Despite the increasing volume of research addressing the relevance of de-Westernizing Media Studies, we lack a comparative evaluation of the performance of Global South universities regarding their presence in the most prestigious domains within the field of Communication. Against this background, the article explores if and to what extent the publication of articles authored by BRICS-based scholars in top-ranked journals is restricted to a handful of academic institutions – making them a national “elite” authorized to engage in the mainstream intellectual debate.

Abstract Background Haemoptysis is a challenging symptom that can be associated with potentially life-threatening medical conditions. Follow-up is key in these patients to promptly detect new or misdiagnosed pathologic findings. Few prospective studies have evaluated long-term prognostic outcomes in patients with haemoptysis. Furthermore, the role played by antiplatelet and anticoagulant drugs on mortality and recurrence rates is unclear. The aim of this study was to assess mortality after 18 months of follow-up. Furthermore, the incidence of recurrence and the risk factors for recurrence and death were evaluated (including the role played by anticoagulant and antiplatelet drugs). Methods Observational, prospective, multicentre, Italian study. Results 451/606 (74.4%) recruited patients with haemoptysis completed the 18 months follow-up. 22/604 (3.6%) diagnoses changed from baseline to the end of the follow-up. 83/604 (13.7%) patients died. In 52/83 (62.7%) patients, death was the outcome of the disease which caused haemoptysis at baseline. Only the diagnosis of lung neoplasm was associated with death (OR (95%CI): 38.2 (4.2–347.5); p-value: 0.0001). 166 recurrences were recorded in 103/604 (17%) patients. The diagnosis of bronchiectasis was significantly associated with the occurrence of a recurrence (OR (95% CI): 2.6 (1.5–4.3)); p-value

Facial surgeries are often challenging for surgeons due to their complex anatomy, aesthetic and functional importance. Interventions in the cosmetic subunit that comprises the eyebrows can be particularly difficult. This case report aims to demonstrate the use of island flap in this region. We performed the excision of melanocytic nevus, with a good outcome and absence of postoperative complications. This technique is commonly used for facial reconstruction; however, the literature describes only a few cases using this method for eyebrow reconstruction.

In the context of oncological disease, the patient’s hospitalization needs to be optimized, aiming to alleviate the suffering of patients and family members. It is necessary to know the profile of the patients who access the service, mainly by emergency services. The aim of the study was to identify the profile of patients undergoing cancer treatment and the emergencies that affect them. This was a quantitative, retrospective, longitudinal, observational cohort study, analyzing data from medical records and hospital cancer records in 2017. 1635 patients were included, predominantly aged between 60 to 69 (27.3%) years, 852 (52.1 %) were male, 1542 (94.1%) of were white, 463 (28.3%) had a family history of cancer, and there was a median of 29 days to start post-diagnosis treatment. The typology with the highest incidence was other malignant neoplasms of the skin (n=538; 32.9%), and the location of the first metastasis in the liver (n=47; 2.8%). The patients admitted via emergency services were separated, resulting in 450 (27.5%) patients, with a mean age of 60.5 years, 240 (53.3%) were males and 239 (53.1%) were smokers and ex-smokers. 446 (99.11%) patients were admitted and four (0.88%) were referred to the outpatient clinic. Of the hospitalized patients, 337 (74.90%) were discharged from the hospital and 113 (25.10%) died. Abdominal pain appeared in 111 (24.6%) and tumor lysis syndrome was the main oncological emergency in 182 (40.4%) cases. Knowing the profile of cancer patients and their emergencies contribute to the understanding of the risks of the clinical manifestation, helping health professionals and improving the care provided.

BACKGROUND. Patients with coronavirus disease 2019 (COVID-19) develop pneumonia generally associated with lymphopenia and a severe inflammatory response due to uncontrolled cytokine release. These mediators are transcriptionally regulated by the JAK/STAT signaling pathways, which can be disabled by small molecules. METHODS. We treated a group of patients (n = 20) with baricitinib according to an off-label use of the drug. The study was designed as an observational, longitudinal trial and approved by the local ethics committee. The patients were treated with 4 mg baricitinib twice daily for 2 days, followed by 4 mg per day for the remaining 7 days. Changes in the immune phenotype and expression of phosphorylated STAT3 (p-STAT3) in blood cells were evaluated and correlated with serum-derived cytokine levels and antibodies against severe acute respiratory syndrome-coronavirus 2 (anti-SARS-CoV-2). In a single treated patient, we also evaluated the alteration of myeloid cell functional activity. RESULTS. We provide evidence that patients treated with baricitinib had a marked reduction in serum levels of IL-6, IL-1[beta], and TNF-[alpha], a rapid recovery of circulating T and B cell frequencies, and increased antibody production against the SARS-CoV-2 spike protein, all of which were clinically associated with a reduction in the need for oxygen therapy and a progressive increase in the P/F (Pa[O.sub.2], oxygen partial pressure/Fi[O.sub.2], fraction of inspired oxygen) ratio. CONCLUSION. These data suggest that baricitinib prevented the progression to a severe, extreme form of the viral disease by modulating the patients' immune landscape and that these changes were associated with a safer, more favorable clinical outcome for patients with COVID-19 pneumonia. TRIAL REGISTRATION. ClinicalTrials.gov NCT04438629. FUNDING. This work was supported by the Fondazione Cariverona (ENACT Project) and the Fondazione TIM., Introduction The pandemic spread of a novel, highly pathogenic coronavirus, severe acute respiratory syndrome-coronavirus 2 (SARS-CoV-2), has found the international medical community largely unprepared in terms of prophylactic and therapeutic [...]

Despite the growing scholarship concerning mis- and disinformation, research has yet to assess how journalism tackles conspiracy theories in settings where news organizations and media professionals have their authority questioned. Against this background, our article poses two research questions: RQ1) Who are the actors mainstream fact-checkers cover when addressing conspiracy theories? RQ2) To what extent does the focus on such actors delegitimize those who challenge news organizations or compete with them for the audience? Using content analysis as our key methodological strategy, we consider 197 fact-checks published between 2018 and 2021 by the Projeto Comprova, a Brazilian initiative currently comprising professionals from 41 media organizations. We found that the most discussed topics were those mobilizing polarized groups, namely, the Covid-19 pandemic and election fraud allegations. The then-President Bolsonaro and his supporters were often cited as disseminators of such plots. In turn, Facebook is pointed out as a thriving environment for the circulation of conspiratorial narratives. The results also reveal the prominence of mainstream news outlets as sources to ground the "factual" information sustained in the fact-checks. More interestingly, our data suggest that fact-checkers have favored specific news values when addressing conspiracy theories. To strengthen our investigation, we use interviews with four professionals contributing to the Comprova to illustrate how the project has brought together rival companies interested in delegitimizing "alternative" sources of information. Considering the claims for "de-Westernizing" Communication and Media studies, we examine the publication trends of BRICS-based researchers between 2012 and 2021. The article compares SciVal and SJR data concerning 7,938 articles to understand the circulation of such academic communities in top-ranked outlets. The results reveal a remarkable productivity growth in all BRICS countries, striking differences within the group in the number of research articles, the emergence of China as the bloc's leading hub, and the lack of cooperation among such five countries. We also discuss and criticize how the "de-Westernization" agenda may eventually become another category domesticated by the Global North. [ABSTRACT FROM AUTHOR]

Abstract: Papular elastorrhexis is an acquired disease of elastic tissue; considered rare, its etiology and pathogenesis remain unknown. The vast majority of cases occur in women in the first or second decade of life. The disease manifests as multiple uniformly-sized, circumscribed, hypochromic and achromic papules located predominantly on the trunk and upper extremities. The lesions are generally asymptomatic and have a stable evolution over years. Its diagnosis is based on clinical and histopathological findings. The benignity of papular elastorrhexis and the subtlety of its clinical changes make the expectant treatment perfectly viable. The authors present one case in a young female patient.

The present study investigated the changes in total and individual glucosinolates (GSLs) in roots and leaves of different clubroot-resistant and -susceptible oilseed rape cultivars following artificial inoculation with Plasmodiophora brassicae isolates with different virulence. The results showed significant differences in clubroot incidence and severity as well as in the amount of total and individual glucosinolates between oilseed rape cultivars in response to virulence of the pathogen. Single among with total aliphatic and total indolic glucosinolate contents were significantly lower in leaves of susceptible cultivars compared to resistant ones due to the infection. Similarly, single and total aliphatic as well as indolic glucosinolate contents in roots were lower in susceptible cultivars compared to resistant cultivars analyzed. The different isolates of P. brassicae seem to differ in their ability to reduce gluconasturtiin contents in the host. The more aggressive isolate P1 (+) might be able to suppress gluconasturtiin synthesis of the host in a more pronounced manner compared to the isolate P1. A possible interaction of breakdown products of glucobrassicin with the auxin receptor transport inhibitor response 1 (TIR1) is hypothesized and its possible effects on auxin signaling in roots and leaves of resistant and susceptible cultivars is discussed. A potential interplay between aliphatic and indolic glucosinolates that might be involved in water homeostasis in resistant cultivars is explained.

Medicine - Abstract
This study aimed to describe real-life microbiological testing of adults hospitalised with community-acquired pneumonia (CAP) and to assess concordance with the 2007 Infectious Diseases Society of America (IDSA)/American Thoracic Society (ATS) and 2011 European Respiratory Society (ERS) CAP guidelines. This was a cohort study based on the Global Initiative for Methicillin-resistant Staphylococcus aureus Pneumonia (GLIMP) database, which contains point-prevalence data on adults hospitalised with CAP across 54 countries during 2015. In total, 3702 patients were included. Testing was performed in 3217 patients, and included blood culture (71.1%), sputum culture (61.8%), Legionella urinary antigen test (30.1%), pneumococcal urinary antigen test (30.0%), viral testing (14.9%), acute-phase serology (8.8%), bronchoalveolar lavage culture (8.4%) and pleural fluid culture (3.2%). A pathogen was detected in 1173 (36.5%) patients. Testing attitudes varied significantly according to geography and disease severity. Testing was concordant with IDSA/ATS and ERS guidelines in 16.7% and 23.9% of patients, respectively. IDSA/ATS concordance was higher in Europe than in North America (21.5% versus 9.8%; p

This article provides a detailed investigation about to what extent the websites of municipalities in the Southern Region of Brazil improve public transparency. Our comparative examination encompasses a study of: a) legally required aspects and b) elements that exceed such mandatory demands in 21 cities. The results, arising from guided navigation and systematic observation techniques, reveal that a considerable part of the legal obligations has been fulfilled but that some flaws remain. The advanced tools available in specific cases suggest that legislation must be updated frequently. The article is relevant since it offers an analytical framework for advancing communication and public policy studies. Background The correct management of immunocompromised patients with pneumonia is debated. We evaluated the prevalence, risk factors, and characteristics of immunocompromised patients coming from the community with pneumonia. Methods We conducted a secondary analysis of an international, multicenter study enrolling adult patients coming from the community with pneumonia and hospitalized in 222 hospitals in 54 countries worldwide. Risk factors for immunocompromise included AIDS, aplastic anemia, asplenia, hematological cancer, chemotherapy, neutropenia, biological drug use, lung transplantation, chronic steroid use, and solid tumor. Results At least 1 risk factor for immunocompromise was recorded in 18% of the 3702 patients enrolled. The prevalences of risk factors significantly differed across continents and countries, with chronic steroid use (45%), hematological cancer (25%), and chemotherapy (22%) the most common. Among immunocompromised patients, community-acquired pneumonia (CAP) pathogens were the most frequently identified, and prevalences did not differ from those in immunocompetent patients. Risk factors for immunocompromise were independently associated with neither Pseudomonas aeruginosa nor non–community-acquired bacteria. Specific risk factors were independently associated with fungal infections (odds ratio for AIDS and hematological cancer, 15.10 and 4.65, respectively; both P = .001), mycobacterial infections (AIDS; P = .006), and viral infections other than influenza (hematological cancer, 5.49; P < .001). Conclusions Our findings could be considered by clinicians in prescribing empiric antibiotic therapy for CAP in immunocompromised patients. Patients with AIDS and hematological cancer admitted with CAP may have higher prevalences of fungi, mycobacteria, and noninfluenza viruses.

Incontinência Urinária. Acidente Vascular Encefálico. Perfil de Saúde. ,Medicine (General) ,R5-920 - Abstract

Background: Several studies have shown that Tai Chi Chuan can improve cardiac function in patients with heart disease. Objective: To conduct a systematic review of the literature to assess the effects of Tai Chi Chuan on cardiac rehabilitation for patients with coronary artery disease. Methods: We performed a search for studies published in English, Portuguese and Spanish in the following databases: MEDLINE, EMBASE, LILACS and Cochrane Register of Controlled Trials. Data were extracted in a standardized manner by three independent investigators, who were responsible for assessing the methodological quality of the manuscripts. Results: The initial search found 201 studies that, after review of titles and abstracts, resulted in a selection of 12 manuscripts. They were fully analyzed and of these, nine were excluded. As a final result, three randomized controlled trials remained. The studies analyzed in this systematic review included patients with a confirmed diagnosis of coronary artery disease, all were clinically stable and able to exercise. The three experiments had a control group that practiced structured exercise training or received counseling for exercise. Follow-up ranged from 2 to 12 months. Conclusion: Preliminary evidence suggests that Tai Chi Chuan can be an unconventional form of cardiac rehabilitation, being an adjunctive therapy in the treatment of patients with stable coronary artery disease. However, the methodological quality of the included articles and the small sample sizes clearly indicate that new randomized controlled trials are needed in this regard.

- Abstract
Background: Aspiration community-acquired pneumonia (ACAP) and community-acquired pneumonia (CAP) in patients with aspiration risk factors (AspRFs) are infections associated with anaerobes, but limited evidence suggests their pathogenic role. Research Question: What are the aspiration risk factors, microbiology patterns, and empiric anti-anaerobic use in patients hospitalized with CAP? Study Design and Methods: This is a secondary analysis of GLIMP, an international, multicenter, point-prevalence study of adults hospitalized with CAP. Patients were stratified into three groups: (1) ACAP, (2) CAP/AspRF+ (CAP with AspRF), and (3) CAP/AspRF- (CAP without AspRF). Data on demographics, comorbidities, microbiological results, and anti-anaerobic antibiotics were analyzed in all groups. Patients were further stratified in severe and nonsevere CAP groups. Results: We enrolled 2,606 patients with CAP, of which 193 (7.4%) had ACAP. Risk factors independently associated with ACAP were male, bedridden, underweight, a nursing home resident, and having a history of stroke, dementia, mental illness, and enteral tube feeding. Among non-ACAP patients, 1,709 (70.8%) had CAP/AspRF+ and 704 (29.2%) had CAP/AspRF-. Microbiology patterns including anaerobes were similar between CAP/AspRF-, CAP/AspRF+ and ACAP (0.0% vs 1.03% vs 1.64%). Patients with severe ACAP had higher rates of total gram-negative bacteria (64.3% vs 44.3% vs 33.3%, P =.021) and lower rates of total gram-positive bacteria (7.1% vs 38.1% vs 50.0%, P <.001) when compared with patients with severe CAP/AspRF+ and severe CAP/AspRF-, respectively. Most patients (>50% in all groups) independent of AspRFs or ACAP received specific or broad-spectrum anti-anaerobic coverage antibiotics. Interpretation: Hospitalized patients with ACAP or CAP/AspRF+ had similar anaerobic flora compared with patients without aspiration risk factors. Gram-negative bacteria were more prevalent in patients with severe ACAP. Despite having similar
