Cancer is a major cause of morbidity and mortality in kidney transplant patients. Unfortunately, the use of new anti-cancer therapies such as immune checkpoint inhibitors (ICPIs) in this population has been associated with rejection rates up to 40%, in retrospective studies. The main challenge is to maintain the patient in a delicate immunologic balance in which, while antitumor therapy defeats cancer the graft is safely protected from rejection. Recent clinical trials with ICPI have included kidney transplant recipients (KTRs) and the results advocate for a paradigm shift in the management of basal immunosuppression. This suggests that downward adjustments should be avoided or, even better, that this adjustment should be “dynamic.” This review summarizes the latest scientific evidence available in renal transplantation under ICPI treatment: case series, prospective studies, histopathologic diagnosis, immunosuppression regimens and new biomarkers. This article will provide the latest information in on this specific field, allowing nephrologists to gain valuable knowledge and to be aware of new approaches to immunosuppression management in oncological kidney transplant patients.

This work was motivated by the difficulties found in the analysis and quantification of levulinic acid (LA) and the reaction products resulting from the catalyzed transformation of LA into γ-valerolactone (GVL) using High Performance Liquid Chromatography (HPLC). The starting study carried out with the Zorbax Eclipse XDB-C8 column and UV/Vis detector (or Diode Array Detector, DAD) was puzzling, mainly because two different signals appeared in the analysis of aqueous LA standards. The use of C8-UV/Vis-MS and the measurements with DAD in range wavelength mode (RWL) allowed to suggest the presence of pseudo-LA (sometimes regarded as unidentified impurities in the reactant) together with LA. The finding of an additional LA species was fundamental for the accurate quantification of LA, necessary to provide reliable results in the analysis of the catalytic LA transformation products. The study carried out also allowed the identification of 4-hydroxypentanoic acid (HPA), a labile intermediate compound. The analysis of the target solution (resulting from the explained catalytic system) was also performed with a REZEX ROA-Organic Acid H+ column and Refractive Index Detector (RID) in order to determine its applicability and to check if the results obtained with the two configurations were coherent. The REZEX-RID configuration led to a simpler analysis, as it did not involve the separation of the LA species, and allowed the detection of 1,4-pentanediol (not possible by UV/Vis analysis). The results obtained with REZEX-RID helped to support the need of considering the presence and quantification of an LA isomer in the standard solutions for the right quantification of LA using the C8-UV/vis (or DAD) configuration. The present study clarifies relevant practical aspects of the HPLC-UV/Vis (or DAD) analysis of LA present in aqueous solutions.

El debate sobre la educación en la España en el siglo XVIII estuvo estrechamente relacionado con los planteamientos pedagógicos europeos, a través de lecturas y traducciones de numerosas obras, en particular francesas, cuyo influjo no siempre ha sido analizado con detenimiento. En este artículo se estudia la obra y la recepción en España de tres célebres escritoras francesas sobre moral y educación, Mme D’Épinay, Mme Le Prince de Beaumont y Mme de Genlis. A través de múltiples indicios (traducciones, reseñas en la prensa, suscripciones, citas, opiniones de lectores, expedientes de censura), se ponen de relieve las complejas formas de apropiación y las lecturas diversas e incluso opuestas que estas autoras y sus escritos despertaron en la sociedad española.

Objectives: The aim of this study is to compare the postoperative complications, perioperative course, and survival among patients from the multicentric Spanish Video-assisted Thoracic Surgery Group database who received video-assisted thoracic surgery lobectomy or video-assisted thoracic surgery anatomic segmentectomy. Methods: From December 2016 to March 2018, a total of 2250 patients were collected from 33 centers. Overall analysis (video-assisted thoracic surgery lobectomy = 2070; video-assisted thoracic surgery anatomic segmentectomy = 180) and propensity score–matched adjusted analysis (video-assisted thoracic surgery lobectomy = 97; video-assisted thoracic surgery anatomic segmentectomy = 97) were performed to compare postoperative results. Kaplan–Meier and competing risks method were used to compare survival. Results: In the overall analysis, video-assisted thoracic surgery anatomic segmentectomy showed a lower incidence of respiratory complications (relative risk, 0.56; confidence interval, 0.37-0.83; P = .002), lower postoperative prolonged air leak (relative risk, 0.42; 95% confidence interval, 0.23-0.78; P = .003), and shorter median postoperative stay (4.8 vs 6.2 days; P = .004) than video-assisted thoracic surgery lobectomy. After propensity score–matched analysis, prolonged air leak remained significantly lower in video-assisted thoracic surgery anatomic segmentectomy (relative risk, 0.33; 95% confidence interval, 0.12-0.89; P = .02). Kaplan–Meier and competing risk curves showed no differences during the 3-year follow-up (median follow-up in months: 24.4; interquartile range, 20.8-28.3) in terms of overall survival (hazard ratio, 0.73; 95% confidence interval, 0.45-1.7; P = .2), tumor progression–related mortality (subdistribution hazard ratio, 0.41; 95% confidence interval, 0.11-1.57; P = .2), and disease-free survival (subdistribution hazard ratio, 0.73; 95% confidence interval, 0.35-1.51; P = .4) between groups. Conclusions: Video-assisted thoracic surgery segmentectomy showed results similar to lobectomy in terms of postoperative outcomes and midterm survival. In addition, a lower incidence of prolonged air leak was found in patients who underwent video-assisted thoracic surgery anatomic segmentectomy.

The new targeted cancer therapies including immune checkpoint inhibitors (ICIs) have been demonstrated to improve the survival of oncological patients, even in cases of metastatic cancer. In the past 5 years, several studies have revealed that ICI can produce several immune-mediated toxicities involving different organs, such as the skin, the gastrointestinal tract, the liver, and, of course, the kidney. The most frequent lesion of immunotoxicity in the kidney is acute interstitial nephritis (AIN), although other nephropathies have also been described as a consequence of the use of ICI, such as glomerulonephritis and acute thrombotic microangiopathy, among others. In addition, kidney rejection has also been reported in kidney transplant patients treated with ICI. Normally randomized clinical trials with ICI exclude patients with end-stage kidney disease, namely, patients undergoing dialysis and kidney transplant patients. Several important questions need to be addressed in relation to immunotherapy and patients with kidney disease: (a) when to start corticosteroid therapy in a patient with suspected acute kidney injury (AKI) related to ICI, (b) the moment of nephrologist referral and kidney biopsy indication, (c) management of ICI in patients undergoing dialysis, and (d) the effect of ICI in kidney transplantation, immunosuppressive personalized treatment, and risk of allograft rejection in kidney transplant patients. The objective of this review was to summarize the recently published literature on a wide spectrum of kidney disease patients with cancer and ICI. This review will address three main important groups of individuals with kidney disease and cancer immunotherapy, AKI associated with ICI, patients undergoing dialysis, and kidney transplant recipients. We believe that the information provided in this review will enlighten the personalized ICI treatment in individuals with a broader spectrum of kidney diseases.

Uniportal VATS has become an accepted approach in minimally invasive thoracic surgery since its first report for lobectomy in 2011. Since the initial restrictions in indications, it has been used in almost all procedures, from conventional lobectomies to sublobar resections, bronchial and vascular sleeve procedures and even tracheal and carinal resections. In addition to its use for treatment, it provides an excellent approach for suspicious solitary undiagnosed nodules after bronchoscopic or transthoracic image-guided biopsy. Uniportal VATS is also used as a surgical staging method in NSCLC due to its low invasiveness in terms of chest tube duration, hospital stay and postoperative pain. In this article, we review the evidence of uniportal VATS accuracy for NSCLC diagnosis and staging and provide technical details and recommendations for its safe performance for that purpose.

En este artículo se estudia la historia textual del poema “Testamento” de Valle-Inclán a partir de una relación actualizada de los testimonios de la tradición directa del texto, para luego editar el primer y desconocido testimonio impreso del poema. Se llega a la conclusión de que tanto los autógrafos como los impresos son en realidad copias de manuscritos de trabajo todavía en una fase redaccional, y se desvelan al menos dos estados genéticos cuya datación se intenta precisar. Asimismo, se presentan argumentos que parecen revelar que los testimonios impresos muestran diversas alteraciones debidas a la manipulación de sus editores.

This introduction to a collective essay collection on life stories, healing and religion revises connections between spiritual experience and perceptions of the body in the lives of early modern women, particularly nuns and women healers in Spain and America. By analysing sources such as court records, mystical writings, letters and legal statements, the journal essays that follow explore how individual identities are expressed through social networks, but also against externally imposed destinities.

El Museo de Xàbia nació a finales de los años 60 del siglo pasado, aunque fue reconocido oficialmente en 1977. Situado en el corazón de la villa de Xàbia (Marina Alta), sus instalaciones ocupan un gran caserón de tres plantas construido en el siglo XVII con una magnífica fachada de piedra «tosca», que fue ampliado en 2008 con la construcción de un nuevo edificio, adosado al viejo Museo. Sus trece salas de exposición conservan numerosos materiales arqueológicos, etnográficos y artísticos. Están representadas todas las épocas desde hace unos 30000 años, hasta la época contemporánea. Destaca la sala monográfica dedicada al yacimiento prehistórico de la Cova del Barranc del Migdia, una cueva de enterramientos múltiples del Calcolítico con pinturas rupestres esquemáticas, o la sala de arqueología submarina, uno de los conjuntos más importantes del País Valenciano. El Museo es además, el responsable a nivel municipal de los diversos aspectos relacionados con el rico patrimonio cultural de Xàbia.

The availability of “big data” has brought about a wave of innovation in projects when conducting health services research. Most of the available secondary data sources are restricted to the geographical scope of a given country and present heterogeneous structure and content. Under the umbrella of the European Innovation Partnership on Active and Healthy Ageing, collaborative work conducted by the partners of the group on “adherence to prescription and medical plans” identified the use of observational and large-population databases to monitor medication-taking behavior in the elderly. This article describes the methodology used to gather the information from available databases among the Adherence Action Group partners with the aim of improving data sharing on a European level. A total of six databases belonging to three different European countries (Spain, Republic of Ireland, and Italy) were included in the analysis. Preliminary results suggest that there are some similarities. However, these results should be applied in different contexts and European countries, supporting the idea that large European studies should be designed in order to get the most of already available databases. This article tells the story of how a crucial work of the French Enlightenment, Françoise de Graffigny’s Lettres d’une péruvienne, was rewritten in peninsular Spain, in a process full of cultural and personal connections –of friendship, patronage, intellectual and literary affinity or rivalry– across the Hispanic world. It combines textual analysis of the Spanish translation by María Rosario Romero (1792), by reconstructing the biographical profile of the translator and the political and intellectual context in which this version was conceived and inserted. In this way, a pattern of relations with certain central figures (M. Rosario Romero, the Countess of Gálvez) and others appearing in the shadows becomes visible. Throughout this story, translation emerges as a cultural practice associated with others –discussion, sociability– which share with it a certain collective dimension and takes its full meaning in precise biographical trajectories which contribute to a more complex analysis of texts themselves.

Virtual walkthroughs have become one of the most effective tools for the dissemination of archaeological heritage, particularly in those cases in which the site, by location or by their morphology, can not be visited. In this paper we describe the workflow for producing such tools, focusing on its two main phases: 3D digital documentation and the use of a game engine to create a virtual walkthrough. In this same line, as a case study, we describe the intervention in the archaeological site “Cova del Barranc del Migdia”, located on the southern face of Mount Montgó, within the Montgó Natural Park, in the town of Javea (Alicante, Spain).

In this paper we are presenting a multi-marker and semi-immersive system for augmented reality to visualize and interact with archaeological sites, specifically those located in inaccessible or complex environments, such as caves or underwater locations. The use of this system in museum exhibitions helps visitors to come closer to archaeological heritage. As an example for the implementation of this system, an archaeological site has been used. It is the “Cova del Barranc del Migdia”, located in the “Sierra del Montgó”, Xàbia (Spain). The product obtained has been exhibited in various museums nationwide.

The University of Alicante has organized the first edition of a specialist degree in Cultural Heritage Virtualization during the academic year 2011-2012. In this paper we intend to show the basis for this project and the program itself. Finally, and from the experience of the first edition, we explain the strategic plan we have defined for the development of future editions, as well as changes that have been introduced, especially in the learning method (offering the option to follow the course online) and the international expansion strategy.

This paper seeks a political and ideological reading of Ramón del Valle-Inclan’s novella Fin de un revolucionario. Aleluyas de la Gloriosa. It was published in the popular collection Los novelistas in 1928, whose second part was entitled "Vísperas de Alcolea", featuring the execution of Benjamín Fernández Vallín on September 25, 1868; this would not be collected later in any volume of El Ruedo Ibérico, receiving no special attention from literary criticism. Based on the selection and manipulation of historical sources by the writer did in order to build his story, both the peculiar structure of the novella in its relation to gender “aleluya” and the narrative techniques focused in the literary mode of the esperpento are analysed, paying special attention to the characterization of the main character, and the narrator`s involvement in the story, whose unheroic and demystifying interpretation of pre-revolutionary historical events seeps into the narrative. In this way, Fin de un revolucionario is revealed as an instrument of unquestionable efficacy to approach the political message of the series and the vision of the history of Spain as an example of the tragic mojiganga that Ramón del Valle-Inclán reflected in his historical novels of the twenties.

As part of the the site musealization "Villa Romana de l'Albir (L'Alfas del Pi, Alicante), a digital reconstruction of the thermal complex of the villa has been produced. The software chosen to create the virtual model is Blender, a powerful set of tools, able to develop all phases of design (modeling, animation, lighting, rendering and video editing). Here we show the results of the thermal complex reconstruction and describe the specific workflow of Blender for virtual reconstruction of archaeological sites.

During the recent campaign of excavation at the Cova del Barranc del Migdia (Javea, Alicante)we have carried out the 3D documentation of the wall painting motifs, which corresponds to the schematic rock art style. To produce the 3D models, we have used Photomodeler Scanner, a commercial digital photogrammetry software with increased use in the cultural heritage sector. The results have been exported to a format compatible with 3D printers, creating a 1/1 scale replica of the original panel. In this paper we describe the typical workflow in a project with Photomodeler Scanner and the results obtained from the x-3 panel.

El presente trabajo muestra la importancia de la Estadística Oficial en la docencia de la Estadística. La Estadística Oficial puede usarse como una herramienta de gran utilidad en el contexto docente, ya que ofrece un elemento motivador para el aprendizaje tanto a nivel universitario como en la enseñanza secundaria. A modo de ejemplo, se detalla una experiencia docente llevada a cabo durante el curso académico 2009/2010 en la que se hace un uso didáctico de la información de tres organismos oficiales: el Instituto Nacional de Estadística, el Instituto Valenciano de Estadística y la Oficina de Estadística del Ayuntamiento de Valencia.

En este artículo se sintetizan las líneas maestras del pensamiento de Anne-Thérèse Marguenat, marquesa de Lambert (1647-1733) y se profundiza en el eco que su obra tuvo en España, aclarando algunos episodios poco conocidos y arrojando luz sobre el contexto en el que se realizó y publicó finalmente la versión castellana de sus ensayos y sobre la personalidad intelectual y circunstancias sociales de su traductora, Cayetana de la Cerda y Vera, condesa de Lalaing (1755-1798). Se pretende así analizar las prácticas de transferencia y mediación cultural, interrogándonos sobre los significados con que su pensamiento y su figura pudieron ser investidos por parte de los sujetos implicados en su lectura, traducción y circulación.

Presentamos el yacimiento protohistórico de la Plana Justa, localizado en el curso de las prospecciones realizadas en los últimos años por el Museu Arqueològic i Etnogràfic Municipal “Soler Blasco” de Xàbia (Alicante). Los restos que se observan en superficie son algunos basamentos de muros y cerámicas –además de otros materiales– que podemos datar entre los ss. VII-V a.C. en base a sus características tecnológicas y a las importaciones presentes, por lo que el asentamiento ofrece buenas perspectivas para el estudio de los períodos más antiguos de la Edad del Hierro. De momentos posteriores son algunas cerámicas de cronología andalusí (ss. X-XI?), indicadoras de una ocupación de escasa entidad que parece situarse en la zona más alta del área que nos ocupa.
