Aesthetic procedures are widely performed by dermatologists and plastic surgeons, often with subjective results. In cosmetic dermatology, there is a lack of studies that use accurate instruments to assess result outcomes. Quality of life (QoL) questionnaires can objectively assess results.To assess the impact of nonsurgical facial cosmetic procedures on QoL.This was a longitudinal observational study performed with 81 patients who underwent facial cosmetic procedures at a University Hospital in Rio de Janeiro, Brazil. The Skin Rejuvenation Outcome Evaluation questionnaire, previously adapted and translated to the Portuguese language spoken in Brazil, was selected, and the results were compared with the Dermatology Life Quality Index. The impact on QoL was statistically evaluated after the interventions.The Skin Rejuvenation Outcome Evaluation scores after the procedures were significantly higher than before ( p.001), independent of sociodemographic characteristics. Patients treated with botulinum toxin and dermal fillers showed greater satisfaction than those treated with laser, chemical peels or microneedling.Minimally invasive cosmetic procedures result in improvements in QoL. The use of translated, adapted, and validated questionnaires favors the comparison of results among different populations and reinforces the confidence on the results of established cosmetic treatments.

Background Vitiligo is a muco-cutaneous, autoimmune, localized, or disseminated disease, which manifests through hypochromic or achromic macules, with loss in quality of life. The prevalence of vitiligo in Brazil was determined to be 0.54%. There is no on-label medication for its treatment. To date, no Brazilian consensus on the treatment of vitiligo had been written. Objectives The objective of this group of Brazilian dermatologists with experience in the treatment of this disease was to reach a consensus on the clinical and surgical treatment of vitiligo, based on articles with the best scientific evidence. Methods Seven dermatologists were invited, and each was assigned two treatment modalities to review. Each treatment (topical, systemic, and phototherapy) was reviewed by three experts. Two experts reviewed the surgical treatment. Subsequently, the coordinator compiled the different versions and drafted a text about each type of treatment. The new version was returned to all experts, who expressed their opinions and made suggestions for clarity. The final text was written by the coordinator and sent to all participants to prepare the final consensus. Results/Conclusion The experts defined the following as standard treatments of vitiligo: the use of topical corticosteroids and calcineurin inhibitors for localized and unstable cases; corticosteroid minipulse in progressive generalized vitiligo; narrowband UVB phototherapy for extensive forms of the disease. Surgical modalities should be indicated for segmental and stable generalized vitiligo. Topical and systemic anti-JAK drugs are being tested, with promising results.

Introduction: Brazil has presented alarming and growing data on infection and deaths from SARS-CoV-2. There are more than 4 500 000 confirmed cases and more than 137 000 deaths ranking second in the world ranking of infected people. In addition to the involvement of the respiratory, cardiovascular, renal and neurological systems, this virus also causes non-specific skin lesions in same patients. Little is known about the pathophysiology of cutaneous involvement. At this moment, we cannot predict a difference in prognosis based on skin lesions. Methods: We conducted an observational case study of 25 patients affected by SARS-CoV-2 who had dermatological lesions. We described the clinical and epidemiological characteristics of each case, demonstrating the polymorphism of the skin lesions and also making a correlation with the chronology of presented systemic symptoms. Results: Twenty five cases of COVID-19 with cutaneous manifestations (urticaria, erythematous rash, maculopapular eruption, pruritus, erythema multiforme-like lesions, dyshidrotic eczema) were observed during, after and before systemic symptoms. A few cases with skin involvement were also observed as isolated symptoms of the viral infection. These data demonstrate the clinical polymorphism related to skin involvement of patients infected with SARS-CoV-2, emphasizing the importance of clinical suspicion by dermatologists when handling suspected cases in the current epidemiological scenario. Conclusion: Although the number of cases in the world stage seems to be regressing, infection by SARS-CoV-2 will be part of the dermatologist's daily routine. As long as we do not have a widely available vaccine and the pandemic takes on an endemic profile, we need to be aware of these manifestations, not only for the proper diagnosis, indication of patient isolation, as well as all the necessary biosafety procedures in dermatology clinics.

Introdução: Com frequência, pacientes solicitam avaliação e tratamento para o rejuvenescimento das regiões perioral e periorbitária. Na maioria das vezes, é preciso usar uma combinação de procedimentos para que o resultado atenda às expectativas. Objetivo: Por meio de um estudo prospectivo, unicêntrico e comparativo, comparar o rejuvenescimento das regiões perioral e periorbital, após tratamento com peeling químico médio com fenol 88% com ou sem a aplicação prévia de toxina botulínica. Métodos: O peeling regional de fenol 88% foi utilizado em 16 pacientes; em oito delas houve aplicação de toxina botulínica duas semanas antes. A avaliação foi feita por meio de fotografias padronizadas e escalas de avaliação por médicos e pacientes. Resultados: A avaliação global por médicos e pacientes no 45º dia foi considerada excelente ou boa para todas as pacientes, com pequena vantagem para o tratamento combinado na região periorbital. Conclusão: A combinação desses dois procedimentos de custo relativamente baixo, muitas vezes negligenciado pelos dermatologistas, resultou na melhora significativa das alterações do envelhecimento dessas áreas.

CAPES - Coordenação de Aperfeiçoamento de Pessoal de Nível Superior A pré-eclampsia (PE) é a maior causa de morbimortalidade materno-fetal e seu diagnóstico inclui hipertensão induzida pela gestação e outros critérios clínicos, que pode ser proteinúria. A terapia anti-hipertensiva prolonga a gestação e diminui desfechos adversos. Porém, 40% das gestantes não respondem à essa terapia e são associadas aos desfechos clínicos mais graves. Na PE ocorre placentação anormal, isquemia da placenta e liberação dos fatores tirosina quinase-1 solúvel (sFLT-1) e endoglina solúvel, o que leva a baixa biodisponibilidade de óxido nítrico (NO) e resulta em disfunção endotelial. A visfatina/Nicotinamida fosforibosiltransferase (NAMPT) induz a produção endotelial de NO e é um potencial biomarcador de disfunção endotelial. Nosso objetivo foi examinar as correlações entre os níveis plasmáticos de visfatina/NAMPT, nitrito (marcador da formação endógena de NO), sFLT-1 e ADMA de 205 gestantes com PE (incluindo subgrupos de responsivas e não responsivas à terapia anti-hipertensiva) e 203 do grupo controle, se polimorfismos de NAMPT e VEGF afetam os níveis de nitrito, e caracterizar interações entre polimorfismos de NAMPT, NOS3 e VEGF nestes grupos. Os genótipos foram determinados por ensaios de discriminação alélica Taqman. Os níveis de nitrito foram mensurados por ensaio de quimioluminescência, e os de vistafina/NAMPT, sFLT-1 e ADMA por ELISA, e analisados por testes de correlação de Spearman. As análises de interação gene-gene foram feitas pelo programa Multifactor Dimensionality Reduction. No grupo PE, os níveis de vistafina/NAMPT tiveram correlação negativa com nitrito (r=-0,257, 95% CI=-0,477/-0,007, P=0,038) e positiva com sFLT-1 (r=0,326, 95% CI=-0,084/0,532, P=0,007). No grupo controle, os níveis de vistafina/NAMPT tiveram correlação positiva com nitrito (r=0,317, 95% CI=0,110/0,497, P=0,002) e negativa com sFLT-1 (r =-0,340, 95% CI=-0,592/-0,027, P=0,029). Em gestantes com PE não responsivas à terapia anti-hipertensiva, os níveis de visfatina/NAMPT e nitrito tiveram correlação negativa (r=-0,376, 95% CI=-0,643/-0,026, P=0,031), e os genótipos TC+CC do polimorfismo rs1319501 de NAMPT foram associados aos menores níves de nitrito plasmático. Foram detectadas interações entre genótipos dos polimorfismos rs1319501 de NAMPT e rs2010963 de VEGF na comparação dos grupos controle e PE (P=0,001), e entre genótipos dos polimorfismos VNTR 27pb do NOS3 e rs2010963 do VEGF na comparação de gestantes com PE responsivas e não responsivas à terapia anti-hipertensiva (P=0,013). Nossos achados sugerem um efeito inibitório da visfatina/NAMPT na formação de NO em PE e no grupo de gestantes não responsivas à terapia anti-hipertensiva. Além disso, que interações gene-gene na via da visfatina/NAMPT podem afetar o risco e a responsividade à terapia anti-hipertensiva em PE. Preeclampsia (PE) is a major cause of maternal and perinatal morbidity/mortality, and is characterized by pregnancy-induced hypertension and other clinical criteria, including proteinuria. Antihypertensive therapy prolongs gestation and decreases adverse outcomes. However, 40% of pregnant are nonresponsive to antihypertensive therapy and associated with adverse outcomes. Abnormal placentation and placental ischemia occur in PE, followed by the release of soluble tyrosine kinase-1 (sFLT-1) and soluble endoglin, which leads to low nitric oxide (NO) bioavailability and results in endothelial dysfunction. Visfatin/Nicotinamide phosphoribosyltransferase (NAMPT) favors endothelial NO production, and is a potential biomarker of endothelial dysfunction. In this study, we examined correlations between the plasma levels of visfatin/NAMPT, nitrite (marker of endogenous NO formation), sFLT-1 and ADMA in 205 pregnant with PE (including responsive and nonresponsive to antihypertensive therapy) and 203 of control group, whether NAMPT and VEGF polymorphisms affect plasma nitrite levels, and characterized gene-gene interactions. Genotypes were determined by Taqman allelic discrimination assays. Plasma nitrite levels were measured by ozone-based chemiluminescence assay, and visfatin/NAMPT, sFLT-1 and ADMA by ELISA, which were analyzed by Spearman’s correlation tests. Gene-gene interactions were characterized using Multifactor Dimensionality Reduction. In pregnant with PE, visfatin/NAMPT levels were negatively correlated to nitrite (r=-0.257, 95% CI=-0.477/-0.007, P=0.038), and positively correlated to sFLT-1 (r=0.326, 95% CI=-0.084/0.532, P=0.007). In the control group, visfatin/NAMPT were positively correlated to nitrite levels (r=0.317, 95% CI=0.110/0.497, P=0.002) and negatively correlated to sFLT-1 (r=-0.340, 95% CI=-0.592/-0.027, P=0.029). In the subgroup of PE who are nonresponsive to antihypertensive therapy, visfatin/NAMPT and nitrite levels were negatively correlated (r=-0.376, 95% CI=-0.643/-0.026, P=0.031), and the TC+CC genotypes of NAMPT polymorphisms rs1319501 were associated with low levels of plasma nitrite. We found interactions between genotypes of NAMPT rs1319501 and VEGF rs2010963 polymorphisms when compared control group and PE (P=0.001), and between genotypes of NOS3 VNTR 27pb and VEGF rs2010963 polymorphisms when compared subgroups of PE who were responsive and nonresponsive to antihypertensive therapy (P=0.013). Our findings suggest an inhibitory effect of the visfatin/NAMPT on NO formation in PE and in the subgroup of pregnant with PE who are nonresponsive to antihypertensive therapy. Moreover, our findings suggest that gene-gene interactions in the NAMPT pathway may affect both the susceptibility to PE and the responsiveness to antihypertensive therapy in PE.

A aplicação de toxina botulínica nos pacientes com sequela de paralisia de Bell é uma terapia auxiliar de extrema valia para a redução da sincinesia e de assimetrias faciais. A paralisia de Bell é a causa mais comum de paralisia do neurônio motor facial. Após a fase de paralisia dos músculos da face, o quadro pode evoluir com assimetria facial e sincinesia. No tratamento da sincinesia, as injeções pontuais no músculo orbicular e platisma aliviam os espasmos. A aplicação seletiva na hemiface não acometida objetiva reduzir a assimetria facial e seu impacto social negativo, com melhora da qualidade de vida.

Background: Scientific literature estimates that around 18 to 40 % of asymptomatic third molars are extracted. The aims of the present study were to determine the indications for extraction of asymptomatic lower third molars in a sample of Spanish and Portuguese dentists, and to relate these indications to the clinicians’ training and professional experience. Material and Methods: A survey consisting of 15 cases of asymptomatic lower third molars was emailed to Portuguese and Spanish dentists. The clinicians were asked to assess the level of difficulty of the extractions and to make a reasoned recommendation based on the panoramic radiographs, gender and age of the patients. Results: 381 clinicians filled in the questionnaires. Most of the professionals had over 13 years of clinical experience. The number of Spanish clinicians with postgraduate degrees in Oral Surgery was significantly higher. On average, 42% of respondents recommended extraction of asymptomatic third molars. O tempo de espera para obtenção de órgão para transplante constitui-se num grande problema na área da saúde em todo o mundo. O número de doadores voluntários não cresce na mesma medida da necessidade de órgãos. Diante deste quadro, surgiu um mercado ilícito de órgãos, em que os compradores e intermediadores se dirigirem aos países onde há uma porcentagem grande de população vulnerável, objetivando adquirir órgãos de indivíduos vivos, mediante pagamento. Neste contexto, esta pesquisa buscou avaliar e comparar as percepções de profissionais de saúde e da população em geral com relação à forma de obtenção dos órgãos, em especial a abordagem de mercado, e comparar a opção desinteressada com outra opção, que tem a percepção do envolvimento da necessidade pessoal ou familiar para obter órgãos com o fim de transplante, utilizando a abordagem de mercado. Para atender aos objetivos deste estudo foi elaborado um questionário para coletar a opinião dos participantes. Sua distribuição foi realizada pessoalmente, de forma aleatória, e também foi elaborada uma versão eletrônica que foi divulgada via página no facebook, criada exclusivamente para este fim. A análise das respostas obtidas foram discutidas ao nível de 5% de significância e consideradas significativas quando o valor de p foi < 0,05. Ao todo, 692 pessoas participaram da pesquisa. Foi identificado que na categoria do profissional de saúde há maior tendência do que entre aqueles que não são profissionais da saúde em discordar dos incentivos indiretos relacionados com a redução de impostos e licença remunerada de 30 dias. Estas foram as únicas associações significativas ligadas aos profissionais da saúde no estudo. A maioria dos participantes (80,1%) concordou que a doação de órgãos deve ser um ato desinteressado e estritamente solidário. Por outro lado, 52% acredita que o mercado poderia ser um sistema justo e benéfico para todos, visando ampliar a possibilidade de realização de transplantes. Em uma situação extrema, de carência absoluta de órgãos, 54,9% dos participantes indicaram que pagariam por um órgão para salvar a sua vida ou a vida de algum familiar. Nesta questão, 24,1% discordaram e 20,2% indicaram que não possuem opinião sobre o assunto. Waiting in aqueue for obtaining organ transplant constitutes a major problem in healthcare worldwide. Given this situation, there was an illicit market for organs, in which buyers and intermediaries to address the countries where there is a large percentage of low-income population, aiming to acquire organs from living individuals, through payment. In this context, this study aimed to evaluate and compare the perceptions of health professionals and the general public regarding the method of obtaining the organs, especially the market approach and compare the disinterested option with another option, which has the perception of involvement personal or family need for organs for the purpose of transplantation, using the market approach. Objectives: The purpose of this study was to determine the incidence and clinical symptoms associated with sharp mandibular bone irregularities (SMBI) after lower third molar extraction and to identify possible risk factors for this complication. Study Design: A mixed study design was used. A retrospective cohort study of 1432 lower third molar extractions was done to determine the incidence of SMBI and a retrospective case-control study was done to determine potential demographic and etiologic factors by comparing those patients with postoperative SMBI with controls. Results: Twelve SMBI were found (0.84%). Age was the most important risk factor for this complication. The operated side and the presence of an associated radiolucent image were also significantly related to the development of mandibular bone irregularities. The depth of impaction of the tooth might also be an important factor since erupted or nearly erupted third molars were more frequent in the SMBI group. Conclusions: SMBI are a rare postoperative complication after lower third molar removal. Older patients having left side lower third molars removed are more likely to develop this problem. The treatment should be the removal of the irregularity when the patient is symptomatic. Key words:Third molar, postoperative complication, bone irregularities, age.

Constata-se que existe uma tendência de olhar para a Educação Física num sentido de grande racionalidade técnica - vinda de fora - que conduz a uma atividade física instrumental, funcional, dicotômica, dualista, muito preocupada com a produtividade, eficácia, tendo como bússola orientadora a dimensão utilitarista. Ora, este fato parece emprestar um olhar moral e ético muito rígido, pouco flexível e de caráter quase intocável. No entanto, outros olhares “contrários” (diríamos complementares) vêm defender uma dimensão mais humanista sustentada pelas visões de intencionalidade fenomenológica, hermenêutica, interpretativa, onde é feito o elogio a um outro olhar axiológico., CIEC - Centro de Investigação em Estudos da Criança, UM (UI 317 da FCT).

The evaluation of environmental effects generated by agricultural production on water quality became essential in Brazil after the creation of policies for the use and conservation of water resources. For such, water quality indices have been considered with the purpose of showing the spatial and temporal variation of water quality in a watershed. The objective of this study was to develop a water quality index (WQI) applying the Multivariate Factorial Analysis (MFA) statistical technique, which could indicate the influence of agricultural activities in the quality of water resources. Water in a predominantly farm watershed was monitored from Sept. 2003 to Sept. 2004. Monthly water collections were carried out at six sample points, and eight parameters were analyzed: nitrate, ammoniacal nitrogen, ammonia, total phosphorus, electrical conductivity, pH, suspended solids and turbidity, which were considered important due to the agricultural management adopted in the region. Results indicated a contamination of agricultural origin along the basin. Factorial analysis showed that ammonia, ammoniacal nitrogen and nitrate parameters were the ones that most contributed in determining the WQI. A avaliação dos impactos gerados pela agricultura sobre a qualidade da água tornou-se primordial no Brasil após o estabelecimento das políticas de uso e conservação dos recursos hídricos, razão por que índices de qualidade de água são propostos com o intuito de se mostrar a evolução da qualidade da água no tempo e no espaço. O objetivo primeiro neste estudo, foi desenvolver um índice de qualidade de água (IQA), utilizando-se a técnica estatística da Análise Fatorial Multivariada, avaliando-se a influência das atividades agrícolas na qualidade dos recursos hídricos; realizou-se, para se atingir o proposto, um monitoramento da qualidade da água em uma bacia hidrográfica de uso predominantemente agrícola, no período de set/03 a set/04. Realizaram-se, também, coletas mensais de água em seis pontos, analisando-se oito variáveis (nitrato, nitrogênio amoniacal, amônia, fósforo total, condutividade elétrica, pH, turbidez e sólidos suspensos), julgadas importantes pelo manejo agrícola adotado na região. Os resultados obtidos indicaram uma contaminação de origem agrícola ao longo da bacia, sendo que, pela análise fatorial, amônia, nitrogênio amoniacal e nitrato foram as variáveis que mais contribuíram na determinação do IQA.

Pycnodysostosis is a rare clinical entity, first described in 1962 by Maroteaux and Lamy. It is a genetic disorder, usually diagnosed at an early age. However, the diagnosis is sometimes late, made as a result of bone fracture, given the severe bone fragility resulting from increased bone density. Oral and maxillofacial manifestations of this disease are very clear. The head is usually large, the nose beaked, the mandibular angle obtuse, and both maxilla and mandible hypoplastic. Dental abnormalities and impaction are observed, as well as alterations in eruption and frequent dental crowding. The differential diagnosis is established with osteopetrosis, cleidocranial dysplasia and idiopathic acro-osteolysis. This article reviews the clinical and radiographic characteristics of pycnodysostosis based on three clinical cases of patients with this disease.

Ellis-van Creveld syndrome is a genetic disorder that was first described by Richard Ellis and Simon van Creveld in 1940. The four principal characteristics are chondrodysplasia, polydactyly, ectodermal dysplasia and congenital heart defects. The orofacial manifestations include multiple gingivolabial musculofibrous fraenula, dental anomalies, hypodontia and malocclusion. The disease can be diagnosed at any age, even during pregnancy. The differentiation should be made between Jeune syndrome and other orofaciodigital syndromes.
