Numerical simulations are performed to investigate the flow within the end-to-side proximal anastomosis of a pulsatile pediatric ventricular assist device (PVAD) to an aorta. The anastomotic model is constructed from a patient-specific pediatric aorta. The three great vessels originating from the aortic arch-- brachiocephalic (innominate), left common carotid, and left subclavian arteries--are included. An implicit large eddy simulation method based on a finite volume approach is used to study the resulting turbulent flow. A resistance boundary condition is applied at each branch outlet to study flow splitting. The PVAD anastomosis is found to alter the aortic flow dramatically. More flow is diverted into the great vessels with the PVAD support. Turbulence is found in the jet impingement area at peak systole for 100% bypass, and a maximum principal normal Reynolds stress of 7081 dyn/[cm.sup.2] is estimated based on ten flow cycles. This may be high enough to cause hemolysis and platelet activation. Regions prone to intimal hyperplasia are identified by combining the time-averaged wall shear stress and oscillatory shear index. These regions are found to vary, depending on the percentage of the flow bypass. We use an implicit large eddy simulation (ILES) method based on a finite volume approach to capture the turbulence in the anastomoses of a left ventricular assist device (LVAD) to the aorta. The order-of-accuracy of the numerical schemes is computed using a two-dimensional decaying Taylor-Green vortex. The ILES method is carefully validated by comparing to documented results for a fully developed turbulent channel flow at [Re.sub.[tau]] = 395. Two different anastomotic flows (proximal and distal) are simulated for 50% and 100% LVAD supports and the results are compared with a healthy aortic flow. All the analyses are based on a planar aortic model under steady inflow conditions for simplification. Our results reveal that the outflow cannulae induce high exit jet flows in the aorta, resulting in turbulent flow. The distal configuration causes more turbulence in the aorta than the proximal configuration. The turbulence, however, may not cause any hemolysis due to low Reynolds stresses and relatively large Kolmogorov length scales compared with red blood cells. The LVAD support causes an acute increase in flow splitting in the major branch vessels for both anastomotic configurations, although its long-term effect on the flow splitting remains unknown. A large increase in wall shear stress is found near the cannulation sites during the LVAD support. This work builds a foundation for more physiologically realistic simulations under pulsatile flow conditions. The closing behavior of mechanical heart valves is dependent on the design of the valve and its housing, the valve composition, and the environment in which the valve is placed. One innovative approach for improving the closure dynamics of tilting disk valves is introduced here. We transformed a normal Delrin occluder into one containing a 'dynamic liquid core' to resist acceleration and reduce the moment of inertia, closing velocity, and impact forces of the valve during closure. The modified occluder was studied in the mitral position of a simulation chamber under the physiologic and elevated closing conditions of 2500 mm Hg/s and 4500 mm Hg/s, respectively. Cavitation energy, detected as high-frequency pressure transients with a hydrophone, was the measure used to compare the modified valve with its unaltered counterpart. The cavitation potential of tilting disk valves is indicative of the extent of blood damage occurring during valve closure. Initial findings suggest that changes to the structure or physical properties of well established mechanical valves, such as the one described here, can reduce closure induced hemolysis by minimizing cavitation. Compared with a normal valve, the cavitation intensity associated with our modified valve was reduced by more than 66% at the higher load. Furthermore, the modified valve took longer to completely close than did the standard tilting disk valve, indicating a dampened impact and rebound of the occluder with its housing. The mortality rate for infants awaiting a heart transplant is 40% because of the extremely limited number of donor organs. Ventricular assist devices (VADs), a common bridge-totransplant solution in adults, are becoming a viable option for pediatric patients. A major obstacle faced by VAD designers is thromboembolism. Previous studies have shown that the interrelated flow characteristics necessary for the prevention of thrombosis in a pulsatile VAD are a strong inlet jet, a late diastolic recirculating flow, and a wall shear rate greater than 500 [s.sup.-1]. Particle image velocimetry was used to compare the flow fields in the chamber of the 12 cc Penn State pediatric pulsatile VAD using two mechanical heart valves: Bjork-Shiley monostrut (BSM) tilting disk valves and CarboMedics (CM) bileaflet valves. In conjunction with the flow evaluation, wall shear data were calculated and analyzed to help quantify wall washing. The major orifice inlet jet of the device containing BSM valves was more intense, which led to better recirculation and wall washing than the three jets produced by the CM valves. Regurgitation through the CM valve served as a significant hindrance to the development of the rotational flow. Hemolysis and thrombosis are among the most detrimental effects associated with mechanical heart valves. The strength and structure of the flows generated by the closure of mechanical heart valves can be correlated with the extent of blood damage. In this in vitro study, a tilting disk mechanical heart valve has been modified to measure the flow created within the valve housing during the closing phase. This is the first study to focus on the region just upstream of the mitral valve occluder during this part of the cardiac cycle, where cavitation is known to occur and blood damage is most severe. Closure of the tilting disk valve was studied in a 'single shot' chamber driven by a pneumatic pump. Laser Doppler velocimetry was used to measure all three velocity components over a 30 ms period encompassing the initial valve impact and rebound. An acrylic window placed in the housing enabled us to make flow measurements as close as 200 [micro]m away from the closed occluder. Velocity profiles reveal the development of an atrial vortex on the major orifice side of the valve shed off the tip of the leaflet. The vortex strength makes this region susceptible to cavitation. Mean and maximum axial velocities as high as 7 m/s and 20 m/s were recorded, respectively. At closure, peak wall shear rates of 80,000 [s.sup.-1] were calculated close to the valve tip. The region of the flow examined here has been identified as a likely location of hemolysis and thrombosis in tilting disk valves. The results of this first comprehensive study measuring the flow within the housing of a tilting disk valve may be helpful in minimizing the extent of blood damage through the combined efforts of experimental and computational fluid dynamics to improve mechanical heart valve designs. Cavitation is known to cause blood element damage and may introduce gaseous emboli into the cerebral circulation, increasing the patient's risk of stroke. Discovering methods to reduce the intensity of cavitation induced by mechanical heart valves (MHVs) has long been an area of interest. A novel approach for analyzing MHV cavitation is presented. A wavelet denoising method is explored because currently used analytical techniques fail to suitably unmask the cavitation signal from other valve closing sounds and noise detected with a hydrophone. Wavelet functions are used to denoise the cavitation signal during MHV closure and rebound. The wavelet technique is applied to the signal produced by closure of a 29-mm Medtronic-Hall MHV in degassed water with a gas content of 5 ppm. Valve closing dynamics are investigated under loading conditions of 500, 2500, and 4500 mm Hg/s. The results display a marked improvement in the quantity and quality of information that can be extracted from acoustic cavitation signals using the wavelet technique compared to conventional analytical techniques. Time and frequency data indicate the likelihood and characteristics of cavitation formation under specified conditions. Using this wavelet technique we observe an improved signal-to-noise ratio, an enhanced time-dependent aspect, and the potential to minimize valve closing sounds, which disguise individual cavitation events. In order to bridge the gap of existing artificial heart technology to the diverse needs of the patient population, we have been investigating the viability of a sealed-down design of the current 70 cc Penn State artificial heart. The issues of clot formation and hemolysis may become magnified within a 50 cc chamber compared to the existing 70 cc one. Particle image velocimetry (PIV) was employed to map the entire 50 cc Penn State artificial heart chamber: Flow fields constructed-firm PIV data indicate a rotational flow patter, that provides washout during diastole. In addition, shear rate maps were constructed for the inner walls of the heart chamber: The lateral walls of the mitral and aortic ports experience high shear rates while the upper and bottom walls undergo low shear rates, with sufficiently long exposure times to potentially induce platelet activation or thrombus formation. In this study, we have demonstrated that PIV may adequately map the flow-fields accurately in a reasonable amount of time. Therefore, the potential exists of employing PIV as a design tool. Particle image velocimetry (PIV) has been gaining acceptance as a routine tool to evaluate the flow fields associated with fluid mechanical devices. We have developed algorithms to investigate the wall shear-rates within the 50cc Penn State artificial heart using low magnification, conventional particle image velocimetry (PIV). Wall shear has been implicated in clot formation, a major post-implant problem with artificial hearts. To address the issues of wall scattering and incomplete measurement volumes, associated with near wall measurements, we have introduced a zero masking and a fluid centroid shifting technique. Simulations using different velocity fields were conducted with the techniques to assess their viability. Subsequently, the techniques were applied to the experimental data collected. The results indicate that the size of the interrogation region should be chosen to be as small as possible to maximize resolution while large enough to ensure an adequate number of particles per region. In the current study, a 16x16 interrogation window performed well with good spatial resolution and particle density for the estimation of wall shear rate. The techniques developed with PIV allow wall shear-rate estimates to be obtained from a large number of sites at one time. Because a planar image of a flow field can be determined relatively rapidly, PIV may prove useful in any preliminary design procedure.

Evaluation of cavitation in vivo is often based on recordings of high-pass filtered random high-frequency pressure fluctuations. We hypothesized that cavitation signal components are more appropriately assessed by a new method for extraction of random signal components of the pressure signals. We investigated three different valve types and found a high correlation between the two methods ([r.sup.2]:0.8806-0.9887). The new method showed that the cavitation signal could be extracted without a priori knowledge needed for setting the high-pass filter cut off frequency, nor did it introduce bandwidth limitation of the cavitation signal.
