Aims: Verruciform acanthotic vulvar intra‐epithelial neoplasia (vaVIN) is an HPV‐independent, p53 wild‐type lesion with distinct morphology and documented risk of recurrence and cancer progression. vaVIN is rare, and prospective distinction from non‐neoplastic hyperplastic lesions can be difficult. CK17, SOX2 and GATA3 immunohistochemistry has emerging value in the diagnosis of HPV‐independent lesions, particularly differentiated VIN. We aimed to test the combined value of these markers in the diagnosis of vaVIN versus its non‐neoplastic differentials in the vulva. Methods and results: CK17, SOX2 and GATA3 immunohistochemistry was evaluated on 16 vaVINs and 34 mimickers (verruciform xanthoma, lichen simplex chronicus, lichen sclerosus, psoriasis, pseudo‐epitheliomatous hyperplasia). CK17 was scored as 3+ = full‐thickness, 2+ = partial‐thickness, 1+ = patchy, 0 = absent; SOX2 as 3+ = strong staining ≥ 10% cells, 2+ = moderate, 1 + =weak, 0 = staining in < 10% cells; and GATA3 as pattern 0 = loss in < 25% basal cells, 1 = loss in 25–75% basal cells, 2 = loss in > 75% basal cells. For analysis, results were recorded as positive (CK17 = 3+, SOX2 = 3+, GATA3 = patterns 1/2) or negative (CK17 = 2+/1+/0, SOX2 = 2+/1+/0, GATA3 = pattern 0). CK17, SOX2 and GATA3 positivity was documented in 81, 75 and 58% vaVINs, respectively, versus 32, 17 and 22% of non‐neoplastic mimickers, respectively; ≥ 2 marker positivity conferred 83 sensitivity, 88 specificity and 86% accuracy in vaVIN diagnosis. Compared to vaVIN, SOX2 and GATA3 were differentially expressed in lichen sclerosus, lichen simplex chronicus and pseudo‐epitheliomatous hyperplasia, whereas CK17 was differentially expressed in verruciform xanthoma and adjacent normal mucosa. Conclusions: CK17, SOX2 and GATA3 can be useful in the diagnosis of vaVIN and its distinction from hyperplastic non‐neoplastic vulvar lesions. Although CK17 has higher sensitivity, SOX2 and GATA3 are more specific, and the combination of all markers shows optimal diagnostic accuracy. [ABSTRACT FROM AUTHOR]

Aims: Extramammary Paget disease (EMPD) is an epithelial neoplasm that can occur at many sites, including the vulva and scrotum. EMPD is characterised by the presence of neoplastic cells, in single cells and clusters, that infiltrate all layers of non‐neoplastic squamous epithelium. The differential diagnosis for EMPD includes melanoma in situ and secondary involvement of tumours from other sites, such as urothelial or cervical; pagetoid spread of tumor cells can also been seen at other sites, such as anorectal mucosa. The most frequently utilised biomarkers for confirming the diagnosis of EMPD include CK7 and GATA3; however, these biomarkers lack specificity. The purpose of this study was to evaluate TRPS1, a newly described breast biomarker, in pagetoid neoplasms of the vulva, scrotum and anorectum. Methods and results: Fifteen cases of primary EMPD of the vulva (two with associated invasive carcinoma) and four primary EMPD of the scrotum showed strong nuclear immunoreactivity for TRPS1. In contrast, five cases of vulvar melanoma in situ, one case of urothelial carcinoma with secondary pagetoid spread into the vulva and two anorectal adenocarcinomas with pagetoid spread into anal skin (one with associated invasive carcinoma) were negative for TRPS1. Additionally, weak nuclear TRPS1 staining was observed in non‐neoplastic tissues (e.g. keratinocytes), but always with less intensity when compared to tumour cells. Conclusions: These results demonstrate that TRPS1 is a sensitive and specific biomarker for EMPD, and may be especially useful for excluding secondary involvement of the vulva by urothelial and anorectal carcinomas. Piracy of subscription-based scholarly literature has reached a new peak with the advent of Sci-Hub and other sites like it. Sci-Hub is a collection of over 68 million items led by and compiled by a researcher from Kazakhstan. This interview was conducted to gain insights from an academic library director, Michael Levine-Clark, who with other colleagues is doing research on how scholars discover and access research materials. Peter Katz, who works for Elsevier, discusses what Sci-Hub means for him as someone responsible for identifying and blocking activity that sets off alerts in their usage system indicating that the activity may be being undertaken by unauthorized users. These two individuals provide thoughtful observations about what Sci-Hub may mean for the future of article discovery in academic libraries and how Open Access models will impact and influence the dynamic between pirated materials and those found behind a paywall. In 2014, University of California, Davis University Library and the California Digital Library collaborated on an Andrew W. Mellon Foundation grant-funded project to explore costs associated with moving scholarly journal subscriptions in the U.S. market entirely to an Article Processing Charge business model, known also as “Gold Open Access.” We contacted MacKenzie Smith, one of the principal investigators, in order to get her reflections on the process of gathering the data, and to discuss some implications of the findings. Mobile technology is in a great state of flux and competition, and the bar keeps getting set higher. What models of service are leading the pack? This session explored the most recent trends to give attendees a sense of the marketplace and what might work best in their own contexts. This article describes an academic library's experience developing and sustaining a literary festival as a collaborative effort. The Eastern North Carolina Literary Homecoming (ENCLH) is a year-long program of events that celebrates the culture and literature of North Carolina. With activities in 6 counties located in the mid-coastal region of North Carolina, the program provides a rich opportunity for people of this area to learn about and meet North Carolina artists. In the past the program was restricted to artists with connections to Eastern North Carolina, but the program is expanding its coverage in 2011. The program theme for 2011 will focus on the impact of environmental literature on social change. This event has been a successful collaboration between a number of cultural institutions, with Joyner Library at East Carolina University serving as the lead. Federal, state and private grant funding has been secured for several years. Key players in the mix include the editor and staff of the North Carolina Literary Review, along with staff from the local public library and members of the ECU faculty as well as librarians from other regional schools. A research project was undertaken by a university library system to develop a sustainable user experience strategy. A task force was convened to conduct a pilot project with members from both libraries in the system to assess staffing responsibilities, guidelines, processes, and data-gathering procedures. In describing the methods employed by the task force in their research, this article outlines the benefits and drawbacks of staffing configurations to initiate user experience practices in an academic library setting. Writers, artists, activists, and others are finding creative ways to engage with, and disrupt and unsettle, commentaries on the climate emergency. In this article, we argue that the do-it-yourself ethic and aesthetic of zines (small-circulation, self-published magazines) and zine-making offers a creative and empowering approach to environmental storytelling, and that zines do different kinds of "work" around positioning, narrating, and responding to ecological problems. Through the idea of zine ecologies, we examine the entanglements between zines and zineing, environmentalism, environmental politics, literature, art, activism and protest, and more. The idea of zine ecologies has a dual existence in this article. We use this idea both as the provocation for a minizine that accompanies this article, but also to scaffold discussion of the quiet politics and activisms of student zine projects responding to, for example, an environmental writer, a piece of activist writing, or an environmental issue or scenario. The Core Competencies for Electronic Resources Librarians (CCERL) defined the role of an electronic resources librarian (ERL) and can serve as a framework for additional applications. The CCERL helps ERLs reorganize departments, create updated job descriptions, and implement Library Services Platforms. Librarians have also used the CCERL to extend skills to digital projects, weeding projects, scholarly communications, and other areas. This article seeks to further connect theory to practical applications by illustrating how the CCERL impacted three librarians at the beginning, middle, and later stages of their ERL careers. An experienced librarian will provide insight on how the development of the CCERL shaped the field of electronic resources librarianship. A recently promoted librarian will demonstrate how the competencies were used to organize a promotion dossier. In early 2008, students approached the University of California at Los Angeles (UCLA) Library with their concerns about the high prices of printed course readers, which are filled with content the library owns or leases. Through investigations of electronic resource licenses, coursepack pilot projects, and campus partnerships, the library worked to reduce the cost of course readers, at times significantly. The exercise highlighted important issues and considerations regarding the licensing of electronic materials, and stressed the importance of communicating specific license terms to users via an electronic resource management system. Objectives: Comparative assessments of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) molecular assays that have been operationalized through the US Food and Drug Administration's Emergency Use Authorization process are warranted to assess real-world performance. Characteristics such as sensitivity, specificity, and false-negative rate are important to inform clinical use.Methods: We compared five SARS-CoV-2 assays using nasopharyngeal and nasal swab specimens submitted in transport media; we enriched this cohort for positive specimens, since we were particularly interested in the sensitivity and false-negative rate. Performance of each test was compared with a composite standard.Results: The sensitivities and false-negative rates of the 239 specimens that met inclusion criteria were, respectively, as follows: Centers for Disease Control and Prevention 2019 nCoV Real-Time RT-PCR Diagnostic Panel, 100% and 0%; TIB MOLBIOL/Roche z 480 Assay, 96.5% and 3.5%; Xpert Xpress SARS-CoV-2 (Cepheid), 97.6% and 2.4%; Simplexa COVID-19 Direct Kit (DiaSorin), 88.1% and 11.9%; and ID Now COVID-19 (Abbott), 83.3% and 16.7%.Conclusions: The assays that included a nucleic acid extraction followed by reverse transcription polymerase chain reaction were more sensitive than assays that lacked a full extraction.
