1. Resting Vagally Mediated Heart Rate Variability is Associated with Financial Risk Preferences under Stress

Abstract

Business financial risk-taking usually occurs under conditions of stress. Biologically, the stress response has two components: one linked to the hypothalamic-pituitary-adrenal (HPA) axis and the other to the autonomic nervous system (ANS). The existing literature on financial risk-taking has primarily focused on the effects of cortisol, related to the HPA axis. This article, however, examines the influence of the ANS, as measured by vagally mediated heart rate variability at rest (VMHRV). A total of 121 participants (60 female) were divided based on whether their VMHRV was below the median. Participants were then randomly assigned to either a stress test group (tsst) or a control group. Financial risk preferences were assessed using an incentive-compatible 50-50% Eckel and Grossman task. Participants in the tsst with high VMHRV had a higher probability of choosing riskier lotteries compared to the other participants (P = 0,0189). This finding suggests that greater parasympathetic modulation enables individuals to make riskier financial decisions when under stress. Thus, the article contributes to the literature by demonstrating that individuals with a higher physiological capacity to cope with external stressors are less risk-averse in financial decisions under social stress conditions.

Abstract

In this work, the feasibility of using electrochemical remediation for treating municipal solid waste is explored. The viability of electrokinetic remediation is investigated prospectively to identify the electrochemical technology that can be applied to soil remediation using scientometrics, bibliometrics, and infometrics techniques. The study relies on Population, Intervention, Comparison, Outcomes, Context analysis to determine the research question, and Bibliometrix software helps build a theoretical background to search for solutions to soil contamination problems. Because electrokinetic remediation employs a significant amount of electrical energy, it is usually discarded as an applicable technique. However, this work emphasizes the studies where the technique works even in low-power situations. Thus, it can be said that this method serves as a backup for municipal soil waste landfill (MSWL) remediation in cases where none exists.

Abstract

A network of agronomists, researchers, and practitioners associated with cacao farming provided open access to their independent field trials across Latin America and the Caribbean (LAC). A centralized dataset was assembled using qualitative and quantitative data from 25 experimental field trials (hereafter referred to as “CacaoFIT”) spanning several LAC agroecosystems. This dataset was used to document the main traits and agroclimatic attributes of the cacao cultivation model being tested within the CacaoFIT network. By synthesizing data from an entire network of cacao trials, this study aimed to highlight specific design features and management practices that may contribute to better cacao farming sustainability. The CacaoFIT network comprises 200 ha of field trials testing over 150 cacao genotypes and set up under different shade canopy design, management, and research goals. Small-sized trials were common across Mesoamerica, whereas medium to large-size trials were distinct to South America. Cacao trials were 15 years old (on average) and ranged from 3 to 25 years of establishment. Most cacao trials were managed conventionally (i.e., 55%), while 20% were under organic practices, and the remaining 25% presented both conventional and organic management approaches. Most field trials (ca. 60%) planted an average of 10 international clones or national cultivars at high (1,230–1,500 plants ha−1) and medium density (833–1,111 plants ha−1). Mixed shade canopies were the dominant agroforestry model, while timber vs. leguminous shade canopies were also common. The diversity and depth of research domains examined across the CacaoFIT network varied widely. Agronomy and agroforestry topics dominated the research agenda across all trials, followed by environmental services domains. Cacao physiology and financial performance were researched to a lesser extent within the network. Five featured field trials from CacaoFIT offered technical guidelines to inform cacao farming within similar contexts. This collaborative work is a scaffold to encourage public–private partnerships, capacity building, and data sharing amongst cacao researchers across the tropics.

5. Disparities Persist Among Hispanic Patients: Completing Evaluation, Waitlisting, and Receiving a Kidney Transplant

Abstract

Background. Hispanic patients receive disproportionately fewer kidney transplants (KT) than non-Hispanic White (NHW) patients. In this observational study, we evaluated disparities in completing evaluation steps to KT among Hispanic patients. Methods. Using medical records of Hispanic and NHW patients initiating evaluation at 4 transplant centers from January 2011 to March 2020, we performed sequential Cox models to compare Hispanic versus NHW patients reaching each step of the evaluation process until receiving a KT. Results. Among all 5197 patients (Hispanic n = 2473; NHW n = 2724) initiating evaluation, Hispanic patients had 8% lower risk to be approved by the kidney pancreas (KP) committee than NHW patients (adjusted hazard ratio [aHR], 0.92; 95% confidence intervals (CI), 0.86-0.98; P = 0.015). Among 3492 patients approved by the KP committee, Hispanic patients had 13% lower risk to be waitlisted than NHW patients (aHR, 0.87; 95% CI, 0.81-0.94; P = 0.004). Among 3382 patients who were waitlisted, Hispanic patients had 11% lower risk than NHW patients to receive KT (aHR, 0.89; 95% CI, 0.81-0.97; P = 0.011). Among all patients initiating evaluation, Hispanic patients had a 16% lower risk than NHW patients to reach KT (aHR, 0.84; 95% CI, 0.76-0.92; P = 0.0002). Conclusions. Our study found that disproportionately fewer Hispanic patients were approved by the KP committee, were waitlisted, and received a KT, particularly a living donor kidney transplant, than NHW patients. Closer oversight of the evaluation process may help patients overcome challenges in access to KT.

Abstract

Agroforestry systems in the Ecuadorian Amazon play a vital role in environmental conservation and the promotion of sustainable agriculture. Therefore, it is crucial to demonstrate the benefits of the associated species within these production systems. This study aimed to assess the impact of agroforestry systems on cocoa yield, carbon sequestration, earthworm presence, and the nutritional contribution of companion species linked to cocoa (Theobroma cacao L.) cultivation under agroforestry systems. The research was conducted at INIAP’s Central Experimental Station of the Amazon using a randomized complete block design with three replications. The agroforestry arrangements were: (1) monoculture; (2) forest (Cedrelinga cateniformis Ducke); (3) fruit forest (Bactris gasipaes Kunth); (4) service (Erythrina poeppigiana (Walp.) O.F.Cook); and (5) forest + service (E. poeppigiana + C. cateniformis). The results indicated that agroforestry systems showed better results than the monoculture in terms of yield (532.0 kg ha−1 compared to 435.4 kg ha−1) and total stored carbon (33.0–42.0 t ha−1 compared to 39.6 t ha−1). Additionally, agroforestry systems provided higher levels of Mg, B, and Ca, contributing to both crop yield and the presence of earthworms. These findings suggest a positive influence of companion species, improving soil nutrition through biomass incorporation and promoting environmental benefits (carbon sequestration). Therefore, agroforestry systems will support sustainable cocoa production in the Ecuadorian Amazon.

Abstract

Abstract High‐grade portal vein thrombosis (PVT) is often considered to be a technically challenging scenario for liver transplantation (LT) and in some centers a relative contraindication. This study compares patients with chronic obliterative PVT who underwent portal vein recanalization–transjugular intrahepatic portosystemic shunt (PVR‐TIPS) and subsequent LT to those with partial nonocclusive PVT who underwent LT without an intervention. This institutional review board‐approved study analyzed 49 patients with cirrhosis with PVT from 2000 to 2020 at our institution. Patients were divided into two groups, those that received PVR‐TIPS due to anticipated surgical challenges from chronic obliterative PVT and those who did not because of partial PVT. Demographic data and long‐term outcomes were compared. A total of 35 patients received PVR‐TIPS while 14 did not, with all receiving LT. Patients with PVR‐TIPS had a higher Yerdel score and frequency of cavernoma than those that did not. PVR‐TIPS was effective in decreasing portosystemic gradient (16 down to 8 mm HG; p 90% of cases. However, veno–veno bypass was used significantly more in patients who did not receive PVR‐TIPS. Additionally, patients without PVR‐TIPS required significantly more intraoperative red blood cells. Overall survival was not different between groups. PVR‐TIPS demonstrated efficacy in resolving PVT and allowed for end‐to‐end portal vein anastomoses. PVR‐TIPS is a viable treatment option for chronic obliterative PVT with or without cavernoma that simplifies the surgical aspects of LT.

10. Implementation and modification of an organizational-level intervention: a prospective analysis

Abstract

Abstract Background Modifications to interventions can jeopardize intervention outcomes. Pre-existing perceived barriers and facilitators to the intervention arising in the implementation preparation phase may help explain why modifications to the intervention may occur during the implementation phase. This two-site comparative case study describes modifications made to a complex organizational-level intervention and examines how known implementation science factors may have enabled such changes to occur. Methods Northwestern Medicine’sTM Hispanic Kidney Transplant Program (HKTP) is a culturally competent transplant center-based intervention designed to reduce disparities in living donor kidney transplantation among Hispanics. In-depth qualitative interviews and discussions were longitudinally conducted with transplant stakeholders (i.e., physicians, administrators, clinicians) at two kidney transplant programs with large Hispanic populations during implementation preparation and implementation phases. The Consolidated Framework for Implementation Research (CFIR) guided interview design and qualitative analysis, and Stirman’s Framework for Reporting Adaptations and Modifications-Expanded (FRAME) guided modification classification. Results Across sites, 57 stakeholders participated in an interview, group discussion, and/or learning collaborative discussion. Site-B made more modifications than Site-A (n = 29 versus n = 18). Sites differed in the proportions of delaying/skipping (Site-A 50% versus Site-B 28%) and adding (Site-A 11% versus Site-B 28%) but had comparable substituting (Site-A 17% versus Site-B 17%) and tweaking (Site-A 17% versus Site-B 14%) modification types. Across sites, the transplant team consistently initiated the most modifications (Site-A 66%; Site-B 62%). While individuals initiated slightly more modifications at Site-B (21% versus Site-A 17%), institutions instigated proportionately slightly more modifications at Site-A (17% versus Site-B 10%). CFIR inner setting factors (i.e., structural characteristics, culture, available resources, implementation climate) that prominently emerged during the implementation preparation phase explained similarities and differences in sites’ modification numbers, types, and agents in the implementation phase. Conclusion Organizations implementing a culturally competent care intervention made modifications. CFIR inner setting factors emerging in the implementation preparation phase largely explained similarities and differences in study sites’ modifications. Identifying factors contributing to modifications may help institutions become better prepared to implement an intervention by addressing known factors in advance, which may foster greater fidelity leading to desired outcomes. Abstract

Background: The coronavirus disease-2019 (COVID-19) pandemic significantly affected management of cardiovascular disease around the world. The effect of the pandemic on volume of cardiovascular diagnostic procedures is not known. Objectives: This study sought to evaluate the effects of the early phase of the COVID-19 pandemic on cardiovascular diagnostic procedures and safety practices in Asia. Methods: The International Atomic Energy Agency conducted a worldwide survey to assess changes in cardiovascular procedure volume and safety practices caused by COVID-19. Testing volumes were reported for March 2020 and April 2020 and were compared to those from March 2019. Data from 180 centers across 33 Asian countries were grouped into 4 subregions for comparison. Results: Procedure volumes decreased by 47% from March 2019 to March 2020, showing recovery from March 2020 to April 2020 in Eastern Asia, particularly in China. The majority of centers cancelled outpatient activities and increased time per study. Practice changes included implementing physical distancing and restricting visitors. Although COVID testing was not commonly performed, it was conducted in one-third of facilities in Eastern Asia. The most severe reductions in procedure volumes were observed in lower-income countries, where volumes decreased 81% from March 2019 to April 2020. Conclusions: The COVID-19 pandemic in Asia caused significant reductions in cardiovascular diagnostic procedures, particularly in low-income countries. Further studies on effects of COVID-19 on cardiovascular outcomes and changes in care delivery are warranted.

Abstract Achieving justice could be considered a complex social decision-making scenario. Despite the relevance of social decisions for legal contexts, these processes have still not been explored for individuals who work as criminal judges dispensing justice. To bridge the gap, we used a complex social decision-making task (Ultimatum game) and tracked a heart rate variability measurement: the square root of the mean squared differences of successive NN intervals (RMSSD) at their baseline (as an implicit measurement that tracks emotion regulation behavior) for criminal judges (n = 24) and a control group (n = 27). Our results revealed that, compared to controls, judges were slower and rejected a bigger proportion of unfair offers. Moreover, the rate of rejections and the reaction times were predicted by higher RMSSD scores for the judges. This study provides evidence about the impact of legal background and expertise in complex social decision-making. Our results contribute to understanding how expertise can shape criminal judges’ social behaviors and pave the way for promising new research into the cognitive and physiological factors associated with social decision-making.

Abstract

Metal solubilization from discarded electrical material and electronic devices (e-waste) using the bioleaching capabilities of bacterial cells is highly effective. However, gaps in understanding about the microbiological processes involved in the bioleaching reaction leads to less efficient metal solubilization in large-scale e-waste processing. In this study, bacterial species belonging to the genera Acidithiobacillus and Pseudomonas were used to leach copper and gold from discarded printed circuit boards (PCB). Through modulation of the cell-to-cell communication system in these bacteria, phenotypic traits directly involved in the bioleaching reaction were regulated in order to improve the metal solubilization. Addition of the long chain synthetic autoinducer molecule N-acyl homoserine lactone (AHL) of the quorum sensing pathway to the bioleaching reaction resulted in a significant enhancement of metal extraction from PCB. Factors such as: cell attachment to PCB, biofilm formation and hydrogen cyanide (HCN) production were regulated by the quorum sensing system and could be directly related to the improvement of metal bioleaching. Bioleaching reactions using bacterial quorum sensing modulation could represent a valuable tool in overcoming limitations at the industrial level imposed by microbiological traits that lead to inefficient metal bioleaching from e-waste.

Synthetic dyes are highly stable, offer a wide range of colors, and have easy industrial applications. Nevertheless, dyes become a problem in aqueous environments due to their long permanence and low biodegradability. White-rot fungi are useful to degrade dyes through their ligninolytic enzymes and/or biosorption. This work evaluated the effect of 2,5-dimethylaniline, glucose, ammonium tartrate, copper sulfate, pH, and temperature on the degradation of Turquoise Blue (TB) dye in aqueous solutions by Leptosphaerulina sp. (Colombian native fungus). Assays were developed applying a response surface experimental design, which found that the ammonium tartrate and pH are the most influential variables on TB removal. Leptosphaerulina sp. decolorizes aqueous solutions with TB (99%, at day 8), mainly through sorption (89%). Therefore, one of the significant findings from this study is that the Leptosphaerulina sp. biomass is a promising alternative as biosorbent, in comparison to physical-chemical traditional methods, for treating wastewaters polluted with dyes

Abstract Traditional and mainstream legal frameworks conceive law primarily as a purely rational practice, free from affect or intuition. However, substantial evidence indicates that human decision-making depends upon diverse biases. We explored the manifestation of these biases through comparisons among 45 criminal judges, 60 criminal attorneys, and 64 controls. We examined whether these groups’ decision-making patterns were influenced by (a) the information on the transgressor’s mental state, (b) the use of gruesome language in harm descriptions, and (c) ongoing physiological states. Judges and attorneys were similar to controls in that they overestimated the damage caused by intentional harm relative to accidental harm. However, judges and attorneys were less biased towards punishments and harm severity ratings to accidental harms. Similarly, they were less influenced in their decisions by either language manipulations or physiological arousal. Our findings suggest that specific expertise developed in legal settings can attenuate some pervasive biases in moral decision processes.

Abstract Introduction: Sustainment refers to continued intervention delivery over time, while continuing to produce intended outcomes, often with ongoing adaptations, which are purposeful changes to the design or delivery of an intervention to improve its fit or effectiveness. The Hispanic Kidney Transplant Program (HKTP), a complex, culturally competent intervention, was implemented in two transplant programs to reduce disparities in Hispanic/Latinx living donor kidney transplant rates. This study longitudinally examined the influence of adaptations on HKTP sustainment. Methods: Qualitative interviews, learning collaborative calls, and telephone meetings with physicians, administrators, and staff (n = 55) were conducted over three years of implementation to identify HKTP adaptations. The Framework for Reporting Adaptations and Modifications-Expanded was used to classify adaptation types and frequency, which were compared across sites over time. Results: Across sites, more adaptations were made in the first year (n = 47), then fell and plateaued in the two remaining years (n = 35). Adaptations at Site-A were consistent across years (2017: n = 18, 2018: n = 17, 2019: n = 14), while Site-B made considerably fewer adaptations after the first year (2017: n = 29, 2018: n = 18, 2019: n = 21). Both sites proportionally made mostly skipping (32%), adding (20%), tweaking (20%), and substituting (16%) adaptation types. Skipping- and substituting-type adaptations were made due to institutional structural characteristics and lack of available resources, respectively. However, Site-A’s greater proportion of skipping-type adaptations was attributed to greater system complexity, and Site-B’s greater proportion of adding-type adaptation was attributed to the egalitarian team-based culture. Conclusion: Our findings can help prepare implementers to expect certain context-specific adaptations and preemptively avoid those that hinder sustainment.

The utilization of venovenous bypass in liver transplantation (LT) has become less frequent and more center dependent over time. Unfortunately, this has left many transplant surgeons, particularly younger trainees, unfamiliar with the techniques and indications for its utilization. We present a case of LT in a patient with complete superior vena cava (SVC) occlusion prohibiting direct vascular access. A multi-disciplinary approach involving interventional radiology, anesthesiology, cardiac surgery, and transplant surgery, was used to diagnose, evaluate, and develop an operative plan for successful LT. In doing so, a novel approach to portosystemic bypass was utilized involving a right mini-thoracotomy with direct cannulation of the right atrium to gain central venous access and optimize venous return during LT. As a center that does not routinely use venovenous bypass, this multidisciplinary approach was crucial identifying the need for a rarely used technique for vascular access and performance of a successful LT.

Abstract

This article introduces a review of research that has implemented oxytocin measurements in different fluids such as plasma, cerebrospinal fluid, urine and, mainly, saliva. The main purpose is to evaluate the level of evidence supporting the measurement of this biomarker implicated in a variety of psychological and social processes. First, a review of the technical developments that allowed the characterization, function establishing, and central and peripheral levels of this hormone is proposed. Then, the article approaches the current discussions regarding the level of reliability of the laboratory techniques that enable the measurement of oxytocin, focusing mainly on the determination of its concentration in saliva through Enzyme-Linked Immunosorbent Assay (ELISA). Finally, research results, which have established the major physiological correlates of this hormone in fields such as social neuroscience and neuropsychology, are collected and discussed in terms of the hormone measurement methods that different authors have used. In this way, the article is expected to contribute to the panorama of debates and current perspectives regarding investigation involving this important biomarker.

Species of the genus Streptomyces are known for their ability to produce multiple secondary metabolites; their genomes have been extensively explored to discover new bioactive compounds. The richness of genomic data currently available allows filtering for high quality genomes, which in turn permits reliable comparative genomics studies and an improved prediction of biosynthetic gene clusters (BGCs) through genome mining approaches. In this work, we used 121 genome sequences of the genus Streptomyces in a comparative genomics study with the aim of estimating the genomic diversity by protein domains content, sequence similarity of proteins and conservation of Intergenic Regions (IGRs). We also searched for BGCs but prioritizing those with potential antibiotic activity. Our analysis revealed that the pan-genome of the genus Streptomyces is clearly open, with a high quantity of unique gene families across the different species and that the IGRs are rarely conserved. We also described the phylogenetic relationships of the analyzed genomes using multiple markers, obtaining a trustworthy tree whose relationships were further validated by Average Nucleotide Identity (ANI) calculations. Finally, 33 biosynthetic gene clusters were detected to have potential antibiotic activity and a predicted mode of action, which might serve up as a guide to formulation of related experimental studies.

Agroforestry systems allow conservation of natural resources and promotion of sustainable agriculture in the Ecuadorian Amazon. Nevertheless, the benefit of the associated species that are part of these production systems needs to be demonstrated. The aim of this study was to find out the influence on the crop yield, carbon sequestration, presence of earthworms, and the nutritional contribution of legume species associated with the naranjilla (Solanum quitoense) crop in an agroforestry system. The research was carried out in the Palora Experimental Farm of INIAP, using a randomized complete block design with three replications. The treatments were made up of cultivation systems (agroforestry systems with or without 50% fertilization) and monoculture as a control, with two levels of conventional fertilization (50 and 100%). In the agroforestry arrangements, Gliricidia sepium and Flemingia macrophylla were used to supply biomass. The results showed that during the three evaluation cycles, the yield of naranjilla was influenced by the quality of the biomass added to the soil and not by the amount of synthetic chemical fertilizer that was supplied. The biomass of G. sepium and F. macrophylla provided a greater amount of Mg, Mn, Zn, B, and Fe; elements that contributed to crop yield and the presence of earthworms. The results suggest that the use of legume species in agroforestry systems positively influenced naranjillla productivity, favoring sustainable agriculture in the Ecuadorian Amazon.

32. Bactericidal Effect of Entomopathogenic Bacterium Pseudomonas entomophila Against Xanthomonas citri Reduces Citrus Canker Disease Severity

Abstract

The bacterium Pseudomonas entomophila has been recognized as an exceptional species within the Pseudomonas genus, capable of naturally infecting and killing insects from at least three different orders. P. entomophila ingestion leads to irreversible gut damage resulting from a global blockage of translation, which impairs both immune and tissue repair systems in the insect intestine. In this study we isolated a P. entomophila bacterial strain from soil samples which displayed a strong activity against Xanthomonas citri subsp, citri (Xcc), the etiological agent of citrus canker disease. The antagonism potential of isolated bacteria against Xcc and its ability to reduce citrus canker severity was assessed both ex planta and in planta. Our findings show that pathogenicity assays in Citrus x limonia by pressure infiltration and spray with a mixture of P. entomophila and Xcc leaded to a significant reduction in the number of canker lesions in high susceptible citrus leaves, at 21 days post-infection. To the best of our knowledge this is the first report of antibacterial activity of P. entomophila against a phytopathogenic bacterium. Collective action of P. entomophila factors such as diketopiperazine production and the type 6 secretion system (T6SS) may be involved in this type of biological control of citrus canker. The results suggest that the P. entomophila strain could be a promising biocontrol agent acting directly against Xcc.

Quishuar es una especie forestal apreciada por sus usos. La explotación intensiva y la oferta insatisfecha de plantas han sido limitantes para cumplir con programas de reforestación. La micropropagación es una técnica que ayudaría a erradicar este problema ya que el propósito es producir mayor cantidad de plantas en menor tiempo. El objetivo de esta investigación fue desarrollar protocolos para la desinfección, establecimiento y multiplicación in vitro, para el efecto se realizaron ensayos partiendo de semillas y brotes de plantas. En la primera fase con semillas, utilizando kilol y benomil, junto con NaOCl al 3 % (10 min), los resultados indican que el porcentaje de germinación fue del 100 % en MS (Murashige y Skoog) y porcentaje de contaminación y oxidación 0 %. Por otro lado, los brotes sometidos a fungicidas con adición de antioxidantes y NaOCl al 1 % (10 min) no presentaron contaminación ni oxidación. El porcentaje de brotación fue del 100 % en WPM (Woody Plant Medium). En la segunda fase en medio MS sin adición de hormonas se observó una mayor longitud de brote (1.95 cm), número de nudos (1.94 nudos) e índice de multiplicación (2.47). Basándose en los resultados, se sugiere que los protocolos son efectivos para la propagación in vitro.

Abstract Background The shortage of organs for kidney transplantation for patients with end-stage renal disease (ESRD) is magnified in Hispanics/Latin Americans in the United States. Living donor kidney transplantation (LDKT) is the treatment of choice for ESRD. However, compared to their representation on the transplant waitlist, fewer Hispanics receive a LDKT than non-Hispanic whites. Barriers to LDKT for Hispanics include: lack of knowledge, cultural concerns, and language barriers. Few interventions have been designed to reduce LDKT disparities. This study aims to reduce Hispanic disparities in LDKT through a culturally targeted intervention. Methods/Design Using a prospective effectiveness-implementation hybrid design involving pre-post intervention evaluation with matched controls, we will implement a complex culturally targeted intervention at two transplant centers in Dallas, TX and Phoenix, AZ. The goal of the study is to evaluate the effect of Northwestern Medicine’s® Hispanic Kidney Transplant Program’s (HKTP) key culturally targeted components (outreach, communication, education) on Hispanic LDKT rates over five years. The main hypothesis is that exposure to the HKTP will reduce disparities by increasing the ratio of Hispanic to non-Hispanic white LDKTs and the number of Hispanic LDKTs. We will also examine other process and outcome measures including: dialysis patient outreach, education session attendance, marketing efforts, Hispanic patients added to the waitlist, Hispanic potential donors per potential recipient, and satisfaction with culturally competent care. We will use mixed methods based on the Promoting Action on Research Implementation in Health Services (revised PARIHS) and the Consolidated Framework for Implementation Research (CFIR) frameworks to formatively evaluate the fidelity and innovative adaptations to HKTP’s components at both study sites, to identify moderating factors that most affect implementation fidelity, and to identify adaptations that positively and negatively affect outcomes for patients. Discussion Our study will provide new knowledge about implementing culturally targeted interventions and their impact on reducing health disparities. Moreover, the study of a complex organizational-level intervention’s implementation over five years is rare in implementation science; as such, this study is poised to contribute new knowledge to the factors influencing how organizational-level interventions are sustained over time. 36. Impact of having potential living donors on ethnic/racial disparities in access to kidney transplantation

Racial/ethnic disparities persist in patients' access to living donor kidney transplantation (LDKT). This study assessed the impact of having available potential living donors (PLDs) on candidates' receipt of a kidney transplant (KT) and LDKT at two KT programs. Using data from our clinical trial of waitlisted candidates (January 1, 2014-December 31, 2019), we evaluated Hispanic and Non-Hispanic White (NHW) KT candidates' number of PLDs. Multivariable logistic regression assessed the impact of PLDs on transplantation (KT vs. no KT; for KT recipients, LDKT vs. deceased donor KT). A total of 847 candidates were included, identifying as Hispanic (45.8%) or NHW (54.2%). For Site A, both Hispanic (adjusted OR = 2.26 [95% CI 1.13-4.53]) and NHW (OR = 2.42 [1.10-5.33]) candidates with PLDs completing the questionnaire were more likely to receive a KT. For Site B, candidates with PLDs were not significantly more likely to receive KT. Among KT recipients at both sites, Hispanic (Site A: OR = 21.22 [2.44-184.88]; Site B: OR = 25.54 [7.52-101.54]), and NHW (Site A: OR = 37.70 [6.59-215.67]; Site B: OR = 15.18 [5.64-40.85]) recipients with PLD(s) were significantly more likely to receive a LDKT. Our findings suggest that PLDs increased candidates' likelihood of KT receipt, particularly LDKT. Transplant programs should help candidates identify PLDs early in transplant evaluation.

Abstract

Palliative radiotherapy is currently the medical standard of care for non-small-cell lung cancer (NSCLC) patients with symptomatic CNS and leptomeningeal disease. We report the case of a 62-year-old male patient with EGFR mutation (del19+) NSLC with symptomatic lymph node, bone, CNS, and leptomeningeal metastases. Taking into account on one hand the response to tyrosine kinase inhibitors (TKIs) and on the other hand the short- to medium-term side effects of radiotherapy and the lack of timely availability in our healthcare system, the patient was treated with afatinib (40 mg daily) and exhibited a rapid response with improvement of neurological symptoms. The patient presented partial response of extracranial, CNS, and leptomeningeal lesions at 3, 6, and 12 months of treatment, currently completing 16 months of progression-free survival despite presenting mild dermatological and gastrointestinal toxicities. Afatinib is an effective and safe option in patients with NSLC EGFR mutation del19+ with CNS and leptomeningeal compromise avoiding or delaying radiotherapy and its side effects, especially in countries where there is a lack of access to this kind of therapy.

Abstract

The recent increase in infiltration of distributed resources has challenged the traditional operation of power systems. Simultaneously, devastating effects of recent natural disasters have questioned the resilience of power infrastructure for an electricity dependent community. In this study, a solution has been presented in the form of a resilient smart grid network which utilises distributed energy resources (DERs) and machine learning (ML) algorithms to improve the power availability during disastrous events. In addition to power electronics with load categorisation features, the presented system utilises ML tools to use the information from neighbouring units and external sources to make complicated logical decisions directed towards providing power to critical loads at all times. Furthermore, the provided model encourages consideration of ML tools as a part of smart grid design process together with power electronics and controls, rather than as an additional feature.

Abstract

An application of a balanced nutrition will improve the soil as well as enhance the yield of naranjilla (Solanum quitoense Lam.) grown in the Ecuadorian Amazon. A field experiment was carried out in Palora, 16 de Agosto (Morona Santiago province) and Fátima (Pastaza province) to find which variables are related with the yield of the naranjilla crop and the yield response when the crop receives complete nutrition with N, P, K, Ca, Mg, and S plus lime and with the omission of each of these nutrients. A Random Complete Block Design with three replications was used in the three environments. The naranjilla crop had higher yields (18.14 Mg ha−1) in the complete treatments (N, P, K, Ca, Mg, and S) and without S and Mg, consequently, these two last nutrients did not limit the production. When N, P, K, and Ca were not applied, the yields fell to 14.62 Mg ha−1. The main environmental effect showed that Palora had the highest fruit yields (19.73 Mg ha−1), followed by 16 de Agosto (13.57 Mg ha−1) and finally Fátima with 11.04 (Mg ha−1). These preliminary results showed that with the treatments without S (18.55 Mg ha−1), without Mg (18.42 Mg ha−1) and complete (17.46 Mg ha−1) the highest yields were obtained, consequently, the production was not affected by the absence of these elements; the opposite happened when N, P, K, and Ca were not present.

Abstract

En el estudio se evaluó la prevalecía de anemia infecciosa equina (AIE) en los caballos de tracción en el municipio de Florencia, buscando relación por sexo, edad, condición corporal y distribución por comunas. Para ello se recolectaron 128 muestras de sangre de equinos (103 machos y 25 hembras) que asistieron a 4 brigadas de salud animal realizadas por el semillero de investigación en equinos “SIEQUUS”, de la Universidad de la Amazonia, durante el periodo comprendido entre mayo de 2014 y diciembre de 2015. Las muestras fueron analizadas en laboratorio con la prueba de inmunodifusión en agar gel. Se realizó análisis estadístico descriptivo y una prueba Chí cuadrado (P10) fue del 14.29 %, y en los animales entre 5 y 10 años fue considerablemente mayor, el 33.33 %. Las condiciones ambientales y socioeconómicas facilitan la transmisión del virus entre los caballos del mismo gremio y con cualquier sistema de manejo y producción equina de la zona, lo que generaría pérdidas económicas y de semovientes de importancia para el sector equino.

Abstract

Hispanic patients receive disproportionately fewer living donor kidney transplants (LDKTs) than non-Hispanic Whites (NHWs). The Northwestern Medicine Hispanic Kidney Transplant Program (HKTP), designed to increase Hispanic LDKTs, was evaluated as a nonrandomized, implementation-effectiveness hybrid trial of patients initiating transplant evaluation at two intervention and two similar control sites. Using a mixed method, observational design, we evaluated the fidelity of the HKTP implementation at the two intervention sites. We tested the impact of the HKTP intervention by evaluating the likelihood of receiving LDKT comparing pre-intervention (January 2011-December 2016) and postintervention (January 2017-March 2020), across ethnicity and centers. The HKTP study included 2063 recipients. Intervention Site A exhibited greater implementation fidelity than intervention Site B. For Hispanic recipients at Site A, the likelihood of receiving LDKTs was significantly higher at postintervention compared with pre-intervention (odds ratio [OR] = 3.17 95% confidence interval [1.04, 9.63]), but not at the paired control Site C (OR = 1.02 [0.61, 1.71]). For Hispanic recipients at Site B, the likelihood of receiving an LDKT did not differ between pre- and postintervention (OR = 0.88 [0.40, 1.94]). The LDKT rate was significantly lower for Hispanics at paired control Site D (OR = 0.45 [0.28, 0.90]). The intervention significantly improved LDKT rates for Hispanic patients at the intervention site that implemented the intervention with greater fidelity. Abstract

Despite increased attention devoted to diversity, equity, and inclusion (DEI) within academic medicine, representation, lack of workforce and leadership diversity, and bias within medicine remain persistent problems. The purpose of the current study was to understand the current efforts and attention to DEI within academic departments of surgery in the United States. 251 department of surgery websites were reviewed, using a standardized data collection form and scoring procedure, accompanied by a 10 percent fidelity check by an independent reviewer. Only 16% of departments of surgery included DEI-specific information, such as a DEI mission statement or initiatives on their departmental sites, with less than seven percent of departments reporting a DEI committee. Such public information may have implications for recruitment and retention of diverse faculty and trainees, downstream effects for patient care, and could be critical to public accountability to improve diversity and create a culture of equity and inclusion.

Abstract

Actinobacteria embroil Gram-positive microbes with high guanine and cytosine contents in their DNA. They are the source of most antimicrobials of bacterial origin utilized in medicine today. Their genomes are among the richest in novel secondary metabolites with high biotechnological potential. Actinobacteria reveal complex patterns of evolution, responses, and adaptations to their environment, which are not yet well understood. We analyzed three novel plant isolates and explored their habitat adaptation, evolutionary patterns, and potential secondary metabolite production. The phylogenomically characterized isolates belonged to Actinoplanes sp. TFC3, Streptomyces sp. L06, and Embleya sp. NF3. Positively selected genes, relevant in strain evolution, encoded enzymes for stress resistance in all strains, including porphyrin, chlorophyll, and ubiquinone biosynthesis in Embleya sp. NF3. Streptomyces sp. L06 encoded for pantothenate and proteins for CoA biosynthesis with evidence of positive selection; furthermore, Actinoplanes sp. TFC3 encoded for a c-di-GMP synthetase, with adaptive mutations. Notably, the genomes harbored many genes involved in the biosynthesis of at least ten novel secondary metabolites, with many avenues for future new bioactive compound characterization-specifically, Streptomyces sp. L06 could make new ribosomally synthesized and post-translationally modified peptides, while Embleya sp. NF3 could produce new non-ribosomal peptide synthetases and ribosomally synthesized and post-translationally modified peptides. At the same time, TFC3 has particularly enriched in terpene and polyketide synthases. All the strains harbored conserved genes in response to diverse environmental stresses, plant growth promotion factors, and degradation of various carbohydrates, which supported their endophytic lifestyle and showed their capacity to colonize other niches. This study aims to provide a comprehensive estimation of the genomic features of novel Actinobacteria. It sets the groundwork for future research into experimental tests with new bioactive metabolites with potential application in medicine, biofertilizers, and plant biomass residue utilization, with potential application in medicine, as biofertilizers and in plant biomass residues utilization. Abstract

This study presents a detailed analysis of the wear presented in [8YSZ/α-Al2O3] multilayer coatings as a function of the bilayer number when exposed to a corro-erosive environment. In this study, the roughness and grain size decreased by 58 and 30%, respectively when comparing the 70 bilayers system with the 1 bilayer system. The surface factors and the higher bilayer number of the n = 70 system, influenced the 67.33% decrease in corrosion rate compared to the 1 bilayer system. Subsequently, by means of SEM and optical microscopy, the degradation of the coatings due to the impact of abrasive particles and the generation of oxides on the substrate surface caused by a partial delamination of the coating was evidenced. This wear was calculated by means of the mass loss after the corro-erosive test, obtaining a decrease of 55.25% for the system with 70 bilayers with respect to the system with 1 bilayer. The results obtained showed that the system with 70 bilayers presented the best set of electrochemical properties and lower wear in corrosive environments, which makes it the best candidate to be used as a protective coating.

Abstract

We investigated behavioural patterns of school subjects from Colombia and Tenerife (Spain) of 10–12, 13–14 and 15–17 years old (150 per age group), during a crossed puzzle game. We video-recorded all sessions, elaborated an ethogram and classified behavioural patterns within functional categories (Empathy, Help Organizing, Agonistic, Cooperation, Selfishness and Tension-Distension); their frequencies were analyzed by Generalized Linear Mixed Models (GLMM). Results showed significant differences between countries in Help Organizing, Cooperation, Agonistic and Tension-Distension; the same categories except Cooperation differed between age ranges, but no category significantly differed between sexes. GLMM of factor scores from a principal component analysis applied to behavioural categories showed subjects from Colombian schools had significantly lower PC1 factor scores (Empathy, Selfishness and Tension-Distension) than those from Tenerife; the contrary occurred for PC2 (Help Organizing and Cooperation) and no significant difference was found for PC3 (Agonistic and Selfishness). We discuss several potential causes of the differences found.
