Getting to Zero is a commonly cited strategic aim to reduce mortality due to both HIV and avoidable deaths among people with HIV. However, no clear definitions are attached to these aims with regard to what constitutes HIV-related or preventable mortality, and their ambition is limited. This Position Paper presents consensus recommendations to define preventable HIV-related mortality for a pragmatic approach to public health monitoring by use of national HIV surveillance data. These recommendations were informed by a comprehensive literature review and agreed by 42 international experts, including clinicians, public health professionals, researchers, commissioners, and community representatives. By applying the recommendations to 2019 national HIV surveillance data from the UK, we show that 30% of deaths among people with HIV were HIV-related or possibly HIV-related, and at least 63% of these deaths were preventable or potentially preventable. The application of these recommendations by health authorities will ensure consistent monitoring of HIV elimination targets and allow for the identification of inequalities and areas for intervention.

Background The recombinant protein-based vaccine, NVX-CoV2373, demonstrated 89.7% efficacy against coronavirus disease 2019 (COVID-19) in a phase 3, randomized, observer-blinded, placebo-controlled trial in the United Kingdom. The protocol was amended to include a blinded crossover. Data to the end of the placebo-controlled phase are reported. Methods Adults aged 18–84 years received 2 doses of NVX-CoV2373 or placebo (1:1) and were monitored for virologically confirmed mild, moderate, or severe COVID-19 (onset from 7 days after second vaccination). Participants who developed immunoglobulin G (IgG) against nucleocapsid protein but did not show symptomatic COVID-19 were considered asymptomatic. Secondary outcomes included anti-spike (S) IgG responses, wild-type virus neutralization, and T-cell responses. Results Of 15 185 participants, 13 989 remained in the per-protocol efficacy population (6989 NVX-CoV2373, 7000 placebo). At a maximum of 7.5 months (median, 4.5) postvaccination, there were 24 cases of COVID-19 among NVX-CoV2373 recipients and 134 cases among placebo recipients, a vaccine efficacy of 82.7% (95% confidence interval [CI], 73.3%–88.8%). Vaccine efficacy was 100% (95% CI, 17.9%–100.0%) against severe disease and 76.3% (95% CI, 57.4%–86.8%) against asymptomatic disease. High anti-S and neutralization responses to vaccination were evident, together with S-protein–specific induction of interferon-γ secretion in peripheral blood T cells. Incidence of serious adverse events and adverse events of special interest were similar between groups. Conclusions A 2-dose regimen of NVX-CoV2373 conferred a high level of ongoing protection against asymptomatic, symptomatic, and severe COVID-19 through >6 months postvaccination. Background HIV-2 is rare in the UK. Many UK centres therefore only treat small numbers of people and there are few clinical trials to guide treatment. The British HIV Association (BHIVA) 2010 guidelines for management of HIV-2 formed the basis for this national audit, which aims to describe current practice and adherence to guidelines. Methods All UK centres providing HIV care were contacted via the BHIVA “Members Matters” newsletter, and asked to submit anonymised, retrospective data for individuals living with HIV-2 accessing care at their service. Results Thirty-five sites responded and data were analysed for 167 individuals. Nearly half of individuals accessed care at one of four large London centres (77/167, 46%). Most people living with HIV-2 have taken antiretroviral therapy (ART) (132/167, 79%). The most common reasons for initiating treatment were clinical disease (34/89, 38%) and pregnancy (11/89, 12%). Most treatment-naïve individuals were initiated on a protease inhibitor based regimen (70/89, 79%). The use of integrase strand transfer inhibitor based regimens has increased over time. A significant minority of patients did not have baseline drug resistance testing performed, despite having a detectable viral load (15/52, 29%). Virological failure occurred in a minority of individuals (21/132, 16%); the most common drug regimen change in this context was the addition of an integrase strand transfer inhibitor (12/26 regimen changes, 46%). Conclusions Most individuals living with HIV-2 were managed according to national guidance, with key areas for improvement including the choice of ART, drug resistance testing and the management of virological failure. It is hoped that this national audit, performed in conjunction with the updated 2021 BHIVA guidelines will improve the care of individuals living with HIV-2 in the UK.

Microplastics (MPs) pollution is becoming one of the most pressing environmental issues globally. MPs in the marine, freshwater and terrestrial environments have been fairly well investigated. However, knowledge of the atmospheric-mediated deposition of MPs within rural environments is limited. Here, we present the results of bulk (dry and wet) atmospheric MPs deposition in a rural region of Quzhou County in the North China Plain (NCP). Samples of MPs in the atmospheric bulk deposition were collected for individual rainfall events over a 12-month period from August 2020 to August 2021. The number and size of MPs from 35 rainfall samples were measured by fluorescence microscopy, while the chemical composition of MPs was identified using micro-Fourier transform infrared spectroscopy (μ-FTIR). The results showed that the atmospheric MPs deposition rate in summer (892–75,421 particles/m2/day) was highest compared to 735–9428, 280–4244 and 86–1347 particles/m2/day in spring, autumn, and winter, respectively. Furthermore, the MPs deposition rates in our study were 1–2 orders of magnitude higher than those in other regions, indicating a higher rate of MPs deposition in the rural region of the NCP. MPs with a diameter of 3–50 μm accounted for 75.6 %, 78.4 %, 73.4 % and 66.1 % of total MPs deposition in spring, summer, autumn, and winter, respectively, showing that the majority of MPs in the current study were small in size. Rayon fibers accounted for the largest proportion (32 %) of all MPs, followed by polyethylene terephthalate (12 %) and polyethylene (8 %). This study also found that a significant positive correlation between rainfall volume and MPs deposition rate. In addition, HYSPLIT back-trajectory modelling showed that the farthest source of deposition MPs may have come from Russia.

Background The safety and immunogenicity profile of COVID-19 vaccines when administered concomitantly with seasonal influenza vaccines have not yet been reported. We therefore aimed to report the results of a substudy within a phase 3 UK trial, by evaluating the safety, immunogenicity, and efficacy of NVX-CoV2373 when co-administered with licensed seasonal influenza vaccines. Methods We did a planned exploratory substudy as part of the randomised, observer-blinded, placebo-controlled, phase 3 trial of the safety and efficacy of the COVID-19 vaccine (NVX-CoV2373) by co-administrating the influenza vaccine at four study hospitals in the UK. Approximately, the first 400 participants meeting the main study entry criteria—with no contraindications to influenza vaccination—were invited to join the substudy. Participants of the main study were randomly assigned (1:1) to receive two intramuscular injections of either NVX-CoV2373 (5 μg) or placebo (normal saline) 21 days apart; participants enrolled into the substudy were co-vaccinated with a single (0·5 mL) intramuscular, age-appropriate (quadrivalent influenza cell-based vaccine [Flucelvax Quadrivalent; Seqirus UK, Maidenhead] for those aged 18–64 years and adjuvanted trivalent influenza vaccine [Fluad; Seqirus UK, Maidenhead] for those ≥65 years), licensed, influenza vaccine on the opposite deltoid to that of the first study vaccine dose or placebo. The influenza vaccine was administered in an open-label manner and at the same time as the first study injection. Reactogenicity was evaluated via an electronic diary for 7 days after vaccination in addition to monitoring for unsolicited adverse events, medically attended adverse events, and serious adverse events. Immunogenicity was assessed with influenza haemagglutination inhibition and SARS-CoV-2 anti-spike protein IgG assays. Vaccine efficacy against PCR-confirmed, symptomatic COVID-19 was assessed in participants who were seronegative at baseline, received both doses of study vaccine or placebo, had no major protocol deviations affecting the primary endpoint, and had no confirmed cases of symptomatic COVID-19 from the first dose until 6 days after the second dose (per-protocol efficacy population). Immunogenicity was assessed in participants who received scheduled two doses of study vaccine, had a baseline sample and at least one post-vaccination sample, and had no major protocol violations before unmasking (per-protocol immunogenicity population). Reactogenicity was analysed in all participants who received at least one dose of NVX-CoV2373 or placebo and had data collected for reactogenicity events. Safety was analysed in all participants who received at least one dose of NVX-CoV2373 or placebo. Comparisons were made between participants of the substudy and the main study (who were not co-vaccinated for influenza). This study is registered with ClinicalTrials.gov, number NCT04583995. Findings Between Sept 28, 2020, and Nov 28, 2020, a total of 15 187 participants were randomised into the main phase 3 trial, of whom 15 139 received treatment (7569 received dose one of NVX-CoV2373 and 7570 received dose one of placebo). 431 participants were co-vaccinated with a seasonal influenza vaccine in the substudy (217 received NVX-CoV2373 plus the influenza vaccine and 214 received placebo plus the influenza vaccine). In general, the substudy participants were younger, more racially diverse, and had fewer comorbid conditions than those in the main study. Reactogenicity events were more common in the co-administration group than in the NVX-CoV2373 alone group: tenderness (113 [64·9%] of 174 vs 592 [53·3%] of 1111) or pain (69 [39·7%] vs 325 [29·3%]) at injection site, fatigue (48 [27·7%] vs 215 [19·4%]), and muscle pain (49 [28·3%] vs 237 [21·4%]). Incidences of unsolicited adverse events, treatment-related medically attended adverse events, and serious adverse events were low and balanced between the co-administration group and the NVX-CoV2373 alone group. No episodes of anaphylaxis or deaths were reported within the substudy. Co-administration resulted in no change to influenza vaccine immune response although a reduction in antibody responses to the NVX-CoV2373 vaccine was noted. Here we describe and evaluate the success of a multi-institutional Centre for Doctoral Training (CDT), which was established to address a UK skills shortage in Soil Science. The government-funded ‘STARS’ (Soils Training And Research Studentships) CDT was established in 2015 across a range of universities and research institutes in the UK. It recruited 41 PhD students equitably split across the institutions under four core research themes identified as being central to the national need, namely, (1) Understanding the soil–root interface, (2) Soils and the delivery of ecosystem services, (3) Resilience and response of functions in soil systems and (4) Modelling the soil ecosystem at different spatial and temporal scales. In addition, the STARS CDT provided a diverse skills programme, including: Holistic training in soils, the promotion of collegiality and joint working, strategies to promote science and generate impact, internships with end users (e.g., policymakers, industry), personal wellbeing, and ways to generate a lasting soils training legacy. Overall, both supervisors and students have reported a positive experience of the CDT in comparison to the conventional doctoral training programmes, which have less discipline focus and little chance for students to scientifically interact with their cohorts or to undertake joint training activities. The STARS CDT also allowed students to freely access research infrastructure across the partner institutions (e.g., long-term field trials, specialised analytical facilities, high-performance computing), breaking down traditional institutional barriers and thus maximising the students' potential to undertake high-quality research. The success and legacy of the STARS CDT can be evidenced in many ways; however, it is exemplified by the large number and diversity of journal papers produced, the lasting collaborations, final career destinations, and creation of a web-based legacy portal including new and reflective video material. Background: Brachiaria humidicola (Bh) has the ability to produce biological nitrification inhibitors (NIs) and release NIs from the root to the soil. Aims: To compare the effects of growing Bh with Brachiaria ruziziensis (Br, which is not able to produceNIs) on soil nitrogen (N) dynamics,Ngases and carbon dioxide (CO2) emissions and nitrifiers and denitrifiers following sheep urine application, a laboratory incubation was conducted in a He/O2 continuous flow denitrification system (DENIS). This incubation was conducted in the absence of light. Hence themeasured effects of Bh and Br on N cycling were the residual effect of biological NIs released into the soil prior to the incubation and released via root death. Methods: The treatmentswere: (1) Bhwith water application (Bh+W); (2) Bh with sheep urine (Bh + U); (3) Br with water application (Br + W); (4) Br with sheep urine (Br + U). Results: Results showed that soil NO3– concentration increased significantly in the soil with sheep urine application after the incubation. Soil nitrous oxide (N2O) and nitric oxide (NO) emissions increased immediately after the sheep urine application and peaked twice during the incubation. Cumulative emissions for the first peak were significantly lower from the Bh + U treatment (0.054 kg N ha–1) compared with the Br + U treatment (0.111 kg N ha–1), but no significant differences were observed in the total cumulative N2O and NO emissions between the Bh + U and Br + U treatment at the end of the incubation. Sheep urine addition did not affect the AOA, nirS and nosZ gene copies, but significantly increased the AOB gene copies after the incubation. Conclusions: We conclude that the residual effect of Bh to mitigate N2O emissions in a highly nitrifying soil is short-lived.

AimHIV is associated with an increased risk of progression to chronic kidney disease (CKD), and this risk is higher in people of West African descent than many other ethnicities. Our study aimed to assess the rates of progression to CKD and predictors of rapid progression in patients receiving antiretroviral therapy (ART) in central Ghana between 2003 and 2018.MethodsThis single-centre retrospective study enrolled people with HIV (PWH) initiating ART in Ghana between 2003-2018. Demographics, hepatitis B (HBsAg) status, ART regimens and eGFR measurements were recorded, and multi-level model linear regression was performed to determine predictors of greater levels of eGFR decline.Results659 participants were included in the study with a median follow-up time of 6 years (IQR 3.6-8.9). 149 participants (22.6%) also had confirmed HBV co-infection. Tenofovir was associated with the highest mean rate of eGFR decline of all Nucleoside/Nucleotide Reverse Transcriptase Inhibitors (NRTIs), representing a statistically significant annual decline -1.08 mL/min/1.73m2/year (CI: -0.24, -1.92) faster than those taking zidovudine. Regarding other ARTs, both nevirapine (-0.78mL /min/173m2/year; CI: -0.17, -1.39) and protease inhibitors (-1.55mL/mil/173m2/year; CI: - 0.41, -2.68) were associated with slower eGFR declines compared with efavirenz. Negative HbsAg status was associated with greater eGFR decline compared with positive HBsAg status (-1.25mL/mil/173m2; CI 0.29. 2.20).ConclusionIncreased rates of eGFR decline amongst PWH in Ghana were associated with tenofovir, nevirapine, and protease inhibitor use as well as negative HBsAg status. Further higher-quality research is needed to explore long-term predictors of eGFR decline in African populations.

Prior studies suggest that adverse events (AEs) following doses one and two of BNT162b2/Pfizer vaccine are worse in those with a prior history of COVID-19. To establish whether this outcome applies to a third/booster dose, we conducted a survey with 534 healthcare workers (HCW) in Northeast England, who reported AEs following all three doses of BNT162b2/Pfizer vaccine. We also explored AEs associated with concurrent seasonal influenza immunisation. For all doses of BNT162b2/Pfizer vaccine there was a cluster of systemic AEs that were consistently worse in HCWs with a prior history of COVID-19. AEs were no worse in HCWs who received their third/booster dose within 7 days of the influenza jab, rather than further apart. Gender and the presence of ongoing COVID-19 symptoms (OCS) had no effect on AEs associated with COVID-19 or influenza vaccination, though younger HCWs experienced more AEs overall. Our findings have implications for vaccine hesitancy and immunisation protocols.

Plant S deficiency is common, but the role of S-containing amino acids such as cysteine in plant S uptake is unknown. We applied 14C-, 35S-, 13C-, and 15N-labelled cysteine to wheat and oilseed rape rhizospheres and traced the plants’ elemental uptake. Both plants absorbed 0.37–0.81% of intact cysteine after 6 h with no further increase after 24 h. They absorbed 1.6–11.5% 35S and 12.3–7.6% 15N from cysteine after 24 h and utilised SO42− as their main S source (75.5–86.4%). Added and naturally occurring cysteine-S contributed 5.6 and 1.1% of total S uptake by wheat and oilseed rape, respectively. Cysteine and inorganic S derived from cysteine contributed 24.5 and 13.6% of uptake for wheat and oilseed rape, respectively, after 24 h. Oilseed rape absorbed ~10-fold more S from cysteine and SO42− than did wheat. The highest absorption of free cysteine should be in the organic-rich soil patches. Soil microorganisms rapidly decomposed cysteine (t1/2 = 1.37 h), and roots absorbed mineralised inorganic N and S. After 15 min, 11.7–14.3% of the 35S-cysteine was retained in the microbial biomass, while 30.2–36.7% of the SO42− was released, suggesting that rapid microbial S immobilisation occurs after cysteine addition. Plants acquire N and S from cysteine via unidirectional soil-to-root nutrient flow, and cysteine is an important S source for plants.

We investigated the contributions of thymidine analogue and tenofovir disoproxil fumarate (TDF) antiretroviral therapy on renal mitochondrial toxicity in Ghanaian people with HIV (PWH). Similar levels of renal biochemical and mitochondrial dysfunction were seen, and there was no increased risk in PWH who had sequenced from thymidine analogue to TDF. However, mild renal impairment was associated with mitochondrial DNA damage in TDF but not thymidine analogue-treated PWH. These data support the continued use of TDF in resource-limited settings.

Drained, lowland agricultural peatlands are greenhouse gas (GHG) emission hotspots and a large but vulnerable store of irrecoverable carbon. They exhibit soil loss rates of ~2.0 cm yr−1 and are estimated to account for 32% of global cropland emissions while producing only 1.1% of crop kilocalories. Carbon dioxide emissions account for >80% of their terrestrial GHG emissions and are largely controlled by water table depth. Reducing drainage depths is, therefore, essential for responsible peatland management. Peatland restoration can substantially reduce emissions. However, this may conflict with societal needs to maintain productive use, to protect food security and livelihoods. Wetland agriculture strategies will, therefore, be required to adapt agriculture to the wetland character of peatlands, and balance GHG mitigation against productivity, where halting emissions is not immediately possible. Paludiculture may substantially reduce GHG emissions but will not always be viable in the current economic landscape. Reduced drainage intensity systems may deliver partial reductions in the rate of emissions, with smaller modifications to existing systems. These compromise systems may face fewer hurdles to adoption and minimize environmental harm until societal conditions favour strategies that can halt emissions. Wetland agriculture will face agronomic, socio-economic and water management challenges, and careful implementation will be required. Diversity of values and priorities among stakeholders creates the potential for conflict. Successful implementation will require participatory research approaches and co-creation of workable solutions. Policymakers, private sector funders and researchers have key roles to play but adoption risks would fall predominantly on land managers. Development of a robust wetland agriculture paradigm is essential to deliver resilient production systems and wider environmental benefits. The challenge of responsible use presents an opportunity to rethink peatland management and create thriving, innovative and green wetland landscapes for everyone's future benefit, while making a vital contribution to global climate change mitigation.

Nitrous oxide (N

Biological nitrification inhibition is a plant‐mediated rhizosphere process where natural nitrification inhibitors can be produced and released by roots to suppress nitrifier activity in soil. Nitrification is one of the critical soil processes in the nitrogen (N) cycle, but unrestricted and rapid nitrification in agricultural systems can result in major losses of N from the plant–soil system (i.e., by NO3− leaching and gaseous N emissions). In this study, we explored the potential efficacy of biological nitrification inhibitors (linoleic acid [LA] and linolenic acid [LN]) and a proven efficient synthetic (dicyandiamide [DCD]) nitrification inhibitor on N dynamics, nitrous oxide (N2O) and carbon dioxide (CO2) emissions in a highly nitrifying soil. 14C‐labelled LA, LN and DCD mineralization was determined in a parallel experiment to explore the fate of inhibitors after application. We found that LA and LN had no effect on soil NH4+ concentrations, but significantly decreased NO3− concentrations. Soil that received DCD had lower NO3− and higher NH4+ concentrations than the control (soil without nitrification inhibitors). LA and LN increased the cumulative N2O and CO2 emissions when they were applied at high concentrations (635 or 1,270 mg kg−1 dry soil). LA and LN had a much greater mineralization rate than that of DCD: 47–56%, 37–61% and 2.7–5.5%, respectively, after 38 days incubation. We conclude that in contrast to the direct inhibition of nitrification caused by DCD, addition of LA and LN may cause apparent nitrification inhibition by promoting microbial immobilization of soil NH4+ and/or NO3−. Future studies on nitrification inhibitors need to clearly differentiate between the direct and indirect effects that result from addition of these compounds to soil.

The Yangtze River Basin (YRB) has been recognized as one of the key strategic development regions in China. Agriculture p52 roduction systems in the YRB have contributed considerably to China’s goal of food security. Realizing Agriculture Green Development (AGD) means agriculture production systems with high productivity, high resource use efficiency and low environmental costs. However, challenges and barriers still exist for realizing AGD in the YRB. Here, we summarize four main challenges for AGD in the YRB, and identify two approaches (top-down and bottom-up) including main strategies needed to achieve AGD. The four challenges include, but are not limited to, (1) low agricultural productivity and nutrient use efficiencies, (2) an uneven agricultural production structure, (3) rapid urbanization, and (4) uncoordinated targets for environmental protection and food production. We conclude that both top-down and bottom-up approaches are needed to deliver AGD in the YRB. Top-down approaches are mainly operated by government and underpinned by research, which uses spatial planning to promote the balance between agricultural production and the ecological environment, and to optimize the proportions of cereal and cash crop production with monogastric and ruminant animal production. The bottom-up approach needs strategies to close the yield gap of various cropping and livestock systems, improve resource use efficiencies to control environmental impacts. Furthermore, training and education are needed to increase awarenessand improve skills for farmers and advisers. Our review can serve as example for other global regions that are in transition from unsustainable agriculture production towards sustainable withclean environment and healthy economies.

Phthalate acid esters (PAEs) are commonly used plastic additives, not chemically bound to the plastic that migrate into surrounding environments, posing a threat to environmental and human health. Dibutyl phthalate (DBP) and di(2-ethylhexyl) phthalate (DEHP) are two common PAEs found in agricultural soils, where degradation is attributed to microbial decomposition. Yet the impact of the plastic matrix on PAE degradation rates is poorly understood. Using

Agricultural soil emissions are a balance between sinks and sources of greenhouse gases (GHGs). The fluxes of GHGs from soils are complex and spatially and temporally heterogenous. While the soil surface is the exchange site with the atmosphere and is commonly where GHG fluxes are measured, it is important to consider processes occurring throughout the soil profile. To reduce emissions and improve agricultural sustainability we need to better understand the drivers and dynamics (production, consumption, diffusion) of these gases within the soil profile. Due to the heterogeneous nature of GHG processes at small to large scales, it is important to test how these processes differ with depth in different systems. In this study, we measured in situ CO2, N2O and CH4 concentration gradients as a function of soil depth over subsequent maize and wheat growing seasons with active gas samplers inserted into an arable field at 10, 20, 30 and 50 cm depths. We found N2O and CH4 concentrations increased with depth, but only CO2 concentrations differed with depth between growing seasons due likely to differences in soil diffusivity driven by soil conditions. Using the concentration gradient method (GM), the CO2 fluxes at each depth and their contribution to the surface flux were calculated and validated against a chamber method (CM) measured surface flux. We found the GM estimated surface CO2 flux was only 6 % different in the wheat, but 28 % lower than the surface measured flux in the maize growing season, due to drought conditions reducing the accuracy of the GM. Finally, we measured fluxes of CO2, N2O and CH4 in ambient and highly concentrated headspaces in laboratory mesocosms over a 72 h incubation period. We provide evidence of depth dependent CH4 oxidation and N2O consumption and possibly CO2 fixation. In conclusion, our study provides valuable information on the applicability of the GM and further evidence of the GHG production, consumption and diffusion mechanisms that occur deeper in the soil in a temperate arable context.

Extreme weather events have become a dominant feature of the narrative surrounding changes in global climate with large impacts on ecosystem stability, functioning and resilience; however, understanding of their risk of co‐occurrence at the regional scale is lacking. Based on the UK Met Office’s long‐term temperature and rainfall records, we present the first evidence demonstrating significant increases in the magnitude, direction of change and spatial co‐localisation of extreme weather events since 1961. Combining this new understanding with land‐use data sets allowed us to assess the likely consequences on future agricultural production and conservation priority areas. All land‐uses are impacted by the increasing risk of at least one extreme event and conservation areas were identified as the hotspots of risk for the co‐occurrence of multiple event types. Our findings provide a basis to regionally guide land‐use optimisation, land management practices and regulatory actions preserving ecosystem services against multiple climate threats.
