Abstract The role for routine whole genome and transcriptome analysis (WGTA) for poor prognosis pediatric cancers remains undetermined. Here, we characterize somatic mutations, structural rearrangements, copy number variants, gene expression, immuno-profiles and germline cancer predisposition variants in children and adolescents with relapsed, refractory or poor prognosis malignancies who underwent somatic WGTA and matched germline sequencing. Seventy-nine participants with a median age at enrollment of 8.8 y (range 6 months to 21.2 y) are included. Germline pathogenic/likely pathogenic variants are identified in 12% of participants, of which 60% were not known prior. Therapeutically actionable variants are identified by targeted gene report and whole genome in 32% and 62% of participants, respectively, and increase to 96% after integrating transcriptome analyses. Thirty-two molecularly informed therapies are pursued in 28 participants with 54% achieving a clinical benefit rate; objective response or stable disease ≥6 months. Integrated WGTA identifies therapeutically actionable variants in almost all tumors and are directly translatable to clinical care of children with poor prognosis cancers.

This paper draws on three recent case studies which have shown encouraging economic results in meeting net zero carbon emissions targets through Smart Local Energy Systems (SLES) in different localities across England, based on the GreenSCIES model. The blueprint for the GreenSCIES SLES was developed in Islington, London, and consists of a 5th generation ambient loop district heat network using wasteheat from a large data centre, and includes electric vehicle charging, storage and solar PV. This allows for heat sharing between buildings and applications for heat recovery from local sources. In Sandwell, Birmingham, where the heating demand is nearly ten times higher than the cooling demand the design is based on a 4th generation heat network with wasteheat from a local metal foundry and some cooling supply and heat storage in the aquifer. A study in Barnsley, Sheffield, explored wasteheat from a local glass factory and considered mine workings for providing seasonal storage of heat for resilience to a single source industry. This scheme could be further expanded, increasing the available carbon emissions savings. These examples provide feasible and economic SLES projects demonstrating the importance of collaboration between researchers, local government and industry. They also illustrate adaptations of the original GreenSCIES concept providing pathways towards net zero carbon for a diverse range of urban locations with different housing density and waste heat sources.

Abstract In this study, we evaluate the impact of whole genome and transcriptome analysis (WGTA) on predictive molecular profiling and histologic diagnosis in a cohort of advanced malignancies. WGTA was used to generate reports including molecular alterations and site/tissue of origin prediction. Two reviewers analyzed genomic reports, clinical history, and tumor pathology. We used National Comprehensive Cancer Network (NCCN) consensus guidelines, Food and Drug Administration (FDA) approvals, and provincially reimbursed treatments to define genomic biomarkers associated with approved targeted therapeutic options (TTOs). Tumor tissue/site of origin was reassessed for most cases using genomic analysis, including a machine learning algorithm (Supervised Cancer Origin Prediction Using Expression [SCOPE]) trained on The Cancer Genome Atlas data. WGTA was performed on 652 cases, including a range of primary tumor types/tumor sites and 15 malignant tumors of uncertain histogenesis (MTUH). At the time WGTA was performed, alterations associated with an approved TTO were identified in 39 (6%) cases; 3 of these were not identified through routine pathology workup. In seven (1%) cases, the pathology workup either failed, was not performed, or gave a different result from the WGTA. Approved TTOs identified by WGTA increased to 103 (16%) when applying 2021 guidelines. The histopathologic diagnosis was reviewed in 389 cases and agreed with the diagnostic consensus after WGTA in 94% of non‐MTUH cases (n = 374). The remainder included situations where the morphologic diagnosis was changed based on WGTA and clinical data (0.5%), or where the WGTA was non‐contributory (5%). The 15 MTUH were all diagnosed as specific tumor types by WGTA. Tumor board reviews including WGTA agreed with almost all initial predictive molecular profile and histopathologic diagnoses. WGTA was a powerful tool to assign site/tissue of origin in MTUH. Current efforts focus on improving therapeutic predictive power and decreasing cost to enhance use of WGTA data as a routine clinical test.

Abstract This is the first report of a NACC2‐NTRK2 fusion in a histological glioblastoma. Oncogenomic analysis revealed this actionable fusion oncogene in a pediatric cerebellar glioblastoma, which would not have been identified through routine diagnostics, demonstrating the value of clinical genome profiling in cancer care.

In this study, we document the coordination of eye gaze and manual signing in a local sign language from Nebaj, Guatemala. We analyze gaze patterns in two conversations in which signers described the book Frog Where Are You to an interlocutor. The signers include a deaf child who narrated the book to a hearing interlocutor and her grandfather, who is also deaf, as he described the same book to his hearing grandson during a separate conversation. We code the two narratives for gaze target and sign type, analyzing the relationship between eye gaze and sign type as well as describing patterns in the sequencing of eye gaze targets. Both signers show a strong correlation between sign type and the direction of their eye gaze. As in previous literature, signers looked to a specialized medial space while producing signs that enact the action of characters in discourse in contrast to eye gaze patterns for non-enacting signs. Our analysis highlights both pragmatic–interactional and discursive–narrative functions of gaze. The pragmatic–interactional use of gaze primarily relates to the management of visual attention and turn-taking, while the discursive–narrative use of gaze marks the distinction between narrator and character perspective within stretches of narration. For both children and adults, communicating with each other effectively depends on having enough knowledge about particular entities, actions, or relations to understand and produce the words being used. Speakers draw on conventional meanings shared with their interlocutors, but do they share every detail of word meaning? They need not have identical, or fully specified, representations for the meanings of all the terms they make use of. Rather, they need only have represented enough about the meanings of the words used by another speaker to understand what is intended in context on a particular occasion. Reliance on partial meanings is common in both children and adults. More detailed, shared, representations of word meanings for a domain depend on acquiring additional knowledge about that domain and its contents. [ABSTRACT FROM AUTHOR]

A hedonic contrast effect occurs when comparing a stimulus to its alternatives makes it better or worse. We find that counterfactual comparisons induce larger hedonic contrast effects when they are also social comparisons. Hedonic contrast effects influence happiness with a food or wage more when another person receives its counterfactual alternative than when no person receives its counterfactual alternative. Social attention, the propensity to attend to the experiences of other people, underlies the larger hedonic contrast effects induced by social comparisons. People pay more attention to counterfactual alternatives when they are also social comparison standards, and this difference in the allocation of attention mediates the larger hedonic contrast effects that social counterfactual comparisons induce. Reducing attentional resources with cognitive load or time pressure reduces the impact of social counterfactual comparisons, and drawing attention to nonsocial counterfactual comparisons increases their impact. Diffuse intrinsic pontine glioma (DIPG) is a devastating pediatric brain cancer with no effective therapy. Histological similarity of DIPG to supratentorial high-grade astrocytomas of adults has led to assumptions that these entities possess similar underlying molecular properties and therefore similar therapeutic responses to standard therapies. The failure of all clinical trials in the last 30years to improve DIPG patient outcome has suggested otherwise. Recent studies employing next-generation sequencing and microarray technologies have provided a breadth of evidence highlighting the unique molecular genetics and epigenetics of this cancer, distinguishing it from both adult and pediatric cerebral high-grade astrocytomas. This review describes the most common molecular genetic and epigenetic signatures of DIPG in the context of molecular subgroups and histopathological diagnosis, including this tumor entity's unique mutational landscape, copy number alterations, and structural variants, as well as epigenetic changes on the global DNA and histone levels. Diffuse intrinsic pontine glioma (DIPG) is the main cause of brain tumour-related death in children. In the majority of cases diagnosis is based on clinical and MRI findings, resulting in the scarcity of pre-treatment specimens available to study. Our group has developed an autopsy-based protocol to investigate the histologic and biologic spectrum of DIPG. This has also allowed us to investigate the terminal pattern of disease and gain a better understanding of what challenges we are facing in treating DIPG. Here, we review 72 DIPG cases with well documented clinical history and molecular data and describe the pathological features of this disease in relation to clinical and genetic features. Fifty-three of the samples were autopsy material (7 pre-treatment) and 19 were pre-treatment biopsy/surgical specimens. Upon histological review, 62 patients had high-grade astrocytomas (18 WHO grade III and 44 WHO grade IV patients), 8 had WHO grade II astrocytomas, and 2 had features of primitive neuroectodermal tumour (PNET). K27M-H3 mutations were exclusively found in tumours with WHO grade II-IV astrocytoma histology. K27M-H3.1 and ACVR1 mutations as well as ALT phenotype were only found in WHO grade III-IV astrocytomas, while PIK3CA mutations and PDGFRA gains/amplifications were found in WHO grade II-IV astrocytomas. Approximately 1/3 of DIPG patients had leptomeningeal spread of their tumour. Further, diffuse invasion of the brainstem, spinal cord and thalamus was common with some cases showing spread as distant as the frontal lobes. These findings suggest that focal radiation may be inadequate for some of these patients. Importantly, we show that clinically classic DIPGs represent a diverse histologic spectrum, including multiple cases which would fit WHO criteria of grade II astrocytoma which nevertheless behave clinically as high-grade astrocytomas and harbour the histone K27M-H3.3 mutation. This suggests that the current WHO astrocytoma grading scheme may not appropriately predict outcome for paediatric brainstem gliomas. [ABSTRACT FROM AUTHOR]

Introduction: Some investigators have suggested that medulloblastomas can be distinguished from other cerebellar neoplasms by demonstrating 'restricted diffusion' on the Apparent Diffusion Coefficient (ADC) map obtained from diffusion-weighted imaging (DWI) sequences on magnetic resonance imaging. Previous authors have postulated that this observed restricted diffusion is a reflection of very high cell density. There has been a tendency to assert that pediatric medulloblastoma uniformly demonstrates restricted diffusion on DWI. However, our clinical observation has been that there are pediatric medulloblastomas that exhibit normal or even increased diffusion on DWI. The current study was undertaken primarily to determine whether restricted diffusion is uniformly present in pediatric medulloblastoma and secondly to look for pathological features that might distinguish medulloblastomas with and without restricted diffusion. Methods: The DWI characteristics of pathologically confirmed medulloblastomas diagnosed at our institution were retrospectively reviewed. The ADC was obtained in two non-overlapping, solid, non-hemorrhagic, non-necrotic regions of tumor and averaged. An ADC below 1 × 10 mm/s was considered to represent restricted diffusion. A detailed pathologic review of each tumor was conducted. Results: Ten cases of medulloblastoma were reviewed, of which two demonstrated average ADCs above 1 × 10 mm/s (1.223 and 1.169 × 10 mm/s, respectively), indicating no restricted diffusion. Pathologic review revealed that both of these non-restricting cases displayed a lack of reticulin deposition by light microscopy. Conclusion: DWI does not appear to be an entirely reliable means of distinguishing medulloblastomas from other cerebellar neoplasms. Myotonic dystrophy type 2 (DM2) is a multisystem degenerative disorder with distinctive clinical and electrophysiological features. Recently, genetic confirmation has become available with the identification of the molecular defect, an expansion of a CCTG repeat located in intron 1 of the zinc finger protein 9 ( ZNF9) gene. We present two first-degree relatives with an athletic clinical phenotype, pathological evidence of subsarcolemmal vacuolation, and molecular genetic confirmation of DM2. When found in the proper clinical context, athleticism and pathological subsarcolemmal vacuoles should not dissuade the clinician from the possible diagnosis of DM2.
