This study aims to evaluate characteristics of delayed and prolonged surgical site discharges and their prognostic significance after craniotomies. Postoperative infections after craniotomy are potentially serious complications warranting vigilance and intervention. Prolonged and delayed surgical site discharge is an overlooked and seldomly reported entity in the neurosurgical literature. We have treated several cases with prolonged discharges from their craniotomy sites in a delayed fashion even years after their surgeries. The tertiary referral center database was reviewed for cases involving surgical site discharges. We found 164 cases of infection following neurosurgical procedures between 2010 and 2020, of which nine involved discharges. Four of these patients were excluded upon the second evaluation, and finally, five cases were included for further analysis. Our cohort comprised three females and two males aged 13–46 years (mean: 28.8 years). The initial surgeries were performed a mean of 61.2 months ago. Subtle discharges from surgical sites began as early as 1 month or as late as 15.5 years following surgery. The culture results implicated Pseudomonas aeruginosa in one case, coagulase-negative Staphylococcus in one, and normal skin flora in two. One case was inconclusive. All patients were operated on, and the surgical site was irrigated. In two patients, the bone flap and the cranioplasty material were also removed. The patients were followed up for a mean 40.8 months and there were no further discharges from the surgical site. Non-purulent, sticky discharges from surgical sites are an overlooked entity in neurosurgical practice. Recognition and familiarity with the natural course of this condition can make earlier exploration of the surgical site possible, with favorable outcomes compared to long-term antibiotic therapy. Staphylococcus aureus is frequently highlighted as a priority for novel drug research due to its pathogenicity and ability to develop antibiotic resistance. Coagulase-negative staphylococci (CoNS) are resident flora of the skin and nares. Previous studies have confirmed their ability to kill and prevent colonization by S. aureus through the production of bioactive substances. This study screened a bank of 37 CoNS for their ability to inhibit the growth of methicillin-resistant S. aureus (MRSA). Deferred antagonism assays, growth curves, and antibiofilm testing performed with the cell-free supernatant derived from overnight CoNS cultures indicated antimicrobial and antibiofilm effects against MRSA indicators. Whole genome sequencing and BAGEL4 analysis of 11 CoNS isolates shortlisted for the inhibitory effects they displayed against MRSA led to the identification of two strains possessing complete putative bacteriocin operons. The operons were predicted to encode a nukacin variant and a novel epilancin variant. From this point, strains Staphylococcus hominis C14 and Staphylococcus epidermidis C33 became the focus of the investigation. Through HPLC, a peptide identical to previously characterized nukacin KQU-131 and a novel epilancin variant were isolated from cultures of C14 and C33, respectively. Mass spectrometry confirmed the presence of each peptide in the active fractions. Spot-on-lawn assays demonstrated both bacteriocins could inhibit the growth of an MRSA indicator. The identification of natural products with clinically relevant activity is important in today's climate of escalating antimicrobial resistance and a depleting antibiotic pipeline. OBJECTIVE: To determine the time to positivity (TTP) of blood cultures among infants with late-onset bacteraemia and predictors of TTP >36 hours. DESIGN: Retrospective cohort study. SETTING: 16 birth centres in two healthcare systems. PATIENTS: Infants with positive blood cultures obtained >72 hours after birth. OUTCOME: The main outcome was TTP, defined as the time interval from specimen collection to when a neonatal provider was notified of culture growth. TTP analysis was restricted to the first positive culture per infant. Patient-specific and infection-specific factors were analysed for association with TTP >36 hours. RESULTS: Of 10 235 blood cultures obtained from 3808 infants, 1082 (10.6%) were positive. Restricting to bacterial pathogens and the first positive culture, the median TTP (25th-75th percentile) for 428 cultures was 23.5 hours (18.4-29.9); 364 (85.0%) resulted in 36 hours. Excluding coagulase-negative staphylococci (CoNS), 275 of 294 (93.5%) cultures were flagged positive by 36 hours. In a multivariable model, CoNS isolation and antibiotic pretreatment were significantly associated with increased odds of TTP >36 hours. Projecting a 36-hour empiric duration at one site and assuming that all negative evaluations were associated with an empiric course of antibiotics, we estimated that 1164 doses of antibiotics would be avoided in 629 infants over 10 years, while delaying a subsequent antibiotic dose in 13 infants with bacteraemia. CONCLUSIONS: Empiric antibiotic administration in late-onset infection evaluations (not targeting CoNS) can be stopped at 36 hours. Longer durations (48 hours) should be considered when there is pretreatment or antibiotic therapy is directed at CoNS.

Biofilm-forming methicillin-resistant Staphylococcus aureus (MRSA) is an emerging pathogen that adversely affects animal and human health. World Health Organization (WHO) has designated MRSA as a high-priority pathogen for research and development. In this study, we have investigated the pathogenic potential of MRSA recovered from mastitic milk of cow. The MRSA was initially characterized for coagulase, haemolytic and DNase activity followed by its biofilm forming ability. Further, an intravenous murine model of MRSA was developed using multiparameter approach comprising of disease activity score, viable bacterial count in blood and tissues; and, detection of biofilm mass in tissue. Infection was successfully established in mice following intravenous inoculation of 3 × 108 colony forming unit (CFU) per mL of MRSA. Fifty percent of MRSA-challenged mice died after infection whereas mice survived exhibited disease activity score >25. Significantly higher MRSA count was recorded in blood, liver and kidney of MRSA-challenged mice as compared to healthy mice (P <0.05). Gram staining revealed the presence of varied size of multiple clusters of Gram-positive biofilm mass in the liver and kidney of MRSA-challenged mice. This study on pathogenesis of MRSA in mice would be useful in not only controlling the MRSA infection, but also in the development of effective therapeutics. [ABSTRACT FROM AUTHOR]

To decrease the incidence of coagulase positive Staphylococcus aureus (CPSA) in chicken meat, chitosan-based films incorporated with carvacrol nano-emulsion (Ch-CNE) and rosemary nanoemulsion (Ch-RNE) were used as an ideal solution to build effective antibacterial food packaging. CPSA was isolated from fresh and frozen chicken meat by using selective media. The prepared nanoemulsions were characterised using a zeta-sizer, Fourier-transform infrared spectroscopy (FTIR), transmission electron microscopy (TEM) and investigated for anti-CPSA activity by the agar diffusion method. The filmogenic mixture was prepared at 0.78% and 1.56% concentrations and then cast, dried, and assessed for physical and mechanical properties. CPSA was isolated from fresh and frozen chicken meat in percentages of 40% and 12%, respectively. The droplet sizes of the CNE and RNE were 54.56 and 44.98 nm, respectively, whereas those by TEM were spherically shaped with average sizes of 40.33 and 48.78 nm and polydispersity indices (PDI) of 0.32 and 0.21, respectively. The minimum inhibitory concentrations of both nano-emulsions against CPSA were 0.78% and 1.56%. Incorporated nano-emulsions with chitosan-based films did not cause a great change in the film appearance and transparency except for Ch-CNE films at 1.56%, which was significantly different in comparison with the control, and enhanced the light barrier property. Additionally, it caused significantly improved changes to the film including physical (water resistance and water vapour permeability) and mechanical (tensile strength and elongation at break) properties and significantly eradicated the CPSA inoculated in chicken meat (6 log10 CFU/cm2) on the 4th day of refrigerated storage (4±1 °C) with good organoleptic properties for 12 days. The Ch-CNE at concentration 1.56% could be considered a promising antimicrobial food packaging material with considerable beneficial packaging properties, substantial inhibition of foodborne pathogen growth, and extension of food shelf life. Staphylococcus schleiferi and Staphylococcus coagulans are opportunistic pathogens of animals and humans. They were previously classified as Staphylococcus schleiferi subs. schleiferi and Staphylococcus schleiferi subs. coagulans, respectively, and recently reclassified as separate species. S. coagulans, is frequently associated with dogs, whereas S. schleiferi is more commonly isolated from humans. Coagulase activity status is a defining characteristic of the otherwise closely related species. However, the use of coagulase tests originally developed to distinguish S. aureus from non-coagulase-producing staphylococci, for this purpose is questionable and the basis for their host preference has not been elucidated. In the current study, a putative coa gene was identified and correlated with coagulase activity measured using a chromogenic assay with human and bovine prothrombin (closely related to canine prothrombin). The results of the tests performed with human prothrombin showed greater reactivity of S. coagulans isolates from humans than isolates obtained from dogs with the same substrate. Our data suggest that unlike S. coagulans isolates from humans, isolates from dogs have more coagulase activity with bovine prothrombin (similar to canine prothrombin) than human prothrombin. Differences in nuc and 16s rRNA genes suggest a divergence in S. coagulans and S. schleiferi. Phenotypic and genotypic variation based on the number of IgG binding domains, and the numbers of tandem repeats in C-terminal fibronectin binding motifs was also found in protein A, and fibronectin-binding protein B respectively. This study identified a coa gene and associated phenotypic activity that differentiates S. coagulans and S. schleiferi and identified key phylogenetic and phenotypic differences between the species. Aims: This study was aimed to monitor the asymptomatic carriage of coagulase-positive staphylococcal bacteria among university male students and detect the prevalence of virulence marker genes that encode methicillin resistance (mecA) and Panton-Valentine leukocidin (PVL) toxin among the isolates. Methodology and results: Single nasal swaps were collected from 144 participating students who resided at four different locations within Al-Madinah city. A total of 112 Gram-positive staphylococcal isolates were recovered from the 144 participants (carriage rate of 77.8%). Coagulase-positive staphylococci were differentiated using duplex PCR amplification of the 16S rRNA and nuc genes and accounted for 30 isolates (carriage rate of 20.8%). These isolates were most prevalent in the northern and southern parts of Al-Madinah city, while the lowest numbers of isolates were detected in students of the eastern part. Coagulase-positive isolates were further phenotypically characterized for methicillin resistance by the disc diffusion method. Uniplex PCR assays were conducted to screen for mecA- and PVL toxin-encoding genes. The mecA gene was amplified from all 15 (50%) methicillin-resistant coagulase-positive isolates, while the PVL toxin-encoding gene was detected in 19 isolates (63.3%), 10 (33.3%) of which contained the mecA gene. Lastly, PCR amplification of the NRPS gene from coagulase-positive isolates revealed the absence of Staphylococcus argenteus, the recently discovered genetically divergent lineage of Staphylococcus aureus. Conclusion, significance and impact of study: An elevated prevalence of coagulase-positive isolates harboring mecA and PVL virulence genes was observed compared with previous investigations. Ice cream is a widely consumed refreshing food, especially in the summer months, and it creates a suitable environment for microbial growth due to its high nutritional value, appropriate pH, and long storage time. This study aimed to evaluate the incidence of enterotoxigenic and meticillinresistant Staphylococcus aureus in unpackaged ice cream samples sold in Istanbul, Turkey. For that purpose, 101 unpackaged ice cream samples were collected and evaluated for coagulase activity, DNase activity, and methicillin resistance. Staphylococcus aureus is determined in 66 (65.3%) samples with a mean load of 3.2±1.0 log cfu/mL. 34.6% of samples exceeded the Turkish Food Codex (2009) limit, which ranged between 10² – 10³ cfu/g. It was found that 61 (64.8%) of the S. aureus strains were coagulase positive, 21 (22.3%) were DNase positive, and 18 (19.1%) had methicillin resistance. The present study's findings revealed that unpackaged ice cream samples in Istanbul have high enterotoxin-producing potential with a high S. aureus load. This study aimed to investigate the presence of mecA and pvl genes in coagulase negative Staphylococcus (CNS) species isolated from bovine mastitis in smallholder dairy farms by using PCR. A total of 602 mammary quarter milk samples belong to 170 cows with mastitis were used. Identification of species was achieved by using the commercial Gram-positive identification kit and a total of 52 (8.6%) CNS species were isolated. The most frequently isolated species was Staphylococcus capitis (n = 15, 28.8%), followed by Staphylococcus saccharolyticus (n = 12, 23.1%), Staphylococcus simulans (n = 8, 15.4%), Staphylococcus haemolyticus (n = 5, 9.6%), Staphylococcus cohnii (n = 4, 7.7%), Staphylococcus lentus (n = 4, 7.7%), Staphylococcus epidermidis (n = 2, 3.8%) and Staphylococcus saprophyticus (n = 2, 3.8%). The mecA gene positivity was found in the 13 (25%) of strains. Of the strains carrying mecA gene, eight also harbored the pvl gene. A total of pvl gene positivity was found as 30.8% (n = 16) in 52 CNS species. In conclusion, the present study showed that CNS isolated from cows with mastitis may be reservoir of mecA and pvl genes. To our knowledge, this is the first study showing the presence of mecA and pvl genes in CNS species isolated from bovine with mastitis in the smallholder dairy farms in Turkey. Context • Postoperative bleeding after resection of colon polyps (CPs) is an extremely common adverse event with endoscopic treatment. Hemocoagulase Bothrops Atrox (HBA) is a newly discovered hemostatic substance that contains thrombin-like and coagulation kinase-like enzymes. However, research is lacking about its use for the treatment of intestinal polyps. Objective • The study intended to examine the hemostatic efficacy and safety of a local spray treatment with HBA, derived from HBA for injection, after CP resection, to provide a new hemostatic method, support HBA’s use, and provide evidence for clinical decision making. Design • The research team performed a randomized controlled study. Setting • The study took place at the Affiliated Hospital of Hebei University in Baoding, Hebei, China. Participants • Participants were 200 patients with CP who received treatment at the hospital between December 2020 and December 2022. Intervention • The research team divided participants into two groups with 100 participants each, an intervention group and a control group, using the random number expression method. For hemostasis, the intervention group received a local spray treatment that used HBA for injection, and the control group received metal-clip closure or electrocoagulation. Outcome Measures • The research team measured: (1) the hemostatic efficacy; (2) clinical outcomes—time to hemostasis, hemostasis rate, rebleeding rate, and incidence of late postoperative bleeding; (3) at baseline and at 24h postintervention, the coagulation function—prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT), and fibrinogen (FIB); (4) at baseline and at 24h postintervention, PLT parameters—platelet count (PLT), procalcitonin (PCT), and mean platelet volume (MPV); (5) economic effects—total number of participants with hemostasis, hospital days, and total hospital costs; and (6) adverse reactions. Results • The total hemostatic efficacy for the intervention group was significantly higher than that of the control group (P = .027), and the time to hemostasis was significantly shorter (P < .001) and the hemostasis rate, rebleeding rate, and incidence of late postoperative bleeding were all significantly lower than those of the control group, at P=.009, P=.009, and P=.048, respectively. In addition, the intervention group’s postoperative PT, TT, APTT, FIB, and MPV were all significantly lower than those of the control group (all P<.05), while its PLT and PCT were significantly higher than those of the control group (both P < .05). The intervention group’s total number of participants with hemostasis, participants with hemostasis, hospital days, and total cost were significantly lower than those of the control group (all P<.05), while no significant difference existed between the groups in the incidence of adverse effects (P>.05). Conclusions • HBA has an excellent hemostatic effect on intestinal polypectomy, with convenient use and high safety. In the future, popularizing the use of HBA in the treatment of intestinal polypectomy can not only effectively guarantee the postoperative safety of patients but also could reduce their economic burden and improve the quality of clinical medical services. Background: The objective of this study was to isolate Staphylococcus spp. and to characterize the resistance profile in nasal samples from pigs slaughtered for consumption. Material and Methods: Intranasal swabs were collected from 100 pigs immediately after bleeding in a slaughterhouse located in the largest pork production region in Brazil, these samples were cultured and isolated to identify Staphylococcus spp. in coagulase positive (CoPS) and coagulase negative (CoNS) and molecular identification of Staphylococcus aureus and then subjected to the disk-diffusion test to identify the bacterial resistance profile and search for the mecA gene. Results: Of the 100 samples collected, it was possible to isolate 79 Staphylococcus spp., of these, 72.15% were classified as CoNS and 27.85% of the isolates classified as CoPS. Among the CoPS isolates, 77.27% were identified as S. aureus. Through the disk-diffusion test, it was possible to verify isolates resistant to clindamycin and erythromycin (98.73%), chloramphenicol (93.67%), and doxycycline (89.87%). There was amplification of the mecA gene in 30.38% of Staphylococcus spp. Conclusion: The results of this study highlight the need for the careful use of antibiotics in swine production, in addition to aiming at continuous surveillance in relation to the rate of multiresistant microorganisms within these environments, focused on large industrial centers; such results also indicate the importance of understanding, through future studies, possible pathways to transmission of these microorganisms directly, or indirectly, through meat products derived from these pigs, which can be considered neglected diffusers of variants of Staphylococcus spp. resistant to antibiotics or carriers of important resistance genes related to One Health. Staphylococcus aureus pathology is caused by a plethora of virulence factors able to combat multiple host defence mechanisms. Fibrinogen (Fg), a critical component in the host coagulation cascade, plays an important role in the pathogenesis of this bacterium, as it is the target of numerous staphylococcal virulence proteins. Amongst its secreted virulence factors, coagulase (Coa) and Extracellular fibrinogen-binding protein (Efb) share common Fg binding motives and have been described to form a Fg shield around staphylococcal cells, thereby allowing efficient bacterial spreading, phagocytosis escape and evasion of host immune system responses. Targeting these proteins with monoclonal antibodies thus represents a new therapeutic option against S. aureus. To this end, here we report the selection and characterization of fully human, sequence-defined, monoclonal antibodies selected against the C-terminal of coagulase. Given the functional homology between Coa and Efb, we also investigated if the generated antibodies bound the two virulence factors. Thirteen unique antibodies were isolated from naïve antibodies gene libraries by antibody phage display. As anticipated, most of the selected antibodies showed cross-recognition of these two proteins and among them, four were able to block the interaction between Coa/Efb and Fg. Furthermore, our monoclonal antibodies could interact with the two main Fg binding repeats present at the C-terminal of Coa and distinguish them, suggesting the presence of two functionally different Fg-binding epitopes. Background: Coagulase-Negative Staphylococci (CoNS) are opportunistic and nosocomial pathogens. The excessive use of antimicrobial agents, including antiseptics, represents one of the world's major public health problems. This study aimed to test the susceptibility of CoNS to antiseptics. Methods: Out of 250 specimens collected from different sections of the hospital, 55 samples were identified as CoNS, categorized into three groups based on their sources: environmental samples (n = 32), healthcare worker carriers samples (n = 14), and clinical infection samples (n = 9). Isolates were examined for susceptibility to antibiotics and antiseptics, such as benzalkonium chloride (BC), cetyltrimethylammonium bromide (CTAB), and chlorhexidine digluconate (CHDG). Mupirocin and antiseptic resistance genes, as well as the mecA gene, were detected using polymerase chain reaction. CoNS isolates with notable resistance to antiseptics and antibiotics were identified using the API-Staph system. Results: A high frequency of multidrug resistance among CoNS clinical infection isolates was observed. Approximately half of the CoNS isolates from healthcare workers were susceptible to CHDG, but 93% were resistant to BC and CTAB. The frequency of antiseptics and antibiotics resistance genes in CoNS isolates was as follows: qacA/B (51/55; 92.7%), smr (22/55; 40.0%), qacG (1/55; 1.8%), qacH (6/55; 10.9%), qacJ (4/55; 7.3%), mecA (35/55; 63.6%), mupB (10/55; 18.2%), and mupA (7/55; 12.7%). A significant difference in the prevalence of smr gene and qacJ genes between CoNS isolates from healthcare workers and other isolates was reported (P value = 0.032 and ˂0.001, respectively). Four different CoNS species; S. epidermidis, S. chromogene, S. haemolyticus, and S. hominis, were identified by API. Conclusions: CoNS isolates colonizing healthcare workers showed a high prevalence of antiseptic resistance genes, while clinical infection samples were more resistant to antibiotics. The aim of current study was to determine in vitro digestibility, some microbiological properties and shelf life of fermented sucuk foods consisting mixture of animal and vegetable natural foods produced for dogs. Graininclusive and grain-free (GF) formulations were prepared. Grain-inclusive group was subgrouped as cooked (CG) and uncooked grain (UCG). In vitro digestibility, nutrient composition, pH and thiobarbituric acid reactive substances (TBARS) values of 3 groups of sucuk foods were determined at 0, 1, 3, and 6 months after production. Microbiological characteristics (Aerobic colony number, E. coli, coagulase positive Staphylococcus, coliform bacteria, yeast mold, Salmonella spp.) and lactic acid levels of sucuks were determined at the end of 1, 3, and 6-months of storage (+4°C). There were differences in nutrient compositions of groups and storage times within groups (P<0.05). All sucuk foods were negative for E. coli and Salmonella spp. Count of aerobic colonies were 5.8x107, 3.0x107 and 3.1x109 CFU/g in CG, UCG and GF, respectively. Total yeast-mold counts were between 5.5x103-9.6x104 CFU/g. The highest pH drop (5.38 to 4.25) and in vitro organic matter digestibility (92.02%) were determined in CG sucuk (P<0.05). TBARS value of UCG group was the highest at the end of the 6-months storage (P<0.05). Lactic acid levels were not different between storage times and groups (P>0.05). As a result, healthy and highly digestible sucuk foods were obtained for dogs, which contain sufficient and balanced nutrients and have a long shelf life. [ABSTRACT FROM AUTHOR]

Background & Objective: Staphylococcus aureus causes problems in hospitals and it has emerged as a serious agent acquired from the environment in recent years. One of the capabilities of S. aureus is the formation of biofilms, in which bacteria can exchange antibiotic-resistance genes among themselves and increase the virulence of other strains of this species (S. aureus). A surface protein attached to the cell wall in S. aureus clumping factor A is a virulence factor in various staphylococcal infections. Materials & Methods: In this study, after the Urea Analysis (UA) test, the urea culture test was applied to the blood agar and Baird-Parker Agar culture media from the infectious urine samples in Imam Hospital, Tehran, to identify S. aureus isolates. Finally, a molecular method was used for the confirmation of identified isolates. The microliter plate method was performed to determine the biofilm formation ability. The disk diffusion method was also used for profiling the antibiotic resistance of the isolates. Results: In the results of this study, 45 out of 160 urinary clinical samples were positive for S. aureus, among which 42 isolates expressed the clfA gene. Moreover, 39 isolates had the ability to form biofilms in vitro. Among these 42 isolates, the highest (88%) and the lowest (16%) rates of antibiotic resistance were observed against penicillin and cefoxitin, respectively. Data analysis with SPSS software and chi-square indicated a significant relationship between gene expression and biofilm production with antibiotic resistance (P < 0.05). Conclusion: The resistance of S. aureus bacteria is increasing strongly due to the repeated use of antibiotics such as beta-lactams, especially in respiratory infections and pharyngitis. Moreover, biofilm formation and virulence factors, such as clfA and clfB, cause concerns to the World Health Organization for treatment, especially for people with sepsis or toxemia.

Staphylococcus aureus is a major human pathogen that utilises many surface-associated and secreted proteins to form biofilms and cause disease. However, our understanding of these processes is limited by challenges of using fluores-cent protein reporters in their native environment, because they must be ex-ported and fold correctly to become fluorescent. Here, we demonstrate the feasibility of using the monomeric superfolder GFP (msfGFP) exported from S. aureus. By fusing msfGFP to signal peptides for the Secretory (Sec) and Twin Arginine Translocation (Tat) pathways, the two major secretion pathways in S. aureus, we quantified msfGFP fluorescence in bacterial cultures and cell-free supernatant from the cultures. When fused to a Tat signal peptide, we detect-ed msfGFP fluorescence inside but not outside bacterial cells, indicating a fail-ure to export msfGFP. However, when fused to a Sec signal peptide, msfGFP fluorescence was present outside cells, indicating successful export of the msfGFP in the unfolded state, followed by extracellular folding and maturation to the photoactive state. We applied this strategy to study coagulase (Coa), a secreted protein and a major contributor to the formation of a fibrin network in S. aureus biofilms that protects bacteria from the host immune system and increases attachment to host surfaces. We confirmed that a genomically inte-grated C-terminal fusion of Coa to msfGFP does not impair the activity of Coa or its localisation within the biofilm matrix. Our findings demonstrate that msfGFP is a good candidate fluorescent reporter to consider when studying proteins secreted by the Sec pathway in S. aureus.

Background Coagulase-negative staphylococci (CoNS) has become increasingly prevalent as the leading cause of late-onset bacteremia in neonatal ICUs. Diagnosing CoNS septicemia poses challenges because this bacterium commonly resides on the skin, leading to potential contamination of blood culture samples. Therefore, this study aimed to identify the bacterial causes of sepsis in neonates who exhibit clinical signs of the condition, also to assess the methicillin susceptibility of CoNS through both phenotypic and molecular methodologies. Materials and methods This study was carried out at Mansoura University Children Hospital. Blood samples were obtained and directly cultured using the BACT/ALERT system. All isolates were identified using BD Phoenix system. CoNS isolates were subjected to a disk diffusion susceptibility test with cefoxitin 30 µg, serving as an initial screening test to identify methicillin resistance. Automated antibiotic susceptibility was done using BD Phoenix system followed by PCR testing to detect the presence of the mecA gene in resistant CoNS isolates. Results Among all the isolated micro-organisms, Klebsiella pneumoniae and CoNS were found to be significantly higher in early-onset sepsis and late-onset sepsis, respectively. Staphylococcus epidermidis constituted 30% of the isolates, with Staphylococcus haemolyticus and Staphylococcus hominis each accounting for 20%, while other Staphylococcus species made up the remaining 20%. Among the identified CoNS, 47.4% tested positive for the mecA gene. The presence of the mecA gene was significantly associated with a higher incidence of late-onset sepsis (P=0.033). Conclusion CoNS are the predominant culprits behind late-onset sepsis in hospitalized newborns. Notably, the presence of the mecA gene was significantly linked to a higher occurrence of late-onset sepsis. Our study has uncovered a concerning surge in antibiotic resistance genes within our community. This underscores the urgency of implementing an active antibiotic surveillance program and exploring alternative therapeutic strategies to effectively combat neonatal sepsis. [ABSTRACT FROM AUTHOR]

The aim of the study was to investigate the effects of Staphylococcus xylosus 39, S. equorum 53, or S. vitulinus 75, previously isolated from pastırma, on the quality characteristics of pastırma, a Turkish dry-cured meat product, and to evaluate their potential use as starter cultures. The pastırma production was carried out with a traditional method. The control pastırma groups were manufactured without adding any starter culture. At the end of production, the groups were subjected to microbiological and physico-chemical analyses. The pH was above 5.5, and the aw value was below 0.90 in all groups. The strains used exhibited good adaptation to the pastırma. The S. equorum 53 decreased the thiobarbituric acid reactive substances (TBARS) value in pastırma, while the S. xylosus 39 increased the redness (a*) color value. The autochthonous strains caused a decrease in the palmitic acid (C16:0). However, they had no significant effect on the stearic acid (C18:0) and the oleic acid (C18:1n-9c). A total of 41 volatile compounds were identified in the groups. S. vitulinus 75 increased both benzaldehyde and 2-methyl-3-phenylpropanal levels. In addition, the principal component analysis (PCA) of volatile compounds provided a good separation, and PC1 separated S. xylosus 39 from other groups. [ABSTRACT FROM AUTHOR]

Staphylococcus schleiferi bacteremia is an underappreciated cause of septic shock in the critical care department. Although nominally a coagulase variable Staphylococcus and associated with otitis externa infections in canine species, it has been associated with the metastatic infection including osteomyelitis, endocarditis, nephritis, and meningitis in humans. This report records a possible zoonotic case of S. schleiferi subspecies coagulans bacteremia following canine otitis externa associated with septic shock and endovascular infection precipitating intensive care admission for vasopressor support in an immunocompetent male. The purpose of the current study was to evaluate the degree of contamination of dairy farms with both Pseudomonas aeruginosa and Staphylococcus aureus, in which, a total of 150 swab samples were collected from 3 different dairy farms (50 each) distributed in Assiut governorate (farm A) and El-Minia governorate (farms B & C), Egypt. The swabs were taken from different locations in the dairy farms such as teats of dairy animals, milking machines, workers' hands and stanchions, etc. All the samples were examined for detection of Ps. aeruginosa (conventionally biochemical followed by MICROBACT™ identification kits) and for Staph. aureus (coagulase +ve). The obtained results revealed the overall incidence of Ps. aeruginosa as 14%, 2% and 4% in farms A, B & C, respectively; while, the incidence of Staph. aureus was 4%, 30% and 4%, respectively. Total 200 sample comprising 100 samples of nasal swab and 100 milk samples were collected from cattle for isolation and characterization of Methicillin Resistant Staphylococcus aureus (MRSA). Samples were grown in oxacillin resistance selective supplement mixed aseptically in to sterile molten mannitol salt agar. Out of 200 samples 62 (23 from nasal swab and 39 from milk samples) isolates were showed growth on the media All the isolates were Gram positive cocci shaped and purple in colour. All the isolates were catalase positive. In nasal swab out of 23 isolates 20 (95.65%) isolates for slide coagulase, 19 (82.61%) isolates for tube coagulase test, 22 (95.65%) isolates for MR and 22 (95.65%) isolates for VP test, 22 (95.65%) isolates for haemolysis on blood agar, 20 (86.96%) for each DNase test, chromagar test and latex agglutination test. In milk samples out of 39 isolates 31 (79.49%) isolates for slide coagulase, 29 (74.35%) isolates for tube coagulase test, 37 (94.87%) isolates for MR and 37 (94.87%) isolates for VP test, 35 (89.74%) isolates for haemolysis on blood agar, 27 (69.23%) for each DNase test, chromagar test and latex agglutination test. Introduction. Staphylococcus (S.) aureus is one of the most omnipresent and dangerous human pathogens, whose main characteristic is the production of the enzyme coagulase. This characteristic serves to identify and assess the pathogenicity of the bacteria. In addition to skin infections, endocarditis, osteomyelitis, and infectious arthritis, it is a common cause of pneumonia both in children and adults. Case report. We described a case of a 65-year-old woman with a dry cough and malaise with patchy areas of consolidation on the chest X-ray and “ground-glass” opacity with bronchial wall thickening and unilateral mediastinal lymphadenopathy on chest computed tomography imaging. Methicillin-resistant S. aureus was isolated from the bronchoalveolar aspirate taken during bronchoscopy. The woman was empirically treated with azithromycin, and later, based on the antibiogram findings, azithromycin was replaced with meropenem, after which her health improved. Conclusion. We presented a rare case of pneumonia with unconvincing symptomatology and laboratory and radiological findings. Paying more attention to such cases in the future is crucial, especially to the use of antibiotics to which staphylococci are increasingly developing resistance.

Acute bacterial endocarditis is a rapid, difficult to manage, and frequently lethal disease. Potent antibiotics often cannot efficiently kill Staphylococcus aureus that colonizes the hearts valves. S. aureus relies on virulence factors to evade therapeutics and the hosts immune response, usurping the hosts clotting system by activating circulating prothrombin with staphylocoagulase and von Willebrand factor-binding protein. An insoluble fibrin barrier then forms around the bacterial colony, shielding the pathogen from immune cell clearance. Targeting virulence factors may provide previously unidentified avenues to better diagnose and treat endocarditis. To tap into this unused therapeutic opportunity, we codeveloped therapeutics and multimodal molecular imaging to probe the host-pathogen interface. We introduced and validated a family of small-molecule optical and positron emission tomography (PET) reporters targeting active thrombin in the fibrin-rich environment of bacterial colonies. The imaging agents, based on the clinical thrombin inhibitor dabigatran, are bound to heart valve vegetations in mice. Using optical imaging, we monitored therapy with antibodies neutralizing staphylocoagulase and von Willebrand factor-binding protein in mice with S. aureus endocarditis. This treatment deactivated bacterial defenses against innate immune cells, decreased in vivo imaging signal, and improved survival. Aortic or tricuspid S. aureus endocarditis in piglets was also successfully imaged with clinical PET/magnetic resonance imaging. Our data map a route toward adjuvant immunotherapy for endocarditis and provide efficient tools to monitor this drug class for infectious diseases.

Staphylococcus aureus pathology is caused by a plethora of virulence factors able to combat multiple host defence mechanisms. Fibrinogen (Fg), a critical component in the host coagulation cascade, plays an important role in the pathogenesis of this bacterium, as it is the target of numerous staphylococcal virulence proteins. Amongst its secreted virulence factors, coagulase (Coa) and Extracellular fibrinogen-binding protein (Efb) share common Fg binding motives and have been described to form a Fg shield around staphylococcal cells, thereby allowing efficient bacterial spreading, phagocytosis escape and evasion of host immune system responses. Targeting these proteins with monoclonal antibodies thus represents a new therapeutic option against S. aureus. To this end, here we report the selection and characterization of fully human, sequence-defined, monoclonal antibodies selected against the C-terminal of coagulase. Given the functional homology between Coa and Efb, we also investigated if the generated antibodies bound the two virulence factors. Thirteen unique antibodies were isolated from naïve antibodies gene libraries by antibody phage display. As anticipated, most of the selected antibodies showed cross-recognition of these two proteins and among them, four were able to block the interaction between Coa/Efb and Fg. Furthermore, our monoclonal antibodies could interact with the two main Fg binding repeats present at the C-terminal of Coa and distinguish them, suggesting the presence of two functionally different Fg-binding epitopes.

(1) Background: Prior to revision hip (THA) or knee arthroplasty (TKA), periprosthetic low-grade infection (PJI) should be ruled out. Despite advances in preoperative diagnosis, unsuspected positive cultures (UPCs) may occur in initially planned aseptic revisions. Particularly, single UPCs pose a diagnostic and therapeutic dilemma, as their impact on outcome is unclear and recommendations are heterogeneous. This review investigates the frequency of single UPCs and their impact on implant survivorship. (2) Methods: In July 2022, a comprehensive literature search was performed using PubMed and Cochrane Library search. In total, 197 articles were screened. Seven retrospective studies with a total of 5821 cases were able to be included in this review. (3) Results: Based on the cases included, UPCs were found in 794/5821 cases (14%). In 530/794 cases (67%), the majority of the UPCs were single positive. The most commonly isolated pathogens were coagulase negative Staphylococci and Cutibacterium acnes. Five of seven studies reported no influence on revision- or infection-free survival following a single positive culture. In two studies, single UPCs following THA revision were correlated with subsequent re-revision for PJI. (4) Conclusions: Single UPCs of a non-virulent pathogen following presumed aseptic TKA revision may be interpreted as contaminants. A single UPC following THA revision may be a risk factor for subsequent PJI. Bacteria that can be found in root canal with chronic apical abscess consist of some Streptococcus and Staphylococcus species. They can be eliminated with calcium hydroxide by releasing the hydroxyl ion. The research objective are identify species from Staphylococcus spp. and Streptococcus spp. in root canals which diagnosed chronic apical abscess and investigate their sensitivity against calcium hydroxide in paste and gel preparation. The methods of this research is true experimental. The sample were taken from the root canal with chronic apical abscess. Streptococcus spp. were identified with RAPID STR and Staphylococcus spp. were identified with RAPID STAPH PLUS. All the samples were tested with calcium hydroxide in paste and gel for measure the diameter of inhibition zones. Six species of Staphylococcus spp. identified are member of coagulase negative Staphylococcus (CoNS), one species of Streptococcus spp. identified is a member of viridans streptococci. Calcium hydroxide in paste and gel preparation produces antibacterial effect with similar inhibition zone diameter on Streptococcus spp. and Staphylococcus spp that have been identified. Streptococcus spp. and Staphylococcus spp. that identified found from the root canal of chronic apical abscess are highly sensitive and have similar sensitiveness to calcium hydroxide in paste and gel preparation. Staphylococci is an opportunistic bacterial population that is permanent in the normal flora of milk and poses a serious threat to animal and human health with some virulence factors and antibiotic-resistance genes. This study was aimed at identifying staphylococcal species isolated from raw milk and to determine hemolysis, biofilm, coagulase activities, and beta-lactam resistance. The raw milk samples were collected from the Düzce (Türkiye) region, and the study data represent a first for this region. The characterization of the bacteria was performed with MALDI-TOF MS and 16S rRNA sequence analysis. The presence of coa, icaB, blaZ, and mecA was investigated with PCR. A nitrocefin chromogenic assay was used for beta-lactamase screening. In this context, 84 staphylococci were isolated from 10 different species, and the dominant species was determined as S. aureus (32.14%). Although 32.14% of all staphylococci were positive for beta hemolysis, the icaB gene was found in 57.14%, coa in 46.42%, mecA in 15.47%, and blaZ in 8.33%. As a result, Staphylococcus spp. strains that were isolated from raw milk in this study contained some virulence factors at a high level, but also contained a relatively low level of beta-lactam resistance genes. However, considering the animal–environment–human interaction, it is considered that the current situation must be monitored constantly in terms of resistance concerns. Objective: To determine the diagnostic accuracy of CHROMagar Orientation for its ability to support the growth and identification of uropathogens by keeping API 20E/20NE as a gold standard. Study Design: Cross-sectional study. Place and Duration of Study: Department of Microbiology, Armed Forces Institute of Pathology, Rawalpindi Pakistan, from Jun to Nov 2019. Methodology: A total of 470 midstream specimen of urine from patients suspected of UTIs were analyzed. Urine specimens were inoculated by 1µL calibrated sterilized loop each on CLED Agar, Blood agar and CHROMagar Orientation media plates. Culture plates were incubated at 35.00±2.00ºC for 18 to 24 hours. As per standard protocols, isolates were identified by colony morphology, Gram stain, catalase, oxidase, DNAase, coagulase, and API 20E/20NE. In addition, the appearance of microorganism on CHROMagar Orientation media were identified according to the manufacturer's instructions. Results: Out of 470 samples, 211 showed no growth, 90 were classified as mix growth (polymicrobial), and 169 samples were positive (unimicrobial) for the growth of uropathogens. All the samples positive for bacterial growth on conventional media were also positive on CHROMagar orientation and vice versa. The sensitivity and specificity of CHROMagar Orientation for differentiation of uropathogens were 99% and 100%, respectively. Conclusion: The overall findings of our study suggested that CHROMagar supports the growth of all uropathogens. It provided the rapid identification of organisms based on their colour morphology, even in a mixed bacterial culture, growth within 24 hours easily. [ABSTRACT FROM AUTHOR]

Traditional cheeses are part of the Portuguese gastronomic identity, and raw milk of autochthonous species is a common primary ingredient. Here, we investigated the presence of Listeria monocytogenes, Coagulase Positive Staphylococci (CPS) and pathogenic Escherichia coli, as well as of indicator microorganisms (E. coli and other Listeria spp.) in 96 cured raw milk cheeses from the Alentejo region. Whole genome sequencing (WGS) of pathogenic E. coli and Listeria spp. as well as antimicrobial resistance (AMR) screening of E. coli isolates was also performed. L. monocytogenes, CPS > 104 cfu/g and Extraintestinal E. coli were detected in 15.6%, 16.9% and 10.1% of the samples, respectively. Moreover, L. monocytogenes > 102 cfu/g and Staphylococcal enterotoxins were detected in 4.2% and 2.2% of the samples, respectively. AMR was observed in 27.3% of the E. coli isolates, six of which were multidrug resistant. WGS analysis unveiled clusters of high closely related isolates for both L. monocytogenes and L. innocua (often correlating with the cheese producer). This study can indicate poor hygiene practices during milk collection/preservation or during cheese-making procedures and handling, and highlights the need of more effective prevention and control measures and of multi-sectoral WGS data integration, in order to prevent and detect foodborne bacterial outbreaks. The aim of this study was to identify the primary bacteria associated with clinical mastitis in dairy cows and to assess the antimicrobial resistance pattern of the commonest isolate using phenotypic methods. The milk samples were collected from 83 domesticated dairy cows suffering from clinical mastitis (CM), from an organised dairy farm as well as cows presented with CM at the University Veterinary Hospital (UVH), Mannuthy, Thrissur, Kerala. Microbiological analysis, using morphological, cultural and biochemical properties as well as molecular identification using polymerase chain reaction (PCR) revealed that coagulase-negative staphylococci (CNS) were the most predominant (40.31 per cent) bacteria, followed by Staphylococcus aureus (32.83 per cent), Micrococci spp. (11.94 per cent), Klebsiella spp. (7.46 per cent) Escherichia coli (4.48 per cent) and Streptococci (2.98 per cent). The majority of the bacteria were resistant to more than one class of antimicrobials (aminoglycosides, β lactams, fluoroquinolones or tetracyclines). Furthermore, our findings revealed that the CNS is highly resistant to β-lactam family of antibiotics and that CNS may play a significant role in clinical mastitis of dairy cows. Coagulase negative staphylococci (CNS) are emerging as the most prevalent causative agent of bovine mastitis. They are resistant to many commonly used antibiotics due to the presence of antimicrobial resistance (AMR) genes. A study was conducted to evaluate the AMR profiling of CNS isolated from bovine subclinical mastitis.Coagulase negative staphylococci were isolated from 49 (44.95 per cent) of the subclinical mastitis samples. Disc diffusion assay revealed that highest resistance was shown against gentamicin (42.85 per cent) followed by methicillin (32.6 per cent), ceftriaxone - tazobactam (24.48 per cent), enrofloxacin (20.4 per cent), tetracycline (16.32 per cent) and least resistance to cotrimoxazole (4 per cent). Genotypic characterisation of AMR genes such as mecA, aacA-aphD and norA by PCR was done for determining resistance to methicillin, gentamicin and fluroquinolone resistance. The CNS carried aacA-aphD, norA and mecA in 44.89 per cent, 32.65 per cent and 14.28 per cent, respectively. Comparison of phenotypic and genotypic characterisation of AMR in CNS was carried out by McNemar test and it was found that there was significant difference between the presence of mecA gene and methicillin resistance. Colonization of the skin by Staphylococcus aureus is associated with exacerbation of atopic dermatitis (AD), but any direct mechanism through which dysbiosis of the skin microbiome may influence the development of AD is unknown. Here, we show that proteases and phenol-soluble modulin α (PSMα) secreted by S. aureus lead to endogenous epidermal proteolysis and skin barrier damage that promoted inflammation in mice. We further show that clinical isolates of different coagulase-negative staphylococci (CoNS) species residing on normal skin produced autoinducing peptides that inhibited the S. aureus agr system, in turn decreasing PSMα expression. These autoinducing peptides from skin microbiome CoNS species potently suppressed PSMα expression in S. aureus isolates from subjects with AD without inhibiting S. aureus growth. Metagenomic analysis of the AD skin microbiome revealed that the increase in the relative abundance of S. aureus in patients with active AD correlated with a lower CoNS autoinducing peptides to S. aureus ratio, thus overcoming the peptides' capacity to inhibit the S. aureus agr system. Characterization of a S. hominis clinical isolate identified an autoinducing peptide (SYNVCGGYF) as a highly potent inhibitor of S. aureus agr activity, capable of preventing S. aureus-mediated epithelial damage and inflammation on murine skin. Together, these findings show how members of the normal human skin microbiome can contribute to epithelial barrier homeostasis by using quorum sensing to inhibit S. aureus toxin production.

The study describes the identification of the etiological agents of subclinical mastitis (SCM) among lactating dairy cows under different phases of lactation in an organised dairy farm of Thrissur district, Kerala. The preliminary diagnosis of subclinical mastitis (SCM) was made using California Mastitis test (CMT), a cow side field test. The somatic cell count (SCC) was estimated and the milk samples of affected quarters were subjected to bacterial isolation and identification. The predominant causative agents isolated from milk of cows with SCM were coagulase negative staphylococci (56.25 per cent) followed by S. aureus (25 per cent) and Micrococci spp. (18.75 per cent). Further species identification of the coagulase negative staphylococci revealed that the majority were S. xylosus (28.23 per cent) followed by S. scuiri (15.63 per cent), S. hominis (9.38 per cent) and S. schleiferi (3.13 per cent). Coagulase positive Staphylococci (CoPS) are the leading cause of canine cutaneous and otic infections. Virulence factors associated with Staphylococci include the expression of mec and panton-valentine leukocidin (pvl) genes. Methicillin-resistance (MR) is commonly associated with mecA gene expression, although a recently identified variant, mecC, has been reported. This study aims to evaluate the prevalence of mecA, mecC and pvl genes in 232 clinical isolates of CoPS collected from dogs with pyoderma. A multiplex PCR, and Kirby-Bauer disk diffusion susceptibility test for cefoxitin was performed for all isolates. PBP2a agglutination test was performed on 127 isolates. Standard MRSA isolates were used as positive controls. The mecA gene was identified in 149/232 isolates (64.2%): 116 S. pseudintermedius, 30 S. coagulans and three S. aureus. The pvl gene was present in only 1 isolate of S. pseudintermedius (0.4%), whereas no isolates carried the mecC gene. 34 isolates were resistant to cefoxitin (14.6%) and they were all mecA positive. The results of this study show an MR prevalence of 64.2% confirming concerns about antibiotic resistance in veterinary medicine. In conclusion, this is the first study analyzing the prevalence of mecC and pvl in comparison to mecA, in a large cohort of CoPS clinical isolates from dogs with pyoderma. A multimodal surveillance on the prevalence of mecC and pvl in veterinary medicine is essential to appropriate antimicrobial management. [ABSTRACT FROM AUTHOR]

Background: Staphylococcal mastitis is a major cause of concern to the dairy industry in India and several countries worldwide. Though Staphylococcus aureus is the major cause, coagulase negative staphylococcal species (CoNS) are being increasingly reported in recent years. Aims: To investigate the incidence of coagulase negative staphylococcal species in bovine mastitis. Methods: Isolation of staphylococci was carried out from 237 milk samples of cows and She buffaloes with clinical and subclinical mastitis from different regions of Andhra Pradesh and Karnataka. CoNS isolates were identified by tube coagulase test using fresh rabbit plasma and coagulase gene PCR. We employed the biochemical test scheme published elsewhere previously for identification of the CoNS isolates up to species and subspecies levels. Seven representative isolates were identified by 16S rDNA sequencing to check the accuracy of biochemical test based identification. Results: The CoNS constitute the majority of the staphylococcal isolates from mastitis (80/125, 64%) in this region. Using biochemical test scheme, the CoNS isolates from bovine mastitis were identified as S. cohnii sub sp. cohnii, S. simulans, S. capitis sub sp. capitis, S. cohnii sub sp. xylosus, and S. lugdunensis. The CoNS species S. schleiferi, S. haemolyticus, S. sciuri, S. xylosus, S. chromogenes, and Macrococcus epidermidis were identified by 16S rDNA sequencing. Conclusion: The 16S rDNA sequencing is the appropriate method for the identification of CoNS species. Background: Staphylococcus haemolyticus (S. haemolyticus) is one of the important organisms in hospital-acquired infections. This strain comprises many enzymes, cytolysins, and surface substances which contribute to its virulence. We in the present study tried to evaluate the prevalence of methicillin-resistant S. haemolyticus causing nosocomial infections in a Tertiary Care Hospital. Methods: A total of n = 95 non-repeated S. hemolyticus were isolated in the Department of microbiology out of n = 2116 various clinical specimen present. The strains collected were initially identified by colony morphology on Nutrient agar, Sheep blood agar, Chocolate agar, Uri Chrom agar, gram staining, catalase, coagulase (both slide coagulase and tube coagulase), mannitol fermentation, Amino acid decarboxylation. Results: n = 95 (4.5%) samples of n = 2116 samples were positive for S. haemolyticus. Most positive cultures of S. haemolyticus were from pus samples n = 40 out of n = 95. The susceptibility pattern revealed none were susceptible to penicillin and 94% susceptibility was found with Linezolid and 100% susceptibility to Nitrofurantoin. The vancomycin Minimum Inhibitory Concentration (MIC) results showed 71.57% (68/95) strains were susceptible; out of these 12 (12.63%) were showing MIC < 1 μg/mL, n = 56 isolates (58.94%) were showing MIC value less than n = 2 μg/mL, and none of them were less than MIC 0.5 μg/mL. Conclusion: In this study, variable antimicrobial resistance susceptibilities were shown by S. haemolyticus isolates. Some clinical situations may require a single strain of S. haemolyticus to be identified up to the species level with their antibiogram. Critical and timely detection of drug-resistant S. haemolyticus in hospital settings will be a helpful guide in the management and prevent further proliferation of drug resistance.
