12. Gesicherte Literaturergebnisse zur Therapie distaler Radiusfrakturen

Abstract

Zusammenfassung: In der Therapie der distalen Radiusfraktur findet sich derzeit ein Trend zur vermehrten operativen Behandlung und zum Einsatz winkelstabiler Platten-Schrauben-Systeme. Zahlreiche Publikationen mit z. T. abweichenden Resultaten erschweren die Ableitung von klinischen Handlungsempfehlungen. Durch eine systematische Literaturrecherche wurden relevante Artikel zur Therapie der distalen Radiusfraktur erfasst, inhaltlich ausgewertet und nach ihrer Evidenzgüte klassifiziert. Aus 7 Cochrane Reviews ließ sich eine Tendenz zur besseren anatomischen Rekonstruktion nach operativer Therapie, jedoch keine Überlegenheit in den klinischen Resultaten ableiten. Zu winkelstabilen Plattenosteosynthesen fanden sich 64 Publikationen, viele in Form biomechanischer Studien und Expertenmeinungen. In 25 identifizierten Fallserien wurden winkelstabile Plattenosteosynthesen überwiegend positiv bewertet und nur selten kritisch kommentiert. Anhand von lediglich 3 vergleichenden Studien konnten zwar radiologische, jedoch keine funktionellen Vorteile für winkelstabile Systeme aufgezeigt werden. Die vorliegenden Publikationen lassen aufgrund geringer Evidenzgüte und sehr heterogener Datenlage keine klaren Empfehlungen für die Behandlung von distalen Radiusfrakturen zu. Insbesondere konnte die Überlegenheit winkelstabiler Plattenosteosynthesen noch nicht erwiesen werden.

Abstract

Background: Pancreatic surgery is associated with considerable morbidity and, consequently, offers a large and complex field for research. To prioritize relevant future scientific projects, it is of utmost importance to identify existing evidence and uncover research gaps. Thus, the aim of this project was to create a systematic and living Evidence Map of Pancreatic Surgery. Methods: PubMed, the Cochrane Central Register of Controlled Trials, and Web of Science were systematically searched for all randomized controlled trials and systematic reviews on pancreatic surgery. Outcomes from every existing randomized controlled trial were extracted, and trial quality was assessed. Systematic reviews were used to identify an absence of randomized controlled trials. Randomized controlled trials and systematic reviews on identical subjects were grouped according to research topics. A web-based evidence map modeled after a mind map was created to visualize existing evidence. Meta-analyses of specific outcomes of pancreatic surgery were performed for all research topics with more than 3 randomized controlled trials. For partial pancreatoduodenectomy and distal pancreatectomy, pooled benchmarks for outcomes were calculated with a 99% confidence interval. The evidence map undergoes regular updates. Results: Out of 30,860 articles reviewed, 328 randomized controlled trials on 35,600 patients and 332 systematic reviews were included and grouped into 76 research topics. Most randomized controlled trials were from Europe (46%) and most systematic reviews were from Asia (51%). A living meta-analysis of 21 out of 76 research topics (28%) was performed and included in the web-based evidence map. Evidence gaps were identified in 11 out of 76 research topics (14%). The benchmark for mortality was 2% (99% confidence interval: 1%–2%) for partial pancreatoduodenectomy and <1% (99% confidence interval: 0%–1%) for distal pancreatectomy. Abstract

Power Demand and Energy Consumption of large-scale sys- tems are current concern in High Performance Computing area. Aiming to increase the efficiency of these systems and better manage the load and consumption, load balancers strategies have been proposed as a good al- ternative. In response to this challenge, we developed a energy-aware load balancer called EnergyLB to manage and reduce the power demand and save energy of systems when iterative applications with imbalanced load are executed. Our proposed load balancer combines dynamic load bal- ancing with DVFS techniques in order to reduce the clock frequency of underloaded computing cores which experience some residual imbalance even after tasks are remapped. Tests carried out with two applications show that EnergyLB results in low overhead, power reductions of 7% in average and energy saving of 6% in average when used together with others Charm++ load balancers.

22. How Does Pedagogical Slipperiness Enable Speculation in/for Teacher Professional Learning?

Abstract

This chapter reports on data generated through a process of COVID-19 attuned metho-pedagogical innovation established in the chapter "what are artists and art educators teaching us about how we can conceive and deliver teacher professional learning into the future?" Analysis of data reported in Coleman and MacDonald’s (2020) chapter articulates how movement between temporal and latent space enables teachers to attend to professional learning during times of COVID-19 driven interruption. The act of moving between the temporal and latent creates what can be described as Deleuzean slippage ; a productive act that yields new possibilities for becoming differently. This follow-up chapter reports data generated via a widened lens of slippage enacted by an expanded authorship team. In so doing, this chapter elicits an example of methodological and pedagogic interchange – with methodology and pedagogy mutually constituting each other, becoming metho-pedagogy. With a/r/tographic documentations of change occurring individually, collectively, and collaboratively, we propose slipperiness as a generative catalyst for change in pedagogic ways of knowing.
