Abstract

Heterotrophic and mesophilic marine bacterium HYD-1545, isolated from the dorsal integument of hydrothermal vent polychaete Alvinella pompejana and categorized as an Alteromonas based on its G + C content and phenotypical features, excretes large amounts of acidic polysaccharide in batch cultures when the growth reaches the stationary phase. The polysaccharide consists of glucose, galactose, glucuronic acid, galacturonic acid and 4, 6-O-(1-carboxyethilidene)-galactose. Its viscosity depends on the ionic strength as it is a polyelectrolyte.

Abstract

The paper shows a comparison between three routing protocols for WSN. The protocols are conceived for particular scenario easily findable in archeological site and take into account energy consumptions and robustness of the transmission. This study will show which of the three protocols better meets the needs of this particular scenario.

Abstract

A multicentre, randomised, open trial with a 2 x 2 factorial design was conducted to compare the benefits and risks of two thrombolytic agents, streptokinase (SK, 1·5 MU infused intravenously over 30-60 min) and alteplase (tPA, 100 mg infused intravenously over 3 h) in patients with acute myocardial infarction admitted to coronary care units within 6 h from onset of symptoms. The patients were also randomised to receive heparin (12 500 U subcutaneously twice daily until discharge from hospital, starting 12 h after beginning the tPA or SK infusion) or usual therapy. All patients without specific contraindications were given atenolol (5-10 mg iv) and aspirin (300-325 mg a day). The end-point of the study was the combined estimate of death plus severe left ventricular damage. 12 490 patients were randomised to four treatment groups (SK alone, SK plus heparin, tPA alone, tPA plus heparin). No specific differences between the two thrombolytic agents were detected as regards the combined end-point (tPA 23·1%; SK 22·5%; relative risk 1·04, 95% Cl 0·95-1·13), nor after the addition of heparin to the aspirin treatment (hep 22·7%, no hep 22·9%; RR 0·99, 95% Cl 0·91-1·08). The outcome of patients allocated to the four treatment groups was similar with respect to baseline risk factors such as age, Killip class, hours from onset of symptoms, and site and type of infarct. The rates of major in-hospital cardiac complications (reinfarction, post-infarction angina) were also similar. The incidence of major bleeds was significantly higher in SK and heparin treated patients (respectively, tPA 0·5%, SK 1·0%, RR 0·57, 95% Cl 0·38-0·85; hep 1·0%, no hep 0·6%, RR 1·64, 95% Cl 1·09-2·45), whereas the overall incidence of stroke was similar in all groups. SK and tPA appear equally effective and safe for use in routine conditions of care, in all infarct patients who have no contraindications, with or without post-thrombolytic heparin treatment. The 8·8% hospital mortality of the study population (compared with approximately 13% in the control cohort of the GISSI-1 trial) indicates the beneficial impact of the proven acute treatments for AMI.

27. Modeling and remodeling of human extraction sockets.

Abstract

INTRODUCTION: The available studies on extraction wound repair in humans are affected by significant limitations and have failed to evaluate tissue alterations occurring in all compartments of the hard tissue defect. AIM: To monitor during a 6-month period the healing of human extraction sockets and include a semi-quantitative analysis of tissues and cell populations involved in various stages of the processes of modeling/remodeling. MATERIAL AND METHODS: Twenty-seven biopsies, representative of the early (2-4 weeks, n=10), intermediate (6-8 weeks, n=6), and late phase (12-24 weeks, n=11) of healing, were collected and analysed. RESULTS: Granulation tissue that was present in comparatively large amounts in the early healing phase of socket healing, was in the interval between the early and intermediate observation phase replaced with provisional matrix and woven bone. The density of vascular structures and macrophages slowly decreased from 2 to 4 weeks over time. The presence of osteoblasts peaked at 6-8 weeks and remained almost stable thereafter; a small number of osteoclasts were present in a few specimens at each observation interval. CONCLUSIONS: The present findings demonstrated that great variability exists in man with respect to hard tissue formation within extraction sockets. Thus, whereas a provisional connective tissue consistently forms within the first weeks of healing, the interval during which mineralized bone is laid down is much less predictable. 28. Depression of contact sensitivity by Pseudomonas aeruginosa-induced suppressor cells which affect the induction phase of immune response

Abstract

The cellular basis of depression of contact sensitivity to oxazolone in mice injected with Pseudomonas aeruginosa was studied. Cells from draining lymph nodes of mice sensitized with oxazolone 18 h previously were able to induce contact sensitivity to normal mice when administered in their footpads. In contrast, cells from draining lymph nodes of P. aeruginosa-injected and oxazolone-sensitized donors failed to induce contact sensitivity when injected in the footpad of normal mice and were capable of actively blocking the immunizing process brought about by lymph node cells from sensitized mice when injected together in the footpad of normal recipients. The P. aeruginosa-induced suppressor cells required antigenic stimulation, had precursors sensitive to cyclophosphamide, and did not affect the effector mechanisms of contact sensitivity. Thus, the results suggest that P. aeurginosa depresses contact sensitivity to oxazolone by enhancing the activity of suppressor cells which normally arise during the sensitization process and which affect the afferent limb of the immune response, probably by inhibiting the normal recruitment of T lymphocytes in the draining lymph nodes.

The depression of this response is dependent on the dose of bacteria injected, on the time interval between microorganism injection and antigen administration, and on the nature of the antigen used. Cell transfer experiments provide evidence that suppressor cells are not operative in this model. Furthermore, the results show that P. aeruginosa induces a marked dose-dependent proliferation of spleen cells in vivo, and the in vitro targets of this proliferative effect are B lymphocytes. It is suggested that whole, heat-killed P. aeruginosa in vivo also behave as cell mitogens on B lymphocytes which, when strongly stimulated to proliferate, temporarily lose their capacity to mount a normal antibody response.

Abstract

Background. Current knowledge of risk assessment in survivors of myocardial infarction is largely based on data gathered before the advent of thrombolysis. It must be determined whether and to what extent available information and proposed criteria of prognostication are applicable in the thrombolytic era. Methods and Results. We reassessed risk prediction in the 10 219 survivors of myocardial infarction with follow-up data available (ie, 98% of the total) who had been enrolled in the GISSI-2 trial, relying on a set of prespecified variables. The 3.5% 6-month all-cause mortality rate of these patients compared with the higher value of 4.6% found in the corresponding GISSI-1 cohort, originally allocated to streptokinase therapy, indicates a 24% reduction in postdischarge 6-month mortality. On multivariate analysis (Cox model), the following variables were predictors of 6-month all-cause mortality: ineligibility for exercise test for both cardiac (relative risk [RR], 3.30; 95% confidence interval [CI], 2.36-4.62) and noncardiac reasons (RR, 3.28; 95% CI, 2.23-4.72), early left ventricular failure (RR, 2.41; 95% Cl, 1.87-3.09), echocardiographic evidence of recovery phase left ventricular dysfunction (RR, 2.30; 95% CI, 1.78-2.98), advanced (more than 70 years) age (RR, 1.81; 95% Cl, 1.43 -2.30), electrical instability (ie, frequent and/or complex ventricular arrhythmias) (RR, 1.70; 95% Cl, 1.32-2.19), late left ventricular failure (RR, 1.54; 95% Cl, 1.17-2.03), previous myocardial infarction (RR, 1.47; 95% CI, 1.14-1.89), and a history of treated hypertension (RR, 1.32; 95% Cl, 1.05-1.65). Early post-myocardial infarction angina, a positive exercise test, female sex, history of angina, history of insulin-dependent diabetes, and anterior site of myocardial infarction were not risk predictors. On further multivariate analysis, performed on 8315 patients with the echocardiographic indicator of left ventricular dysfunction available, only previous myocardial infarction was not retained as an independent risk predictor. Conclusions. A decline in 6-month mortality of myocardial infarction survivors, seen within 6 hours of symptom onset, has been observed in recent years. Ineligibility for exercise test, early left ventricular failure, and recovery-phase left ventricular dysfunction are the most powerful (RR, >2) predictors of 6-month mortality among patients recovering from myocardial infarction after thrombolysis. Qualitative variables reflecting residual myocardial ischemia do not appear to be risk predictors. The lack of an independent adverse influence of early post-myocardial infarction angina on 6-month survival represents a major difference between this study and those of the prethrombolytic era.

The protocols are conceived for particular scenario easily findable in archeological site and take into account energy consumptions and robustness of the transmission. This study will show which of the three protocols better meets the needs of this particular scenario.

Abstract

Recently, we have presented an idea for using metamaterial layers to enhance the transmission through a sub-wavelength aperture in an opaque screen. Our work was inspired and motivated by the experiments performed in the past few years by several groups worldwide, in which they demonstrated a significant enhancement of optical transmission through a single sub-wavelength hole in a metallic screen when the aperture is surrounded by properly designed periodic corrugations. Oliner, Jackson and their co-workers have elegantly explained this effect in terms of the leaky-wave theory, revealing how this phenomenon may be enhanced by a judicious choice of corrugation periods. Two important features were shown to be essential in their theory: (1) the screen material must have a negative permittivity at the operating frequency, thus allowing presence of the surface plasmons on the screen; (2) the corrugation should have a certain periodicity to excite the leaky waves. Here we present a review of our recent theoretical results, which rely on a different setup: a homogeneous metamaterial slab is placed over a perfectly conducting flat screen with a small hole. In some recent works, we have shown theoretically how a proper choice of material parameters for the metamaterial cover may lead to an analogous enhancement of transmission through the hole. In this problem, the screen may be perfectly conducting, and unlike the cases studied by others, no corrugation or periodicity on this screen is needed here. The leaky wave at the surface of the metamaterial cover provides similar effects both in collecting power from an incident plane wave and directing it into the hole and in increasing the wave transmission in the broadside direction on the other side of the screen. In this chapter, an overview of this idea is given, and the interested reader is referred to the references with detailed information.

Abstract

A new bacterial polysaccharide '1644' is studied in aqueous solutions and in the gel state. The viscometric measurements for dilute and semi-dilute solutions in presence of monovalent salts are usual; however, in presence of divalent cations like Ca2+ an original behaviour is observed, related to intermolecular bonds. For larger polymer concentrations a clear gel is obtained with divalent counterions. This gel exhibits unusual properties compared to other gelling polysaccharides: it is very elastic and resistant because of low crosslinking. The mechanical properties of this gel depend closely on the fraction of divalent ions linked to the polymer and so on thermodynamic parameters.

Abstract

We have assessed the prognostic significance of ST segment depression in the anterior precordial leads in patients with an acute inferior infarction. Eighty-four patients with ST segment depression greater than or equal to 1 mm in at least 2 chest leads (Group A) and 82 patients without ST depression (Group B), all admitted to the hospital within 24 hours from the onset of an acute inferior myocardial infarction, were evaluated. Patients with an old infarction, those with intraventricular conduction abnormalities or other causes that could modify the ST segment were excluded from the study. The number of patients affected by complications during the hospital stay was significantly higher in group A (54 patients of group A vs 27 of group B, p less than 0,001). Death, left ventricular failure, ventricular arrhythmias were considerably higher in group A. Moreover we observed that the persistence of the ST segment depression for more than 24 hours identified a subgroup of patients with a very strong risk of complications, particularly death and left ventricular failure. The follow up after 3-6 months, however, did not show any significant difference in both groups. In conclusion, from our study it appears that patients with an inferior infarction precordial ST segment depression have a graver prognosis in the acute phase while their mid-term fate does not seem to be influenced by the presence of this electrocardiographic abnormality.

Abstract

LPS or DxS impair contact sensitivity to oxazolone in mice, evaluated as ear thickness and DNA synthesis of regional lymph node cells in vitro. Lymphoid cells from LPS- or DxS-injected and sensitized mice were able to transfer specifically the suppression when injected into animals sensitized immediately before cell transfer. This cell suppressor activity was eliminated by nylon wool filtration. These results suggest that B-cell mitogens may enhance suppressor cell activity of regulatory cells which arise during conventional immunization.

The clinical, haematological, cytomorphological, cytochemical, ultrastructural, and bone marrow biopsy histological data observed in 4 cases of tricholeukocytic leukaemia are reported, along with some unusual findings. Stress is placed on the varied course of this disease and the difficulties associated with its treatment. Some criteria for the choice of therapy are also proposed.
