Introduction: Ductal carcinoma in situ (DCIS) of the breast is a precursor of invasive carcinoma, accounting for 20-25% of all breast cancer diagnoses. Preoperative percutaneous biopsy does not always identify foci of invasion, as a consequence, the postoperative reports of invasive carcinoma implies another surgery for sentinel lymph node biopsy (SLNB).The objective of this study is to analyze clinical, pathological and radiological factors in patients with a preoperative diagnosis of DCIS that allows to predict the risk of postoperative invasive carcinoma and in whom it would be necessary to perform SLNB to avoid another surgery. Methods: A retrospective study of 304 patients with a preoperative diagnosis of DCIS by percutaneous biopsy was conducted at an institution between 2007-2021. The association between the variables and the diagnosis of invasive carcinoma in the postoperative period was analyzed with logistic regression models using univariate and multivariate analysis. Results: A total of 304 patients with a diagnosis of noninvasive DCIS were identified on percutaneous biopsy and 85 (28%) of them presented invasive carcinoma in the postoperative analysis. In the multivariate analysis, age (OR 1,06; 95% CI 1,01-1,11), lesion size (OR 1,03; 95% CI 1,01-1,05) and negative hormone receptors (OR 5,47; 95% CI 1,61-18,56) were found to be associated with a higher risk of invasion. Conclusion: In patients with DCIS, variables age, size, and hormone receptor status are prognostic factors for invasion in the postoperative biopsy. Objectives: This article aims to discuss the transformative impact of artificial intelligence (AI) on the identification and management of breast diseases, with a specific focus on breast cancer. Materials and methods: Healthcare providers have integrated AI technologies such as surgical robots and remote monitoring systems to improve the accuracy and efficiency of breast disease diagnosis and treatment. Results: Surgical robots equipped with AI algorithms provide real-time guidance to surgeons, analyze imaging data, and ensure precise procedures for better outcomes in breast cancer surgeries. AI-driven remote monitoring systems allow for close monitoring of patient health data, disease progression prediction, and personalized treatment recommendations. Conclusions: The incorporation of AI technology into healthcare practices for breast diseases has revolutionized patient care delivery, enhancing outcomes, promoting patient engagement, and improving overall quality of care. These advancements offer personalized and tailored healthcare strategies that meet the unique needs of each patient, reshaping the healthcare industry. Resumen: Objetivos: Este artículo tiene como objetivo discutir el impacto transformador de la inteligencia artificial en la identificación y manejo de enfermedades de mama, co con un enfoque específico en el cáncer de mama. Materiales y Métodos: Los proveedores de atención médica han integrado tecnologías de IA como robots quirúrgicos y sistemas de monitoreo remoto para mejorar la precisión y eficiencia del diagnóstico y tratamiento de enfermedades de mama. Resultados: Los robots quirúrgicos equipados con algoritmos de IA brindan orientación en tiempo real a los cirujanos, analizan datos de imágenes y aseguran procedimientos precisos para obtener mejores resultados en cirugías de cáncer de mama. Los sistemas de monitoreo remoto impulsados por IA permiten un seguimiento cercano de los datos de salud del paciente, predicciones sobre la progresión de la enfermedad y recomendaciones de tratamiento personalizado. Conclusiones: La incorporación de la tecnología de IA en las prácticas de atención médica para enfermedades de mama ha revolucionado la prestación de cuidados a los pacientes, mejorando los resultados, fomentando la participación del paciente y mejorando la calidad general de la atención. Estos avances ofrecen estrategias de atención médica personalizadas que satisfacen las necesidades únicas de cada paciente, reconfigurando la industria de la salud.

Apesar de iniciativas médicas e políticas públicas desenvolvidas no Brasil desde a década de 1920, o câncer de mama continua sendo um dos tipos de câncer mais prevalentes e de maior letalidade entre as mulheres. Com vistas a enfrentar esse desafio, a adoção de mamógrafos inovadores é considerada essencial. No entanto, barreiras são encontradas para implementação desses equipamentos em sistemas públicos de saúde. Nesse contexto, este estudo objetiva investigar os fatores que impactam a adoção de mamógrafos inovadores no Sistema Único de Saúde (SUS). Foram analisados dados públicos de 448 regiões de saúde brasileiras. Os resultados mostram influência positiva do uso de mamógrafos simples, da taxa de médicos especialistas, técnicos em radiologia e mulheres acima de 40 anos na adoção desses equipamentos. Observa-se também uma distribuição desigual de mamógrafos inovadores no País. Tais resultados contribuem para orientar ações no SUS, desenvolver políticas públicas mais efetivas e direcionar estudos futuros acerca do tema.

Resumo Por se tratar de um tumor sólido, a cirurgia do câncer de mama sempre será necessária. Em algumas ocasiões, diante do avanço local da doença, a mastectomia, ou seja, a retirada completa da mama, pode ser necessária. Para atenuar as alterações corporais causadas pela mastectomia, a reconstrução mamária é uma possibilidade de reparação local e uma alterativa de reaproximação do que socialmente se considera um corpo reestabelecido. Este artigo, em forma de ensaio, parte de uma breve pesquisa em bases de dados científicos, interligando reconstrução mamária e representações sociais. Encontramos uma possível lacuna de problematização do que seria um corpo normal e reparado e quais seriam as forças que interferem na decisão de uma mulher recorrer às cirurgias ditas reparadoras. Ao questionarmos as diferentes visões do que é ou não um corpo saudável, abordamos teóricos como Foucault, Butler e Le Breton, para uma compreensão ampliada dos conceitos de corporeidade.

Objetivo: Analisar a produção científica acerca do itinerário diagnóstico e terapêutico de mulheres com câncer de mama. Método: Revisão integrativa da literatura que priorizou o interesse pela busca de fenômenos subjetivos acerca do caminho percorrido por mulheres com câncer de mama. Para tanto, foram selecionados apenas artigos pautados em metodologias qualitativas a fim de responder a seguinte questão: “Quais as produções presentes na literatura acerca do itinerário diagnóstico e terapêutico de mulheres com câncer de mama? Para a seleção dos artigos, as buscas realizaram-se nos meses de junho e julho de 2022 nas seguintes bases de dados/bibliotecas através do Portal de Periódicos da Capes: National Library of Medicine, Cumulative Index to Nursing and Allied Health Literature, Web of Science, Scientific Eletronic Library Online e Literatura Latino-americana e do Caribe em Ciências da Saúde. Resultados: Foram selecionadas dez publicações que ermitiram discutir duas categorias: Identificação precoce, rastreamento e prevenção do câncer de mama e Experiências de mulheres em Busca do diagnóstico e tratamento para o câncer de mama. Conclusão: Foi possível evidenciar as diversas realidades de cada país a partir das vivências, impressões, opiniões e pontos de vista além de obter informações sobre as motivações, dificuldades e as atitudes das participantes. Fatores como estigma social, questões financeiras, falta de sistemas de apoio e diagnóstico incorreto ao longo da trajetória do paciente foram achados consistentes.

El cáncer de mama es la enfermedad neoplásica más común en las mujeres, esta plantea una serie de dificultades multidimensionales. Estos desafíos, desde su etiología multifactorial hasta sus repercusiones psicológicas tras el diagnóstico y tratamiento quirúrgico, requiere un abordaje integral. En el desarrollo de este tipo de cáncer, influyen múltiples factores entre genéticos, ambientales y el estilo de vida de cada paciente. El tratamiento va a depender de su estadio y el estado de salud de la paciente. Sin embargo, cuando se trata de procedimientos quirúrgicos, específicamente de mastectomía radical, estos pueden llegar a desencadenar ansiedad y depresión, manifestándose con sentimientos de tristeza y anhedonia, a su vez, este procedimiento puede llegar a afectar la percepción del cuerpo y la identidad de las pacientes. El abordaje integral de estas manifestaciones involucra desde la reconstrucción mamaria post mastectomía hasta la terapia cognitivo conductual, esta última es un factor importante para el desenlace positivo de estas pacientes. Se destaca la importancia de un enfoque colaborativo con un grupo multidisciplinario de profesionales para abordar tanto las dimensiones físicas como emocionales de la enfermedad y su debido tratamiento.

La terapia de reemplazo hormonal ha sido un tema tabú para muchas personas a lo largo de los años, incluso dentro del gremio médico, debido a que a inicios de los años 2000 se anunciaron estudios referentes al uso de la terapia de reemplazo hormonal (TRH), los cuales hablaban de resultados más perjudiciales que beneficiosos, recibió amplia publicidad, lo que creó pánico entre la multitud, influenciando de manera negativa a los médicos para su prescripción. Posteriormente, considerando todos los efectos de la menopausia, sintomatología, riesgo cardiovascular y afectación en calidad de vida, se realizaron nuevos estudios donde se demuestra que el uso de la TRH en mujeres más jóvenes y posmenopáusicas precoces, es muy beneficiosa a nivel de riesgo cardiovascular, reduciendo enfermedad coronaria y mortalidad, entre otros, mejorando no solo síntomas vasomotores, sino también el área de sexualidad y a nivel psicosocial. Pese a esto, hoy por hoy la opinión pública no ha cambiado y se sigue pensando que la TRH es igual a cáncer, entre otros efectos adversos erróneamente atribuidos a esta, lo cual genera un impacto negativo con consecuencias para la salud y calidad de vida para este grupo poblacional.

El objetivo de este estudio es evaluar la calidad de vida en pacientes con cáncer de mama. Elestudio tipo descriptivo, cuantitativo, diseño no experimental, enfoque transversal, participaron 39 pacientes con diagnóstico de cáncer de mama atendidas en 3 unidades especializadas oncológicas del Ecuador, muestra no probabilística a conveniencia, se aplicó un cuestionario validado de calidad de vida (WHOQOL-BREF) para evaluar estado de salud en áreas: física, entorno, relaciones sociales, salud psicológica. Los resultados indican que del 100% de mujeres, 41% oscilan entre 30 a 39 años, el 87,2% son mestizas, el 41% son casadas, el 38,5% tiene instrucción secundaria, el 38,5% tiene empleo, el 48,7% vive con su cónyuge e hijos, el 20,5% necesitan el tratamiento para funcionar en la vida diaria; asimismo, un 35,9% no está satisfecha con la calidad del sueño, 28,2% con la habilidad para realizar actividades de la vida diaria, el 25,6% con la capacidad de trabajo, el 12,8% con relaciones sociales y la vida sexual, el 23,1% con el apoyo de amigos, el 17,9% con la seguridad en su vida diaria, el 41% con el dinero para cubrir sus necesidades, el 15,4% considera que su vida tiene poco sentido y están poco satisfechos de sí mismas. En conclusión, el cáncer de mama produce impacto en calidad de vida, daños en la salud física, problemas de sueño, relaciones sociales, impacto económico y psicológico; la seguridad, autonomía física y económica, son factores que influyen en la calidad de vida, que se debe tomar en cuenta en el trabajo multidisciplinario del personal de salud.

Objetivos: presentar un caso de una lesión en mama de origen metastásico a partir de un adenocarcinoma gástrico de células en anillo de sello (AGCES), y hacer una revisión sistemática (RS) de la literatura sobre la epidemiología, el diagnóstico, tratamiento y pronóstico de las pacientes con este tipo de lesiones mamarias. Materiales y métodos: se reporta el caso de una paciente que acudió a una institución de referencia y alta complejidad en Bogotá por un nódulo en mama derecha. La paciente tenía el antecedente de un AGCES tratado quirúrgicamente, con quimioterapia y radioterapia adyuvante, tres años atrás. Se identificaron adenopatías axilares bilaterales. La biopsia del nódulo reportó un adenocarcinoma con patrón de células en anillo de sello. Posteriormente, por medio de la tomografía por emisión de positrones, se identificaron metástasis en otras localizaciones. La paciente falleció a los 10 meses. Se realizó una RS de la literatura, incluyendo estudios de cohorte, reportes y series de casos de pacientes con AGCES metastásico a la mama, que presentaran información sobre la epidemiología, las características clínicas, radiológicas e histopatológicas, el tipo de tratamiento recibido y pronóstico de esta enfermedad. Resultados: se identificaron 363 referencias, de las cuales 26 cumplieron los criterios de elegibilidad. El AGCES metastásico a la mama predominó en el continente asiático (75,9 %). La edad media de presentación fue de 42,5 años. En el 62 % de los casos esta condición se manifestó clínicamente como un nódulo mamario. Desde el punto de vista radiológico, la mayoría de las lesiones mamarias se presentaron como uno o varios nódulos de características sospechosas (55,2 %). En la biopsia de las lesiones mamarias, la positividad para citoqueratina 7 y citoqueratina 20 ocurrió en el 41,4 % y 37,9 % de los casos, respectivamente. Solo el 31 % de las pacientes fueron llevadas a algún tipo de cirugía de mama. Falta información sobre el tratamiento y pronóstico del AGCES metastásico a la mama. Conclusiones: toda paciente con antecedente de cáncer que presente un nódulo o cambios clínicos en la mama debe ser llevada a una biopsia mamaria para descartar secundarismo. Es importante realizar más estudios prospectivos sobre este tipo de presentación del AGCES para definir los efectos del tratamiento y el pronóstico de este subgrupo de pacientes.

Using Social Network Analysis, it addresses the playlist of videos about breast cancer stored and offered by YouTube when a user seeks to view content about the disease. The aim is to identify the most relevant videos and the concerns of those who publish and comment on the topic. The results highlight the significance of content shared by medical professionals and health-related organizations, as well as the importance of personal stories as sources of hope and support for those affected by the illness. Regarding video comments, in addition to medical aspects and personal testimonies, the significance of faith and prayer as aids in overcoming breast cancer is also emphasized. 25. Causal effect of sarcopenia-related traits on the occurrence and prognosis of breast cancer - A bidirectional and multivariable Mendelian randomization study.

Background and aims: although sarcopenia is associated with several types of cancer, there is limited research regarding its effect on breast cancer. We aimed to explore the causality between sarcopenia-related traits and the incidence and prognosis of breast cancer. Methods: two-sample bidirectional and multivariate Mendelian randomization (MR) analyses were utilized in this study. Genome-wide association studies were used to genetically identify sarcopenia-related traits, such as appendicular lean mass, grip strength of both hands, and walking pace. Data on the incidence and prognosis of breast cancer were collected from two extensive cohort studies. Multivariate MR analysis was used to adjust for body mass index, waist circumference, and whole-body fat mass. The primary method used for analysis was inverse-variance weighted analysis. Results: a significant association was found between appendicular lean mass and ER- breast cancer (OR = 0.873, 95 % CI: 0.817-0.933, p = 6.570 × 10-5). Increased grip strength of the left hand was associated with a reduced risk of ER- breast cancer (OR = 0.744, 95 % CI: 0.579- 0.958, p = 0.022). Stronger grip strength of the right hand was associated with prolonged survival time of ER+ breast cancer patients (OR = 0.463, 95 % CI: 0.242-0.882, p = 0.019). In the multivariable MR analysis, appendicular lean mass, grip strength of both hands, and walking pace were still genetically associated with the development of total breast cancer and ER-/+ breast cancer. Conclusions: several sarcopenia-related traits were genetically associated with the occurrence and prognosis of breast cancer. Objectives: To determine the general state of scientific evidence published in the last 20 years on gastric signet ring cell (SRC) adenocarcinoma metastatic to the breast, and present a case. Materials and methods: The case of a patient who attended a high-complexity reference institution in Bogotá for a breast metastasis from a gastric SRC adenocarcinoma is reported. A Systematic Review of the literature was carried out, including cohort studies, reports and case series of patients with gastric SRC adenocarcinoma metastatic to the breast, which will present information on the epidemiology, clinical, radiological and histopathological characteristics, and the treatment and prognostic of this disease. Results: There were identified 363 references, twenty-six of which met the eligibility criteria. Gastric SRC adenocarcinoma metastatic to the breast predominated in the Asian continent (75.9%). The mean age at presentation was 42.5 years. In 62% (n=18) of cases this condition manifested clinically as a breast mass. From a radiological point of view, the majority of breast lesions presented as one or several nodules with suspicious characteristics (55.2%; n=16). In the biopsy of breast lesions, positivity for cytokeratin 7 and cytokeratin 20 occurred in 41.4% (n=12) and 37.9% (n=11) of cases, respectively. Only nine patients (31%) underwent any type of breast surgery. There is a lack of information on the treatment and prognosis of gastric SRC adenocarcinoma metastatic to the breast. Conclusions: The frequency of cases of gastric SRC adenocarcinoma metastatic to the breast published in the last 20 years is low. The general practitioner and the gynecologist could be the first medical professionals to face this rare condition, so knowing it and suspecting it is essential, especially if we take into account that the median overall survival of these patients is low. Objective: Return to work and permanence in employment of women who survive breast cancer are topics that become important as incidence and survival rates increase. Self-efficacy as a modifiable element is of special interest in this process. The objective of this study is to measure the level of self-efficacy in female breast cancer survivors, according to sociodemographic, work and treatment characteristics and their relationship with return to work. Methods: This was a cross-sectional study, based on a survey of breast cancer patients about their demographic and work characteristics, the return to work process, permanence in the same job and the level of work self-efficacy. Differences in the level of self-efficacy with respect to characteristics were evaluated using bivariate analyses and hypothesis statistical testing. A value of p<0.05 was considered statistically significant. Results: One hundred and twenty-four women were included, 87.9% had returned to work, the majority (56.4%) between one and six months after treatment, 67.7% remained in the same job. Higher levels of work self-efficacy were related to a higher probability of returning to work and staying there, and a shorter time to return to work; these differences were statistically significant. Conclusion: Occupational health and risk prevention services must consider and strengthen work self-efficacy and organizational support in breast cancer survivors to achieve a successful return to work [ABSTRACT FROM AUTHOR]

Objective: To describe the epidemiological profile of patients undergoing mastectomy as a treatment for breast cancer in the northern region of Minas Gerais from 2017 to 2022. Method: Cross-sectional study on a secondary database, which included patients with breast cancer undergoing mastectomy, of both sexes, over 20 years of age, and excluded patients who underwent other surgical intervention modalities and deaths. Results: 309 medical records were analyzed; showed that the largest portion corresponds to female patients, totaling 98.05% female. With regard to age, a prevalence was observed among patients aged 50 to 69 years, which makes up 40.1%. Conclusion: Among the patients with breast cancer undergoing mastectomy and being treated at a public service in the city of Montes Claros-MG, there was a predominance of females, brown race, low level of education and as the main surgical approach, the radical mastectomy. Breast cancer treatment has undergone significant changes over the last few decades. The surgical techniques used today are increasingly improved to promote the best possible result, according to specific indications for each patient. Conservative surgeries are considered the standard treatment for early-stage breast cancer as they offer several benefits, such as a lower risk of complications and better aesthetic results. With the improvement in the primary breast cancer screening system, the number of patients eligible for conservative treatment is increasing, however, a series of factors deserve attention in therapeutic planning for each case. This study provides an overview of the basic foundations of surgical treatment for breast carcinoma in order to demonstrate the favorable results achieved with conservative surgery when well indicated. Breast cancer is a major cause of morbidity and mortality in women worldwide. Conventional therapies have undesirable side effects and in some cases are ineffective; therefore, the search for alternative therapies, such as photodynamic therapy (PDT), is essential. In this work, the effect of PDT on two breast cancer cell lines and one non-cancerous breast cell line was investigated using non-porphyrin photosensitizers (PS), such as methylene blue (MB) and rose bengal (RB). The efficacy of each treatment was evaluated by measuring cell viability with the MTT assay and by morphological observations. Before evaluating the effects of PDT, the intracellular uptake of each PS was measured in the cell lines studied. The results showed that the MDA-MB-231 cell line exhibited the highest sensitivity to PDT for both PS. The T47D cell line showed almost 50% inhibition in both cases. Surprisingly, the non-cancerous cell line MCF10A showed a selective response to PDT. In particular, no significant inhibition was observed with MB-PDT treatment. However, inhibition was observed with RB-PDT treatment. The morphological changes supported the cytopathic effect induced by PDT, it was found that non-cancerous MCF10A cells showed remarkable changes only with the highest dose of RB-PDT. In conclusion, this study presents, to the best of our knowledge, for the first time the effect of PDT using RB as a PS for the inhibition of MDA- MB-231, T47D and MCF10A breast cells, and using MB for PDT inhibition onT47D cells. Furthermore, no direct relationship was observed between the effect of PDT and the intracellular concentration of PS. Finally, these findings highlight the potential of MB and RB as effective PS in PDT and suggest the importance of considering cell line-specific responses and characteristics in optimizing PDT efficacy and selectivity. La SPECT portátil puede ser una técnica de imagen útil para la planificación preoperatoria de la biopsia selectiva del ganglio centinela (BSGC) ya que permite la localización del ganglio centinela (GC) mediante imágenes tomográficas en 3D y en tiempo real y determina su profundidad, después de unos minutos de exploración. El objetivo del estudio fue evaluar la correlación entre el número de GC detectados entre las imágenes de la SPECT portátil y la linfogammagrafía (LG). Cien pacientes con diagnóstico de cáncer de mama infiltrante y sin evidencia clínica de afectación ganglionar, se sometieron prospectivamente a una BSGC. El estudio preoperatorio incluyó imágenes de SPECT portátil a los 15 min tras la inyección y de LG a los 25 y 60-90 min (precoz y tardía). Se analizó el acuerdo observado y se realizó un estudio de concordancia entre el número de GC detectados con SPECT portátil y LG. El acuerdo observado en la detección de GC entre SPECT portátil y LG precoz fue del 72%; entre SPECT portátil y LG tardía del 85%, y entre la LG precoz y la tardía de un 87%. En el estudio de concordancia se registró una concordancia moderada entre la SPECT portátil y la LG precoz (coeficiente kappa: 0,42); una concordancia moderada-alta entre la SPECT portátil y la LG tardía (coeficiente kappa: 0,60), y una concordancia de moderada-alta entre la LG precoz y la tardía (coeficiente kappa: 0,70), sin diferencias significativas entre ellos (valor p = 0,16). La SPECT portátil presentó una concordancia moderada-alta con los estudios de imagen convencional y podría ser una alternativa válida para el estudio prequirúrgico de la BSGC en el cáncer de mama. Freehand SPECT can be a useful imaging technique for preoperative planning of sentinel lymph node biopsy (SLNB) as it allows localization of the sentinel node by 3D and real-time tomographic imaging and determines its depth after a few minutes of scanning. The aim of the study was to evaluate the correlation between the number of detected SNs between freehand SPECT images and lymphoscintigraphy (LS). One hundred patients with a diagnosis of invasive breast cancer and no clinical evidence of lymph node involvement prospectively underwent SLNB. The preoperative study included freehand SPECT imaging at 15 min after injection and LS imaging at 25 and 60–90 min after injection (early and late). The observed agreement was analyzed and a concordance study was performed between the number of SNs detected with freehand SPECT and LS. The observed agreement in the detection of SNs between freehand SPECT and early LS was 72%; between freehand SPECT and late LS was 85%; and between early and late LS was 87%. In the concordance study, there was moderate concordance between freehand SPECT and early LS (kappa coefficient: 0.42); moderate-high concordance between freehand SPECT and late LS (kappa coefficient: 0.60); and moderate-high concordance between early and late LS (kappa coefficient: 0.70), with no significant differences between them (p -value = 0.16). Freehand SPECT showed a moderate-high concordance with conventional imaging studies and could be a valid alternative for the presurgical study of SLNB in breast cancer. [ABSTRACT FROM AUTHOR]

Evaluar la tasa de detección y la implicación terapéutica de la infiltración de la cadena mamaria interna (ICMI) mediante tomografía por emisión de positrones (PET) y resonancia magnética (RM) con
18 F-fluorodesoxiglucosa (18 F-PET/RM) en la estadificación de pacientes con cáncer de mama. Estudio prospectivo, 41 mujeres con cáncer de mama (estadio ≥ IIB) estadificadas mediante18 F-FDG-PET/RM. Estudio en dos fases: imágenes mamarias (decúbito prono), cuerpo completo (supino). Estadificación TNM por consenso entre especialista en Medicina Nuclear y Radiología. Estudio vaso aferente (VA) a cadena mamaria interna (CMI) por RM mamaria. Correlación ICMI con edad, VA-CMI, estadificación T, cuadrante, infiltración axilar y a distancia. Revaloración terapéutica en comité multidisciplinar. Tasa de detección de ICMN de 34% (14/41), siendo 8/14 < 55 años. Todas las 14 pacientes con ICMI muestran VA-CMI, en seis de ellas (43,9%) sin VA-axilar. De 27/41 sin ICMI, en 13 (48,1%) solo VA-axilar, en los 14 restantes (51,9%) VA-axilar y VA-CMI. Un total de 57% (8/14) son multicéntricos y 42% (6/14) focales, en cuadrantes internos en 4/6 (66,7%). En 1/14 (7,1%) solo ICMI, en 9/14 (64,3%) axilar y CMI y en 4/14 (28,6%) lesiones a distancia. Decisión del comité: sin tratamiento adicional en 27/41 (65,8%), radioterapia torácica en 10/41 (24,4%) y terapia sistémica en 4/41 (9,7%). La tasa de detección de la ICMI en la estadificación del cáncer de mama mediante18 F-FDG PET/RM es de 34%. Son factores asociados la edad, los tumores multicéntricos, los de cuadrantes internos, la existencia de VA-CMI, la estadificación NM. La evidencia de ICMI permite la individualización de la terapia, indicando la radioterapia torácica en 24,4%. To evaluate the detection rate and therapeutic implication of the infiltration of the internal mammary chain (IMCI) by [18 F]FDG PET/MRI for staging of patients with breast cancer. Prospective study including 41 women with breast cancer (stage ≥IIB) staged by [18 F]FDG PET/MR. Two-phase exam: breast imaging (prone), whole-body (supine). TNM stage assessed by peer consensus with Nuclear Medicine and Radiology specialists. Study of the afferent vessel (AV) to IMC by breast MRI. IMCI was correlated with age, AV-IMC, T stage, breast quadrants, axillary and distant infiltration. Therapeutic re-evaluation by a multidisciplinary committee. IMCI detection rate of 34% (14/41), with 8/14 patients under 55 years of age. All 14 patients with IMCI showed AV-IMC, 6 of them (43.9%) without VA-axillary. Of 27/41 patients without IMCI, in 13 (48.1%) only AV-axillary was found, in the remaining 14 (51.9%), AV-axillary and AV-IMC was found. In 57% (8/14) tumours were multicentric and 42% (6/14) focal, in inner quadrants in 4/6 (66.7%). In 1/14 patient (7.1%) only IMCI was found, in 9/14 (64.3%) axillary and IMC, in 4/14 patients (28.6%) distant lesions were detected. Committee re-evaluation: no further treatment in 27/41 patients (65.8%), thoracic radiotherapy in 10/41 patients (24.4%), systemic therapy in 4/41 patients (9.7%). Our detection rate of IMCI in breast cancer staging by [18 F]FDG PET/MR was 34%. Related factors were age, multicentric tumours, inner quadrants, detection of AV-IMC, NM staging.The evidence of IMCI allowed tailored therapy, with thoracic radiotherapy implementation in 24.4% of patients. 46. Síntesis de imagen médica postcontraste en estudios de DCE-MRI de mama usando aprendizaje profundo

El cáncer de mama es una de las principales causas de muerte en mujeres en el mundo, por lo que su detección de forma temprana se ha convertido en una prioridad para salvar vidas. Para el diagnóstico de este tipo de cáncer existen técnicas como la imagen de resonancia magnética dinámica con realce de contraste (DCE-MRI, por sus siglas en inglés), la cual usa un agente de contraste para realzar las anomalías en el tejido de la mama, lo que mejora la detección y caracterización de posibles tumores. Como limitación, los estudios de DCE-MRI suelen tener un costo alto, hay poca disponibilidad de equipos para realizarlos, y en algunos casos los medios de contraste pueden generar efectos adversos por reacciones alérgicas. Considerando lo anterior, este trabajo tuvo como objetivo el uso de modelos de aprendizaje profundo para la generación de imágenes sintéticas postcontraste en estudios de DCE-MRI. La metodología consistió en el desarrollo de una función de costo denominada pérdida en las regiones con realce de contraste que aprovecha el comportamiento de la captación del agente de contraste. Como resultado se entrenaron dos nuevas arquitecturas de aprendizaje profundo, las cuales hemos denominado G- RiedGAN y D-RiedGAN, para la generación de imágenes postcontraste en estudios de DCE-MRI, a partir de imágenes precontraste. Finalmente, se concluye que las métricas proporción máxima señal ruido, índice de similitud estructural y error absoluto medio muestran que las arquitecturas propuestas mejoran el proceso de síntesis de las imágenes postcontraste preservando mayor similitud entre las imágenes sintéticas y las imágenes reales, esto en comparación con los modelos base en el estado del arte.

La cirugía es uno de los tratamientos principales para el cáncer de mama en mujeres, existen dos tipos principales de tratamiento quirúrgico: la cirugía conservadora y la radical. Ambas opciones tienen como objetivo extirpar el tumor y el tejido afectado circundante; Se diferencian en la cantidad de tejido mamario extirpado. La elección entre cirugia conservadora o radical en mujeres con cáncer de mama es una decisión importante que puede tener un impacto significativo en su salud y calidad de vida. La cirugía conservadora tiene un menor riesgo de complicaciones postoperatorias, pequeña pérdida de tejido mamario, un impacto positivo en la autoestima y la imagen corporal, una recuperación más rápida y menos dolor postoperatorio, la posibilidad de preservar la sensibilidad del pezón y la capacidad de amamantar en el futuro. Mientras que la cirugía radical tiene mayor riesgo de complicaciones, como infecciones y problemas de cicatrización.

48. Análise do efeito da alocação de mamógrafo sobre indicadores de saúde da mulher

Resumo: A detecção precoce de câncer de mama permite formas de tratamentos mais eficazes. Entretanto, o acesso generalizado à principal ferramenta de rastreamento, a mamografia, ainda é um desafio para o sistema público de saúde brasileiro. Este estudo tem o objetivo de analisar o efeito da alocação de mamógrafos sobre indicadores de saúde da mulher. Em 2013, dentre os 4.557 municípios que não tinham o equipamento, 260 receberam até 2019. A principal hipótese é que o efeito de receber o mamógrafo seja heterogêneo entre as localidades e que receber o equipamento dependa de variáveis observáveis (pareamento por escore de propensão) e não observáveis (modelo de efeitos fixos). Os resultados indicam que os municípios brasileiros que tiveram mamógrafo em uso a partir de 2014 obtiveram aumentos na realização de exames, porém sem efeitos de curto prazo para diagnósticos e óbitos por neoplasia maligna da mama. Além de equipamentos, uma estrutura mais complexa que envolve outros fatores como acesso a consultas, profissionais qualificados, tempo de espera etc. são importantes para melhorar os indicadores de saúde femininos no recorte de municípios analisados.

Este relato, com linguagens textual e imagética, sistematiza o processo de criação e produção de um material de educação em saúde e popularização científica: a graphic novel “180 graus: minhas reviravoltas com o câncer de mama”1. O livro aborda o câncer de mama por meio da linguagem dos quadrinhos para discutir este problema de saúde pública que, segundo estimativa do Instituto Nacional do Câncer (INCA), em 2023, afetaria mais de 73 mil mulheres no Brasil. O relato está organizado em cinco etapas: articulação de equipe e parcerias estratégicas; pesquisa e pré-produção do material para a história em quadrinhos (HQ); desenvolvimento da HQ; elaboração de materiais pedagógicos complementares à HQ; e edição e divulgação da HQ. Ao final, há também resultados, lições aprendidas e próximos passos.

Objetivo: relatar a experiência na elaboração e apresentação de recurso educacional para a saúde sexual e reprodutiva em grupo de planejamento familiar. Materiais e Métodos: trata-se de um relato de experiência de uma elaboração educativa, realizada pelos acadêmicos do 5° período do curso de enfermagem, na disciplina Seminário Integrador V, em uma Unidade de Saúde da Família, em maio de 2023, no Município de Montes Claros - MG. Elaborou-se em sala de aula um avental de plástico com mamas de tricô, para demonstrar, de forma ativa, a importância do autoexame e a prevenção do câncer de mama. Posteriormente, foi elaborado um convite para a comunidade, com data e horário estabelecidos e entregues pelos agentes comunitários de saúde. Durante o grupo de planejamento familiar, foram apresentados os métodos contraceptivos, além de informações sobre fatores de risco, manifestações clínicas, prevenção precoce e tratamento do câncer de mama. Foi demonstrada a técnica correta de como realizar o autoexame das mamas, utilizando a mama demonstrativa. Resultados: a educação em saúde foi positiva. Pela participação da comunidade, foi possível esclarecer medidas de prevenção, riscos à saúde, diagnóstico precoce e tratamento do câncer de mama e apresentação dos métodos contraceptivos fornecidos pelo Sistema Único de Saúde (SUS). Ao final da palestra, foram sanadas as dúvidas da comunidade, o que contribuiu para o engajamento do grupo com a equipe de saúde. Conclusão: os acadêmicos puderam informar e conscientizar a população sobre a importância da prevenção e diagnóstico precoce do câncer de mama, assim como fortalecer o vínculo e a confiança entre a comunidade e a equipe de saúde na melhoria da qualidade de vida coletiva.
