6. Computer-Enhanced or Computer-Enchanted? The Magic and Mischief of Learning with Computers.

Abstract

This paper reports on recent research in human-computer interactions, gives examples of some of the most promising uses of educational technology, and categorizes the effective and ineffective applications. The purpose of this paper is to provoke discussion about the problems and the potential benefits of computer use among young people. With a focus on the elements of audience, interactivity, and creative integration, the paper discusses the educational possibilities offered uniquely by the computer, arguing that the computer's greatest potential--its ability to draw out students' creativity--is being neglected. The discussion begins by acknowledging the down-side to computer-enhanced learning. The purpose is not to argue for the abandonment of educational technology, but to encourage a more tempered view. The next section proposes types of computer activities that may be particularly valuable for college-level students. Examples of promising educational technology applications are given, and the development of some original material relating to this topic is described. The paper concludes with some personal reflections from a parent's perspective. (AEF)

Interactive multimedia electronic journals, or IMEJ journals, are a publication medium particularly suited for research in computer-enhanced learning. This paper describes the challenges and potential rewards in publishing such a journal; presents ideas for design and layout; and discusses issues of collaboration, copyrighting, and archiving that are unique to IMEJ publications. Comments, based on the authors' experiences in founding a new IMEJ at Wake Forest University, are directed to those interested in developing their own interactive multimedia electronic journal, publishing their research in such a journal, or considering the evolution of scholarly publication in the light of new technology. Abstract

UnlabelledCholesteryl ester storage disease (CESD), an inherited deficiency of lysosomal acid lipase (LAL), is an underappreciated cause of progressive liver disease with no approved therapy. Presenting features include dyslipidemia, elevated transaminases, and hepatomegaly. To assess the clinical effects and safety of the recombinant human LAL, sebelipase alfa, nine patients received four once-weekly infusions (0.35, 1, or 3 mg·kg(-1) ) in LAL-CL01, which is the first human study of this investigational agent. Patients completing LAL-CL01 were eligible to enroll in the extension study (LAL-CL04) in which they again received four once-weekly infusions of sebelipase alfa (0.35, 1, or 3 mg·kg(-1) ) before transitioning to long-term every-other-week infusions (1 or 3 mg·kg(-1) ). Sebelipase alfa was well tolerated, with mostly mild adverse events unrelated to sebelipase alfa. No antidrug antibodies were detected. Transaminases decreased in patients in LAL-CL01 and increased between studies. In seven patients receiving ongoing sebelipase alfa treatment in LAL-CL04, the mean ± standard deviation (SD) decreases for alanine transaminase and aspartate aminotransferase at week 12 compared to the baseline values in LAL-CL01 were 46 ± 21 U/L (-52%) and 21 ± 14 U/L (-36%), respectively (P ≤ 0.05). Through week 12 of LAL-CL04, these seven patients also showed mean decreases from baseline in total cholesterol of 44 ± 41 mg/dL (-22%; P = 0.047), low density lipoprotein-cholesterol of 29 ± 31 mg/dL (-27%; P = 0.078), and triglycerides of 50 ± 38 mg/dL (-28%, P = 0.016) and increases in high density lipoprotein-cholesterol of 5 mg/dL (15%; P = 0.016).ConclusionThese data establish that sebelipase alfa, an investigational enzyme replacement, in patients with CESD is well tolerated, rapidly decreases serum transaminases, and that these improvements are sustained with long-term dosing and are accompanied by improvements in serum lipid profile.

Abstract

The transverse aortic constriction (TAC) model is frequently used to study adverse cardiac remodeling upon pressure overload. We set out to define the most important characteristics that define the degree of cardiac remodeling in this model. A systematic review and meta-analyses were performed on studies using the TAC mouse/rat model and reporting echocardiographic outcome parameters. We included all animal studies in which a constriction around the transverse aorta and at least one of the predefined echocardiography or MRI outcome parameters were assessed. A total of 502 articles and > 3000 wild-type, untreated animals undergoing TAC were included in this study and referenced to a control group. The duration of aortic constriction correlated to the degree of adverse remodeling. However, the mouse data is strongly biased by the preferential use of male C57Bl/6 mice (66% of studies). Furthermore, mostly ketamine/xylazine anesthetics, 27G needle constriction, and silk sutures are used. Nonetheless, despite the homogeneity in experimental design, the model contained a substantial degree of heterogeneity in the functional outcome measures. When looking at study quality, only 12% reported randomization, 23% mentioned any sort of blinding, 25% adequately addressed the outcomes, and an amazingly low percentage (2%) showed sample size calculation. Meta-analyses did not detect specific study characteristics that explained the heterogeneity in the reported outcome measures, however this might be related to the strong bias towards the use of specific mouse lines, sex as well as age or to poor reporting of characteristics of study quality.

Abstract

Cytomegalovirus (CMV) infection is linked to acceleration of solid organ transplant vascular sclerosis (TVS) and chronic rejection (CR). Donor latent CMV infection increases cardiac‐resident macrophages and T cells leading to increased inflammation, promoting allograft rejection. To investigate the role of cardiac‐resident passenger macrophages in CMV‐mediated TVS/CR, macrophages were depleted from latently ratCMV (RCMV)‐infected donor allografts prior to transplantation. Latently RCMV‐infected donor F344 rats were treated with clodronate, PBS, or control liposomes 3 days prior to cardiac transplant into RCMV‐naïve Lewis recipients. Clodronate treatment significantly increased graft survival from post‐operative day (POD)61 to POD84 and decreased TVS at rejection. To determine the kinetics of the effect of clodronate treatment's effect, a time study revealed that clodronate treatment significantly decreased macrophage infiltration into allograft tissues as early as POD14; altered allograft cytokine/chemokine protein levels, fibrosis development, and inflammatory gene expression profiles. These findings support our hypothesis that increased graft survival as a result of allograft passenger macrophage depletion was in part a result of altered immune response kinetics. Depletion of donor macrophages prior to transplant is a strategy to modulate allograft rejection and reduce TVS in the setting of CMV + donors transplanted into CMV naïve recipients. 37. Diagnostic Workup and Therapeutic Intervention of Hiatal Hernias Discovered as Incidental Findings on Computed Tomography

Abstract

Background Computed tomography imaging routinely detects incidental findings; most research focuses on malignant findings. However, benign diseases such as hiatal hernia also require identification and follow-up. Natural language algorithms can help identify these non-malignant findings.Methods Imaging of adult trauma patients from 2010 to 2020 who underwent CT chest/abdomen/pelvis was evaluated using an open-source natural language processor to query for hiatal hernias. Patients who underwent subsequent imaging, endoscopy, fluoroscopy, or operation were retrospectively reviewed.Results 1087(10.6%) of 10 299 patients had incidental hiatal hernias: 812 small (74.7%) and 275 moderate/large (25.3%). 224 (20.7%) had subsequent imaging or endoscopic evaluation. Compared to those with small hernias, patients with moderate/large hernias were older (66.3 ± 19.4 vs 79.6 ± 12.6 years, P< .001) and predominantly female (403[49.6%] vs 199[72.4%], P< .001). Moderate/large hernias were not more likely to grow (small vs moderate/large: 13[7.6%] vs 8[15.1%], P= .102). Patients with moderate/large hernias were more likely to have an intervention or referral (small vs moderate/large: 6[3.5%] vs 7[13.2%], P= .008). No patients underwent elective or emergent hernia repair. Three patients had surgical referral; however, only one was seen by a surgeon. One patient death was associated with a large hiatal hernia.Conclusions We demonstrate a novel utilization of natural language processing to identify patients with incidental hiatal hernia in a large population, and found a 10.6% incidence with only 1.2%. (13/1087) of these receiving a referral for follow-up. While most incidental hiatal hernias are small, moderate/large and symptomatic hernias have high risk of loss-to-follow-up and need referral pipelines to improve patient outcomes.

Abstract

The article presents a strategic plan for ubiquitous laptop computing. Computers are becoming standard equipment on university campuses, as basic to scholarship as libraries, books, and pencils, and as essential to communication as telephones. It has become clear that a relevant education in the 21st century must give students experience with the computer tools that will be an inevitable part of their work and lives. If educators are to communicate effectively with their students, they must also be able to speak the language of a computer nurtured generation. This article reports on the experience of one university that has realized a campus-wide computer initiative, and the two-prong strategy that is making this initiative a success as standardization of platforms, and decentralization of implementation. Wake Forest University, Winston Salem, North Carolina took its first steps toward campus-wide computer adoption when it began its triennial strategic planning process in 1994. Laptop computers were chosen as the standard hardware platform at Wake Forest.
