ABSTRACT Objectives: to investigate the influence of the leukoreduction moment (preor post-storage) of blood components on the clinical outcomes of patients transfused in the emergency department. Methods: retrospective cohort study of patients aged 18 years or older who received preor post-storage leukoreduced red blood cell or platelet concentrate in the emergency department and remained in the institution for more than 24 hours. A generalized mixed-effects model was applied in the analyses. Results: in a sample of 373 patients (63.27% male, mean age 54.83) and 643 transfusions (69.98% red blood cell), it was identified that the leukoreduction moment influenced the length of hospital stay (p0.050) on transfusion reactions, healthcare-associated infections, or mortality. Conclusions: patients who received pre-storage leukoreduced blood components in the emergency department had a shorter length of hospital stay.

ABSTRACT This study aimed to determine the effectiveness of topical HoofCare® in the treatment of digital dermatitis (DD) in dairy cows. Thirty-six high-yielding Holstein cows with DD in at least one limb were included and randomly divided into two groups, making 42 limbs. The alternating treatment group consisted of 21 limbs treated for three alternating days every 48h, and the continuous treatment group consisted of 21 limbs treated every 24h for five days. The cows underwent lameness examination, thermographic analysis, and qualitative and quantitative analysis of the lesions on D0 (time of identification of the lesion and prior to treatment), D4 (fourth day of treatment), and D10 (five days after the end of treatment). The data were compared between different time points and groups. There was no significant difference between the groups for the variables evaluated. However, both groups presented a reduction in the degree of lameness around the lesion, in the local temperature, and regression from the active to the inactive stage of the lesions. Thus, this short-term study demonstrates the suggestive efficacy of HoofCare® in treating DD and recommends conducting further long-term studies.

Air medical transport during the coronavirus disease 2019 pandemic was essential for transferring critically ill patients. This study aimed to comparatively analyze air-transported patients with and without coronavirus disease 2019 according to their clinical condition and complications that occurred during the flight. This was a retrospective cohort study that analyzed the digital records of adult patients transported by fixed-wing aircraft from the interior of the state of Amazonas to the state capital Manaus, Brazil, from June 2019 to May 2021. Pearson's chi-squared, Fisher exact, and Wilcoxon-Mann-Whitney tests were applied (significance level of P <.05). The sample consisted of 741 patients (60.59% men, median age 54 years). The incidence of complications during the flight was 7.28%, with emphasis on dyspnea, psychomotor agitation, and pain. There was a significant difference between patients with (n = 466) and without coronavirus disease 2019 (n = 275) regarding the variables age (P <.001), comorbidities (P <.001), body mass index (P <.001), impact (P <.001) and priority (P =.002) of the transfer, physiological severity (P <.001), use of vasoactive drugs when boarding the aircraft (P =.033), and occurrence of respiratory complications during air medical transport (P =.003). Patients with coronavirus disease 2019 were older, had more comorbidities and were severely ill, and had higher body mass index, frequency of vasoactive drug use, and respiratory complications. Although there are minimal differences among these patients, the role that interhospital transfer plays in reducing burden on local, less well-equipped hospitals is a primary role of medical transport, particularly during pandemics. Purpose: The impact of social support on comprehensive measures of results (clinical and functional) of the course of schizophrenia was studied, understood and evaluated as a multidimensional construct differentiating sources of support (family vs. nonfamily). Methods: One hundred fifty-two patients diagnosed with schizophrenia were assessed with the Mannheim Interview on Social Support (MISS) and the Social Functioning Scale (SFS). The hypotheses were explored in a prospective longitudinal design, using a causal correlational analysis for their evaluation by applying structural equation models. Results: The only explanatory factor of social functioning was Nonfamily social support, while the only explanatory factor of clinical result measurements was Family social support, observing a clearly differentiated impact of the different sources of support on the schizophrenia result measurements. It was also found that while Family social support explained 6.8% of the variance in the clinical result measurements, Nonfamily social support explained 13.7% of the variance in social functioning. Conclusion: The results confirmed the differential importance of social support variables (family vs. nonfamily) in the clinical and functional result measurements of people with schizophrenia. Abstract Criminological and sociodemographic variables, such as previous criminal convictions, increased risk of violence, early onset of mental disorder, antisocial personality, psychosis and low social support, have all been related to longer length of stay (LoS) and poorer outcome in long stay forensic services. The factors impacting on LoS and clinical response in acute care specialized units are poorly documented. To address this issue, we examined the psychiatric records of all cases admitted between January 1st and December 31th 2020 in the sole acute ward for detained persons located in the central prison of the Geneva County, Switzerland. Information on judicial status included pre-trial versus sentence execution, previous incarcerations, and age of the first incarceration. Sociodemographic data included age, gender, marital status, and education attainment. Previous inpatient stays prior to incarceration were recorded. All of the ICD-10 clinical diagnoses were made by two independent, board-certified psychiatrists blind to the scope of the study. The standardized assessment was based on the HoNOS (Health of Nation Outcome Scales) at admission and discharge, HONOS-secure at admission, HCR-20 (Historical Clinical Risk 20) version 2, PCL-R (Psychopathy Checklist Revised), and SAPROF (Structured Assessment of Protective Factors). Stepwise forward multiple linear regression models predicting the LoS and delta HONOS respectively were built with the above mentioned parameters. The selected variables were then used in univariate and multivariable regression models. Higher HCR-scores (mainly on clinical items), and longer LoS were related to higher delta HONOS scores. In contrast, cases in pre-trial detention showed a worst clinical outcome. In multivariable models, all three variables remained independent predictors of the clinical outcome and explained 30.7% of its variance. Only education and diagnosis of borderline personality were related to the LoS and explained 12.6% of its variance in multivariable models. Our results suggest that the use of acute wards specialized in forensic psychiatry are mainly useful for patients with prior inpatient care experience, and higher violence risk during sentence execution. In contrast, they seem to be less performant for persons in pre-trial detention that could benefit from less restrictive clinical settings.

Tuberculosis (TB) is an infectious and contagious disease that poses a challenge in pediatric practice, since children can present rare manifestations, which makes their diagnosis difficult. Five cases of uncommon manifestations of TB are presented in children vaccinated with BCG at birth and with no history of previous contact with bacilliferous adults. The difficulty and importance of the diagnostic suspicion of TB in rare cases of the disease in children is emphasized.

ABSTRACT Purpose To characterize changes in the functioning aspects, in the perception of children and adolescents with speech and language disorders under speech-language follow-up, using the ICF. Methods Descriptive, analytical and longitudinal research, with a qualitative and quantitative approach, whose sample consisted of 60 children and adolescents: 30 with speech and language disorders and 30 with typical speech and language development. Data collection was carried out in two moments: beginning of the research and six months later. A semi-structured questionnaire was administered to the participants, and a medical records analysis was performed. From these data, functioning was classified using he ICF categories. The Wilcoxon test and thematic content analysis were used to compare the interviews. Results The use of ICF allowed characterizing changes resulting from speech-language follow-up. Participants with speech and language disorders presented a decrease in the magnitude of the qualifiers in the categories: articulation and fluency, social relationships, daily activities, engagement in play, people's attitude barriers, and how to handle stress. Conclusion The findings show changes in components of Body Functions, Activities and Participation, and the influence of Environmental Factors after speech-language follow-up, in the perception of the studied group, which brings relevant subsidies for a greater understanding of functioning and therapeutic intervention. The use of the ICF enabled the longitudinal analysis in a biopsychosocial approach, contemplating, in addition to biological aspects, the social impact of speech and language disorders in the lives of these children and adolescents.

Introduction: The prevalence of psychiatric disorders has not shifted widely through the COVID pandemic, except for some specific groups such as young people or women. Our objective is to examine prospectively the evolution of children and adolescents who consulted in a psychiatric emergency service during the COVID-19 confinements. Method: We collected prospective clinical information about 296 young people under 18 who visited a tertiary hospital for psychiatric reasons during the confinement periods in Spain. Clinical diagnoses, suicide attempts, hospital admissions, and pharmacological prescriptions were extracted from electronic health records through 2020, 2021, and 2022. Features of those who maintained psychiatric care and those who did not were compared. Results: Three out of four children and adolescents who visited the psychiatric emergency department during the confinements continued psychiatric care at the end of 2022. Those who did not showed better premorbid adjustment at baseline. During follow-up, diagnoses of neurodevelopmental disorders and eating disorders, as well as the dosage of psychotropic drug prescriptions, increased. The diagnoses of major depressive disorder and eating disorder at baseline were associated with attempting suicide during follow-up. Patients with internalizing symptoms were admitted earlier than those with externalizing symptoms but no differences were found in terms of suicide attempts. Conclusions: The continuity of psychiatric care after an initial emergency visit during the confinements implied greater clinical severity, as reflected by changes in clinical diagnoses and pharmacological regimens. Emergent symptoms of depression or eating disorders after social distancing or isolation could predict subsequent suicidal behavior in young populations. Highlights: A large majority (76.3%) of children and adolescents presenting a psychiatric emergency during the COVID-19 confinements continued psychiatric care for at least 18 months. Non-continuity of psychiatric care after the index episode was associated with less severe psychiatric diagnoses and better premorbid adjustment at baseline. Compared to baseline, diagnoses of neurodevelopmental disorders and eating disorders, as well as psychotropic drug prescriptions, increased at the end of the study period. The objective of this research was to learn about the effects of music as a non-pharmacological therapeutic intervention applied to Alzheimer’s disease. To this aim, we evaluated its results regarding symptomatology and caregiver burden. Methods: This systematic review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) criteria. We searched Pubmed/Medline (NLM), Web of Science, Scopus, Cochrane and Google Scholar, including articles from 1 January 2011–20 July 2021, using the keywords “Alzheimer’s disease”, “Music therapy”, “Caregivers overload”, “Amnesia retrograde” and “Clinical evolution”. To select the articles our criteria included complete text availability, quantitative research of an experimental nature and studies which, at least, figured in SJR or in JCR. Results: We found a relationship between the application of music therapy in patients with Alzheimer’s disease and an improvement regarding symptomatology, as it smoothed down the progress of the disease. Using music therapy in Alzheimer’s patients also led to a decrease in caregivers’ burden and an increase in their well-being. Conclusions: Our results showed the benefits of music therapy, as it improved both patients’ symptomatology and caregivers’ burden.

Background: Crohn's disease is characterized by damage to the intestines, which can affect the gastrointestinal tract (from the mouth to the anus). The risks of Crohn's disease appear to be linked to changes in the gut Macrobiota or disturbances in the mucosa and intestinal genetics. Objectives: We aimed to study the implication of appendectomy on the risk of the development of Crohn's disease in the western Algeria population. Methods: It was a retrospective analytical study of 403 patients from 2007 to 2020 carried out at the level of general surgery departments and university hospitals in western Algeria. Results: Four hundred three CD patients were included in our study. Among them, 81 cases have undergone an appendectomy. The average age was 36.88 ± 12.47, and the most affected location was the ileocecal location with P = 0.001. However, no significant association was noted between appendectomy andCDphenotype. Most of them suffered from constipation (P< 0.0001), came from urban areas, and presented extra-intestinal manifestations (P < 0.0001). Treatment of the disease was often medical, while 68 cases of the 81 appendectomized patients underwent surgery. Early relapse of CD was also observed in appendectomized patients with P = 0.035. Moreover, many complications (anal fissures, anal fistula) were also noted but with no significant association. Conclusions: From our results, it appears that Crohn's disease risk can be associated with a previous appendectomy and mainly manifested by ileocecal localization and constipation. Introduction: The T315I mutation in patients with chronic myeloid leukemia (CML) has been associated with therapeutic resistance and an unfavourable prognosis.Aim: To study the frequency of T315I mutation in patients with CML, BCR-ABL (+), their clinical characteristics, disease evolution, and median survival.Patients and methods: We studied 75 patients with CML and BCR-ABL1 (+). T315I mutation was detected by digital droplet PCR and BCR-ABL1 was analyzed by RT-PCR. A comparative analysis was performed by sex, age, disease phase, risk group, treatment, molecular response (MR), and median survival in T315I (+) and T315I (−) patients.Results: T315I mutation was detected in 11 patients (14.7%). No significant difference was found in the phase, risk group, and first-line therapy. A significantly higher proportion of T315I (+) did not achieve MR >3.5 log: 8 (72.7%) vs. 22 (34.4%) (p=0.023). The lowest mean BCR-ABL1 levels were significantly higher in the CML T315I (+) group compared to the CML T315I (−) group: 12.1±6.0 vs. 3.77±1.28 (p=0.009). Aim of the study: studying the literature review about the etiology, classification, particularities of clinical evolution, diagnosis and treatment of pediatric hydrocephalus. Materials and methods: A systematic search was conducted using various databases, including Google Search, Hinari, PubMed, Z-library, NCIB, and Medscape. The search criteria included access to full-text articles, reports, books, and book chapters, focusing on pediatric hydrocephalus. Articles published within the last decade were included. Data were synthesized to highlight key clinical, diagnostic, and treatment aspects of pediatric hydrocephalus. Additionally, the bibliographies of relevant publications were reviewed to identify additional sources Outcomes: Timely diagnosis and appropriate management are crucial for optimizing outcomes in pediatric hydrocephalus. Long-term prognosis varies depending on factors such as the severity of the condition and the effectiveness of treatment interventions. Complications like shunt malfunction, infections, and neurological deficits can impact overall prognosis significantly. Shunt malfunction, if left untreated, can lead to increased intracranial pressure and brain herniation. Similarly, shunt infections can result in life-threatening conditions such as meningitis and sepsis. Neurological deficits arising from hydrocephalus, such as motor and cognitive impairments, can profoundly affect a child's quality of life. Early intervention with rehabilitative therapies is vital for maximizing developmental potential and promoting independence. In summary, the prognosis for pediatric hydrocephalus hinges on factors like early detection, effective management of complications, and access to rehabilitative care. Baseline and follow-up data of COPD subjects with a chronic bronchitis (CB) or emphysema (EM) phenotype were collected. The frequency of COPD symptoms during the 24 hours (gathered via the night-time, morning and day-time symptoms of COPD questionnaire) and the anxiety and depression levels (via the HADS Scale) were recorded at each visit.Results: A total of 261 CB and 159 EM patients were analyzed. CB patients with ≥ 1 night-time symptom seemed to be more frequent (51.7%, 41.8% and 41.4% at baseline, 6-month and 12-month follow-up, respectively) than EM (37.7%, 32.1% and 30.2% at study visits) even if no statistical differences were observed at time points between phenotypes (chi-square test p-values presence/absence of night-time symptoms in CB vs EM at study visits > 0.0007). In the first 6 months, the frequency of patients with ≥ 1 night-time symptom decreased of 9.9% in CB and of 5.6% in EM. A clinically relevant decline of DLCO % predicted over 1 year in EM was observed, the mean (SD) being 61.5 (20.8) % at baseline and 59.1 (17.4) % at 12-month follow-up. EM had higher levels of anxiety and depression than CB (median (25th-75th percentile) HADS total score in CB: 7.0 (4.0– 13.0) and 7.0 (3.0– 12.0), in EM: 9.0 (3.0– 14.0) and 9.5 (3.0– 14.0) both at baseline and at 6-month follow-up, respectively), considering 1.17 as minimally clinical important difference (MCID) for the total score.Conclusion: EM patients, evaluated in a real-world setting, seem to suffer from a worse clinical condition and health status compared to CB patients, appearing to have “more treatable” traits.Keywords: COPD, clinical phenotype, clinical evolution, quality of life

Nutcracker syndrome, whose prevalence and natural history are still poorly known, is a clinical syndrome caused by left renal vein compression between the superior mesenteric artery and the aorta. Long-term results and treatment outcomes are not well known. Our group aimed to characterize 7 patients diagnosed with nutcracker syndrome in childhood and to describe their clinical manifestations, diagnostic approaches, and mostly their clinical evolution, rate of complications, and treatment outcomes.

Background: Vitiligo has an unpredictable course and a variable response to treatment. Furthermore, the improvement of some vitiligo lesions cannot be considered a guarantee of a similar response to the other lesions. Instruments for patient-reported outcome measures (PROM) can be an alternative to measure complex constructions such as clinical evolution. Objective: The aim of this study was to validate a PROM that allows to measure the clinical evolution of patients with nonsegmental vitiligo in a simple but standardized way that serves to gather information for a better understanding of the disease. Methods: The instrument was created through expert consensus and patient participation. For the validation study, a prospective cohort design was performed. The body surface area affected was measured with the Vitiligo Extension Score (VES), the extension, the stage, and the spread by the evaluation of the Vitiligo European Task Force assessment (VETFa). Reliability was determined with test-retest, construct validity through hypothesis testing, discriminative capacity with extreme groups, and response capacity by comparing initial and final measurements. Results: Eighteen semi-structured interviews and 7 cognitive interviews were conducted, and 4 dermatologists were consulted. The instrument Clinical Evolution-Vitiligo (CV-6) was answered by 119 patients with a minimum of primary schooling. A wide range was observed in the affected body surface; incident and prevalent cases were included. The average time to answer the CV-6 was 3.08 ± 0.58 min. In the test-retest (n = 53), an intraclass correlation coefficient was obtained: 0.896 (95% CI 0.82–0.94; p < 0.001). In extreme groups, the mean score was 2 (2–3) and 5 (4–6); p < 0.001. The initial CV-6 score was different from the final one and the change was verified with VES and VETFa (p < 0.05, n = 92). Conclusions: The CV-6 instrument allows patient collaboration, it is simple and brief, and it makes it easier for the doctor to focus attention on injuries that present changes at the time of medical consultation. Introduction: Stroke is one of the leading causes of morbidity and mortality worldwide, resulting in a variety of deficits, motor and cognitive-behavioral, as well as emotional. Thus, rehabilitation nursing has great importance in helping the patient regain lost capacities and become independent again, including family in this process. However, the work of the rehabilitation nurse should not be restricted to the acute phase and should be extended over time, especially after returning home, when problems related to a higher degree of dependence arise. Objective: Analyze deviations from the plan defined at discharge by the rehabilitation nurse. Method: An exploratory, descriptive, retrospective and quantitative study involving 339 patients. Results: Patients have a mean age of 71 years, are predominantly male, and the predominant vascular event was ischemic stroke. The majority of patients presented functional and motor improvement after discharge. Several problems were identified in the consultation and corrective measures were taken to prevent them from worsening and to promote future corrections. Statistically significant relationships were established between the degree of dependence and the institutionalization of the patients, as well as between the cognitive alterations and the degree of dependence. Conclusion: The results made it clear that even after discharge, patients with stroke need support, support, guidance, teaching and training to improve outcomes and prevent complications. Hence, the work of the rehabilitation nurse should extend beyond the discharge of the patient with stroke, with a view to the future with greater security.

ABSTRACT Rational: The metabolic response to surgical trauma is enhanced by prolonged preoperative fasting, contributing to increased insulin resistance. This manifestation is more intense on the 1st and 2nd postoperative days and is directly proportional to the size of the operation. Aim: To compare whether preoperative fasting abbreviation and early postoperative refeeding associated with intraoperative and postoperative fluid restriction interfere in the evolution of patients undergoing gastrojejunal bypass. Methods: Eighty patients indicated for Roux-en-Y gastrojejunal bypass were selected. They were randomly divided into two groups: Ringer Lactate (RL) group, who underwent a 6 hours solids fasting, with the administration of 50 g of maltodextrin in 100 ml of mineral water 2 hours before the beginning of anesthesia; and Physiologic Solution (PS) group, who underwent a 12 hours solids and liquids fasting. Anesthesia was standardized for both groups. During the surgical procedure, 1500 ml of ringer lactate solution was administered in the RL and 2500 ml of physiological solution (0.9% sodium chloride) in the PS. In both groups, the occurrence of bronchoaspiration was analyzed during intubation, and the residual gastric volume was measured after opening the abdominal cavity. In the postoperative period in Group RL, patients started a liquid diet 24 hours after the end of the operative procedure; whilst for PS group, fasting was maintained for the first 24 hours, it was prescripted 2000 ml of physiological solution and a restricted liquid diet after 36 hours. Each patient underwent CPK, insulin, sodium, potassium, urea, creatinine, PaCO2, pH and bicarbonate dosage in the immediate postoperative period, and 48 hours later, the exams were repeated. Results: There were no episodes of bronchoaspiration and gastrojejunal fistulas in either group. In the analysis of the residual gastric volume of the PS and RL groups, the mean volumes were respectively 16.5 and 8.8, which shows statistical significance between the groups. In laboratory tests, there was no difference between groups in sodium; PS group showed a higher level of serum potassium (p=0.029); whilst RL group showed a higher urea and creatinine values; CPK values were even for both; PS group demonstrated a higher insulin level; pH was higher in PS group; sodium bicarbonate showed a significant difference at all times; PaCO2 values in RL group was higher than in PS. In the analysis of the incidence of nausea and flatus, no statistical significance was observed between the groups. Conclusions: The abbreviation of preoperative fasting and early postoperative refeeding of Roux-en-Y gastrojejunal bypass with the application of ERAS or ACERTO Project accelerated the patient’s recovery, reducing residual gastric volume and insulin level, and do not predispose to complications.

Introduction: Body liposuction and abdominoplasty are surgeries often performed together to obtain superior results in body modeling. Since associated surgeries often increase spoliation, being aware of the evolution of hemoglobin (Hb) in the postoperative period and during the recovery of the patients undergoing these associated surgeries is important for their safety. This study aimed to analyze the decrease in Hb and the clinical and laboratory results throughout the recovery of patients undergoing body liposuction associated with lipoabdominoplasty. Methods: A prospective study was conducted with patients undergoing body liposuction and lipoabdominoplasty. CBCs were collected before anesthetic induction, at the end of the surgery, before hospital discharge, after the 1st, 2nd, and 4th postoperative weeks, and during their clinical follow-up period. Results: The average Hb values at the end of surgery and hospital discharge were 10.4 g/dL (standard deviation (SD) 0.76) and 8.92 g/dL (SD 0.86), respectively. The average values during the recovery of Hb after the 1st, 2nd, and 4th weeks were 2.4% (SD 18.07), 41.6% (SD 18.4), and 74% (SD 15.2), respectively. This is in relation to the reduction between the initial Hb and at hospital discharge. Complaints of weakness and lipothymia were frequent until the second day. Conclusion: Clinical improvement was observed until the second postoperative day (PO day). Hemoglobin required approximately 1 month to normalize in most patients. These patients were treated only with oral iron replacement and did not require blood transfusions.

Objectives: The COVID-19 infection was declared pandemic in March 2020. Since then, multiple studies have attempted to correlate clinical factors with the risk of complications from COVID-19. However, cancer patients are underrepresented in clinical trials and the results vary between different cohorts. Our goal is to describe a cohort of cancer patients and COVID-19. Methods: We conducted a multicenter retrospective study, based on a systematic review of medical records, including nine cancer centers, located in five different Brazilian cities. Patients were diagnosed with COVID-19 through RT-PCR between March 15th, 2020, and August 13th, 2020. Poisson regression models were then used to test for an association between clinical characteristics and severity of COVID-19 infections. Results: 102 patients had data collected for analysis, 85 (83.3%) of whom were hospitalized due to complications from COVID-19 infection. The median age was 65.8 years, most were female patients (61.8%) and white (73,5%). 78.4% had a performance status of 0-1, and the most common cancer subtypes were gastrointestinal (30.4%), breast (22.6%), and hematological (13.7%). Almost 40% of the population had stage IV disease. The mortality rate for all hospitalized patients was 36.5%, while that for those admitted to ICU was 68.4%. Key univariable risk factors for mortality included age (RR 1.03), ECOG = 2 (RR 1.83), hypertension (RR 1.72), lung metastasis (RR 1.67), and lymphocytes = 1000 admission (RR 2.40). At the multivariable analysis, the risk factors were also age (RR 1.02), primary lung cancer (RR 2.61), lung metastasis (RR 2.86), and coronary disease (RR 3.76). Conclusions: Despite the high mortality of patients hospitalized with COVID-19, cancer is a heterogeneous disease and some risk factors should be considered as the main responsible for the worst prognosis. Cancer patients should be carefully monitored in pandemic periods of infectious diseases and their management must be individualized.

Background: The disease caused by the dengue virus constitutes a serious health problem for tropical countries. The last decade has seen the re-emergence of this entity in Cuba, with evident changes in its clinical behavior. Objective: to compare the clinical-epidemiological behavior between two series of cases of patients with dengue confirmed diagnosis, in different periods of time. Methods: a descriptive, comparative study that included two series of cases of hospitalized patients with dengue confirmed diagnosis: series A, sample of 83 patients seen in 2017; and series B, sample of 327 seen in 2018. Variables such as: age, signs and symptoms, clinical findings, alarm signs, severe forms, and indicators of quality of care in dengue patients were analyzed. Results: in general, the acute febrile syndrome predominated, although other signs and symptoms such as asthenia, anorexia, rash and retro-ocular pain, manifested a higher incidence in series B. In both groups the most frequent paraclinical finding was lymphocytosis. In series B, the quality standards for the care of patients with dengue were more affected; as well as it also presented a higher rate of alarm signs, serious forms and complications. Conclusions: Although the study analyzed series of patients in two consecutive periods, the more complex behavior was evident in the second, due to the higher incidence of symptoms and less favorable figures for the quality standards established for the disease. Introduction: Trauma injuries are sudden aggravations to health that may lead to temporary disabilities and interfere with the victim's quality of life. The reconstructive plastic surgery (RPS) unit of the Urgency and Emergency Metropolitan Hospital (Hospital Metropolitano de Urgência e Emergência - HMUE) is a referral unit for the treatment of trauma patients in the state of Pará, Brazil. Methods: This was an observational, analytical, and cross-sectional prospective study. The study population was composed of 78 patients treated from December 2015 to December 2016. Results: The study population was predominantly male, autonomous, and aged between 21 and 30 years. Traffic accidents were the most frequent cause of trauma. The most affected area was the lower limbs, and the most common type of surgery performed was grafting. Viability was in the range of 90-100% in both patients undergoing graft and flap surgery. No significant association was found between the age range of patients and the degree of viability. However, there was a relationship between the number of days from the accident until the medical intervention and the degree of graft viability. Conclusion: Patients admitted to the hospital on the same day of the accident were six times more likely to present graft viability above 80%, and therefore, to have a favorable outcome.

ABSTRACT Objective to map the knowledge about the clinical findings, treatment and outcome of newborns and children infected with COVID-19. Methods a scoping review with search of eight databases and electronic search engine in April 2020. Results the 12 studies analyzed showed that the main clinical findings in this population were nasal congestion, fever, respiratory distress, diarrhea, fatigue, dry cough, increased C-reactive protein, leukopenia, lymphopenia, thrombocytopenia, elevated procalcitonin, bilateral ground-glass opacity, pulmonary consolidation, and pneumonia. Antivirals, respiratory support, immunomodulatory therapy, glucocorticoids, antibiotics and alpha interferon were used as treatment. The presence of a cure with hospital discharge is present in most cases. Final considerations most patients required hospitalization, but it evolved to cure. This study provided a greater scientific basis by showing clinical findings, treatment, and outcomes in neopediatric patients with COVID-19.

ABSTRACT Objective: To verify the interval of responsiveness to the scales Segmental Assessment of Trunk Control (SATCo-BR), Performance of Upper Limbs (PUL), and Jebsen Taylor Test (JTT) in patients with Duchenne Muscular Dystrophy (DMD). Methods: We assessed patients with DMD aged 6 to 19 years old and with mini-mental (MMSE) score above 10 points. The assessments were performed individually, in a single session. The upper limb function was performed by PUL and JTT, and trunk control by SATCo-BR. Assessments were repeated six and 12 months after the initial assessment. The repeated-measures analysis of variance model and Bonferroni’s multiple comparison method were employed as post hoc analysis; when the ANOVA assumptions were not met, the Friedman test was applied. Results: The sample consisted of 28 patients evaluated in three moments (initial, and six and 12 months after the beginning). There was a time effect for the Upper Limb function performance in the total JTT, and for the subtests, except for subtests 1 and 6, which did not show a difference between the different moments. There was also a time effect for the score of total PUL, proximal PUL, intermediate PUL, and distal PUL. In the SATCo-BR, this effect was observed between the initial and 6 months, and between the initial and 12 months. Conclusions: The JTT, PUL, and SATCo-BR scales can detect changes over time, and they showed responsiveness to detect the evolution of the disease in the 6-month interval.

Nutcracker syndrome, whose prevalence and natural history are still poorly known, is a clinical syndrome caused by left renal vein compression between the superior mesenteric artery and the aorta. Long-term results and treatment outcomes are not well known. Objective: to identify the factors associated with death due to COVID-19 among Brazilian postpartum women in the first five months of the pandemic and five subsequent months, and describe the sociodemographic and clinical characteristics of postpartum women who developed the disease. Method: cross- sectional population-based study using a secondary database available in the Sistema de Informação de Vigilância Epidemiológica da Gripe -SIVEP-Gripe (Influenza Epidemiological Surveillance Information System), Brazilian Ministry of Health. A total of 869 postpartum women were included, and the analysis considered the first five months of the pandemic and subsequent five months. Association between the variables of interest and outcome (death due to COVID-19/cure) was investigated using logistic regression. Results: most participants were aged between 20 and 34, of mixed race or Caucasian, and lived in the urban/peri-urban area. The proportion of deaths was 20.2% in the first period and 11.2% in the second. The likelihood of death increased in both periods due to the presence of respiratory signs and symptoms: dyspnea, respiratory distress, and oxygen saturation below 95%, in addition to the need for ventilatory support and intensive care. Conclusion: the proportion of deaths among postpartum women was high and decreased in the second period under study. ABSTRACT Objective: Cross-cultural adaptation of the National Early Warning Score 2 to Brazilian Portuguese. Methods: A methodological study of a cross-cultural adaptation of a scale, based on the Beaton et al. framework, authorized by the Royal College of Physicians. Judges from nine Brazilian states, nurses and physicians evaluated the semantic, idiomatic, cultural, and conceptual equivalence between the original instrument and the translated versions. The nurses, working in inpatient or emergency units, conducted the pilot test, applying the final version to three case studies. Psychometric tests were used for data analysis: Content Validity Index (CVI), Kappa Coefficient, and Cronbach's Alpha. Results: The adaptation showed a mean CVI of 0.98 and perfect/almost perfect inter-rater agreement, with scores above 0.80. The consistency of the scale was 0.712. Conclusion: The process of cross-cultural adaptation of the scale to Brazilian Portuguese was successful, providing Brazilian professionals with an instrument aligned with patient safety.

Abstract Oral leukoplakia (OL) is a white lesion of an indeterminate risk not related to any excluded (other) known diseases or disorders that carry no increased risk for cancer. Many biological markers have been used in an attempt to predict malignant transformation; however, no reliable markers have been established so far. Objective To evaluate cell proliferation and immortalization in OL, comparing non-dysplastic (Non-dys OL) and dysplastic OL (Dys OL). Methodology This is a cross-sectional observational study. Paraffin-embedded tissue blocks of 28 specimens of Non-dys OL, 33 of Dys OL, 9 of normal oral mucosa (NOM), 17 of inflammatory hyperplasia (IH), and 19 of oral squamous cell carcinomas (OSCC) were stained for Ki-67 and BMI-1 using immunohistochemistry. Results A gradual increase in BMI-1 and K-i67 expression was found in oral carcinogenesis. Sepsis is currently the leading cause of death in burned patients. There are few studies on sepsis in burned patients at intensive care unit (ICU). To determine demographic profile, clinical presentation, evolution, procedures, and treatments used for burned patients affected by sepsis. Retrospective study in medical records of severe burned adult patients who developed sepsis between November 2015 and May 2018 in a university hospital in Curitiba, Brazil. Patients' details about hospitalization and sepsis were collected. Were included 44 patients, 75% men, and mean age of 42.1±16.88 years. The median TBSA was 50% that was significantly associated with mortality (P = .013). Outcome of death was observed in 50% of the patients. The median duration of hospitalization was 35 days, and in the ICU was 21.5 days. Orotracheal intubation and tracheostomy were the most prevalent aggravating procedures conducted during the hospitalization (77.2% and 56.8%, respectively). The median time to the first sepsis episode was 7 days, and the average total time in sepsis was 13.2 days. The median length of hospital stay among patients with septic shock who died was significantly lower than that of patients who did not die (P = .031). Blood culture was positive in 79.5% of cases, with the majority being typical ICU bacteria. Sepsis occurs more frequently in patients with higher TBSA and long hospitalization time accompanied by aggravating procedures and complications. Objective To determine the sensitivity to antibiotics of germs responsible for bacterial meningitis in children and the clinical course of treatment Methods This was a contemporary cross-sectional study, conducted from 15 December 2016 to 15 December 2017 in the Pediatrics Department and laboratory at Bonzola General Referral Hospital in Mbujimayi on 31 cases of meningitis aged from 1 month to 12 years. Results The prevalence of meningitis was 7.93%. The average age was 64 months with a sex ratio of 1.2 in favor of boys. Parents were of average socioeconomic status in 58.06% of cases. The clinical picture was dominated by fever and convulsions followed by consciousness disorders. The most frequently isolated bacterium was Streptococcus pneumoniae (74,19%) followed by Haemophilus influenzae (19,36%). Both showed high sensitivity to cephalosporins of 3rd generation and resistance to penicillins. Cefotaxime combined with other antibiotics or alone gave a cure rate between 58% and 78%. Overall mortality was high at 32,26% of cases. Conclusion The bacterial meningitis of the child is mainly due to Streptococcus pneumoniae, but also to Haemophilus influenzae. These germs are very sensitive to cephalosporins of 3rd generation and most often resistant to penicillins. Mortality is important around 32%. Because CFTR gene studies now represent one of the most frequent genetic analyses routinely performed worldwide, the number of rare CFTR variants identified in various clinical situations, regularly increases. To provide appropriate diagnosis and prognosis to CF patients as well as appropriate genetic counseling to families, the clinical impact and the phenotypic spectrum of variants identified by diagnostic techniques need to be characterized. Three complementary locus specific databases, called CFTR1, CFTR2 and CFTR -France were developed to address these issues. Besides, the growing knowledge of the CF pathophysiology and the technical evolution in molecular biology allowed to identify candidate modifier genes, regulatory loci, epigenetic profiles and trans -regulators that could help to refine genotype-phenotype correlations at the individual level. These different factors may contribute to the large phenotypic variability between patients with CF, even when they carry identical CFTR variants, regarding lung function, meconium ileus susceptibility or the risk for developing CFTR-related diabetes and liver disease. Finally, the availability of new therapies that target the CFTR protein for numbers of CF patients led to the identification of 'good' and 'poor' responders, thus raising questions of pharmacogenetics factors that may influence treatment efficiency as a novel feature of the complexity of CF patients' management. © 2020 French Society of Pediatrics. Foundation: The study of dengue as a clinical and epidemiological entity is becoming increasingly important given the tendency to increase the number of cases and the rate of reported complications presented by patients suffering from it. Osbjective: to characterize the clinical-epidemiological evolution of patients diagnosed with dengue. Methods: a retrospective descriptive study of a series of cases was carried out at the General Hospital of Cienfuegos, in the period January-June 2017. All patients with a confirmed diagnosis of dengue by IgM (N = 83) were included. Results: the average age was 42 years. The most represented age group was 40-49 years (28.9%). Patients with white skin color (74.7%), and female sex (59.0%) predominated. The municipality with the most cases was Cienfuegos. Fever was the main reason for consultation (96.4%), followed by headache (78.3%) and retroocular pain (64.4%). 36.1% of the cases presented an alarm sign. The most frequent paraclinical finding was lymphocyte predominance (95.2%). No ultrasonographic abnormalities were reported. Conclusion: all the patients studied presented a favorable evolution. Less than half of them presented some warning sign, and none developed any of the so-called serious forms of the disease. [ABSTRACT FROM AUTHOR]
