Migraine is a high-prevalence and disabling neurological disorder. However, information migraine management in real-world settings could be limited to traditional health information sources. In this paper, we (i) verify that there is substantial migraine-related chatter available on social media (Twitter and Reddit), self-reported by migraine sufferers; (ii) develop a platform-independent text classification system for automatically detecting self-reported migraine-related posts, and (iii) conduct analyses of the self-reported posts to assess the utility of social media for studying this problem. We manually annotated 5750 Twitter posts and 302 Reddit posts. Our system achieved an F1 score of 0.90 on Twitter and 0.93 on Reddit. Analysis of information posted by our 'migraine cohort' revealed the presence of a plethora of relevant information about migraine therapies and patient sentiments associated with them. Treatment of actinic keratoses (AKs) can help lower the risk of eventual skin cancer and address field pre-cancerization. This review compares the different therapeutic options for actinic keratosis. Databases used include Medline, EMBASE, Web of Science and the Cochrane Library from inception to December 2019. Randomized control trials that were related to any approved or recognized treatment for actinic keratosis were included. 1186 studies were found, of which 80 with 6748 patients were included in the analysis. A network meta-analysis was not possible due to interstudy heterogeneity. The greatest degree of improvement was seen with photodynamic therapy (PDT) used adjunctively with other modalities, but this was not significantly different compared to other treatments. PDT, cryotherapy, imiquimod, ingenol mebutate (IMB), 5-fluorouracil (5-FU), trichloroacetic acid (TCA), and ablative fractional laser (AFXL), were all non-inferior to one another in terms of percent clearance of AKs, but the lowest rates of clearance were seen with diclofenac sodium. When results were substratified by body site, 5-FU, combination PDT and combination 5-FU with calcipotriol were the most beneficial for AKs on the head and neck, although they often caused the highest proportion of initial side effects. Absence of randomized control trials for surgical treatments and non-ablative laser limits comparison of these treatments to other modalities. Limitations include the lack of standardized outcome reporting limited the comparability of results across trials. The results of this analysis do not account for individual patient risk or cumulative risk for development of skin cancer. At present, PDT, cryotherapy, imiquimod, IMB, 5-FU, TCA, AFXL, and combination treatments are similarly efficacious in reducing AKs in immunocompetent patients.Registration: N/A.

EEG signals are commonly analyzed in the frequency-domain. Analysis of the power spectrum commonly demonstrates a reproducible fall-off of high frequencies, sometimes referred to as the “1/f slope”. The 1/f slope comes about from a linear fit of the power spectral density in log-log form and takes typical values between (minus) 0–4. This slope has been recently hypothesized to correlate with the ratio of excitatory to inhibitory synaptic weights. However, we demonstrate models of excitatory-inhibitory (E/I) balance only explain small values of 1/f slope (< 2). We seek to construct a model to explain large slopes (> 2), which have been reported under conditions of anesthesia. Using simulations of clustered spike pulses, we find that pulse widths on the order of 1-10 ms result in a transition to steep 1/f slopes between 3–4. This trend also holds when measuring the 1/f slope from a spiking recurrent network model in a synchronous regime. We conclude that steep 1/f slopes result from synchronous spiking in bulk microcircuit activity. This suggests interpretations of steep 1/f slopes should consider spike synchrony in addition to other factors like E/I balance.

Autoignition and detonation development are foundational events in the combustion community and are fundamentally relevant to engine knocking and detonation propulsion. Autoignition-induced reaction front propagation modes have been extensively investigated, addressing the role of thermal and concentration inhomogeneities. In this work, we have further investigated the nonmonotonic response of detonation development to temperature gradients for low-carbon fuels (hydrogen and syngas) and have found additional detonation regimes, which can depict the panorama of reaction front propagation modes. Results show that separate detonation regimes can be observed when hotspot sizes are below some critical thresholds, with the first corresponding to the known “Bradley detonation peninsula” and the second newly identified featuring broader detonation regions. Despite this, distinct combustion characteristics are observed in the demarcation of detonation regimes between hydrogen and syngas fuels. Specifically, the upper branch of the first detonation regimes for hydrogen is sensitive to temperature gradients at various hotspot sizes, while it exhibits similar behaviors in the lower branch of the second one for syngas, which results in narrower detonation regions. Meanwhile, hydrogen possesses a larger critical hotspot size compared to syngas, and the underlying mechanism is ascribed to the chemical reactivity when hotspot autoignition and the difference of energy density between hotspot interior and exterior. Finally, various detonation regimes are summarized in dimensionless detonation diagrams, in which hydrogen and syngas show similar distributions of detonation peninsula. Despite this, those distinctions in the detonation characteristics between hydrogen and syngas can still be manifested quantitatively. The current work can provide useful insights into knocking inhabitation and detonation promotion.

Background Recently risedronate is suggested to be effective for the prevention and treatment of for osteoporosis in total hip arthroplasty. This meta-analysis aimes to evaluate the efficacy of risedronate in reducing femoral periprosthetic bone mineral density loss in patients undergoing primary total hip arthroplasty. Methods A systematic search was performed in Medline (1966-31 October 2017), PubMed (1966-31 October 2017), Embase (1980-31 October 2017), ScienceDirect (1985-31 October 2017) and the Cochrane Library. Only randomized controlled trial (RCT) were included. Fixed/random effect model was used according to the heterogeneity tested by I2 statistic. Meta-analysis was performed using Stata 11.0 software. The outcome measures included periprosthetic bone mineral density, length of stay and adverse effects. Results Four RCTs including 198 patients met the inclusion criteria. The present meta-analysis showed that there were significant differences between treatment groups in terms of periprosthetic bone mineral density in Gruen zones 1 (standard mean difference (SMD) = 0.758, 95% CI 0.469 to 1.047, P = 0.000), 2 (SMD = 0.814, 95% CI 0.523 to 1.106, P = 0.000), 3 (SMD = 0.340, 95% CI 0.059 to 0.622, P = 0.018), 6 (SMD = 2.400, 95% CI 2.029 to 2.771, P = 0.000), and 7 (SMD = 2.400, 95% CI 2.029 to 2.771, P = 0.000). Conclusion Oral risedronate could significantly reduce periprosthetic bone resorption around an uncemented femoral stem (Gruen zones 1, 2, 3, 6, and 7) up to 6 months after THA. In addition, no severe adverse events were identified. Future trials of risedronate treatment after THA should focus on clinically relevant end points such as the risks of fracture and revision arthroplasty.

Polymerization for modern dental resin-based composites (RBCs) not only occurs immediately upon light exposure but also continues for another 24 hours, well beyond after light is terminated. However, many questions still remain about the role of polymerization kinetics in optimizing the physical properties of a new RBC type-the bulk-fill.The aim is to study the post-cure polymerization kinetics of bulk-fill RBCs and to compare their degree of polymeric conversion (DC) and depth-of-cure (DoC) with an incremental-fill, conventional RBC.Five representative bulk-fill RBCs [Surefil SDR+Stress Decreasing Resin Flow Plus (SDRFP), Tetric EvoCeram Bulk Fill (TECB), Filtek 1 Bulk Fill (F1B), Venus Bulk Fill (VB), and Sonicfill (SF3)] and one conventional RBC [Filtek Supreme Ultra (FSU)] were investigated. The upper surface per RBC specimen was exposed to a light curing unit (Paradigm, 3M-ESPE, irradiance=1221 ± 5 mW/cm2) for 20 seconds. The DC per RBC brand were measured at the bottom surface (specimen Ø=4 mm, thickness=3 mm and 5 mm) as a function of post-curing times using a Fourier transform infrared attenuated total reflection spectrometer. Real-time data recording for post-cure DC began immediately upon light exposure and continued at steady intervals, up to15 min, then again after 24 hours. The DoC of all six RBC brands (n=6 / group) were measured according to ISO-4049. Data were analyzed with nonlinear regression and analysis of variances (ANOVA)/Tukey (α=0.05).Mean DC for the six RBCs with 5 mm curing height after 24 hours were: TECB=79.5%, VB=75.7%, SDRFP=69.2%, SF3=65.8%, F1B=51.8%, and FSU=44.0%. Bulk-fill RBCs showed higher DC efficiency than the conventional RBC for both the 3 mm and 5 mm curing heights. Significant differences in DoC were found amongst the six RBC brands: VB=5.1 mm, SDRFP=4.6 mm, F1B=3.8 mm, TECB=3.5 mm, FSU=3.0 mm, and SF3=2.7 mm.DCs were more affected by specimen thickness, through which the curing light was attenuated, than RBC types. Clinician should be aware not all bulk-fill RBCs have a DoC greater than or equal to 4 mm. Also, a bulk-fill RBC that has a high DC after a post-cure time of 24 hours may not have a high DoC, which is typically measured relatively soon after light exposure.

This study utilized a statistical nanoindentation analysis technique (SNT) to measure the amount of organic and inorganic constituents of twenty different brands of dental resin-based composites (RBCs) and tested whether their macro-property such as flexural modulus could be approximated by the proportions of constituents' micromechanical signatures using various rules of mixtures. The probability density function (PDF) of constitutive moduli per RBC brand were measured for three groups, comprised of different indent arrays and inter-indent spacings. SNT was then applied to deconvolute each PDF, from which the effective filler (μF) and matrix (μM) moduli and filler (VF) and matrix (VM) volume fractions per RBC brand were computed. VF and VM values obtained via SNT were strongly correlated with VF and VM obtained via Thermogravimetric Analysis and Archimedes method. The "observed" flexural modulus (EcFS) measured under macro-experiment were well associated with "predicted" effective modulus (EcEff) measured under nano-experiment, thereby establishing that global modulus was strongly affected by the constituents' micromechanics. However, the "predicted" EcEff were proportionally higher than the "observed" EcFS. VF was a confounder to EcFS and EcEff, whereby the influence of VF on both modular ratios (EcFS/μM and EcEff/μM) was best modeled by an exponential regression.

Although COVID-19 has a milder course in pediatric patients than in adults, it can have a severe and fatal course in children with an underlying disease (UD).In this study, we aimed to evaluate the demographic, clinical, laboratory, and radiological characteristics, treatment methods, and prognosis of pediatric patients diagnosed with COVID-19.The files of patients aged 0-18 years diagnosed with COVID-19 were retrospectively evaluated. Clinically and radiologically suspicious cases were accepted as confirmed cases if SARS-CoV-2 PCR positivity was found in nasopharyngeal swab samples. The severity of the disease was defined as asymptomatic, mild, moderate, and severe according to clinical, laboratory, and radiological features.A total of 322 pediatric patients, 51.2% male and 48.8% female, were included in the study. The median age of the patients was 12.08 years (1 month-18 years). Of the 322 patients, 81 (25.1%) were asymptomatic. Disease severity was as follows: 218 were (67.7%) mild, 14 were (4.3%) moderate, and 9 (2.7%) were severe. 35.7% of the patients were hospitalized. Six percent were admitted to the intensive care unit, and three (0.93%) patients died. The mortality rate in patients with the UD was 3.3%.In our study, we determined that the disease had a more severe course in patients with initial procalcitonin, D-dimer, troponin increase, and thrombocytopenia. Although COVID-19 has a mild course in children, this is unfortunately not true for children with an UD.

OBJECTIVE: To investigate anxiety and/or depression status of patients with chronic lumbocrural pain, and to further analyze related risk factors of anxiety and/or depression . METHODS: Retrospective analysis of the medical data of patients who suffered from chronic lumbocrural pain caused by lumbar disc herniation and/or lumbar spinal stenosis and received minimally invasive surgery from March 2018 to April 2018. General data (including age, gender, education levels, past history, sleep order and medical insurance), numeric rating scale(NRS), Japanese Orthopedic Association(JOA) back pain scale and hospital anxiety and depression scale(HADS) were collected for analysis. The basic demographic data and clinic data were analyzed, possible related risk factors associated were analyzed by univariate analysis, and multivariate Logistic regression analysis was further used to find the relative independent risk factors and included all the predictive variables with P values less than 0.05 as covariates. RESULTS: A total of 91 patients met the inclusion criteria and finished this study, the mean HADS score for anxiety was 8.1±4.2, 48(52.7%) respondents were screened positive for anxiety, while the rest 43(47.3%) patients had negative anxiety state, the mean HDDS score for depression was 6.9±4.9, 38(41.8%) respondents were screened positive for depression, and the rest 53(58.2%) patients were not depressed, and 56(61.5%) patients experienced anxiety or depression. Auditory signals are often used by forest species to attract mates, define and defend territories, and locate prey, and thus these signals may be monitored and used to estimate species presence, richness and acoustic complexity of a patch of habitat. We analyzed recordings from a biodiversity hotspot in the rainforests of Batang Ai National Park in Sarawak, Malaysian Borneo. Three recording sites were established in the forest understory and continuous recordings were made for an acoustic snapshot of approximately 40 h. From these recordings, the bioacoustic index (BI) and acoustic complexity index (ACI) were computed. These acoustic indices exhibited clear periodicity with periods on the order of >6 h. The ACI and BI time series also showed oscillations, with peaks separated by 12 h corresponding to the alternation between bird and frog activity during the day and night, respectively. We quantified the relationships between the acoustic index values and anuran and avian richness, and environmental variables (rainfall intensity and time of day) using correlative and information theoretic techniques. ACI and BI were moderately positively and negatively correlated with rainfall intensity, respectively. ACI and BI were also weakly-to-moderately correlated to species richness, but with mixed directions between recording sites. However, the correlations and mutual information values, a model-free estimator of the relationship strength of random variables, were highest for the relationships between ACI and BI with respect to the rate of individual frog calls, rather than species richness alone. We conclude that acoustic indices are most useful for monitoring ecological dynamics on timescales longer than 6 h. We suggest that acoustic indices are best applied to studying changes in acoustic activity at the level of ecological populations rather than for estimating species richness, as they have been commonly applied in the past.

Energy input and friction behaviour are two of the key phenomena related with the welding bond of ultrasonic consolidation (UC) process. In this study, the effects of welding parameters, such as the building height, travel speed, oscillation amplitude and applied force, on the power consumption of the horn and friction coefficient of the welding interface of UC are investigated for aluminium alloys 2024 and 7075. Multiple regression equations are developed to estimate the power consumption and friction coefficient using the welding parameters. A line-contact friction test is performed to examine the friction coefficient of UC under the various applied forces and preheating temperatures. The quantitative relationships among the welding parameters, friction coefficient and power consumption are derived.

The yellow fever virus (YFV) epidemic in Brazil is the largest in decades. The recent discovery of YFV in Brazilian Aedes species mosquitos highlights a need to monitor the risk of reestablishment of urban YFV transmission in the Americas. We use a suite of epidemiological, spatial, and genomic approaches to characterize YFV transmission. We show that the age and sex distribution of human cases is characteristic of sylvatic transmission. Analysis of YFV cases combined with genomes generated locally reveals an early phase of sylvatic YFV transmission and spatial expansion toward previously YFV-free areas, followed by a rise in viral spillover to humans in late 2016. In this work, the microstructural changes occurring during cooling of friction-stir welded aluminum alloy AA1100 were evaluated. To this end, friction-stir welding (FSW) was performed in a wide range of cooling rates of 20–62 K/s and the evolved microstructures were studied by using electron backscatter diffraction. Below 0.6 Tm (Tm being the melting point), the stir zone material was found to experience no significant changes during cooling. At higher FSW temperatures, however, notable changes occurred in the welded material, including grain growth, sharpening of texture, reduction of the fraction of high-angle boundaries and material softening.

Heat input is one of the key parameters governing the quality and service properties of friction stir welds. By using a calorimetric technique, the heat inputs generated during the friction stir welding of the aluminium alloys, 1100 and 5083, were measured over a wide range of welding parameters. An empirical equation to estimate the heat input using the welding parameters was established based on a multiple regression analysis of the results. The effect of the heat input on the final grain size of the stir zone was also investigated by the electron backscatter diffraction method. The quantitative relationships between the input variables, heat inputs, and final grain sizes in the stir zone were derived.

The Bi-2223 Ag/tapes with the composition Bi1.8Pb0.4Sr1.9Ca2.1Cu3.5Oy + Xn (X1: un-doped; X2: 1wt% MgO + 1wt% Ag2O; X3: 1wt% MgO + 1wt% Ag2O + 0.05wt% SnO2; X4: 1wt% MgO + 1wt% Ag2O + 0.05wt% B2O3; X5: 1wt% MgO + 1wt% Ag2O + 0.05wt% Li2O; X6: 1wt% MgO + 1wt% Ag2O + 0.05wt% La2O3) were prepared by sintering at 837°C for 120 h after partial-melting at 850°C for 1 h. The B2O3 mix-doped tape (X4) and the Li2O mix-doped tape (X5) shows lower conversion of Bi-2212 phase to Bi-2223 phase and lower J c value than the un-doped tape (X1). The MgO and Ag2O mix-doped tape (X2) shows the same conversion of Bi-2212 phase to Bi-2223 phase as the un-doped tape (X1), but higher J c value than the un-doped tape (X1). The SnO2 mix-doped tape (X3) and the La2O3 mix-doped tape (X6) show higher conversion of Bi-2212 phase to Bi-2223 phase and higher J c value than the un-doped tape (X1). However, the La2O3 mix-doped tape (X6) shows the highest proportion of Bi-2223 phase and the highest critical current density. It is suggested that the critical current density depends on the flux pinning and the conversion of Bi-2212 phase to Bi-2223 phase. The MgO doping can increase J c due to improving the flux pinning, and the SnO2 mix-doping and La2O3 mix-doping can increase J c due to increase the conversion of Bi-2212 phase to Bi-2223 phase.

To investigate the clinical effectiveness of polytheretherketone (PEEK) cages assisted anterior cervical discetomy and fusion (ACDF) to treat cervical spondylosis with sympathetic symptoms.Retrospective analysis was undertaken for 39 patients who were diagnosed as cervical spondylosis with sympathetic symptoms and underwent ACDF with PEEK cages. Radiographs obtained before surgery, after surgery, and at the final follow-up were assessed for quality of fusion. The following criteria were used for assessing radiographic success of fusion: (1) endplate obliterated with no lucent lines; (2) obliteration of disc space by bony trabeculae; (3) less than 2°of intervertebral motion or 2 mm of motion between the spinous processes at the operated segment on flexion-extension lateral radiographs. The sympathetic symptoms including vertigo, headache, tinnitus, nausea and vomiting, heart throb, hypomnesia and gastroenterological discomfort were scored by 20-point system preoperatively, 2 months postoperatively and at the final follow-up. The recovery rate and clinical satisfaction rate were also evaluated. Surgical complications were also assessed.They were followed up for at least one year. The mean follow-up was 15.6 months. Radiographs of the cervical spine at the last follow-up revealed a solid fusion with no signs of a pseudoarthrosis in 36 cases. In two patients delayed union and bony fusion were achieved at the end of 9 and 11 months. Pseudoarthosis was found in 1 case but the patient had no symptoms. The score of sympathetic symptoms before surgery, 2 months after surgery and at the final follow-up were 8.4±1.0,2.2±0.3,and 2.4±0.3, respectively. There were 22 excellent cases, 15 good cases, 1 fair case and 1 bad case in terms of RR. Good to excellent results were attained in 95% of theses patients. The sympathetic symptoms improved in all the patients and the score was significantly improved after surgery. There was one patient who had cerebral spinal fluid leakage but he recovered one week after surgery. Two patients felt a mild swallowing discomfort, but it disappeared within one month after surgery. Subcutaneous hematoma occurred in one patient due to obstructed drainage. It was cleared two days after surgery.Cervical spondylosis patients with sympathetic symptoms may be managed successfully with ACDF using PEEK cages. Successful clinical results regarding symptom improvement and general satisfaction with the surgical procedure depend not only on obtaining successful decompression and radiographic fusion but also on patient selection.

To describe the application of polymethylmethacrylate (PMMA) augmentation of cement-injectable cannulated pedicle screws for the treatment of degenerative lumbar scoliosis with osteoporosis.Retrospective cohort study was used to compare cement injectable cannulated pedicle screws (CICPs) group with PMMA augmentation and control group with traditional method in the correction surgery for Lenke-silva level III and level IV degenerative scoliosis cases with osteoporosis. Both groups were followed up for 1 year. The clinical results were assessed by visual analog scale (VAS) of pain on lumbar and lower limbers, Oswestry disability index (ODI) score and EuroQol-5 dimensions (EQ-5D) score. The coronal major curve Cobb angel in coronal plane and thoracic kyphosis Cobb angle, lumbar lordosis Cobb angle and sagittal vertical axis (SVA) in sagittal plane were tested in whole long spine X ray. The fusion rates were evaluated by lumbar X ray and dynamic X ray.In this study 34 cases were enrolled, 15 cases in CICPs group and 19 cases in control group. The general characteristics including age, gender, weight, height, BMI and BMD were without statistical difference between the two groups. There were (5.7±2.2)PMMA augmentation screws in CICPs group. The operation time, blood loss and blood transfusion were higher in CICPs group than in control group, but without statistical difference. Lumbar VAS, lower limbers VAS, ODI score and EQ-5D were all better in 1 month postoperation, 6 months postoperation and 1 year postoperation than in preoperation in both groups. lumbar VAS scores of CICPs group in 6 months postoperation (CICPs group 3.1±1.3 vs. control group 4.4±1.4, P0.01) together with lumbar VAS scores (CICPs group 3.3±1.0 vs. control group 5.2±1.4, P0.01), ODI scores (CICPs group 22.7±17.2 vs. control group 31.4±18.5, P0.01) and EQ-5D in 1 year postoperation (CICPs group 2.9±2.0 vs. control group 3.5±2.5, P0.01)were lower than those of control group. The coronal major curve Cobb angels were all lower in 1 month postoperation, 6 months postoperation and 1 year postoperation than in preoperation in both groups; thoracic kyphosis Cobb angle and lumbar lordosis Cobb angle were all higher in 1 month postoperation, 6 months postoperation and 1 year postoperation than in preoperation in both groups. The coronal major curve Cobb angel was lower in CICPs group than that in control group in 1 year postoperation (CICPs group 17.6°±6.9° vs. control group 21.2°±7.2°, P0.01)and thoracic kyphosis Cobb angle was higher in CICPs group than that in control group in 6 months postoperation (CICPs group -33.5°±8.8 ° vs. control group -28.9°±8.3°, P0.01) and 1 year postoperation (CICPs group -33.0°±8.1° vs. control group -26.3°±7.4°, P0.01) together with lumbar lordosis Cobb angle were higher in CICPs group than that in control group in 1 year postoperation (CICPs group 26.4°±8.1° vs. control group 22.1°±7.3°, P0.01).Polymethylmethacrylate augmentation of bone cement-injectable cannulated pedicle screws for the treatment of degenerative lumbar scoliosis with osteoporosis was effective and safe, the short-term clinical result was good.
