater agreement and 0.68 for intra-rater agreement, while the surgeon group displayed k values of 0.72 for inter-rater agreement and 0.83 for intra-rater agreement. Conclusion The app-guided assessment of acetabular defects offers a promising innovative approach to simplify complex situations. It makes the challenging field of acetabular revision arthroplasty more approachable especially for less experienced surgeons and offers insight and guidance in the planning stage as well as intra-operative setting.

Wiederherstellung des Streckapparates bei fortgeschrittenem Sehnendefekt im Rahmen der Revisionsendoprothetik des Kniegelenkes. Rekonstruktion und Augmentation mittels vaskularisiertem Muskel- und Sehnengewebe als gefasgestielte Augmentationsplastik. Fortgeschrittene Degeneration des Streckapparates (Patellasehne; Quadrizepssehne) mit und ohne Kontinuitatsunterbrechung. Persistierende Infektion oder anstehende Revision einer einliegenden Knietotalendoprothese (TEP). Schadigung von Gastrocnemius, Soleus oder Achillessehne. Erweiterung des Zuganges uber den Unterschenkel medial, Praparation des M. gastrocnemius medialis, Auftrennen der Raphe und Praparation der Sehne vom Soleusanteil. Weit distale Abtrennung der Sehne, Transposition auf den Defekt. Einschlagen der Sehne und Augmentation bzw. Rekonstruktion des Defektes, der Muskelbauch dient dabei der suffizienten Bedeckung der Sehne wie auch des ventralen Kniegelenkes. Meshdeckung des Muskels. Immobilisation von Knie und Sprunggelenk bis zur Einheilung der Meshgraft fur 10 Tage. Schrittweise Steigerung der Bewegung mit 30°/60°/90° alle 2 Wochen. Belastung mit gestrecktem Knie, keine Belastung bei gebeugtem Knie fur 6 Wochen postoperativ. Nachuntersucht wurden insgesamt 9 Patienten nach orthopadisch notwendiger Wechselendoprothetik des Kniegelenkes (3 aseptisch, 6 septisch), von denen 3 eine vollstandige Ruptur der Patellasehne, 5 eine fortgeschrittene Destruktion des Streckapparates und 1 Patient eine vollstandiger Ruptur der Quadrizepssehne vorwiesen. Der mittlere Nachuntersuchungszeitraum betrug 11,2 Monate. Bereits nach 6 Monaten zeigte das nachuntersuchte Kollektiv ein gutes funktionelles Ergebnis mit aktiver Streckung des Kniegelenkes sowie eine Stand- und Gangstabilitat.

Background There are different procedures for both, the diagnosis and the therapy of a periprosthetic joint infection (PJI), however, national or international guidelines for a standardised treatment regime are still lacking. The present paper evaluates the use of the predominant treatment protocols for PJI in certified total joint replacement centres (EPZ) in Germany based on an EndoCert questionnaire. Materials and methods The questionnaire was developed in cooperation with the EndoCert Certification Commission to survey the treatment protocols for septic revision arthroplasties in EPZ. Questions targeted the various treatment options including prosthesis preserving procedures (DAIR - Debridement, antibiotics, irrigation, and retention of the prosthesis), one-stage revision, two-stage revision, removal of the endoprosthesis and diagnostic sampling prior to re-implantation. All certified EPZ participated (n = 504) and the results from the current survey in 2020 were compared to data from a previous analysis in 2015. Results The number of centres that performed DAIR up to a maximum of 4 weeks and more than 10 weeks after index surgery decreased since 2015, while the number of centres that provided a one-stage revision as a treatment option increased (hip: + 6.3%; knee: + 6.6%). The majority of the centres (73.2%) indicated a 4–8 week period as the preferred interval between prosthesis removal and re-implantation in two-stage revision in hip as well as knee revisions. Centres with a higher number of revision surgeries (> 200 revisions/year), opted even more often for the 4–8 week period (92.3%). In two-stage revision the use of metal-based spacers with/without reinforcement with antibiotic-containing cement as an interim placeholder was significantly reduced in 2020 compared to 2015. There was also a clear preference for cemented anchoring in two-stage revision arthroplasty in the knee in 2020, whereas the majority of hip replacements was cementless. Additionally, in 2020 the number of samples for microbiological testing during the removal of the infected endoprosthesis increased and 72% of the centres took five or more samples. Overall, the number of EPZ with a standardised protocol for the procedure expanded from 2015 to 2020. Conclusion While there was a trend towards standardised therapeutic algorithms for PJI with more uniform choices among the centres in 2020 compared to 2015, the treatment often remains an individual decision. However, since a consistent treatment regime is of vital importance with an expected rise of total numbers of revision arthroplasties, uniform definitions with regard to comparability and standardisation are necessary for the further development of the EndoCert system.

Im Fachgebiet der Orthopadie und Unfallchirurgie besteht ein standiges Bestreben, die Ergebnisqualitat in der Endoprothetik zu verbessern. Um jedoch zielfuhrende Anderungen eruieren zu konnen, bedarf es einer dezidierten Dokumentation der aktuellen Ergebnisqualitat. Hierzu dienen die sog. externe Qualitatssicherung, die Auswertung von Routinedaten der Krankenkassen, die Analyse klinischer Studien und der Blick in nationale Register. Um zusatzlich eine Verbesserung der Versorgungsqualitat zu erreichen, wurden eine gesetzliche Vorgabe von Mindestmengen und von Seiten der Fachgesellschaft (Deutschen Gesellschaft fur Orthopadie und Orthopadische Chirurgie e. V., DGOOC) die Einhaltung von Prozess- und Strukturmerkmalen im Rahmen des Zertifizierungssystems EndoCert® eingefuhrt. Zur Bewertung der Schwere des orthopadisch-chirurgischen Eingriffs fehlt bisher ein valider Score zur Risikoadjustierung. Diesen gilt es, kunftig bezogen auf die patientenspezifische Anatomie zu entwickeln. Durch die Kombination von Zertifizierungsprozessen, der Erfassung und Auswertung externer, stationarer Qualitatssicherungsdaten und Qualitatssicherung mit Routinedaten und der Weiterentwicklung adaquater Mindestmengenregelungen kann es gelingen, die Ergebnisqualitat weiter nachhaltig zu verbessern.

In 2020, the SARS-CoV-2 pandemic necessitated an adjustment to the care structure in all medical facilities in Germany. In this study we analysed the effects of the SARS-CoV-2 pandemic using the data from the annual balance sheet of a university department and maximum care provider for orthopaedics and trauma surgery.Data analysis was performed on the clinic's performance parameters from 2019 and 2020. The analysis included 52943 outpatient and 7980 inpatient cases, together with the Case Mix Index (CMI), and the costs and revenues.As a result of the reduction in scheduled surgeries, the mean number of inpatients in March and April 2020 decreased by 10.4% (p = 0.004). Due to the focus on complex cases, the CMI increased by 29.5% (p = 0.007). The number of outpatient surgical procedures increased by 16.9% (n = 84). Rising costs of consumables due to the purchase of protective equipment were offset by a reduced need for orthopaedic implants.By reducing the number of elective inpatient surgical treatments, the focus was shifted to treating complex cases; this is reflected in the increased CMI. Due to internal compensation mechanisms, the clinic was able to conserve intensive care capacities while still achieving an increase in revenue.EINFüHRUNG: Im Jahr 2020 erforderte die SARS-CoV-2-Pandemie eine Anpassung der Versorgungsstruktur in allen medizinischen Einrichtungen in Deutschland. Preoperative optimization of the therapeutic regime of comorbidities can lead to an improvement of the postoperative outcome and has the potential to reduce the financial burden on the health care system in revision hip arthroplasty. Patient-related factors and an increasing incidence of comorbidities lead to a higher risk of implant failure and revision for all causes. Important and potentially modifiable risk factors like preoperative anemia, coagulopathy, infectious disease (dental status, urinary tract infections, colonization with staphylococcus), metabolic conditions (obesity, malnutrition, diabetes mellitus, osteoporosis), and smoking need to be addressed. To achieve an optimal preoperative condition a multidisciplinary approach should be applied.Eine Optimierung der Behandlung von Komorbiditäten vor endoprothetischen Wechseleingriffen am Hüftgelenk hat das Potenzial für eine Verbesserung der Ergebnisse und eine Reduzierung der Kosten für das Gesundheitssystem. Nichtimplantatbezogene Einflussfaktoren, wie z. B. Adipositas, führen mit steigender Inzidenz zu einer deutlichen Erhöhung der Ausfallwahrscheinlichkeit. Wichtige und potenziell beeinflussbare Komorbiditäten sind hierbei die präoperative Anämie, Koagulopathien, infektiologische Erkrankungen (sanierungsbedürftiger Zahnstatus, Harnwegsinfekte, Besiedelung mit Staphylokokken), Stoffwechselerkrankungen (Adipositas, Malnutrition, Diabetes mellitus, Osteoporose) und Nikotinabusus. Zur zielführenden Optimierung bedarf es in der Regel eines interdisziplinären Therapieansatzes, um den maximalen Erfolg zu generieren.

Video consultations could support patient care in hand surgery during social distancing in the COVID-19 era. According to the literature, images of hand and fingers can support telediagnosis in hand emergencies. We present this feasibility study on online video consultation in hand surgery.A structured examination was designed to query the medical history and examine motor skills, sensitivity, strength and function tests of the hand. Thirty examinations on both hands were carried out by online video consultation, then in direct contact and compared with each other.With 4560 evaluated range of movement of the hand and finger joints, there was a high correlation between the measurement methods of R = 0.995 (p 0.0001, confidence interval 0.9946 - 0.9954). In the video-based examination, 84.6% of the measured values showed a deviation of less than 5° compared to the direct examination, 92.8% less than 10° deviation. Good accordance was also found in the patient's medical history. An estimating examination of sensitivity, function and strength during video examination is feasible with simple auxiliary aids. Deficits are evident in the detection of scars, in function tests and the absence of haptic findings.Online video consultation allows hand examination with sufficient documentation of hand and finger movements (range of motion) and proper evaluation of symptoms. It cannot replace direct examination but complement patient care in hand surgery even beyond the current COVID-19 pandemic.Videosprechstunden können bei Social Distancing in der COVID-19-Pandemie die Patientenversorgung unterstützen. Während zur Einholung handchirurgischer Fach- und Zweitmeinungen über Bildbefunde eine gute Literaturlage vorliegt, fehlt diese für die Videosprechstunde. Dies soll mit der vorliegenden Machbarkeitsstudie zur handchirurgischen Befunderhebung per Video nachgeholt werden.Ein strukturierter Untersuchungsbogen wurde entworfen zur Abfrage der Anamnese und Untersuchung von Motorik, Sensibilität, Kraft sowie spezifischer Funktionstests an der Hand. 30 Untersuchungen an jeweils beiden Händen wurden per Videosprechstunde und nachfolgend in direktem Kontakt durchgeführt und miteinander verglichen.Bei 4560 Bewegungsumfängen an der Hand zeigte sich eine hohe Korrelation zwischen den Messmethoden von R = 0,995 (p 0,0001, Konfidenzintervall 0,9946 – 0,9954). Dabei zeigten bei der videobasierten Untersuchung 84,6% der Messwerte eine Abweichung von weniger als 5° im Vergleich mit der direkten Untersuchung, 92,8% weniger als 10° Abweichung. Eine gute Übereinstimmung zeigte sich auch bei der Anamnese. Die orientierende Untersuchung von Sensibilität, Funktion und Kraft ist mit einfachen Hilfsmitteln gut möglich in der Videosprechstunde. Defizite zeigen sich bei der Erkennung von Narben, bei differenzierten Funktionstests sowie durch das Fehlen haptischer Befunde.Die Untersuchung der Hand in der Videosprechstunde erlaubt eine sehr gute Untersuchung des Bewegungsumfanges an den Gelenken der Hand, die Aufnahme der Beschwerden und eine Eingrenzung der Befunde. Sie kann die traditionelle Fachsprechstunde mit direktem Kontakt nicht vollständig ersetzen, stellt jedoch eine sinnvolle Ergänzung dar und besitzt eine Berechtigung über die aktuelle COVID-19-Pandemie hinaus.

Due to the current COVID-19 pandemic, the German Health Ministry has issued restrictions applying to the field of orthopaedics and trauma surgery. Besides postponement of elective surgeries, outpatient consultations have been drastically reduced. Parallel to these developments, an increase in telemedical consultations has reflected efforts to provide sufficient patient care. This study aims to evaluate the feasibility of a clinical examination of the hip joint and pelvis by way of a telemedical consultation.Twenty-nine patients of a German university clinic were recruited and assessed in both telemedical and conventional examinations. Agreement between the two examinations was then assessed, and connections between the observed agreement and patient-specific factors such as age, BMI and ASA classification were investigated.The inspections agreed closely with a mean Cohen's kappa of 0.76 ± 0.37. Palpation showed adequate agreement with a mean Cohen's kappa of 0.38 ± 0.19. Function showed good agreement with a mean Cohen's kappa of 0.61 ± 0.26 and range of motion showed adequate agreement with a mean Cohen's kappa of 0.36 ± 0.19. A significant positive correlation was observed between the number of deviations in the different examinations and age (p = 0.05), and a significant positive correlation was shown between the number of non-feasible examinations and age (p 0.01), BMI (p 0.01) and ASA classification score (p 0.01).Inspection and function can be reliably evaluated, whereas the significance of palpation, provocation and measurement of range of motion is limited. The small sample size puts limitations on the significance of a statistically relevant correlation between patient-specific factors such as age, BMI and ASA classification score and valid and successful implementation of a telemedical examination. The authors recommend targeted patient selection. If, however, patients are being evaluated who are very old ( 75 years), obese (BMI 30) or with multiple comorbidities (ASA 3 and above), caution is advised. Large, prospective studies are needed in the future to fully validate telemedical consultations in the fields of orthopaedics and trauma surgery.A telemedical examination of the hip joint and pelvis can be performed with certain limitations. Patient-specific factors such as age, BMI, and extent of comorbidities appear to have a relevant impact on validity and execution of the examination. Patients with multiple comorbidities (ASA 3 and above), advanced age ( 75 years) or obesity (BMI 30) should, whenever possible, be examined in a conventional outpatient setting.Aufgrund der aktuellen COVID-19-Pandemie erfolgten Einschränkungen im Fach der Orthopädie und Unfallchirurgie durch das Gesundheitsamt. Neben der Verlegung von elektiven operativen Eingriffen erfolgte auch die drastische Reduktion der Sprechstunden. Hierdurch kam es zu einem deutlichen Anstieg von telemedizinisch durchgeführten Sprechstunden zur Gewährleistung der Patientenversorgung. Die Zielsetzung dieser Arbeit ist, die Umsetzbarkeit einer klinisch-orthopädischen Untersuchung der Hüfte und des Beckens durch telemedizinische Verfahren im Rahmen einer Videosprechstunde zu untersuchen.Im Rahmen der vorliegenden Auswertung wurden 29 stationäre Patienten eines deutschen Universitätsklinikums rekrutiert und im Rahmen einer Videosprechstunde und konventionell untersucht. Anschließend wurde die Übereinstimmung der erhobenen Befunde ausgewertet und Zusammenhänge mit patientenspezifischen Daten wie Alter, BMI und ASA-Klassifikation untersucht.Es zeigte sich eine gute Übereinstimmung der Inspektion mit einem durchschnittlichen Cohens Kappa von 0,76 ± 0,37, eine angemessene Übereinstimmung der Palpation mit einem durchschnittlichen Cohens Kappa von 0,38 ± 0,19, eine gute Übereinstimmung der Funktion mit einem durchschnittlichen Cohens Kappa von 0,61 ± 0,26 und eine angemessene Übereinstimmung der Bewegungsausmaße mit einem Cohens Kappa von 0,36 ± 0,19. Es zeigte sich ein signifikanter positiver Zusammenhang zwischen Anzahl der Abweichungen der verschiedenen Untersuchungen und Alter (p = 0,05) und ein positiver signifikanter Zusammenhang zwischen Anzahl der nicht durchführbaren Untersuchungen und Alter (p 0,01), BMI (p 0,01) und ASA-Klassifikationswert (p 0,01).Vor allem die Inspektion und Funktion sind gut untersuchbare Parameter während Palpation, Provokation und die Erfassung der Bewegungsausmaße eine limitierte Aussagekraft zeigen. Bei begrenzter Anzahl der Probanden ist die Aussagekraft eines statistisch relevanten Zusammenhangs zwischen patientenspezifischen Faktoren wie Alter, BMI und ASA-Klassifikationswert und der validen und erfolgreichen Durchführung einer telemedizinischen Untersuchung limitiert. Die Autoren sprechen sich für eine gezielte Patientenselektion aus. Sollten dennoch sehr alte ( 75 Jahren), adipöse (BMI 30) oder stark vorerkrankte Patienten (ab ASA 3) untersucht werden, ist Vorsicht geboten. Es bedarf in der Zukunft weiterer großangelegter, prospektiver Studien, um eine ausreichende Validierung einer telemedizinischen Untersuchung in der Orthopädie und Unfallchirurgie zu erreichen.Die telemedizinische Untersuchung von Hüftgelenk und Becken ist mit Einschränkungen möglich. In times of a pandemic threat, such as COVID-19, and the need for reduced direct doctor-patient contact, internet-based telemedicine has attracted more and more attention as a surrogate service. Suspending the diagnosis and treatment of non-virus related diseases for longer periods of time is not a viable option since this would only exacerbate problems on the patient and national level. The need for alternative treatment modalities increased rather quickly. So far, telemedical applications have mainly focused on teleradiological diagnosis, follow-up and monitoring of psychiatric and internal diseases, as well as geriatric patient care. As far as these authors are aware, orthopaedic physical examination of the knee joint, including trauma work-up, has not been the subject of any studies to date. This feasibility study explores how video consultation can be designed and implemented in the context of history taking and physical examination in knee joint complaints.21 patient actors (PA) with simulated complaints of the knee joint were examined individually for each diagnosis, first via video consultation and then directly by a specialist (SP). One PA group has a medical background, the other was made up of laypersons. The time was measured for both types of consultation. The physician documented the detected symptoms, the quality of implementation of the self-examination steps, and the derived diagnosis on an assessment form. After completion of both consultation sessions, the PAs were handed a questionnaire on the respective examination modality.With the video consultation the examination lasted 8.63 (± 2.5) minutes on average and with the regular consultation in person 5.63 (± 1.7) minutes (p 0.001). For the group with medical background the examination lasted 7.67 (± 1.4) minutes on average, while for the lay group the video consultation took 9.7 (± 3.1) minutes (p = 0.049). With increased age, the video consultation was prolonged (p = 0.032; r = 0.47). The mean value for self-examination of leg axis, gait pattern and degrees of freedom was 9.32 (± 0.4) of 10 points. The following functional tests resulted in lower mean values (points): Payr 7.2 (± 2.3), Merke 5.9 (± 2.8), no-touch Lachmann 6.4 (± 2.7), gravity sign-recurvatum 6.7 (± 2.4). The mean grade by the PAs for the feasibility of self-examination was 2.43 (± 0.98) out of 5 points.The video consultation for musculoskeletal complaints of the knee joint allows exploratory remote examination and helps to minimise the number of patients in hospitals and practices. It takes longer for the physician to perform and does not permit functional testing for ligament injuries of the knee joint. Um individuellen oder gesamtwirtschaftlichen Problemen entgegenzuwirken, darf die Erkennung und Behandlung anderer Erkrankungen keine längerfristige Unterbrechung erfahren. In kurzer Zeit stieg der Bedarf alternativer Methoden zur Patientenbetreuung. Bisherige telemedizinische Einsatzgebiete lagen vor allem im Bereich der radiologischen Befundung, der Nachsorge und des Monitorings psychiatrischer und internistischer Erkrankungen sowie geriatrischer Patientenbetreuung. Studien zur Einsatzmöglichkeit für eine orthopädisch-unfallchirurgische Untersuchung des Kniegelenks existieren nach Autoreninformationen bislang nicht. Wie sich der Einsatz einer Videosprechstunde in der speziellen Anwendung für die Anamnese und Untersuchung bei Beschwerden des Kniegelenks konzipieren und umsetzen lässt, soll im Rahmen dieser Machbarkeitsstudie erarbeitet werden.21 Schauspielpatienten (SP) mit simulierten Beschwerden des Kniegelenks wurden für jede Diagnose einzeln zunächst über Videotelefonie und anschließend im direkten Kontakt durch einen Facharzt (FA) untersucht. Eine Gruppe der SP war medizinisch vorgebildet, die andere bestand aus Laien. Von beiden Untersuchungseinheiten wurde die Zeit gemessen. Die erkannten Symptome, die patientenseitige Durchführungsqualität der Eigenuntersuchungsschritte und die abgeleitete Diagnose wurde auf einem Bewertungsbogen durch den Arzt dokumentiert. Die SPs erhielten zum Abschluss beider Untersuchungseinheiten einen Bewertungsbogen zur jeweiligen Untersuchungsmethodik.Die Untersuchungseinheit der Videosprechstunde dauerte im Mittel 8,63 (± 2,5) min, die der normalen Sprechstunde 5,63 (± 1,7) min (p 0,001). Die Dauer der Videountersuchung zeigte für die Gruppe mit medizinischem Vorwissen einen Wert von 7,67 (± 1,4) min, wohingegen die Untersuchung der Laiengruppe im Mittel 9,7 (± 3,1) min dauerte (p = 0,049). Die Durchführbarkeit der Eigenuntersuchungen bewerteten die SP im Mittel mit 2,43 (± 0,98) von 5 Punkten.Die Videosprechstunde für muskuloskelettale Beschwerden des Kniegelenks ermöglicht eine orientierende Fernuntersuchung und hilft, das Patientenaufkommen in Kliniken und Praxen zu minimieren. Sie geht mit einem zeitlichen Mehraufwand für den Arzt einher und findet ihre Limitierung in den Funktionstests der Kniebinnenstrukturen. Die telemedizinische Untersuchung ersetzt in der heutigen Form keine persönliche Sprechstunde.

Cartilage regeneration with cell-free matrices has developed from matrix-associated autologous cartilage cell transplantation (MACT) over ten years ago. Adjustments to the legal framework and higher hurdles for cell therapy have led to the procedures being established as an independent alternative to MACT. These procedures, which can be classified as matrix-induced autologous cartilage regeneration (MACR), all rely on the chemotactic stimulus of a cross-linked matrix, which mostly consists of collagens. Given the example of a commercially available type I collagen hydrogel, the physicochemical properties of such a matrix are explained and the available experimental data highlighted in more detail. The interaction between different cell types and the chemotactic properties of the collagen has been investigated extensively and, from a clinical point of view, today offers various reference points for a smart modification of the described method to further improve clinical outcomes. Since the origin of the cells in the ultimately formed repair tissue is still unrevealed, further investigations to clarify the exact mechanism are crucially needed.Die Knorpelregeneration mit zellfreien Matrices hat sich vor mittlerweile über 10 Jahren aus der matrixassoziierten autologen Knorpelzelltransplantation (MACT) entwickelt. Anpassungen der rechtlichen Rahmenbedingungen und höhere Hürden für die Zelltherapie haben dazu geführt, dass sich die Verfahren als eigenständige Alternative zur MACT etabliert haben. Die als matrixinduzierte autologe Knorpelregeneration (MACR) einzuordnenden Verfahren setzen sämtlich auf den chemotaktischen Reiz einer vernetzten Matrix, die zumeist aus Kollagenen besteht. Am Beispiel eines marktüblichen Kollagen-Typ-I-Hydrogels werden die physikochemischen Eigenschaften einer solchen Matrix erläutert und die hierzu vorliegenden experimentellen Daten näher beleuchtet. Das Zusammenspiel zwischen verschiedenen Zelltypen und den chemotaktischen Eigenschaften des Kollagens wurde umfangreich untersucht und bietet aus klinischer Sicht heute unterschiedliche Anknüpfungspunkte zur sinnvollen Modifikation des Verfahrens zur Verbesserung des klinischen Outcomes. Da der Ursprung der Zellen im letztlich gebildeten Regeneratgewebe nach wie vor ungeklärt ist, sind weitere Untersuchungen zur Aufklärung des genauen Wirkmechanismus jedoch zwingend erforderlich.

BACKGROUND AND OBJECTIVE: Musculature affected during spondylodesis surgery may benefit and recover faster if supported by spine orthosis postoperatively. METHODS: This prospective study included 50 consecutive patients undergoing one- or two-level spondylodesis. The intervention group received a lumbar spine orthosis (n= 23), while the control group remained without orthosis (n= 27). Patients were assessed for pain (Visual Analogue Scale, VAS), Oswestry Disability index (ODI) as well as the use of analgesics. RESULTS: Patients wearing an orthosis postoperatively reported a higher degree of subjective stability. However, both intervention as well as control group did not show any significant differences for each of the follow-up points regarding VAS, ODI or the use of analgesics. CONCLUSION: Wearing an orthosis has neither impact on subjective pain, the need for analgesics nor for postoperative functionality. However, patients reported that they profited from wearing the orthosis by feeling more stable, thereby nicely improving their postoperative mobility. Thus, novel patient reported outcome measures have to be developed to assess these features in future studies.

Infection is one of the most challenging complications after total joint arthroplasties affecting up to 30,000 patients in the US per year. This study investigates the psycho-social burden induced by the two-stage intervention in infected hip or knee replacements. All patients were treated with a two-stage exchange and were assessed at three different timepoints regarding their psychological conditions. Our findings suggest that psychological sequelae after treatment of periprosthetic joint infection are clearly underestimated in the literature and psychological correlates and side effects should be further highlighted during the training process of young surgeons.

It has been demonstrated in the clinical case series summarized here over the past few years that the use of the matrix is not only safe but also yields good clinical-functional and MR-tomographic results for both small (~ 10 mm) and large ( 10 mm) focal cartilage lesions. Somit nimmt die MACR mittels Kollagen-Typ-I-Matrix, je nach Defektgrößte, sowohl in der direkten Konkurrenz zur Mikrofrakturierung als auch zur MACT eine wichtige Stellung als Alternativverfahren ein.

Wiederherstellung der Kniegelenkfunktion bei endoprothetischen Revisionseingriffen mit fortgeschrittenen Knochendefekten durch Verwendung tibialer metaphysarer Komponenten. Metaphysare Defekte entsprechend AORI(Anderson Orthopaedic Research Institute)-II- (a und b) und -III-Defekten in der Revisionsendoprothetik des Kniegelenks. Persistierender oder aktueller Gelenkinfekt, genereller Infekt (Pneumonie, Harnwegsinfekt [HWI]), fehlende Metaphyse, insuffizienter Streckapparat. Standardzugang uber bestehende Hautnarbe, Arthrotomie, Entfernung Zementspacer, wenn notwendig, und Entnahme multipler Gewebeproben; Praparation Femur first; anschliesend Praparation Tibia. Referenzierung der Rotation und Gelenklinienhohe mit entsprechender Entscheidung zur Verwendung von Augmenten. Tibiale Osteotomie intramedullar referenziert. Bestimmung der metaphysaren Defektgrose und dementsprechend Wahl der Komponentengrose. Positionierung der Raspelfuhrung fur die metaphysare Komponente (Sleeve); stufenweise Praparation des Bettes fur die metaphysare Komponente. Belassen der Komponente in der gewunschten Tiefe (bei Verwendung von Augmenten +5 oder +10 mm dementsprechend 5 bzw. 10 mm oberhalb des Tibiaplateaus). Montage des Tibiaplateaus auf dem liegenden Sleeve und Reposition. Probekopplung, Kontrolle der Implantatlage und der Gelenklinienhohe rontgenradiologisch. Verwendung der definitiven Implantate. Vollbelastung; regelmasige Wundkontrolle; Limitation des Flexionsgrades nur bei Augmentation oder geschwachtem Streckapparat. Zwischen Mai 2018 und August 2019 wurden 14 metaphysare Komponenten bei 14 Patienten tibial implantiert. Der mittlere Nachuntersuchungszeitraum betrug 10,4 Monate. Die Nachuntersuchung umfasste die klinische Untersuchung, den KSS (Knee Society Score) sowie eine Rontgen- und Versagensanalyse. Eine signifikante Verbesserung der ROM („range of movement“) von 75 ± 16° auf 100 ± 14° (p 200 m) nach 6 Monaten; tibiale Schaftschmerzen wurden von allen Patienten verneint. Das nachuntersuchte Kollektiv zeigt im Beobachtungsintervall eine Implantatuberlebensrate von 100 %.

Die Inzidenz der periprothetischen Frakturen des proximalen Femurs steigt aufgrund der wachsenden Anzahl an Hufttotalendoprothesenimplantationen im hoheren Alter. Die Versorgung dieser Frakturen stellt den behandelnden Arzt vor viele Herausforderungen und macht ein standardisiertes Vorgehen notwendig. Zur Festlegung einer Behandlungsstrategie bietet sich das Unified Classification System (UCS) an: Anhand der Frakturlokalisation und -ausdehnung, der Knochenqualitat, der Stabilitat der einliegenden Huftprothese und des Vorhandenseins weiterer Implantate wird die Fraktur klassifiziert und ein Behandlungsalgorithmus festgelegt. Diese Arbeit soll eine Ubersicht uber die Diagnostik, die Klassifikation, die Besonderheiten der verschiedenen periprothetischen Frakturtypen und deren Behandlung geben.

Zusammenfassung Einleitung Bei der Konsultation eines Facharztes für Orthopädie und Unfallchirurgie kommt der persönlichen Anamnese, Einschätzung und manuell-händischen klinischen Untersuchung durch den Arzt vor Ort eine elementare Rolle zu. Durch das „Digitale-Versorgung-Gesetz“ wurde die Durchführung von Videosprechstunden erleichtert. Die vorliegende Pilotstudie untersucht die Möglichkeit der videobasierten Einschätzung der Behandlungsdringlichkeit entsprechend einer strukturierten medizinischen Ersteinschätzung. Material und Methoden Im Rahmen eines Ex-ante-Designs wurden 40 Patienten mit einem orthopädisch-unfallchirurgischen Krankheitsbild mit elektiver, dringlicher oder Notfallindikation aus Sprechstunde oder Notaufnahme ausgewählt. Es wurde entsprechend den getätigten Angaben ein Kurzfragebogen ausgefüllt und durch einen ärztlichen Kollegen ein Bewegungsablauf entsprechend den Patientenangaben simuliert. Modifiziert nach den in der Wirbelsäulenorthopädie etablierten „red flags“ und „yellow flags“ wurden 9 kurze Fragen erfasst, welche die Dringlichkeit einer ärztlichen Vorstellung erfragten. Der videobasierte Bewegungsablauf orientiert sich an einem 60 s langen Instruktionsvideo, in dem motorische Tests und Bewegungsabläufe demonstriert werden: HWS-Bewegung in allen Ebenen, Elevation der oberen Extremität, Test des Finger-Boden-Abstandes beim „Vornüberbeugen“, Möglichkeit, aus dem Stand in die Hocke zu gehen und sich selbst wieder aufzurichten, sowie Zehen- und Hackenstand. Ergebnisse In 91,1% (n = 328) der Fälle war die Diagnose dem gleichen Gelenk bzw. der gleichen pathologischen Entität zugeordnet. In 37,5% (n = 135) der Fälle wurde eine Notfallindikation zur ärztlichen Vorstellung, in 10,8% (n = 39) der Fälle eine dringliche Indikation gesehen und in 51,6% (n = 186) der Fälle eine elektive Indikation gestellt. 12,5% (n = 45) der Fälle wurden „falsch positiv“ hinsichtlich einer Notfallvorstellung oder dringlichen Vorstellung evaluiert. Das bedeutet, dass die Probanden als „Notfall“ oder „dringlich“ eingestuft wurden, obgleich kein abwendbar gefährlicher Verlauf oder medizinischer Notfall vorlag. 18 Fälle (5%) wurden als „falsch negativ“ bewertet. Diskussion Der vorgestellte Screening-Fragebogen und das Kurz-Video-Assessment sind technisch umsetzbare und praktikable Methoden zur Erstevaluation vor der direkten, interaktiven videobasierten ärztlichen Onlinekonsultation. Zudem waren bei den von uns bewerteten Probanden der vorgestellte Fragebogen in Kombination mit dem Video-Assessment als Instrument geeignet, die Behandlungsdringlichkeit einzuschätzen und abwendbar gefährliche Verläufe und akute Notfälle zu selektieren. Schlussfolgerung Der Fragebogen ist in Kombination mit dem Video-Assessment während einer limitierten Zugänglichkeit des medizinischen Systems eine geeignete Methode während des notwendigen Social Distancings. Eine Rate von 5% falsch negativen, nicht korrekt eingeschätzten Patienten erscheint den Autoren für die klinische Praxis nicht akzeptabel und sollte daher optimiert werden.

When consulting a specialist in orthopaedics and trauma surgery, personal assessment and manual clinical examination by the doctor on site are essential. Nevertheless, implementation of video consultation hours has been made legally easier in Germany. The present pilot study examines the possibility of video-based assessment of the urgency of treatment in outpatient orthopaedic and trauma patients.Within an ex ante study design, 40 patients with an orthopaedic clinical picture for trauma surgery with elective, urgent or emergency indication were selected from consultation hours. A short questionnaire was filled in based on the information provided and a medical colleague simulated a movement sequence based on the patient information. After modification to the "red flags" and "yellow flags" established in spinal orthopaedics, nine short questions were recorded, which inquire about the urgency of a medical consultation. The video-based movement sequence is based on a 60 s long instruction video in which motor tests and movement sequences are demonstrated: Cervical spine movement in all levels, elevation of the upper extremity, test of the finger-floor distance when "bending forward over-bending", possibility of crouching from standing and getting up again, standing on heels and toes.In 91.1% (n = 328) of the cases, the diagnosis was associated with the same joint or the same pathological entity. In 37.5% (n = 135) of the cases an emergency indication was seen, in 10.8% (n = 39) of the cases an urgent indication and in 51.6% (n = 186) of the cases it was seen to offer an elective indication. 12.5% (n = 45) of the cases were evaluated as "false positives" with regard to an emergency or urgent presentation. This means that the test persons were classified as "emergency" or "urgent", although there was no preventable dangerous course or medical emergency. 18 cases (5%) were evaluated as "false negative".The screening questionnaire presented and the short video assessment are technically feasible and practicable method for the initial evaluation in video-based online medical consultation. In addition, the questionnaire presented in combination with the short video assessment was suitable as an instrument for assessing the urgency of the consultation and selecting preventable dangerous processes and acute emergencies.The questionnaire, in combination with the short video assessment, is a suitable method for the social distancing requirements during limited accessibility of the medical system. Das bedeutet, dass die Probanden als „Notfall“ oder „dringlich“ eingestuft wurden, obgleich kein abwendbar gefährlicher Verlauf oder medizinischer Notfall vorlag. 18 Fälle (5%) wurden als „falsch negativ“ bewertet.Der vorgestellte Screening-Fragebogen und das Kurz-Video-Assessment sind technisch umsetzbare und praktikable Methoden zur Erstevaluation vor der direkten, interaktiven videobasierten ärztlichen Onlinekonsultation. Zudem waren bei den von uns bewerteten Probanden der vorgestellte Fragebogen in Kombination mit dem Video-Assessment als Instrument geeignet, die Behandlungsdringlichkeit einzuschätzen und abwendbar gefährliche Verläufe und akute Notfälle zu selektieren.Der Fragebogen ist in Kombination mit dem Video-Assessment während einer limitierten Zugänglichkeit des medizinischen Systems eine geeignete Methode während des notwendigen Social Distancings. Eine Rate von 5% falsch negativen, nicht korrekt eingeschätzten Patienten erscheint den Autoren für die klinische Praxis nicht akzeptabel und sollte daher optimiert werden.

Numerous studies have shown that geriatric cooperation models in traumatology lead to significantly reduced morbidity and mortality rates. Even though national and international orthopedic and trauma societies recommend expanding these concepts to include nontrauma-related musculoskeletal diseases, unfortunately, to date only a few of these projects have been implemented. The present article deals with the necessary personnel and infrastructural implementation of a so-called orthogeriatric concept and puts the special features of this new type of treatment in direct comparison with known traumatological cotreatment concepts.Zahlreiche Studien konnten belegen, dass geriatrische Kooperationsmodelle in der Traumatologie zu einer signifikant reduzierten Morbiditäts- und Mortalitätsrate führen. Auch wenn nationale und internationale Fachgesellschaften seit Jahren eine Ausweitung dieser Konzepte auch auf andere, nichttraumabedingte muskuloskelettale Erkrankungen fordern, sind bisher wenige derartige Projekte umgesetzt worden. Der vorliegende Artikel beschäftigt sich mit der personellen, inhaltlichen und infrastrukturellen Umsetzung eines sog. orthogeriatrischen Konzeptes und stellt die Besonderheiten dieser neuartigen Therapieform in direktem Vergleich zu den bekannten traumatologischen Co-Behandlungskonzepten. Essenziell ist dabei die Möglichkeit, orthogeriatrische Patienten oft lange im Vorfeld einer Operation präoperativ vorbereiten zu können und so das postoperative Outcome zu verbessern. Die stationären als auch poststationären Konzepte halten sich eng an die Vorgaben der zertifizierten Traumazentren Alterstrauma-Zentren (ATZ) der Deutschen Gesellschaft für Unfallchirurgie (DGU) und können überwiegend in orthogeriatrische Behandlungskonzepte übernommen werden.

In the course of the corona pandemic, resource conservation and the protection of further infections have made it necessary to break new ground in the organisation of orthopaedic and trauma surgery consultations. One solution is consistent digitisation and the offer of video consultation hours. In this study, non-contact examination of patients with shoulder disorders is described and critically examined.Thirty patients who presented with pathologies of the shoulder joint in a university outpatient clinic were subjected to a physical examination in a conventional and contactless manner. The data obtained on mobility, function and provocation test of both examinations were compared to draw conclusions about the virtual feasibility.46% of the patients suffered from a traumatic shoulder lesion, and 54% showed degenerative lesions. The assessment of mobility showed a high correlation of 70 - 90% between the two examinations. Common tests to evaluate the supraspinatus, infraspinatus, subscapularis and the long head of the biceps could be adequately performed in a contactless version by more than three quarters of the patients, but with low-to-moderate performance values.Contact-less examination is particularly disadvantageous when evaluating stability criteria. For the medical history and functional test, there were no significant differences between the classic consultation and contactless consultation. Although virtual consultation is a widespread and valuable addition in pandemic times, it cannot replace a safe assessment and indication by personal examination.In der Coronapandemie ist es zur Vermeidung weiterer Infektionen sowie zur Optimierung des Einsatzes von medizinischem Klinikpersonal erforderlich, neue Wege bei der Ablauforganisation von orthopädischen und unfallchirurgischen Sprechstunden zu gehen. Ein Lösungsansatz ist eine konsequente Digitalisierung und das Angebot von Videosprechstunden. In dieser Arbeit soll die Durchführung von Anamnese und Untersuchung von Patienten mit Schultergelenkspathologien beschrieben und kritisch hinterfragt werden.30 Probanden, die sich mit Pathologien im Bereich des Schultergelenks in einer Hochschulambulanz vorstellten, wurden auf herkömmliche Weise und kontaktlos einer Untersuchung unterzogen. Die gewonnenen Daten von Beweglichkeit, Funktion und Provokationstests beider Untersuchungen wurden verglichen, um Rückschlüsse auf die virtuelle Durchführbarkeit zu ziehen.46% der Probanden litten an einer traumatischen Schulterläsion und 54% an einer degenerativen Erkrankung. Die Bewertung der Beweglichkeit zeigte eine hohe Korrelation (70 – 90%) zwischen beiden Untersuchungen. Herkömmliche Tests für die Evaluierung der Supraspinatussehne, der Infraspinatussehne, der Subscapularissehne und der langen Bizepssehne konnten von mehr als ¾ der Patienten in ihrer kontaktlosen Anpassung angemessen durchgeführt werden, zeigten jedoch geringe bis milde Performance-Werte.Insbesondere bei der Bewertung von Stabilitätskriterien ist die kontaktlose Untersuchung nachteilhaft. Bei der Durchführung von Anamnese und Funktionsprüfung zeigten sich keine signifikanten Unterschiede zwischen der klassischen Konsultation und einer kontaktlosen Konsultation. Obwohl die digitale Konsultation in Pandemiezeiten eine verbreitete und wertvolle Ergänzung darstellt, kann Sie eine sichere Bewertung und Indikationsstellung durch persönliche Untersuchung nicht ersetzen.

BACKGROUND Ankle arthropathy is a frequent complication of haemophilia, reducing the patients' quality of life. Despite intensive conservative therapy, end-stage arthropathy requires surgical treatment, either by ankle fusion (AF) or total ankle replacement (TAR). METHODS Eleven consecutive AFs were performed in nine patients and 11 TARs were implemented in 10 patients. Outcomes were assessed clinically by AOFAS score and radiologically by the Pettersson and Gilbert scores. RESULTS The mean age of the patients in these groups were 35.7 years and 49.4 years, respectively. Of the 11 ankles that underwent fusion, 10 showed bony consolidation not later than 12 weeks after surgery, whereas one still showed non-union after 6 months. VAS pain scores decreased significantly in both groups. Mean AOFAS scores also improved significantly, from 28.1 before to 80.3 after AF and from 21.5 before to 68.0 after ankle replacement. No perioperative complications were observed in either group. Late deep infection was observed in two patients that underwent TAR, which required removal of the implant. CONCLUSION Our data indicate that both AF and TAR result in significantly reduced pain in patients with haemophilia with end-stage haemophilic arthropathy. While TAR is associated with a higher risk of deep infection and minimal persistent pain, it preserves the pre-operative range of motion. AF on the other hand is associated with the risk of non-union and a longer post-operative recovery period but results in greater pain reduction.

The goal of this study is to evaluate the primary stability of a cementless augment-and-modular-cage system with and without the addition of cranial straps in a standardized in vitro setting. As the surrogate parameter for the evaluation of primary stability, the measurement of relative motion between the implant components themselves and the bone will be used. Acetabular revision components with a trabecular titanium augment in combination with a large fourth-generation composite left hemipelvis were assembled. These constructs were divided into two groups with (S) and without cranial straps (nS). A total of 1000 cycles was applied at each of three load levels. Relative movements (RM) between the components were measured. Load levels display a significant effect on the amount of RM at all interfaces except between shell/augment. The group assignment appears to have an effect on RM due to significantly differing means at all interfaces. Between bone/shell RM increased as load increased. NS displayed significantly more RM than S. Between shell/augment RM remained constant as load increased. Between shell/cup S showed more RM than nS while both groups’ RM increased with load. We conclude a significant increase of primary stability between the shell and the bone through the addition of cranial straps. Relative motion between components (shell/cup) increases through the addition of cranial straps. A clinical impact of this finding is uncertain and requires further investigation. Finally, the cementless fixation of the augment against the rim-portion of the shell appears stable and compares favorably to prior investigation of different fixation techniques.

Olecranon fractures are common injuries that can occur in patients of any age. To assess the postoperative outcome, multiple scoring systems like the DASH, MEPS und Morrey Score have been described. The goal of this paper is to compare the subjective elbow value (SEV) to these commonly used scoring systems. We hypothesized that the SEV would have a high correlation and practicability for the evaluation of elbow function after isolated olecranon injury.Clinical data of 40 patients were collected and retrospectively analysed. All patients suffered an isolated olecranon fracture and were surgically treated by tension band wiring or plate fixation. In the follow-up examinations the Mayo Score, DASH Score, Morrey Score and SEV were measured for correlation purposes.The mean follow-up time was 70.5 ± 45.6 months. The median DASH Score was 9.9 (4.1 - 20.4) and 100 (85 - 100) for the MEPS. The median Morrey Score was 98 (92 - 100). The median SEV was 90% (80 - 95%). The Pearson correlation between the DASH and SEV was -0.85 and 0.80 between the MEPS and the SEV.The SEV shows a high correlation to the most commonly used scoring systems for outcome evaluation after elbow injury. It is easy to use and can be seen as a valuable tool to assess patients after olecranon fractures.Olekranonfrakturen sind häufige Verletzungen bei Patienten aller Altersgruppen. Für die Beurteilung des postoperativen Ergebnisses werden in der Literatur verschiedene, teils komplexe Scoring-Systeme beschrieben wie DASH-Score, MEPS und Morrey-Score. Ziel der vorliegenden Arbeit ist ein Vergleich der subjektiven Ellenbogenfunktion (SEF) mit gängigen Scoring-Systemen unter der Hypothese einer hohen Korrelation und der praktikablen klinischen Anwendung der SEF.Klinische Daten von 40 Patienten nach operativer Versorgung von Olekranonfrakturen wurden erhoben und retrospektiv ausgewertet. Die Patienten wurden zwischen 2010 und 2017 mittels Plattenosteosynthese oder Zuggurtungsosteosynthese operativ therapiert. In der klinischen und funktionellen Nachuntersuchung wurden der Mayo-, DASH-, Morrey-Score sowie die subjektive Ellenbogenfunktion erfasst und die Pearson-Korrelation für alle möglichen Kombinationen der Scores berechnet.Der mittlere Nachuntersuchungszeitraum lag bei 70,5 ± 45,6 Monaten. Der Median des DASH-Scores lag bei 9,9 (4,1 – 20,4) und bei 100 (85 – 100) für den MEPS. Der Median des Morrey-Scores betrug 98 (92 – 100). Der Median der subjektiven Ellenbogenfunktion betrug 90% (80 – 95%). Die Pearson-Korrelation zwischen dem DASH und der SEF lag bei −0,85 und zwischen dem MEPS und der SEF bei 0,80.Die SEF zeigt eine hohe Korrelation mit den beiden am meisten verwendeten Scoring-Systemen. In der klinischen Praxis ist die Anwendung und Dokumentation der SEF daher als ein wertvoller Parameter in der Qualitäts- und Verlaufskontrolle nach operativ versorgter Olekranonfraktur anzusehen.

Shoulder stiffness is a condition of restricted glenohumeral range of motion, which can arise spontaneously or as consequence of a known cause, including surgical procedures on the shoulder. Several approaches to shoulder stiffness have been proposed and high-level evidence is available to analyze and discuss their results. The aim of this review was to summarize the current concepts on conservative and operative treatment of shoulder stiffness and discuss the results of the available studies with a high level of evidence, which should be considered to guide clinical practice. Treatment of shoulder stiffness should be tailored to the patient's clinical situation and the stage of its shoulder pathology and should aim at pain reduction, restoration of range of motion, functional regain and shortening of symptoms duration. When possible, known risk factors for primary shoulder stiffness and causes of secondary shoulder stiffness should be addressed to avoid relapse. Conservative therapy is the mainstay of treatment for shoulder stiffness and should include a multimodal and activity-oriented program. Intra-articular injection of a low dose of corticosteroid is safe and effective, provides immediate benefits, and is recommended in combination to an appropriate rehabilitation protocol. In conservative refractory cases, arthrolysis and capsular release can be performed with an arthroscopic approach.Die Schultersteife ist charakterisiert durch einen eingeschränkten glenohumeralen Bewegungsumfang mit schmerzhaftem Funktionsverlust in der aktiven und passiven Beweglichkeit. Entsprechend der Ätiologie lassen sich primäre und sekundäre Formen unterscheiden. Verschiedene Therapien der Schultersteife wurden vorgeschlagen, die bei Vorhandensein umfassender Evidenz in ihren Ergebnissen kritisch analysiert und diskutiert werden müssen. Ziel dieser Übersichtsarbeit ist es, die aktuellen Konzepte in der konservativen und operativen Behandlung der Schultersteife zusammenzufassen, die Ergebnisse der verfügbaren Studien mit hohem Evidenzgrad zu diskutieren und dadurch einen Therapiealgorithmus für die klinische Praxis abzuleiten. Die Behandlung der Schultersteife sollte auf die Situation des Patienten und das Stadium seiner Schulterpathologie zugeschnitten sein und auf eine Schmerzreduktion, Wiederherstellung des Bewegungsumfanges sowie eine funktionelle Wiederherstellung und Verkürzung der Symptomdauer abzielen. Wenn möglich, sollten bekannte Risikofaktoren für die primäre Schultersteife und Ursachen der sekundären Schultersteife berücksichtigt werden, um Rückfälle zu vermeiden. Die konservative Therapie ist der wesentliche Pfeiler in der Behandlung der Schultersteife und sollte einen stadiengerechten, multimodalen und aktivitätsorientierten Ansatz beinhalten. Die intraartikuläre Injektion niedrig dosierter Kortikosteroide ist sicher und wirksam, bietet unmittelbare Vorteile und wird in Kombination mit einem geeigneten Rehabilitationsprotokoll in der „Freezing“-Phase empfohlen. In therapierefraktären Fällen kann eine, trotz adäquater konservativer Therapie, an die Bewegungseinschränkung angepasste, arthroskopische Arthrolyse mit Kapselrelease durchgeführt werden.

This article offers a German perspective on orthopaedic management of geriatric populations in higher-income countries. Specific orthopaedic procedures necessitate a more intensive level of care in older adult patients, especially with co-morbidities, than in younger patients. During the pre-, peri- and post-operative periods, several aspects such as patient blood management principles improve the outcomes of geriatric patients. New interdisciplinary geriatric models of care including geriatric and orthopaedic specialties have to be established to take care of the special needs of geriatric patients. Background Postoperative complications are the main consumers of technical, medical, and human resources. Especially in the field of elective joint replacement surgery, a specialized, easy-to-obtain, and cost-efficient preoperative stratification and risk-estimation model is missing. Methods With preoperatively surveyed patient parameters, we identified the most relevant parameters to predict postoperative medical complications. We devised a prospective risk model, measuring the individual probability for intermediate care unit (IMC) or intensive care unit (ICU) admission. The study includes all patients (n = 649) treated with primary or revision total knee arthroplasty in our clinic from 2008 to 2012. Results The association between general comorbidity scores and mortality risk is well known. Among different comorbidity scores, the Charlson Comorbidity Index is not only relevant for overall postoperative complications (odds ratios [OR] = 2.20) but also predictive of specific complications such as the postoperative need for blood transfusion (OR = 1.94) and unexpected adverse events (OR = 1.74). Considering adverse events, c-reactive protein and leukocyte levels are also highly relevant. Upon predicting a necessary postoperative transfer to an IMC or ICU, the preoperative hemoglobin level, the Charlson Comorbidity Index, and the Index of Coexistent Disease stood out. The latter indicates an increased rate for an IMC/ICU stay by 341% per point. Condensing the most influential predictors, the probability for postoperative IMC/ICU transfer can be calculated for each individual patient. Using the routinely assessed patient's variables, no steadier prediction is possible. Conclusion The introduced risk-estimation model offers a specialized preoperative resource-stratification method in knee joint replacement surgery. It condenses the most influential, individual risk factors to avoid clinical test redundancy and improve resource efficiency and presurgical care planning. A prospective follow-up study could help validating the risk model in clinical routine.

Periprosthetic loosening, either aseptic or induced by periprosthetic joint infection remains a major long term complication and challenge in orthopedics and trauma surgery. Sensitivity of potential loosening of the material and other causes of a painful prosthesis is essential for choosing the respective treatment option and providing the needed resources. 18F-Fluoride is a radiopharmaceutical which shows a high affinity to bone and a rapid blood clearance. The objective of this study was to assess 18F-Fluoride PET/CT´s sensitivity and specificity in diagnosing periprosthetic loosening in total hip and knee arthroplasty. We included 26 patients with 24 hip and 13 knee prostheses in our retrospective study with radiological or clinical suspicion of peri-prosthetic loosening at least one year after implantation. Results of 18F-Fluoride PET/CT imaging were compared with surgical results or clinical follow-up if surgery was not performed. On the basis of our data we found a sensitivity of 95.00 %, a specificity of 87.04 % and an accuracy of 89.19 % for 18F-Fluoride PET/CT. The results of our study show that 18F-Fluoride PET/CT is a useful and promising technique in diagnosing periprosthetic loosening of total hip and knee arthroplasties. Further investigation should focus on different uptake patterns of the isotope in periprosthetic joint infection and therefore distinguishing aseptic from septic loosening and enhancing the diagnostic value of this imaging method.
