Abstract

Fasted lean and fed and fasted obese rats are demonstrated in vivo to have mitochondrial metabolism dysfunction. Lowered phosphocreatine (PCr)/inorganic phosphate (Pi) and enhanced ADP are evidence for obese rats at rest. After stimulation-alternated PCr, regulating and ADP metabolization rate are evident.

Abstract

Neuropeptide Y (NPY) mediates feeding behavior through a local hypothalamic network formed by the arcuate and paraventricular nuclei (the AP axis). In the hypothalamus, NPY is mainly synthesized in neurons of the arcuate nucleus. These neurons project to the paraventricular nucleus, the site where NPY has the strongest stimulatory effects on food intake of Sprague-Dawley rats. In the adult Zucker fatty rat (a genetic model of obesity with a well-established hyperphagia), NPY concentrations in these nuclei are higher than in its lean counterpart. We measured hypothalamic NPY before the appearance of altered eating behavior, e.g., in very young (16-d-old) lean and obese Zucker pups, and in pups at an age when overeating had begun, e.g., a few days after weaning at 30 d. At 30 d, NPY concentrations were significantly higher in obese than in lean rats in the arcuate nucleus (14.2 + or - 0.7 vs. 11.6 + or - 0.5 pmol/mg protein, P < 0.01.). This difference was not observed at 16 d. A 160% increase was noted in the paraventricular nuclei of obese rats between 16 and 30 d of life compared with a 100% increase in the lean rats (P < 0.001). Neuropeptide Y concentration was greater in 30-d-old rats than in 16-d-old rats in other areas involved in the regulation of feeding behavior, such as the dorsomedian nuclei and lateral hypothalamus, but the values did not differ between genotypes. Higher NPY concentration was therefore detected early in young obese rats in the main hypothalamic site of NPY synthesis. Because the change in NPY concentration in the paraventricular nucleus was greater in obese rats than in lean rats between 16 and 30 d, the AP axis seemed overactivated in young obese rats. These changes coincided with changes in eating behavior. Because a comparable phenomenon has already been described in another NPY network including the suprachiasmatic nuclei, the internal clock regulating feeding rhythms, we conclude that NPY plays a major role in the development of overeating in the Zucker fatty rat.

Abstract

The purpose of this study was to compare the early postoperative effects of heart and heart-lung transplantation on the secretion of atrial natriuretic peptide (alpha-ANP), renin, aldosterone, and vasopressin. This was carried out from the first to the eight postoperative day in ten heart and five heart-lung recipients. The changes in the release of these hormones were similar in both groups. Vasopressin release remained stable while that of the renin-angiotensin-aldosterone system progressively returned to more normal levels. Grafted heart tissue was capable of high alpha-ANP release early on in both heart and in heart-lung recipients. This sustained alpha-ANP release was not a function of the resulting overall atrial tissue mass. Our findings suggest that it might be the consequence of an intrinsic hypersecretion of alpha-ANP resulting from the loss of normal heart innervation occurring in both heart and heart-lung transplantation. Abstract

Les carences nutritionnelles témoignent d'un déséquilibre entre les apportsalimentaires et les besoins en macronutriments (protéines, lipides, glucides) etmicronutriments (vitamines et oligo-éléments). Elles peuvent concerner tout sujet(enfant, adulte, personne âgée) présentant une pathologie ou un trouble ducomportement alimentaire. Indépendamment d'une baisse globale d'apport, on peutobserver des carences en micronutriments à différents âges de la vie (nouveau-nés,enfants en période de croissance, femmes non ménopausées, personnes âgées).Il existe des signes d'alerte de la dénutrition relativement simples à mettre enévidence: une diminution du poids corporel de 5 % à 10 % sur une période de 6mois (en l'absence de toute restriction alimentaire de la part du sujet), un index demasse corporelle (rapport entre le poids et la taille au carré) inférieur à 18,5 kg/m2(à l'exception des maigreurs constitutionnelles) ou un taux d'albumine Criqueinférieur à 35 g/1. Chez l'enfant, un rapport poids/taille inférieur à 85 % ou unindex de masse corporelle inférieur au troisième percentile doivent alerter lesoignant. De même, une dénutrition doit être suspectée chez tout enfant quittantson couloir de croissance pondérale présenté sur la courbe de croissance du carnetde santé.Les personnes âgées sont les plus exposées à la dénutrition, essentiellement pourdes raisons socio-économiques. Il est important de prendre en compte la dimensionmultifactorielle d'une dénutrition (socio-économique, culturelle, psychologique etrelationnelle. ). Dans le cadre d'une dénutrition liée à une pathologie, si le premiergeste à effectuer est de traiter la pathologie elle même, il est également nécessaired'envisager de façon précoce une prise en charge nutritionnelle adaptée.Choisis selon le contexte clinique, différents marqueurs permettentde reconnaître la dénutrition,d'apprécier sa gravité et d'en suivre l'évolutionLes marqueurs de la dénutrition protéino-énergétique ont pour but d'aider à lareconnaissance d'un état clinique ou infraclinique de dénutrition, à l'appréciation desa gravité, à la compréhension de sa physiopathologie et à l'évaluation del'efficacité de son traitement.Compte tenu de l'extrême diversité et complexité des facteurs étiologiques encause, aucun marqueur ne dispose d'une sensibilité et d'une spécificité telles275qu'il pourrait être considéré isolément. De plus, l'intérêt clinique réel de la mesure d'unmarqueur de la dénutrition passe par la comparaison à des valeurs de référence et par la définition de seuils pathologiques. De telles valeurs et de tels seuils restent à établir pour denombreux marqueurs aujourd'hui disponibles.L'association de marqueurs anthropométriques (poids, index de corpulence, circonférencebrachiale. ), biologiques (créatininurie des 24 heures, albumine plasmatique. ) et/ouimmunologiques (comptage Lymphocytaire, sensibilité cutanée retardée. ) est proposéedans un grand nombre de situations. Le choix des associations dépend de l'objectif(dépistage, évaluation du risque de morbidité/mortalité, décision d'une thérapeutiquenutritionnelle adaptée), du contexte clinique et technique dans lequel se déroule l'évaluationde l'état nutritionnel (domicile, cabinet médical, hôpital. ), du degré d'urgence thérapeutiqueet de l'expérience de l'évaluateur qu'il soit praticien ou diététicien. Des grilles d'évaluationmultifactorielle ont été élaborées et parfois validées. L'étude de la composition corporellenécessite également l'association de marqueurs. C'est dans ce cadre que l'impédancemétriepourrait occuper demain une place de choix dans le diagnostic et le suivi thérapeutique desdénutritions. Des marqueurs fonctionnels restent à développer pour l'évaluation du statutnutritionnel, en association avec d'autres marqueurs...

38. Consequences of Coronary Occlusion on Changes in Regional Interstitial Myocardial Neuropeptide Y and Norepinephrine Concentrations

Abstract

An attempt to determine the consequences of prolonged ischemia on simultaneous regional changes in norepinephrine (NE) and neuropeptide Y (NPY) interstitial myocardial concentrations in a pig modelin vivowas made. The aim of the authors was to investigate further the mechanism of the major NE release previously observed in perfused hearts preserved using a Langendorff technique. Regional myocardial ischemia was induced by ligation of the left anterior descending coronary artery (LAD) in ten anesthetized pigs. NE and NPY release was studied using interstitial microdialyis, a technique initially used to monitor neurotransmitter kinetics in brain dialysate samples. Four dialysis probes were implanted into the left ventricular wall of the beating heart. Two were implanted into the ischemic region (LAD) (for NE and NPY determinations, respectively) and the remaining two into the non-ischemic left circumflex coronary artery region (LCX). Dialysate NE and NPY concentrations, as indices of interstitial myocardial NE and NPY concentrations, were measured by HPLC and RIA, respectively. A slight but significant increase in NPY levels was observed in both territories (LAD: from 190±27 to 349±62 pmol/l, LCX: 146±30 to 257±52 pmol/l) suggesting moderate stimulation of cardiac sympathetic nerve activity following LAD occlusion. On the contrary, a marked but progressive increase in NE release was observed in the ischemic region (from 8.8±1.0 to 251.4±44.8 nmol/l), when NE levels in the non-ischemic region remained stable (from 10.3±2.1 to 11.0±1.9 nmol/l). These results demonstrate the utility of regionalin-vivomyocardial NE and NPY monitoring using microdialysis. The strong and sustained NE accumulation occurring in the ischemic region is consistent with the hypothesis of a local non-exocytotic metabolic NE release in case of prolonged myocardial ischemia, when exocytotic release remain only minimal as attested by the slight increase in NPY observed.
