Abstract

Metabolic studies using the in vitro non-recirculating blood-perfused isolated heart model require large volumes of blood. The present study was designed to determine whether heterologous pig blood collected from a slaughterhouse can be used as perfusate for isolated pig hearts perfused under aerobic and constant reduced flow conditions. Eight isolated working pig hearts perfused for 90 min at a constant flow of 1.5 ml g-1 min-1 with non-recirculated blood diluted with Krebs-Henseleit bicarbonate buffer at a hematocrit of 23% were compared to eight hearts subjected to the same protocol but perfused only with Krebs-Henseleit bicarbonate buffer solution. Hearts were paced at 100 bpm and subjected to aerobic perfusion at 38ºC. Hearts were weighed before perfusion and at the end of the experiment and the results are reported as percent weight gain (mean ± SD). Abstract

Objectifs Cette etude de cohorte retrospective et monocentrique visait a evaluer les caracteristiques cliniques, l’evolution de la fonction pulmonaire et la survie globale des patients atteints d’une PID-FP autre que FPI en vie reelle. Methodes Tous les patients hospitalises dans un centre de reference des maladies pulmonaires rares entre 2010 et 2017, âges de 18 ans ou plus, presentant une fibrose pulmonaire d’au moins 10 % ont ete preselectionnes. Les patients presentant une PID-FP selon les criteres suivants ont ete inclus : – baisse relative de la capacite vitale forcee (CVF) ≥ 10 % avec ou sans deterioration clinique ; – ou baisse relative de la CVF entre 5 % et 10 % associee a une aggravation des symptomes respiratoires ou a une augmentation de l’etendue de la fibrose ; – ou augmentation de l’etendue de la fibrose avec aggravation des symptomes respiratoires avec ou sans diminution de la CVF. Un appariement probabiliste a ete realise afin de chainer les patients de cette cohorte au Systeme national des donnees de sante (SNDS) afin d’extraire les consommations de soins et de les valoriser. Resultats Au total, 165 patients (dont 57 % de femmes) ont ete inclus avec un âge moyen de 60 ans (± 14 ans). La plupart des patients (65 %) ont ete inclus avec le critere (a), 24 % avec le critere (b) et 9 % avec le critere (c). Les estimations de la survie globale etaient respectivement de 99 %, 82 % et 66 % a 1, 3 et 6 ans. La mortalite etait associee de maniere significative a une CVF Conclusion Cette cohorte est la premiere decrivant l’histoire naturelle de toutes les PID-FP autres que la FPI. Le chainage des donnees de la cohorte a celles du SNDS a permis d’obtenir une grande quantite d’informations visant a mieux caracteriser la prise en charge et l’evolution de ces maladies. L’histoire naturelle des PID-FP suit un schema caracterise par une aggravation des symptomes respiratoires, un declin progressif de la fonction pulmonaire et une mortalite precoce malgre une prise en charge appropriee.

Abstract

Introduction Les pneumopathies interstitielles diffuses (PID) regroupent un ensemble heterogene de maladies caracterisees par un processus inflammatoire, diffus, et parfois fibrosant (PID) [1] . Certains patients atteints de PID fibrosante, peuvent developper un phenotype d’evolution progressive comparable a celui observe dans la fibrose pulmonaire idiopathique ; ces pathologies sont regroupees sous le terme de PID de phenotype ou de forme progressive (PID-FP) [2] . L’objectif est de decrire la frequence, les caracteristiques cliniques et l’evolution des patients PID-FP hors FPI. Methodes L’etude a porte sur tous les patients successifs hospitalises dans un centre de reference des maladies pulmonaires rares entre janvier 2010 et decembre 2017, âges de 18 ans et plus, et presentant une PID fibrosante diagnostiquee par scanner thoracique. Parmi eux, les patients presentant une PID-FP ont ete selectionnes sur les criteres de progression definis par une baisse de la CVF ≥ 10 % avec ou sans deterioration clinique, ou une baisse de la CVF entre 5 et 10 % associee a une aggravation des symptomes respiratoires, ou une baisse de la CVF entre 5 et 10 % associee a une etendue croissante de la fibrose au scanner en moins de 24 mois. Cette etude a ete approuvee par la CNIL le 09/2018 (918305). Resultats Actuellement 149 patients presentant une PID-FP ont ete inclus. La majorite des patients etaient des femmes (57 %) d’âge moyen 59 ± 14 ans. La CVF moyenne etait de 73,3 ± 21 % de la theorique et la DLco de 43,0 ± 17,5 %. Pour les 104 patients dont l’information est disponible, la duree entre le diagnostic de la PID et celui de la progression etait de 2,7 ± 3,8 ans. La repartition etiologique etait : 8 % de pneumopathie d’hypersensibilite, 6 % de pneumopathie interstitielle non specifique idiopathique, 24 % de PID inclassable, 48 % de PID auto-immune, et 14 % d’autres PID. Les criteres de progression etaient une baisse de la CVF ≥ 10 % (65 %), une baisse de la CVF de 5–10 % associee a une aggravation des symptomes (24 %), une baisse de la CVF de 5–10 % associee a une etendue croissante de la fibrose au scanner (2 %) ou une aggravation des symptomes et une etendue croissante de la fibrose au scanner (9 %) en moins de 24 mois. Conclusion Ces resultats preliminaires fournissent des elements sur l’epidemiologie des patients PID-FP pour lesquels un essai therapeutique avec le nintedanib est actuellement en cours.

Abstract

Resume L’imagerie par resonance magnetique (IRM) est une technique de reference en imagerie cardiaque du fait de son rapport signal–bruit eleve, de ses hautes resolutions spatiale et temporelle, et enfin de la sequence de rehaussement tardif. Cependant, les sequences actuellement disponibles sont insuffisantes pour evaluer certains processus pathologiques comme l’œdeme myocardique. Le T2-mapping est une sequence generant une image parametrique sur la base du temps de relaxation transversal T2 de chaque voxel. Le temps de relaxation T2 est allonge en presence d’œdeme. Cet article resume les connaissances actuelles du T2-mapping dans l’evaluation de l’œdeme myocardique par IRM cardiaque.

Methodes Un algorithme base sur la CIM-10, et reposant sur des algorithmes americains, a ete developpe en France afin de selectionner entre le 01/01/2010 et le 31/12/2017 les cas prevalents et incidents de PID-FP non-FPI au sein du Systeme national des donnees de sante (SNDS). Un algorithme sera aussi developpe a partir de la cohorte clinique de patients avec PID-FP non-FPI suivis par le Centre national de reference des maladies pulmonaires rares de Lyon. Resultats Un premier algorithme a ete developpe pour extraire les sejours des patients atteints de PID-FP, base sur la selection de l’ensemble des sejours avec un code de diagnostic de fibrose pulmonaire puis l’exclusion des patients FPI. Les formes progressives seront enfin extraites a partir des donnees de demandes de remboursement pour des traitements specifiques, de consultations chez le pneumologue, d’actes d’imagerie, d’examens de la fonction pulmonaire et/ou d’hospitalisations. La seconde approche consistera a creer un algorithme via une approche par apprentissage automatique grâce au recoupement des donnees du SNDS et de la cohorte clinique. Conclusion Le developpement de ces deux algorithmes par des approches differentes permettra de comparer les resultats obtenus et de choisir l’approche la plus valide pour approfondir les connaissances sur ces pathologies. Cette etude permettra d’estimer le nombre de patients atteints de PID-FP non-FPI, de decrire leur parcours de soins et les couts associes, ainsi que d’explorer la correlation entre la capacite vitale forcee de ces patients et leur morbi-mortalite.

Abstract

Transcatheter aortic valve implantation is a therapeutic option for high-risk patients with severe aortic valve stenosis and with cardiac symptoms. This procedure requires the preliminary evaluation by a "heart team" and presents some contraindications. We report the case of a 58-year-old man with severe bicuspid aortic valve stenosis and cardiogenic shock. In spite of contraindications and because of the failure of balloon aortic valvuloplasty, transcatheter aortic valve implantation was performed in emergency.

Abstract

D∗± production via e+e−→D∗±X has been measured at an average CM energy of 34.4 GeV. The D∗± energy spectrum is hard, with a maximum near χ = 0.6. The size of the D∗ cross section, R D ∗ = σ( e + e − → D ∗ X ) σ μμ = 2.50 ± 0.64 ± 0.88 (assuming R D ∗0 = R D ∗+ ) indicates that a large fraction of charm quark production yields D∗ mesons. The D∗± angular distribution exhibits a forward—backward asymmetry, A = −0.28 ± 0.13. This is consistent with that expected in the standard theory for weak neutral currents and leads to |gAc| = 0.89 ± 0.44 for the axial vector coupling of the charm quark.

Abstract

We present an analysis of electroweak leptonic couplings from high statistics experiments on Bhabha scattering and μ pair production at an energy of 34.5 GeV. The forward-backward charge asymmetry of the μ pairs was measured to be −0.098±0.023±0.005. The data were found to agree well with the standard theory of electroweak interaction giving sin2θW=0.27±0.07. The leptonic weak couplings were determined to begv=0.000±0.170 andgA=−0.481±0.055. The data were also used to investigate a class of composite models for leptons.

Abstract

Three-jet events produced by e+e- annihilation into hadrons at 34.6 GeV c.m. energy were studied by comparing them with 2nd order QCD and two different models of fragmentation. The distribution of low energy particles in the 3-jet plane is found to be better described by the LUND color string model than by the independent jet model. The opposite is true for more energetic particles flowing between the 3 jets. The average transverse momenta in jets can be described with values of σq between 350 and 500 MeV/c for the gluon jet. © 1985 Springer-Verlag.

Abstract

Inclusive photoproduction of D*+- in ep collisions at HERA has been measured with the ZEUS detector for photon-proton centre of mass energies in the range 115 < W < 280 GeV and photon virtuality Q**2 < 4 GeV**2. The cross section sigma(ep --> D*X) integrated over the kinematic region pt(D*) > 3 GeV and -1.5 < eta(D*) < 1.0 is (10.6+-1.7(stat.)+1.6(syst.)-1.3(syst.)) nb. Differential cross sections as functions of pt(D*), eta(D*) and W are given. The data are compared with two next-to-leading order perturbative QCD predictions. For a calculation using a massive charm scheme the predicted cross sections are smaller than the measured ones. A recent calculation using a massless charm scheme is in agreement with the data., 18 pages including 4 figures

Abstract

The inclusive production of π ± mesons in e + e − annihilation has been measured at c.m. energies of 14, 22 and 34 GeV for pion momenta between 0.3 ans 10 GeV/ c . The fraction of pions among the charged hadrons is above 90% at 0.4 GeV/ c and decreases to about 50% at high momenta. The scaled cross sections ( s β ) d σ d x at 14, 22 and 34 GeV as well as the 5.2 GeV data from DASP have a rather similar x dependence. After integration over the x range from 0.2 to 0.6 the cross sections indicate a monotonic decrease with increasing centre-of-mass energy.

Abstract

The full TASSO data have been used to study the inclusive production of strange mesons in e+e- annihilations. Differential and total cross sections have been measured in the centre of mass energy range 14 to 44 GeV for K0, {Mathematical expression} and 34.5 to 44 GeV for K*± (892). We have investigated the strange meson production properties in jets by studying the rapidity and pt2 distributions as well as the evolution of the multiplicities as a function of the event sphericity. We find no evidence that the strange meson yields increase with increasing sphericity faster than the total charged multiplicity. © 1990 Springer-Verlag.

Abstract

The gluon momentum density xg ( x , Q 2 ) of the proton was extracted at Q 2 = 20 GeV 2 for small values of x between 4 × 10 −4 and 10 −2 from the scaling violations of the proton structure function F 2 measured recently by ZEUS in deep inelastic neutral current ep scattering at HERA. The extraction was performed in two ways. Firstly, using a global NLO fit to the ZEUS data on F 2 at low x constrained by measurementsfrom NMC at larger x ; and secondly using published approximate methods for the solution of the GLAP QCD evolution equations. Consistent results are obtained. A substantial increase of the gluon density is found at small x in comparison with the NMC result obtained at larger values of x .

Abstract

We present an analysis of ρ0ρ0 production by two photons in the ρ0ρ0 invariant mass range from 1.2 to 2.0 GeV. From a study of the angular correlations in the process γγ→ρ0ρ0→π- π+π- we exclude a dominant contribution from JP=0- or 2- states. The data indicate sizeable contributions from JP=0+ for four pion masses M4π1.7 GeV. The data are also well described by a model with isotropic production and uncorrelated isotropic decay of the ρ0,s. The cross section stays high below the nominal ρ0ρ0 threshold, i.e. M4π

Abstract

We have measured the processe + e −→e + e −+hadrons, where one of the scattered electrons was detected at large angles, withQ 2 ranging from 7 to 70 (VeV/c)2. The photon structure functionF 2 γ (x, Q2) was determined at an averageQ 2 of 23 (GeV/c)2. The measurements were compared to theoretical predictions of the Quark Parton Model and Quantum Chromodynamics. In both models a hadronic part was added. Within the errors the data are in agreement with the QPM using quark masses of 300 MeV/c2 for the light quarks. The data also agree with a QCD calculation including higher order corrections. A fit yielded a $$\Lambda _{\overline {MS} } $$ value of 140 −65 +190 MeV, where the errors include statistical and systematic uncertainties.

Abstract

Hadronic events from e+e--annihilations with a high energy isolated photon have been investigated at C.M. energies between 14 GeV and 44 GeV. A forward-backward asymmetry Aγ of the positively charged jet with respect to the incident positron direction has been found:Aγγ=-0.32±0.07, providing evidence for photon bremsstrahlung from quarks. The forward-backward asymmetry of all hadronic events at C.M. energies between 30 GeV and 36 GeV has been measured. The value, corrected for the limited polar angle acceptance of the detector, is:A=+0.021±0.005. © 1988 Springer-Verlag.

Abstract

Jet properties ine + e − annihilation at center of mass energies of 14, 22, 35 and 43.7 GeV were studied with the data collected in the TASSO detector at PETRA, using the same evaluation procedures for all the energies. The total hadronic cross section ratio for the center of mass energy interval 39–47 GeV was determined to be ℛ=4.11±0.05 (stat)±0.18(syst.) at $$\langle \sqrt s \rangle = 43 - 7$$ GeV. Corrected distributions of global shape variables are presented as well as the inclusive charged particle distributions for scaled momentum and transverse momentum. The center of mass energy evolution of the average sphericity, thrust, aplanarity and particle momentum is shown.

Abstract

Inclusive charged particle production ine + e − annihilation into hadrons is studied in terms of the particle fractional momentumx p . Thex p distribution for gluon jets is extracted by comparing two data samples measured in the TASSO detector: nearly symmetric three jet events at centre-of-mass energyW∼35 GeV and two jet events atW∼22 GeV, yielding quark and gluon jets of similar energies (∼11.5 GeV). No significant difference is observed between quark and gluon jets. Monte Carlo models based on parton showers describe the trend and energy variation of the data better than a model with second order matrix element in α s .

Abstract

A high statistics experiment was performed on Bhabha scattering at energies between 14 and 34 GeV. Good agreement with QED was observed. The combined data on Bhabha scattering and μ pair production were found to agree with the standard theory of electroweak interaction giving sin2 θ = 0.27-0.07+0.06. Assuming for the Z0 mass a value of 90 GeV the leptonic weak coupling constants were determined to gV2 = -0.04 ± 0.06 and gA2 = 0.35 ± 0.09. A search for scalar leptons sets lower limits on the mass of scalar electrons ofMse > 16.6 GeV and of scalar muons of Msμ > 16.4 GeV. © 1982.

Abstract

Background and Objectives. Asthma is a common childhood condition that has an impact on the affected children, their parents, and the community. Poor management of this condition is evident. This study evaluated a structured process for quality improvement of asthma care in children. Settings and Design. Intervention strategies included the following: ( a) the dissemination of clinical practice guidelines for the care of asthma in pediatrics, ( b) flow sheets in the patient chart, ( c) audit feedback, and ( d) academic detailing. The outcome performance measures were in the areas of history, physical assessment, documentation, clinical outcome, quality of life, prescribing, knowledge, and use of health care services. Methods. A randomized controlled study of 7 centers: 3 in the intervention group and 4 in the control group. Results. The effectiveness of academic detailing was apparent in improved patient education and patients’ knowledge of trigger factors. There was a lack of any improvement in all other performance measures that were comprehensively assessed in this study. Conclusions. Academic detailing and targeted patient education were shown to be effective. Strategies that did not demonstrate effectiveness in this setting were the single session of audit and feedback, distribution of guidelines, and distribution of chart flow sheets. 27. Restoration of foothills rough fescue grassland following pipeline disturbance in southwestern Alberta

Abstract

The effects of pipeline construction and reclamation techniques on the restoration of rough fescue plant communities following pipeline construction in southwestern Alberta, Canada were evaluated after 7-40 years. The pipeline construction right-of-way (ROW) sites varied from no recovery of rough fescue grassland to moderate recovery. The ROW sites had a higher proportion of introduced grasses and forbs, less topsoil, and poorer rangeland health than the adjacent undisturbed grassland. Within the ROW sites, less topsoil was present on those with larger diameter pipe and which had topsoil fully stripped from the ROW during construction. Introduced grasses, Festuca ovina (sheep fescue) and Poa compressa (Canada bluegrass), succeeded in establishment following seeding and persisted for at least 40 years. Poa pratensis (Kentucky bluegrass) dominated many of the ROW sites. Contributing factors to moderate recovery of rough fescue grassland were related to post-growing season pipeline construction, ideally, between August and March, summer or fall seeding, and minimum disturbance trench-only stripping. Reclamation practices appeared more important than time since restoration in the restoration of rough fescue grassland.

Resume Objectifs Rationaliser les prescriptions dans un service de reanimation postchirurgie cardiovasculaire et thoracique en tenant compte de l’apport de la plethysmographie, de la capnographie, de l’echographie transthoracique et des avis des medecins et pharmaciens correspondants, de 2006 a 2008. Type d’etude Evaluation monocentrique des pratiques medicoeconomiques. Methodes Des reunions des medecins, pharmaciens et internes ont ete organisees deux a trois fois par an de fin 2006 a 2008 pour reflechir a ces prescriptions, les ameliorer et en observer les resultats. Les prescriptions systematiques etaient decouragees au profit de prescriptions ciblees. Les internes etaient systematiquement avertis de ce programme en debut de semestre. L’annee 2006 etait prise comme reference. Parallelement, les reunions de morbi-mortalite mensuelles etaient poursuivies afin d’observer d’eventuels effets adverses. Resultats Alors que l’index de complexite relative du service augmentait de 3,3 % et que les deces potentiellement evitables diminuaient de 34 %, les depenses diminuaient d’approximativement 777 000 euros durant la troisieme annee. Les prescriptions de biologie, de radiographies thoraciques au lit, de tomodensitometries et des medicaments cibles diminuaient respectivement de 30, 10, 16 et 35 %. Cependant, une augmentation de la prescription de quatre medicaments cibles durant la derniere annee fut observee. Conclusion Une reflexion multiprofessionnelle peut servir de base pour optimiser les prescriptions d’examens complementaires et de medicaments couteux en reanimation. Cependant, un interessement des prescripteurs pourrait etre necessaire a la perennisation de la demarche.

RésuméL’IRM s’est imposé, au fil des années, comme une technique d’imagerie incontournable en pathologie cardio-vasculaire grâce, notamment, à ses performances dans l’évaluation des fonctions ventriculaires droite et gauche et aux séquences de « viabilité » acquises tardivement après injection de gadolinium. Ses principales indications cliniques actuelles de classe I sont les suivantes:●la viabilité pour les cardiopathies ischémiques et les syndromes coronariens aigus,●l’étiologie et l’évaluation pronostique des cardiopathies non ischémiques incluant les myocardites et les dysplasies arythmogènes du VD,●les péricardites chroniques et les masses cardiaques,●les anévrysmes et dissections aortiques en dehors de l’urgence,●les cardiopathies congénitales : malformations vasculaires et contrôle de chirurgie correctrice ou palliative.L’IRM est un examen complet, non opérateur dépendant, particulièrement intéressant dans la surveillance au long cours des patients car il peut être répété étant donné son absence d’irradiation.AbstractMRI has acquired over the years a role in the evaluation of cardiovascular pathology especially with regards to its ability to assess right and left ventricular function and delayed postcontrast « viability » sequences. Current class I clinical indications include:●viability for patients with ischemic cardiomyopathy and acute coronary syndrome,●etiology and prognostic evaluation of non-ischemic cardiomyopathies including myocarditis and arrhytmogenic right ventricular cardiomyopathy,●chronic pericarditis and cardiac masses,●non-urgent aortic aneurysm and dissection,●congenital cardiopathies: vascular malformations and follow-up after curative or palliative surgery.MRI provides a complete non-operator dependent evaluation, and is particularily useful for follow-up since it may be repeated due to its absence of ionizing radiation.

Abstract

Objective. To evaluate the long-term impact of a structured approach to improving the quality of diabetes care in general practice in the United Arab Emirates. Design. Controlled before–after trial within a health district with three primary health centres (PHCs) in the intervention group and the six remaining serving as controls. Outcomes and adherence to guidelines were measured over the year before the intervention began and for a second 1-year period at the end of the intervention period. Data were collected by chart abstraction. Setting. The study was performed in PHCs in the United Arab Emirates, a newly developed country on the Arabian peninsula. Study participants. Subjects continuously followed in nine PHCs for diabetes care for the period of the study ( N = 738) were included in the study. Intervention. Structured diabetes care, including the development of general practice diabetes clinics, a patient education program, a health care professional education program, and improved recording of clinical information, was provided for the 33-month time period. Results. There was a statistically significant improvement in three of the process of care variables (ordering HbA1c, cholesterol, and documenting foot examinations) whereas the four remaining variables did not improve. There was limited impact on outcome variables. Conclusions. The intervention described in this study demonstrated an improvement in some process of care measures suggesting an impact of this type of delivery model in this environment.

Abstract

Medical students represent a significant community investment and promoting their health preserves this investment. In order to design health promotion programs for medical students in the United Arab Emirates, students' needs were assessed by means of a cross-sectional self-administered questionnaire survey. Major findings of the survey included: 14% of students were underweight while 24% of students were overweight or obese; the majority believed their activity levels were insufficient (77%), their stress levels too high (65%) and their diet unhealthy (50%); 33%were not sufficiently active to meet minimum recommended levels; few students (22%) had seen a doctor in the past year and many were unaware of important personal health parameters. When presented with hypothetical health situations, many made inappropriate choices. This survey identified significant health-promotion opportunities for these students that can be carried out during medical school in order to establish a healthier physician population.

Abstract

A small electromagnetic sampling calorimeter, installed in the ZEUS experiment in 1995, significantly enhanced the acceptance for very low x and low Q 2 inelastic neutral current scattering, e+p → e+X, at HERA. A measurement of the proton structure function F2 and the total virtual photon-proton ( γ ∗ p ) cross-section is presented for 0.11 ≤ Q 2 ≤ 0.65 GeV2 and 2 × 10−6 ≤ x ≤ 6 × 10−5, corresponding to a range in the γ ∗ p c.m. energy of 100 ≤ W ≤ 230 GeV. Comparisons with various models are also presented.

Abstract

Transcatheter aortic valve implantation is a therapeutic option for high-risk patients with severe aortic valve stenosis and with cardiac symptoms. This procedure requires the preliminary evaluation by a "heart team" and presents some contraindications. We report the case of a 58-year-old man with severe bicuspid aortic valve stenosis and cardiogenic shock. In spite of contraindications and because of the failure of balloon aortic valvuloplasty, transcatheter aortic valve implantation was performed in emergency.

Abstract

Deep inelastic charged-current reactions have been studied ine+p ande−p collisions at a center of mass energy of about 300GeV in the kinematic regionQ2>200GeV2 andx>0.006 using the ZEUS detector at HERA. The integrated cross sections forQ2>200GeV2 are found to be \(\sigma _{e^ + p \to \bar \nu X} = 30.3_{ - 4.2 - 2.6}^{ + 5.5 + 1.6} pb\) and \(\sigma _{e^ - p \to \nu X} = 54.7_{ - 9.8 - 3.4}^{ + 15.9 + 2.8} pb\). Differential cross sections have been measured as functions of the variablesx, y andQ2. From the measured differential cross sectionsdσ/dQ2, theW boson mass is determined to be \(M_W = 79_{ - 7 - 4}^{ + 8 + 4} GeV\). Measured jet rates and transverse energy profiles agree with model predictions. A search for charged-current interactions with a large rapidity gap yielded one candidate event, corresponding to a cross section of \(\sigma _{e^ + p \to \bar \nu X} (Q^2 > 200 GeV^2 ; \eta _{\max }< 2.5) = 0.8_{ - 0.7}^{ + 1.8} \pm 0.1 pb\)

Abstract

A functional approach of the rabbit portal ischemia was performed on five New Zealand rabbits using magnetic-resonance (MR) imaging and an MR-specific contrast agent for the liver. The hepatic vascularization and the functionality of the phagocytosis cells were both studied with a single low dose of a unique contrast agent-superparamagnetic iron oxide particles (SPIOs). After a rapid i.v. injection of SPIOs, functional vessels and normally perfused liver parenchyma appeared with positive signal enhancement, whereas the ischemic area remained dark (cold spot). After the intravascular time period, the well-known negative enhancement induced by these particles on normal parenchyma was observed, with the difference of the ischemic liver, and could be related to the uptake of SPIOs by functional Kupffer cells.

Abstract

We report on a measurement of the proton structure function F 2 in the range 3.5×10-5≤x≤4×10-3 and 1.5 GeV2≤Q 2≤15GeV2 at the ep collider HERA operating at a centre-of-mass energy of √s=300GeV. The rise of F 2 with decreasing x observed in the previous HERA measurements persists in this lower x and Q 2 range. The Q 2 evolution of F 2, even at the lowest Q 2 and x measured, is consistent with perturbative QCD. © 1996 Springer-Verlag.

Abstract

Deep inelastic $e~-p$ scattering has been studied in both the charged-current (CC) and neutral-current (NC) reactions at momentum transfers squared, $Q~2$, between 400 GeV$~2$ and the kinematic limit of 87500 GeV$~2$ using the ZEUS detector at the HERA $ep$ collider. The CC and NC total cross sections, the NC to CC cross section ratio, and the differential cross sections, $ d\sigma/dQ~2 $, are presented. For $Q~2 \simeq M_W~2$, where $M_W$ is the mass of the $W$ boson, the CC and NC cross sections have comparable magnitudes, demonstrating the equal strengths of the weak and electromagnetic interactions at high $Q~2$. The $Q~2$ dependence of the CC cross section determines the mass term in the CC propagator to be $M_{W} = 76 \pm 16 \pm 13$GeV.

Abstract

Events with a large rapidity gap and total transverse energy greater than 5 GeV have been observed in quasi-real photoproduction at HERA with the ZEUS detector. The distribution of these events as a function of the $\gamma p$ centre of mass energy is consistent with diffractive scattering. For total transverse energies above 12 GeV, the hadronic final states show predominantly a two-jet structure with each jet having a transverse energy greater than 4 GeV. For the two-jet events, little energy flow is found outside the jets. This observation is consistent with the hard scattering of a quasi-real photon with a colourless object in the proton., Comment: 19 pages, latex, 4 figures appended as uuencoded file

Abstract

The purpose of this article is to present a brief theoretical review of the models characterizing delayed myocardial enhancement applicable to both MR and CT imaging, review the different characteristics of commercially available gadolinium-based and iodinated contrast materials, and summarize the literature on the potential value of dedicated MR imaging contrast currently in development for the diagnosis of myocardial viability. The intensity of myocardial enhancement following infarction is related to two factors: expansion of the interstitial volume (15+/-2% in normal myocardium and 80+/-3% within necrosis) secondary to cell necrosis and perfusion abnormalities due to the absence of revascularization or lesions to the microcirculation. A kinetic model of contrast material properties within myocardium could be constructed from Kety's equation with regards to enhancement within the different myocardial tissues (viable myocardium, necrotic myocardium, fibrosis, no-reflow zones, stunned or hibernating myocardium). This model can be applied to both CT and MR since clinically available contrast agents are extracellular, inert and kinetically comparable. The development of dedicated contrast agents for viability and necrosis or molecular contrast agents open new horizons for preclinical research.

Abstract

Background The cost effective provision of quality care for chronic diseases is a major challenge for health care systems. We describe a project to improve the care of patients with the highly prevalent disorders of diabetes and hypertension, conducted in one of the major cities of the United Arab Emirates. Settings and Methods The project, using the principles of quality assurance cycles, was conducted in 4 stages. The assessment stage consisted of a community survey and an audit of the health care system, with particular emphasis on chronic disease care. The information gleaned from this stage provided feedback to the staff of participating health centers. In the second stage, deficiencies in health care were identified and interventions were developed for improvements, including topics for continuing professional development. In the third stage, these strategies were piloted in a single health centre for one year and the outcomes evaluated. In the still ongoing fourth stage, the project was rolled out to all the health centers in the area, with continuing evaluation. The intervention consisted of changes to establish a structured care model based on the predicted needs of this group of patients utilizing dedicated chronic disease clinics inside the existing primary health care system. These clinics incorporated decision-making tools, including evidence-based guidelines, patient education and ongoing professional education. Results The intervention was successfully implemented in all the health centers. The health care quality indicators that showed the greatest improvement were the documentation of patient history (e.g. smoking status and physical activity); improvement in recording physical signs (e.g. body mass index (BMI)); and an improvement in the requesting of appropriate investigations, such as HbA1c and microalbuminurea. There was also improvement in those parameters reflecting outcomes of care, which included HbA1c, blood pressure and lipid profiles. Indicators related to lifestyle changes, such as smoking cessation and BMI, failed to improve. Conclusion Chronic disease care is a joint commitment by health care providers and patients. This combined approach proved successful in most areas of the project, but the area of patient self management requires further improvement.

Abstract

To evaluate whether intensivists would accept to optimize their orderings of biological samplings, x-rays and target drugs and to assess the consequence on patient's outcome.Monocentric evaluation of medical economic procedure.Meetings of consultants, registrars and residents started on Dec 21, 2006 with two to three sessions a year in order to evaluate the process of medical ordering. The physicians and pharmacists gave the results of orderings at each meeting. Orderings of systematic samplings, bedside x-rays and unjustified expansive drugs were discouraged, but target samplings and lung ultrasonography were encouraged. New residents were systematically taught about this programme. Meanwhile, monthly morbidity-mortality meetings were pursued in order to assess the consequences of this politics.While ICU total production increased by 3.4% and potentially evitable deaths decreased by 34%, annual expenses decreased by approximatively € 777,000 from 2006 to 2008. This was due to decreased orderings in biology by 30%, bedside x-rays by 10%, computed tomographic scans by 16% and target drugs by 35%. However, an increased ordering in four target drugs was observed in 2008 as compared with 2007.Multidisciplinary optimization of medical ordering can be efficient in ICU. However, a profit-sharing with ordering physicians would be necessary to prolong these effects.

Abstract

MRI has acquired over the years a role in the evaluation of cardiovascular pathology especially with regards to its ability to assess right and left ventricular function and delayed postcontrast "viability" sequences. Current class I clinical indications include: viability for patients with ischemic cardiomyopathy and acute coronary syndrome, etiology and prognostic evaluation of non-ischemic cardiomyopathies including myocarditis and arrhytmogenic right ventricular cardiomyopathy, chronic pericarditis and cardiac masses, non-urgent aortic aneurysm and dissection, congenital cardiopathies: vascular malformations and follow-up after curative or palliative surgery. MRI provides a complete non operator dependent evaluation, and is particularly useful for follow-up since it may be repeated due to its absence of ionizing radiation

Abstract

In the United Arab Emirates (UAE) cardiovascular mortality is a leading cause of death. Yet, there is a dearth of data on its risk factors.A cross-sectional community based study on established cardiovascular risk factors carried out between February 2004 - February 2005 in Al-Ain City, UAE. Subjects were interviewed, blood pressure, weight, height, fasting blood sugar, and lipid profile measured. Framingham risk scores were used for risk assessment.Eight hundred seventeen subjects (403 males and 414 females) completed the survey, of these 28.4% had a Framingham risk assessment score of more than 20%, 23.3% had diabetes mellitus, 20.8% hypertension, 37.3% obesity, 22.7% metabolic syndrome, and 19.6% of male smoked. Coronary heart disease was reported in 2.4%. Lipid profiles were abnormal in 64% of the males, and in 53.9% of the females, mostly due to low high-density lipoproteins or high triglycerides levels.The overall cardiovascular risk assessment of the population screened calls for targeted interventions.

Abstract

The forward-backward asymmetry in the reaction e+e-→bb has been determined at centre of mass energies near 35 GeV using the TASSO detector. We report results from three different methods. The combined result of the measurements is\(\overline A _b = - 0.21 \pm 0.08\) ignoring\(B^0 \bar B^0 \) mixing. Taking mixing into account leads to a corrected asymmetry of\(\overline {A_b^{cor} } = - 0.28 \pm 0.11\). The axial vector coupling constant of theb quark as calculated from the corrected asymmetry isab=−1.2±0.5 to be compared with the valueabSM=−1 from the standard model.

Abstract

The full TASSO data have been used to study the orientation of three-jet events ine+e− annihilation. The polar angle distributions of the normal to the three-jet plane as well as the polar angle distribution of the most energetic jet have been measured as a function of the thrust cut-off used to select the three-jet sample. The data corrected for radiation and detector effects are compared to QCD predictions and fair agreement is found. As a consistency check we also present measurements of the azimuthal correlations between the lepton and hadron planes. A significant azimuthal dependence is found, consistent again with the QCD predictions.
