BACKGROUND: Intra-articular injections containing a corticosteroid are used frequently, and periprosthetic joint infection is a serious complication after total joint arthroplasty. There is debate regarding whether intra-articular corticosteroid injections before arthroplasty increase periprosthetic joint infection after surgery. QUESTIONS/PURPOSES: (1) Does a previous intra-articular corticosteroid injection increase the odds of infection after subsequent hip or knee arthroplasty? (2) Does this risk vary based on how soon before the arthroplasty (such as less than 3 months before surgery) the injection is administered? METHODS: Using the PubMed, Embase, Cochrane Library, and Web of Science databases from inception to July 2021, we searched for comparative studies in English on patients who received intra-articular corticosteroid injections before arthroplasty and that tracked the frequency of infection after arthroplasty. We extracted data on the risk of infection after subsequent joint arthroplasty. The keywords included “corticosteroid,” “steroid,” “arthroplasty,” “knee replacement,” and “hip replacement.” Eleven retrospective, comparative studies from four countries were included, of which 10 reported the specific diagnosis criteria and one did not. These articles included data on 173,465 arthroplasties in the hip or knee, as well as of 73,049 injections and 100,416 control patients. The methodologic quality of the included studies was evaluated according to the Newcastle-Ottawa Quality Assessment Scale; the articles’ scores ranged from 6 to 7 (the score itself spans 0 to 9, with higher scores representing better study quality). We found no evidence of publication bias based on the Egger test, and tests of heterogeneity generally found heterogeneity, so a random-effects model was used of our meta-analyses. A meta-analysis was performed with Review Manager 5.3 software and Stata version 12.0 software. RESULTS: Overall, there were no differences in the odds of periprosthetic joint infection between the injection group and the control group among patients who received any kind of injection (odds ratio 1.22 [95% CI 0.95 to 1.58]; p = 0.12). However, in a subgroup analysis, there was a higher OR for postoperative PJI in patients with an intra-articular corticosteroid injection in the knee or hip within 3 months (OR 1.39 [95% CI 1.04 to 1.87]; p = 0.03). There were no differences in the infection risk in patients who had injections between 3 and 6 months before arthroplasty (OR 1.19 [95% CI 0.95 to 1.48]; p = 0.13) or between 6 and 12 months before arthroplasty. CONCLUSION: The current evidence suggests ipsilateral intra-articular corticosteroid injections within 3 months before arthroplasty were associated with an increased risk of periprosthetic joint infection during subsequent joint arthroplasty. We recommend against performing total joint arthroplasty on a patient who has received an intra-articular corticosteroid injection within 3 months. Further high-quality studies on this topic from registries, national databases, or insurance company data are still required to confirm and extend our findings. The relative impact of soft tissue balance and knee laxity on clinical outcomes after total knee arthroplasty (TKA) is not fully understood. We analyzed associations among knee laxity, kinematics, and patient-reported outcomes.Knee Society Scores were recorded in 67 patients before and after primary TKA for osteoarthritis with varus deformity (N = 78). Varus and valgus laxity was measured in 78 knees using stress radiographs. Knee kinematics were measured fluoroscopically during stair ascent in 16 knees.On average, varus laxity decreased significantly, and valgus laxity increased significantly after TKA, although the net combined varus-valgus laxity did not change significantly. Postoperatively, the magnitude of valgus laxity correlated significantly with greater patient symptoms and satisfaction scores. Patients with medial pivot kinematics scored higher on patient satisfaction and standard activities.We found that postoperative valgus laxity and medial pivot kinematics were significantly associated with better clinical outcomes. These results emphasize the importance of careful preoperative planning and attention to intraoperative alignment and ligament balancing.

Knee stability is the ability for the joint to maintain an appropriate functional position throughout its range of motion. Knee instability can be defined as excessive laxity during activities of daily living. Intraoperative knee laxity can be affected by implant design, alignment of components, and soft-tissue balancing. Soft-tissue balance is a major contributor to knee instability. Mechanical balancing instruments can be classified as spacer blocks or joint-distraction devices. Conventional wisdom favors rectangular and equal flexion-extension gaps. However, knee balance is elusive even with mechanical balancing instruments. First-generation electronic balancing devices are equivalent in concept to spacer blocks instrumented with force sensors. Second-generation electronic balancing devices are equivalent in concept to mechanical distraction devices instrumented with pressure and displacement sensors. Electronic ligament balancers can be useful in documenting intraoperative knee laxity for quantifiable correlation with postoperative outcomes, thus directly relating postoperative stability to surgical balance, and may predict outcomes and knee stability.

The impact of self-reported metal allergy (SRMA) in total knee arthroplasty (TKA) remains controversial. In the absence of objective tests, SRMA is often used as a screening tool for implant selection. The objective of this study was to determine the effect of SRMA on early outcomes after TKA. Between 2010 and 2014, 168 patients with SRMA underwent TKA; 150 (89%) received nickel-free implants, and 18 (11%) received cobalt-chrome implants that contained nickel. Mean age was 67 years, and 95% were female. A cohort of 858 TKA patients (mean age, 68 years) without SRMA matched by sex served as the control group. Outcomes included Knee Society Score (function [KSS-F] and knee [KSS-K]), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) scores, knee flexion, further surgery, and complications. No differences were seen in KSS-F and KSS-K between patients with and without SRMA. The mean WOMAC pain scores were 89.1 for patients with SRMA and 85.2 for patients without SRMA ( P =.030). Stiffness and physical function scores were similar. Knee flexion was similar. No differences were found between nickel-free and cobalt-chrome SRMA groups. Patients with SRMA and those without demonstrated similar early functional outcomes. Patients with SRMA who received standard cobalt-chrome implants had no significant difference in functional outcomes compared with patients with nickel-free implants. Background Nearly 14% to 39% TKA patients report dissatisfaction causing incomplete return of function. We proposed that the kinematics of knees implanted with patient-specific prostheses using patient-specific cutting guides would be closer to normal. Methods Eighteen matched cadaver lower limbs were randomly assigned to two groups: group A was implanted with patient-specific implants using patient-specific cutting guides; group B, the contralateral knee, was implanted with a standard design using intramedullary alignment cutting guides. Knee kinematics were measured on a dynamic closed-kinetic-chain Oxford knee rig, simulating a deep knee bend and in a passive rig testing varus–valgus laxity. Results The difference from normal kinematics was lower for group A compared to group B for active femoral rollback, active tibiofemoral adduction, and for passive varus–valgus laxity. Conclusions Our results support the hypothesis that knees with patient-specific implants generate kinematics more closely resembling normal knee kinematics than standard knee designs. Clinical Relevance Restoring normal kinematics may improve function and patient satisfaction after total knee arthroplasty.

Background Knee instability is emerging as a major complication after total knee arthroplasty (TKA), with ligament laxity and component alignment listed as important contributory factors. Knee balancing remains an art and is largely dependent on the surgeon's subjective “feel.” The objectives were to measure the accuracy of an electronic balancing device to document the magnitude of correction in knee balance after soft-tissue releases and measure change in knee laxity after medial release. Methods The accuracy of a second-generation electronic ligament-balancing device was compared with that of 2 mechanical balancing instruments. TKA was performed in 12 cadaver knees. Soft-tissue balance was measured sequentially before TKA, after mounting a trial femoral component, after medial release, and after resecting the posterior cruciate ligament. Coronal laxity of the knee under a 10 Nm valgus moment was measured before and after medial release. Results The electronic balancing instrument was more accurate than mechanical instruments in measuring distracted gap and distraction force. On average, before TKA, the flexion gap was wider than the extension gap, and the medial gap was tighter than the lateral gap. Medial release increased the medial gap in flexion and increased passive knee valgus laxity. Posterior cruciate ligament release increased the tibiofemoral gap in both flexion and extension with a greater increase in the lateral gap. Conclusion The second-generation electronic balancing device was significantly more accurate than mechanical instruments and could record knee balance over the entire range of flexion. More accurate soft-tissue balance may enhance outcomes after TKA.

Hip dislocation is a major short-term complication after total hip arthroplasty (THA). One factor thought to reduce the risk for dislocation is head size. We constructed subject-specific computer models to study the effect of head size on risk for postoperative dislocation. Femoral and acetabular geometry was constructed after segmenting CT scans of nine hips. CAD models of THA components with four head diameters (28, 32, 36, and 44 mm) were virtually implanted. Hip capsular ligaments were simulated using rigid-body ellipsoids connected by non-linear springs. Posterior dislocation was simulated during a rise from a low chair; anterior dislocation was simulated during a pivot activity. Intraoperative stability tests were simulated for anterior or posterior dislocation. While rising from a low chair (posterior dislocation) and during the pivot activity (anterior dislocation), increasing head size significantly increased hip flexion angle at dislocation and generated higher dislocation moments. Larger heads reduced the risk for dislocation. Intraoperative stability tests detected the relative increased resistance to dislocation despite differences in the absolute magnitude of moments. This model can be useful preclinical tool for assessing design changes, the effect of component placement, and the activity-based risk for dislocation. © 2014 Orthopaedic Research Society. Malalignment of the tibial tray in total knee arthroplasty has been linked to several complications including tibial tray subsidence, leading to revision surgery. However, quantitative biomechanical evidence to directly support the mechanism of failure is not available. We developed a model to study tibial tray micromotion and subsidence in vitro under multiaxial physiological loading conditions for up to 100,000 cycles. In Phase I, we tested four cadaver knees and two surrogate bone models to determine whether the surrogate models could reproduce the fatigue damage induced by cyclic loading. In Phase II, we tested six cadaver knees in a pairwise manner under conditions representing either neutral or varus malalignment. The surrogate bone models did not reproduce the progressive damage that was seen in human cadaver specimens. The altered loading conditions used to represent varus tray alignment increased the cyclic strain at the start of the fatigue loading and increased the cyclic strain at the end of the fatigue loading as well as the subsidence of the tray. The increase in final cyclic strain was greater in the varus condition indicating reduction in stiffness due to bone damage. This in vitro testing represents the clinical reports of early tray migration that lead to eventual aseptic loosening. This study provides biomechanical evidence supporting the hypothesis that a varus tray alignment could increase the risk of failure.

This study compares the incidence of venous thromboembolic event (VTE) in 55 patients (110 knees) undergoing simultaneous bilateral TKA with 287 patients (287 knees) undergoing unilateral TKA using a mobile compression device as monotherapy prophylaxis in both groups. All patients were clinically evaluated 3months after surgery with symptomatic confirmed VTE as an endpoint. Deep venous thrombosis (DVT) was documented by duplex ultrasound and pulmonary embolism (PE) was documented by spiral CT. The simultaneous bilateral TKA group had 6 VTEs (10.9%) with 2 PEs (3.6%). The unilateral TKA group had 9 VTEs (3.1%), and 0 PE. Patients undergoing simultaneous bilateral TKA yielded more than twice the rate of VTE compared with patients undergoing unilateral TKA using a mobile compression device as sole thromboprophylactic modality.

Mechanical loading is believed to be a critical factor in the development and treatment of knee osteoarthritis. However, the contact forces to which the knee articular surfaces are subjected during daily activities cannot be measured clinically. Thus, the ability to predict internal knee contact forces accurately using external measures (i.e., external knee loads and muscle electromyographic [EMG] signals) would be clinically valuable. We quantified how well external knee load and EMG measures predict internal knee contact forces during gait. A single subject with a force-measuring tibial prosthesis and post-operative valgus alignment performed four gait patterns (normal, medial thrust, walking pole, and trunk sway) to induce a wide range of external and internal knee joint loads. Linear regression analyses were performed to assess how much of the variability in internal contact forces was accounted for by variability in the external measures. Though the different gait patterns successfully induced significant changes in the external and internal quantities, changes in external measures were generally weak indicators of changes in total, medial, and lateral contact force. Our results suggest that when total contact force may be changing, caution should be exercised when inferring changes in knee contact forces based on observed changes in external knee load and EMG measures. Advances in musculoskeletal modeling methods may be needed for accurate estimation of in vivo knee contact forces.

Lower body positive pressure (LBPP) treadmills permit significant unweighting of patients and have the potential to enhance recovery following lower limb surgery. We determined the efficacy of an LBPP treadmill in reducing knee forces in vivo. Subjects, implanted with custom electronic tibial prostheses to measure forces in the knee, were tested on a treadmill housed within a LBPP chamber. Tibiofemoral forces were monitored at treadmill speeds from 1.5 mph (0.67 m/s) to 4.5 mph (2.01 m/s), treadmill incline from � 108 to þ108, and four treadmill chamber pressure settings adjusted to decrease net treadmill reaction force from 100% to 25% of the subject's body weight (BW). The peak axial tibiofemoral force ranged from 5.1 times BW at a treadmill speed of 4.5 mph (2.01 m/s) and a pressure setting of 100% BW to 0.8 times BW at 1.5 mph (0.67 m/s) and a pressure setting of 25% BW. Peak knee forces were significantly correlated with walking speed and treadmill reaction force (R 2 ¼ 0.77, p ¼ 0.04). The LBPP treadmill might be an effective tool in the rehabilitation of patients following lower-extremity surgery. The strong correlation between tibiofemoral force and walking speed and treadmill reaction forces allows for more precisely achieving the target knee forces desired during early rehabilitation. 2012 Orthopaedic Research Society. Introduction Malalignment can result in poor clinical outcomes and increased wear. However, component malalignment can occur even when overall limb mechanical axis is within the normal anatomic range. We studied the effect of component malalignment in the presence of acceptable knee alignment in knee arthroplasty. Methods Sixteen tibial inserts retrieved at revision surgery were laser-mapped to measure wear. Average implantation duration was 7.7 years (range, 1 to 13). Early (postprimary) and final (prerevision) radiographs were analyzed for overall alignment (limb, femoral and tibial components) and osteolysis. Results The tibial components were initially aligned in a mean of 1.3 ± 1.7° varus (range, –1.5 to 4.5°), which increased to 3.2 ± 2.9° (range, –2.0 to 8.0°) at the time of revision (p = 0.05). Tibial components initially placed in greater than 3° varus were associated with almost twice the volumetric penetration rate. Anatomic knee angles were 5.4 ± 0.9° valgus (range, 4.0 to 7.0°) in the post-primary radiographs and decreased in prerevision radiographs to 3.8 ± 2.6° (range, –1.0 to 7.5°), (p = 0.04). Discussion Tibial varus was associated with increased medial compartment wear and total wear, thus affecting osteolysis in addition to local destruction of the bearing surface. Varus malalignment as low as 3° may result in accelerated wear, even if overall limb alignment is nearly ideal. These results indicate that tibial component alignment is an important factor associated with tibial tray subsidence and polyethylene wear even when limb alignment is neutral.

We constructed patient-specific models from computed tomography data after total knee arthroplasty to predict knee flexion based on implant-bone impingement. The maximum flexion before impingement between the femur and the tibial insert was computed using a musculoskeletal modeling program (KneeSIM; LifeModeler, Inc, San Clemente, California) during a weight-bearing deep knee bend. Postoperative flexion was measured in a clinical cohort of 21 knees (low-flex group: 6 knees with 125° of flexion at 2 years). Average predicted flexion angles were within 2° of clinical measurements for the high-flex group. In the low-flex group, 4 cases had impingement involving the bone cut at the posterior condyle, and the average predicted knee flexion was 102° compared with 93° measured clinically. These results indicate that the level of the distal femoral resection should be carefully planned and that exposed bone proximal to the tips of the posterior condyles of the femoral component should be removed if there is risk of impingement.

Dynamic loading and perfusion culture environments alone are known to enhance cartilage extracellular matrix (ECM) production in dedifferentiated articular chondrocytes. In this study, we explored whether a combination of these factors would enhance these processes over a free-swelling (FS) condition using adult human articular chondrocytes embedded in 2% alginate. The alginate constructs were placed into a bioreactor for perfusion (P) only (100 μL/per minute) or perfusion and dynamic compressive loading (PL) culture (20% for 1 h, at 0.5 Hz), each day. Control FS alginate gels were maintained in six-well static culture. Gene expression analysis was conducted on days 7 and 14, while cell viability, immunostaining, and mechanical property testing were performed on day 14 only. Total glycosaminoglycan (GAG) content and GAG synthesis were assessed after 14 days. Col2a1 mRNA expression levels were significantly higher (at least threefold; p0.05) in both bioreactor conditions compared with FS by days 7 and 14. For all gene studies, no significant differences were seen between P and PL treatments. Aggrecan mRNA levels were not significantly altered in any condition although both GAG/DNA and (35)S GAG incorporation studies indicated higher GAG retention and synthesis in the FS treatment. Collagen type II protein deposition was low in all samples, link protein distribution was more diffuse in FS condition, and aggrecan deposition was located in the outer regions of the alginate constructs in both bioreactor conditions, yet more uniformly in the FS condition. Catabolic gene expression (matrix metalloproteinase 3 [MMP3] and inducible nitric oxide synthase [iNOS]) was higher in bioreactor conditions compared with FS, although iNOS expression levels decreased to approximately fourfold less than the FS condition by day 14. Our data indicate that conditions created in the bioreactor enhanced both anabolic and catabolic responses, similar to other loading studies. Perfusion was sufficient alone to promote this dual response. PL increased the deposition of aggrecan surrounding cells compared with the other conditions; however, overall low GAG retention in the bioreactor system was likely due to both perfusion and catabolic conditions created. Optimal conditions, which permit appropriate anabolic and catabolic processes for accumulation of ECM and tissue remodeling for neocartilage development, specifically for humans, are needed.

Quantifying muscle and joint forces over a broad range of weight bearing loads during exercise may provide data required to improve prosthetic materials and better protect against muscle and bone loss. Collectively, leg intramuscular pressure (IMP), ground reaction force (GRF), and the instrumented tibial tray force measurements provide a comprehensive assessment of leg muscle and joint biomechanical effects of gravity during exercise. Titration of body weight (BW) by lower body negative pressure (LBNP) and lower body positive pressure (LBPP) can reproducibly modulate IMP within leg muscle compartments. In addition, previous studies document peak tibial forces during various daily activities of 2.2 to 2.5 BW. The study objective was to determine the IMPs of the leg, axial compressive force on the tibia in vivo, vertical GRF, and knee range of motion during altered BW levels using LBPP and LBNP treadmill exercise. We hypothesize that peak GRF, peak tibial forces, and peak IMPs of the leg correlate linearly with percent BW, as generated across a broad range of upright LBPP and supine LBNP exercise. When running at 2.24 m/s the leg IMPs significantly increased over the loading range of 60% to 140% BW with LBPP and LBNP ( P < 0.001); as expected, leg IMPs were significantly higher when running compared with standing ( P < 0.001). During upright LBPP, total axial force at the knee increased linearly as a function of BW at 0.67 m/s ( R2 = 0.90) and 1.34 m/s ( R2 = 0.98). During supine LBNP, total axial force at the knee increased linearly as a function of BW at 0.67 m/s ( R2 = 0.98) and 1.34 m/s ( R2 = 0.91). The present study is the first to measure IMPs and peak tibial forces in vivo during upright LBPP, upright LBNP, and supine LBNP exercise. These data will aid the development of rehabilitation exercise hardware and prescriptions for patients and astronauts.

Solution Deposition Can Generate Consistent Bioceramic Coats on Porous Surfaces that More Closely Mimic the Trabecular Pattern and Biochemistry at the Bone Interface. we Compared Bone Response to the Following Implants: Porous-Coated Ti6al4v Cylinders with 1 of 3 Treatments: Plasma Sprayed with Hydroxyapatite (HA), Coated with a Solution-Deposited Biomimetic Apatite Coating (BA), and Untreated (Control). Bilateral Femurs in 36 Rabbits Were Implanted with One of the above Implants. Bone Ingrowth for HA and BA Surfaces Was Significantly Higher than that for Control Surfaces. No Fragmentation or Debris Production Was Evident in the Apatite Coat of the BA Group. A Biomimetic Coat of Solution-Deposited Apatite May Be Resistant to Coating Delamination and Particle Generation.

Background Many patellofemoral complications such as anterior knee pain, subluxation, fracture, wear, and aseptic loosening after total knee arthroplasty are attributed to malrotation of the femoral component. Rotating-platform mobile bearings can reduce malrotation between the tibial and femoral components and may also improve patellofemoral maltracking. Methods A computer model (LifeMOD/KneeSIM) of a weight-bearing deep knee bend was validated using cadaver knees tested in an Oxford-type knee rig. Changes in knee kinematics and patellofemoral forces were measured after femoral component malrotation of ± 3°. The effect of a rotating–bearing on these kinematics and forces was determined. Findings In a fixed-bearing arthroplasty femoral component internal malrotation increased tibiofemoral internal rotation by 3.4°, and external malrotation increased tibiofemoral external rotation by 4°. Femoral component malrotation affected patellofemoral lateral shift by up to 2.5 mm, and patellofemoral lateral shear by up to 19 N. When the malrotated femoral component was tested against a rotating–bearing the change in tibiofemoral rotation and patellofemoral lateral shift was less than 1° and 1 mm respectively. The rotating–bearing reduced peak lateral shear by 7 N and peak medial shear by 17 N. Increasing the conformity of the rotating–bearing reduced changes in tibiofemoral rotation due to femoral malrotation and increased the net rotation of the bearing (by approximately 5°) during flexion. Interpretation Our results are consistent with one randomized clinical outcome study and emphasize the value of computational modeling for preclinical design evaluation. It is important to continue to improve existing methodologies for accurate femoral component alignment especially in rotation.

Tibial tray malalignment has been associated with increased subsidence and failure. We constructed a finite element model of knee arthroplasty to determine the biomechanical factors involved in increasing the risk of subsidence with malalignment. Four fresh-frozen human knees were implanted with a tibial tray and subjected to forces representative of walking for up to 100,000 cycles. Cyclic displacement was measured between the tray and proximal tibia. The vertical load was shifted medially to generate a load distribution ratio of 55:45 (medial/lateral) to represent neutral alignment or 75:25 to represent varus alignment. Subjected specific geometry and material properties were obtained from qCT scans of tibia to construct a finite element model. The tray was subjected to a single load cycle representing experimental conditions. Tray displacement computed by the model matched that measured experimentally. Forces representing varus tray alignment generated greater strains in the proximal tibia and a greater volume of bone was subjected to strains higher than the fatigue threshold. Local compressive strains directly correlated with experimental subsidence and failure. Our results indicate that failure after tray malalignment is likely due to fatigue damage to the proximal tibia rather than shear across the implant–bone interface or failure of the cement mantle. © 2010 Orthopaedic Research Society. Background: Thromboembolic disease is a common complication of total hip arthroplasty. The purpose of this study was to compare a new mobile compression device with low-molecular-weight heparin with regard to their safety and effectiveness for the prevention of venous thromboembolic disease. Methods: Patients who had a total hip arthroplasty were randomized to receive prophylaxis with a mobile compression device or low-molecular-weight heparin for ten days. Use of the compression device began intraoperatively, and the patients in this group could receive 81 mg of aspirin daily after the surgery. The first injection of the low-molecular-weight heparin began between twelve and twenty-four hours after the surgery. After ten to twelve days, all patients underwent bilateral lower-extremity duplex ultrasonography to screen for deep venous thrombi in the calf and thigh. Any clinical symptoms of pulmonary embolism were evaluated with spiral computed tomography lung scans. Bleeding events and utilization of (i.e., compliance with) prophylactic treatment in both groups were documented. Clinical evaluation to look for evidence of deep venous thrombi and pulmonary emboli was performed at twelve weeks postoperatively. Results: Four hundred and ten patients (414 hips) were randomized; 392 of these patients (395 of the hips) were evaluable with regard to the safety of the intervention and 386 patients (389 hips) were evaluable with regard to its efficacy. Demographics were similar clinically between the groups. The rate of major bleeding events was 0% in the compression group and 6% in the low-molecular-weight heparin group. The rates of distal and proximal deep venous thrombosis were 3% and 2%, respectively, in the compression group compared with 3% and 1% in the heparin group. The rates of pulmonary embolism were 1% in the compression group and 1% in the heparin group, and there were no fatal pulmonary emboli. Within the twelve-week follow-up period, two events (one deep venous thrombosis and one pulmonary embolus) occurred in one patient in the compression group following negative findings on duplex ultrasonography on the twelfth postoperative day. There was no difference between the groups with regard to the prevalence of venous thromboembolism. Conclusions: When compared with low-molecular-weight heparin, use of the mobile compression device for prophylaxis against venous thromboembolic events following total hip arthroplasty resulted in a significant decrease in major bleeding events. Evidence-based medicine integrates research findings, clinical expertise, and patient values to provide the highest quality patient care. An orthopaedic outcomes database can assist surgeons in assessing associations between surgical procedures, function, survivorship, complication rates, patient satisfaction, and quality of life as guidance for practice. Outcomes are used to validate techniques or procedures, to benchmark best practices, and to identify areas requiring more clinical research. Tracking the success and failure rates of implants and surgical procedures is important. An outcomes database allows for short-term, midterm, and long-term prospective follow-up studies of specific cohorts and also facilitates studies that are retrospective in design. Several countries have initiated orthopaedic outcomes registries or databases. The oldest registry is the Swedish Knee Arthroplasty Register that was started in 19751. Other countries have followed with the development of their own joint registries in England, New Zealand, Germany, Finland, Denmark, and Norway2-7. In 1994, the American Academy of Orthopaedic Surgeons (AAOS) attempted to create an outcomes evaluation system. The Musculoskeletal Outcomes Data Evaluation and Management System did not come to fruition and was terminated in 2000. The attempt was made to collect information on patients undergoing total hip and knee arthroplasty or spine surgery. Difficulties were multifactorial, but were mainly due to inadequate data, record-keeping, or follow-up, which made cleaning the data to extract useful information impossible8. Although the United States lacks a national orthopaedic registry, several institutions have developed their own databases. For example, Mayo Clinic initiated a database in 1969 that included over 56,000 joint arthroplasties by 19979. The Anderson Orthopaedic Research Institute has over two decades' worth of hip and knee arthroplasty outcomes in its database10, while the Center for Hip and Knee Surgery in Indiana11, the Hospital for Special …

Osteochondral grafting for cartilage lesions is an attractive surgical procedure; however, the clinical results have not always been successful. Surgical recommendations differ with respect to donor site and graft placement technique. No clear biomechanical analysis of these surgical options has been reported. We hypothesized that differences in graft placement, graft biomechanical properties, and graft topography affect cartilage stresses and strains. A finite element model of articular cartilage and meniscus in a normal knee was constructed. The model was used to analyze the magnitude and the distribution of contact stresses, von Mises stresses, and compressive strains in the intact knee, after creation of an 8-mm diameter osteochondral defect, and after osteochondral grafting of the defect. The effects of graft placement, articular surface topography, and biomechanical properties were evaluated. The osteochondral defect generated minimal changes in peak contact stress (3.6 MPa) relative to the intact condition (3.4 MPa) but significantly increased peak von Mises stress (by 110%) and peak compressive strain (by 63%). A perfectly matched graft restored stresses and strains to near intact conditions. Leaving the graft proud by 0.5 mm generated the greatest increase in local stresses (peak contact stresses = 6.7 MPa). Reducing graft stiffness and curvature of articular surface had lesser effects on local stresses. Graft alignment, graft biomechanical properties, and graft topography all affected cartilage stresses and strains. Contact stresses, von Mises stresses, and compressive strains are biomechanical markers for potential tissue damage and cell death. Leaving the graft proud tends to jeopardize the graft by increasing the stresses and strains on the graft. From a biomechanical perspective, the ideal surgical procedure is a perfectly aligned graft with reasonably matched articular cartilage surface from a lower load-bearing region of the knee.

This review focuses on the prevention of postoperative instability and describes current operative options that have the potential to significantly reduce the risk of instability and the necessity of post-dislocation management. The latest research findings are presented to improve the readers' grasp of this complex issue. Increasing evidence supports the use of larger femoral heads as the promising preventive material currently available, meticulous posterior capsular repair when a posterior approach is used, hip resurfacing and higher offset. Future research is needed to more precisely determine the risk of instability associated with the use of postoperative patient restrictions and exercises, minimal incision surgery and computer-assisted navigation. Instability after total hip arthroplasty is troublesome for the patient and challenging for the surgeon. This review provides the surgeon with an opportunity to thoroughly understand the factors that may contribute to instability after total hip arthroplasty, the operative strategies that can be used to prevent and treat postoperative instability and the necessity of developing a plan for optimally preventing and managing dislocation.

This preliminary prospective study to determine the rate of deep venous thrombosis (DVT) examined 277 patients undergoing total knee or total hip arthroplasty (TKA or THA) who were randomized to use a portable, continuous enhanced circulation therapy (CECT) compression device and low-molecular-weight heparin (LMWH) or to receive LMWH alone. Patients were screened for DVT using duplex ultrasound at hospital discharge and followed clinically for 3 months. In TKA, 5 DVTs (6.6%) occurred in the CECT + LMWH group compared with one pulmonary embolism and 14 DVTs (19.5%) in the LMWH group (P = .018). In THA, 1 DVT (1.5%) occurred in the CECT + LMWH group and 2 DVTs (3.4%) occurred in the LMWH group. This preliminary study demonstrated significant reduction in rate of DVT after TKA when the CECT device was combined with LMWH.

Total knee arthroplasty (TKA) is successful for the treatment of severe osteoarthritis of the knee. However, TKA patients often experience moderate to severe postoperative pain. In the perioperative period the type of anesthesia, the surgical approach, and the use of local anesthetics and analgesics affect postoperative pain management. Optimum pain management requires action at all stages of the TKA process and begins preoperatively with patient education. Preemptive analgesia is used to prevent the pathologic pain caused by incisional and inflammatory injury. Anesthesia is used to inhibit intraoperative neural impulses from reaching the central nervous system. The midvastus surgical approach has shown positive results in decreasing postoperative pain and in increasing functional outcome. Postoperative pain is potentially best managed with a multimodal approach that blocks pain perception at a variety of sites in the peripheral and central nervous systems. Treatment options include combinations of systemic analgesics and/or regional analgesia, with or without opioids. Multimodal pain management approaches have proven effective by reducing pain, narcotic usage, length of hospital stay, and the side effects associated with anesthesia and narcotic analgesics. Any pain management approach used should be the product of collaboration between physicians, nurses, pharmacists, and patients.

Analysis of polyethylene component wear and implant loosening in total knee arthroplasty (TKA) requires precise knowledge of in vivo articular motion and loading conditions. This study presents a simultaneous, in vivo measurement of tibiofemoral articular contact forces and contact kinematics in three TKA patients. These measurements were accomplished via a dual fluoroscopic imaging system and instrumented tibial implants during dynamic single leg lunge and chair rising-sitting. The measured forces and contact locations were also used to determine mediolateral distribution of axial contact forces. Contact kinematics data showed a medial pivot during flexion of the knee, for all patients in the study. Average axial forces were higher for lunge compared to chair rising-sitting (224% body weight vs. 187% body weight). In this study we measured peak anteroposterior and mediolateral forces averaging 13.3% BW, during lunge and 18.5% BW during chair rising-sitting. Mediolateral distributions of axial contact force were both patient and activity specific. All patients showed equitable medial-lateral loading during lunge but greater loads at the lateral compartment during chair rising-sitting. The results of this study may enable more accurate reproduction of in vivo loads and articular motion patterns in wear simulators and finite element models. This in turn may help advance our understanding of factors limiting longevity of TKA implants, such as aseptic loosening and polyethylene component wear, and enable improved TKA designs.

First-generation crosslinked polyethylene developed for total hip arthroplasty has not gained wide acceptance for knee arthroplasty because of the increased potential for failure under high stresses and the increased risk for oxidative damage caused by free radicals. Sequentially crosslinked polyethylene (SQXL) is a second-generation crosslinked polyethylene that is reported to reduce the level of free radicals and preserve mechanical properties. Three groups of ultrahigh molecular weight polyethylene inserts were wear tested after artificial aging as per ASTM F2003: gamma-irradiated in air (GA-aged), sequentially crosslinked (SQXL-aged), and electron-beam irradiated and remelted (EBeam-aged). Inserts were tested in an AMTI knee wear simulator under malalignment conditions that were two standard deviations from the mean reported for computer-navigated and conventionally aligned techniques. GA-aged inserts delaminated by 500,000 cycles, and were severely damaged after 1 million cycles. None of the highly crosslinked inserts (SQXL-aged or EBeam-aged) delaminated or showed any signs of severe wear. Mean wear rate for GA-aged inserts was 124.6 ± 49.4 mg/million cycles. Mean wear rates for SQXL-aged and EBeam-aged inserts were significantly lower (1.74 ± 0.3 and 4.72 ± 0.7, respectively). These results support the low levels of free radicals and preservation of mechanical properties reported in second-generation crosslinked ultrahigh molecular weight polyethylene. © 2008 Orthopaedic Research Society. We implanted an electronic knee prosthesis to measure tibial forces in vivo during activities of daily living after total knee arthroplasty. We used tibial forces and knee kinematic data collected in vivo to calculate contact stresses using finite element analysis. The polyethylene insert was modeled as an elastoplastic material, and predicted contact stresses were validated using pressure sensitive sensors. Peak contact stresses generated during walking were similar but about 18% lower than those calculated for International Standards Organization (ISO)-recommended wear simulation conditions. Stair climbing generated higher contact stresses (32 MPa) than walking (26 MPa). However, both high flexion activities (lunge and kneel) generated even higher contact stresses, with the lunge activity generating the highest stresses (56 MPa). The activities that generated high contact stresses also resulted in high equivalent plastic strain. However, the lunge activity generated dramatically higher plastic equivalent strain than the other activities. In vivo measurement of kinematics, forces, and contact stresses may be used to develop more clinically relevant wear simulator protocols. Contact stresses generated during high flexion activities were substantially higher and were largely due to the reduced contact area in deep flexion rather than due to an increase in contact forces. Our results support the use of "high flexion" designs that improve contact conditions and preserve contact area at high flexion angles.

Evidence-based nursing practice provides the highest quality of patient care. In an orthopaedic setting, the study of outcomes across a patient population offers the ability to determine associations between the treatment regime and function, complication rates, patient satisfaction, and quality of life as evidence for practice. These associations give the orthopaedic nursing community valuable insight into patient results. An outcomes database can both assist nurses in assessing the results of their nursing interventions and in providing the basis for submitting papers to journals and scientific symposiums. These papers can provide information and education for establishment of an evidence-based nursing practice. This article describes the planning, development, and implementation of a comprehensive orthopaedic outcomes program including nursing intervention outcomes at one institution. We offer a step-by-step guide based on our experience for other institutions that plan to initiate an outcomes program with inclusion of nursing outcomes.

Wear debris from metal-on-polyethylene articulation in conventional total hip arthroplasty (THA) may limit THA longevity. Bearing surfaces made of modern ceramic material, with high wear resistance and low fracture risk, have the potential to extend the longevity of THA and make the procedure more suitable for young, active patients. Concerns regarding a ceramic-on-ceramic bearing surface have included potential for a higher incidence of dislocation caused by limited modular neck length and liner options. This prospective study assessed the early dislocation incidence for a ceramic-on-ceramic THA system. Out of the 336 consecutive ceramic-on-ceramic THA performed at our institution over an 8-year (1997–2005) period, 2 (0.6%) sustained dislocation during, and none after, the first postoperative year. Both dislocations were treated with closed reduction. No component fracture or revision for any reason has occurred in this series.

A projected increase in total hip arthroplasties, shortfalls in blood availability, and awareness of complications of transfusion make blood management in orthopaedic surgery important. In a multicenter, randomized, double-blind, placebo-controlled study, we hypothesized use of aprotinin would reduce blood transfusions (any and allogeneic) and blood loss in total hip arthroplasty. Using an intent-to-treat approach, we recruited 393 patients stratified by preoperative autologous blood donation or none and then randomized them to receive aprotinin (176 patients receiving a 10,000 kallikrein inhibitor units [KIU] test dose, 2 million KIU load, 0.5 million KIU per hour) or placebo (177 patients). We assessed patients at baseline; postoperative days 1, 2, 3, and 7 (or discharge); and 6 +/- 2 weeks. Primary efficacy was percentage of patients having blood transfusion through day 7 or discharge. We based safety on reported adverse events. Aprotinin reduced transfusions by 46% (30 of 176 versus 56 of 177 patients). Aprotinin reduced the total number of any blood units and the number of allogeneic blood units transfused relative to placebo (48 versus 109 units and 30 versus 72 units, respectively). Serious complications were similar in the two groups (placebo, 11%; aprotinin, 10%). Our data suggest full-dose aprotinin is safe and effective in decreasing blood transfusion in total hip arthroplasty.

Hip range of motion after total hip arthroplasty has been shown to be dependent on prosthetic design and component placement. We hypothesized that bony anatomy would significantly affect range of motion. Computer models of a current generation hip arthroplasty design were virtually implanted in a model of pelvis and femur in various orientations ranging from 35 degrees to 55 degrees cup abduction, 0 degrees to 30 degrees cup anteversion, and 0 degrees to 30 degrees femoral anteversion. Four head sizes ranging from 22.2 to 32 mm and two neck sizes ranging from 10-mm and 12-mm diameter were tested. Range of motion was recorded as maximum flexion-extension, abduction-adduction, and axial rotation of the femur before any contact between prosthetic components or bone was detected. Bony impingement preceded component impingement in about 44% of all conditions tested, ranging from 66% in adduction to 22% in extension. Range of motion increased as head size increased. However, increasing head size also increased the propensity for bony impingement, which tended to reduce the beneficial effect of increased head size on range of motion. Reducing neck diameter had a greater effect on prosthetic impingement (mean, 3.5 degrees increase in range of motion) compared to bone impingement (mean, 1.9 degrees ). This model allowed for a clinically relevant assessment of range of motion after total hip arthroplasty and may also be used with patient-specific geometry [such as that obtained from preoperative computed tomography (CT) scans] for more accurate preoperative planning.

Complications after total knee arthroplasty, such as malalignment, instability, and excessive wear, have been attributed to poor soft tissue balance. Traditional instruments that assist in intraoperative soft tissue balancing rely on static measurements. We used a custom tibial trial, instrumented with force transducers, for dynamic measurement of soft tissue balance. Six cadaver knees and two patients were implanted with the trial tray along with a standard femoral component and a tibial insert. We recorded tibial forces during passive knee flexion, after the initial bone cuts were made, after soft tissue balancing, and after replacing the selected optimal insert with one that was 2 mm thicker. In all knees, substantial imbalance in tibial forces initially was recorded. Soft tissue balancing substantially reduced the imbalance. Although reasonable balance was achieved at 0-degree and 90-degree flexion, there was some measurable imbalance at flexion angles other than 0 degrees and 90 degrees. Increasing the thickness of the insert by 2 mm substantially increased net tibial forces. Inconsistent soft tissue balance may explain some of the wide variation in knee kinematics. Surgical navigation systems have reduced the variability in component alignment. An instrumented tibial trial can be a valuable adjunct to directly measure soft tissue balance.

Background: The outcomes of knee arthroplasty have been shown to be affected by component alignment. Intramedullary and extramedullary alignment instrumentation are fairly effective for achieving the desired mean tibial component coronal alignment. However, there are outliers representing >3° of varus or valgus alignment with respect to the anatomic tibial shaft axis. We measured the efficacy of a custom tibial planing device for reducing the outliers in tibial alignment. Methods: We designed a tibial planing tool in an effort to improve tibial alignment. In one cohort (100 knees), we used traditional intramedullary alignment instrumentation to make the tibial bone cut. In a second cohort (120 knees), we used intramedullary alignment instrumentation to make the cut and also used a custom tool to check the cut and to correct an inexact cut. Tibial tray alignment relative to the long axis of the tibial shaft was measured in the coronal and sagittal planes on postoperative radiographs. Background: The Press-Fit Condylar total knee arthroplasty implant design has been used by many orthopaedic surgeons over the last twenty years. The design has survived with relatively minor modifications and remains a popular implant system today. The present study represents the fourteen to seventeen-year results for our patients who were described in our previous report on the eight to ten-year results. Methods: The senior author performed 160 consecutive total knee arthroplasties with use of the Press-Fit Condylar cruciate-retaining device in 134 patients between 1986 and 1989. The mean age of the patients at the time of the index procedure was 70.5 years. Follow-up data were available for sixty-three of sixty-four surviving knees. Surviving patients were followed for a mean of 15.8 years. The knees were evaluated on the basis of Knee Society function and clinical scores, radiographs, and survivorship analysis. Results: The mean Knee Society function score was 65, and the mean clinical score was 89. The overall survival rate of the knee was 91.5% with revision for any reason as the end point and 97.2% with aseptic loosening as the end point. The rate of revision of the tibial insert because of wear-related aseptic loosening was 2.5%. We found no relationship between revision and the shelf life or method of sterilization of the polyethylene insert. Radiolucent lines were present in 62% (twenty-one) of thirty-four knees; all radiolucent lines were nonprogressive. None of the implants were loose according to the criteria of the Knee Society. Conclusions: This long-term analysis indicates that the Press-Fit Condylar total knee implant is a successful implant system with excellent longevity. Level of Evidence: Therapeutic Level IV. Low-molecular-weight heparin has been studied extensively in total knee arthroplasty (TKA) and provides highly effective and safe prophylaxis against deep venous thrombosis (DVT). Low-molecular-weight heparin received the highest rating (A1) in the American College of Chest Physicians recommendations for DVT prophylaxis after elective TKA. Prevalence of DVT with low-molecular-weight heparin prophylaxis was 33% in TKA data pooled from six randomized studies, with a proximal DVT rate of 7.1%. A metaanalysis reported a 2.4% rate of major bleeding with low-molecular-weight heparin. Low-molecular-weight heparin, given by subcutaneous injection, can be started before surgery or after surgery. A synthetic pentasaccharide (fondaparinux), which received an A1 rating in the American College of Chest Physicians recommendations, also is available. As with all treatments, the benefit must be considered against the risk when using these anticoagulants.

Although total hip arthroplasty is a common and highly successful procedure, its long-term durability has been undermined by the cellular response to polyethylene wear debris and the subsequent effects on periprosthetic bone. Research elucidating the effects of sterilization on polyethylene wear has facilitated the development of a more wear-resistant material-highly cross-linked polyethylene. Laboratory testing has demonstrated that highly cross-linked polyethylene has markedly improved wear resistance compared with conventional polyethylene under a variety of conditions. Early clinical data have supported these results. To make informed decisions about this already widely available and frequently used product, the practicing orthopaedic surgeon should have a basic understanding of the production process as well as knowledge of the most current laboratory and clinical data.

Tibial forces were measured in vivo during the first year after total knee arthroplasty in a 66 kg, 80-year-old man. Forces were measured during activities of daily living, rehabilitation, and exercise. Peak tibial forces recorded during walking increased up to 12 months postoperatively (2.8 times body weight). Tibial forces correlated with increasing speed during treadmill walking. Rising from a chair generated peak forces of 2.6 times body weight. Stair descent generated higher peak forces than stair ascent (3.3 versus 2.9 times body weight, respectively). Exercising on a stair-climbing machine generated forces close to two times body weight whereas stationary bicycling generated even lower forces, near one times body weight. In general, the tibial forces recorded during walking and stair climbing were lower than most predicted values. These measurements can be used to validate in vitro and mathematical models of the knee. This should lead to refined surgical techniques and to enhanced prosthetic designs that will improve patient function, patient quality of life, and longevity of total knee arthroplasty implants.

Modular acetabular systems have several advantages; however, increased polyethylene wear has been recognized as a disadvantage. In our study, an acetabular shell design with a highly polished inner surface, a locking mechanism that minimizes micromotion, and a high polyethylene liner to shell conformity was evaluated. A prospective cohort of 50 consecutive hips was followed up for a mean of 3.6 years (range, 2–7 years). One patient required an early revision for recurrent instability. Mean linear head penetration rates were 0.242 mm/year (range, 0.048–0.655 mm/year). The true linear wear rate was calculated after subtracting the linear wear in the first follow-up year from the total wear at the end of the follow-up. The true linear wear rate was 0.173 mm/year (range, 0.03–0.423 mm/year). A positive correlation was found between wear rate and male gender. The low head penetration rates suggest that a polished liner with an improved locking mechanism and increased liner conformity can reduce polyethylene wear.

Total knee arthroplasty implant designs with larger extensor moment arms theoretically should generate lower extensor forces for the same externally applied loads. This study measured knee kinematics, quadriceps forces, and patellofemoral forces under conditions of dynamic knee extension under load in two knee designs with differing quadriceps moment arms. Six human cadaver knees were tested both before implantation and after sequential implantation with two posterior cruciate retaining designs. The extensor moment arm of the LMA (long extensor moment arm design, Scorpio, Howmedica Osteonics, Rutherford, NJ) was approximately 1 cm longer than that of the Control design (7000, Howmedica Osteonics). Quadriceps tension was measured during dynamic closed kinetic chain knee extension. Patellar compressive and shear forces were also recorded using a patellar component instrumented with a custom triaxial load transducer. Knee kinematics were monitored using a three-dimensional electromagnetic tracking device. Both designs produced similar patterns of femoral rollback and tibial rotation. Quadriceps tension was lower in the LMA design compared with the Control design. Patellofemoral compressive forces were also significantly reduced in the LMA design when compared with Control (8-18% lower at angles greater than 50 degrees flexion). The design with the longer extensor moment arm required less quadriceps force to extend the knee under load and reduced patellofemoral compressive forces. Reduced quadriceps forces may facilitate postoperative rehabilitation and activities such as stair climbing. Reduction in patellofemoral forces could reduce patellar complications such as anterior knee pain, component wear, and loosening.

Alumina ceramic is an excellent material for biologic implantation. Decreased particulate wear debris should increase implant longevity. The purpose of this study is to examine clinical and radiological results of ceramic-on-ceramic hip implants compared to cobalt chrome on polyethylene. Four cementless systems were compared, three alumina-on-alumina bearing systems: System I, porous coated cup; System II, hydroxyapatite-coated cup; Trident system, hydroxyapatite-coated cup with metal sleeve backing on ceramic cup liner; and System III (control), porous-coated cup with polyethylene and cobalt chromium bearing system. Patients were randomized to receive System I, II, or III. Trident patients were not randomized. Examinations are performed at 6 months, 1 year and yearly thereafter including x-rays, clinical exam and Harris Hip Score (HHS). Minimum 24-month followup was performed in 562 ceramic hips and 154 control hips. Age, height, weight, gender and diagnosis were similar in all groups. HHS was rated good/excellent by 95 percent of ceramic hips and 97 percent of control hips. Radiographic results demonstrated radiolucency in Femoral Gruen Zone 1 in 3.8 percent (18/474) of ceramic hips and in 8 percent (10/128) of control hips. Unstable acetabular components were reported in none of ceramic hips and in 3.2 percent (5/154) of control hips. Revision was performed in 7 (1.2 percent) ceramic hips, none due to failure of ceramic materials, and in 9 (5.8 percent) control hips. Alumina ceramic materials show promise, but continued evaluation of long-term clinical results is needed.

Implants with a highly porous coating of Tritanium Dimensionalized MetalÔ have the advantage of simulating the trabecular structure of bone to provide maximum available porous space for bone ingrowth. Plasma-sprayed hydroxyapatite coatings work well on non-porous substrates but do not coat the inner surfaces of open-porous substrates. Solution deposition can produce a consistent bioceramic coating of precise thickness on porous surfaces. This report compares bone response to a highly porous titanium surface with a solution deposited coating of hydroxyapatite. Ti6AL4V rods were implanted bilaterally in the intramedullary canals of 40 rabbit femurs. The implants had a 1.5 mm CPTi coating, which was >65% porous with pore sizes of 250-400 microns. (Tritanium Dimensionalized MetalÔ). Twenty implants (T-HA) were coated with hydroxyapatite by a solution deposition method (Peri-ApatiteÒ). The other 20 implants (T) had no hydroxyapatite coating. Implants were provided with a final diameter of 5 mm and length of 23 mm (Howmedica Osteonics, Mahwah, NJ). Rabbit femurs were harvested at 6 and 12 weeks after surgery sectioned at two levels: in the diaphyseal and metaphyseal portion of the femoral canal. Scanning electron images (SEM) in backscattered mode were digitally captured. Osseointegration was measured by automated computerized histomorphometry of the SEM images. Hydroxyapatite (HA) coatings for femoral stems have been successful in short- to intermediate-term follow up. However, the outcomes of HA-coated cups have not been as successful as comparable non-HA-coated designs. This has been attributed to component design and to poor quality HA-coating processes. This is a clinical outcome study of a newer generation acetabular design with an improved plasma-sprayed HA-coat. Ninety consecutive primary total hip replacements implanted with a HA-coated metal-backed cup were prospectively followed up for 2–5 years after surgery. Mean age was 69 years, male:female ratio was 36:47; mean-weight was 77 kg. Polyethylene wear was measured on digitized images. Component position, component migration, and implant-bone interface radiolucent lines were analyzed by computerized image analysis. Hip-scores improved from preoperative mean 59(±12) to a final follow-up mean of 91(±12). One hip was revised for recurrent dislocation. No hips were revised for aseptic loosening. Mean acetabular abduction angle was 46°(±8.5) and anteversion was 18°(±8.7). Incomplete lucent lines were noted on early postoperative radiographs in 5% of the cases. With ongoing follow up radiolucent line thickness decreased by mean 13% and radiolucent line density increased by 11%. No cup had complete continuous radiolucent lines or lysis. Typical plasma-sprayed hydroxyapatite coatings work well on non-porous substrates but do not coat the inner surfaces of open-porous substrates. Solution deposition can produce consistent bioceramic coats of precise thickness on porous surfaces. The resultant “biomimetic” surface more closely mimics the trabecular pattern and biochemistry at the bone interface. This report compares bone response to porous surfaces with biomimetic hydroxyapatite coatings. Implants were manufactured as Ti6Al4V cylinders (5-mm diameter, 41-mm long) coated with c.p-Ti PorocoatÒ porous layer with a thickness of 750 (± 250 µm). Implants were divided into three groups based on surface treatments. The porous surfaces of control group implants did not receive any treatment. The porous surfaces of HA group implants were plasma sprayed with hydroxyapatite. The porous surfaces of BAp group implants were coated with a biomimetic apatite (BAp) coating using a lowtemperature solution-based process that mimics bone mineralization. BAp coating is pure apatite coating of uniform structure and composition, with a thickness of approximately 15 µm on the outer beads. Because of the reduced thickness, the BAp coating does not block the pores or alter the porous structure. Bilateral femurs in thirty-six rabbits were implanted with one of the above groups. Twelve rabbits each were euthanized at 2, 4, and 12 weeks. Osseointegration was measured by automated computerized histomorphometry of scanning electron microscopy images of sections taken through the implant. Bone ingrowth on the Control surface was 45 % at 2 weeks and 47% at 12 weeks. Bone ingrowth on the PS surface increased from 51% at 2 weeks to 67% at 12 weeks. Bone ingrowth on the BAp surface increased from 45 % at 2 weeks to 71% at 12 weeks. At both time points mean bone ingrowth on PS and BAp coated implants was significantly higher than the control uncoated implants (p < 0.01). By 12 weeks the PS hydroxyapatite coat began showing evidence of fragmentation and debris production on SEM. This was not evident in the BAp coat. This study supports the hypothesis that apatite coating benefits osseointegration. A biomimetic coat of solution deposited apatite may not show the disadvantages of coating delamination and particle generation. Biomimetic apatite coatings may be attractive alternatives for noncemented total hip arthroplasty.

The need for prophylaxis against venous thromboembolic disease following lower-extremity joint replacement is well documented and accepted. Although low molecular weight heparin is approved for inpatient and extended prophylaxis beyond the hospital environment, orthopedic surgeons must determine the duration of injectable low molecular weight heparin based on the period of venous thromboembolic disease risk and on patient compliance with self-administration of medication at home. Lack of confidence in patients' ability to self-administer injections may contribute to hesitancy in prescribing injectable prophylaxis. This observational study evaluated the effectiveness of a self-injection education program. Findings demonstrate that when properly instructed, patients safely and efficaciously self-administer low molecular weight heparin at home. With shorter hospital stays, self-administration of low molecular weight heparin may optimize the clinical outcomes of orthopedic patients.
