Abstract

BACKGROUND Professional home care nurses are often tasked with the medication management of older, polymedicated clients. The medication use process is often complex and prone to medication errors. From the perspective of professional caregivers, the interprofessional interfaces of care as well as the integration of clients and informal caregivers are a��major challenge. AIM With this study, we explored the perspective of home care clients and their informal caregivers on the medication use process supported by professional caregivers with a��focus on medication safety. METHOD We chose a��qualitative research approach, using guideline-based interviews with eight clients and five caregivers. Data were analyzed applying the topical analysis according to Braun and Clarke. RESULTS AND DISCUSSION Before engaging professional home care nurses in the medication use process, clients reached their physical, psychological and social limits. They were relieved when the professional home care organization took care of the overall coordination of the medication use process and felt safe. They trust professional caregivers and see limited need to be preoccupied with medication safety themselves, despite risks inherent in self-medication, adherence and transition of care. CONCLUSION Through the relief provided by professional caregivers, home care clients and their informal caregivers no longer perceive themselves as active partners in maintaining medication safety. Healthcare professionals should keep an eye on self-medication as well as adherence and support the use of the medication plan., Hintergrund: Eine h��ufige Aufgabe der professionellen h��uslichen Pflege ist die Unterst��tzungbetagter, polypharmazierter Klient:innen beim Medikationsmanagment. Die medikament��se Versorgung ist oft komplex and anf��llig f��r Medikationsfehler. Aus der Perspektive von Fachpersonen sind insbesondere die zahlreichen interprofessionellen Schnittstellen im Medikationsprozess sowie die Integration von Klient:innen und Angeh��rigen gro��e Herausforderungen. Ziel: Mit dieser Studie wurde exploriert, wie Klient:innen und deren Angeh��rige den Medikationsprozess mit Unterst��tzung der h��uslichen Pflege im Hinblick auf die Medikationssicherheit erleben. Methode: Es wurde ein qualitativer Forschungsansatz gew��hlt und leitfadengest��tzte Interviews mit 8 Klient:innen und 5 Angeh��rigen durchgef��hrt. Die Daten wurden anhand der thematischen Analyse nach Braun und Clarke ausgewertet. Ergebnisse und Diskussion: Vor dem Einbezug der h��uslichen Pflege stie��en Klient:innen im Medikationsmanagement physisch, psychisch und sozial an ihre Grenzen. Sie erlebten mit der ��bernahme des Medikationsprozesses durch die h��usliche Pflege Entlastung und Sicherheit. Dabei bringen sie dem Fachpersonal gro��es Vertrauen entgegen und sehen wenig Bedarf, sich selber in Medikationssicherheitsbestrebungen einzubringen, obwohl Gefahrenquellen im Bereich Selbstmedikation, Adh��renz und Schnittstellen vorhanden sind. Schlussfolgerungen: Mit der Entlastung durch die professionelle Unterst��tzung sehen sich Klient:innen der h��uslichen Pflege und deren Angeh��rige kaum mehr als aktive Partner:innen im Wahren der Medikationssicherheit. Es braucht ein Augenmerk der Fachpersonen auf die Bereiche Selbstmedikation und Adh��renz sowie eine Unterst��tzung bei der Nutzung des Medikamentenplans.

Abstract

Currently, heat-sensitive flexible endoscopes are reprocessed in central RUMEDs, a RUMED situated within the endoscopy department as well as in decentral RUMEDs. Before deciding where reprocessing is to be carried out, structural and technical prerequisites, manpower as well as organizational and logistical structures must be considered. Regardless of the choice of reprocessing site, reprocessing must meet the same quality requirements. Following manual precleaning and cleaning, flexible endoscopes should preferably undergo automated reprocessing in an automated endoscope reprocessor (AER) using validated processes (EN ISO 15883-4 [5]). Depending on the type of endoscope used, validated reprocessing processes must meet various requirements and include manual precleaning , automated cleaning and disinfection in an AER, drying, and possibly sterilization and storage. That calls for personnel with commensurate specialist knowledge and qualifications. The structural and technical facilities are the key to consistent separation between unclean and clean work steps, reprocessing safety and infection prevention. If sterilization is indicated for medical and hygiene reasons for heat-sensitive components, a suitable low-temperature process should be chosen in accordance with the manufacturer's instructions. Abstract

The 96-kDa surface antigen of Entamoeba histolytica was demonstrated through extensive immunologic evaluation with monoclonal and monospecific antibodies to be identical to or an isoform of the amebic alcohol/aldehyde dehydrogenase (EhADH2). EhADH2 was secreted, excreted, or shed into the culture medium in quantities commensurate with amebic growth when studied in a novel culture system. Of importance, using RNase protection assays, specific mRNA coding for the EhADH2 gene product(s) was up-regulated by treatment of viable trophozoites with the enzyme substrate ethanol. Abstract

A novel, computer-assisted program was developed to analyze the time course of Na+-glucose cotransport by rat renal cortical brush-border membrane vesicles (BBMV). Transporter characteristics can be measured, which routine kinetic analyses fail to distinguish: cotransporter membrane density is derived from the picomoles of D-glucose bound per milligram of protein. Binding is stereospecific, blocked by phlorizin, and supported equally well by Na+ or K+ (but not Cs+). Quasi-first-order influx and efflux rate constants for the composite Na+-driven influx and the (presumed) Na+-independent efflux processes were highly dependent on glucose concentration. Either two Na+-glucose transporters exist in proximal tubules or a single mechanism abruptly changes rate when glucose falls to low levels. The major operation mode is slow, has a high capacity but low affinity, and may have a 2 Na+:2 glucose stoichiometry (Hill coefficient is unity). The minor system is a fast, smaller-capacity, higher-affinity operation with a 2 Na+:1 glucose stoichiometry that was not distinguishable when the same data were analyzed in conventional kinetic plots. Results with streptozocin-induced diabetic rats illustrate the method's utility. Low-glucose-affinity cotransporters were upregulated in hyperglycemic, but not in cachectic, ketoacidotic animals. Rate constants, especially for efflux, were decreased in diabetes.

40. Relationship between the OmpC and LamB proteins of Escherichia coli and its influence on the protein mass of the outer membrane

Abstract

Cultures of Escherichia coli K-12 which possessed varied amounts of the LamB protein were found to contain reduced amounts of the OmpC protein. This process was regulated in part at the level of transcription. However, additional controls were inferred from anomalously high levels of the OmpC protein present at low levels of the LamB protein. Both proteins were present at increased levels, and this led to an increase in the total outer membrane protein mass per cell. The absolute amount of outer membrane protein per cell was found not to be a constant as had been tacitly assumed.

Abstract

Protein 1 was shown to be the receptor for phage PA-2 by the observations that the purified protein inactivates the phage, mutants lacking the protein are resistant to the phage, and mutants selected for PA-2 resistance have altered protein. Protein 1 appears as two bands (1a and 1b) on high-resolution polyacrylamide gels. The most abundant classes of mutants (ParI and ParII) selected for PA-2 resistance were found to lack band 1b. The mutations responsible for the ParI and ParII phenotypes were mapped at a locus termed par, which is near nalA on the Escherichia coli chromosome. The cyanogen bromide peptides of proteins 1a and 1b are similar, suggesting that these bands represent modified forms of the same polypeptide. Strains carrying the tolF mutation produce only band 1b. When a par tolF double mutant was constructed, this strain produced only band 1a. These results suggest that genes at the par and tolF loci are involved in modification of protein 1, or regulation of such modification, and are not structural genes for protein 1.

42. Outer membrane proteins of Escherichia coli. V. Evidence that protein 1 and bacteriophage-directed protein 2 are different polypeptides

Abstract

Protein 1 from the outer membrane of Escherichia coli K-12 and protein 2 from a phage PA-2 lysogen of the same strain were isolated by differential sodium dodecyl sulfate extraction and purified by ion-exchange and gel filtration chromatography. Rabbit antisera were prepared against these proteins and showed no cross-reaction between proteins 1 and 2. The proteins have the same N-terminal amino acid but show small yet significant differences in amino acid composition. The proteins were cleaved with cyanogenbromide in solvents containing both formic acid and trifluoroacetic acid. By comparing the cleavage in these solvents, it was established that protein 1 yielded 5 cyanogen bromide peptides, and the sum of the molecular weights of these was equivalent to the molecular weight of the uncleaved protein. Protein 2 yielded 4 cyanogen bromide peptides, none of which was identical to those of protein 1, and the sum of these peptides was also equivalent to the apparent molecular weight of the uncleaved protein. Significant differences were also observed when tryptic peptides from the two proteins were compared. These results indicate that protein 1 and the phage-directed protein 2 are distinct, different, and apparently homogeneous proteins.

43. Phospholipid metabolism in Pseudomonas BAL-31 infected with lipid-containing bacteriophage PM2

Abstract

Infection of Pseudomonas BAL-31 with the lipid-containing bacteriophage PM2 resulted in no detectable change in the rate of phosphatidylglycerol (PG) or phosphatidylethanolamine (PE) biosynthesis. An increase in the PG content of infected cultures was not seen until the cultures began to lyse, and this increase was in fact only a relative increase resulting from the extensive turnover of PE at the onset of culture lysis. Turnover studies revealed that the glycerol, phosphorus fatty acid, and ethanolamine moieties of PE turned over simultaneously at the time of lysis, and therefore made it unlikely that there was a PE to PG conversion during the latent period of the phage. The lipid found in the bacteriophage did not reflect a preferential selection for lipid synthesized before or after infection, but in fact reflected the composition of the host membrane at the time the phage were assembled. The use of a modified medium that allowed the cultivation of Pseudomonas BAL-31 as a prototroph and resulted in reliable lysis times of infected cultures led us to the conclusion that PM2 infection effects little change in host phospholipid metabolism, and that there is sufficient PG in the host cytoplasmic membrane to account for a full burst of phage. As a result of the reliable lysis times that we have achieved, we concluded that certain metabolic events, i.e., PE turnover, are lytic phenomena and must not be confused with events relevant to the biosynthesis and maturation of the phage.

Abstract

The major outer membrane proteins from Escherichia coli K-12 are modified to contain alpha-aminoadipic acid delta-semialdehyde (allysine). The allysine was found to be derived from lysine and it was identified by derivatizing it to chloronorleucine by reduction, alpha-aminoadipic acid by oxidation, and to alpha,epsilon-diaminopimelic acid by reacting it with CN- and NH3. The alpha-aminoadipic acid was identified by mass spectrometry. Two major outer membrane proteins were found to possess allysine, a modified lysine characteristically found to connective tissue.

45. Verification of the protein in the outer membrane of Bdellovibrio bacteriovorus as the OmpF protein of its Escherichia coli prey

Abstract

Two research groups showed that several Bdellovibrio strains incorporated into their outer membranes intact OmpF porin proteins derived from their Escherichia coli prey. These results could not be reproduced by another group using Bdellovibrio bacteriovorus 109J. They showed that a major protein appearing in the Bdellovibrio Triton X-100-insoluble outer membrane was coded for by the bdellovibrios. We reconciled these results by examining the strain used by this group and by reviving a freeze-dried culture of strain 109J which had been stored for almost 9 years. B. bacteriovorus 109J failed to acquire substantial amounts of the OmpF protein from E. coli ML35, and a protein coded for by the bdellovibrios was expressed in its place. However, B. bacteriovorus 109J incorporated the OmpF protein from rough K-12 strains of E. coli, and the revived 9-year-old culture of B. bacteriovorus 109J incorporated more of the OmpF protein from the smooth E. coli ML35 than did its contemporary counterpart. The protein isolated from the outer membrane of the bdellovibrios was identified as the OmpF protein of E. coli by its protease peptide profile on sodium dodecyl sulfate-polyacrylamide gel electrophoresis and by Western blot analysis. This confirmed that bdellovibrios relocalize outer membrane proteins from their prey, but relocalization may be an unstable trait which can be influenced by the prey.

46. Acquisition of Escherichia coli outer membrane proteins by Bdellovibrio sp. strain 109D

Abstract

The ability of Bdellovibrio sp. to acquire the OmpF major outer membrane protein from its Escherichia coli prey was examined to determine if there were other outer membrane proteins which could or could not be acquired. Growth of bdellovibrios on mutant prey which were defective in the expression of outer membrane proteins revealed that Bdellovibrio sp. could acquire the OmpC protein in the absence of the OmpF protein. However, the OmpA, LamB, and protein 2 proteins could not be found in the Bdellovibrio Triton-insoluble outer membrane. The disappearance of the OmpF and OmpC proteins from the bdelloplast surface was measured, and it was determined that Bdellovibrio sp. exhibited a kinetic and temporal preference for the OmpF protein. Bdellovibrios could be grown on porin-deficient prey, and the progeny bdellovibrios possessed outer membranes with a protein mass deficiency.

47. Heterogeneity in Lipid Composition of the Outer Membrane and Cytoplasmic Membrane of PseudomonasBAL-31

Abstract

The outer membranes and cytoplasmic membranes of the marine bacterium PseudomonasBAL-31 were separated by washing the cells three times in 0.5 M NaCl and twice in 0.5 M sucrose. Electron microscopy during the removal of membranes revealed that the outer membranes fragmented in a regular manner to give rise to fairly uniform vesicles measuring approximately 140 nm in diameter. Isolated outer membranes had a buoyant density in sucrose of 1.230 g per cm3, whereas the cytoplasmic membranes had a density of 1.194 g per cm3. The removal of the outer membrane during the application of this procedure was monitored by measuring the release of 2-keto-3-deoxyoctulosonic acid and phospholipid. The cells lost 85.5% of their 2-keto-3-deoxyoctulosonic acid and 47.3% of their phospholipid during this treatment. Complete recovery of outer membrane material could be achieved. The removal of 25.5% of the 2-keto-3-deoxyoctulosonic acid and 0.9% of the phospholipid rendered the cells sensitive to lysis with Triton X-100. The phospholipid composition of the outer membrane was calculated to be 78.9% phosphatidylethanolamine and 16.1% phosphatidylglycerol. The phospholipid composition of the cytoplasmic membrane proved to be 71.5% phosphatidylethanolamine and 23.5% phosphatidylglycerol. The fatty acid composition was also found to be quantitatively heterogeneous between the two membranes.

Abstract

Cholera toxin was labeled with tritium by the Wilzbach technique, and highly purified radiolabeled toxin was obtained by Sephadex column chromatography and disc gel electrophoresis. 3H-labeled cholera toxin retained its biological activity and chemical stability and had a specific activity of 405.9 muCi/mumol. The methods utilized in extraction and purification of 3H-labeled toxin may be advantageous for preparation of other biologically active radiolabeled proteins.

49. Facultatively Parasitic Strain of Bdellovibrio bacteriovorus

Abstract

A strain of Bdellovibrio bacteriovorus(designated strain UKi2) was isolated which was capable of growing either saprophytically in host-free medium or endoparasitically in Escherichia coliB/r. It was quantitatively determined that each bdellovibrio could develop in solid medium to produce a colony, and 65% of the cells in a late exponential-phase culture were capable of inducing E. coliB/r spheroplasts. A photomicrographic sequence of single E. colispheroplasts containing bdellovibrios demonstrated that parasitically derived B. bacteriovorusUKi2 could develop saprophytically after release from the host cells. Strain UKi2 appears to be morphologically quite similar to previously described obligately parasitic bdellovibrios; biochemical data on this strain suggests its close relationship to some of the previously described host-independent strains of Bdellovibrio.

50. An Analysis of Estimating Errors on Government Contracts.

Abstract

This thesis considers what measures should be used to evaluate estimating error of government estimates on competitively bid construction contracts. A data set from the U.S. Army Corps of Engineers was subjected to analysis using two measures of accuracy. One objective of this research is to develop mathematical models and find cumulative distributions which may describe estimating practices within the Corps of Engineers. Other objectives are to offer large scale adjustments to compensate for any error present and to give a baseline for future studied and/or attempts to improve estimating accuracy. It is found that there were significant estimating errors in the Corps of Engineer estimates. The variability of these errors is more than expected. The principal conclusions are: to retain as a measure of error, the standard percentage form, i.e., ERROR = ((OBSERVED - PREDICTED)/PREDICTED)*100; and large scale adjustments are not useful. Baselines were established for further research and study of estimating errors.
