Abstract

The protein interleukin-2 (IL-2) is involved in the activation and proliferation of T lymphocytes. When IL-2 binds to a cell surface receptor that is specific for IL-2, the cells become stimulated. A soluble form of the receptor (sIL-2R) is present in human blood serum. High levels of sIL-2R have been found in a number of pathological conditions. Levels of sIL-2R have been examined in patients with chronic lymphoproliferative disorders. Chronic lymphoproliferative disorders of T lymphocytes include two major clinical conditions: chronic lymphocytic leukemia of helper T cells (Th-CLL); and lymphoproliferative disease of granular lymphocytes (LDGL). Th-CLL contains both an endemic (commonly occurring but with low mortality) and a non-endemic form of adult T cell leukemia (ATL). The endemic form of ATL is associated with infection with the human T-leukemia virus type 1. Levels of sIL-2R were increased in 12 patients with Th-CLL. When Th-CLL progressed rapidly, patients had significantly higher levels of sIL-2R; levels were approximately 16 times greater when compared with those with a less aggressive disease. The serum concentration of sIL-2R correlated with the number of helper T cells. Twenty-one patients with LDGL also showed an increase of sIL-2R in their serum, but the concentration was lower than levels measured in patients with Th-CLL. The concentration of sIL-2R in the serum of a patient can be used as a clinical marker for Th-CLL and LDGL and can be used to monitor the progression of disease in Th-CLL. Abstract

In order to elucidate the role of anthracycline based combination chemotherapy regimens for the treatment of follicular lymphoma we conducted a retrospective study on a large series of patients with a histologically confirmed diagnosis of follicular lymphoma. The Italian lymphoma intergroup (ILI) promoted a retrospective study of patients with follicular lymphoma treated in cooperative trials between 1985 and 1996. Six hundred and thirty three cases were treated with an anthracycline-containing regimen and 128 patients were treated without anthracyclines. The two groups were prognostically comparable; in particular, no difference was observed according to both IPI and ILI prognostic index. Results showed a complete remission (CR) rate for patients treated with anthracyclines was 69.2% and overall response rate was 92.5%. After a median follow-up of 51 months (54 months for patients still alive), the 5- and 10-year overall survival (OS) rates were 80 and 66%, respectively. Disease-free survival (DFS) and failure-free survival (FFS) rates at 5 years were 61 and 49%, respectively. In the group of patients treated with combination chemotherapy not including anthracyclines, the CR rate was 67.5% and the overall response rate was 85.4%. A longer OS (80% at 5 years) was observed in patients treated with anthracyclines compared to 67% OS rate in patients treated without anthracyclines ( p =0.0004). FFS was significantly longer in patients treated with anthracyclines (49 vs. 34% p =0.006 ). Patients treated with anthracyclines with low or intermediate risk according to ILI prognostic index showed a significantly longer OS ( p =0.0001 and p =0.0009, respectively); those in the high-risk group showed a trend for a longer survival. In conclusion, this retrospective study shows that patients with follicular lymphoma treated with an anthracycline containing regimen had a better outcome compared to patients treated with other combination regimens non including anthracyclines in terms of CRs, OS and FFS. 44. Prognosis of follicular lymphoma: a predictive model based on a retrospective analysis of 987 cases

Abstract

Patients (n-987) with a histologically confirmed diagnosis of follicular lymphoma were studied with the aim of developing a prognostic model specifically devised for this type of lymphoma. We collected information on age, sex, Ann Arbor stage, number of extranodal disease sites, bone marrow (BM) involvement, bulky disease, B symptom criteria (fever, night sweats, and weight loss), performance status (PS), serum lactate dehydrogenase (LDH) level, serum albumin level, hemoglobin level, and erythrocyte sedimentation rate (ESR). In the training sample of 429 patients with complete data, multivariate analysis showed that age, sex, number of extranodal sites, B symptoms, serum LDH level, and ESR were factors predictive for overall survival. Using these 6 variables, a prognostic model was devised to identify 3 groups at different risk. The 5- and 10-year survival rate was 90% and 65% for patients at low risk, respectively; 75% and 54% for patients at intermediate risk; and 38% and 11% for those at high risk (log-rank test, 86.62; P < .0001). The model was also predictive (P = .0001) in the validation sample of 265 patients with complete data only for the 6 variables used in the development of the model and even in the group of 210 patients from the validation sample uniformly treated with doxorubicin-containing regimens (P = .0001). The prognostic model appears to be very useful in identifying patients with follicular lymphoma at low, intermediate, or high risk.

45. Mechanisms Accounting for the Defective Natural Killer Activity in Patients With Hairy Cell Leukemia

Abstract

Natural killer (NK) cell activity is severely impaired in untreated patients with hairy cell leukemia (HCL). In an attempt to investigate whether this impairment is related to a defect at the target cell binding and/or at the post target cell binding level, we evaluated the peripheral blood mononuclear cells (PBMC) of HCL patients for their ability to: (1) bind and kill K-562 NK-sensitive targets at the single cell binding level; (2) release the NK cytotoxic factor (NKCF) under different in vitro stimuli, including K-562 and phytohemoagglutinin; and (3) kill K-562 targets in a lectin-dependent cellular cytoxicity (LDCC) assay. This study demonstrates that untreated HCL patients' PBMC show a low ability to form conjugates with K-562 targets at the single cell binding level (5.7% ± 1.0%) with respect to patients studied after treatment (9.3% ± 1.3%) and controls (15.0% ± 4.0%); P< .05 and P< .001, respectively. A decreased ability to kill the bound target was demonstrated in untreated cases (1.2% ± 1.1%) versus patients studied after treatment and controls (12.3% ± 1.6%, 17.0% ± 3.1% respectively); P < .001 in both conditions. After activation of effector cells with interleukin-2 (IL-2) in vitro, an increase in the ability of PBMC to form conjugates with K-562 targets and kill the bound target was demonstrated in each group of patients. Moreover, IL-2 was able to increase the cytotoxicity against NK-sensitive targets in all patients tested. Evaluation of NKCF production showed that untreated patients release low levels of NKCF when PBMC were incubated in the presence of K-562 stimulators (1.8% ± 0.7%) with respect to patients after interferon-α (IFN-α) therapy (7.6% ± 2.1%) and controls (12.9% ± 2.2%); P< .02 and P < .001, respectively. When the recognition mechanisms were bypassed by triggering the cells with lectins in an LDCC assay, we demonstrated an increase of the lytic activity in both groups of patients with respect to the baseline values. However, the cytotoxic capacity observed in untreated patients was significantly lower than that observed in subjects after IFN-α therapy and controls (P< .001). These findings suggest that the impaired NK activity observed in patients with HCL is related to defects both at the target and posttarget cell binding levels.

46. Expression and Functional Role of Tumor Necrosis Factor Receptors on Leukemic Cells From Patients With Type B Chronic Lymphoproliferative Disorders

Abstract

Two receptors for tumor necrosis factor (TNF) with different molecular weight (75-Kd and 55-Kd) and binding affinity have been recently discovered. To investigate the distribution and the functional role of these receptors on leukemic B cells from hairy cell leukemia (HCL) and B-cell chronic lymphocytic leukemia (B-CLL) patients, we evaluated: (1) the cytofluorimetric pattern of uncultured and cultured leukemic B cells incubated with utr-1 and htr-9 monoclonal antibodies (MoAbs), which specifically recognize the 75-Kd and 55-Kd TNF receptors (TNFR), respectively; (2) the effect of TNF-α and TNF-βon leukemic B cells in an in vitro proliferation assay; (3) the role of anti-TNFR MoAbs on TNF-αand TNF-β-driven B-cell growth; and (4) the proliferative effect of utr-1 and htr-9 MoAbs on in vitro cultured leukemic cells. Our study shows that the high affinity (75-Kd) but not the low affinity (55-Kd) TNFR molecules are expressed on freshly isolated leukemic B cells recovered from HCL and B-CLL patients. The expression of these receptors was neither upregulated nor downregulated by different stimuli, including TNF-α, TNF-β, B-cell growth factor, and interleu-kin-2. TNF-α efficiently triggers the proliferation of HC and, to a lesser extent, the growth of B-CLL cells. TNF-βwas also able to transduce the proliferative signal in HCL, but not in B-CLL patients. TNF-α— and TNF-β-driven B-cell proliferation was inhibited by the preincubation of leukemic B cells with utr-1 but not htr-9 MoAb. Moreover, anti-75-Kd, but not anti-55-Kd TNFR MoAb, was able to trigger the proliferation of leukemic B cells, and in particular of HC These results show that leukemic B cells from patients with HCL and B-CLL are equipped with a fully functional high affinity TNFR.

Abstract

The cell membrane expression and functional role of the interleukin-2 receptor (IL-2R) was analyzed in nine patients with lymphoproliferative disease of granular lymphocytes (LDGL) using monoclonal antibodies (MoAbs) specific for the p75 (TU27) and the p55 (anti-Tac) subunits of IL-2R. Four patients were characterized by the proliferation of CD3+CD8+ granular lymphocytes (GL) expressing the α/βT-cell receptor (Tαβ) and one case by the proliferation of CD3+CD4–CD8– GL expressing the γ/δ T-cell receptor (Tγδ); in four additional cases proliferating cells were CD3 negative GL. Consistent with data observed on normal GL, phenotypic analysis demonstrated that patients’ GL lack the expression of the p55 IL-2R, whereas the p75 subunit is constitutionally expressed by expanding GL of both T-cell (either Tαβand Tγδ) and natural killer (NK) origin in variable proportions (11% to 94% of cells). The analysis of the cytotoxic and proliferative activity demonstrated that the anti-p55 MoAb failed to inhibit IL-2– mediated activation, whereas a marked inhibition of both cytotoxicity and proliferation were obtained using the anti-p75 chain specific MoAb. These data indicate that the p75 chain of IL-2R is responsible for IL-2 signal transduction in both CD3+ and CD3– LDGL patients’ GL.

48. Immunobeads Test (Antibody Coated Polyacrylamide Beads) in the Immunological Characterization of Lymphoproliferative Disorders

Abstract

Three groups of patients with immunoproliferative disorders (15 multiple myeloma, 11 non-Hodgkin's lymphoma, 21 chronic lymphocytic leukemia) were studied by immunological characterization and compared to a group of 20 normal subjects (controls) using anti-immunoglobulin coated polyacrylamide beads (T-B Quantigen test, QT), erythrocyte rosettes (ER), surface immunoglobulin (SIg), and monoclonal antibodies for T and B cells (OKT3; OKT11; OKT8; OKT4; IaDR); null cells (NC) and double marker (DM) cells were also considered. The values for normal subjects for T-B, NC and DM cells were comparable.Results for the patient groups strikingly differed. There were progressively larger differences between the T and B percentages obtained with different techniques. The largest differences were seen in patients with chronic lymphocytic leukemia and the smallest in multiple myeloma patients; values were intermediate in non-Hodgkin lymphoma. The different findings were related to the number of DM cells (ER +, SIg+ QT+) and the different tests used.The importance of these findings in the diagnostic appproach to lymphoproliferative disorders is discussed.

Abstract

Forty-nine patients with previously untreated advanced Hodgkin's disease were treated in our Institution between 1973 and 1981. Treatment modalities of these patients were reviewed, and they were divided into 3 groups according to the treatment employed: 13 patients received MOPP only, 22 patients received MOPP plus involved field radiotherapy, and 14 received alternating MOPP/ABVD chemotherapy. The response rates for the 3 groups were respectively 38.5%, 63.6% and 78.6%. A longer follow-up is needed to assess a significant difference in survival curves. The advantages of adjuvant radiotherapy and alternating non-cross-resistant drugs in advanced Hodgkin's disease are discussed.
