Abstract

Background In the general population with coronavirus disease 2019 (COVID-19), obesity is associated with an increased risk of mortality. Given the typically observed obesity paradox among patients on kidney function replacement therapy (KFRT), especially dialysis patients, we examined the association of obesity with mortality among dialysis patients or living with a kidney transplant with COVID-19. Methods Data from the European Renal Association COVID-19 Database (ERACODA) were used. KFRT patients diagnosed with COVID-19 between 1 February 2020 and 31 January 2021 were included. The association of Quetelet's body mass index (BMI) (kg/m2), divided into: Results In 3160 patients on KFRT (mean age: 65 years, male: 61%), 99 patients were lean, 1151 normal weight (reference), 1160 overweight, 525 obese I and 225 obese II/III. During follow-up of 3 months, 28, 20, 21, 23 and 27% of patients died in these categories, respectively. In the fully adjusted model, the hazard ratios (HRs) for 3-month mortality were 1.65 [95% confidence interval (CI): 1.10, 2.47], 1 (ref.), 1.07 (95% CI: 0.89, 1.28), 1.17 (95% CI: 0.93, 1.46) and 1.71 (95% CI: 1.27, 2.30), respectively. Results were similar among dialysis patients (N = 2343) and among those living with a kidney transplant (N = 817) (Pinteraction = 0.99), but differed by sex (Pinteraction = 0.019). In males, the HRs for the association of aforementioned BMI categories with 3-month mortality were 2.07 (95% CI: 1.22, 3.52), 1 (ref.), 0.97 (95% CI: 0.78. 1.21), 0.99 (95% CI: 0.74, 1.33) and 1.22 (95% CI: 0.78, 1.91), respectively, and in females corresponding HRs were 1.34 (95% CI: 0.70, 2.57), 1 (ref.), 1.31 (95% CI: 0.94, 1.85), 1.54 (95% CI: 1.05, 2.26) and 2.49 (95% CI: 1.62, 3.84), respectively. Conclusion In KFRT patients with COVID-19, on dialysis or a kidney transplant, obesity is associated with an increased risk of mortality at 3 months. This is in contrast to the obesity paradox generally observed in dialysis patients. Additional studies are required to corroborate the sex difference in the association of obesity with mortality.

Abstract

Background Early reports on the pandemic nature of coronavirus disease 2019 (COVID-19) directed the nephrology community to develop infection prevention and control (IPC) guidance. We aimed to make an inventory of strategies that dialysis centres followed to prevent infection with COVID-19 in the first pandemic wave. Methods We analyzed IPC measures taken by hemodialysis centres treating patients presenting with COVID-19 between 1 March 2020 and 31 July 2020 and that completed the European Renal Association COVID-19 Database centre questionnaire. Additionally, we made an inventory of guidelines published in European countries to prevent spread of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in dialysis centres. Results Data from 73 dialysis units located in and bordering Europe were analyzed. All participating centres implemented IPC measures to mitigate the impact of SARS-CoV-2 during the first pandemic wave. Measures mentioned most often included triage with questions before entering the dialysis ward, measuring body temperature, hand disinfection, masking for all patients and staff, and personal protective equipment for staff members. These measures were also recommended in most of the 14 guidelines that were identified in the inventory of national guidelines and were also scored as being among the most important measures by the authors of this paper. Heterogeneity existed between centres and national guidelines regarding the minimal distance between dialysis chairs and recommendations regarding isolation and cohorting. Conclusions Although variation existed, measures to prevent transmission of SARS-CoV-2 were relatively similar across centres and national guidelines. Further research is needed to assess causal relationships between measures taken and spread of SARS-CoV-2.

Abstract

The finding of white matter damage on brain magnetic resonance imaging may correspond to a wide variety of etiologies. The differential diagnosis may be particularly difficultin immunocompromised patients with a specific autoimmune disease or who are receiving medications after a solid-organ transplant. Herein, we describe the case of a 22-year-old woman who developed serious neurological complications after an acute rejection of a kidney graft that she had received a few months previous to treat a systemic lupus erythematosus-related nephritis.We discuss the possible hypotheses underlying the development of acute leukoencephalopathy in this setting.

Abstract

In March 2022, Kidney Disease: Improving Global Outcomes (KDIGO) held a virtual Controversies Conference to address the important but rarely examined phase during which the kidney transplant is failing or has failed. In addition to discussing the definition of a failing allograft, 4 broad areas were considered in the context of a declining functioning graft: prognosis and kidney failure trajectory; immunosuppression strategies; management of medical and psychological complications, and patient factors; and choice of kidney replacement therapy or supportive care following graft loss. Identifying and paying special attention to individuals with failing allografts was felt to be important in order to prepare patients psychologically, manage immunosuppression, address complications, prepare for dialysis and/or retransplantation, and transition to supportive care. Accurate prognostication tools, although not yet widely available, were embraced as necessary to define allograft survival trajectories and the likelihood of allograft failure. The decision of whether to withdraw or continue immunosuppression after allograft failure was deemed to be based most appropriately on risk-benefit analysis and likelihood of retransplantation within a few months. Psychological preparation and support was identified as a critical factor in patient adjustment to graft failure, as was early communication. Several models of care were noted that enabled a medically supportive transition back to dialysis or retransplantation. Emphasis was placed on the importance of dialysis-access readiness before initiation of dialysis, in order to avoid use of central venous catheters. The centrality of the patient to all management decisions and discussions was deemed to be paramount. Patient "activation," which can be defined as engaged agency, was seen as the most effective way to achieve success. Unresolved controversies, gaps in knowledge, and areas for research were also stressed in the conference deliberations.

Abstract

L’année 2022 a été celle du retour progressif à la normale après la pandémie COVID-19. Une occasion d’optimiser les processus parfois mis à mal et de mettre à jour les procédures. Une opportunité de se lancer dans de nouveaux défis : réfléchir à l’acquisition de nouveaux équipements pour les années à venir, à l’implémentation de nouvelles techniques plus sensibles, plus rapides, mieux adaptées aux besoins des prescripteurs. Au sein du Laboratoire d’hématologie spéciale, et en particulier dans le secteur de l’hémostase spécialisée, l’automatisation du dosage de la protéine ADAMTS13 était indispensable pour le diagnostic et le suivi biologique des patients présentant un purpura thrombotique thrombocytopénique immun (PTTi). Le décours de cette affection, peu fréquente, peut être rapidement péjoratif en l’absence d’un diagnostic précoce et d’une prise en charge adaptée, guidée par le dosage d’ADAMTS13. Ce dernier permet également d’identifier les patients à risque de rechute qui doivent ainsi bénéficier d’un traitement préventif. La détermination des groupes sanguins érythrocytaires s’effectue la plupart du temps sans difficulté, par des techniques sérologiques. Cependant, dans certaines situations, il est nécessaire de recourir à des techniques de biologie moléculaire. Le Laboratoire d’immunohématologie – biologie moléculaire a implémenté une technique de PCR en temps réel qui permet de réaliser un génotypage érythrocytaire très étendu et de détecter des variants antigéniques passés inaperçus en technique sérologique. Il s’agit d’un outil indispensable pour la détermination de groupes sanguins chez des patients polytransfusés et pour la sélection de poches pour des personnes ayant un groupe sanguin variant ou en délétion. On sait depuis plus de 50 ans que le système HLA joue un rôle central dans l’immunité humaine et qu’il constitue aussi l'un des principaux obstacles à la transplantation d’organe. L’implication des anticorps anti-HLA dans le rejet précoce et tardif est indiscutable. Le Laboratoire d’immunologie leuco-plaquettaire a implémenté en 2021 un nouveau test pour une identification plus spécifique des anticorps anti-HLA cliniquement plus pertinents, permettant de faciliter l’attribution d’un greffon. Durant cette année 2022, les dossiers des patients en attente d’une greffe rénale et qui présentaient des anticorps anti-HLA ont été revus et des contrôles ont été réalisés sur d’anciens sérums en utilisant la nouvelle technique. Ceci a permis de retirer des spécificités anticorps « interdites » chez plus de la moitié des patients augmentant ainsi leur chance de bénéficier d’un greffon compatible. Six patients ont reçu un greffon rénal portant un antigène HLA précédemment répertorié comme interdit avec l’ancienne technologie et retiré après la révision. Actuellement, ces patients ont, tous, montré un greffon fonctionnel sans complication immunologique. Ces résultats seront présentés au congrès de l’EFI (European Foundation for Immunogenetics) en avril 2023. Bien que la transfusion soit une procédure thérapeutique courante et bénéfique pour le patient, elle peut s’accompagner de réactions transfusionnelles. Le diagnostic de celles-ci est parfois flou et malaisé. Le secteur Hémovigilance de la transfusion a dessiné de nouveaux algorithmes décisionnels à partir de la symptomatologie majeure présentée par le patient et du composant sanguin incriminé dans la réaction. Ces algorithmes ont pour but d’orienter vers un diagnostic et, pour ce faire, de déterminer les contrôles et les tests de laboratoire qui doivent être réalisés dans chaque situation. Ces algorithmes ont fait l’objet d’une publication dans la revue de la Société Francophone de Transfusion Sanguine. Bien que des prémices le laissaient suspecter déjà en 2021, la situation de l’approvisionnement en concentrés érythrocytaires de groupe sanguin O négatif s’est encore fortement aggravée durant l’année 2022. La Banque de sang, en concertation avec la direction médicale et le Comité de transfusion, a dû y faire face en mettant en place différentes actions pour tenter d’endiguer l’« hémorragie ». Ceci fait l’objet d’une procédure particulière : mise en place d’indicateurs très stricts et surveillés de façon très étroite, priorisation des indications transfusionnelles, distribution unitaire exclusive (en dehors des situations hémorragiques), décision médicale de transfuser des concentrés érythrocytaires Rh positifs à des receveurs Rh négatifs. Diverses évaluations très précises de cette situation sont encore en cours et seront soumises prochainement à publications. Une action concertée a été mise en place au sein des hôpitaux du réseau et des initiatives sont également prises au niveau national. Finalement, l’année 2022 a également été celle de la publication d’une nouvelle directive européenne et de nouveaux standards EDQM (European Directorate for the Quality of Medicines) définissant désormais le sang comme SoHO (Substance of Human Origin), au même titre que les cellules et les tissus. C’est la raison pour laquelle, il a été décidé que la Banque de sang et le laboratoire d’immunologie érythrocytaire rejoignaient le Département des Banques le 1er janvier 2023.
