8. Attitudes of anesthetists towards an anesthesia-led nurse practitioner model for low-risk colonoscopy procedures: a cross-sectional survey

Abstract

BACKGROUND: The mounting pressure on the Australian healthcare system is driving a continual exploration of areas to improve patient care and access and to maximize utilization of our workforce. We hypothesized that there would be support by anesthetists employed at our hospital for the design, development, and potential implementation of an anesthesia-led nurse practitioner (NP) model for low-risk colonoscopy patients. METHODS: We conducted a cross-sectional, mixed methods study to ascertain the attitudes and acceptability of anesthetists towards a proposed anesthesia-led NP model for low-risk colonoscopy patients. An online survey using commercial software and theoretical questions pertaining to participants' attitudes towards an anesthesia-led NP model was e-mailed to consultant anesthetists. Participants were also invited to participate in a voluntary 20-min face-to-face interview. RESULTS: A total of 60 survey responses were received from a pool of 100 anesthetists (response rate = 60%, accounting for 8.04% margin of error). Despite the theoretical benefits of improved patient access to colonoscopy services, most anesthetists were not willing to participate in the supervision and training of NPs. The predominant themes underlying their lack of support for the program were a perception that patient safety would be compromised compared to the current model of anesthesia-led care, the model does not meet the Australian and New Zealand College of Anesthetists guidelines for procedural sedation and analgesia, and the program may be a public liability prone to litigation in the event of an adverse outcome. Concerns about consumer acceptance and cost-effectiveness were also raised. Finally, participants thought the model should be pilot tested to better understand consumer attitudes, logistical feasibility, patient and proceduralist attitudes, clinical governance, and, importantly, patient safety. 9. Randomised controlled trial to investigate the relationship between mild hypercapnia and cerebral oxygen saturation in patients undergoing major surgery

Abstract

OBJECTIVES: The effects of hypercapnia on regional cerebral oxygen saturation (rSO2) during surgery are unclear. We conducted a randomised controlled trial to investigate the relationship between mild hypercapnia and rSO2. We hypothesised that, compared with targeted normocapnia (TN), targeted mild hypercapnia (TMH) during major surgery would increase rSO2. DESIGN: A prospective, randomised, controlled trial in adult participants undergoing elective major surgery. SETTING: A single tertiary centre in Heidelberg, Victoria, Australia. PARTICIPANTS: 40 participants were randomised to either a TMH or TN group (20 to each). INTERVENTIONS: TMH (partial pressure of carbon dioxide in arterial blood, PaCO2, 45-55 mm Hg) or TN (PaCO2 35-40 mm Hg) was delivered via controlled ventilation throughout surgery. PRIMARY AND SECONDARY OUTCOME MEASURES: The primary endpoint was the absolute difference between the two groups in percentage change in rSO2 from baseline to completion of surgery. Secondary endpoints included intraoperative pH, bicarbonate concentration, base excess, serum potassium concentration, incidence of postoperative delirium and length of stay (LOS) in hospital. RESULTS: The absolute difference between the two groups in percentage change in rSO2 from the baseline to the completion of surgery was 19.0% higher in both hemispheres with TMH (p<0.001). On both sides, the percentage change in rSO2 was greater in the TMH group than the TN group throughout the duration of surgery. The difference between the groups became more noticeable over time. Furthermore, postoperative delirium was higher in the TN group (risk difference 0.3, 95% CI 0.1 to 0.5, p=0.02). LOS was similar between groups (5 days vs 5 days; p=0.99). CONCLUSION: TMH was associated with a stable increase in rSO2 from the baseline, while TN was associated with a decrease in rSO2 in both hemispheres in patients undergoing major surgery. 12. My true face: unmasking one’s own face representation

Abstract

Face recognition has been the focus of multiple studies, but little is still known on how we represent the structure of one’s own face. Most of the studies have focused on the topic of visual and haptic face recognition, but the metric representation of different features of one’s own face is relatively unknown. We investigated the metric representation of the face in young adults by developing a proprioceptive pointing task to locate face landmarks in the first-person perspective. Our data revealed a large overestimation of width for all face features which resembles, in part, the size in somatosensory cortical representation. In contrast, face length was compartmentalised in two different regions: upper (underestimated) and bottom (overestimated); indicating size differences possibly due to functionality. We also identified shifts of the location judgments, with all face areas perceived closer to the body than they really were, due to a potential influence of the self-frame of reference. More importantly, the representation of the face appeared asymmetrical, with an overrepresentation of right side of the face, due to the influence of lateralization biases for strong right-handers. We suggest that these effects may be due to functionality influences and experience that affect the construction of face structural representation, going beyond the parallel of the somatosensory homunculus.

21. Doorway-provoked freezing of gait and its treatment in Parkinson’s disease

Abstract

Freezing of gait in Parkinson's disease can be difficult to study in the laboratory. Here we investigate the use of a variable-width doorway to provoke freeze behavior together with new objective methods to measure it. With this approach we compare the effects of anti-parkinsonian treatments (medications and deep-brain stimulation of the subthalamic nucleus) on freezing and other gait impairments. Ten “freezers” and 10 control participants were studied. Whole-body kinematics were measured while participants walked at preferred speed in each of 4 doorway conditions (no door present, door width at 100%, 125%, and 150% of shoulder width) and in 4 treatment states (offmeds/offstim, offmeds/onstim, onmeds/offstim, onmeds/onstim). With no doorway, the Parkinson's group showed characteristic gait disturbances including slow speed, short steps, and variable step timing. Treatments improved these disturbances. The Parkinson's group slowed further at doorways by an amount inversely proportional to door width, suggesting a visuomotor dysfunction. This was not improved by either treatment alone. Finally, freeze-like events were successfully provoked near the doorway and their prevalence significantly increased in narrower doorways. These were defined clinically and by 2 objective criteria that correlated well with clinical ratings. The risk of freeze-like events was reduced by medication but not by deep-brain stimulation. Freeze behavior can be provoked in a replicable experimental setting using the variable-width doorway paradigm, and measured objectively using 2 definitions introduced here. The differential effects of medication and deep-brain stimulation on the gait disturbances highlight the complexity of Parkinsonian gait disorders and their management.

Abstract

To attract attention to that group of metabolic disturbances that manifests itself by the development of edema without the cardiovascular mechanism necessarily being involved, we are using the term nephrosis. These disturbances are commonly believed to be of renal origin or to be associated with kidney dysfunction of some kind. The fact is well established that cardiovascular disturbances and kidney disease may lead to generalized edema, and the explanation for the occurrence of edema under these circumstances seems quite clear. It is not clear how generalized edema may occur in the absence of primary disease in these systems. It is conceivable that some underlying fault of metabolism may bring about characteristic changes in the kidneys which are not necessarily confined to these organs but which may, on careful search, be shown to involve other structures of the body in a similar way.We present a study of a case of

Abstract

We present here a study of the lipoid content of the various tissues of the body in a case of Schüller-Christian's disease together with a review and discussion of the literature to date. Because of the severe anemia encountered in this case we include a parallel study on the tissue of a child of about the same age with similar anemia due to another cause. It was our idea that the latter might in a way act as a control. It has been our hope that should a case of accident come to autopsy we could present our own normal figures. This opportunity has not presented itself. METHODS The material was obtained from the autopsy table as soon as possible. All extraneous blood and tissue were carefully removed.
