Each physician had more than 30 fibromyalgia patients in their practice. The study design consisted of four phases. In phase one, physicians undertook a self-assessment of their practice. Phase two of the study involved diagnosis and treatment of a virtual case vignette. The third phase consisted of analysis of the data from phase two and providing feedback from an expert rheumatologist, and the fourth phase was to complete patient report forms for five patients in their practice. The year-long study was completed by 12 physicians and resulted in data on 60 patients. The results of this study provide an insight into how physicians are diagnosing and treating patients with fibromyalgia. However, the cost burden is felt even more by smaller, less well-funded continuing medical education (CME) programs. It is possible to overcome financial and participant-related barriers to measuring Level 6 outcomes, which are patient health outcomes. The Temple University School of Medicine’s Office for Continuing Medical Education developed a sequential tool for attaining cost-effective outcomes measurement for determining the likelihood of a CME intervention to produce significant changes in physician performance. The appropriate selection of the CME topic and specific practice change indictors drive this tool. This tool walks providers through a simple YES or NO decision-making list that guides them toward an accurate prediction of potential programmatic outcomes. Factors considered during the decision-making process include whether: (a) the intended change(s) will have a substantial impact on current practice; (b) the intended practice change(s) are well supported by clinical data, specialty organization/government recommendations, expert opinion, etc; (c) the potential change(s) affects a large population; (d) external factors, such as system pressures, media pressures, financial pressures, patient pressures, safety pressures, etc, are driving this intended change in performance; (e) there is a strong motivation on the part of physicians to implement the intended change(s); and (f) the intended change(s) is relatively easy to implement within any system of practice. If each of these questions can be responded to positively, there is a higher likelihood that the intended practice-related change(s) will occur. Two-dimensional, time-dependent magnetohydrodynamic computer simulations facilitated the study of formation of a hot hydrogen working fluid that can be incorporated in multiple concentric solid-density linear implosions. Electron number densities and temperature and neutral densities of approximately 0.3 to 1.0 times 10 to the power 19 per cm cubed were obtained at low limits.

12. An Imaging Laser Altimeter for Lunar Scientific Exploration

Abstract

A new approach to laser altimetry is offered by the development of micro-lasers and pixilated detectors that enable very high resolution measurement of topography and relatively wide swath observations. An imaging altimeter with a 8x8 array detector working at a probability of less than a single photon/shot could map the Moon or similar sized body in approximately 2 years and provide 5 meter horizontal resolution topography and a 10 centimeter vertical accuracy. In addition, it would provide surface roughness and surface slopes on similar length scales of 5 meters and be able to address a range of problems for which topography or lunar shape is important at the decimeter level. This includes the topography of the polar regions, where ice is thought to have been identified, and also the cratering history of the Moon which could be assessed with a dataset of uniform quality and high resolution.

For over 40 years, NASA's global network of satellite laser ranging (SLR) stations has provided a significant percentage of the global orbital data used to define the International Terrestrial Reference Frame (ITRF). The current NASA legacy network is reaching its end-of-life and a new generation of systems must be ready to take its place. Scientific demands of sub-millimeter precision ranging and the ever-increasing number of tracking targets give aggressive performance requirements to this new generation of systems. Using lessons learned from the legacy systems and the successful development of a prototype station, a new network of SLR stations, called the Space Geodesy Satellite Laser Ranging (SGSLR) systems, is being developed. These will be the state-of-the-art SLR component of NASA's Space Geodesy Project (SGP). Each of SGSLR's nine subsystems has been designed to produce a robust, kilohertz laser ranging system with 24/7 operational capability and with minimal human intervention. SGSLR's data must support the aggressive goals of the Global Geodetic Observing System (GGOS), which are 1 millimeter (mm) position accuracy and 0.1 mm per year stability of the ITRF. This paper will describe the major requirements and accompanying design of the new SGSLR systems, how the systems will be tested, and the expected system performance.

Abstract

Compression by a spherical solid liner of a gold target surrounded by a hydrogen plasma is simulated. Two-dimensional simulations that treat only a subset of the physics included in the one-dimensional code were performed in an attempt to assess multidimensional effects. A one-dimensional numerical code has been developed to study the effects of thermal radiation and conduction. Results of pressure, density, and energy deposited for different initial plasma conditions are presented and discussed. Results from both one- and two-dimensional codes show that the average target density at peak compression is 39-43 g/cu cm, using the SHIVA Star facility at 90 kV discharge.

Abstract

The scientific areas which used data from the Lunar Laser Ranging Experiment, collected from measurements to the Apollo 11, 14, and 15 and Lunakhod 2, include lunar science (i.e., studies of variations in the lunar angular orientation from that for uniform rotation, lunar tidal displacements, and the lunar mass distribution), geodynamics, astrometry, and gravitational physics. This paper argues that the placement of microwave and optical transponders on the moon would improve the accuracy of laser range measurements by nearly two orders of magnitude and would simplify the measurements. The K-band microwave transponders would be operated at the lunar base and at two remote sites on the moon surface, yielding much improved lunar libration and tidal displacement measurements. A two-wavelength laser transponder also would be operated at the lunar base, allowing accurate tropospheric propagation corrections to be made. This would introduce major improvements in measurements of the lunar orbit and of the earth's rotation, and in tests of general relativity.

Abstract

A demonstration-prototype CO2-laser heterodyne spectrometer operating at 9-12 microns and suitable for long-term space missions is described and illustrated with extensive diagrams, drawings, photographs, and graphs of test performance data. The spectrometer has total volume 0.63 cu m, mass 30 kg, and power requirement 60-70 W, compatible with miniature-class Space Shuttle experiment payload specifications. It comprises three modules: (1) an optical front end with reflecting optics, a 2-GHz BW HgCdTe photomixer, and a 0-2-GHz 40-dB RF preamplifier; (2) a local oscillator with an RF-excited waveguide CO2 laser, a 75-percent-efficiency RF amplifier, a stepper-driven grating mode selector, and an etalon stabilized for over 30,000 h of use; and (3) an RF-filter-bank spectral-line receiver with a 25-MHz RF channel, 1.6-GHz IF spectral coverage, onboard instrument control, a serial link to the host computer, and highly integrated design.

Abstract

The goal of the Field-Reversed Configuration Heating Experiment (FRCHX) is to demonstrate magnetized plasma compression and thereby provide a low cost approach to high energy density laboratory plasma (HEDLP) studies, which include such topics as magneto-inertial fusion (MIF). A requirement for the field-reversed configuration (FRC) plasma is that the trapped flux in the FRC must maintain confinement of the plasma within the capture region long enough for the compression process to be completed, which is approximately 20 microseconds for FRCHX. Current lifetime measurements of the FRCs formed with FRCHX show lifetimes of only 7 9 microseconds once the FRC has entered the capture region. A description of the pulsed power systems that comprise FRCHX will be presented along with an overview of the magnetic and plasma diagnostics fielded on the experiment. Results from recent FRCHX experiments will then be presented, and possible reasons for the lifetime limitations will be discussed along with several approaches for overcoming these limitations. 35. Performance of a Compact, Cascade FCG System

Abstract

A system of two FCG s coupled via flux-trapping is described. The driver FCG, designated SAM, was custom-designed for this application. The output of SAM is a single-turn loop that is tightly coupled to the first winding section of a larger FCG, designated JILL. The single-turn driver loop, coupled to 35 turns of the input winding of JILL, provides a calculated flux gain of 28. For the first experimental test of the SAM/JILL system, the SAM generator was seeded with 1.0 kA (flux 0.29 Wb) and produced a current of 472 kA (flux 0.10 Wb) in the coupling loop at crowbar time of the JILL generator. Based on the calculated mutual inductance of 5.95 H, the JILL generator began operation with a seed flux of 2.81 Wb. With this seed flux, the expected output current for JILL driving a 0.8 H load is 1.8 MA. The measured output current was 884 kA, roughly one half of the expected current. Analysis of the I-dot data from the test shows that this low performance was due to multiple electrical breakdowns in the JILL generator during the interval when the armature-stator contact point was underneath the coupling loop. Subsequent analysis suggests that the electrical breakdowns were the result of flux compression in the coupling loop. Details of the experiment and analysis will be presented.
