4. Severe Hyperkalemia: Electrocardiographic Tips for Early Recognition Based on a Case Report

Background: Rapid correction of severe hyperkalemia is mandatory to survival due to its induction of fatal cardiac arrhythmias. The electrocardiogram serves as the diagnostic tool that can provide insight into such fatal arrhythmias. We present two relevant alterations seen in an 84-year-old female patient with previous anterior myocardial infarction, angioplasty of the circumflex coronary artery, left ventricular ejection fraction of 35%, hypertension, dyslipidemia, paroxysmal atrial fibrillation, and diabetes. The Case: The patient had 4-days with asthenia, adynamia and dyspnea. Lung auscultation showed bilateral base rales with cardiomegaly and interstitial edema identified on chest x-ray. Lab work revealed severe metabolic acidosis, increased plasma urea, creatinine, and severe hyperkalemia (7.9 mEq/liter) considered secondary to acute renal failure. Treatment was initiated with 0.9% sodium chloride, bicarbonate, ASA diuretics and polarizing solution (insulin), resulting in a reduction of hyperkalemia to 6.1 mEq/liter. The patient suffered a cardiorespiratory arrest with recovery and needed intubation and dopamine for hemodynamic support but died 15 hours after admission. Conclusion: The electrocardiographic findings characteristic of severe hyperkalemia were: (i) regular rhythm (cycle length 920-950 ms) without discernible P-waves, which may have a junctional or ventricular origin and less probably could be a manifestation of sinoventricular conduction (preferential conduction from the sinus node to the AV node through specialized tracts without activation of the atrial cardiomyocytes), and (ii) sine wave morphology (markedly wide QRS, absence of ST-segment and broadly based T-waves). These electrocardiographic features, typical of hyperkalemia exceeding 7.0 mEq/liter, are harbingers of malignant arrhythmias and should prompt immediate therapy. more...

Introduction: Cardiopulmonary arrest (CPA) is the cessation of effective ventilation and circulation. It is also known as cardiac arrest or circulatory arrest. In adults, it is most likely to be caused by a primary cardiac event. The most common electrical mechanism which is responsible for 50 to 80% of cardiopulmonary arrest is ventricular fibrillation (VF). While, 20% to 30% which represents the less common causes of dysrhythmias involve Pulseless electrical activity (PEA), and asystole. Pulseless sustained ventricular tachycardia (VT) is a less common mechanism. Objective: To study the rate of survival to discharge after in-hospital cardiac arrest and its associated factors in an emergency department of a tertiary care hospital. Materials and Methods: This prospective observational study was conducted in the Department of Cardiology in a Tertiary care Teaching hospital over a period of 6 months among all patients above 18 years old, who suffered witnessed cardiac arrest, after arrival in the emergency department. A semi-structured questionnaire was used to collect data (socio demographic details, chief complaints, comorbidities). Initial documented rhythm, duration of CPR, use of defibrillator, and presumed cause of cardiac arrest and others were collected from the case records. Results: After CPR, 10 (20%) of 50 patients were discharged fully conscious, whereas 40 (80%) patients died; 20 of them died immediately, whereas 20 patients developed hypoxic encephalopathy and died during hospitalization. There was no statistically significant association between age, sex, and associated comorbidities and the outcome of CPR. However, there was a significant inverse relation between the duration of CPR and its outcome. The duration of CPR was significantly lower in survivors (3.80 ±1.90 min) compared with non survivors (11.31 ± 3.41 min) (P<0.001). GCS was significantly higher in survivors group compared with non-survival groups (12.38±0.3 vs 3.68±1.20, P<0.001). Conclusion: These results suggest that out-of-hospital cardiac arrest among children has a very poor prognosis, especially when efforts at resuscitation continue for longer than 20 minutes and require more than two doses of epinephrine. Objective: to evaluate the knowledge of nursing staff professionals in the care of cardiorespiratory arrest and to compare before and after training. Method: prospective and comparative research, conducted in a public hospital in São Paulo, in 2021, with 68 professionals. A questionnaire on cardiorespiratory arrest before and after training was applied for comparison. Results: most professionals recognize the classic signs of cardiorespiratory arrest and the correct sequence of care, however, had difficulty identifying the shockable rhythms and actions after defibrillation. The average number of correct answers before and after the training was 43.65 and 66.11, respectively, and corroborates the effectiveness of the educational action. Results: There was a significant increase (p<0.05) in the level of knowledge after the simulation. When evaluating the design scale, an average of 4.55 was found for agreement and 4.55 for importance with the clinical simulation. There was an average score of 4.56 on the scale of satisfaction and self-confidence perceived by the participants in the clinical simulation. Conclusion: Clinical simulation enhances participants' learning, promotes satisfaction and self-confidence, and using instruments to evaluate and apply the scenario are guidelines for effective clinical simulation. Cardiorespiratory arrest has a high prevalence among cardiovascular emergencies and the care provided by health professionals must be of excellence. Therefore, there is a need to prepare health professionals to deal with such situations with regard not only to the performance of technical skills, but also to the performance of non-technical skills, with leadership being exercised within the teams, crucial for a satisfactory outcome. The study aims to identify evidence in the literature regarding the strategies used in teaching leadership to health professionals in the context of cardiac arrest. This is an integrative literature review with searches in the CINAHL, MEDLINE, LILACS and WEB OF SCIENCE databases. The final sample included 4 studies, carried out in developed countries and with a relatively low level of evidence; among the strategies for teaching leadership, the practice of simulation in association with learning assessment methods predominated, showing good results in terms of leadership performance and the outcome of cardiac arrest. It is recommended that other studies evaluate leadership teaching strategies in the context of cardiorespiratory arrest in heterogeneous teams and in developing countries. more...

Cardiopulmonary arrest (CRA) is a very serious clinical emergency that can have the ultimate consequence of death. The success of cardiac arrest care is determined by rapid recognition of signs of cardiac arrest and prompt emergency care. This study aimed to verify the role of nurses in the care of cardiorespiratory arrest. This is an integrative review research on the proposed theme, without interference from the researcher in the research environment. Data collection was performed from information collected in the electronic databases LILACS, SCIELO and BDENF. The authors surveyed emphasize that Nursing is responsible for the patient most of the time, so the nurse must observe the main factors that can develop cardiorespiratory arrest and care in hypothermia after CRA. Resumen: Objetivo: Analizar la curva de olvido en la adquisición de competencias en soporte vital básico con la modalidad formativa de blended learning. Material y métodos: Las habilidades en soporte vital básico fueron evaluadas en 271 alumnos del grado de medicina en la Universidad de Granada. Para ello, se impartió una modalidad docente semipresencial (blended learning) y los discentes fueron evaluados en dos ocasiones, separadas en el tiempo por dos meses. Resultados: En la primera evaluación, todos los participantes realizaron correctamente las maniobras de perfil práctico y presentaron un menor aprendizaje inicial en las de carácter cognitivo y actitudinal. En la reevaluación, las maniobras con mayor olvido fueron la apertura de la vía aérea y la realización de dos ventilaciones. En cambio, las mejor recordadas fueron la petición de ayuda y la alternancia de compresiones y ventilaciones siguiendo una secuencia de 30:2. En el análisis por sexos, las mujeres presentaron mayor curva de olvido en la realización de las maniobras en comparación con los varones, en los que se pudo observar una mejor retención de algunas habilidades respecto a la evaluación inicial. Conclusiones: El sistema de enseñanza basado en blended learning es útil en el proceso de adquisición de las competencias en soporte vital básico. Abstract: Purpose: To analyse the forgetting curve in the acquisition of basic life support competencies by blended learning in third year students of Medicine Degree at the University of Granada. Material and methods: Basic life support skills were evaluated in 271 medical degree students at the University of Granada. For this, a blended learning modality was taught and the students were evaluated on two occasions, separated in time by two months. Results: Practical maneuvers were performed correctly by all students in the first evaluation. But cognitive and attitudinal ones presented less initial learning. The maneuvers with the greatest forgetfulness were opening the airway and performing two ventilations, on revaluation. On the other hand, the best remembered ones were the request for help and the alternation of compressions and ventilations following a sequence of 30: 2. Differences by sex showed that women presented a higher forgetting curve compared to men. Some skills had a better retention by men compared to the initial evaluation. 19. Clinician Wire Puncture Injury to the Hand from Chest Compressions on a Patient with a Median Sternotomy: A Case Report.

Standard precautions, including protections from blood and body fluid exposure, are designed to protect health care providers from infections. Sharps safety practices rarely include the potential for the unconscious patient's own body to be a potential source of clinician percutaneous injury from sharp objects outside of the perioperative setting. This case report reviews a percutaneous injury to the hand of a physician who was performing chest compressions on a patient with an out-of-hospital cardiac arrest. The 76-year-old patient in cardiac arrest had undergone a medial sternotomy surgery 15 years before the arrest. The sternal wire rotated owing to the initial chest compressions, breaking the clinician's nitrile glove and producing an open wound on the thenar region of the clinician's right hand. Application of a 10 × 10 12-ply gauze pack on the chest of the patient in cardiac arrest allowed the resuscitation team to continue with the compressions with no further wounds from the wire. This case report is a novel contribution to the published literature and advances standard precautions considerations in patients with out-of-hospital cardiac arrest, with the sternotomy wire from previous surgery as a source of percutaneous clinician injury during chest compression. 21. Outcome of cardiorespiratory arrest in patients with respiratory diseases in emergency department

Objective Cardiorespiratory arrest is rapidly fatal without resuscitation. The authors aimed to determine the outcome of cardiopulmonary resuscitation (CPR) for patient with respiratory disease presenting to the emergency setting with cardiorespiratory arrest. Materials and methods This prospective cohort study was carried out on 60 patients who presented to the emergency room with respiratory disorders and presented with or developed cardiorespiratory arrest. CPR was done guided by international guidelines for CPR. Patient demographic and clinical data, frequency of cardiopulmonary arrest, duration of resuscitation, length of hospital and ICU stay, and the outcome of resuscitation were recorded. Clinical and neurological examination and Glasgow coma scale were applied for all patients. Psychological assessment was done only for survivors (15 patients) using Beck depression inventory-II scale and Taylor manifest anxiety scale. Results Of 60 patients, 15 (25%) were discharged fully conscious, whereas 45 (75%) patients died. Age, sex, comorbidities, and cause of arrest did not show any statistically significant association with the outcome (P>0.05 for all). The duration of CPR was significantly lower in survivor group compared with nonsurvivor group (4.50±2.10 vs 12.87±8.52 min; P=0.001). The aim of this study was to evaluate the theoretical knowledge of nurses on the care of cardiorespiratory arrest. Cross-sectional, quantitative study, carried out in a public university hospital, in state of São Paulo, with 44 nurses at critical care and emergency unit. For data collection, a previously validated questionnaire was used, updated in accordance with current international guidelines, in order to contemplate the topics: characterization of the professional and knowledge about cardiorespiratory arrest and cardiopulmonary resuscitation. For data analysis, descriptive statistics were used for continuous and categorical data, tests of association between categorical variables and comparison between quantitative and qualitative variables. The results allow us to conclude that the participating nurses have theoretical knowledge about the care for cardiorespiratory arrest and resuscitation and that they have frequent contact with the topic, through training or clinical practice; although they report difficulties in team attendance to this occurrence. Objective: To verify the knowledge of high school students about cardiopulmonary arrest and cardiopulmonary resuscitation and to evaluate learning immediately and late after theoretical and practical training. Methods: Longitudinal, prospective, quantitative experimental study, carried out in a public and a private institution, using a questionnaire on basic life support. The statistics were descriptive and inferential by the Friedman and Mann Whitney tests. Results: The students had insufficient knowledge, showing a significant improvement immediately and late. The two institutions had an equivalent performance in terms of correct answers in the immediate and late post-training periods. The stop signs, resuscitation maneuvers and steps to use the external defibrillator had lower success rates. 27. Central Venous Catheter Misplacement Into Pleural Cavity Causing Hypertensive Pleural Effusion: A Case Report.

Central venous catheter (CVC) insertion is a routine procedure in the management of critically ill patients. We report a clinical case of inadvertent placement of an internal jugular vein CVC into the right pleural cavity, despite employing clinical and imaging-based techniques to ensure proper catheter positioning. Infusion of fluids and vasopressors through this misplaced catheter led to hypertensive pleural effusion and subsequent cardiorespiratory arrest. Return of spontaneous circulation was achieved after two cycles of cardiopulmonary resuscitation. While multiple imaging modalities are recommended for confirming appropriate CVC placement, each method has inherent limitations. Abstract Background Cardiorespiratory arrest can result in a spectrum of hypoxic ischemic brain injury leading to global hypoperfusion and brain death (BD). Because up to 40% of patients with BD are viable organ donors, avoiding delayed diagnosis of this condition is critical. High b-value diffusion weighted imaging (DWI) measures primarily molecular self-diffusion; however, low b-values are sensitive to perfusion. We investigated the feasibility of low b-value DWI in discriminating the global hypoperfusion of BD and hypoxic ischemic encephalopathy (HIE). Methods We retrospectively reviewed cardiorespiratory arrest subjects with a diagnosis of HIE or BD. Inclusion criteria included brain DWI acquired at both low (50 s/mm2) and high (1000–2000 s/mm2) b-values. Automated segmentation was used to determine mean b50 apparent diffusion coefficient (ADC) values in gray and white matter regions. Normal subjects with DWI at both values were used as age- and sex-matched controls. Results We evaluated 64 patients (45 with cardiorespiratory arrest and 19 normal). Introduction: Cardiopulmonary resuscitation is a sequence of techniques that combines chest compression with artificial ventilation to manually maintain the circulation to preserve intact brain function. The aim is to maintain circulation and breathing in a person who is in cardiac arrest until emergency aid arrives. Effective teamwork by Code Blue team raises chances of a successful outcome. The advanced cardiac life support (ACLS) guidelines were developed by the American Heart Association using the comprehensive review of resuscitation literature performed by the International Liaison Committee on Resuscitation. Aim: To assess adherence of Code Blue team to ACLS protocol, to assess outcome of resuscitation, and to compare outcome between those where ACLS guidelines were followed and those not followed. Patients and Methods: A clinical audit was done between 2014 and 2015 at Lourdes Hospital, Kochi, on inpatients aged between 30 and 80 years, with witnessed cardiac arrests/respiratory arrest. Pregnant and unwilling patients were excluded. Results: The common arrest rhythm was pulseless electrical activity, followed by asystole. ACLS protocol was followed in 58.7%. The most common deviation was usage of inappropriate drugs. Return of spontaneous circulation (ROSC) was attained in 53.3%, of which 28.5% were discharged (P < 0.05), which suggests a significant association between the adherence to ACLS protocol and ROSC. Conclusion: Although Code Blue team is ACLS trained, deviations occurred in nearly half of the resuscitations, which need to be reduced. The study was developed with the purpose of investigating the difficulties faced by nursing professionals in the mobile pre-hospital care to the cardiorespiratory arrest. It is a narrative review study that was conducted after the publication of publications, restricted to the period from 2008 to 2018. The research was carried out using the platform provided by the Virtual Health Library (VHL) through the main descriptors: nursing, cardiorespiratory arrest and cardiopulmonary resuscitation. The participation of nursing professionals in the care of patients with CRA is important because they have a leadership and management role in relation to emergency care, but if the lack of knowledge and inexperience are present, it is problematic to offer a quality care. We emphasize the importance of being prepared to offer adequate care to patients in out-of-hospital settings, through the SAMU, leads to understanding that there is more development of health education, being promoted by nurses in their respective institutions, to reduce this lack of preparation of the team and improve the quality of care. Background Cardiorespiratory arrest (CA) secondary to traumatic cervical spinal cord injury can occur in minor accidents with low-impact trauma and may be overlooked as the cause of CA in patients admitted in the coronary care unit. Case summary We present two patients admitted to the coronary care unit because of suspected CA of cardiac origin. Both patients were found in CA with asystole, one after collapsing in a shopping mall and falling down a few steps and the other in the street next to his bicycle. They underwent early pharmacologically induced coma and hypothermia precluding neurological examination. Both patients remained in coma after rewarming, with preserved brainstem reflexes but absent motor response to pain. One patient had post-anoxic myoclonus in the face without limb involvement. In both patients, median nerve somatosensory evoked potentials demonstrated bilateral absence of thalamocortical N19 responses and abnormal cervicomedullary junction potentials (N13 wave). Extensive diagnostic work-up did not find a cardiac cause of the CA, pulmonary thromboembolism, or intracranial haemorrhage. In both patients, cervical spinal cord injury was diagnosed incidentally 5 and 6 days after CA, when a brain magnetic resonance imaging performed to assess post-anoxic brain injuries detected spinal cord hyperintensities with fracture and luxation of the odontoid. Both patients died 11 and 8 days after CA. Discussion Low-impact traumatic cervical spinal cord injury should be considered in the diagnostic work-up of patients with CA of unknown cause. Background: Meningoceles are congenital herniation of meninges and cerebrospinal fluid (CSF) through the skull and are bereft of any cerebral tissue. They are commonly found over the anterior fontanelle. Although some cases of cervical dysraphism have been described in the literature, a true meningocele has rarely been seen. The child usually presents with hydrocephalus with features of raised increased intracranial pressure. Sensory, motor, and sphincter functions may be involved depending on the level of lesion. Closure of meningocele should be ideally done within the first 48 h of birth. Case Description: Complications associated with meningocele range from learning disabilities, seizures, and bowel dysfunction to complete paralysis below the level of the lesion. The postoperative complications reported are wound infection, CSF leak/collection, urinary tract infection, deterioration of deficit, and death. Here, we present a postoperative case of an 11-month-old child with cervical meningocele who had an unusual complication almost 2 h after an uneventful surgery in the form of sudden cardiorespiratory arrest was revived successfully. Conclusion: A meningocele surgery is usually not associated with severe postoperative complications which can be encountered in meningomyelocele surgery. Here, in our case, the child with uneventful meningocele surgery arrested 2 h postsurgery with the possible cause being cervical cord edema. Objective: to analyze the scientific production on cardiorespiratory arrest in Brazilian nursing journals. Method: bibliometric study carried out through electronic access to the websites of the 14 Brazilian nursing journals, classified in the Sucupira Platform, for analysis of articles published between 2010 and 2019. Results: 18,266 articles were analyzed, of which 50 were selected to integrate the sample. It was possible to observe that 50% of the articles were published in Qualis A extract journals and 80% of the productions were derived from the Southeast Region. Regarding the type of study, 54% were descriptive and 64% quantitative. Regarding the most investigated theme, 28% portrayed the knowledge of nurses and 22% contemplated nursing training on cardiorespiratory arrest. Conclusion: the scientific production of Brazilian journals showed a predominance of articles published in Qualis A journals, from the Southeast Region, descriptive, quantitative, which predominantly investigated the knowledge and training of professionals. El presente trabajo de fin de grado tiene como objetivo comparar la efectividad de dos materiales didácticos para trabajar el soporte vital básico con el alumnado de Educación Infantil. Para ello, se diseñó una intervención educativa en una escuela pública de Santiago de Compostela con cuatro grupos experimentales de entre 4 y 6 años. La propuesta constaba de dos sesiones formativas sobre principios básicos de actuación en caso de parada cardiorrespiratoria a través de unas cartas de bajo coste o un libro sin fin. La evaluación analizó la capacidad a corto plazo del alumnado para actuar ante una emergencia de este tipo una vez que se impartió la formación y un mes después. Los resultados mostraron que ambos materiales contribuyen al aprendizaje, aunque en el caso del libro se detecta una mejor respuesta en la llamada a los servicios de emergencias médicas, la adecuada comunicación de los hechos durante la llamada y la realización correcta de todos los pasos de la cadena de supervivencia, pudiendo considerarlo más efectivo, O presente traballo de fin de grao ten como obxectivo comparar a efectividade de dous materiais didácticos para traballar o soporte vital básico co alumnado de Educación Infantil. To examine the components of visual attention that maintain situational awareness during simulation training in undergraduate nursing students with different instruction levels. Eye-tracking can provide deep insight into the nurses' attention during simulated practice. Knowing which gaze patterns promote situational awareness can significantly improve nurse instruction. A comparative observational study investigated the role of visual attention on the performance quality, psychophysiological parameters (vital signs, anxiety and stress) and socioemotional competencies (cognitive workload, motivation and self-efficacy) of nursing students with various experience levels. Thirty nursing students divided into two groups according to their academic level: first cycle (n=14) and second-cycle (n=16) faced a clinical simulation scenario to resolve a cardiorespiratory arrest event. Eye tracking-based analysis required the selection of six areas of interest. The monitorization of vital signs included measuring blood pressure, heart rate, respiratory rate and oxygen saturation before and after the simulation practice. Participants completed the socioemotional questionnaire (NASA-TLX). They answered the state subscale of the State-Trait Anxiety Inventory (STAI), the Visual Analogue Scale (VAS) of stress, the Situational Motivation Scale (SIMS) and the Baessler and Schwarzer General Self-Efficacy Scale. The first-cycle group displayed higher vital sign scores than the second cycle, apart from the post-simulation respiratory rate. All physiological parameters increased in mean value after the clinical simulation, except oxygen saturation. Anxiety was the only parameter in the socioemotional domain to present a statistically significant difference between the groups. First-year nursing students showed greater anxiety, stress, mental workload, identified regulation and intrinsic motivation, while second-year students showed higher levels of amotivation, external regulation and perceived self-efficacy. Eye-tracking data (revisits, gaze and duration of fixations) exhibited statistically significant differences depending on the area of interest in both groups (p =. 05). The performance outcomes showed a negative and moderate association with gaze the total number of gazes in the second-cycle group (rho = −0.640, p = 0.010). O objetivo deste trabalho é demonstrar grau de conhecimento do enfermeiro frente a um paciente vítima de parada cardiorrespitória (PCR).Trata-se de uma revisão integrativa que utilizou-se de uma pesquisa bibliográfica, como bases de dadosScientific Electronic LibraryOnline (Scielo) e Biblioteca Virtual de Saúde (BVS), publicados em português como critérios de inclusão dos artigos, como resumos, textos entre o período de 2019 a 2021. Após a leitura foram escolhidos 10 artigos para análise proposta. Os resultados demonstraram que o enfermeiro é primordial para o sucesso em uma parada cardiorrespitória (PCR), sendo ele a iniciar os primeiros cuidados, como a de reanimação cardiopulmonar, verificou-se que é de suma importância a educação e capacitação desses profissionais, onde possa manter uma padronização nos atendimentos de uma parada cardiorrespitória (PCR) para a obtenção do sucesso das intervenções aplicadas aos pacientes vítimas de PCR.  more...

Evaluate the level of knowledge of physical education students at a University Center in the Federal District regarding recognition, attitude and practice in the face of cardiorespiratory arrest. This is a descriptive and quantitative research, which was conducted in a University Center of the Federal District, through a questionnaire directed to physical education students who were in accordance with the selection criteria and wished Participate. All academics have demonstrated to have some kind of knowledge about CRP, but do not declare themselves able to perform some kind of intervention. It is possible to note that academics have a relative knowledge about CRP and the first care to be provided to the victim, but still show insecurity for performing CPR maneuvers efficiently, which suggests the application of course extensionists, updating and improving these undergraduates. Introduction:Cardiorespiratory arrest (CRP) has high mortality rates in Brazil. Although we do not have an exact measure of this problem, the data considered is around 200 thousand PCR/year in the country, with an even split between extra and in-hospital environments. CRP can be defined as the sudden cessation of heartbeat and respiratory movements. Therefore, it is important that the nurse is able to identify and attend to an episode of CRP by performing cardiopulmonary resuscitation (CPR) with skill and efficiency.Methods: Descriptive and prospective study aiming to present the level of information of nurses working in the ICU on the care of PCR in the hospital environment. Being carried out with 27 nurses from general, coronary, surgical and neurotrauma ICUs of a public hospital of high complexity and reference for these specialties, located in the Federal District. Data were collected between June and August 2016, using a validated questionnaire. The results were described as percentages calculated by the Microsoft Excel®2007 program.Results:The average number of correct answers was 80%. The questions with the maximum score of correct answers were: those related to the most used drugs, their respective routes of administration and the ventilation of the patient without a definitive airway. The question with the lowest success rate was about the conceptual definition of respiratory arrest.Conclusion:Nurses showed good knowledge on the subject, presenting better results with practical approaches and greater difficulty on predominantly theoretical issues. more...

This research was structured with the objective of describing the nurse's assistance during cardiorespiratory arrest in the emergency room. This is a narrative review of the literature based on a literature search carried out in July 2020 in the respective data sources: Medical Literature Analysis and Retrieval System Oline (Medline) and Scientific Electronic Library Online (SciELO). The nurse is responsible for performing Basic Life Support (BLS) to maintain the circulation of the main vital organs and, secondly, to assist in Advanced Life Support (ALS), which aims to treat and revert to cardiac arrest. These professionals are responsible for administering drugs according to medical advice or protocol; bladder and nasogastric catheterization; performing the nursing evolution, checking medications and reorganizing the sector where the event took place, as well as providing assistance to family members, whether in cases of reversal of the arrest or in cases of deaths. In this perspective, nursing professionals need to appropriate their skills and make use of continuing education to promote the improvement of the quality of care for victims, since the success of interventions made in this clinical emergency depends on the level of preparation of professionals. Esta investigación se estructuró con el objetivo de describir la asistencia del enfermero durante el paro cardiorrespiratorio en el servicio de urgencias. Esta es una revisión narrativa de la literatura basada en una búsqueda bibliográfica realizada en julio de 2020 en las respectivas fuentes de datos: Medical Literature Analysis and Retrieval System Oline (Medline) y Scientific Electronic Library Online (SciELO). El enfermero es responsable de realizar el Soporte Vital Básico (SVB) para mantener la circulación de los principales órganos vitales y, en segundo lugar, asistir en el Soporte Vital Avanzado (SVA), cuyo objetivo es tratar y revertir el paro cardíaco. Estos profesionales son los encargados de administrar los medicamentos según indicación médica o protocolo; cateterismo vesical y nasogástrico; realizar la evolución de enfermería, controlar medicamentos y reorganizar el sector donde ocurrió el evento, así como brindar asistencia a los familiares, ya sea en casos de reversión de la detención o en casos de muerte. En esa perspectiva, los profesionales de enfermería necesitan apropiarse de sus competencias y hacer uso de la educación permanente para promover la mejora de la calidad de la atención a las víctimas, ya que el éxito de las intervenciones realizadas en esta emergencia clínica depende del nivel de preparación de los profesionales. Essa pesquisa estruturou-se com o objetivo de descrever a assistência da (o) enfermeira (o) durante a parada cardiorrespiratória na emergência. Trata-se de uma revisão narrativa da literatura a partir de uma pesquisa bibliográfica realizada em julho de 2020 nas respectivas fontes de dados: Medical Literature Analysis and Retrieval System Oline (Medline) e Scientific Electronic Library Online (SciELO). A (o) enfermeira (o) tem a responsabilidade de realizar o Suporte Básico de Vida (SBV) para manter a circulação dos principais órgãos vitais e, segundo, que auxiliem no Suporte Avançado de Vida (SAV), que tem como objetivo tratar e reverter à parada cardiorrespiratória. É atribuída a esses profissionais a administração de fármacos conforme orientação médica ou protocolo; cateterização vesical e nasogástrica; realização da evolução de enfermagem, checagem das medicações e reorganização do setor onde aconteceu o evento, como também prestar assistência aos familiares, seja em casos de reversão da parada ou nos casos de óbitos.
