Abstract

Introduction: World Health Organization (WHO) 6 steps for effective Hand Hygiene (HH) practice to prevent cross infections emphasises on coverage of all aspects of the hand with the Alcohol Based Hand Rub (ABHR). The quality of hand rubbing usually remains un-monitored among healthcare workers. Hence, interventions to ensure both compliance and quality combined with periodic training would fill the deficiency in these practices in the healthcare setting. Aim: To analyse the quality of HH using an Ultraviolet (UV) sensitive ABHR among healthcare workers in the Operation Theatre (OT) and post-surgical Intensive Care Unit (ICU). Materials and Methods: This cross-sectional study was conducted in Regional Cancer Centre, Trivandrum, Kerala, India. Study population included 104 healthcare providers in the OT and post-surgical ICU, over a period of two months, from June 2017 to July 2017. Participants were asked to disinfect their hands according to WHO 6 steps using an ABHR to which an UV light sensitive pigment was added, and then place them in an UV light sensitive cabinet. Digital images of both sides of their hands were recorded. Every area of the hand not covered by the hand rub appeared as a dark spot and was considered an error. Abstract

Abstract Background Dexmedetomidine is a widely used alpha-2 adrenoreceptor agonist in perioperative patient care. Its postsynaptic activation of the receptors in the central nervous system is responsible for reduced neuronal firing with resultant sedation, anxiolysis, analgesia, hypotension and bradycardia leading to effective stress response attenuation seen during laryngoscopy and orotracheal intubation. Major head and neck surgeries demand nasotracheal intubation which is more stressful than orotracheal as it traverses through the nasopharynx which is very pain-sensitive. This is to protect the airway in the immediate postoperative period from oedema or haematoma in the oral cavity or neck. Though its stress response attenuation, haemodynamic stability during the intraoperative period following orotracheal intubation has been well studied, its role in the prevention of emergence delirium and tube tolerance following nasotracheal intubation in major head and neck surgery is not found in the literature. Our aim was to evaluate whether dexmedetomidine effectively attenuates the stress response following nasotracheal intubation, perioperative haemodynamic fluctuations and quality of emergence in patients undergoing head and neck oncosurgeries. Methods A total of 150 patients were randomly assigned to one of the two groups; group D (dexmedetomidine group) and group S (control group with saline). Group D patients received a bolus dose of dexmedetomidine 1 μg/kg in 10 ml saline over 10 mts before induction of GA followed by an infusion at 0.4 μ/kg/h during surgery. Statistical analysis was done using SPSS version 11.0 (SPSS Ltd., Chicago, IL). Categorical data were represented using frequencies and percentages. Continuous variables were represented using mean and standard deviation. The association between categorical variables was assessed using the chi-square or Fisher’s exact test, and continuous variables following normality assumption with respect to the two groups were assessed using an independent sample t-test. P value < 0.05 was considered to be statistically significant. Results Patients in the D group showed statistically significant attenuation of heart rate (P < 0.05) and blood pressure (P < 0.05) throughout the surgical period compared to saline. Also, there was a significant reduction in blood loss (P = 0.042), cough score (P = 0.001) and sedation score (P = 0.001) in the D group. Conclusions We conclude that a bolus dose of dexmedetomidine 1 μg/kg given 10 min before induction of anaesthesia followed by an infusion at 0.4 μg/kg/h during surgery effectively attenuates the haemodynamic responses during nasotracheal intubation and provides smooth emergence as evidenced by reduced coughing, agitation and arousable sedation without respiratory depression which facilitates tube tolerance following major head and neck oncosurgeries. Blood loss was also found to be significantly reduced.

Abstract

Abstract Background The bispectoral index (BIS), a parameter derived from electroencephalogram, has been used to assess the depth of anesthesia. The objectives of this study were to evaluate the effect of BIS monitoring on sevoflurane consumption and recovery profile at the end of anesthesia. After obtaining Institutional Review Board approval and written informed consent, 25 American Society of Anesthesiologists (ASA) physical status classification 1 and 2 patients undergoing breast cancer surgeries who had BIS monitoring in addition to standard ASA monitoring (BIS GROUP) were compared against 25 controls (control group). In the control group, adequate depth of anesthesia was maintained using routine clinical parameters like heart rate (HR), mean arterial pressure (MAP), and minimum alveolar concentration (MAC) of sevoflurane, while in the BIS group, it was maintained by keeping the BIS score between 40 and 60 (mean 50). Data including demographics, sevoflurane consumption, hemodynamic variables, and recovery profile at the end of anesthesia was assessed in terms of time for eye opening (TEO), time for motor response (TMR), time for extubation (TE), and modified Aldrete scoring (MAS). Results The mean sevoflurane consumption was lower (P = 0.019) in the BIS group. TEO (P = 0.001), TMR (P = 0.0001), and TE (0.003) were shorter in the BIS group. Difference in MAS between the 2 groups was not statistically significant (P = 0.085). Conclusions BIS monitoring during anesthesia resulted in significant reduction in the sevoflurane consumption. Patients who had BIS monitoring awoke earlier and had better recovery profile at the end of anesthesia.

Abstract

Introduction: Postoperative Delirium (POD) is one of the most common complications in geriatric surgical patients. The POD has immense implication because it can initiate a cascade of deleterious events leading to functional decline, reduced cognitive function, prolonged hospitalisation, and death. The risk factors for POD include increased age, cognitive, visual or sensory impairment, functional dependence, infection, and electrolyte abnormalities. Published literature on POD in patients aged >60 years undergoing oncosurgeries are few. According to the American Geriatric Society, evidence for recommendation of intraoperative risk factors is lacking. The relation between intraoperative haemodynamics and postoperative pain with POD is controversial, as studies provide differences in evidence. Hence this relation needs to be studied further. Aim: To find the incidence of POD in patients >60 years of age undergoing oncosurgery, and its association with intraoperative hypotension and blood pressure fluctuations. The secondary objective was to assess the relation of POD with postoperative pain. Materials and Methods: The present study was prospective cohort study which was carried on 50 patients aged >60 years undergoing cancer surgery. Intraoperative hypotension and blood pressure fluctuations were measured based on predefined criteria. Postoperative pain and total opioid consumption were also noted. Delirium was assessed with the short Confusion Assessment Method (short CAM), on the first three postoperative days. The association between hypotension and intraoperative blood pressure fluctuations with POD were analysed with Fisher exact test and Mann-Whitney U test. Association of pain scores and total dose of opioid with POD was performed using Student’s t-test and Mann-Whitney U test. Results: The mean age of the study population was 69.5±2.8 years. The mean preoperative Addenbrook’s Cognitive Examination (ACE) score was 83.78. The incidence of POD was 22%. There was a significant association between intraoperative blood pressure fluctuation and POD. The mean BP variance ranged from 126.89 to 111.13 mmHg. Increased age and more co-morbidities ≥3 (present in 58%) showed an association with POD. Conclusion: The incidence of POD was high (22%) among the elderly requiring oncosurgery. Intraoperative blood pressure fluctuation as well as hypotension was associated with increased risk of POD. Hence in elderly patients, tight blood pressure control is advisable during surgery.

Abstract

Summary: We identify amino acid variants within dominant SARS-CoV-2 T cell epitopes by interrogating global sequence data. Several variants within nucleocapsid and ORF3a epitopes have arisen independently in multiple lineages and result in loss of recognition by epitope-specific T cells assessed by IFN-γ and cytotoxic killing assays. Complete loss of T cell responsiveness was seen due to Q213K in the A∗01:01-restricted CD8+ ORF3a epitope FTSDYYQLY207-215; due to P13L, P13S, and P13T in the B∗27:05-restricted CD8+ nucleocapsid epitope QRNAPRITF9-17; and due to T362I and P365S in the A∗03:01/A∗11:01-restricted CD8+ nucleocapsid epitope KTFPPTEPK361-369. CD8+ T cell lines unable to recognize variant epitopes have diverse T cell receptor repertoires. These data demonstrate the potential for T cell evasion and highlight the need for ongoing surveillance for variants capable of escaping T cell as well as humoral immunity.

Abstract

Our aim was to evaluate the role of traditional versus newer markers of obesity, in risk assessment of CAD. 50 consecutive ACS patients and 20 controls were enrolled.Visceral and Subcutaneous fat (VFAT and SFAT) analysis was done using multi-slice abdominal MRI. Syntax score was calculated from coronary angiogram. In our study, BMI and Waist/Hip ratios showed poor correlation with Syntax score. VFAT and SFAT showed strong correlation with Syntax score (p-0.01,0.03) and a more significant correlation was noted with their areas at L3-L4 levels (p-0.01,0.05). Statistically significant ROC- area under curve was observed with Indexed SFAT.

Abstract

Aim: The Elder Heart Registry aims to study the demographics, clinical profile, in-hospital, and 1 year outcomes of octogenarian patients (≥80 years) with acute cardiovascular disease. Materials and Methods: This is a prospective observational, longitudinal cohort, single-center registry of octogenarian cardiac patients admitted to the cardiology intensive care unit (CICU) at a tertiary care teaching hospital in Kerala, India for 2 years. Data analysis was performed using SPSS version 20 software. Results: From a total of 4199 patients admitted to the CICU, the Registry enrolled 406 patients aged ≥80 years, which constituted 9.6% of the total admissions (mean age 84.24 ± 3.67 years, male-to-female ratio of 1.15:1, mean follow-up of 1.2 years). Acute coronary syndromes (ACS) constituted 42.1% of admissions, with ST-elevation myocardial infarction in 22.2%, non-ST elevation myocardial infarction in 64.3%, and unstable angina in 13.4%. Presentation with heart failure (HF) was seen in 36.4% of patients, with HF with preserved ejection fraction (HFpEF) in 51.7%. The rates of in-hospital mortality, mortality on follow-up and rehospitalization were 14.0%, 9.3%, and 21% in the ACS group, and 9.4%, 8.1% and 18.9% in the HF group, respectively (12.1%, 8.6%, and 15.5% in the HF with reduced ejection fraction subset and 10%, 8.3%, and 20% in the HFpEF subset). Atrial fibrillation was the most common arrhythmia (16.5%). Apart from systemic hypertension (77.3%) and diabetes mellitus (53.9%), co-morbidities noted were chronic kidney disease (16.7%), obstructive airway disease (8%), thyroid disorders (4%), significant anemia (3%), depression (2.5%), and malignancy (2%). Conclusions: Octogenarians constitute almost 10% of all admissions to the CICU. ACS and HF are the major cardiac causes for hospitalization among these very elderly patients. The Elder Heart Registry is currently the largest series of octogenarian cardiac patients from India.

Abstract

Abstract Background Castleman’s disease is a rare lymphoproliferative disease which can mimic other malignant tumours and can be associated with paraneoplastic pemphigus especially in the paediatric age group. This presentation can be challenging even to the seasoned anaesthesiologist when it comes to managing such a case presenting for laparotomy. Case presentation Here such a case in a 12-year-old who was initially diagnosed as retroperitoneal sarcoma complicated with paraneoplastic pemphigus and inferior venacaval thrombus is presented and its anaesthetic management and challenges are discussed. Special care needs to be taken while anaesthetizing such a patient keeping in mind the difficult airway, adrenocortical suppression, and exacerbation of skin lesions due to various anaesthetic procedures. For this case, the histopathology turned out to be Castleman’s disease. Conclusion The anaesthetic challenges associated with paraneoplastic pemphigus in paediatric age group receiving perioperative thromboprophylaxis need special mention. Here we are discussing the challenges and anaesthetic management of such a case as literature on this aspect is few.

Abstract

Summary: We report severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) spike ΔH69/V70 in multiple independent lineages, often occurring after acquisition of receptor binding motif replacements such as N439K and Y453F, known to increase binding affinity to the ACE2 receptor and confer antibody escape. In vitro, we show that, although ΔH69/V70 itself is not an antibody evasion mechanism, it increases infectivity associated with enhanced incorporation of cleaved spike into virions. ΔH69/V70 is able to partially rescue infectivity of spike proteins that have acquired N439K and Y453F escape mutations by increased spike incorporation. In addition, replacement of the H69 and V70 residues in the Alpha variant B.1.1.7 spike (where ΔH69/V70 occurs naturally) impairs spike incorporation and entry efficiency of the B.1.1.7 spike pseudotyped virus. Alpha variant B.1.1.7 spike mediates faster kinetics of cell-cell fusion than wild-type Wuhan-1 D614G, dependent on ΔH69/V70. Therefore, as ΔH69/V70 compensates for immune escape mutations that impair infectivity, continued surveillance for deletions with functional effects is warranted.

Summary: Background: The SARS-CoV-2 variant B.1.1.7 was first identified in December, 2020, in England. Methods: We did an ecological study to examine the association between the regional proportion of infections with the SARS-CoV-2 B.1.1.7 variant and reported symptoms, disease course, rates of reinfection, and transmissibility. Data on types and duration of symptoms were obtained from longitudinal reports from users of the COVID Symptom Study app who reported a positive test for COVID-19 between Sept 28 and Dec 27, 2020 (during which the prevalence of B.1.1.7 increased most notably in parts of the UK). From this dataset, we also estimated the frequency of possible reinfection, defined as the presence of two reported positive tests separated by more than 90 days with a period of reporting no symptoms for more than 7 days before the second positive test. The proportion of SARS-CoV-2 infections with the B.1.1.7 variant across the UK was estimated with use of genomic data from the COVID-19 Genomics UK Consortium and data from Public Health England on spike-gene target failure (a non-specific indicator of the B.1.1.7 variant) in community cases in England. We used linear regression to examine the association between reported symptoms and proportion of B.1.1.7. We assessed the Spearman correlation between the proportion of B.1.1.7 cases and number of reinfections over time, and between the number of positive tests and reinfections. We estimated incidence for B.1.1.7 and previous variants, and compared the effective reproduction number, Rt, for the two incidence estimates. Findings: From Sept 28 to Dec 27, 2020, positive COVID-19 tests were reported by 36 920 COVID Symptom Study app users whose region was known and who reported as healthy on app sign-up. We found no changes in reported symptoms or disease duration associated with B.1.1.7. For the same period, possible reinfections were identified in 249 (0·7% [95% CI 0·6–0·8]) of 36 509 app users who reported a positive swab test before Oct 1, 2020, but there was no evidence that the frequency of reinfections was higher for the B.1.1.7 variant than for pre-existing variants. Reinfection occurrences were more positively correlated with the overall regional rise in cases (Spearman correlation 0·56–0·69 for South East, London, and East of England) than with the regional increase in the proportion of infections with the B.1.1.7 variant (Spearman correlation 0·38–0·56 in the same regions), suggesting B.1.1.7 does not substantially alter the risk of reinfection. We found a multiplicative increase in the Rt of B.1.1.7 by a factor of 1·35 (95% CI 1·02–1·69) relative to pre-existing variants. However, Rt fell below 1 during regional and national lockdowns, even in regions with high proportions of infections with the B.1.1.7 variant. Interpretation: The lack of change in symptoms identified in this study indicates that existing testing and surveillance infrastructure do not need to change specifically for the B.1.1.7 variant. In addition, given that there was no apparent increase in the reinfection rate, vaccines are likely to remain effective against the B.1.1.7 variant. Abstract

Introduction: Left ventricular false tendons (LVFTs) are discrete, fibromuscular structures of varying length and thickness that traverse the left ventricular (LV) cavity. The aim of the present study was to describe the echocardiographic characteristics and associations of 100 consecutive cases of LVFTs that presented to the echocardiography laboratory at a tertiary care teaching hospital in India. Subjects and Methods: One hundred consecutive patients with LVFTs were studied. They were classified based on the number (single and multiple), morphology (simple and complex), and location (apical, mid, and basal). The individuals were divided into three groups (Group 1: with LV dilatation and/or LV hypertrophy [LVH], Group 2: abnormal echocardiogram other than Group 1, and Group 3: normal echocardiogram). Analysis was done using SPSS 15 software. Results: A total of 121 LVFTs were identified in the 100 individuals. The LVFTs were more commonly single (81%), simple (92.56%), and apically located (52.90%). Complex LVFTs and apical location were more common among males, while multiple LVFTs and mid location were more common among females. LVFTs were most commonly visualized in the apical four-chamber view. There was a preponderance of males (67%), patients with structural heart disease (79%), and patients with LVH and/or dilatation (44%) in the study group. Conclusions: LVFTs are readily identifiable on echocardiography and can be characterized based on their number, location, and morphology. Their relationship to structurally abnormal hearts, male gender, LVH, and dilatation requires further study.

Abstract

Background and Aims: Immunomodulatory consequences of allogeneic blood transfusion may outweigh the advantages of improved oxygen delivery and tissue perfusion, especially in patients with cancer. In colorectal cancer, there is evidence of cancer relapse in patients who received transfusion. This retrospective analysis was undertaken to evaluate the relationship between perioperative blood transfusion and cancer recurrence in patients undergoing oncosurgery. Methods: In this case-control study, we retrospectively analysed the case sheets of 194 patients who had perioperative transfusion and underwent cancer surgery from March to December 2013(Study group, Cases). They were compared with controls matched for cancer site and TNM staging who did not receive perioperative transfusions (Control Group, Controls). We intended to find out if the Study group had any increased risk of cancer relapse compared with the controls. Records from the institute cancer registry were analysed in 2018 to give a follow-up period of 5 years. Continuous variables were analysed using Student's T test and Mann Whitney U test for normally distributed and skewed data respectively. For Categorical data Fisher's exact test and Chi square test were applied. The risk for recurrence was estimated using odds ratio. Results: The recurrence rate in cases and controls was 53.09% and 19.59% respectively and the odds ratio, 4.647 (CI: 2.954, 7.309). In Cases, significant relapse was noted for carcinomas of ovary, colorectal, bladder, larynx, head of pancreas and liver. Conclusion: In surgical oncology patients, ABT is associated with greater rate of recurrence.

Abstract

Advances in medical research and intelligent modeling techniques have led to developments in anaesthesia management. The present study aims to estimate the depth of anaesthesia using cognitive signal processing and intelligent modeling techniques. The neurophysiological signal that reflects the cognitive state of anaesthetic drugs is the electroencephalogram signal. The information available from electroencephalogram signals during anaesthesia is extracted from the relative wave energy features of those signals. Discrete wavelet transform is used to decompose electroencephalogram signals into four levels and the relative wave energy is computed from approximate and detailed coefficients of the signal sub-bands. Relative wave energy is extracted to determine the degree of importance of different electroencephalogram frequency bands associated with different anaesthetic phases, for example, the awake, induction, maintenance, and recovery phases. The Kruskal-Wallis statistical test is applied to relative wave energy features to check the discriminative capability of the relative wave energy features classified as awake, light anaesthesia, moderate anaesthesia, and deep anaesthesia. A novel depth of anaesthesia index is generated by implementing an adaptive neuro-fuzzy inference system based on a fuzzy c-means clustering algorithm which uses relative wave energy features as inputs. Finally, the generated depth of anaesthesia index is compared with a commercially available depth of anaesthesia monitor, the Bispectral index.

Abstract

Patients with incidental bullous lesion of lung occasionally present for non-thoracic surgeries, these patients are at increased risk of potential complications which can arise due to bullae, during perioperative period. Anaesthetic management of these patients planned for a major non-lung volume reduction surgery is challenging as it is rare and requires careful understanding of anatomical and pathophysiological variation of disease. Safe conduct of anaesthesia can be delivered without isolation of the lung or sub-segment in which bulla is involved in these patients so that perioperative respiratory crippling conditions like pneumothorax, emphysema, atelectasis of the surrounding lung parenchyma leading to postoperative respiratory failure can be avoided. Eternal vigilance, monitoring, ventilator strategies are required to avoid possible perioperative complications and a successful outcome, few essential precautionary measures to be taken include vigilant monitoring of patient perioperatively, avoidance of nitrous oxide, low airway pressures, immediate availability of chest drains, periodic auscultation of chest, arterial blood gas analysis. Here, the present authors report a successful perioperative anaesthetic management of a patient with giant bulla in left upper lobe and right upper lobe developed as post tubercular sequelae that underwent inferior partial maxillectomy with neck dissection and reconstruction for carcinoma upper alveolus without isolation of lung.

Abstract

COG-UK is supported by funding from the MRC part of UK Research & Innovation, the National Institute of Health Research (Grant code MC_PC_19027), and Genome Research Limited, operating as the Welcome Sanger Institute. Background The first epidemic wave of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) in Scotland resulted in high case numbers and mortality in care homes. In Lothian, over one-third of care homes reported an outbreak, while there was limited testing of hospital patients discharged to care homes. Aim To investigate patients discharged from hospitals as a source of SARS-CoV-2 introduction into care homes during the first epidemic wave. Methods A clinical review was performed for all patients discharges from hospitals to care homes from 1st March 2020 to 31st May 2020. Episodes were ruled out based on coronavirus disease 2019 (COVID-19) test history, clinical assessment at discharge, whole-genome sequencing (WGS) data and an infectious period of 14 days. Clinical samples were processed for WGS, and consensus genomes generated were used for analysis using Cluster Investigation and Virus Epidemiological Tool software. Patient timelines were obtained using electronic hospital records. Findings In total, 787 patients discharged from hospitals to care homes were identified. Of these, 776 (99%) were ruled out for subsequent introduction of SARS-CoV-2 into care homes. However, for 10 episodes, the results were inconclusive as there was low genomic diversity in consensus genomes or no sequencing data were available. Only one discharge episode had a genomic, time and location link to positive cases during hospital admission, leading to 10 positive cases in their care home. Conclusion The majority of patients discharged from hospitals were ruled out for introduction of SARS-CoV-2 into care homes, highlighting the importance of screening all new admissions when faced with a novel emerging virus and no available vaccine. Abstract

Background: Endotracheal tube tolerance is a major concern in patients who require postoperative retention of endotracheal tube following major head and neck cancer surgeries. This study investigates the role of Dexmedetomidine for endotracheal tube tolerance, opioid sparing effect and sedation. Methods: Prospective randomised double blind control study. Sixty-four patients scheduled for head and neck surgery lasting longer than 2 h who require post operative endotracheal tube retention were randomly allocated into two groups of 32 each. The Dexmedetomidine group (Group D, n = 32) received Dexmedetomidine 1 μg kg−1 diluted to 10 ml, i.v. over 10 min prior to reversal and thereafter 0.2 μg kg−1 h−1 i.v. infusion till extubation while the control group (Group P, n = 32) received 10 ml normal saline i.v. over 10 min prior to reversal and volume-matched normal saline infusion as placebo till extubation. The two groups of patients were compared for haemodynamic changes during administration of the bolus drug, after reversal and in the ICU. The quality of endotracheal tube tolerance, analgesic requirement and sedation were also compared between the two groups in the ICU. Background and Aims : With the availability of modern workstations and heightened awareness on the environmental effects of waste anaesthesia gases, anaesthesiologists worldwide are practicing low flow anaesthesia (LFA). Although LFA is being practiced in India, hard evidence on the current practice of the same from anaesthesiologists practicing in India is lacking and hence, we conducted this survey. Methods : A questionnaire containing 16 questions was distributed among a subgroup of anaesthesiologists who attended the 2014 National Conference of Indian Society of Anaesthesiologists. The filled-in questionnaires were computed and analysed with SPSS version 11. Results: The response rate to the survey was 82%. Abstract

Aims: Difficult tracheal intubation still contributes significantly to anaesthesia related morbidity and mortality. Poor visualisation of laryngeal structures and multiple attempts at intubation are the leading causes with the conventional laryngoscopes. Though the recently introduced video assisted devices have significantly improved the ease of intubation by their superior laryngeal visualisation, the duration of intubation may vary. Here we compared the ease of tracheal intubation using Macintosh conventional direct laryngoscope (DL) and C- MAC videolaryngoscope (VL) in patients with expected difficult tracheal intubation. Materials and Methods: A total of 140 patients undergoing elective surgery under general anaesthesia with Modified Mallampati Class 3 and 4 found during the preoperative airway assessment were equally recruited to either of the groups. We compared the duration of tracheal intubation, visualisation of the laryngeal inlet, additional optimising manoeuvres required, and number of attempts at intubation and incidence of oral trauma assessed at extubation between the two groups. Statistical Analysis: Analysis done using Statistical Packages for the Social Sciences (SPSS) software; Windows version 11.0 (SPSS Inc., Chicago, IL, USA). Results: Intubation time was significantly longer in patients with VL than DL (P 0.0001) whereas visualisation of laryngeal inlet was significantly better with VL (P 0.001). Additional optimisingmanoeuvres (P 0.001) and incidence of oral trauma (P 0.012) were significantly less with VL whereas intubation attempts were found comparable (P 0.586). Conclusion: Though VL provided significantly better laryngeal view with less need for optimising manoeuvres and less oral trauma compared to DL, the duration of intubation was significantly more with the former. Abstract

Background The bispectoral index (BIS), a parameter derived from electroencephalogram, has been used to assess the depth of anesthesia. The objectives of this study were to evaluate the effect of BIS monitoring on sevoflurane consumption and recovery profile at the end of anesthesia. After obtaining Institutional Review Board approval and written informed consent, 25 American Society of Anesthesiologists (ASA) physical status classification 1 and 2 patients undergoing breast cancer surgeries who had BIS monitoring in addition to standard ASA monitoring (BIS GROUP) were compared against 25 controls (control group). In the control group, adequate depth of anesthesia was maintained using routine clinical parameters like heart rate (HR), mean arterial pressure (MAP), and minimum alveolar concentration (MAC) of sevoflurane, while in the BIS group, it was maintained by keeping the BIS score between 40 and 60 (mean 50). Data including demographics, sevoflurane consumption, hemodynamic variables, and recovery profile at the end of anesthesia was assessed in terms of time for eye opening (TEO), time for motor response (TMR), time for extubation (TE), and modified Aldrete scoring (MAS). Results The mean sevoflurane consumption was lower (P = 0.019) in the BIS group. TEO (P = 0.001), TMR (P = 0.0001), and TE (0.003) were shorter in the BIS group. Difference in MAS between the 2 groups was not statistically significant (P = 0.085). Conclusions BIS monitoring during anesthesia resulted in significant reduction in the sevoflurane consumption. Patients who had BIS monitoring awoke earlier and had better recovery profile at the end of anesthesia.

Abstract

Background: Anaesthetics and anthracyclines can affect the QT interval of the electrocardiogram (ECG). This study investigated whether the use of isoflurane in anthracyclines pretreated patients may induce or augment the QT prolongation to an arrhythmogenic level. Materials and methods: Fifty-four female patients with breast cancer scheduled for mastectomy were included in the study, 27 received anthracyclines based chemotherapy before surgery, whereas 27 did not. All patients received a standardized balanced anaesthetic in which 0.5% end tidal concentration of isoflurane was used. The QT and corrected QT intervals (QTc) were measured before anaesthesia, after 1, 5, 15, 30, 60 min, respectively, following intubation and during recovery from anaesthesia. Results: Statistically significant QTc prolongation was observed in patients who received anthracycline chemotherapy even prior to the administration of anaesthesia. The comparison of QTc interval at different intervals of isoflurane anaesthesia also showed a statistically significant difference between the two groups namely anthracycline treated group (study group) versus control group. A sample size of 27 in each group was calculated in order to achieve a study power of 80% with type 1 error rate of 5%. Abstract

Central venous catheter (CVC) insertions are increasingly performed in surgical patients and intensive therapy. A simple and invasive procedure performed under strict sterile precautions with complications ranging from arrhythmias; infections; and life-threatening complications such as pericardial tamponade, cardiac perforation and even death. A post-procedure chest X-ray (CXR), though does not accurately assess the tip of the catheter in relation to the superior vena cava (SVC) and right atrium (RA), can detect malpositions, safety of catheter tip, pneumothorax and kinking. We would like to share some of the malpositions we encountered in our centre, their management and a brief review of the literature on optimal catheter tip location.

Abstract

A 33 year old lady with no obvious reason to suspect susceptibility to malignant hyperpyrexia (MH) succumbed to this unexpected complication in spite of attempts to save her life with aggressive supportive measures. We are reporting this case of possible malignant hyperthermia probably the seventh reported case in India and second from Kerala in the last decade (Saxena and Dua, 2007; Punj et al., 2001; Gopalakrishnan et al., 2010; Jain, 2010; Ramakant and Singh, 2012; Sharma et al., 2012). This was the first ever case of MH in our institution in 25 years This emphasises the need to ensure availability of Dantrolene and MH kits, diagnostic centres with appropriate lab controls and above all high index of suspicion and heightened awareness among the anaesthesiologists of this potential albeit rare risk of MH in Indian population.

Background: Stress being high among practicing anaesthesiologists has effects on the quality of life. Methods to mitigate the stress have to be ensured to achieve job satisfaction. Methods: A survey was conducted through a questionnaire to know the various aspects of job satisfaction and job stress. The results of the data obtained were analyzed. Results: An anaesthetists work area may vary from a small private hospital to a large tertiary centre.Depending on the number of anaesthetists in a particular hospital, the working hours and on call duties would be distributed. Overworked anaesthetists are prone to burnout due to sleep deprivation. This could lead to fatigue related error. Lesser the number of anaesthetists would mean less support from colleagues in the event of complications. Having a good rapport with surgical colleagues also helps to prevent stress.Anaesthesiologists should have adequate monitors to avoid error in judgement. Chronic stress has serious health hazards. Keeping updated with latest developments in our field helps to improve the quality of care provided. Anaesthetists should also receive the recognition and remuneration due to them. Conclusion: To improve the quality of care provided to a patient,anaesthesiologists must cope with job stress. An anaesthetist must enjoy the work rather than be burdened by it.

Abstract

Background: \ud The SARS-CoV-2 delta (B.1.617.2) variant was first detected in England in March, 2021. It has since rapidly become the predominant lineage, owing to high transmissibility. It is suspected that the delta variant is associated with more severe disease than the previously dominant alpha (B.1.1.7) variant. We aimed to characterise the severity of the delta variant compared with the alpha variant by determining the relative risk of hospital attendance outcomes.\ud \ud Methods: \ud This cohort study was done among all patients with COVID-19 in England between March 29 and May 23, 2021, who were identified as being infected with either the alpha or delta SARS-CoV-2 variant through whole-genome sequencing. Individual-level data on these patients were linked to routine health-care datasets on vaccination, emergency care attendance, hospital admission, and mortality (data from Public Health England's Second Generation Surveillance System and COVID-19-associated deaths dataset; the National Immunisation Management System; and NHS Digital Secondary Uses Services and Emergency Care Data Set). The risk for hospital admission and emergency care attendance were compared between patients with sequencing-confirmed delta and alpha variants for the whole cohort and by vaccination status subgroups. Stratified Cox regression was used to adjust for age, sex, ethnicity, deprivation, recent international travel, area of residence, calendar week, and vaccination status.\ud \ud Findings: \ud Individual-level data on 43 338 COVID-19-positive patients (8682 with the delta variant, 34 656 with the alpha variant; median age 31 years [IQR 17–43]) were included in our analysis. 196 (2·3%) patients with the delta variant versus 764 (2·2%) patients with the alpha variant were admitted to hospital within 14 days after the specimen was taken (adjusted hazard ratio [HR] 2·26 [95% CI 1·32–3·89]). 498 (5·7%) patients with the delta variant versus 1448 (4·2%) patients with the alpha variant were admitted to hospital or attended emergency care within 14 days (adjusted HR 1·45 [1·08–1·95]). Most patients were unvaccinated (32 078 [74·0%] across both groups). The HRs for vaccinated patients with the delta variant versus the alpha variant (adjusted HR for hospital admission 1·94 [95% CI 0·47–8·05] and for hospital admission or emergency care attendance 1·58 [0·69–3·61]) were similar to the HRs for unvaccinated patients (2·32 [1·29–4·16] and 1·43 [1·04–1·97]; p=0·82 for both) but the precision for the vaccinated subgroup was low.\ud \ud Interpretation: \ud This large national study found a higher hospital admission or emergency care attendance risk for patients with COVID-19 infected with the delta variant compared with the alpha variant. Results suggest that outbreaks of the delta variant in unvaccinated populations might lead to a greater burden on health-care services than the alpha variant.\ud \ud Funding: \ud Medical Research Council; UK Research and Innovation; Department of Health and Social Care; and National Institute for Health Research.

Abstract

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infections were rising during early summer 2021 in many countries as a result of the Delta variant. We assessed reverse transcription polymerase chain reaction swab positivity in the Real-time Assessment of Community Transmission–1 (REACT-1) study in England. During June and July 2021, we observed sustained exponential growth with an average doubling time of 25 days, driven by complete replacement of the Alpha variant by Delta and by high prevalence at younger, less-vaccinated ages. Prevalence among unvaccinated people [1.21% (95% credible interval 1.03%, 1.41%)] was three times that among double-vaccinated people [0.40% (95% credible interval 0.34%, 0.48%)]. However, after adjusting for age and other variables, vaccine effectiveness for double-vaccinated people was estimated at between ~50% and ~60% during this period in England. Increased social mixing in the presence of Delta had the potential to generate sustained growth in infections, even at high levels of vaccination.

Abstract

Background Castleman’s disease is a rare lymphoproliferative disease which can mimic other malignant tumours and can be associated with paraneoplastic pemphigus especially in the paediatric age group. This presentation can be challenging even to the seasoned anaesthesiologist when it comes to managing such a case presenting for laparotomy. Case presentation Here such a case in a 12-year-old who was initially diagnosed as retroperitoneal sarcoma complicated with paraneoplastic pemphigus and inferior venacaval thrombus is presented and its anaesthetic management and challenges are discussed. Special care needs to be taken while anaesthetizing such a patient keeping in mind the difficult airway, adrenocortical suppression, and exacerbation of skin lesions due to various anaesthetic procedures. For this case, the histopathology turned out to be Castleman’s disease. Conclusion The anaesthetic challenges associated with paraneoplastic pemphigus in paediatric age group receiving perioperative thromboprophylaxis need special mention. Here we are discussing the challenges and anaesthetic management of such a case as literature on this aspect is few.

The primary aim was to compare the effect of IT and IV lignocaine on attenuation of airway reflex to endotracheal extubation and the secondary aim was to compare the hemodynamic responses to extubation, using lignocaine by the two different routes.Seventy-five female patients with comparable age, body mass index, and American Society of Anesthesiologists Physical Status undergoing carcinoma breast surgery were randomized into three groups. Group A received 2% lignocaine 3 mg/kg intratracheally 5 min and Group B received 2% lignocaine 1.5 mg/kg intravenously 3 min before extubation. Group C was control group. The airway and hemodynamic responses were noted in terms of episodes of cough during emergence and extubation. Categorical variables assessed using Fisher's exact test and continuous variables assessed using one-way analysis of variance.Cough suppression was present in Groups A and B, with better results observed with IT than with IV lignocaine. In the control group, Grade III cough reflex was present predominantly. There was a statistically significant difference (p0.001) in blood pressure and heart rate between Group A versus Group C and in Group B versus Group C, but not between Group A and Group B.IT lignocaine administered before extubation significantly attenuates post-extubation cough reflex than IV lignocaine. Both IT and IV lignocaine can effectively attenuate the airway and hemodynamic response to extubation.

Abstract

Global dispersal and increasing frequency of the SARS-CoV-2 spike protein variant D614G are suggestive of a selective advantage but may also be due to a random founder effect. We investigate the hypothesis for positive selection of spike D614G in the United Kingdom using more than 25,000 whole genome SARS-CoV-2 sequences. Despite the availability of a large dataset, well represented by both spike 614 variants, not all approaches showed a conclusive signal of positive selection. Population genetic analysis indicates that 614G increases in frequency relative to 614D in a manner consistent with a selective advantage. We do not find any indication that patients infected with the spike 614G variant have higher COVID-19 mortality or clinical severity, but 614G is associated with higher viral load and younger age of patients. Abstract

Introduction: Left ventricular false tendons (LVFTs) are discrete, fibromuscular structures of varying length and thickness that traverse the left ventricular (LV) cavity. The aim of the present study was to describe the echocardiographic characteristics and associations of 100 consecutive cases of LVFTs that presented to the echocardiography laboratory at a tertiary care teaching hospital in India. Subjects and Methods: One hundred consecutive patients with LVFTs were studied. They were classified based on the number (single and multiple), morphology (simple and complex), and location (apical, mid, and basal). The individuals were divided into three groups (Group 1: with LV dilatation and/or LV hypertrophy [LVH], Group 2: abnormal echocardiogram other than Group 1, and Group 3: normal echocardiogram). Analysis was done using SPSS 15 software. Results: A total of 121 LVFTs were identified in the 100 individuals. The LVFTs were more commonly single (81%), simple (92.56%), and apically located (52.90%). Complex LVFTs and apical location were more common among males, while multiple LVFTs and mid location were more common among females. LVFTs were most commonly visualized in the apical four-chamber view. There was a preponderance of males (67%), patients with structural heart disease (79%), and patients with LVH and/or dilatation (44%) in the study group. Conclusions: LVFTs are readily identifiable on echocardiography and can be characterized based on their number, location, and morphology. Their relationship to structurally abnormal hearts, male gender, LVH, and dilatation requires further study.

Abstract

Background and Aims: The aim of the study was to evaluate the efficacy and usefulness of the current practice of various investigations and consultations being done during preanesthetic evaluation in patients undergoing oncosurgical procedures in our hospital. We also evaluated the delay caused due to these and its value in predicting postoperative complications. Material and Methods: The preanesthetic charts of 300 elderly patients >65 years of age were reviewed, and the incidence of abnormal investigations and number of consultations advised were noted. The incidence and predictive values of these were assessed. Results: More than half the number of patients had more than one comorbidity and were advised various consultations based on history, National Institute of Clinical and Health Excellence (NICE) guidelines, and institutional protocol. Multiple visits to preanesthetic clinic were required in patients who had abnormal thyroid tests or respiratory complaints which was the main reason for delay in scheduling surgery. However, despite multiple comorbidities not more than 12.7% of the blood tests ordered were found to be abnormal. Abnormal blood tests were not significantly associated with higher incidence of postoperative complications. Conclusion: Blood investigations do not predict postoperative complication rate and do not influence anesthetic management of elderly patients undergoing oncosurgical procedures but are rather influenced by surgical procedure and presence of comorbidities. Hence, preanesthetic clinic should assess patients based on other predictive tests rather than relying on blood investigations alone.

Abstract

Background: Numerous factors affect the risk of recurrence and metastasis after cancer surgery. Studies have observed that anaesthetic techniques have effects on tumour recurrence. Methods: Medical records of newly diagnosed ovarian serous adenocarcinoma patients who underwent radical hysterectomy with bilateral salpingoopherectomy from 1995-2008 were analysed for the effect of anaesthetic techniques and drugs on tumour recurrence & metastasis free survival rate and mortality rate. Univariate association between overall survival and anaesthesia technique was assessed using Kaplan-Meier survival estimates and Cox regression. Multivariate association was tested after adjusting potential confounding factors . Results: The overall survival rate (RR at 95% CI=3.16(1.79-5.60) was significantly better in patients who received regional anaesthesia for surgery than those who had general anaesthesia. Other factors significantly associated with overall survival rate in univariable analysis were,perioperative blood transfusion,preoperative Ca 125 level, FIGO stage, tumour size and lymphatic metastasis.Kaplan Meier survival curve showed that regional anesthesia group had higher overall survival rate.Recurrence rate did not show significant difference in univariable(Odds 95% CI 1.42 P = 0.273)and multivariable(Odds 95% CI = 0 P = 0.846) analysis.Al1the 18 patients who had metastasis underwent surgery under GA. Conclusions: This study showed marked increase in overall survival rate in patients who underwent surgery under regional anaesthesia when compared to those who had surgery under general anaesthesia. Prospective randomized control trials are needed for better evaluation.

Abstract

Background and Aims : With the availability of modern workstations and heightened awareness on the environmental effects of waste anaesthesia gases, anaesthesiologists worldwide are practicing low flow anaesthesia (LFA). Although LFA is being practiced in India, hard evidence on the current practice of the same from anaesthesiologists practicing in India is lacking and hence, we conducted this survey. Methods : A questionnaire containing 16 questions was distributed among a subgroup of anaesthesiologists who attended the 2014 National Conference of Indian Society of Anaesthesiologists. Abstract

The advancement in medical research and intelligent modeling techniques has lead to the developments in anaesthesia management. The present study is targeted to estimate the depth of anaesthesia using cognitive signal processing and intelligent modeling techniques. The neurophysiological signal that reflects cognitive state of anaesthetic drugs is the electroencephalogram signal. The information available on electroencephalogram signals during anaesthesia are drawn by extracting relative wave energy features from the anaesthetic electroencephalogram signals. Discrete wavelet transform is used to decomposes the electroencephalogram signals into four levels and then relative wave energy is computed from approximate and detail coefficients of sub-band signals. Relative wave energy is extracted to find out the degree of importance of different electroencephalogram frequency bands associated with different anaesthetic phases awake, induction, maintenance and recovery. The Kruskal-Wallis statistical test is applied on the relative wave energy features to check the discriminating capability of relative wave energy features as awake, light anaesthesia, moderate anaesthesia and deep anaesthesia. A novel depth of anaesthesia index is generated by implementing a Adaptive neuro-fuzzy inference system based fuzzy c-means clustering algorithm which uses relative wave energy features as inputs. Finally, the generated depth of anaesthesia index is compared with a commercially available depth of anaesthesia monitor Bispectral index.

Abstract

Background: Propofol is widely used for the induction of anesthesia. Sevoflurane, a popular inhalational induction agent for pediatric patients, may also be used in adults with needle phobias. Aims and Objectives: This study was done to compare the two popular induction agents, namely, propofol and sevoflurane. The speed of achieving the conditions ideal for intubation, in the background of the synergistic action of sevoflurane with vecuronium, the non-depolarizing muscle relaxant was also studied. Materials and Methods: In this prospective randomized controlled study, female patients between 20 and 65 years of age belonging to the American Society of Anaesthesiologists Grade I and II who underwent elective surgeries for breast cancer under general anesthesia were allocated randomly into two groups. Group 1 patients received propofol as the induction agent and in Group 2 sevoflurane was used as the induction agent. Induction with vital capacity breath technique using 8% sevoflurane was compared with standard induction with propofol. Results: It was found that propofol caused a greater drop in blood pressure (BP), although the time for induction was marginally lower (insignificant) with propofol. Sevoflurane potentiated the effect of vecuronium. One case in the sevoflurane group developed laryngospasm and hence was excluded from the study. Three patients in the propofol group had pain on injection. One patient from each group had abnormal transient groaning sound on induction and two patients from sevoflurane group had coughing on induction. Conclusion: Propofol and sevoflurane are equally fast in inducing anesthesia. Sevoflurane potentiates the action of non-depolarizing muscle relaxants when compared to propofol during induction and may be considered as a better option for patients in whom fall in BP is unlikely to be tolerated even for a short period.

Abstract

Acute onset of severe pain in cancer patients may be due to multiple causes. Irrespective of the etiology, adequate analgesia has to be provided as quickly as possible. The standard practices of relieving pain by using syringe pumps (syringe drivers) or infusion pumps may not be feasible in resource-scarce developing nations where many cancer patients first present at advanced stages of disease for management. This study compared the efficacy of the subcutaneous and intravenous routes of morphine administration continuously using a simple and economic technique for cancer pain management. Both routes were found to be equally effective in producing good analgesia without side effects. The drip method is a cost-effective way of providing subcutaneous morphine infusion for cancer patients and is applicable for both inpatients and home care.
