BACKGROUND: There is modest evidence that exercise referral schemes increase physical activity in inactive individuals with chronic health conditions. There is a need to identify additional ways to improve the effects of exercise referral schemes on long-term physical activity. OBJECTIVES: To determine if adding the e-coachER intervention to exercise referral schemes is more clinically effective and cost-effective in increasing physical activity after 1 year than usual exercise referral schemes. DESIGN: A pragmatic, multicentre, two-arm randomised controlled trial, with a mixed-methods process evaluation and health economic analysis. Participants were allocated in a 1 : 1 ratio to either exercise referral schemes plus e-coachER (intervention) or exercise referral schemes alone (control). SETTING: Patients were referred to exercise referral schemes in Plymouth, Birmingham and Glasgow. PARTICIPANTS: There were 450 participants aged 16-74 years, with a body mass index of 30-40 kg/m2, with hypertension, prediabetes, type 2 diabetes, lower limb osteoarthritis or a current/recent history of treatment for depression, who were also inactive, contactable via e-mail and internet users. INTERVENTION: e-coachER was designed to augment exercise referral schemes. Participants received a pedometer and fridge magnet with physical activity recording sheets, and a user guide to access the web-based support in the form of seven 'steps to health'. e-coachER aimed to build the use of behavioural skills (e.g. self-monitoring) while strengthening favourable beliefs in the importance of physical activity, competence, autonomy in physical activity choices and relatedness. All participants were referred to a standard exercise referral scheme. PRIMARY OUTCOME MEASURE: Minutes of moderate and vigorous physical activity in ≥ 10-minute bouts measured by an accelerometer over 1 week at 12 months, worn ≥ 16 hours per day for ≥ 4 days including ≥ 1 weekend day. SECONDARY OUTCOMES: Other accelerometer-derived physical activity measures, self-reported physical activity, exercise referral scheme attendance and EuroQol-5 Dimensions, five-level version, and Hospital Anxiety and Depression Scale scores were collected at 4 and 12 months post randomisation. RESULTS: Participants had a mean body mass index of 32.6 (standard deviation) 4.4 kg/m2, were referred primarily for weight loss and were mostly confident self-rated information technology users. Primary outcome analysis involving those with usable data showed a weak indicative effect in favour of the intervention group (n = 108) compared with the control group (n = 124); 11.8 weekly minutes of moderate and vigorous physical activity (95% confidence interval -2.1 to 26.0 minutes; p = 0.10). Sixty-four per cent of intervention participants logged on at least once; they gave generally positive feedback on the web-based support. The intervention had no effect on other physical activity outcomes, exercise referral scheme attendance (78% in the control group vs. 75% in the intervention group) or EuroQol-5 Dimensions, five-level version, or Hospital Anxiety and Depression Scale scores, but did enhance a number of process outcomes (i.e. confidence, importance and competence) compared with the control group at 4 months, but not at 12 months. At 12 months, the intervention group incurred an additional mean cost of £439 (95% confidence interval -£182 to £1060) compared with the control group, but generated more quality-adjusted life-years (mean 0.026, 95% confidence interval 0.013 to 0.040), with an incremental cost-effectiveness ratio of an additional £16,885 per quality-adjusted life-year. LIMITATIONS: A significant proportion (46%) of participants were not included in the primary analysis because of study withdrawal and insufficient device wear-time, so the results must be interpreted with caution. The regression model fit for the primary outcome was poor because of the considerable proportion of participants [142/243 (58%)] who recorded no instances of ≥ 10-minute bouts of moderate and vigorous physical activity at 12 months post randomisation. FUTURE WORK: The design and rigorous evaluation of cost-effective and scalable ways to increase exercise referral scheme uptake and maintenance of moderate and vigorous physical activity are needed among patients with chronic conditions. CONCLUSIONS: Adding e-coachER to usual exercise referral schemes had only a weak indicative effect on long-term rigorously defined, objectively assessed moderate and vigorous physical activity. The provision of the e-coachER support package led to an additional cost and has a 63% probability of being cost-effective based on the UK threshold of £30,000 per quality-adjusted life-year. The intervention did improve some process outcomes as specified in our logic model. TRIAL REGISTRATION: Current Controlled Trials ISRCTN15644451. Commander in Chief is een deelbiografie van president Franklin Delano Roosevelt waarin Roosevelts rol als opperbevelhebber van de gewapende strijdkrachten van de Verenigde Staten in de Tweede Wereldoorlog het hoofdthema is. Het boek concentreert zich op het jaar 1943: een jaar waaraan biografen van president Roosevelt tot nu toe weinig aandacht schonken, en waarin Roosevelt een militaire taak vervulde die door historici ernstig is onderschat. Het proefschrift begint in januari 1943 met de vliegreis van Roosevelt van Miami naar Noord-Afrika – daarmee was Roosevelt de eerste president van de Verenigde Staten die in functie vloog. Een nieuwe datering van Roosevelts voorbereiding voor de reis stelt ons in staat het doel van de president met deze reis opnieuw te onderzoeken, niet alleen in het als Amerikaans opperbevelhebber inspecteren van zijn troepen in het strijdperk, maar ook in het uitrollen van een nieuw militair doel voor alle westelijke geallieerden op de Conferentie van Casablanca: de ‘onvoorwaardelijke overgave’ van de asmogendheden, zonder mogelijkheid tot onderhandeling. Roosevelts beslissing om zijn eigen gezamenlijke Amerikaanse legerbevelhebbers (George Marshall, Ernest King, Henry Arnold) te overrulen en een Mediterrane veldtocht in 1943 uit te voeren, in plaats van een verplaatsing in gang te zetten van Amerikaanse strijdkrachten naar Groot-Brittannië voor een invasie door een oversteek van het Kanaal, zoals zij aanbevalen, wordt ook nader bekeken. In tegenstelling tot contemporaine en ook latere beweringen dat Roosevelt in Casablanca ‘toegaf’ aan Churchills strategie, maakt Commander in Chief duidelijk dat Roosevelt een duidelijk en onwrikbaar Amerikaans doel voor ogen had: dat de Amerikaanse strijdkrachten eerst in een ‘veilige’ omgeving de mogelijkheid zouden krijgen om te leren hoe ze de Wehrmacht in een oorlog konden verslaan, voordat zoiets ambitieus als een invasie door een oversteek van het Kanaal dat jaar ondernomen kon worden. De nederlaag van het Amerikaaanse leger in de Slag om de Kasserinepas, drie weken na de Conferentie, toonde de wijsheid van Roosevelts militaire stap-voor-stap-strategie aan – de Amerikaanse strijdkrachten konden daarop in mei terugslaan en de onvoorwaardelijke overgave afdwingen van meer dan een kwart miljoen manschappen van de asmogendheden in Noord-Afrika. “Husky”, de Landing op Sicilië, vond twee maanden later op 10 juli 1943plaats, in overeenstemming met het tijdschema van de president: een reusachtige amfibische operatie door de geallieerde legers waardoor Hitler gedwongen werd Operatie Citadel, Duitslands twee miljoen man sterke offensief tegen de Sovjets aan het oostelijk front, af te breken om het zuidelijk front te kunnen verdedigen. Op dat moment hadden de geallieerden echter al de Middellandse Zee in hun macht, en de strijd leverde bevelhebbers (Patton, Bradley, Montgomery) en door strijd geharde legers (luchtmacht, marine, grondtroepen) op die noodzakelijk waren om in de lente van 1944 een succesvolle invasie (Overlord) op Noord-Frankrijk uit te voeren: met landingen die, zoals Hitler zelf verklaarde, de uitkomst van de oorlog zouden bepalen. Door in te stemmen met de militaire strategie van de president verwees de Britse premier Winston Churchill publiekelijk naar zichzelf als president Roosevelts ‘vurige luitenant’. Commander in Chief werpt echter nieuw licht op hoe Churchill achter de schermen – gesteund door een klein leger van adviseurs – het hele jaar door probeerde Roosevelts militair-strategische beslissingen tegen te werken: hij reisde in 1943 tweemaal over zee naar Noord-Amerika – in mei en in augustus – om Roosevelt ervan te overtuigen Overlord af te blazen, of de landingen uit te stellen tot 1945 of 1946, om zo de operatie in het Middellandse Zee-gebied voort te kunnen zetten. Dit fundamentele verschil in militaire strategie tussen de twee belangrijkste westelijke oorlogsleiders, Churchill en Roosevelt, verklaart de ondertitel van het proefschrift: FDR’s Battle With Churchill, 1943 (FDR’s strijd met Churchill, 1943). Wanneer president Roosevelt niet zijn jongere Britse collega had teruggefloten, dan zouden de geallieerden de oorlog in Europa heel goed verloren kunnen hebben – zoals de ongelukkige geallieerde invasie van Zuid-Italië en de Ionische Eilanden in de herfst van 1943 ook lieten zien. Door het gebruik van nieuwe bronnen, vooral dagboeken, persoonlijke aantekeningen en andere egodocumenten, biedt Commander in Chief een radicaal nieuw perspectief op de militaire prestaties van zowel Roosevelt als Churchill tijdens strategische momenten in de Tweede Wereldoorlog. Background: Promoting physical activity (PA) via primary care exercise referral schemes (ERS) is common but there is no rigorous evidence for long term changes in PA (Pavey et al, 2011) among those with chronic conditions. From July 2015, for 15 months, the e-coachER trial began to recruit 1400 patients (in SW England, Birmingham and Glasgow) with one or more chronic conditions including diabetes, obesity, hypertension, osteoarthritis, or depression, who are eligible and about to attend an ERS. The two-arm parallel RCT is powered to determine if the addition of a web-based, interactive, theory-driven and evidence-based support system called e-coachER (hosted on the ‘LifeGuide’ platform) will result in at least 10% more patients who do 150 mins or more per week of accelerometer assessed moderate or vigorous physical activity (MVPA) at 12 months. Recruitment into the trial is within primary care, using both mail-merged patient invitations and opportunistic GP invitations (and exercise referrals). Within the trial, after participants are screened, provide consent and complete baseline assessments, they are randomised to receive usual ERS at each site or usual ERS plus a mailed Welcome Pack with registration details to access e-coachER on-line. Inclusion criteria for entering the trial are: (1) Aged 16-74 years; (2) with one or more of the following: obesity (BMI 30-35), hypertension (SBP 140-179 or DBP 90-109, type 2 diabetes, lower limb osteoarthritis, recent history of treatment for depression; (3) Participants who are in the two lowest (of four) groups using the GP Physical Activity Questionnaire; (4) have an e-mail address and access to the internet; (5) Eligible for an ERS. The intervention rationale, design and content are reported in another presentation. Aims: This presentation will provide initial findings from a 3 month internal pilot phase with a focus on trial recruitment and initial intervention engagement. We will present data on the achievement of our targets for recruiting 180 participants across the three sites and proportion who initially go online to register and use e-coachER. We also aim, based on detailed process evaluation work, to present a summary of factors influencing recruitment and intervention engagement, and any adaptations needed for the recruitment process. Methods: Data is being captured to inform a CONSORT chart with details on number of patient invitations, number and reasons for trial exclusion, and numbers and characteristics of those recruited. The LifeGuide system will provide data on numbers/proportion assigned to the intervention arm who register and start to use e-coachER. Qualitative data on recruitment processes and intervention engagement, as part of the process evaluation, will be summarised. Results: Due to a number of factors recruitment began 2-4 months later than planned across the 3 sites. Data will be presented on achieved recruitment and e-coachER engagement relative to our targets in the internal pilot trial. Qualitative data will be presented on the acceptability and feasibility of the trial methods and intervention. Conclusion: A summary of what we have learned about recruitment and intervention engagement will be presented and ideas for improvement, as appropriate. Carrying out research about adult social care services for visually impaired people presents challenges that are not necessarily found in other fields. The purpose of this review is to draw attention to these challenges and to guide the researcher through them. It does so by drawing on the academic and grey literature. The review covers the ideological context of research in this field; definitions of visual impairment and their appropriate and inappropriate uses in research; misleading claims and reliable evidence about the size and characteristics of the visually impaired people and the reasons that these are important issues for research in the field. Challenges also cover the main topic areas of research and the methodological approaches, both quantitative and qualitative, that researchers have taken to deal with them. Issues common to both types of method cover sampling; the instruments used to collect data; the means of obtaining informed consent from visually impaired people, and organisations that can potentially assist researchers in this field.
