5. Meatotomie moyenne : indications et resultats

- Abstract
Introduction : La méatotomie moyenne a pour objectif le rétablissement du drainage muco-ciliaire et de l’aération du sinus maxillaire. La méatotomie d’accès ou d’abord permet des gestes de résection et la méatotomie de drainage et d’aération vise à restaurer la fonction ostiale altérée. Le but de notre travail est d’analyser les indications de la méatotomie moyenne et de discuter nos résultats en les comparant à ceux de la littérature.Patients et Méthodes: Il s’agit d’une étude rétrospective menée sur une période de 16ans pendant la quelle nous avons colligé 339 méatotomies moyennes pratiquées chez 281 patients. Les résultats de la méatotomie moyenne ont été évaluées cliniquement et endoscopiquement , dans certaines situations un scanner de contrôle a été demandé.Résultats: Nous avons scindé notre série en deux groupes selon deux volets d’indication : des méatotomie moyennes d’abord prati- quées dans 131 cas et des meatotomies d’aération du sinus maxillaire dans 208 cas. Les résultats anatomiques étaient jugés bons dans 88,97% des cas, alors qu’un échec fonctionnel de la méatotomie était noté dans 28,67% des cas. La seule complication tardive était la synéchie notée dans 10.61% des cas.Conclusion : La méatotomie moyenne est la technique de choix dans le traitement des pathologies pseudo tumorales maxillaires. Dans les méatotomies de drainage, l’indication doit être mûrement réfléchie au vu d’un bilan préopératoire rigoureux pour garantir de bons résultats fonctionnels. Une technique rigoureuse et des soins postopératoires rapprochés sont nécessaires pour garantir un bon résultat anatomique.Introduction: The medium meatotomy aims the restoration of mucociliary drainage and aeration of the sinuses. The meatotomy access makes tumor and pseudo-tumor resection and meatotomy drainage and aeration is to restore ostial function. The Aim of our study is to analyse indications and our results of medium meatotomie, and to compare it with the review of literature.Patients and Methods: It is a retrospective study during 16 years, about 339 medium meatotomie, the post operative results are appreciated by cliniquely and by endoscopies, and in some cases a scan was followed.Results: we have 2 indications of medium meatotomie : medium meatotomie of access in 131cases and those to drain the maxillary sinus in 208 cases. The anatomic result was good in 88,97%, functional failure was noted in 28,67%. Synechie was the only later complication noted in 28,67% of the casesConclusion: The medium meatotomie is the best technique in the pseudo tumours of maxillary sinus .In the draining meatotomie we must deeply think about indications, and undergone rigorous preoperative analyses, to guarantee good functional results . A rigorous technique and repeated postoperative cares are necessary to guarantee good anatomic results.

6. Pyocele du cornet moyen : a propos d’un cas

Introduction : Les mucocèles sont des formations kystiques bénignes mais expansives, tapissées par un épithélium respiratoire, sans connexion avec les cavités sinusiennes ou nasales. Elles se développent le plus souvent dans le complexe ethmoïdofrontal. Le terme pyocèle correspond à une mucocèle dont le contenu est infecté. La localisation turbinale moyenne est exceptionnelle.Observation : Nous rapportons l’observation d’un patient âgé de 75 ans qui se plaignait d’une obstruction nasale chronique gauche. L’examen clinique montrait un cornet moyen élargi obstruant la totalité de la fosse nasale gauche. L’Imagerie (TDM et IRM) a révélée une formation ovalaire d’allure kystique expansive développée aux dépens du cornet moyen, associée à un polype postérieur. Le diagnostic de mucocèle sur concha bullosa a été porté. Le patient a été opéré par voie endonasale avec marsupialisation et polypectomie. Les suites opératoires étaient simples avec amélioration des signes cliniques.Conclusion : Les pyocèles du cornet moyen sont des lésions rares bénignes, toutefois destructrices. Un bilan radiologique est indispensable pour guider le choix du traitement chirurgical. La voie endonasale reste le traitement de référence pour cette pathologie.Mots clés : Concha bullosa, Pyocèle, Obstruction nasaleIntroduction : A mucocele is an epithelium-lined cavity that contains mucus that fills the sinus and is capable of expansion. The majority of mucoceles are situated in the frontal and/or ethmoid sinuses. A pyocele occurs when a mucocele becomes infected. Concha bullosa mucopyocele is a rare entity that presents with persistent nasal symptoms.Case report : This paper presents patient aged 75 who were admitted with persistent nasal complaints. Anterior rhinoscopy showed enlarged middle turbinates obstructing the nasal cavities. MRI and CT Scann revealed expansif cystic lesion in the middle turbinate with a nasal polyp. With the diagnosis of concha bullosa mucocele, lateral part of the affected middle turbinate and the polyp were resected with an endoscopic approach. There were no intraoperative or postoperative complications. Nasal obstruction vanished postoperatively.Conclusion: Mucopyocele of middle turbinate are rare benign lesions. Radiological assessment is essential to guide the choice of surgical treatment. Endonasal approach is the standard treatment for this disease.Key words : Concha bullosa, Pyocele, Nasal obstruction

Introduction: Les goitres plongeants, devenus rares de nos jours, posent des problèmes diagnostiques et chirurgicaux particuliers. Leur prise en charge est facilitée par l’apport de la tomodensitometrie permettant de planifier l’attitude thérapeutique.Le but de ce travail est de préciser les modalités d’exploration et de prise en charge chirurgicale des goitres plongeants.Matériels et méthodes : Nous rapportons une étude rétrospective portant sur42 cas de goitre plongeant colligés sur une période de 11 ans entre 2001 et 2011.Résultats : L’âge moyen de nos patients était de 56,9 ans (36-86 ans). Le sex-ratio était de 0,13. La tuméfaction basi-cervicale était le motif de consultation le plus fréquent, rapportée dans 74 % des cas associée à des signes de compression oesotrachéale dans 45% des cas.Une paralysie récurrentielle unilatérale a été objectivée dans 3 cas.Une radiographie de thorax a montré un élargissement médiastinal dans 23% des cas avec déviation trachéale chez63 % des malades. Une TDM cervico-thoracique pratiquée dans 85% des cas a confirmé le diagnostic. Le traitement chirurgical était mené par voie cervicale exclusive chez tous les patients. Une paralysie récurentielle a été observée dans 3 cas. Aucune hypoparathyroïdie définitive n’a été rapportée avec un reculmoyen de 24mois.Conclusion : Les goitres plongeants devenus rares du fait de la prise en charge plus précoce des nodules thyroïdiens. Ils posent actuellement moins de difficultés thérapeutiques. L’indication chirurgicale est toujours formelle devant le risque vital qu’ils posent.Mots clés : goitre cervicothoracique, thyroïdectomie, paralysie récurrentielle, hypoparathyroïdie.Introduction: Substernal goiters, becoming rare, present diagnositic and therapeutic problems.Their management is facilitated by the contribution of computed tomography for planning the therapeutic approach. The aim of this study is to specify the procedures for exploration and surgical management of substernal goiters.Materials and Methods: We report a retrospective study of 42 cases of substernalgoiters collaged over a period of 11 years between 2001 and 2011.Results: The mean age was 56,9 years (36- 86 years). The sex ratio was 0,13. Cervical swelling was the most common reason for consultation, reported in 74% of cases associated with signs of oesotrachealcompression in 45% of cases. Unilateral laryngeal palsy has been objectified in 3 cases. Chest-x-Ray showed widening of mediastinal shadowwith tracheal deviation in 63% of patients. A cervico-thoracic CT performed in 85% of cases confirmed the diagnosis of substernal goiters. The cervical approach has been used in all cases. Laryngeal nerve palsy was observed in 3 cases. No definitive hypoparathyroidism have been reported with a mean of 24 months.Conclusion: Substernal goiters become rare due to the earlier diagnosis of thyroid nodules. They currently present fewer therapeutic difficulties, and must be managed surgically because of vital risk.Key words: substernal goiter, thyroidectomy, laryngeal nerve palsy, hypoparathoidism.

13. Locally advanced nasopharyngeal follicular dendritic cell sarcoma misdiagnosed as meningioma.

- Abstract
Follicular dendritic cell sarcoma (FDCS) is a rare malignancy, often challenging to diagnose due to its nonspecific presentation and resemblance to other neoplasms. This case highlights a locally advanced nasopharyngeal FDCS initially misdiagnosed as a meningioma, underscoring the importance of differential diagnosis in unusual tumor presentations. A 77-year-old patient presented with nasal obstruction for 3 months. Imaging studies revealed a mass in the nasopharynx prolapsing into the oropharynx, leading to an initial diagnosis of meningioma based on its radiologic appearance. Surgical resection was performed, and histopathological examination, supplemented by immunohistochmical staining, revealed features consistent with follicular dendritic cell sarcoma. The tumor was locally advanced, involving surrounding structures, which complicated the initial clinical and radiologic assessment., Competing Interests: Declarations. - Abstract
Vibsatin A is a new neurotrophic vibsane-type diterpenoid comprising a bridged bicyclo[4.2.1]nonane skeleton. Inspired by Sawamura's works, we generated the bicyclic backbone through a Conia-ene-derived 7- exo-dig cyclization from an enantiomerically enriched TIPS-based silyl enol ether. The reaction, catalyzed by a sensitive gold(I) complex, was efficiently performed on a large scale by glovebox free techniques. Furthermore, the shape of this system was exploited for subsequent installation of all of the stereogenic centers.

- Abstract
Malignant tumors of the temporal bone are rare. They include a wide histological variety. They are mostly primary tumors with an estimated incidence of 0.8-1.0 per 1,000,000 inhabitants per year [1]. Metastases form an uncommon subgroup. The clinical features of these temporal bone metastases are nonspecific and predominantly consist of hearing loss, vertigo, facial palsy, tinnitus, headache, otalgia or otorrhoea [2]. The aim of our publication is to report a rare case of metastasis in the temporal bone as initial manifestation of lung cancer. Our patient was a 44-year-old man who presented for a right mastoid swelling with a grade VI right facial nerve palsy. Imaging and biopsy confirmed the diagnosis of small cell lung carcinoma petrous metastasis. The treatment was palliative. An osteolytic lesion of the temporal bone in an elderly patient may fit with a primary tumor of the temporal bone. It must, also, suggest the diagnosis of metastasis. The malignant tumor most frequently responsible is breast cancer, but lung cancer must also be considered in smoking patients.

A formal [3+2] cyclization mediated by silver(I) oxide and 1,8-diazabicyclo[5.4.0]undec-7-ene (DBU) is described herein. Through a broad variety of carbonyl compounds, this system can promote cyclization reactions with high yield (up to 85 %) and diastereoselectivity (up to 95:5) for a straightforward access to complex and congested dihydrofuran derivatives in one step under mild conditions. Background: By virtue of their multiple functions, lips are essential organs. Thus, lip cancers represent a distinct entity in head and neck surgery. Their treatment is fundamentally surgical. We aimed, in this study, to define modalities of repairing techniques of lip defects after surgical resection and post-operative complications., Methods: We conducted a 4 years retrospective descriptive study including 70 adult patients admitted with lip cancers., Results: Suture, local and regional flaps were the performed techniques for repairing defects (respectively n=28, 35 and 7). The most common postoperative complications were wound dehiscence (n=4), and surgical site necrosis (n=2) and infection (9 patients). Functional sequelae were dominated by microstomia (n=26), (food leakage) (n=9) and gingival exposure (n=11). Aesthetic results were good, fair or poor respectively in 54.3%; 32.9% and 12.8% of patients., Conclusion: Lip cancers surgery is relatively well-codified. However, it may be difficult to choose the best repairing technique, especially in case of large lip defect considering functional and aesthetic prejudices.

- Abstract
The aim was to evaluate the clinical impact of IGF-1/IGF-1R in Tunisian laryngeal carcinoma. A high IGF-1R immunohistochemical expression was found in our series (81.43%). A tendency toward an association between IGF-1R expression and lymph node metastasis was found ( p = 0.068). Patients with positive IGF-1R expression showed a short disease free survival ( p = 0.053) and a high recurrence rate. Furthermore, circulating IGF-1 levels sera, detected by ELISA, were higher among patients compared to controls ( p < 0.001). IGF-1R might have a prognostic significance and could be a factor of tumor recurrence. However, high levels of IGF-1 increase the risk of developing of LSCC disease.

- Abstract
Introduction: The contribution of transoral laser microsurgery (TLM) in the management of early glottic cancer is now well established. The anterior commissure (AC) is a delicate anatomic subsite due to its difficult access and the high risk of tumour spread from this site to the thyroid cartilage, the subglottis and the pre epiglottic space. These reasons make the endoscopic approach controversial in case of AC involvement., Methods: This is a single centre retrospective study performed between the years 2012 and 2015. Eighteen patients treated with advanced Va laser cordectomies were included., Results: All tumours were staged T1 (78% were T1a and 12% T1b). After laser surgery 4 patients (22.22%) had positive margins. These patients were candidate to second look procedure allowing detection and treatment of residual disease. Oncologic outcomes were studied. Two patients experienced local relapses within a mean delay of 7 months. The local control rate was 88.9% while the local control using laser only was 83.33%. One patient underwent a total laryngectomy (TL) followed by adjuvant radiation therapy (RT) after a massive recurrence in the AC with infiltration of the thyroid cartilage. The laryngeal preservation rate was 94.44%. The 3 years overall survival rate reached 100% while the 3 years disease free survival was 88.9%., Conclusion: the TLM offers acceptable oncologic outcomes in the treatment of T1 laryngeal cancer with AC involvement.

20. Ti(II) and Rh(I) Complexes as Reagents toward a Thapsigargin Core.

- Abstract
A novel approach toward the [5-7]fused bicyclic core of thapsigargin, a subnanomolar inhibitor of the endo/sarcoplasmic calcium ATPase (SERCA), is presented. The synthetic route includes an original Ti(II)-mediated hydroxy-directed reductive coupling of an enantiomerically enriched propargylic alcohol and an intramolecular Rh(I)-catalyzed cyclocarbonylation reaction as key steps. Interestingly, through the first experiments of titanocene-mediated reductive cyclization of a 1,8-enyne, a seven-membered cycle was isolated as a unique product with a total diastereoselectivity.

Laryngocele is a rare benign lesion of the larynx caused by an abnormal dilatation of the laryngeal saccule. We report the case of a 78-year-old man presenting bilateral cervical painless soft mass. Bilateral external laryngocele diagnosis was confirmed by CT scan and the patient underwent a surgical resection.

- Abstract
Introduction: Pharyngocutaneous fistula is the most frequent complication of primary and secondary laryngectomy and pharyngolaryngectomy. It has an important impact on the patient, his entourage and the healthcare team. The risk factors for pharyngostoma are multiple and its treatment is not codified., Aim: To identify the various factors that may predispose to this complication and to describe the different therapeutic means to control it., Methods: This retrospective study examined 68 cases of laryngeal cancer treated by total laryngectomy or pharyngolaryngectomy at Salah Azaїez's head and neck oncologic surgery department between 2015 and 2017., Results: Most of our patients were male (97%) who were major consumer of tobacco and alcohol. The most common stage were T4aN0M0 (34%) and T4aN1M0 (22%). Regarding treatment, we found that 73.5% of patients were operated on by total laryngectomy, 17.7% of patients were operated on by total pharyngolaryngectomy and 8.8% of patients were operated on by square laryngectomy. Pharyngostoma was diagnosed in 20.5% of cases within an average of 15 days. Among the risk factors studied, those that were statistically correlated with the occurrence of CPF in univariate studies were pre-laryngeal infiltration (p = 0.006), postoperative CRP level (p= 0.017), extension to piriform recess (p = 0.009), TNM stage (p=0.039), an associated thyroid surgery (p = 0.020) and pharyngeal closure under tension (p=0.000). In multivariate studies, five of the risk factors identified in the univariate analysis were found. This testifies to their major role as a risk factor for pharyngostoma. These factors were pre-laryngeal infiltration (p=0.001), postoperative CRP level (p=0.000), piriform recess involvement (p=0.004) Associated thyroid surgery (p=0.012) and pharyngeal closure under tension (p = 0.000)., Conclusion: By identifying predictive factors for pharyngostoma, we can identify patients in which this complication may occur and manage the medical care means to prevent it more efficiently.

A strategy for the assembly of the entire carbon backbone of a stereoisomer of the antitumor marine natural product hemicalide has been investigated. 24. Laryngocele after Subtotal Laryngectomy.

- Abstract
Introduction: Laryngocele is an air-filled dilatation of the laryngeal saccule that extends upward within the false vocal folds. Different etiologies lead to laryngocele congenital malformation, weakness of the laryngeal tissues and increased intralaryngeal pressure. Laryngocele may be a secondary iatrogenic complication following subtotal laryngectomy., Case Report: We report the case of a 61-year-old patient who presented an external laryngomucocele 8 years after a supracricoid partial laryngectomy with cricohyoidoepiglottopexy (SCPL-CHEP). We focus on the clinical aspects and therapeutic attitude, then discuss the physiopathological conditions that could generate this late complication., Conclusion: Laryngocele after subtotal laryngectomy should be considered a late iatrogenic complication. Histological examination is necessary after surgical management of laryngocele, as the association with cancer is frequent.

- Abstract
Synovial cell sarcoma is a soft tissue tumor that occurs predominately in the lower limbs near the joints. Lesions of the head and neck are extremely rare. The larynx is the least frequent site with only about 20 cases reported in the literature. Treatment of these tumors is controversial and should follow the guidelines for other tumor sites. We report the case of a 37-year-old man with primary laryngeal synovial cell sarcoma, who was surgically managed by a narrow field laryngectomy.

The synthesis of the C1-C27 fragment of hemicalide, a marine metabolite displaying a unique potent antiproliferative activity, has been accomplished. The synthetic approach highlights a remarkably efficient ring-closing metathesis reaction catalyzed by Nolan ruthenium indenylidene complexes to elaborate the highly substituted δ-lactone framework.

- Abstract
Ewing's sarcoma is a high-grade neuroectodermal primary bone tumor. This is the second primary bone tumor in children afterosteosarcoma and represents 4 to10% of cases. It can occur in all skeletal bones. However, the location at the facial bones is uncommon (1to2%) and extremely rare at the sphenoid sinus. We report the clinical results of a rare case of Ewing's sarcoma of the sphenoid with intraorbitaland intracranial extension.

The development of an intramolecular rhodium(I)-catalyzed Pauson-Khand reaction of alkoxyallene-ynes with a proximal alkoxy group is reported. This reaction, in the presence of a [Rh(cycloocta-1,5-diene)Cl]2/propane-1,3-diylbis(diphenylphosphane) system under a CO atmosphere, constitutes a powerful tool for selectively accessing carbo- and heterobicyclo[5.3.0] frameworks featuring an enol ether moiety.
