Recently, baby healthcare monitoring systems have emerged as a key research area. However, numerous research studies have been conducted on babies and their state of either awakening or sleeping. But there is a lack of comprehensive and systematic review in this area of research. The existing research considers mostly the children's monitoring vital health parameters, in turn, giving relaxation to the parents in caring of their babies. Still, the fundamental practical issues related to daily life are missing. As a result, the safety of babies against suffocation and falling hazards for employed parents or mothers involved in the workforce has been overlooked. Only a few studies exist that cover the age range of 0 months to 4 years, with further focus on the age range of 6 months to 4 years. In response to these gaps in previous research and existing technologies, this review work proposes a vision for a baby safety system that utilizes various sensors and technologies like machine learning and image processing to ensure a safer environment for babies. This paper proposes an analytical model to investigate the performance of periodical broadcast of safety messages in vehicular ad hoc networks (VANETs), regarding a hybrid medium access control (MAC) protocol, called NC-PNC MAC. We first examine the percent of vehicles involved in subcarrier collisions during a random orthogonal frequency-division multiple access scheme, and then theoretically analyze the total number of transmissions and the packet delivery ratio (PDR) during one dissemination period. Based upon the analysis, this paper not only reveals how the subcarrier collision probability influences the PDR performance, but mathematically obtains the maximum PDR. To confirm the results, by adjusting a pivotal parameter of the MAC protocol, the PDR performance for basic safety message broadcast in VANETs can be enhanced by 10% to 30% under the constraint of the delivery latency. Background: This case reports the synchronous diagnosis of two rare unrelated diseases; leiomyosarcoma and tenosynovial giant cell tumor of the knee. It focuses on the challenges of diagnosing tenosynovial giant cell tumor, including cognitive biases in clinical medicine that delay diagnosis. It also demonstrates the pathogenic etiology of tenosynovial giant cell tumor, evidenced by the transient deterioration of the patients' knee symptoms following the administration of prophylactic granulocyte colony-stimulating factor given as part of the chemotherapeutic regime for leiomyosarcoma. Case presentation: A 37-year-old Caucasian man presented with a left groin lump and left knee pain with swelling and locking. Investigations including positron emission tomography-computed tomography and biopsy revealed leiomyosarcoma in a lymph node likely related to the spermatic cord, with high-grade uptake in the left knee that was presumed to be the primary site. His knee symptoms temporarily worsened each time granulocyte colony-stimulating factor was administered with each cycle of chemotherapy for leiomyosarcoma to help combat myelosuppressive toxicity. Subsequent magnetic resonance imaging and biopsy of the knee confirmed a tenosynovial giant cell tumor. His knee symptoms relating to the tenosynovial giant cell tumor improved following the completion of his leiomyosarcoma treatment. Conclusions: Tenosynovial giant cell tumor remains a diagnostic challenge. We discuss the key clinical features and investigations that aid prompt diagnosis. The National Comprehensive Cancer Network clinical practice guidelines for soft tissue sarcoma have recently been updated to include the pharmacological management of tenosynovial giant cell tumor. Our case discussion provides an up-to-date review of the evidence for optimal management of patients with tenosynovial giant cell tumor, with a particular focus on novel pharmacological options that exploit underlying pathogenesis. Due to the broadcast nature of wireless channels, overhearing plays an important role in interflow network coding in that it serves as a way to acquire the side information for the decoding of network-coded packets. Since overhearing only takes effect in the one-hop neighborhood of nodes, it, in turn, restricts the identification of network coding opportunities. That is, if the side information for packet decoding cannot be overheard, no coding opportunity can be identified. Hence, in this paper, we aim to relax such a restriction to further explore network coding opportunities. Specifically, we propose a virtual overhearing (VOH) technique to enable network coding when the side information cannot be obtained from traditional overhearing previously mentioned. VOH allows a node to obtain a packet sent by another node that can be two or more hops away. As a result, VOH can serve as a supplementary way for side information acquirement, thus still enabling network coding between network flows when traditional overhearing is not possible to obtain the needed side information. The additional coding opportunities brought by VOH can be translated into additional performance gains. We first analyze network coding with VOH in a simple topology and show that with VOH, network capacity could be improved by over 12% (in some situations, network throughput could be improved by 50%). SUMMARY This paper proposes a cooperative quadrature physical layer network coding (CQPNC) scheme for a dual-hop cooperative relay network, which consists of two source nodes, one relay node and one destination node. All nodes in the network have one antenna, and the two source nodes transmit their signals modulated with quadrature carriers. In this paper, a cooperative quadrature physical layer network coded decode-and-forward (DF) relay protocol (CQPNC-DF) is proposed to transmit the composite information from the two source nodes via the relay node to the destination node simultaneously to reduce the number of time slots required for a transmission. The proposed CQPNC-DF relay protocol is compared with time-division multiple-access amplify-and-forward (TDMA-AF), TDMA-DF, cooperative network coded DF (CNC-DF) and cooperative analog network coded AF (CANC-AF) relay protocols to demonstrate its effectiveness in terms of bit error rate (BER) and system throughput under different propagation conditions. The simulation results reveal that the proposed CQPNC-DF relay protocol can significantly improve the network performance. Compared with two TDMA schemes and CNC-DF, the proposal can provide up to 100% and 50% throughput gains, respectively. SUMMARY This paper proposes a new physical-layer network coding (PNC) scheme, named combined orthogonal PNC (COPNC), for fading two-way relay channels. The scheme is based on orthogonal PNC (OPNC). In the scheme, the two source nodes employ orthogonal carriers, and the relay node makes an orthogonal combining of the two information bits rather than exclusive or (XOR), which is employed in most PNC schemes. The paper also analyzes the bit error rate (BER) performance of PNC, OPNC, and COPNC for Rayleigh fading model. Simulation results for Rayleigh and Nakagami- m fading channels show that COPNC can provide outstanding BER performance compared with PNC and OPNC, especially when the uplink channel conditions are asymmetric. The results in Nakagami- m channels also imply that COPNC will provide higher BER gain with more severe fading depth. Potential works about COPNC are also presented in this paper. This study assessed the acceptability and feasibility of a question prompt list (QPL) to facilitate informed treatment decision-making in men with suspected localised prostate cancer, which involves values-based choices between options with similar efficacy but different side effects. The QPL was developed through iterative consultation with consumers, clinicians and researchers. Acceptability was assessed using study-specific questions regarding QPL satisfaction and usefulness and qualitative interviews. Feasibility was determined via the proportion of men given the QPL according to medical records and the completion of standardised measures of decisional outcomes. Quantitative data were analysed using descriptive and univariate statistics. Qualitative data were thematically analysed. Fifty-two men consented; 34 provided data for analysis. The QPL recipients reported moderate–high content satisfaction (70.6%) and perceived usefulness in guiding appointments when receiving biopsy results (64.7%). Two main qualitative themes also indicated the QPL acceptability: (1) the freedom to ask—acceptable timing, flexible usage and usefulness of the QPL, and (2) satisfaction with the QPL content. However, only 18.4% of eligible men received the QPL, indicating limited feasibility. Multihop cellular network (MCN) has been proposed to incorporate the flexibility of ad hoc networks into traditional single-hop cellular networks (SCNs). The performance analysis of MCN through analytical models is not trivial because the classic Erlang B formula no longer applies to MCN where multihop transmission is allowed. In this paper, we first propose a clustered MCN (cMCN) architecture with the use of dedicated information ports (DIPs), which are deployed wireless ports functioning as central controllers for multihop users. The proposed cMCN can be considered as a complement of the existing cellular network. Then, we study the feasibility of modeling time division multiple access (TDMA)-based cMCN with fixed channel assignment (FCA) scheme for uplink transmission. An exact multi-dimensional Markov chain model to analyze the performance of cMCN with FCA is developed. Furthermore, an approximated model which results in reduced complexity is also presented. The analytical results from both models are matched with the simulation results closely. The results show that cMCN with the proposed FCA scheme can reduce the call blocking probability significantly as compared to SCNs with either the conventional FCA or a dynamic channel assignment (DCA) scheme. Background: There are no current standards of care guiding perioperative fluid administration, and clinicians continue to debate restrictive versus liberal fluid administration. We sought to simultaneously evaluate the opinions and practice of surgeons, intensivists and anesthesiologists in a single centre regarding perioperative fluid resuscitation. Methods: A postal survey sent to surgeons, intensivists and anesthesiologists in a single academic health care centre presented case-based scenarios followed by questions on fluid requirements and 5-point Likert scales involving statements about resuscitation. We performed a retrospective chart review to evaluate amount and type of intraand postoperative (72-h) fluid administration, urine output and postoperative ventilation time in patients undergoing uncomplicated esophagectomy or pneumonectomy. Results: Seventy-four of 77 respondents (96%) agreed that precise fluid resuscitation diminishes the risk of adverse events. Surgeons overall gave less fluids than anesthesiologists or intensivists and used fewer resuscitation end points to estimate fluid requirements perioperatively. For right hemicolectomies, only 3% of surgeons chose a fluid rate greater than 150 mL/h, compared with 55% of intensivists. We reviewed 49 patients' charts (25 who had pneumonectomies, 24 who had esophagectomies) retrospectively. The coefficient of variation (COV = standard deviation divided by its mean) of fluid administration intraoperatively was 0.56 for pneumonectomy and 0.35 for esophagectomy; postoperatively, the COV was greater than 0.50 for the first 24 hours, but less than 0.50 after 24 hours postoperatively. Conclusion: The presence of substantial variability of both opinion and practice of perioperative fluid resuscitation in a single centre supports the need for further research to identify objective methods to define perioperative fluid requirements and standards of perioperative resuscitation. Multihop TDD-CDMA cellular system becomes a promising candidate for fourth generation (4G) cellular systems because it provides wider coverage and higher throughput. In this paper, we analyze the uplink system capacity of a multihop TDD-CDMA cellular system. We first analyze a one-hop TDD-CDMA cellular system with different base station's (BS's) direct coverage area. We find that the system capacity can be improved if a BS only covers a center small area of the cell. Besides, the high downlink transmission power of the surrounding BSs can degrade the capacity severely. Thus, we propose a multihop TDD-CDMA cellular system to overcome the high downlink inter-cell interference. In our system, a cell area is divided into two regions: inner-region and outer-region. The mobile users in the inner- and outer-region of the cell communicate with BS and relay station (RS), respectively. Based on this model, the uplink system capacity in both regions with the assumption that the inter-cell and intra-cell interference follow the Gaussian distribution is obtained. A comparison equation to provide a fair comparison with conventional one-hop TDD-CDMA cellular system is presented. Objectives: We sought to determine whether serial intraoperative laryngeal electromyography (L-EMG) or evoked L-EMG predicts vocal fold (VF) recovery following iatrogenic injury. Methods: Six beagles were sedated, and bipolar needle electrodes were inserted into each thyroarytenoid (TA) muscle. Endotracheal tube surface electrodes were also placed. As the sedation lightened. L-EMG activity was recorded from all electrodes with an intraoperative nerve monitoring system. The neck was opened, and direct recurrent laryngeal nerve (RLN) stimulation was performed. Subsequently, 4 animals underwent crush injury of the left RLN, and 2 animals underwent nerve transection. The L-EMG procedures were repeated every 1 to 2 weeks until left VF motion was observed in the dogs that suffered RLN crush injury. At each time point, the neck was opened and both RLNs were stimulated. Results: Fibrillation potentials were detected in ail animals after RLN injury. A change to electrical silence was seen in the animals in the crush injury group that were evaluated during the week preceding VF recovery. Fibrillation potentials and VF immobility persisted in the transection group throughout the complete time course of these experiments. The first appearance of an evoked response coincided with occurred after the return of left VF motion in the crush injury group. The threshold, latency, and amplitude differed from those of the controls and approached normal values over time. No response was detected in the transected nerves. Conclusions: The disappearance of fibrillations on intraoperative L-EMG was noted in the animals tested the week before the return of VF motion, and the return of motor unit action potentials was seen along with return of VF function. Evoked L-EMG was not helpful in predicting the return of VF mobility, but it may help quantify degrees of RLN injury and predict the speed of recovery. [ABSTRACT FROM AUTHOR]

System capacity and grade of service (GoS) are both important for the rapid growth of cellular communication services. In this paper, we propose a two-tier TDMA-based cellular system with macrocell overlaid on microcell clusters by implementing fixed channel assignment (FCA) scheme and fixed reuse partitioning (FRP) scheme in microcell layer and macrocell layer, respectively, named FCA–FRP overlay scheme. Improvement can be achieved in both system capacity and GoS. Theoretical analysis based on the overlay scheme without overflow and with overflow is first presented. It shows that the simulation results are agreed with the analytical results. Then, simulation results, obtained from the overlay scheme with and without overflow, show that the performance in terms of the call blocking probability, the call dropping probability and system capacity of such a system can be greatly improved compared with a conventional one-tier cellular system deployed with FCA or FRP scheme. Summary: Objectives: The primary objective of this study was to determine whether a simplified technique for intraoperative laryngeal electromyography was feasible using standard nerve integrity monitoring electrodes and audiovisual digital recording equipment. Our secondary objective was to determine if laryngeal electromyography data provided any additional information that significantly influenced patient management. Methods: Between February 2006 and February 2007, 10 children referred to our institution with vocal fold immobility underwent intraoperative laryngeal electromyography of the thyroarytenoid muscles. A retrospective chart review of these 10 patients was performed after institutional review board approval. Results: Standard nerve integrity monitoring electrodes can be used to perform intraoperative laryngeal electromyography of the thyroarytenoid muscles in children. In 5 of 10 cases reviewed, data from laryngeal electromyography recordings meaningfully influenced the care of children with vocal fold immobility and affected clinical decision-making, sometimes altering management strategies. In the remaining 5 children, data supported clinical impressions but did not alter treatment plans. Two children with idiopathic bilateral vocal fold paralysis initially presented with a lack of electrical activity on one or both sides but went on to develop motor unit action potentials that preceded recovery of motion in both vocal folds. Conclusions: Our findings suggest that standard nerve monitoring equipment can be used to perform intraoperative laryngeal electromyography and that electromyographic data can assist clinicians in the management of complex patients. Additionally, there may be a role for the use of serial intraoperative measurements in predicting recovery from vocal fold paralysis in the pediatric age group. [Copyright &y& Elsevier]

The trend of the wireless communication system is to provide various types of services such as voice, data and video etc. Due to the limited radio resources with international agreement, how to achieve the optimum system capacity becomes a paramount issue. In this paper, we use the idea of channel partitioning (CP) employing different reuse factors to support multiple services that require different signal-to-interference ratios (SIRs) in cellular systems. Two types of services are considered in this paper. Thus, we use a large reuse factor to support high SIR required service while we use a small reuse factor to support low SIR required service. From the system point of view, the average reuse factor becomes smaller and the system capacity can be improved. The system performance of CP with fixed channel allocation (FCA) scheme, namely fixed channel partitioning (FCP), is first proposed and analysed using Markov chain in a single cell model. Then a dynamic channel allocation scheme with CP called dynamic channel partitioning with interference information (DCP-WI) is proposed and studied in the multiple-cell model by computer simulation. The analysis and simulation results show that our proposed schemes can improve the system capacity depending on the traffic load fraction for each service. For equal arrival rate for both services, FCP and DCP-WI provide about 33 and 60% capacity improvement respectively over a conventional FCA system using a single reuse factor to support two types of services. Reuse partitioning (RP) is a simple technique that can be used to increase the traffic capacity in a cellular system. With RP, a cell is divided into several concentric regions, each associated with a different cluster size. In this article, a traffic model is developed to analyze the impact of mobile users on a two-region RP system using fixed channel assignment. The influence of user speed and cell size on the new call blocking probability, P[sub b], and the call dropping probability, P[sub d], is investigated. A simpler model that can be used to estimate P[sub b] and P[sub d] in some cases is described. The effect on system capacity of reserving some channels for handoff calls is also studied. It is found that even though prioritized handoff can reduce P[sub d], it may also degrade the capacity. The accuracy of the analysis is verified using simulation for three mobility models. Unmanned aerial vehicle (UAV)-based data gathering from wireless sensor networks is one of the recent research topics that has currently attracted research interest. One of the challenges for the UAV-aided WSN data collection efforts is to design an energy-efficient UAV/drone communication with arbitrarily dispersed ground sensors by improving the ground network structure. This paper aims to develop a technique titled UAV Fuzzy Travel Path' that supports UAV smooth path design and enables ground network topology shifting. A comprehensive UAV-based data collection model is proposed to enable dynamic orchestration/re-orchestration of wireless ground sensors to jointly improve network performance and UAV path fluidity. This provides a more flexible ground network framework that can be restructured based on network demands and UAV optimal paths, effectively allowing for a software-defined network concept. The main contribution of this work is the implementation of the software-defined wireless sensor network on the ground network that adaptably supports the movement of the UAV and enhances the communication network’s energy efficiency with a proposed latency analytical analysis via network orchestration/re-orchestration phases. The main significance of this research is in offering a flexible span for UAV path design than being fixed in one strict route for data gathering purposes. Four various simulation tools are employed for modelling and performance evaluation, namely MATLAB, CupCarbon, Contiki-Cooja and Mission Planner. The proposed software-defined ground network system demonstrates encouraging results in terms of network performance metrics including energy consumption of UAV versus ground sensor nodes energy usage, packet delivery rate, and the communication time of the ground orchestrated or/and re-orchestrated network. A Hybrid LiFi and WiFi network (HLWNet) integrates the rapid data transmission capabilities of Light Fidelity (LiFi) with the extensive connectivity provided by Wireless Fidelity (WiFi), resulting in significant benefits for wireless data transmissions in the designated area. However, the challenge of decision-making during the handover process in HLWNet is made more complex due to the specific characteristics of electromagnetic signals' line-of-sight transmission, resulting in a greater level of intricacy compared to previous heterogeneous networks. This research work addresses the problem of handover decisions in the Hybrid LiFi and WiFi networks and treats it as a binary classification problem. Consequently, it proposes a handover method based on a deep neural network (DNN). The comprehensive handover scheme incorporates two sets of neural networks (ANN and DNN) that utilize input factors such as channel quality and the mobility of users to enable informed decisions during handovers. Following training with labeled datasets, the neural-network-based handover approach achieves an accuracy rate exceeding 95%. A comparative analysis of the proposed scheme against the benchmark reveals that the proposed method considerably increases user throughput by approximately 18.58% to 38.5% while reducing the handover rate by approximately 55.21% to 67.15% compared to the benchmark artificial neural network (ANN); moreover, the proposed method demonstrates robustness in the face of variations in user mobility and channel conditions. Smart cities require interactive management of water supply networks and water meters play an important role in such a task. As compared to fully mechanical water meters, electromechanical water meters or fully electronic water meters can collect real-time information through automatic meter reading (AMR), which makes them more suitable for smart cities applications. In this paper, we first study the design principles of existing water meters, and then present our design and implementation of a self-powered smart water meter. The proposed water meter is based on a water turbine generator, which serves for two purposes: (i) to sense the water flow through adaptive signal processing performed on the generated voltage; and (ii) to produce electricity to charge batteries for the smart meter to function properly. In particular, we present the design considerations and implementation details. The wireless transceiver is integrated in the proposed water meter so that it can provide real-time water flow information. This article describes the development and ocean forecast evaluation of an atmosphere–ocean coupled prediction system for the Maritime Continent (MC) domain, which includes the eastern Indian and western Pacific oceans. The coupled system comprises regional configurations of the atmospheric model MetUM and ocean model NEMO at a uniform horizontal resolution of 4.5 km × 4.5 km, coupled using the OASIS3-MCT libraries. The coupled model is run as a pre-operational forecast system from 1 to 31 October 2019. Hindcast simulations performed for the period 1 January 2014 to 30 September 2019, using the stand-alone ocean configuration, provided the initial condition to the coupled ocean model. This paper details the evaluations of ocean-only model hindcast and 6 d coupled ocean forecast simulations. Direct comparison of sea surface temperature (SST) and sea surface height (SSH) with analysis, as well as in situ observations, is performed for the ocean-only hindcast evaluation. For the evaluation of coupled ocean model, comparisons of ocean forecast for different forecast lead times with SST analysis and in situ observations of SSH, temperature, and salinity have been performed. Overall, the model forecast deviation of SST, SSH, and subsurface temperature and salinity fields relative to observation is within acceptable error limits of operational forecast models. Summary: In this paper, a novel reinforcement learning (RL) approach with cell sectoring is proposed to solve the channel and power allocation issue for a device‐to‐device (D2D)‐enabled cellular network when the prior traffic information is not known to the base station (BS). Further, this paper explores an optimal policy for resource and power allocation between users intending to maximize the sum‐rate of the overall system. Since the behavior of wireless channel and traffic request of users in the system is stochastic in nature, the dynamic property of the environment allows us to employ an actor‐critic RL technique to learn the best policy through continuous interaction with the surrounding. The proposed work comprises of four phases: cell splitting, clustering, queuing model, and channel allocation and power allocation simultaneously using an actor‐critic RL. The implementation of cell splitting with novel clustering technique increases the network coverage, reduces co‐channel cell interference, and minimizes the transmission power of nodes, whereas the queuing model solves the issue of waiting time for users in a priority‐based data transmission. With the help of continuous state‐action space, the actor‐critic RL algorithm based on policy gradient improves the overall system sum‐rate as well as the D2D throughput. The actor adopts a parameter‐based stochastic policy for giving continuous action while the critic estimates the policy and criticizes the actor for the action. This reduces the high variance of the policy gradient. Through numerical simulations, the benefit of our resource sharing scheme over other existing traditional scheme is verified. Background: Decision-making in the management of patients with retroperitoneal sarcoma is complex and requires input from a number of different specialists. The aim of this study was to evaluate the levels of agreement in terms of resectability, treatment allocation, and organs proposed to be resected across different retroperitoneal sarcoma multidisciplinary team meetings. Methods: The CT scans and clinical information of 21 anonymized retroperitoneal sarcoma patients were sent to all of the retroperitoneal sarcoma multidisciplinary team meetings in Great Britain, which were asked to give an opinion about resectability, treatment allocation, and organs proposed to be resected. The main outcome was inter-centre reliability, which was quantified using overall agreement, as well as the chance-corrected Krippendorff's alpha statistic. Based on the latter, the level of agreement was classified as: 'slight' (0.00-0.20), 'fair' (0.21-0.40), 'moderate' (0.41-0.60), 'substantial' (0.61-0.80), or 'near-perfect' (>0.80). Results: Twenty-one patients were reviewed at 12 retroperitoneal sarcoma multidisciplinary team meetings, giving a total of 252 assessments for analysis. Consistency between centres was only 'slight' to 'fair', with rates of overall agreement and Krippendorff's alpha statistics of 85.4 per cent (211 of 247) and 0.37 (95 per cent c.i. 0.11 to 0.57) for resectability; 80.4 per cent (201 of 250) and 0.39 (95 per cent c.i. 0.33 to 0.45) for treatment allocation; and 53.0 per cent (131 of 247) and 0.20 (95 per cent c.i. 0.17 to 0.23) for the organs proposed to be resected. Depending on the centre that they had attended, 12 of 21 patients could either have been deemed resectable or unresectable, and 10 of 21 could have received either potentially curative or palliative treatment. Conclusions: Inter-centre agreement between retroperitoneal sarcoma multidisciplinary team meetings was low. The dynamic nature of vehicular networks imposes a lot of challenges in multihop data transmission as links are vulnerable in their existence due to associated mobility of vehicles. Thus, packets frequently find it difficult to get through to the destination due to the limited lifetimes of links. The conventional broadcasting based vehicular ad-hoc network (VANET) routing protocols struggle to accurately analyze the link dynamicity due to the unavailability of global information and inefficiencies in their route discovering schemes. However, with the recently emerged software defined vehicular network (SDVN) paradigm, link stability can be better scrutinized pertaining to the availability of global network information. Thus, in this paper, we introduce an optimization based novel packet routing scheme with a source routing based flow instantiation (FI) operation for SDVN. The routing framework closely analyzes the stability of links in selecting the routes and the problem is formulated as a minimum cost capacitated flow problem. Furthermore, an incremental packet allocation scheme is proposed to solve the routing problem in a less time complexity. The objective is to find multiple shortest paths which are collectively stable enough to deliver a given number of packets. The FI scheme efficiently delivers and caches flow information in the required nodes with a reduced extent of communication with the control plane. With the help of realistic simulation, we show that the proposed routing framework excels in terms of the performance over the existing routing schemes of both SDVN and conventional VANET. In this paper, we study the impact of obstacles (such as buildings) in the radio propagation in the urban vehicular network's environment. The impact has been realized not only by the network-level performance metrics, but also by the application-level performance metrics. A majority of the existing works consider perfect physical layer, which ignores the impact of path loss and shadowing effect in radio propagation caused by the urban canyon. First, we show that ignoring the impact of obstacles in radio propagation provides very high superficial network-level performance. Second, under obstacle shadowing, we perform analytical analysis and Ns-3 based realistic urban vehicular simulation of current dedicated short-range communication based safety message broadcasting to study the achievable performance of application-level performance metrics. This study reveals the answer of the key question, whether the achieved network-level performance metric is good enough for the safety-critical applications under the acute attenuation in radio propagation in urban environment. Third, we propose opportunistic vehicle-assisted or dedicated road side unit-assisted network coded relaying for improving reliability at road intersection. Simulation results show that the proposed approach improves more than 65% reliability for delay sensitive and around 25% reliability for less delay sensitive applications at intersections. This article discusses a case of proximal penile fracture in a man in his 30s. The initial examination did not recognize the fracture, but an ultrasound performed later suggested a possible fracture. Magnetic resonance imaging (MRI) was then used to confirm the fracture and determine its precise location. The patient underwent urgent operative management using a perineal approach, which provided efficient access to the root of the penis. Globally, the increases in vehicle numbers, traffic congestion, and road accidents are serious issues. Autonomous vehicles (AVs) traveling in platoons provide innovative solutions for efficient traffic flow management, especially for congestion mitigation, thus reducing accidents. In recent years, platoon-based driving, also known as vehicle platoon, has emerged as an extensive research area. Vehicle platooning reduces travel time and increases road capacity by reducing the safety distance between vehicles. For connected and automated vehicles, cooperative adaptive cruise control (CACC) systems and platoon management systems play a significant role. Platoon vehicles can maintain a closer safety distance due to CACC systems, which are based on vehicle status data obtained through vehicular communications. This paper proposes an adaptive traffic flow and collision avoidance approach for vehicular platoons based on CACC. The proposed approach considers the creation and evolution of platoons to govern the traffic flow during congestion and avoid collisions in uncertain situations. Different obstructing scenarios are identified during travel, and solutions to these challenging situations are proposed. The merge and join maneuvers are performed to help the platoon's steady movement. A Software Defined Vehicular Network (SDVN) is a new paradigm that enhances programmability and flexibility in Vehicular Adhoc Networks (VANETs). There exist different architectures for SDVNs based on the degree of control of the control plane. However, in vehicular communication literature, we find that there is no proper mechanism to collect data. Therefore, we propose a novel data collection methodology for the hybrid SDVN architecture by modeling it as an Integer Quadratic Programming (IQP) problem. The IQP model optimally selects broadcasting nodes and agent (unicasting) nodes from a given vehicular network instance with the objective of minimizing the number of agents, communication delay, communication cost, total payload, and total overhead. Due to the dynamic network topology, finding a new solution to the optimization is frequently required in order to avoid node isolation and redundant data transmission. Therefore, we propose a systematic way to collect data and make optimization decisions by inspecting the heterogeneous normalized network link entropy. The proposed optimization model for data collection for the hybrid SDVN architecture yields a 75.5% lower communication cost and 32.7% lower end-to-end latency in large vehicular networks compared to the data collection in the centralized SDVN architecture while collecting 99.9% of the data available in the vehicular network under optimized settings. Aphasia is a type of speech disorder that can cause speech defects in a person. Identifying the severity level of the aphasia patient is critical for the rehabilitation process. In this research, we identify ten aphasia severity levels motivated by specific speech therapies based on the presence or absence of identified characteristics in aphasic speech in order to give more specific treatment to the patient. In the aphasia severity level classification process, we experiment on different speech feature extraction techniques, lengths of input audio samples, and machine learning classifiers toward classification performance. Aphasic speech is required to be sensed by an audio sensor and then recorded and divided into audio frames and passed through an audio feature extractor before feeding into the machine learning classifier. According to the results, the mel frequency cepstral coefficient (MFCC) is the most suitable audio feature extraction method for the aphasic speech level classification process, as it outperformed the classification performance of all mel-spectrogram, chroma, and zero crossing rates by a large margin. Furthermore, the classification performance is higher when 20 s audio samples are used compared with 10 s chunks, even though the performance gap is narrow. Finally, the deep neural network approach resulted in the best classification performance, which was slightly better than both K-nearest neighbor (KNN) and random forest classifiers, and it was significantly better than decision tree algorithms. Therefore, the study shows that aphasia level classification can be completed with accuracy, precision, recall, and F1-score values of 0.99 using MFCC for 20 s audio samples using the deep neural network approach in order to recommend corresponding speech therapy for the identified level. This paper develops a comprehensive analytical framework with foundations in stochastic geometry to characterize the performance of cluster-centric content placement in a cache-enabled device-to-device (D2D) network. Different from device-centric content placement, cluster-centric placement focuses on placing content in each cluster, such that the collective performance of all the devices in each cluster is optimized. Modeling the locations of the devices by a Poisson cluster process, we define and analyze the performance for three general cases: 1) k -Tx case: the receiver of interest is chosen uniformly at random in a cluster and its content of interest is available at the k{\mathrm{ th}} closest device to the cluster center; 2) \ell $ -Rx case: the receiver of interest is the \ell {\mathrm{ th}} closest device to the cluster center and its content of interest is available at a device chosen uniformly at random from the same cluster; and 3) baseline case: the receiver of interest is chosen uniformly at random in a cluster and its content of interest is available at a device chosen independently and uniformly at random from the same cluster. Easy-to-use expressions for the key performance metrics, such as coverage probability and area spectral efficiency of the whole network, are derived for all three cases. Our analysis concretely demonstrates significant improvement in the network performance when the device on which content is cached or device requesting content from cache is biased to lie closer to the cluster center compared with the baseline case. Based on this insight, we develop and analyze a new generative model for cluster-centric D2D networks that allows to study the effect of intra-cluster interfering devices that are more likely to lie closer to the cluster center. This paper describes a new comparison-based model for fault diagnosis in wireless ad hoc networks. Fault diagnosis is crucial for ensuring the dependability of systems. Wireless ad hoc networks are highly prone to faults as consequence of their dynamical conditions. The comparison approach is a practical diagnosis model that has been used to develop self-diagnosis systems in wired and wireless networks. This approach can detect and diagnose hard and soft faults in systems. The traditional fault diagnostic models were designed for static networks. Thus, they cannot provide complete and correct fault diagnosis in mobile wireless networks. In this paper, we introduce a time-free self-diagnosis model that respects the design requirements of mobile wireless networks. That is, it adapts to the topology's changes, it imposes no known bounds on time delays, and it requires limited network information. Further, we develop a fault diagnosis protocol that can correctly diagnose faulty nodes undergoing static and dynamic faults in mobile ad-hoc networks (MANETs). Both an analytical model and a simulation study have been used to prove and evaluate the efficiency of our protocol under various scenarios. Furthermore, the performance of our protocol is compared with related protocols. The results show that our proposed protocol is efficient in terms of communication and time complexity. Amyloidomas of the peripheral nervous system have been reported rarely. We describe a patient with a history of localized amyloidosis of the salivary glands who presented a few years later with paresthesias of her left medial arm, forearm, and fifth digit. A mass affecting the left ulnar nerve was confirmed by MRI studies. It was excised and proven on pathological examination to be an amyloidoma. Abstract: Background: The appropriate timing for laparoscopic cholecystectomy in the treatment of acute cholecystitis remains controversial. More recent evaluation indicates early laparoscopic surgery may be a safe option in acute cholecystitis, although conversion rates may be higher. No conclusive evidence establishing best practice in terms of clinical benefit exists. Methods: All randomized clinical studies published between 1987 and 2006 comparing early versus delayed laparoscopic cholecystectomy for acute cholecystitis were analyzed, irrespective of language, blinding, or publication status. Exclusions were quasi-randomized trials, inadequate follow-up description, or allocation concealment. Endpoints included conversion rates, postoperative complications, total hospital stay, and operation time. Random and fixed-effect models were used to aggregate the study endpoints and assess heterogeneity. Results: Four studies containing 375 patients were included. No significant study heterogeneity or publication bias was found. There was no significant difference in conversion rates (odds ratio = .915 [95% confidence interval (CI), .567–1.477], P = .718) and postoperative complications (odds ratio = 1.073 [95% CI, .599–1.477], P = .813) between both groups. Operation time was significantly reduced (weighted mean difference [WMD] = .412 [95% CI, .149–.675], P = .002) with delayed cholecystectomy. The total hospital stay was significantly reduced (WMD = .905 [95% CI, .630–1.179], P = .0005) with early cholecystectomy. The postoperative stay was significantly reduced in the delayed group (WMD = .393 [95% CI, .128–.659], P = .004). Conclusions: These meta-analysis data suggest that early laparoscopic cholecystectomy allows significantly shorter total hospital stay at the cost of a significantly longer operation time with no significant differences in conversion rates or complications. Abstract: Objective: Multifocal motor neuropathy with conduction block (MMN) is an immune-mediated neuropathy, characterized by progressive muscle weakness. Although demyelination is regarded as the underlying pathophysiologic mechanism of MMN, recently, it was reported that different pathophysiologic mechanisms were responsible for disease in the upper and lower limbs. Specifically, demyelination in the upper limbs and axonal loss in the lower limbs. Consequently, the aim of the present study was to assess, through clinical neurophysiology studies, whether different pathophysiologic mechanisms were occurring in the upper and lower extremities. Furthermore, we wanted to investigate whether the presence of conduction block (CB) correlated with axonal degeneration (AD), and to determine the electrophysiological abnormalities that correlate with muscle weakness. Methods: We reviewed medical records of 18 patients with MMN for clinical features (using the Medical Research Council score and Guys Neurology Disability Scale) and neurophysiologic abnormalities (CB, AD prolongation of distal motor and F-wave latencies, and reduction of conduction velocity in the demyelinating range). Results: Electrophysiological abnormalities deemed specific of demyelination were non-significantly different in the upper and lower extremities. The presence of axonal degeneration correlated significantly with conduction block (odds ratio 10.4, 95% CI 4.2–25.6), and both parameters correlated with muscle weakness (P <0.01). Conclusion: Our study suggests that the same pathophysiologic process occurs in the upper and lower extremity nerves. Moreover, one pathophysiologic process may be responsible for the development of CB and AD, and therefore muscle weakness. Abstract: Objective: Cervical nerve root stimulation (CRS) allows the assessment of conduction in the proximal segments of motor fibers destined to the upper extremities, which are not evaluated by routine nerve conduction studies (NCS). Since many primary demyelinating polyneuropathies (PDP) are multifocal lesions may be confined to the proximal nerve segments. CRS may therefore increase the yield of neurophysiologic studies in diagnosing PDP. Methods: We reviewed clinical and neurophysiologic data from 38 PDP patients and compared them to 35 patients with motor neuron disease (MND), and 21 healthy controls (HC). Results: Mean onset-latency was significantly prolonged in PDP patients. The optimal onset-latency cutoff necessary to distinguish PDP from MND and controls was 17.5ms for the abductor pollicis brevis (APB) and abductor digiti minimi (ADM), and 7ms for Biceps and Triceps. Mean reduction in proximal to distal CMAP amplitude to APB and ADM was significantly greater in PDP patients, with an optimal cutoff in proximal to distal CMAP amplitude reduction necessary to distinguish PDP from MND and HC being 45%. Conclusions: CRS is effective in distinguishing PDP from MND and HC based on prolonged onset latency and conduction block criteria. Significance: CRS may increase the diagnostic yield in cases where demyelinating lesions are confined to the proximal peripheral neuraxis. [Copyright &y& Elsevier]

This paper presents the design and implementation of a dual-band low noise amplifier (DBLNA) operating at 1.1 GHz and 2.4 GHz. The DBLNA employs a second-order dual-band matching network, which is designed using the proposed impedance and frequency transformation technique. The DBLNA has two stages in cascade topology for its high output impedance and high voltage gain. Modified output load is designed to optimize interference rejection. The implemented circuit is fabricated as a microwave integrated circuit (MIC) on the FR-4 substrate. The designed DBLNA achieves a very high measured voltage gain of 24.4 dB and 20.1 dB at 1.1 GHz and 2.4 GHz, respectively. The measured NF of the DBLNA is 2.6 dB and 3 dB at 1.1 GHz and 2.4 GHz, respectively. The interference rejection between passbands at 1.6 GHz is 53.2 dB and stopband rejection ratio is 40.4 dB. The LNA consumes 108 mW from a 3 V supply. The measured P 1 dB performance is −10 dBm and −5.1 dBm at 1.1 GHz and 2.4 GHz, respectively. [ABSTRACT FROM AUTHOR]

Light Fidelity (LiFi) is a new candidate for wireless networking that utilizes the visible light spectrum and exploits the existing lighting infrastructure in the form of light-emitting diodes (LEDs). It provides point-to-point and point-to-multipoint communication on a bidirectional channel at very high data rates. However, the LiFi has small coverage, and its optical gain is closely related to the receiver's directionality vis-à-vis the transmitter, therefore it can experience frequent service outages. To provide reliable coverage, the LiFi is integrated with other networking technologies such as wireless fidelity (WiFi) thus forming a hybrid system. The hybrid LiFi/WiFi system faces many challenges including but not limited to seamless integration with the WiFi, support for mobility, handover management, resource sharing, and load balancing. The existing literature has addressed one or the other aspect of the issues facing LiFi systems. There are limited free source tools available to holistically address these challenges in a scalable manner. To this end, we have developed an open-source simulation framework based on the network simulator 3 (ns-3), which realizes critical aspects of the LiFi wireless network. Our developed ns-3 LiFi framework provides a fully functional AP equipped with the physical layer and medium access control (MAC), a mobility model for the user device, and integration between LiFi and WiFi with a handover facility. Simulation results are produced to demonstrate the mobility and handover capabilities, and the performance gains from the LiFi-WiFi hybrid system in terms of packet delay, throughput, packet drop ratio (PDR), and fairness between users. The article presents a study which aims to assess re-do varicose vein surgery as one day case is safe and feasible. The study shows that 91% of patients were satisfied with the initial surgical result but declined to 81% in the longer term. It reveals that 89% would have their surgery done again as one day case. The study concludes that re-do sapheno-popliteal or sapheno-femoral can be done safely as a day case.

A partial receiving scheme based on limited angular aperture multi-beam receiving and demultiplexing can solve the difficulty caused by the divergence of the vortex beam in the conventional whole beam receiving scheme and realize the long-distance transmission of the vortex wave. The propagation of the radio vortex beam in atmospheric turbulence is of significant importance in theoretical study and practical applications. In this paper, the influence of atmospheric turbulence on the performance of a radio vortex (RV) communication system based on a partial angular aperture receiving (PAAR) scheme under the horizontal non-Kolmogorov channel model is studied. The spiral spectrum of the PAAR scheme and the channel capacity of the RV communication system using the PAAR scheme are derived. Simulation results demonstrate that the selected transmission frequency range has a great influence on the RV communication system based on the PAAR scheme, and the choice of the orbital angular momentum (OAM) mode number L has an influence on the propagation distance. The capacity of RV communication systems based on the PAAR scheme increases with the increase of the transmission frequency in the selected transmission frequency range of 10 GHz–60 GHz. When the number of orbital angular momentum (OAM) modes L is small, we can improve the signal-to-noise ratio (SNR) to obtain a larger capacity of the RV communication system based on the PAAR scheme over a longer propagation distance. Anti-myelin-associated glycoprotein (MAG) neuropathy is a primary demyelinating sensorimotor polyneuropathy that can be very debilitating and is known to be resistant to treatment. There are only a few conflicting reports on the effect of Rituximab in anti-MAG neuropathy. We present three patients who improved remarkably with Rituximab infusions. An autonomous V2V communication mode (also known as side-link mode 4), which facilitates V2V communication in out of eNB coverage areas, has recently been introduced into the Long term evolution (LTE) standard. Recent research has studied the performance of this LTE-V2V autonomous mode for a highway use case. However, performance analysis for a highway use case cannot be easily applied to an intersection use case as it may contain non-line-of-sight (NLOS) communication links. In this paper, we analyze and evaluate the safety message broadcasting performance of LTE-V2V autonomous mode in an urban intersection scenario. Considering practical path loss models, we present the impact of NLOS communication link on the overall message dissemination performance. Through the analytical and simulation results, we show that the overall message dissemination performance degrades drastically with increasing vehicle density and increasing distance of the transmitting vehicle from the intersection. To improve the performance, we propose a vehicle-assisted relaying scheme in which the relaying vehicle is selected in an autonomous manner. We also present two resource allocation strategies for the relaying vehicle. Unmanned aerial vehicles (UAVs) have attracted a lot of attention due to their enormous potentiality in civil and military applications over the past years. In order to allow accurate control action of UAV, a robust and real-time state estimation technique is required. In this paper, we propose a Kalman filter based UAV state estimation technique when the communication takes place over wireless links in an Internet of Things (IoT) network. We consider that a set of sensors observes the state of the UAV and transmits the observation to a control center (central server) over a distributed wireless IoT network. To deal with the communication impairments due to wireless communication links between the UAV's sensors and the IoT system components, e.g., IoT gateways, a Bose–Chaudhuri–Hocquenghem coded communication system is presented. Based on the received signals at the IoT gateways, a global state estimation technique is proposed. Performance of the proposed communication and estimation scheme is demonstrated through numerical results for different conditions. From the comparison with a conventional estimation scheme, it is observed that the proposed scheme significantly outperforms the conventional scheme in terms of state estimation and error performance. Visible light communication (VLC) is a new paradigm that could revolutionise the future of wireless communication. In VLC, information is transmitted through modulating the visible light spectrum (400–700 nm) that is used for illumination. Analytical and experimental work has shown the potential of VLC to provide high-speed data communication with the added advantage of improved energy efficiency and communication security/privacy. VLC is still in the early phase of research. There are fewer review articles published on this topic mostly addressing the physical layer research. Unlike other reviews, this article gives a system prespective of VLC along with the survey on existing literature and potential challenges toward the implementation and integration of VLC.
