1. Influence of crop management on stability rye yield and some grain quality traits.

Abstract

The study explored the performance of hybrid and population rye (Secale cereale) cultivars under two different crop management intensities in Poland: moderateintensity and high-intensity management. The focus was grain yield, grain quality, yield components, and variety stability in two growing seasons (2018/2019 and 2019/2020) at three locations. Hybrid cultivars consistently yielded higher grain yields (9.81 t ha−1) than population cultivars (7.90 t ha−1), with increase of 24.9%. However, hybrid cultivars had lower protein content (8.94%) than population cultivars (9.77%). Spike number was the most influential factor on yield, followed by 1000-grain weight and grains per spike, regardless of cultivar type. Hybrid cultivars displayed a lower degree of stability as assessed using the ranking sum of the Shukla variance and the multi-trait stability index. Hybrid cultivars were strongly dependent on resistance to fungal diseases, including Septoria (Mycosphaerella graminicola), and increased stability under moderate-intensity management. 2. Bipolar endo-epicardial RF ablation: Animal feasibility study.

Abstract

Bipolar radiofrequency ablation (B-RFA) is a method used to treat the arrhythmia substrate resistant to unipolar ablation. Few studies have addressed endo-epicardial B-RFA. The aim of the study was to evaluate chronic lesions resulting from endo-epicardial B-RFA and to determine optimal settings for such procedures in an animal model. In 7 pigs, up to 5 radiofrequency applications per animal were performed with 2 electrodes placed on both sides of the left ventricular free wall. Current was delivered for 60 seconds by a generator dedicated for B-RFA with power settings of 25, 30, 35, 40, and 50 W. At 12 weeks after ablation, 31 lesions were assessed. Their maximal cross-sectional area ranged from 7.2 to 68 mm2 and correlated with total power delivered (r = 0.53), with temperature increment at the endocardial catheter (r = 0.65), and inversely with temperature decrement at the epicardial catheter (r = 0.54). For power values between 30 and 40 W, the lesion area did not differ significantly (P =.92). Lesion depth ranged from 1.9 to 11 mm and correlated with impedance decrement (r = 0.5). Lesions were transmural in 8 cases. Lesion depth/wall thickness ratio was on average 0.6 ± 0.3, with the smallest value for 25 W (0.5 ± 0.3) and the largest for 50 W (0.8 ± 0.3). Steam pops occurred at a power range of 30–50 W, with an incidence of 1 in 5 applications, with 1 case of fatal tamponade at 40 W. Impedance decrement, endocardial catheter temperature increment, and endocardial electrogram amplitude decrement were greater during applications with steam pops. Chronic lesions resulting from endo-epicardial B-RFA appear smaller and less often transmural compared with acute lesions described in the literature. Etripamil is a fast-acting, intranasally administered calcium-channel blocker in development for on-demand therapy outside a health-care setting for paroxysmal supraventricular tachycardia. We aimed to evaluate the efficacy and safety of etripamil 70 mg nasal spray using a symptom-prompted, repeat-dose regimen for acute conversion of atrioventricular-nodal-dependent paroxysmal supraventricular tachycardia to sinus rhythm within 30 min.

Abstract

Despite extensive conventional endoepicardial ablation, significant intramural arrhythmogenic substrate may remain out of reach of unipolar radiofrequency ablation (RFA). The authors present clinical findings and procedural workflow for bipolar radiofrequency ablation (B-RFA) with 1 catheter placed against the endocardium and the other in the pericardial sac to ablate refractory ventricular arrhythmias. No serious adverse events occurred during B-RFA procedures, and the short-term and midterm clinical results were satisfactory. Optimal catheter choice and ablation parameters settings for B-RFA remain to be determined.

Abstract

Epicardial access is becoming increasingly important for various cardiovascular interventions. Access to dry pericardial space can be challenging and is often associated with significant complications. A novel concealed-needle blunt-tip device is designed to capture the parietal pericardium layer and retract it into the distal end of the device, which houses a fixated concealed needle, in a bid to minimize the likelihood of lacerating the visceral layer of the pericardium. This prospective single-arm study evaluated the feasibility of use of this device in 11 human subjects with successful access attained in 91% (10 of 11) of cases without adverse events. 14. Grain yield performance and stability of winter wheat and triticale cultivars in a temperate climate

Abstract

The productivity of modern triticale (× TriticosecaleWittm. ex A. Camus) cultivars makes them more viable as an alternative to wheat (Triticum aestivumL.) grains in many environments. We tested 51 winter wheat and 18 winter triticale cultivars in 52 locations nested in six regions in Poland. The aim of the study was to check whether the yield of triticale is comparable with the yield of wheat, especially under unfavorable soil and climatic conditions. The grain yield of winter wheat ranged from 9.22 to 15.72 t ha–1depending on the region, and the yield of triticale ranged from 9.24 to 14.82 t ha–1. The analysis showed that triticale is well adapted to the temperate environment, and its yield is more stable and less dependent on the weather conditions of the year than that of winter wheat. The results indicated that triticale and wheat grown in lower quality soils will obtain a very similar yield. We also recommend that triticale cultivar selection should be based on specific end use requirements. So far, a similar comparison has been made for Mediterranean and temperate climates, but this is the first work for a transitional temperate climate. We compare grain yield stability and adaptability of wheat and triticale cultivars.The adaptability of wheat and triticale cultivars strongly depends on environmental conditions.The yield of triticale is more stable than that of wheat under unfavorable growth conditions.Triticale is a good alternative to wheat in terms of grain yield value.

15. Cardiac Pulsed Field Ablation Lesion Durability Assessed by Polarization-Sensitive Optical Coherence Reflectometry.

Abstract

BACKGROUND: Pulsed field ablation uses electrical fields to cause nonthermal cell death over several hours. Polarization-sensitive optical coherence reflectometry is an optical imaging technique that can detect changes in the tissue ultrastructure in real time, which occurs when muscular tissue is damaged. The objective of this study was to evaluate the ability of a polarization-sensitive optical coherence reflectometry system to predict the development of chronic lesions based on acute changes in tissue birefringence during pulsed field ablation. METHODS: Superior vena cava isolation was performed in 30 swine using a biphasic, bipolar pulsed field ablation system delivered with a nonirrigated focal tip catheter. Acute changes in tissue birefringence and voltage abatement were analyzed for each individual lesion. A high-resolution electroanatomical map was performed at baseline and 4 to 12 weeks after ablation to locate electrical gaps in the ablated area. RESULTS: A total of 141 lesions were delivered and included in the analysis. Acute electrical isolation based on the electroanatomical map was achieved in 96% of the animals, but chronic isolation was only seen in 14 animals (46%). The mean voltage abatement of lesions that showed recovery was 82.8%±14.6% versus 84.4%±17.4% for those that showed fibrosis (P =0.7). The mean acute reduction in tissue birefringence in points demonstrating fibrosis was 63.8%±11.3% versus 9.1%±0.1% in the points that resulted in electrical gaps. A threshold of acute reduction of birefringence of ≥20% could predict chronic lesion formation with a sensitivity of 96% and a specificity of 83%. CONCLUSIONS: Acute tissue birefringence changes assessed with polarization-sensitive optical coherence reflectometry during pulsed field ablation can predict chronic lesion formation and guide the ablation procedure although limited by the tissue thickness. 20. Characteristic features of patients with multiple accessory pathways

Abstract

AbstractObjective Only limited clinical and electrophysiological data concerning patients (pts) with multiple accessory pathways (MAP) in comparison to large control groups are available.The aim of our study was to analyse these data from the largest cohort of patients with multiple accessory pathways and a large control group.Method and results We analysed data from pts with MAP (group 1) and pts with a single accessory pathway (AP) (group 2) referred for radiofrequency catheter ablation (RFCA) at our tertiary centre.Group 1 consisted of 124 pts (M 62.10%, mean age 33.00 ± 5.26) with MAP and RFCA. Group 2 consisted of 376 pts (M 51.20%, mean age 35.87 ± 16.15) with a single accessory pathway and RF ablation.Group 1 exhibited a higher incidence of overt APs (P <0.0001), Ebstein anomaly (P =0.001), ventricular fibrillation (P =0.012), antidromic atrioventricular re-entrant tachycardia (A AVRT) (P =0.025) and male gender (P =0.038). The mean age at the first documented atrioventricular re-entrant tachycardia (AVRT) episode was lower in pts with MAP than in pts with single APs: 16.79 ± 13.41 vs 20.84 ± 14.29, respectively (P =0.001). Concealed accessory pathways (P <0.0001) occurred more frequently in the control group. Group 1 had more right-lateral (P =0.0001), mid-septal (P =0.0001), left-posterior (P =0.01), left-anterior (P =0.013) and left-lateral localizations of AP (P <0.037).Conclusions The MAP group included statistically significantly more men, Ebstein anomaly and overt APs. The mean age of the first episode of atrioventricular re-entrant tachycardia was lower in pts with MAP. Certain distribution patterns are apparent for single and MAP. Pts with MAP are at higher risk of VF and antidromic atrioventricular re-entrant tachycardia.

Abstract

Sleep disturbance is common in Parkinson’s disease (PD). In this study we investigated the effect of a novel therapeutic tool, repetitive transcranial magnetic stimulation (rTMS) on sleep quality in PD patients. The study group consisted of 11 PD patients who underwent ten daily rTMS sessions at 15Hz. Their sleep patterns were monitored with polysomnography. After the stimulation, non-REM stage-1 sleep and the number of nocturnal arousals decreased, thus improving sleep quality. These changes were probably related to the improvement of motor symptoms observed in UPDRS and in the 9 Hole peg test. [ABSTRACT FROM AUTHOR]

Introduction:Atrial fibrillation (AF) is the most common sustained arrhythmia. Catheter Ablation (CA) is considered as a first-line therapy for AF. The risk of 30-days all-cause mortality associated with CA for AF may be underestimated in clinical trials and selected-cohort studies due to selection bias.Hypothesis:We evaluated 30-days all-cause mortality associated with CA for AF in the entire country of PolandMethods:The data was collected from the Polish national healthcare service (the National Health Fund of Poland). In the Polish health system covers >99% of CA for AF for the entire country. All consecutive CA in patients >18 years of age, between Jan 2012 and Dec 2019 were included in the study. CA for AF was identified by unique codes assigned to pulmonary vein isolation and CA for AF. The mortality rate was calculated for each age group.Results:During 8-year follow-up period, 31,214 CA of AF were performed in 26,767 patients. 9315 (34.8%) patients were female. 13758 (51.4%) patients had Cryoballoon ablation. Thirty-two deaths (0.1%) within 30-days of the CA were identified. The mortality rate was low and similar in each age group except in the oldest age group (age>80). The oldest age group had significantly higher 30-days mortality than any other age group. (Figure).Conclusions:In the large, consecutive patient population in the entire country of Poland, 30- days mortality following AF ablation for past 8-years remains low. In real-world clinical practice patients over the age of 80 have higher risk of 30-day all-cause mortality than any other age group found over last seven years. Systematic review is needed to further risk-stratify these findings.

Abstract

Atrioventricular nodal reentry tachycardia base on reentry circulation in nodal-perinodal area. The radical treatment of choice is radiofrequency ablation. Procedure approached from the anterior-superior (fast) region sufficient a few seconds of energy delivery for success, however this can result in A-V block. The possibility that arrhythmias substrate may lie very superficially (success of ablation) and damage the normal structures (complication) in the perinodal region must be considered. In order to confirm this hypothesis we examined the autopsy material of 100 normal hearts, both sexes from 18 to 105 years of age (control) and 50 hearts with A-V total block 45-95 years of age (block). We paid attention to the morphology of the nodal artery (NA), atrial inputs (AI) and transitional inputs (TI). It was observed that NA at the level of the central fibrous body was positioned in 94% in the central and in 6% in the inferior part of Koch's triangle. It was removed from the endocardium 3-6 mm in control and 2-5 mm in block group respectively (NS). In the perinodal area we distinguished AI that directly joined the A-V compact node: superficial (right part of the interatrial septum) or deep (left part). The former occurred in 100% of controls and in 80% of block groups (NS), and the latter in 80% of control group and in 34% in block respectively (p0.05). The real substrate of arrhythmia in anterior-superior region lies very superficially and far from the conduction tissue; NA in examined hearts was lying deep beneath the endocardium; ablation close to the node could result in A-V block.
