Abstract In many real-life contexts, observers are required to search for targets that are rarely present (e.g. tumours in X-rays; dangerous items in airport security screenings). Despite the rarity of these items, they are of enormous importance for the health and safety of the public, yet they are easily missed during visual search. This is referred to as the prevalence effect. In the current series of experiments, we investigate whether unequal reward can modulate the prevalence effect, in a multiple target search task. Having first established the impact of prevalence (Experiment 1) and reward (Experiment 2) on how efficiently participants can find one of several targets in the current paradigm, we then combined the two forms of priority to investigate their interaction. An unequal reward distribution (where lower prevalence items are more rewarded; Experiment 3) was found to diminish the effect of prevalence, compared to an equal reward distribution (Experiment 4) as indicated by faster response times and fewer misses. These findings suggest that when combined with an unequal reward distribution, the low prevalence effect can be diminished.

Abstract Human memory is malleable by both social and motivational factors and holds information relevant to workplace decisions. Retrieval-induced forgetting (RIF) describes a phenomenon where retrieval practice impairs subsequent memory for related (unpracticed) information. We report two RIF experiments. Chinese participants received a mild self-threat manipulation (Experiment 2) or not (Experiment 1) before an ethnicity-RIF task that involved practicing negative traits of either in-group (Chinese) or an out-group (Japanese) target. After a subsequent memory test, participants selected their preferred applicant for employment. RIF scores correspond to forgetting of unpracticed positive traits of one target (Rp−) relative to the recall of practiced negative traits of the other target (Rp+). Enhanced forgetting of positive traits was found in both experiments for both targets. Across experiments, a significant target by threat interaction showed that target ethnicity modified RIF (an ethnicity-RIF effect). Inducing a self-protecting motivation enhanced RIF effects for the out-group (Japanese) target. In a subsequent employment decision, there was a strong bias to select the in-group target, with the confidence in these decisions being associated with RIF scores. This study suggests that rehearsing negative traits of minority applicants can affect metacognitive aspects of employment decisions, possibly by shaping the schemas available to the majority (in-group) employer. To disrupt systemic racism, recruitment practices should aim to offset a human motivation to protect one-self, when exposed to a relatively mild threat to self-esteem. Discussing the negative traits of minority applicants is a critical, and sensitive, aspect of decision-making that warrants careful practice. These data suggest that recruiting individuals should be reminded of their personal strengths in this context, not their vulnerabilities, to secure their decision-making for fairer recruitment practice.

Abstract Visual search in dynamic environments, for example lifeguarding or CCTV monitoring, has several fundamentally different properties to standard visual search tasks. The visual environment is constantly moving, a range of items could become targets and the task is to search for a certain event. We developed a novel task in which participants were required to search static and moving displays for an orientation change thus capturing components of visual search, multiple object tracking and change detection paradigms. In Experiment 1, we found that the addition of moving distractors slowed participants’ response time to detect an orientation changes in a moving target, showing that the motion of distractors disrupts the rapid detection of orientation changes in a moving target. In Experiment 2 we found that, in displays of both moving and static objects, response time was slower if a moving object underwent a change than if a static object did, thus demonstrating that motion of the target itself also disrupts the detection of an orientation change. Our results could have implications for training in real-world occupations where the task is to search a dynamic environment for a critical event. Moreover, we add to the literature highlighting the need to develop lab-based tasks with high experimental control from any real-world tasks researchers may wish to investigate rather than extrapolating from static visual search tasks to more dynamic environments.

Anxiety disorders affect approximately one third of people during their lifetimes and are the ninth leading cause of global disability. Current treatments focus on therapy and pharmacological interventions. However, therapy is costly and pharmacological interventions often have undesirable side-effects. Healthy people also regularly suffer periods of anxiety. Therefore, a non-pharmacological, intuitive, home intervention would be complementary to other treatments and beneficial for non-clinical groups. Existing at-home anxiety aids, such as guided meditations, typically employ visual and/or audio stimuli to guide the user into a calmer state. However, the tactile sense has the potential to be a more natural modality to target in an anxiety-calming device. The tactile domain is relatively under-explored, but we suggest that there are manifold physiological and affective qualities of touch that lend it to the task. In this study we demonstrate that haptic technology can offer an enjoyable, effective and widely accessible alternative for easing state anxiety. We describe a novel huggable haptic interface that pneumatically simulates slow breathing. We discuss the development of this interface through a focus group evaluating five prototypes with embedded behaviours (‘breathing’, ‘purring’, ‘heartbeat’ and ‘illumination’). Ratings indicated that the ‘breathing’ prototype was most pleasant to interact with and participants described this prototype as ‘calming’ and ‘soothing’, reminding them of a person breathing. This prototype was developed into an ergonomic huggable cushion containing a pneumatic chamber powered by an external pump allowing the cushion to ‘breathe’. A mixed-design experiment (n = 129) inducing anxiety through a group mathematics test found that the device was effective at reducing pre-test anxiety compared to a control (no intervention) condition and that this reduction in anxiety was indistinguishable from that of a guided meditation. Our findings highlight the efficacy of this interface, demonstrating that haptic technologies can be effective at easing anxiety. We suggest that the field should be explored in more depth to capture the nuances of different modalities in relation to specific situations and trait characteristics.

Background: Acute skin toxicity is a common and usually transient side-effect of breast radiotherapy although, if sufficiently severe, it can affect breast cosmesis, aftercare costs and the patient's quality-of-life. The aim of this study was to develop predictive models for acute skin toxicity using published risk factors and externally validate the models in patients recruited into the prospective multi-center REQUITE (validating pREdictive models and biomarkers of radiotherapy toxicity to reduce side-effects and improve QUalITy of lifE in cancer survivors) study.Methods: Patient and treatment-related risk factors significantly associated with acute breast radiation toxicity on multivariate analysis were identified in the literature. These predictors were used to develop risk models for acute erythema and acute desquamation (skin loss) in three Radiogenomics Consortium cohorts of patients treated by breast-conserving surgery and whole breast external beam radiotherapy (n = 2,031). The models were externally validated in the REQUITE breast cancer cohort (n = 2,057).Results: The final risk model for acute erythema included BMI, breast size, hypo-fractionation, boost, tamoxifen use and smoking status. This model was validated in REQUITE with moderate discrimination (AUC 0.65), calibration and agreement between predicted and observed toxicity (Brier score 0.17). The risk model for acute desquamation, excluding the predictor tamoxifen use, failed to validate in the REQUITE cohort.Conclusions: While most published prediction research in the field has focused on model development, this study reports successful external validation of a predictive model using clinical risk factors for acute erythema following radiotherapy after breast-conserving surgery. This model retained discriminatory power but will benefit from further re-calibration. A similar model to predict acute desquamation failed to validate in the REQUITE cohort. Future improvements and more accurate predictions are expected through the addition of genetic markers and application of other modeling and machine learning techniques.

Abstract Brain tumour detection and diagnosis requires clinicians to inspect and analyse brain magnetic resonance images. Eye-tracking is commonly used to examine observers’ gaze behaviour during such medical image interpretation tasks, but analysis of eye movement sequences is limited. We therefore used ScanMatch, a novel technique that compares saccadic eye movement sequences, to examine the effect of expertise and diagnosis on the similarity of scanning patterns. Diagnostic accuracy was also recorded. Thirty-five participants were classified as Novices, Medics and Experts based on their level of expertise. Participants completed two brain tumour detection tasks. The first was a whole-brain task, which consisted of 60 consecutively presented slices from one patient; the second was an independent-slice detection task, which consisted of 32 independent slices from five different patients. Experts displayed the highest accuracy and sensitivity followed by Medics and then Novices in the independent-slice task. Experts showed the highest level of scanning pattern similarity, with medics engaging in the least similar scanning patterns, for both the whole-brain and independent-slice task. In the independent-slice task, scanning patterns were the least similar for false negatives across all expertise levels and most similar for experts when they responded correctly. These results demonstrate the value of using ScanMatch in the medical image perception literature. Future research adopting this tool could, for example, identify cases that yield low scanning similarity and so provide insight into why diagnostic errors occur and ultimately help in training radiologists.

We present and evaluate the concept of FeelMusic and evaluate an implementation of it. It is an augmentation of music through the haptic translation of core musical elements. Music and touch are intrinsic modes of affective communication that are physically sensed. By projecting musical features such as rhythm and melody into the haptic domain, we can explore and enrich this embodied sensation; hence, we investigated audio-tactile mappings that successfully render emotive qualities. We began by investigating the affective qualities of vibrotactile stimuli through a psychophysical study with 20 participants using the circumplex model of affect. We found positive correlations between vibration frequency and arousal across participants, but correlations with valence were specific to the individual. We then developed novel FeelMusic mappings by translating key features of music samples and implementing them with “Pump-and-Vibe”, a wearable interface utilising fluidic actuation and vibration to generate dynamic haptic sensations. We conducted a preliminary investigation to evaluate the FeelMusic mappings by gathering 20 participants’ responses to the musical, tactile and combined stimuli, using valence ratings and descriptive words from Hevner’s adjective circle to measure affect. These mappings, and new tactile compositions, validated that FeelMusic interfaces have the potential to enrich musical experiences and be a means of affective communication in their own right. FeelMusic is a tangible realisation of the expression “feel the music”, enriching our musical experiences.

Magnetic resonance imaging (MRI) is a crucial tool for clinical brain tumor detection and delineation. Since the process of gross tumor volume delineation resides with clinicians, a better understanding of how they perform this task is required if improvements in life expectancy are to be made. Novice-expert comparison studies have been used to examine the effect of expertise on abnormality detection, but little research has investigated expertise-related differences in brain tumor delineation. In this study, undergraduate students (novices) and radiologists (experts) inspected a combination of T1 and T2 single and whole brain MRI scans, each containing a tumor. Using a tablet and stylus to provide an interactive environment, participants had an unlimited amount of time to scroll freely through the MRI slices and were instructed to delineate (i.e., draw a boundary) around any tumorous tissue. There was no reliable evidence for a difference in the gross tumor volume or total number of slices delineated between experts and novices. Agreement was low across both expertise groups and significantly lower at peripheral locations within a tumor than central locations. There was an interaction between expertise level and location within a tumor with experts displaying higher agreement at the peripheral slices than novices. An effect of brain image set on the order in which participants inspected the slices was also observed. The implications of these results for the training undertaken by early career radiologists and current practices in hospitals are discussed.

Although the cascading impact of predators depends critically on the relative role of lethal predation and predation risk, we lack an understanding of how human-caused stressors may shift this balance. Emergent evidence suggests that pollution may increase the importance of predator consumptive effects by weakening the effects of fear perceived by prey. However, this oversimplification ignores the possibility that pollution may also alter predator consumptive effects. In particular, contaminants may impair the consumptive effects of predators by altering density-dependent interactions among prey conspecifics. No study has directly compared predator consumptive and non-consumptive effects in polluted versus non-polluted settings. We addressed this issue by using laboratory mesocosms to examine the impact of sublethal doses of copper on tri-trophic interactions among estuarine predator crabs Cancer productus, carnivorous whelk prey Urosalpinx cinerea, and the basal resource barnacles Balanus glandula. We investigated crab consumptive effects (whelks culled without crab chemical cues), non-consumptive effects (whelks not culled with crab chemical cues), and total effects (whelks culled with crab chemical cues) on whelks in copper polluted and non-polluted waters. Realistic copper concentrations suppressed the effects of simulated crab lethal predation (whelk culling) by removing density-dependent feeding by whelks. Specifically, reductions in conspecific density occurring in elevated copper levels did not trigger the normal increase in whelk consumption rates of barnacles. Weakened effects of fear were only observed at extremely high copper levels, suggesting consumptive effects were more sensitive to pollution. Thus, pollution may shape communities by altering the roles of predators and interactions among prey.

Abstract Introduction Transversus abdominis plane (TAP) blocks are increasingly popular to manage postoperative pain. Because the target structure of the block is the neuro-fascial plane between the internal oblique muscle (IOM) and the transverse abdominis muscle rather than a single neural structure, this block is ideal to introduce novices to ultrasound-guided regional anesthesia. Methods This simulation-based introduction boot camp to ultrasound-guided regional anesthesia has been made mandatory for all anesthesia residents in our large North American academic institution. This resource provides learners the opportunity to learn the technical skills associated with ultrasound-guided needle placement, abdominal wall anatomy (i.e., muscles, nerves, vessels, peritoneum, and colon), and how to safely perform a bilateral ultrasound guided TAP block. Results Since the implementation of this resource, the author have observed that attending physicians appreciate that new residents have had a chance to practice this skill in a simulation lab. Simultaneously, residents greatly appreciate the structured introduction into regional anesthesia in the relaxed setting of the simulation lab, devoid of time constraints. Discussion TAP blocks are an ideal procedure for introducing anesthesiology residents to ultrasound-guided regional anesthesia during boot camp.

The increasing ubiquity of haptic displays (e.g., smart phones and tablets) necessitates a better understanding of the perceptual capabilities of the human haptic system. Haptic displays will soon be capable of locally deforming to create simple 3D shapes. This study investigated the sensitivity of our haptic system to a fundamental component of shapes: edges. A novel set of eight high quality shape stimuli with test edges that varied in sharpness were fabricated in a 3D printer. In a two alternative, forced choice task, blindfolded participants were presented with two of these shapes side by side (one the reference, the other selected randomly from the remaining set of seven) and after actively exploring the test edge of each shape with the tip of their index finger, reported which shape had the sharper edge. We used a model selection approach to fit optimal psychometric functions to performance data, and from these obtained just noticeable differences and Weber fractions. In Experiment 1, participants performed the task with four different references. With sharpness defined as the angle at which one surface meets the horizontal plane, the four JNDs closely followed Weber's Law, giving a Weber fraction of 0.11. Comparisons to previously reported Weber fractions from other haptic manipulations (e.g. amplitude of vibration) suggests we are sufficiently sensitive to changes in edge sharpness for this to be of potential utility in the design of future haptic displays. In Experiment 2, two groups of participants performed the task with a single reference but different exploration strategies; one was limited to a single touch, the other unconstrained and free to explore as they wished. As predicted, the JND in the free exploration condition was lower than that in the single touch condition, indicating exploration strategy affects sensitivity to edge sharpness.

Background: Use of an in situ epidural catheter has been suggested to be efficient to provide anesthesia for postpartum tubal ligation (PPTL). Reported epidural reactivation success rates vary from 74% to 92%. Predictors for reactivation failure include poor patient satisfaction with labor analgesia, increased delivery-to-reactivation time and the need for top-ups during labor. Some have suggested that this high failure rate precludes leaving the catheter in situ after delivery for subsequent reactivation attempts. In this study, we sought to evaluate the success rate of neuraxial techniques for PPTL and to determine if predictors of failure can be identified. Methods: After obtaining IRB approval, a retrospective chart review of patients undergoing PPTL after vaginal delivery from July 2010 to July 2016 was conducted using CPT codes, yielding 93 records for analysis. Demographic, obstetric and anesthetic data (labor analgesia administration, length of epidural catheter in epidural space, top-up requirements, time of catheter reactivation, final anesthetic technique and corresponding doses for spinal and epidural anesthesia) were obtained. Results: A total of 70 patients received labor neuraxial analgesia. Reactivation was attempted in 33 with a success rate of 66.7%. Patient height, epidural volume of local anesthetic and administered fentanyl dose were lower in the group that failed reactivation. Overall, spinal anesthesia was performed in 60 patients, with a success rate of 80%. Conclusions: Our observed rate of successful postpartum epidural reactivation for tubal ligation was lower than the range reported in the literature. Our success rates for both spinal anesthesia and epidural reactivation for PPTL were lower than the generally accepted rates of successful epidural and spinal anesthesia for cesarean delivery. This gap may reflect a lower level of motivation on behalf of both the patients and anesthesia providers to tolerate “imperfect” neuraxial anesthesia once fetal considerations are removed.
