Background/Objectives: To assess dietary patterns (DPs) in European adolescents and to examine their relationship with healthy eating determinants.Subject/Methods: A total of 2205 European adolesce...

Few studies have evaluated the association between dietary free sugars intake (FSI) and obesity in adolescents. We examined the relation between FSI and their contributors from the main food groups and obesity in European adolescents. We included 843 adolescents (51.6% male) from the cross-sectional HELENA study with two completed 24 h recalls and anthropometric data. Linear mixed models were applied to investigate the relation between FSI and different anthropometric indices. Odds ratios for having a high body mass index (BMI) were also estimated by multilevel ordinal regression. Total FSI was higher in males than females (102.60 g and 87.58 g, respectively, p &lt, 0.001). No effect was observed between free sugar from the main food groups and BMI. Consumers of FSI from &ldquo, cakes, pies and biscuits&rdquo, in males (odd ratio (OR) = 0.455, 95% Confidence interval (CI) 0.251, 0.824) and from &ldquo, breakfast cereals&rdquo, in females had a lower probability of having obesity (OR = 0.423, 95%CI 0.204, 0.878), whereas females consuming FSI from &lsquo, fruit and vegetables juices&rsquo, had a higher probability of obesity (OR= 2.733, 95% CI 1.286, 5.810). This study provides no evidence that increased FSI is associated with obesity in adolescents. Further studies are needed to assess the longitudinal exposure to FSI and their effect on obesity development.

[Background]: Cardiovascular diseases (CVDs) are responsible for 31% of all deaths worldwide. Genetic predisposition to CVDs in adolescents remains largely unknown. The aim of this study was to examine the association of UCP1, UCP2 and UCP3 gene polymorphisms with CVD risk factors in European adolescents., [Method]: A cross-sectional study that involves 1.057 European adolescents (12–18 years old) from the HELENA study. A total of 18 polymorphisms of UCP1, UCP2 and UCP3 genes were genotyped. We measured serum total cholesterol, high-density lipoprotein,low-density lipoprotein, ApoA1, ApoB, leptin, triglycerides, glucose, insulin and blood pressure, and calculated HOMA (homeostatic model assessment), Quantitative Insulin Sensitivity Check Index (QUICKI) and a CVD risk score., [Results]: The G allele of UCP2 rs2735572 and T allele of UCP2 rs17132534 were associated with higher diastolic blood pressure (P = 0.001; false discovery rate [FDR] = 0.009 and P = 8e–04; FDR = 0.009, respectively). We observed that the AATAG haplotype of UCP1 was associated with higher serum ApoB/ApoA1 (P = 0.008; FDR = 0.031) and ApoB levels (P = 0.008; FDR = 0.031). Moreover, the ACC haplotype of UCP3 was associated with a higher CVD risk score (P = 0.0036; FDR = 0.01)., [Conclusions]: Two UCP2 polymorphisms and haplotypes of UCP1 and UCP3 were associated with CVD risk factors. Background:Detailed information on global individual-level consumption patterns is imperative for informed policy making. However, such data are dispersed and incomplete.Objective:To review and discuss the methodologies, observed data availability, challenges, and opportunities pertaining to global dietary surveillance.Methods:This investigation provides an extensive review of global dietary assessment methodologies and challenges, including at the survey level, the dietary collection and assessment level, and the dietary data processing and analysis level. The focus is on nationally representative individual-level data, and additional types of dietary data, such as dietary biomarkers, household assessment, and food availability, are reviewed as alternatives. Practical guidance is provided to inform key decisions when designing dietary surveys and collecting, analyzing, and using dietary data. This article further identifies and describes existing global and regional dietary initiatives/data sets.Results:Harmonized and standardized primary individual-level dietary data collection, processing, and analysis worldwide are currently not available. Evaluation and decision-making should be based on best available data, that is, secondary nonharmonized yet to the extent possible, standardized individual-level dietary data. Existing initiatives differ substantially in methodologies, including survey design/representativeness, coverage, diet assessment, and dietary metric standardization and processing. Data gaps have been identified that were more profound for certain countries, certain dietary indicators across countries, population subgroups, representativeness, or time periods.Conclusions:Optimizing worldwide dietary habits to improve population health requires systematically identified and evaluated data on a continuing basis. Leveraging existing available dietary data and efforts is an indispensable prerequisite for informed priority setting targeting the intersections between diet and disease.

Individual food consumption surveys (IFCS) are performed to evaluate compliance with food/nutrient intake requirements or exposure to potential harmful dietary contaminants/components. In this review, we inventoried methods and designs used in national IFCS and discussed the methodologies applied across countries. Literature searches were performed using fixed sets of search terms in different online databases. We identified IFCS in thirty-nine countries from six world continents. National IFCS systems are available in most of the high-income countries, while such surveys are scarce in low- and middle-income countries (e.g. Africa, Eastern Europe and several Asian countries). Few countries (n9) have their national IFCS incorporated into national health and nutrition surveys, allowing the investigation of dietary-related disease outcomes. Of the integrated surveys, most have the advantage of being continuous/regular, contrary to other IFCS that are mostly erratic. This review serves as the basis to define gaps and needs in IFCS worldwide and assists in defining priorities for resource allocation. In addition, it can serve as a source of inspiration for countries that do not have an IFCS system in place yet and advocate for national IFCS to be incorporated into national health and nutrition surveys in order to create: (1) research opportunities for investigating diet–disease relationships and (2) a frame to plan and evaluate the effect of diet-related policies (e.g. promotion of local nutrient-rich foods) and of nutrition recommendations, such as food-based dietary guidelines. Countries that integrate their IFCS within their national health and nutrition survey can serve as proof-of-principle for other countries.

Objective: The aim of this study is to define a healthy and sustainable diet model with low GHGE, fulfilling dietary requirements, and considering current Italian food consumption patterns. Design: A duly designed database was developed, linking food nutritional composition and GHGE based on 921 food items consumed in Italy according to the last national food consumption survey (INRAN-SCAI 2005-2006). Linear programming was used to develop new diet plans separately for males and females, aged 18-60 years (n = 2,098 subjects), in order to minimize GHGE. The program is based on dietary goals and acceptability constraints as well as on 13 nutrient requirement constraints aiming to reach a healthy and acceptable diet for the Italian population. Results: Diet optimization resulted in a nutritionally adequate pattern minimizing GHGE values (4.0 vs. 1.9 kg CO2e/day for males and 3.2 vs. 1.6 kg CO2e/day for females). In both sexes, the nutrient intake of the optimized diet was at the established lower bound for cholesterol and calcium and at the established upper bound for free sugar and fiber. In males, intake of zinc was at the established lower bound whereas iron was at the established upper bound. Consumption of red meat and fruit and vegetables was at the established lower and upper bound, respectively, in both males and females. Despite the decrease in meat consumption, especially red meat, in the optimized diet with respect to the observed diet, levels of iron intake in females increased by 10% (10.3 vs. 11.3 mg/day) but remained below the adequate intake established in Italian national DRIs. Conclusions: An attainable healthy dietary pattern was developed that would lead to the reduction of GHGE by 48% for males and by 50% for females with respect to current food consumption in the Italian adult population. Health-promoting dietary patterns can substantially contribute to achieve related Sustainable Development Goals.

OBJECTIVES: To systematically identify, compile, and harmonize national and sub-national individual-level dietary data around the world using a common food classification and description system (FCDS); promote collaboration with data owners worldwide; and publicly disseminate methods and datasets to the global nutrition community. METHODS: The Global Dietary Database (GDD) and the FAO/WHO Global Individual Food consumption data Tool (FAO/WHO GIFT) initiative, in collaboration with the European Food Safety Authority (EFSA), developed a framework to standardize the characterization of 24-hour recall and food record survey data at their finest level, using FoodEx2 (Figure). FoodEx2 is a comprehensive FCDS developed by EFSA, adapted for international high-quality food characterization and methodologically superior to prior international FCDS. Single food items are characterized through a classification system (hierarchical nested food groups) and 32 description elements (facets), such as cooking process. Standardized criteria, procedures, and templates were developed to: (1) identify and (2) prioritize eligible surveys for FoodEx2 mapping; (3) efficiently contact data owners and obtain approval for public dissemination; (4) allocate financial assistance as appropriate; (5) obtain survey information and assess survey and diet tool quality; (6) train data owners on dataset preparation and FoodEx2 mapping; (7) collect harmonized microdata, including structure, sociodemographics, diet metrics, units of measure, and food characterization; and (8) publicly disseminate methods and datasets via the GDD and FAO/WHO GIFT platforms. RESULTS: A total of 151 surveys have been identified as high priority. Harmonization has been completed for 24 European surveys by EFSA, which will be incorporated in the GDD and FAO/WHO GIFT platforms upon approval of data owners; 8 surveys by FAO/WHO GIFT; and is ongoing for another 4. Thus far, the data owners of 53 surveys from 41 countries, including Brazil, India, and Iran, confirmed interest in joining; all surveys either used none or a local FCDS. CONCLUSIONS: These new methods developed will provide the most comprehensive and standardized databases of dietary intakes worldwide. Knowing who eats what, understanding the various eating habits of different population groups, according to the geographical area, is critical to develop evidence-based policies for nutrition and food safety. The FAO/WHO Global Individual Food consumption data Tool (FAO/WHO GIFT) is a novel open-access online platform, hosted by FAO and supported by WHO, providing access to harmonised individual quantitative food consumption (IQFC) data, especially in low- and middle-income countries (LMIC). FAO/WHO GIFT is a growing repository, which will serve as the global FAO/WHO hub to disseminate IQFC microdata. Currently five datasets from LMIC are available for dissemination, and an additional fifty datasets will be made available by 2022. To facilitate the use of these data by policy makers, ready-to-use food-based indicators are provided for an overview of key data according to population segments and food groups. FAO/WHO GIFT also provides an inventory of existing IQFC data worldwide, which currently contains detailed information on 188 surveys conducted in seventy-two countries. In order for end-users to be able to aggregate the available data, all datasets are harmonised with the European Food Safety Authority's food classification and description system FoodEx2 (modified for global use). This harmonisation is aimed at enhancing the consistency and reliability of nutrient intake and dietary exposure assessments. FAO/WHO GIFT is developed in synergy with other global initiatives aimed at increasing the quality, availability and use of IQFC data in LMIC to enable evidence-based decision-making and policy development for better nutrition and food safety.

On behalf of the HELENA study group: et al., [Objectives]: To determine dietary patterns (DPs) and explain the highest variance of vitamin B6, folate, and B12 intake and related concentrations among European adolescents., [Methods]: A total of 2173 adolescents who participated in the Healthy Lifestyle in Europe by Nutrition in Adolescence study met the eligibility criteria for the vitamin B intake analysis (46% boys) and 586 adolescents for the biomarkers analysis (47% boys). Two non-consecutive, 24-h, dietary recalls were used to assess the mean intakes. Concentrations were measured by chromatography and immunoassay testing. A reduced rank regression was applied to elucidate the combined effect of food intake of vitamin B and related concentrations., [Results]: The identified DPs (one per vitamin B intake and biomarker and by sex) explained a variability between 34.2% and 23.7% of the vitamin B intake and between 17.2% and 7% of the biomarkers. In the reduced rank regression models, fish, eggs, cheese, whole milk and buttermilk intakes were loaded positively for vitamin B intake in both sexes; however, soft drinks and chocolate were loaded negatively. For the biomarkers, a higher variability was observed in the patterns in terms of food loads such as alcoholic drinks, sugars, and soft drinks. Some food items were loaded differently between intakes and biomarkers such as fish products, which was loaded positively for intakes but negatively for plasma folate in girls., [Conclusions]: The identified DPs explained up to 34.2% and 17.2% of the variability of the vitamin B intake and plasma concentrations, respectively, in European adolescents. Objective: To carry out an inventory on the availability, challenges, and needs of dietary assessment (DA) methods in Africa as a pre-requisite to provide evidence, and set directions (strategies) for implementing common dietary methods and support web-research infrastructure across countries. Methods: The inventory was performed within the framework of the " Africa's Study on Physical Activity and Dietary Assessment Methods" (AS-PADAM) project. It involves international institutional and African networks. An inventory questionnaire was developed and disseminated through the networks. Eighteen countries responded to the dietary inventory questionnaire. Results: Various DA tools were reported in Africa; 24-Hour Dietary Recall and Food Frequency Questionnaire were the most commonly used tools. Few tools were validated and tested for reliability. Face-to-face interview was the common method of administration. No computerized software or other new (web) technologies were reported. No tools were standardized across countries. Conclusions: The lack of comparable DA methods across represented countries is a major obstacle to implement comprehensive and joint nutrition-related programmes for surveillance, programme evaluation, research, and prevention. There is a need to develop new or adapt existing DA methods across countries by employing related research infrastructure that has been validated and standardized in other settings, with the view to standardizing methods for wider use.

An increasing number of low-income countries (LICs) exhibit high rates of malnutrition coincident with rising rates of overweight and obesity. Individual-level dietary data are needed to inform effective responses, yet dietary data from large-scale surveys conducted in LICs remain extremely limited. This discussion paper first seeks to highlight the barriers to collection and use of individual-level dietary data in LICs. Second, it introduces readers to new technological developments and research initiatives to remedy this situation, led by the International Dietary Data Expansion (INDDEX) Project. Constraints to conducting large-scale dietary assessments include significant costs, time burden, technical complexity, and limited investment in dietary research infrastructure, including the necessary tools and databases required to collect individual-level dietary data in large surveys. To address existing bottlenecks, the INDDEX Project is developing a dietary assessment platform for LICs, called INDDEX24, consisting of a mobile application integrated with a web database application, which is expected to facilitate seamless data collection and processing. These tools will be subject to rigorous testing including feasibility, validation, and cost studies. To scale up dietary data collection and use in LICs, the INDDEX Project will also invest in food composition databases, an individual-level dietary data dissemination platform, and capacity development activities. Although the INDDEX Project activities are expected to improve the ability of researchers and policymakers in low-income countries to collect, process, and use dietary data, the global nutrition community is urged to commit further significant investments in order to adequately address the range and scope of challenges described in this paper.

On behalf of the HELENA project group., Various definitions are used to define metabolic syndrome in adolescents. This study aimed to compare, in terms of prevalence and differences, five frequently used definitions for this population: International Diabetes Federation, National Cholesterol Education Program Adult Treatment Panel III (NCEP-ATP) modified by Cook, pediatric American Heart Association (AHA), World Health Organization, and Jolliffe and Janssen. A sample of 1004 adolescents (12.5–17.0 years) from the Healthy Lifestyle in Europe by Nutrition in Adolescence (HELENA) study was considered. The components of the definitions (waist circumference/BMI, plasma lipids, glycemia, and blood pressure) were applied, and definitions were compared by using crosstabs, sensitivity, specificity, and kappa coefficient. The prevalence of metabolic syndrome varied from 1.6 to 3.8% depending on the used definitions. Crosstabs comparing the definitions showed the fewest cases being misclassified (having metabolic syndrome or not) between NCEP-ATP and AHA. Analyses for kappa coefficient, sensitivity, and specificity confirmed this finding. [Conclusion]: The different definitions do not classify the same adolescents as having MS and prevalence varied between diagnostic methods. Flavouring substances may be naturally present in a food and/or intentionally added. It is essential to have a method to estimate exposure that is rapidly performed and does not underestimate exposure. Recently, a new approach for flavouring, the “Added Portions Exposure Technique” (APET), was proposed by the European Food Safety Authority. This paper explains step by step this technique using furaneol as case study. APET's main advantage is that it allows the assessment of dietary exposure in adults, children and infants, from both added flavourings and from flavourings naturally contained in food, and considering both acute and chronic exposure.

Dietary exposure assessment to flavourings presents a challenge due to the very large number of food products in which they may be present and the lack of knowledge of their actual presence. Industry claims to be adding more than 2800 different chemically defined flavourings in the European Union (EU). Within the Flavours, Additives and Food Contact Material Exposure Task (FACET) EU project, an occurrence survey was performed to explore the possible use of the information that can be collected from the labels to reduce the uncertainty in exposure assessment. An ad hoc procedure based on the use of market share data of food and drinks was developed for the selection of products to be collected. A database of information retrieved from the food labels of branded products from large and small companies was created. Such information may be used to populate some databases of the software developed within the FACET project to assess total dietary exposure to flavourings.

On behalf of the HELENA Study Group., [EN]: Objective: To assess whether adolescents with high body mass index (BMI), or fat mass index (FMI), in combination with insulin resistance (assessed with the Homeostatic Model Assessment [HOMA] index), had also lower blood vitamin B, folate and vitamin B concentrations. Methods and materials: Six hundred and fifteen adolescents from the Healthy Lifestyle in Europe by Nutrition in Adolescence (HELENA) study, with data on B-vitamins (both intakes and status), and BMI, FMI, HOMA, were selected. Intakes were assessed by two non-consecutive 24-h recalls. B-vitamins biomarkers were measured by chromatography and immunoassay. Analysis of covariance was applied to elucidate the differences in B-vitamins between combinations of groups defined according to the median of the z-scores of markers of body composition and insulin sensitivity. Results: When considering energy intakes and education of the mother in the model, in females, vitamin B intakes were higher in the high BMI/ high HOMA group than in the high BMI-low HOMA group. Similarly, vitamin B intakes were higher in the high FMI/high HOMA group than in the low FMI/low HOMA group. Plasma vitamin B was significantly lower in males in the high FMI/high HOMA group than in the low FMI/low HOMA group, keeping also significant their trends throughout the groups, a fact that can be observed also for females (p < 0.05). Conclusion: Adolescents with combined higher adiposity and higher HOMA insulin sensitivity showed lower vitamin B plasma concentrations. These differences do not seem to be explained by dietary vitamin B intake., [ES]: Objetivo: Evaluar si los adolescentes con mayor índice de masa corporal (IMC), o de masa grasa (IMG), en combinación con la resistencia a la insulina (medida con el Modelo de Valoración Homeostática [índice HOMA]), ingieren y tienen valores más bajos de vitamina B6, folato y vitamina B12. Métodos y materiales: Seiscientos quince adolescentes participantes en el estudio Healthy Lifestyle in Europe by Nutrition in Adolescence (HELENA), con valores de ingesta y concentraciones de las vitaminas B6, folato y B12, e IMC, IMG y HOMA, fueron seleccionados. Las ingestas se midieron mediante dos recuerdos de 24 horas no consecutivos. Los biomarcadores de las vitaminas fueron obtenidos mediante cromatografía e inmunoensayo. Se aplicó el análisis de la covarianza para evaluar las diferencias entre las vitaminas (ingesta y concentraciones) entre las combinaciones de los grupos definidos según las medianas de los valores z de los marcadores de la composición corporal y de la sensibilidad a la insulina. Resultados: Considerando la ingesta energética y la educación de la madre en el modelo en chicas, las ingestas de vitamin B6 fueron mayores en el grupo de mayor IMC/mayor HOMA que en el grupo mayor IMC/menor HOMA. Del mismo modo, el grupo constituido por mayor IMG/mayor HOMA presentó mayores ingestas de esta vitamina que el grupo formado por la combinación entre menor IMG/menor HOMA. La vitamina B12 plasmática en chicos fue significativamente menor en el grupo formado por mayor IMG/mayor HOMA que en el grupo menor IMG/menor HOMA, manteniendo también la tendencia significativa en los grupos, lo que también se puede observar en las chicas (p < 0.05). Conclusiones: Los adolescentes con mayor adiposidad en combinación con una mayor sensibilidad a la insulina mostraron menores valores de vitamina B12 plasmática. Estas diferencias no parecen estar explicadas por diferencias en la ingesta de vitamina B12.

[Purpose]: To examine the underlying reasons for the positive relation between socio-economic status (SES) and the diet quality of adolescents. [Methods]: In 2081 adolescents (12.5–17.5 years) of the European HELENA study, a continuous variable on diet quality via 2-day 24-h recalls was available. SES was reflected by parental education, parental occupation and family affluence. Mediation by several psychosocial determinants was tested: self-efficacy, availability at school and home, social support, barriers, benefits, awareness and some self-reported influencers (parents, school, taste, health, friends, food readily available, easy preparation, hunger, price and habits). Multiple mediation analyses were adjusted for age, sex and country. [Results]: The availability of soft drinks and fruit at home, social support, parental influence, barriers, price influence, taste influence, health influence and food being readily available were significant mediators. The multiple mediation indirect effect accounted for 23–64% of the total effect. Both occupation and education and both maternal and paternal factors could be explained by the mediation. The unavailability of soft drinks was the strongest mediator (17–44% of the total effect). [Conclusion]: Up to 64% of the positive relation between SES and the diet quality in adolescence could be explained by several healthy eating determinants. Focusing on these factors in low-SES populations can minimize social inequalities in diet and health by improving the diet of these specific adolescents., The HELENA Study was carried out with the financial support of the European Community Sixth RTD Framework Programme (Contract FOODCT-2005-007034).

This study examined the correlates of dietary energy under-reporting (UR) and over-reporting (OV) in European adolescents. Two self-administered computerised 24-h dietary recalls and physical activity data using accelerometry were collected from 1512 adolescents aged 12·5–17·5 years from eight European countries. Objective measurements of height and weight were obtained. BMI was categorised according to Cole/International Obesity Task Force (IOTF) cut-off points. Diet-related attitudes were assessed via self-administered questionnaires. Reported energy intake (EI) was compared with predicted total energy expenditure to identify UR and OV using individual physical activity objective measures. Associations between misreporting and covariates were examined by multilevel logistic regression analyses. Among all, 33·3 % of the adolescents were UR and 15·6 % were OV when considering mean EI. Overweight (OR 3·25; 95 % CI 2·01, 5·27) and obese (OR 4·31; 95 % CI 1·92, 9·65) adolescents had higher odds for UR, whereas underweight individuals were more likely to over-report (OR 1·67; 95 % CI 1·01, 2·76). Being content with their own figures (OR 0·61; 95 % CI 0·41, 0·89) decreased the odds for UR, whereas frequently skipping breakfast (OR 2·14; 95 % CI 1·53, 2·99) was linked with higher odds for UR. Those being worried about gaining weight (OR 0·55; 95 % CI 0·33, 0·92) were less likely to OV. Weight status and psychosocial weight-related factors were found to be the major correlates of misreporting. Misreporting may reflect socially desirable answers and low ability to report own dietary intakes, but also may reflect real under-eating in an attempt to lose weight or real over-eating to reflect higher intakes due to growth spurts. Factors influencing misreporting should be identified in youths to clarify or better understand diet–disease associations.

L’analyse de la construction identitaire des individus dans les organisations offre un prolongement a la recherche sur les retributions du militantisme, laquelle s’est encore peu interessee aux transformations de l’image de soi et aux logiques d’emancipation qui sont a l’œuvre dans les carrieres militantes. S’il est vrai que l’engagement agit en permanence sur la perception de soi, ce « travail de l’identite » conditionne en retour le rapport au role militant, parfois dans le sens d’une reflexivite critique generatrice de defection. L’etude des « carrieres du desengagement » permet de cerner les mecanismes de ce processus et d’eclairer la question des incidences biographiques du militantisme sous l’angle de la modification, dans et par l’engagement, des determinants objectifs et subjectifs qui l’avaient initialement rendu possible. Cette reflexion, qui s’appuie sur le cas d’un ancien permanent du Parti communiste francais, montre comment les ressources accumulees pendant une phase d’investissement total peuvent « se retourner » contre l’engagement en produisant les conditions d’un accomplissement individuel hors l’institution.

Background and aims Italian National Food Consumption Survey, INRAN-SCAI 2005–06, is the third national food consumption survey performed in Italy. This study describes energy and nutrient intakes in Italy. Methods and results A national cross-sectional food consumption survey was conducted using consecutive 3-day food records between October 2005 and December 2006. A sample of 3323 males and females aged 0.1–97.7 years living in private households was investigated. Individual food records were converted into energy and nutrient intakes with the use of recently updated national food composition databases. For each subject, intakes of energy and of 27 nutrients were calculated, including six minerals (i.e., iron, calcium, phosphorus, magnesium, potassium and zinc) and 10 vitamins (i.e., thiamine, riboflavin, vitamin C, vitamin B 6 , retinol, β-carotene, vitamin A as retinol equivalents (REs), vitamin E, vitamin D and vitamin B 12 ). On average, 36% of calories appeared to derive from fat (11% from saturated fatty acids) and 45% from available carbohydrates (15% from soluble carbohydrates). Conclusions The results of the INRAN-SCAI 2005–06 survey in terms of nutrient intakes provide an important piece of information for nutrition surveillance of the population and may also be used to identify priorities for further research.

In 2009 competent organisations in the European Union provided the European Food Safety Authority (EFSA) with data from the most recent national dietary survey at the level of individuals' consumption. Twenty different Member States provided EFSA with data from 22 different national dietary surveys, with consumption figures for adults and, when available, for children. Member States' dietary data were assembled into the EFSA Comprehensive European Food Consumption Database. In this paper an overview of the methodologies and protocols employed in the different national dietary surveys is provided. Specifically, details about dietary assessment methods, interview administration, sampling design, portion size estimation, dietary software, evaluation of under-reporting and non-dietary information collected are described. This information is crucial to evaluate the level of accuracy of food consumption data and to anticipate and acknowledge the utmost important sources of heterogeneity of national databases included in the Comprehensive Database. The Comprehensive Database constitutes a unique resource for the estimation of consumption figures across the European Union and represents a useful tool to assess dietary exposure to hazardous substances and nutrient intake in Europe. Nevertheless, the many substantial methodological differences that characterise the Comprehensive Database are acknowledged and critically discussed.

Quantification of estragole content in commercial fennel herbal teas was carried out in order to allow for a more accurate estimate of the dietary exposure to estragole. A simple and rapid analytical method, based on Stir Bar Sorptive Extraction and GC-MS, was developed for this purpose. Fennel teas obtained from different types of commercial products were analysed. Concentration levels ranged from 241 to 2058 μg L(-1) in teas from teabags, from 9 to 912 μg L(-1) in diluted instant teas, from 251 to 1718 μg L(-1) in teas from not packaged seeds. Based on these data and considering the daily consumption of three portions of herbal tea, a maximum exposure to estragole for adults of 10 μg/kg bw/day was calculated. The relatively high level observed in diluted instant teas of some brands deserves attention since these products are designed for infant consumption. Estimated exposure in infants was up to 51 μg/kg bw/day for teas from teabags, and up to 23 μg/kg bw/day for instant teas. A generalization of the use of suitable technologies in production processes of instant teas could substantially reduce the exposure to estragole in the vulnerable population groups (infants, young children, pregnant and breastfeeding women) who consume these products.

A critical analysis of existing food consumption databases was performed with particular regard for their current and potential use for the assessment of dietary exposure to additives, flavourings and residues of food contact materials. Within the European Food Consumption Validation project (EFCOVAL), a questionnaire on critical aspects of such datasets was developed and administered to researchers responsible for the collection/analysis of national food consumption data in European countries. Information collected was complemented through a review of the literature and of grey publications in order to provide an inventory of the main food consumption surveys performed in Europe from 1994 to 2007, for a total of 23 countries and 37 surveys. It appeared that existing European food consumption surveys have as a main objective the assessment of nutrient intake in the population. On the other hand, most of the databases were shown to be used also for the purpose of dietary exposure assessment.

ObjectiveThe current paper aims to present the main results of the Italian National Food Consumption Survey INRAN-SCAI 2005–06.DesignA cross-sectional study was performed. Households were randomly selected after geographical stratification of the national territory. Food consumption was assessed on three consecutive days through individual estimated dietary records.SettingItaly.SubjectsThe final study sample comprised 3323 subjects (1501 males and 1822 females) aged 0·1 to 97·7 years belonging to 1329 households: fifty-two infants (0–2·9 years), 193 children (3–9·9 years), 247 teenagers (10–17·9 years), 2313 adults (18–64·9 years) and 518 elderly (65 years and above).ResultsParticipation rate was 33 %. The mean ratio of estimated energy intake to estimated BMR was 1·41 in adults. Indicators of mean and high individual consumption are presented for fifteen large categories and fifty-one subcategories of foods and beverages, in the total population and in consumers, by age and sex categories. The overall consumption of fruit and vegetables was 418 g/d. The consumption of red meat was approximately 700 g/week, expressed as raw weight. Some specific aspects of the Italian food consumption pattern were confirmed: a large contribution from bread, pasta and pizza to cereals, from olive oil to fats and from wine to alcoholic beverages.ConclusionsThe database obtained from the survey will be the key reference for Italian food consumption during the coming years and will be utilized for a variety of purposes including the assessment of nutrient intakes and risk analysis.

The aim of the present study was to assess the total daily energy intake of infants (aged 0-4 months) exclusively fed with infant formulae available on the European Union market. All of the products were identified from the European market share data of the year 2007 covering 22 countries. The calculated daily energy intake based on the instructions labelled on these products was compared with the energy requirements based on World Health Organization recommendations, considering the standard body weight established by the Food and Agriculture Organization. In addition, the daily energy intake was also calculated based on direct measurements of the amount of formulae contained in the spoon provided by the supplier. It was found that the kcal/day intakes provided by the supplier were higher than the values recommended by international organizations. The energy intake values calculated based on the direct spoon measurements were even higher.

To provide an overview of the Healthy Lifestyle in Europe by Nutrition in Adolescence Cross-Sectional Study (HELENA-CSS) design, with particular attention to its quality control procedures. Other important methodological aspects are described in detail throughout this supplement. Description of the HELENA-CSS sampling and recruitment approaches, standardization and harmonization processes, data collection and analysis strategies and quality control activities. The HELENA-CSS is a multi-centre collaborative study conducted in European adolescents located in urban settings. The data management systems, quality assurance monitoring activities, standardized manuals of operating procedures and training and study management are addressed in this paper. Various quality controls to ensure collection of valid and reliable data will be discussed in this supplement, as well as quantitative estimates of measurement error. The great advantage of the HELENA-CSS is the strict standardization of the fieldwork and the blood analyses, which precludes to a great extent the kind of immeasurable confounding bias that often interferes when comparing results from isolated studies.

In this study, the use of recombinant estrogen receptor alpha (ERalpha)-based affinity columns was reported, for the isolation and the identification of estrogenic substances present in complex matrices, focusing on bioactive compounds present in foodstuff. The capability of affinity columns to trap high, but also low-affinity radio-labelled ligands (17beta-estradiol, genistein and bisphenol A) was demonstrated. Three pooled samples of infant formulas (milk-based, hypoallergenic and soy-based formulas for infants aged 0-4 months) from a EU market basket were prepared by the CASCADE Network of Excellence. After determining the estrogenic activity of these food samples, human recombinant ERalpha ligand binding domain (LBD) based affinity columns combined with suitable analytical methods (high resolution LC-MS/MS) were used to identify the bioactive compounds present in the soy-based formula extract, namely phytoestrogens (genistein and daidzein) involved in the agonistic activity measured. Incubations of genistein with liver microsomes were carried out and the extracts analysed following the same protocol, demonstrating that hERalpha affinity columns can also be used for trapping active metabolites. This approach combining bioluminescent cell lines with this useful tool based on hERalpha-LBD affinity columns thus allowed the purification and the concentration of both known and unknown estrogenic ligands prior to investigation of their structure using LC-MS.
