News aggregator databases offer different content and features, but there is little guidance on how these databases compare. This article compares the coverage scope, availability of 35 U.S. newspaper titles, and features of six news aggregators available to U.S. academic libraries: Newspaper Source Plus (EBSCO), Factiva, News (Gale OneFile), Nexis Uni (LexisNexis), Access World News (NewsBank), and U.S. Newsstream (ProQuest). Factiva and U.S. Newsstream have most of the titles examined, but others offer multimedia content and batch downloads. The impact of climate change on the Earth's environments has been widely discussed, although there is still little consensus on the degree of influence, and to what extent the effects are positive, negative or neutral. Predicting the impacts of climate change on organisms and their response to this process has been a growing challenge for ecologists in recent years. In this review, we surveyed the published research on the relationship between lizards and global climate change. We surveyed the keywords "climate change" and "warming", combined with "lizard*" (there is, all words with this prefix), in three reference databases. We identified 401 relevant papers, and analyzed in further detail the group of studies (59 papers, 14.7% of the total) that developed thermoregulatory models to predict the persistence of lizards in a scenario of global warming. These 59 papers focused on species of 13 lizard families found on five continents. Overall, 62.5% of the papers that predicted the impacts of climate change on lizards indicated negative effects, while 21.9% reported positive effects, and 15.6%, a neutral scenario. The lizards identified as the most vulnerable to warming were tropical, viviparous, and thermoconformers, whereas species adapted to cooler climates would be the most likely to benefit from warming. On a broader scale, however, this scenario would lead to competition between lowland and highland lizards, for example, for thermally favorable areas. We conclude that it will be important to develop more detailed models that contemplate the specific features of each group for the development of more reliable predictions, in addition to the need for social conservation projects and the systematic identification of priority areas for conservation. The current study examined school readiness for children placed in care of child protection services before age 5. This association was assessed using a population-based cohort of children born in Manitoba, Canada, between 2000 and 2009 (n = 53,477) and subcohorts of discordant siblings (one sibling taken into care, one sibling not taken into care; n = 809) and discordant cousins (n = 517). In the population analysis, children placed in care were significantly less likely to be ready for school; this difference was not seen in the discordant sibling or cousin analysis. We aimed to determine whether malignancy after pediatric HTx for ACM affects overall post- HTx survival. Patients <18y listed for HTx for ACM in the PHTS database between 1993 and 2014 were compared to those with DCM. A 2:1 matched DCM cohort was also compared. Wait-list and post- HTx survival, along with freedom from common HTx complications, were compared. Eighty subjects were listed due to ACM, whereas 1985 were listed for DCM. Although wait-list survival was higher in the ACM group, post- HTx survival was lower for the ACM cohort. Neither difference persisted in the matched cohort analysis. Primary cause of death in the ACM group was infection, which was higher than the DCM group. Malignancy rates were not different. All ACM malignancies were due to PTLD without primary cancer recurrence or SMN. Long-term graft survival after pediatric HTx for ACM is no different than for matched DCM peers, nor is there an increased risk of any malignancy. However, risk of infection and death from infection after HTx are higher in the ACM group. • Metallic fuel database was reviewed to understand the evolution of the top of the fuel. • EBR-II radiographs were analyzed to determine axial swelling and fluff height. • Correlation with operating parameters were developed for these two phenomena. • BISON simulations are used for the first time to suggest fluff formation mechanisms. During the operation of EBR-II, it was found that a highly porous structure (over 40% area fraction) formed at the top of several fuel columns. Previous work has shown that this structure, designated fluff in this paper, contains a significant fraction of fuel elements (e.g., U and Pu) which could potentially impact neutronics. This work aims in analyzing the formation mechanism of this microstructure so its impact can be incorporated into future metallic fuel modeling codes and algorithms. This paper details a preliminary examination into the formation mechanisms of fluff by performing qualitative and statistical analysis of EBR-II experimental data. The operating conditions that have the greatest impact on fluff formation were determined based on this data set. Also, BISON fuel code simulations were used to help postulate potential fluff formation mechanisms. From this analysis it was found that the largest contributors to fluff formation were fuel burnup and composition, with fluff formation exhibiting a roughly linear positive correlation with increasing burnup and a negative correlation with increasing Pu content. The emergency department (ED) is a vital entry point in the health care system for children who experience maltreatment. This study fills a gap in the maltreatment literature by presenting systematic, national estimates of maltreatment-related ED visits in the United States by children ?3 years old, from 2006 to 2011, using the Nationwide Emergency Department Sample (NEDS). Children who experienced and likely experienced maltreatment were identified via International Classification of Diseases, Ninth Revision, Clinical Modification diagnostic codes. Maltreatment was classified as physical or sexual abuse, neglect, or poly-victimization. The clinical and demographic profiles of children who experienced maltreatment were described. Approximately 10,095 children who experienced maltreatment (0.1% of total ED visits) and 129,807 children who likely experienced maltreatment (1.2% of total ED visits) were documented each year. Maltreatment was associated with significantly greater risk of injury, hospitalization, and death in the ED setting. Physical abuse was the most common explicit maltreatment diagnosis (33 ED visits per 100,000 children ?3 years old) and neglect was the most common likely maltreatment diagnosis (436 ED visits per 100,000 children ?3 years old). Background Waitlist mortality among children listed for primary heart transplant (HTx) has been well characterized, whereas limited data exist for cardiac retransplantation (CRTx) after pediatric primary HTx. We sought to characterize the population listed for CRTx and to determine the factors that affect waitlist mortality. Methods All individuals listed for CRTx >1 year after pediatric primary HTx between October 1, 1987, and October 14, 2012 were identified in the Organ Procurement and Transplantation Network database. Baseline characteristics and waitlist mortality were compared between age groups (< 11 years, 11–18 years, and > 18 years) and during 3 successive eras (1987–1999, 1999–2006, and 2006–2012). Results The cohort comprised 632 patients who were listed for CRTx > 1 year after pediatric primary HTx. Median age was 4 years at primary HTx and 14 years at relisting. Median time from primary HTx to relisting was 7.3 years. Median waiting time was 75.3 days. Overall mortality was 25.2% (159 of 632). The most frequent relisting diagnosis was related to graft vasculopathy (62.5%). The leading causes of death were chronic rejection and vasculopathy (52%). Waitlist mortality significantly decreased after 2006 (31% vs 17%; p < 0.01), despite a relatively constant CRTx rate (67% vs 65%). Univariate analysis showed era, age, listing status, and life support (mechanical circulatory support device, extracorporeal membrane oxygenation, mechanical ventilation) were significant predictors of mortality. Multivariate analyses showed that later era (2006–2012), ages 11 to 18 years, and United Network of Organ Sharing listing status 2 predicted decreased mortality, whereas life support increased mortality. Purpose Total ankle replacement (TAR) and ankle arthrodesis (AA) are two commonly performed procedures for end-stage arthritis of the ankle joint. The aim of this study was to analyze the differences in the rates of complications and reoperations at both 30 days and one year within a matched sample of TAR and AA patients from a large database population. Methods A commercially available patient database record, known as the PearlDiver database (www.pearldiverinc.com, Colorado Springs, CO, USA), was used for this study. Patients undergoing TAR and AA were identified using Current Procedural Terminology (CPT) codes. After matching both TAR and AA groups for confounding variables, such as diabetes, smoking, obesity, and comorbidities scores, the differences in the rates of complications at 30 days and one year and the rate of reoperation at one year were evaluated in both groups. Results After matching for confounding variables, there were 1287 patients in each group. There was no significant difference in the male/female ratio. Within each group, 430 patients were diabetic, 102 patients smoked, and 543 patients were obese. The rate of surgical site infection (SSI) and wound dehiscence were higher at 30 days in the AA group. About 63.45% of complications happened after 30 days. The AA group showed a higher rate of SSI, wound dehiscence, mechanical complications, and pneumonia at one year. The rate of reoperation was also higher in the AA group at one year. Conclusion Ankle arthrodesis is associated with a higher rate of local and systemic complications at 30 days and one year, along with a higher reoperation rate at one year, when compared to total ankle replacement. Background: Several clinical trials have shown an association between the use of aprotinin in cardiothoracic surgery (CTS) patients and an increased risk of adverse renal, cardiovascular, and cerebrovascular events. Other antifibrinolytic agents-aminocaproic acid (AA) and tranexamic acid (TA)-have not shown elevated risks. Using a large administrative data set, we sought to examine these findings.Methods: In our observational database study of CTS patients who were discharged from 20 academic medical centers from October 2002 through September 2005, we assessed the use of antifibrinolytic therapy on select patient outcomes using descriptive and inferential statistics to compare the various groups.Results: For the CTS patients, AA was used in 9,751 (15.5% of patients) and aprotinin was used in 6,855 (10.9% of patients). Only 17 patients from four hospitals received TA; therefore, TA was excluded from further analysis. A quarterly analysis showed a slow decline in the use of AA, with a gradual increase in the use of aprotinin over the study time period. Variation by hospital using each option was considerable (range, 0%-50%). Statistically significant differences in mortality rates (P < 0.001) occurred with AA (2.6%), aprotinin (5.2%), and control patients (n = 46,123), who did not use any antifibrinolytic agents (3.9%). Rates of acute renal failure were 6.2% with AA, 10.9% with aprotinin, and 6.1% in controls; hemodialysis rates were 2.8%, 6.4%, and 2.6%, respectively. Postoperative acute myocardial infarction occurred in only two cases of patients receiving AA, in none of those using aprotinin, and in 63 controls.Conclusion: Although the use of aprotinin has been increasing, compared with AA, the overall use of antifibrinolytic agents in patients undergoing CTS has remained relatively stable over a three-year period, at under 30%. Significant differences in patient outcomes were observed between the two treatment groups. Given the growing body of evidence for the use of antifibrinolytic therapy, hospitals might be best served by examining existing patterns of use and by instituting restrictions of aprotinin for patients facing an increased risk for bleeding during CTS. Background: The effectiveness of human albumin administration in prevention of severe ovarian hyperstimulation syndrome (OHSS) was reviewed using the Cochrane Menstrual Disorders and Subfertility Group literature search strategy.Methods: Only randomized controlled trials (RCT) comparing the effect of human albumin with placebo or no treatment were included in this review. Seven RCT were identified, five of which met our inclusion criteria and enrolled 378 women (193 in the albumin-treated group and 185 in the control group). Trials under consideration were evaluated for methodological quality and appropriateness for inclusion without consideration of their results. The five included trials were single-centre parallel RCT. Relevant data were extracted independently by two reviewers using the standardized data extraction sheet. Validity was assessed in terms of method of randomization, completeness of follow-up, presence or absence of crossover and co-intervention.Results: There was significant reduction in severe OHSS on administration of human albumin [odds ratio (OR): 0.28; 95% confidence interval (95% CI) 0.11-0.73]. Relative risk was 0.35 (95% CI 0.14-0.87) and absolute risk reduction was 5.5. For every 18 women at risk of severe OHSS, albumin infusion will save one more case. There was no evidence of an increase in the pregnancy rate [OR 1.09 (95% CI 0.65-1.83)].Conclusions: This Cochrane review shows a clear benefit from administration of i.v. albumin at the time of oocyte retrieval in prevention of severe OHSS in high-risk cases. Whether the number needed to treat would justify the routine use of albumin infusion in cases at risk of severe OHSS needs to be judged by clinical decision-makers. [ABSTRACT FROM AUTHOR]

Objective: To compare health-care expenditures over a 12-month horizon for chronic pain patients with implanted intrathecal drug delivery systems (IDDS) who eliminated or continued systemic opioids postimplant., Methods: Claims data from commercial and Medicare databases were searched for patients who had an IDDS, used systemic opioids before implant, and had 12 months pre- and 13 months postimplant continuous medical and pharmacy coverage. The number and characteristics of patients who eliminated or continued systemic opioids were determined at four times postimplant: 30 days (allowing a systemic opioid washout period), 120 days, 150 days, and 210 days. Multivariable models evaluated the effect of eliminating opioids on health-care expenditures at each of those times., Results: Three hundred eighty-nine patients met inclusion criteria, and 51% completely eliminated systemic opioids (12% within the 30-day washout and an additional 39% by the end of the one-year horizon). Systemic opioid elimination within 120 to 210 days postimplant was associated with a reduction of $3,388 to $4,465 in inpatient and outpatient expenditures, and $4,689 to $5,571 in inpatient, outpatient, and drug expenditures., Conclusions: Fifty-one percent of patients completely eliminated systemic opioids in the year after IDDS implant.
