3. Impact of Ramadan on sleep quality and habits: Comparative study between sick and healthy populations.

Fasting Ramadan can affect the normal circadian pattern, especially in terms of sleep times and patterns. The objective was to analyze sleep quality and habits in diseased and healthy population using the Pittsburgh Sleep Quality Index (PSQI) both during and outside of Ramadan. A cross-sectional descriptive study was carried out on 240 subjects (cardiovascular diseases [CVD], diabetes [DM]) and 120 healthy subjects. Data entry and processing were performed with R. 3.2.4 software. The significance level was 0.05. Regarding fasting, 88.33% of subjects with CVD fasted during Ramadan against 55.83% of subjects with DM (P > 0.05). PSQI results showed that subjects with CVD and DM were poor sleepers (PSQI > 5), regardless of the period during or outside Ramadan, unlike healthy subjects who had a good quality of sleep during Ramadan (PSQI ≤ 5). Sleep latency did not change for all subjects regardless of the period (P > 0.05). While the duration of sleep was always higher during Ramadan despite the population (P < 0.05). The observance of Ramadan influenced the potential duration of sleep, which was found to be longer during the holy month. However, no significant impact was observed on the quality of sleep of diseased subjects, nor on sleep latency for all subjects. [ABSTRACT FROM AUTHOR]

- Abstract
Diabetes causes peripheral neuropathy with loss of sensitivity of feet to pain, predisposing diabetic patients to a high risk of severe burns. Our retrospective study aimed to look at epidemiological, clinical, therapeutic and outcome characteristics of feet burns occurring in patients with diabetes, hospitalized in the burn trauma center of Tunis over 4 years (from 2019 to 2022). We included 34 patients, among which 9 had only feet burns. Their mean age was 60 years (range: 41-83 years), with male predominance (sex ratio = 1.83). A quarter of patients (n=9) were on oral antidiabetic drugs (OADs) and more than half (n=18) were at the stage of degenerative complications. At admission, blood glucose level was higher than 10 mmol/l in 73% of patients. TBSA was 19%. Twenty-two patients had deep feet burns, among which 5 patients underwent aponerrotomy for deep, circular burns. Amputation was done in 15 patients: toes (n=9), one limb (n=3) and two limbs (n=3). Duration of ICU stay was 18.3 days and mortality was 20.58%. [ABSTRACT FROM AUTHOR]

Gestational diabetes is a major problem during pregnancy, as the complications and repercussions for the foetus and the pregnant woman are so significant. The occurrence of acid ketosis during gestational diabetes remains exceptional and is rarely described in the scientific literature. For this reason, we present a case report of a 23-year-old primiparous woman of African origin with no known medical history who presented to emergency with vomiting associated with abdominal pain during pregnancy. Diabetes is a self-quantified chronic disease, where measuring blood sugars is the very grounds for understanding and surviving the condition. This article analyzes how people with type 1 diabetes who use continuous glucose monitors (CGMs) share their CGM data visualizations on social media platforms and develop data intimacies with other diabetics around the visual vernaculars of CGM graphs: the "flat line" ideal and the "shitty graph." CGM graphs mediate relationships between people with type 1 diabetes and how they feel their numbers. Based on a review of posts on r/diabetes_t1 over a period of nine months (August 2022-April 2023), the authors identified and analyzed two recurring visual vernaculars in screenshots of CGM graphs and the talk that occurred around them. As "domains of affective practice," these subreddit posts and talk around screenshots of CGM graphs mediate the "bad feelings" of diabetes and materialize the charged platform data intimacies between diabetics. Burns in diabetics are quite frequent and serious in relation to diabetic neuropathy, which is common in this population, delaying healing and predisposing to the risk of infection. The objective of this study was to describe the characteristics of burns in diabetics hospitalized at the CTB of Tunis over 18 months. During the study period, 891 patients were hospitalized, including 43 diabetics (5%). The average age of our patients was 57 years old with a male predominance (65%). Type 2 diabetes was present in 86% of cases and type 1 diabetes in 14% of cases. Degenerative complications were reported in 10 cases (23%), such as diabetic retinopathy (n=6), diabetic nephropathy (n=4) and diabetic neuropathy (n=6). The circumstances of burns were related to a domestic accident in 2/3 of the cases (76.7%). Thermal burns were involved in 83.7% of cases. 86% of the patients had decompensated their diabetes during their hospitalization. Sepsis marked the evolution of the patients in 55.8% of cases. In our study, poor prognostic factors were: a glycosylated haemoglobin >13%, an extent of burns greater than 20%, and a delay in consultation greater than 6 hours. The mortality rate was 18.6%. 12. The effect of herbal supplement and exercise training on plasma lipid profile in diabetic male rats

The present study was conducted to investigate the effect of six weeks of intermittent exercise and curcumin consumption on the lipid profile of diabetic male rats. In an experimental trial, 30 male Wistar rats were randomly divided into six groups (5 rats in each group): control, diabetic, healthy intermittent exercise, intermittent exercise +diabetes, curcumin +diabetes and curcumin+ exercise +diabetes. Diabetes was induced by intraperitoneal injection of streptozotocin poison (50 mg/kg). Intermittent training consisted of 5-12 bouts of intense work (75 to 100% of maximum speed) of 60 seconds with active rest intervals of 75 seconds, six days a week for six weeks. Curcumin was fed to animals by gavage at a dose of 50 mg per kilogram of body weight. After six weeks, unconscious animals and blood samples were collected from their hearts. The data were analyzed using ANOVA statistical test and Bonferroni post hoc test. Induction of diabetes caused an increase in cholesterol, TG and LDL levels (P=0.01). Intermittent training and curcumin, each alone, caused them to decrease (P=0.01). In addition, diabetes decreased HDL while intermittent exercise increased it, but curcumin supplementation did not have a significant effect. The effect of combining interval training with curcumin supplement was significant only for TG and did not have a significant effect on other data. Exercise training and curcumin can have an anti-inflammatory effect with reducing lipid profile and increasing HDL in diabetic animal.

IntroductionWe tried to apply a new surgical method to treat obesity combined with pancreatic pseudocyst and achieved satisfactory results.Case and presentationWe report a case of a severely obese patient with pancreatic pseudocyst who underwent robotic-assisted sleeve gastrectomy, while the pseudocyst was incised and cyst-jejunostomy was performed. The operation was successful, and the patient was discharged on the 8th day after the procedure. There were no complications during the perioperative period. After 12 months of follow-up examinations, the patient's pancreatic pseudocyst disappeared. Additionally, there was a significant decrease in body weight, body mass index, and other indicators. As a result, obesity and related metabolic diseases were completely relieved.ConclusionsThis case summarizes and presents the experience of using robotic bariatric surgery for the treatment of pancreatic pseudocyst. This case report indicates that this surgical procedure is both safe and effective for patients with pancreatic pseudocyst who also have obesity and related metabolic diseases.

14. Étude ethno-botanique, chimique et activité glycophage in vitro de Synsepalum dulcificum et S. stipulatum en RD Congo

Cette étude a été réalisée dans le cadre de la valorisation des produits forestiers non ligneux de la République Démocratique du Congo. Elle porte sur la connaissance de l’importance, des propriétés biochimiques et de l’activité glycophage in vitro des fruits de deux espèces de Synsepalum, à savoir S. dulcificum et S. stipulatum, provenant du Territoire de Bikoro, dans la Province de l’Equateur. Le matériel végétal est constitué des pulpes des fruits des deux espèces. Une enquête ethnobotanique, l’analyse des minéraux, les dosages des métabolites primaires et secondaires, et l’activité glycophage ont été réalisés sur les extraits aqueux des deux espèces. Les résultats montrent que ces deux plantes ont une grande importance alimentaire et médicinale. Les fruits sont essentiellement utilisés dans l’alimentation comme édulcorant, alors que la racine, l’écorce et dans une moindre mesure les feuilles, sont utilisés dans la prise en charge des hémorroïdes, la carie dentaire et les faiblesses sexuelles. Ces deux plantes contiennent essentiellement les éléments minéraux majeurs tels que sodium, magnésium, calcium, fer et phosphore. Les pulpes des fruits de deux espèces ont presque une composition presque similaire en métabolites primaires, elles sont riches en polyphénols, alcaloïdes, flavonoïdes, quinones, anthocyanes, tanins condensés et saponines et possèdent un effet glycophage. Introduction. La revue annuelle de la qualité des produits (RAQP), exigence internationale pour les formes finies et les substances actives, est particulièrement scrutée lors d’inspections et d’audits. Elle s'apparente à une validation rétrospective continue permettant de détecter toute dérive et de s’assurer de la fiabilité des procédés et de la qualité des produits. La RAQP du Gardénal 50mg (Phénobarbital) fabrique sur le site de Dakar en boite de 30 comprimes a porté sur cinq (5) lots Matériels et méthodes. L’évaluation des OOS/OOT a suivi la procédure CQ PRO 015. Les fiches de déclaration des résultats hors tendance ou non conformes obtenus lors de l’analyse au laboratoire, la description et la cause d’anomalie, le numéro de la déviation correspondante et de suivi des plans d’actions correctives et préventives (CAPA) ont été renseignées. Résultats. Les rendements, de 82,22-88,05%, étaient inférieurs à 98%. L’évaluation du poids moyen a montré des résultats conformes : 86.695-88.245 mg, pour une valeur cible de 87.500 mg. Les temps de désagrégation étaient conformes (60-637 sec), en dessous de la limite maximale de 900 sec. Il en a été de même pour le titre en Phénobarbital avec des résultats conformes de 47,80-50,80 mg/cp (moyenne 49,21 mg/cp). Deux déviations impactant deux lots ont été notées, respectivement, sur le dosage en principe actif pour le lot 2, le temps de désagrégation in process et le test de dissolution pour le lot 3 occasionnant son rejet après retraitement. Le lot 2 a fait l’objet d’une fiche OOT. Discussion. Quatre lots ont été libérés dans les 19 à 50 jours suivants leur fabrication. Huit actions correctives et préventives ont été clôturées à 100 % dans les délais. Seul le lot 3 a été rejeté après retraitement, suite à une dissolution non conforme 61% (limite 75%). Aucun lot n’a fait l’objet de réclamation, de rappel ou de retour. L’étude permet d’affirmer que le produit détient les qualités attribuées : l’évaluation du dosage en principe actif, des paramètres critiques in process, des paramètres de libération disponibles a donné des résultats conformes. Le process de fabrication a été validé.

Introduction. L’étude entre dans le cadre de valorisation des ressources de la pharmacopée africaine. Elle visait le recensement de plantes utilisées dans de la région de Thiès, centre du Sénégal, pour traiter le diabète et l'hypertension artérielle. Matériel et méthodes. Soixante-quinze (75) tradipraticiens, 78,66% d’hommes et 21,34% de femmes également répartis dans les trois départements de la région ont été interrogés sur un questionnaire et les données analysées quantitativement. Résultats. Sur 49 plantes recensées, 36,73% servaient au traitement du diabète, 26,53% à l’HTA et 36,73% pour les deux pathologies. Les plus citées furent : Sclerocarya birrea (18,05%), Combretum micrantum (16,9%), Rauwolfia vomitoria (9,98%), Catharantus roseus (7,32%), Balanites aegyptiaca (7,14%), Lonchocarpus sepium (7,35%) et Combretum glutinosum (6,62%). Les plus fréquemment prescrites étaient les mêmes : Sclerocarya birrea (15,46%), Combretum micrantum (14,9%), Rauwolfia vomitoria (8,39%), Balanites aegyptiaca (7,14%, Catharantus roseus (6%), Lonchocarpus sepium (5,84%) et Combretum glutinosum (5,84%). L’exploitation des feuilles (48,67%), racines (26,22%), fruits (15,44%), écorces (13,33%), graines (10,88%), fleurs (5,92%), bulbes (5,33%) et plantes entières (4,08%) permettait la préparation de décoctés (58,39%), d’infusés (44,28%), de poudres (18,53%), d’huiles (10,74%) essentiellement. Discussion. La position géographique de la région de Thiès, au centre du Sénégal, a permis d’enrôler des interlocuteurs de diverses ethnies pour une plus grande diversité de remèdes. Bien que nettement minoritaires, les tradipraticiennes avaient un important taux de fréquentation. Une convergence des pratiques est à noter en ce qui concerne les ressources utilisées et les préparations. La standardisation des procédures de prélèvement, de transformations et des doses administrées restent des écueils pour l’exploitation rationnelle de la Pharmacopée traditionnelle.

Diabetes is a chronic disease whose perceptions among the Malinké of Côte d'Ivoire lead to various social interpretations that influence its management. This qualitative study of the impact of perceptions on the therapeutic itinerary of diabetes in the Malinké elderly took place in the commune of Bouaké. Data were collected using purposive sampling and snowball methods. It appears from the analysis of the data that the perceptions related to diabetes in the Malinké populations are of sociocultural strains and are based on the symptoms and causes of diabetes. As a result, the therapeutic route of diabetes is defined by perceptions. However, the therapeutic choice of elderly diabetics is mixed based on both modern and traditional medicine. 24. Effect of eight weeks of vitamin D supplementation and water-based exercise on cardiometabolic profile in women with type 2 diabetes.

The aim of this study was to investigate the effect of eight weeks of vitamin D supplementation and water exercise on cardiometabolic profile in women with type 2 diabetes. Forty women with type 2 diabetes participated in this study voluntarily and randomly divided to four groups: 1) vitamin D+ water exercise, 2) vitamin D, 3) water exercise, and 4) control groups. Water exercise groups (groups 1 and 3) performed 3 sessions of water training per week during 8 weeks (15 minutes of warm-up, 45 to 60 minutes of main activity with a Borg scale of 14 and 10 minutes of cooling). Vitamin D groups (groups 1 and 2) received 50,000 (IU) vitamin D supplements once a week. Levels of fasting blood sugar (FBS), insulin, hemoglobin A1c (HbA 1c), C- reactive protein (CRP) levels, as well as lipid profile were measured before and after intervention. Insulin resistance was also obtained by calculating HOMA-IR. Weight, waist circumference, and body fat mass were significantly decreased after intervention in Vitamin D+ water exercise group (P = 0.001, 0.032, and 0.013 respectively), as well as CRP, glycemic and lipid profile were significantly improved after intervention in water exercise group. The results of the present study showed that water exercise plus vitamin D supplementation had synergism beneficial effects on anthropometric indices and lipid and glycemic status of type 2 diabetic patients and can be used as a non-pharmacological and non-invasive method to improving these mentioned parameters of type 2 diabetes. Pour traiter l'acidocétose diabétique pédiatrique, il faut porter une attention particulière aux liquides et aux électrolytes pour limiter le risque de complications, telles qu'une lésion cérébrale, associée à une morbidité et une mortalité élevées. L'incidence d'œdème cérébral en cas d'acidocétose diabétique n'a pas diminué malgré les protocoles visant la limitation des liquides qui s'appuient sur la restriction de la réanimation liquidienne initiale. Selon de nouvelles données probantes, l'administration précoce de liquides isotoniques n'entraîne pas de risque supplémentaire et peut améliorer les résultats cliniques chez certains patients. Les protocoles et les directives cliniques sont adaptés et axés particulièrement sur la surveillance et le remplacement initiaux et continus des liquides et des électrolytes. Il est maintenant recommandé de commencer par une réanimation à l'aide de liquides isotoniques chez tous les patients dans les 20 à 30 minutes suivant leur arrivée à l'hôpital, suivie par la réplétion du déficit volumique sur une période de 36 heures, en association avec une perfusion d'insuline et des suppléments d'électrolytes, ainsi qu'avec la surveillance et la prise en charge attentives d'une éventuelle lésion cérébrale. Background: Retinal vascular occlusions are a common cause of visual impairment. Studies on retinal vascular occlusions in sub-Saharan Africa (SSA) have primarily been retrospective and on retinal vein occlusion (RVO) only. The aim of this study, therefore, was to determine the prevalence and pattern of retinal vascular occlusions and their systemic associations in SSA. Materials and Methods: This was a hospital-based, cross-sectional study involving all new patients presenting at the general ophthalmic and specialty retina clinics in four hospitals in Nigeria over a 1 year period. All the patients underwent a comprehensive eye examination. The demographic and clinical data of patients with retinal vascular occlusions were entered into an excel sheet and analyzed using the Statistical Package for the Social Sciences (SPSS) software version 22.0. Statistical significance was indicated by P < 0.05. Results: A total of 8614 new patients were seen, and a diagnosis of retinal vascular occlusion was made in 90 eyes of 81 patients giving a disease prevalence of 0.9%. Eighty-one eyes of 72 (88.9%) patients had RVO, while 9 eyes of 9 (11.1%) patients had retinal artery occlusion (RAO). The mean age of patients with RVO and RAO was 59.5 years and 52.4 years, respectively. Increasing age, hypertension, and diabetes were the significant associations with retinal vascular occlusion with P < 0.0001. Conclusion: Retinal vascular occlusions are an increasing cause of retinal disease in SSA and tend to occur at an earlier age. They are associated with hypertension, diabetes, and increasing age. Further studies will, however, be required to establish the demographic and clinical profile of patients with RAO in the region. [ABSTRACT FROM AUTHOR]

Although the beneficial effects of regular physical activities have been well established in diabetes management over the past decade, different methods of physical activity have not been fully developed yet. This study aimed to examine whether performing training in water improves glucose homeostasis and lipocalins in women with type 2 diabetes mellitus (T2DM). Forty women with T2DM and 20 women without T2DM with an age range of 50 to 60 years participated in this study. They were randomly divided into three groups, including diabetic exercise (n = 20), diabetic control (n = 20), and healthy control (n = 20). Exercise group completed three sessions of exercise per week at 60–80% of maximum heart rate, under supervision, for three months. Blood samples were collected before and after the exercise regimen. Plasma Glycated hemoglobin (HbA1 C), retinol-binding protein 4 (RBP4), fatty acid-binding proteins (FABP), lipocalin-2 (LCN-2), insulin, and glucose were measured by ELISA method to calculate insulin resistance index. These findings indicated that after 12 weeks of exercise, TC, TG, LDL-C, FABP, LCN-2, and RBP4 were significantly reduced in the experimental group (p <0.05). Performing training in water seems to reduce the FBS, insulin, and HbA1 C to a higher degree as compared to the control group. Exercise type seems to be correlated with health benefits. Considering the improved lipocalin family proteins and lipid profile in postmenopausal women with T2DM, Performing training in water may help reduce cardiovascular risk factors. Chronic calcifying pancreatitis is a chronic inflammatory disease of the pancreas which leads to the development of parenchymal and ductal fibrosis. The main cause is chronic alcoholism. From its lesions arise symptoms, acute and chronic complications which will tag the evolution of this disease which takes place over 15 to 20 years. In addition to the clinical and paraclinical check-ups necessary for acute complications, the GI specialist must ensure the long-term follow-up. It will rely on several practitioners from different specialties reflecting the multidisciplinary nature of this disease. At the clinical level, a six-monthly consultation will address the problem of chronic pain (type, evolution, treatment, opiate medications, dependence), nutritional status, alcohol and tobacco consumption, signs of pancreatic exocrine insufficiency (weight, diarrhea) and diabetes, as well as complications related to chronic alcohol and tobacco consumptions. Biologically, exocrine insufficiency is detected by measuring fecal elastase (annually), and diabetes by measuring glycated hemoglobin every six months. It is also necessary to monitor the serum albumin level, the hydroelectrolytic, renal and hepatic parameters and the vitamin D level. Imaging (cholangiopancreatography MRI and/or CT scan) is necessary in case of complications or new symptoms and systematically over the long term to detect the occurrence of pancreatic adenocarcinoma, particularly in cases of hereditary chronic pancreatitis (imaging every year or two depending on the cause of the pancreatitis). If there is any doubt, an endoscopic ultrasound with biopsy will complete this assessment. It will therefore be necessary to adapt the treatment with analgesics, pancreatic extracts, oral antidiabetics or insulin, or even to discuss in certain cases a therapeutic endoscopy in an expert center. The multidisciplinary approach will involve the general practitioner, diabetologist, psychiatrist, addictologist, algologist, biliary and pancreatic endoscopist and sometimes even the pancreas surgeon. Objectif : Identifier les étiologies des infections et déterminer les facteurs de risque de mortalité chez le diabétique à Bouaké. Patients et méthode : Il s’agissait d’une étude rétrospective menée en médecine interne de janvier 2020 à décembre 2021. La population d’étude était constituée de patients diabétiques hospitalisés et infectés. L’analyse des données a été faite avec le logiciel Epi Info 7.2.3.1. Résultats : La prévalence de l’infection chez le diabétique hospitalisé était de 75,1%. L’âge moyen des patients était de 52 ans. Le sex-ratio était de 0,7. Le diabète était de découverte fortuite dans 50% et le diabète de type 2 (88,2%) prédominait. Les motifs d’hospitalisation étaient dominés par l’acidocétose (58,1 %), le déséquilibre glycémique (19,1 %) et l’hyperglycémie (10,3 %). La fièvre était présente dans 41,2 %. Les principales infections étaient les infections urinaires (29,4%), les pneumopathies (28,7%), le paludisme (21,3%), les infections cutanées (13,2%) et le foyer indéterminé (7,3%). Les germes identifiés étaient le plasmodium (21,3%), Escherichia coli (8,8%), staphylocoque (8,3%), des levures (8,3%) et enterobacter (6,7%). Les bêtalactamines (75,6%) étaient le traitement anti infectieux le plus prescrit. La mortalité était de 14,7% en lien avec le diabète de type 1 (p=0,001), l’ancienneté du diabète supérieur à 5 ans (p=0,005), la latence d’hospitalisation supérieure à 7 jours (p=0,001), le foyer cutanéomuqueux (p=0,005) et le sepsis (p=0,001). Conclusion : Les infections du diabétique sont fréquentes et les étiologies sont variées. Elles doivent être systématiquement recherchées chez le diabétique hospitalisé

Introduction Human toxocariasis is a parasitic disease caused by the larvae of Toxocara canis (T. canis) and can cause many clinical complications in the liver, lungs, eyes, brain and kidney. Despite its significant effect on high-risk groups (people with reduced immune system), this parasite is usually ignored. The present study was conducted with the aim of investigating the serum prevalence and risk factors of toxocariasis in diabetic patients and hemodialysis patients in Zahedan (southeast of Iran). Material and Method This study was conducted in Sistan and Baluchistan province, Zahedan city with 400 diabetic and hemodialysis patients. A clot sample was taken to perform the ELISA test along with a questionnaire containing demographic information. All serum samples were tested for the presence of IgG antibodies against Toxocara species by ELISA serology method. Data analysis was done using SPSS version 22 software and descriptive statistics and chi-square test. Results The prevalence of serum toxocariasis in diabetic and hemodialysis patients was reported as 2.8% and 3.5%, respectively. Among the variables of the study, contact with dogs and cats in the group of diabetic patients (OR=11.60 –% 95CI: 2.80-48.06). and in the group of hemodialysis patients contact with cats and dogs (OR=0.85- %95CI: 0.70-1.02) and blood group had a significant relationship with the prevalence of toxocariasis. P value ≤ 0.05. Conclusion Considering the few studies in the world, more epidemiological studies are needed to understand that diabetes and the hemodialysis process can be introduced as a factor in toxocariasis. Il s'agit de déterminer la fréquence hospitalière des patients diabétiques ayant un déclin rapide de leur fonction rénale et d'en rechercher les facteurs associés. Une étude transversale descriptive et analytique a été menée sur 12 mois (du 1
er mai 2019 au 31 avril 2020). Les patients diabétiques âgés de 18 ans et plus, ayant réalisé au moins 3 créatinines pendant les deux années précédentes, mais espacées d'au moins 6 mois, et ayant un débit de filtration glomérulaire estimé calculé de leur dernière créatinine supérieure à 30 mL/min par la formule du CDK-EPI ont été inclus. Nous avons évalué le débit de filtration glomérulaire estimé par la formule CDK-EPI, en utilisant le calculateur développé par le CHU de Poitiers et l'unité Inserm de la Société francophone du diabète. Au total, 80 dossiers de patients ont été retenus. Le déclin rapide de la fonction rénale était retrouvé chez 28 patients, soit une fréquence hospitalière de 35 %. Le sex-ratio H/F était de 1,5. L'âge moyen était de 62,93 ans (extrêmes 18–85 ans). L'hypertension artérielle était la comorbidité la plus retrouvée (92,5 %). Le très haut risque cardiovasculaire était prédominant dans 82,5 % des cas. Le risque rénal très élevé était retrouvé chez 20 patients, soit 25 %. L'analyse univariée et multivariée a montré que le déclin rapide de la fonction rénale était associé au très haut risque cardiovasculaire (p = 0,037) et au débit de filtration glomérulaire (p ˂ 0,001). Cette étude montre une fréquence hospitalière élevée du déclin rapide de la fonction rénale au Togo (35 %). Nos résultats ont permis d'identifier le très haut risque cardiovasculaire et le débit de filtration glomérulaire comme étant des facteurs de risque. L'originalité de notre étude était la mise en évidence de la proportion élevée du très haut risque cardiovasculaire (82,5 %) et du risque rénal très élevé (25 %) dans l'évaluation du risque rénal et cardiovasculaire. To determine the hospital frequency of diabetic patients with a rapid decline in their renal function, to look for the associated factors. Descriptive and analytical cross-sectional study carried out over 12 months (May 1, 2019 to April 31, 2020). Were included all patients aged 18 and over, having achieved at least 3 creatinine during the previous 2 years but spaced at least 6 months apart and having an eGFR calculated from their last creatinine greater than 30 mL/min by the formula of CDK-EPI. We evaluated the eGFR by the CDK-EPI formula using the calculator developed by the Poitiers University Hospital and the Inserm unit of the Francophone Diabetes Society. A total of 80 patients medical files were retained. The rapid decline in renal function was found in 28 patients, either a frequency of 35%. The sex ratio M/F was 1.5. The mean age was 62.93 years (range 18–85 years). Hypertension was the most common comorbidity (92.5%). The very high cardiovascular risk was predominant in 82.5% of cases. The very high renal risk was found in 20 patients, either 25%. Univariate and multivariate analysis showed that the rapid decline in renal function was associated with very high cardiovascular risk (P = 0.037) and glomerular filtration rate (P ˂0.001). this study showed a high hospital frequency of the rapid decline in renal function in Togo (35%). Our results have identified the very high cardiovascular risk and glomerular filtration rate as risk factors. The originality of our study was the demonstration of the high proportion of very high cardiovascular risk (82.5%) and very high renal risk (25%) in the evaluation of renal and cardiovascular risk. Objective: This study aims to investigate the effect of single urine C peptide/creatinine (UCPCR) in assessing the islet b Cell function of type 2 diabetes mellitus (T2DM) patients with different renal function. Methods: A total of 85 T2DM patients were recruited in this study, all the patients were assigned to one group with estimated glomerular filtration rate (eGFR)=60 ml·min
-1 ·1.73 m-2 and another group complicatedwith eGFR>60ml·min-1 ·1.73 m-2 . Serum creatinine, urine creatinine, serum fasting C-peptide (FCP), fasting blood glucose (FBG), glycosylated hemoglobin (HbA1C) and 24-hour urinary C-peptide (24hUCP) weremeasured. The modified homeostasis model assessment-islet β cell function [HOMA-islet (CP-DM)], the modified homeostasis model assessmentinsulin resistance [HOMA-IR(CP)] and UCPCR were calculated. Results: When compared with group eGFR =60 ml·min-1 ·1.73 m-2 , the levels of UCPCR, FCP, the modified HOMA-IR(CP) and HOMA-islet (CP-DM) were promoted and the concentrations of HbA1C, FPG, creatinine were decreased in the patients of eGFR>60 ml·min-1 ·1.73 m-2 (P<0.05); FCP was uncorrelated with 24hUCP while associated with UCPCR in the patients of eGFR = 60 ml·min-1 ·1.73m-2 ; UCPCR was positively correlatedwith FCP andHOMA-IR(CP) in the T2DM patients with different levels of renal function; the cut-off (UCPCR = 1.13 nmol/g) had 88.37% sensitivity and 95.24% specificity [95% confidence interval (CI):0.919-0.997] for identifying severe insulin deficiency in T2DM patients[area under the curve (AUC) 0.978]. Face à l’inflammation douloureuse d’origine dentaire,le diabétologue a prescrit des anti-inflammatoires non stéroïdiens. Deux semaines plus tard, le sujet revient avec un tableau clinique marqué par une nécrose tissulaire maxillo-faciale étendue. Résultats : La prise en charge s’est faite en urgence au bloc opératoire sous anesthésie générale. Les suites opératoires ont été simples. Cependant, on note des préjudices esthétiques, fonctionnels et un surcout financier non négligeable. Conclusion : La triade infection dentaire, diabète déséquilibré et prise d’anti-inflammatoire non stéroïdiens constitue un cocktail explosif susceptible de mettre en jeu le pronostic vital du patient.

La première utilisation réussie sur un être humain d’un extrait pancréatique – le 23 janvier 1922, sur la personne de Leonard Thompson, un garçon de 13 ans souffrant d’un diabète sévère traité à l’Hôpital général de Toronto – est généralement considérée comme le point culminant de la découverte de l’insuline. Cet évènement crucial de l’histoire de la médecine soulève pourtant une question évidente : « Ça marche, d’accord ; mais ensuite ? » L’histoire aurait bien pu finir abruptement sans la vision et l’esprit d’initiative du Dr John G. FitzGerald, directeur des laboratoires Connaught Antitoxin de l’Université of Toronto, installés dans le sous-sol du même bâtiment des sciences médicales où F. G. Banting et C. H. Best, sous la supervision de J. J. R. Macleod, avaient mené les recherches ayant conduit aux premiers tests effectués sur le jeune Thompson. Fait révélateur, c’est l’intégration à l’équipe du biochimiste J. B. Collip qui a permis la réussite du test le 23 janvier, grâce à ses efforts pour purifier l’extrait. En une journée, cependant, des tensions au sein du groupe, en particulier entre Banting et Collip, et la menace de Collip de quitter le groupe en emportant son brevet, ont poussé FitzGerald à l’action. Le 25 janvier, il mettait la dernière touche à une entente primordiale entre les laboratoires Connaught et Banting, Best, Collip et Macleod, entente qui allait assurer le développement des méthodes de préparation à grande échelle de l’extrait, dans le but réaliser d’autres tests cliniques. À partir de là, les laboratoires Connaught allaient jouer un rôle essentiel dans l’histoire de l’insuline, même si leur importance historique est souvent minimisée et mal comprise, éclipsée par le poids de la première découverte et la contribution cruciale d’Eli Lilly et Cie. Cet article traite de la participation étroite des laboratoires Connaught à l’histoire de l’insuline entre janvier 1922 et l’été 1924. On y suit la piste des difficultés et des innovations qui ont marqué le développement des méthodes de production à grande échelle, l’établissement et l’expansion de la capacité de production d’insuline au Canada, et le rôle de fer de lance joué par les laboratoires Connaught dans la distribution mondiale de l’insuline. This study aimed to demonstrate the efficacy of sildenafil citrate in order to improve the distal necrosis of randomized flaps in diabetic rats, and to explore new methods to reduce distal necrosis encountered in flap surgery. This is an experimental study in rats. The rats were first divided into three groups: Control(C), Diabetes(D), and Sildenafil(S). Streptozotocin 40 mg/kg was administered intraperitoneally to the rats in groups D and S that would develop diabetes. Two days after the procedure, blood glucose was measured from the tail vein of the rats, and the rats with a blood glucose level of 250 mg/dL and above were considered diabetic. 7 × 3 cm McFarlane flap was randomly planned on the back of the rats. In the flaps, ischemia was measured at the 30th minute with Na fluorescein, flap necrosis was measured on days 4 and 7, and specimens were collected from the critical zone and distal zone regions for histological examination. The comparison of ischemia and necrosis regions, ischemia was found to be more significant in groups D and S compared to group C (P < 0.05). In the comparison of necroses on days 4 and 7, it was determined that necrosis occurred significantly (P < 0.05) less in group S compared to the other groups. It was determined that necrosis in group D was significantly (P < 0.05) higher on days 4 and 7 compared to the other groups. The distal circulation is affected worse in randomized flaps in diabetes. Sildenafil citrate significantly increased the flap viability (P < 0.05). [ABSTRACT FROM AUTHOR]

46. Prédiction du taux de glucose chez les patients diabétiques comme séries temporelles biologiques

- Abstract
Le diabète est une maladie chronique caractérisée par un taux élevé de glucose dans le sang, résultant d'une production insuffisante d'insuline par le pancréas ou d'une utilisation inefficace de l'insuline par le corps. Le suivi continu du taux de glucose sanguin est crucial pour la gestion du diabète, permettant d'éviter les complications aiguës et chroniques associées à la maladie. Ce mémoire se concentre sur l'évaluation de la performance de différents modèles prédictifs – autorégressifs, d’apprentissage automatique, et de réseaux de neurones (ARIMA, XGBoost, LSTM, TCN et Prophet) – dans la prédiction du taux de glucose sanguin chez les patients diabétiques, en exploitant des données de capteurs de glucose en continu (SGC). La recherche s'est articulée en trois phases : la préparation des données de SGC, l'implémentation de divers modèles prédictifs, et leur évaluation à l'aide de métriques telles que l'erreur moyenne absolue (MAPE) et l'erreur quadratique moyenne (RMSE). Les résultats révèlent que les modèles autorégressifs sont préférables pour des prédictions à court terme, tandis que les modèles d’apprentissage automatique et les réseaux de neurones sont plus efficaces sur des périodes plus longues, avec une performance supérieure des réseaux de neurones à architectures profondes. Cette étude met en lumière l'importance de sélectionner le modèle approprié selon la durée de la prédiction, contribuant à l’amélioration des systèmes de surveillance et de gestion du diabète.

- Abstract
Several studies have detected a direct association between serum uric acid (SUA) and cardiovascular (CV) risk. In consideration that SUA largely depends on kidney function, some studies explored the role of the serum creatinine (sCr)-normalized SUA (SUA/sCr) ratio in different settings. Previously, the URRAH (URic acid Right for heArt Health) Study has identified a cut-off value of this index to predict CV mortality at 5.35 Units. Therefore, given that no SUA/sCr ratio threshold for CV risk has been identified for patients with diabetes, we aimed to assess the relationship between this index and CV mortality and to validate this threshold in the URRAH subpopulation with diabetes; the URRAH participants with diabetes were studied (n = 2230). The risk of CV mortality was evaluated by the Kaplan–Meier estimator and Cox multivariate analysis. During a median follow-up of 9.2 years, 380 CV deaths occurred. A non-linear inverse association between baseline SUA/sCr ratio and risk of CV mortality was detected. In the whole sample, SUA/sCr ratio > 5.35 Units was not a significant predictor of CV mortality in diabetic patients. However, after stratification by kidney function, values > 5.35 Units were associated with a significantly higher mortality rate only in normal kidney function, while, in participants with overt kidney dysfunction, values of SUA/sCr ratio > 7.50 Units were associated with higher CV mortality. The SUA/sCr ratio threshold, previously proposed by the URRAH Study Group, is predictive of an increased risk of CV mortality in people with diabetes and preserved kidney function. While, in consideration of the strong association among kidney function, SUA, and CV mortality, a different cut-point was detected for diabetics with impaired kidney function. 48. Time trends in liver-related mortality in people with and without diabetes: Results from a population based study

- Abstract
Context: Patients with diabetes are at increased risk of dying from liver-related events, but little is known on whether this increased risk changed in recent years. Objective: The aim of the present study is to describe time trends in cause-specific liver-related mortality in people with and without diabetes from the general Italian population. Methods: Data were retrieved from the healthcare utilization databases of Lombardy, a region of Italy that accounts for about 16% (almost ten million) of its population. Annual cause-specific mortality rates and proportionate mortality were computed among individuals with and without diabetes from 2010 to 2019. Liver-related deaths were categorized as viral, alcohol related and non-viral non-alcohol related (NVNA). Results: Liver diseases were responsible for 2% and 1% of deaths in people with and without diabetes (2019). Among patients with diabetes, the crude mortality rate for liver diseases decreased from 1.13 to 0.64 deaths per 1,000 person-years from 2010 to 2019. The largest proportion of liver-related deaths was attributable to NVNA diseases and it increased from 63% in 2010 to 68% in 2019, with a corresponding relative reduction of viral causes (from 27% to 23%). The Standardized Mortality Ratio for patients with diabetes was 3.35 (95% CI 2.96-3.76) for NVNA, 1.66 (95% CI 1.33-2.01) for viral hepatitis and 1.61 (95% CI 1.13-2.17) for alcoholic liver disease and it remained relatively stable over time. Excess mortality risk in patients with diabetes for liver-related mortality was higher than for cardiovascular mortality and cancer. Conclusion: While liver-related mortality rates decreased significantly among patients with diabetes, NVNA causes comprised the majority of cases. Excess mortality for liver-related causes in patients with diabetes compared with controls remained constant in the studied period.

- Abstract
Aims: To evaluate the impact of adherence to glucagon like peptide-1 receptor agonists (GLP1-RA) and sodium-glucose transporter two inhibitors (SGLT2-I) on clinical outcomes and costs in patients with type 2 diabetes mellitus (T2DM). Materials and Methods: The 121,115 residents of the Lombardy Region (Italy) aged ≥40 years newly treated with metformin during 2007–2015 were followed to identify those who started therapy with GLP1-RA or SGLT2-I. Adherence to drug therapy over the first year was defined as the proportion of days covered >80%. Within each drug class, for each adherent patient, one non-adherent patient was matched for age, sex, duration, adherence to metformin treatment and propensity score. The primary clinical outcome was a composite of insulin initiation, hospitalisation for micro- and macrovascular complications and all-cause mortality after the first year of drug treatment. Costs were evaluated based on reimbursements from the national healthcare system. Results: After matching, 1182 pairs of adherent and non-adherent GLP1-RA users and 1126 pairs of adherent and non-adherent SGLT2-I users were included. In both groups, adherent patients experienced a significantly lower incidence of the primary outcome (HR: 0.85, 95% CI 0.72–0.98 for GLP1-RA and HR: 0.69, 95% CI 0.55–0.87 for SGLT2-I). A significant reduction in hospitalizations was found for adherent patients in the GLP1-RA group but not for the SGLT2-I group. Results were consistent when analyses were stratified by age and sex. While higher drug-related costs in the adherent group were counterbalanced by decreased hospitalisation costs in SGLT2-I treated patients, this was not the case for GLP1-RA. Conclusions: Higher adherence to drug treatment with GLP1-RA and SGLT2-I during the first year of the drug intake is associated with a lower incidence of adverse clinical outcomes in a real-world setting.
