This study explored the experiences of inclusion and exclusion of African academics in Norway in various sectors of the society and their participation in these sectors. Using a mixed method research approach, 166 African academics completed a 20-item questionnaire entitled Perceived Exclusion Scale (PES) and two open-ended questions about their mental effects and coping mechanisms of exclusion. Descriptive statistics and qualitative analysis procedures were used to analyze the data. The results showed that the participants experienced exclusion in almost all the sectors of the Norwegian society with concomitant effects of depression and insomnia for most of the participants. Participants mentioned acceptance, confrontational strategy, avoidance strategy, theological group discussion and positive attitudes as key coping mechanisms to exclusion and discrimination. This study could be used as a baseline for future research on the psychological and mental health effects of discrimination of Africans and African-Norwegians. The study is a pointer to the public discourses on the positive sides of immigration in general and the role of migrants' contribution to the Norwegian society.

Abstract Background The efficacy of lung cancer screening (LCS) to reduce lung cancer specific mortality is heavily dependent on adherence to recommended screening guidelines, with real-world adherence rates reported to be drastically lower than rates described in clinical trials. There is a dearth in the literature on reminder processes and clinical workflows used to address adherence and robust data is needed to fully understand which clinical set-ups, processes, and context enhance and increase continued LCS participation. This paper describes a protocol for an environmental scan of adherence and reminder processes that are currently used in LCS programs across the United States. Methods This study will triangulate data using a 3-step explanatory sequential mixed methods design to describe mechanisms of current adherence and reminder systems within academic and community LCS programs to pinpoint clinic or system barrier and facilitator combinations that contribute to increased adherence. In step 1, surveys from a nationally representative sample of LCS programs will yield quantitative data about program structure, volume, and tracking/reminder processes and messages. After completion of the survey, interested LCS program personnel will be invited to participate in an in-depth interview (step 2) to explore current processes and interventions used for adherence at the participant and program level. Finally, in step 3, triangulation of quantitative and qualitative data will be completed through qualitative comparative analysis to identify combinations of components that affect higher or lower adherence. Discussion This research advances the state of the science by filling a gap in knowledge about LCS program characteristics and processes associated with better adherence which can inform the development and implementation of interventions that are scalable and sustainable across a wide variety of clinical practice settings.

Abstract There has been a growing interest in implementing technological food systems to enhance the quality and safety of food items. One such example of this endeavor is the study of the applications of natural biopolymers as food stabilizers. An "emulgel," also known as an emulsion gel, is a gel-like substance that contains a dispersion of emulsion. This emulsion can have a continuous or dispersed phase, creating networks throughout the substance and giving it a semi-solid texture. Gels have garnered significant attention in the food industry due to their ability to be easily shaped and structured and their potential to possess desirable physical, chemical, and mechanical properties. They offer greater stability and controlled release capabilities for the encapsulated bioactive compounds. The review demonstrated that different emulgels exhibit varied relationships between their structure, physicochemical properties, unique applications, and outcomes from different preparation methods. Emulgels enhance the yield, sensory characteristics, and nutritional quality of foods while also controlling lipid oxidation and microbiological development. Emulgels find primary utility in the food business for fat replacement and distribution. Viscosity, temperature, pH, water activity, and ionic strength are influential factors that impact the destabilization mechanisms of emulgels. This article thoroughly summarizes the practical use of emulgels in muscle-based foods and explains their impact on the functioning and quality of such edibles. There is a recommendation to promote the utilisation of emulgels as effective colloids and delivery systems in the future. Additionally, it is proposed that a description of potential new applications in the food industry be provided.

Abstract The crosstalk between adipose tissue and the liver is finely controlled to maintain metabolic health. Yet, how adipose tissue controls toxic free fatty acid overflow into the liver remains incompletely understood. Here, we show that adipocyte activating transcription factor 3 (ATF3) was induced in human or mouse obesity. Adipocyte Atf3 −/− (Atf3 Adi−/− ) mice developed obesity, glucose intolerance, and metabolic dysfunction-associated steatohepatitis (MASH) in chow diet, high-fat diet, or Western diet-fed mice. Blocking fatty acid flux by inhibiting hepatocyte CD36, but not the restoration of hepatic AMPK signaling, prevented the aggravation of MASH in Atf3 Adi−/− mice. Further studies show that the loss of adipocyte ATF3 increased lipolysis via inducing adipose triglyceride lipase, which in turn induced lipogenesis and inflammation in hepatocytes. Moreover, Atf3 Adi−/− mice had reduced energy expenditure and increased adipose lipogenesis and inflammation. Our data demonstrate that adipocyte ATF3 is a gatekeeper in counteracting MASH development under physiological and pathological conditions.

Abstract The impact of preterm babies’ admission at the Neonatal Intensive Care (NICU) on the mental health of mothers is a global challenge. However, the prevalence and predictors of Common Mental Disorders (CMDs) among this population remain underexplored. This study assessed the predictors of CMDs among mothers of preterm infants in the NICUs in the Upper East Region of Ghana. A cross-sectional study was conducted, targeting mothers of preterm babies in two hospitals in the Upper East Region. The Self-Report Questionnaire (SRQ-20) was used to collect data from 375 mothers of preterm babies admitted to the NICUs. Statistical analyses were done using SPSS version 20. The study found a prevalence of 40.9% for CMDs among mothers of preterm babies admitted to the two NICUs. The predictors of CMDs were unemployment (aOR 2.925, 95% CI 1.465, 5.840), lower levels of education (aOR 5.582, 95% CI 1.316, 23.670), antenatal anxiety (aOR 3.606, 95% CI 1.870, 6.952), and assisted delivery (aOR 2.144, 95% CI 1.083, 4.246). Conversely, urban residence (aOR 0.390, 95% CI 0.200, 0.760), age range between 25 and 31 (aOR 0.238, 95% CI 0.060, 0.953), and having a supportive partner (aOR 0.095, 95% CI 0.015, 0.593) emerged as protective factors. This study emphasizes the imperative of addressing maternal mental health within the NICU setting for preterm births.

Abstract We present the Manually Annotated GONG Filaments in H-alpha Observations (MAGFiLO v1.0) dataset. This dataset contains 10,244 annotated filaments from 1,593 observations captured by the Global Oscillation Network Group (GONG), spanning the years 2011 through 2022. Each annotation details one filament’s segmentation, minimum bounding box, spine, and magnetic field chirality. With a total of over one thousand person-hours of annotation, and a double-blind review process, we ensured high-quality ground-truth data. Our inter-annotator agreement reaches a Kappa score of 0.66. We also verified that the hemispheric preference of filaments as annotated in MAGFiLO aligns with the findings from similar datasets of much smaller sample sizes. MAGFiLO is the first dataset of its size, enabling advanced deep learning models to identify filaments and their features with unprecedented precision. It also provides a testbed for solar physicists interested in large-scale analysis of filaments. In this report, we document the details of the annotation and the post-processing phases that were applied.

Climate variability significantly impacts plant growth, making it crucial to monitor ecosystem performance for optimal carbon sequestration, especially in the context of rising atmospheric CO2 levels. Net Primary Productivity (NPP), which measures the net carbon flux between the atmosphere and plants, serves as a key indicator. This study uses the CASA (Carnegie–Ames–Stanford Approach) model, a radiation use efficiency method, to assess the spatio-temporal dynamics of NPP in Togo from 1987 to 2022 and its climatic drivers. The average annual NPP over 36 years is 4565.31 Kg C ha−1, with notable extremes in 2017 (6312.26 Kg C ha−1) and 1996 (3394.29 Kg C ha−1). Productivity in natural formations increased between 2000 and 2022. While climate change and land use negatively affect Total Production (PT) from 2000 to 2022, they individually enhance NPP variation (58.28% and 188.63%, respectively). NPP shows a strong positive correlation with light use efficiency (r2 = 0.75) and a moderate one with actual evapotranspiration (r2 = 0.43). Precipitation and potential evapotranspiration have weaker correlations (r2 = 0.20; 0.10), and temperature shows almost none (r2 = 0.05). These findings contribute to understanding ecosystem performance, supporting Togo’s climate commitments.

INTRODUCTION: In the hypothalamic-pituitary-gonadotrophin axis, estrogen plays a key role in the regulation of bone maturation and growth plate closure. This study was designed to explore the link between single nucleotide polymorphisms (SNPs) in the estrogen receptor 1 (ESR1) gene with idiopathic short stature (ISS) susceptibility in a North Indian population. METHODS: Four SNPs of ESR1 (rs543650, rs6557177, rs2234693 and rs9340799) were genotyped by Sanger sequencing in ISS patients and controls. Linkage disequilibrium (LD) and haplotyping were done by SNPStat and SHEsisPlus software. The extent of LD was determined by calculating D’ and R2 values in SNP paired combinations. RESULTS: Fifty-two ISS patients were compared with 68 controls. A significant positive association was found between rs6557177 and rs543650 genotype and ISS susceptibility. The frequencies of the rs6557177 CC genotype [p=0.030; odds ratio (OR)=0.13; 95% confidence interval (CI): 0.01-1.10] and rs543650 genotype TT (p=0.043; OR=0.29; 95% CI: 0.09-0.92) were increased in the ISS group compared with controls. However, no significant correlation was observed between clinical parameters of patients and these SNPs. rs543650 showed strong LD with rs2234693 and rs9340799, similarly rs2234693 and rs9340799. DISCUSSION AND CONCLUSION: Our study showed that the CC genotype at rs6557177 and TT genotype at rs543650 of ESR1 constituted a risk factor for developing ISS in North Indian children. These findings may lead to a better understanding of the SNPs associated with ISS susceptibility.

The implementation of inclusive education continues to pose challenges to teachers world-wide and Botswana is no exception. This study aimed to explore the teaching competencies general education teachers in Botswana primary schools perceived as important to their teaching in inclusive classrooms. Using a quantitative research approach, 116 general education primary school teachers completed a 14-Item questionnaire titled Perceived Competencies for Inclusive Teaching Scale, and an open-ended question about the supportive resources they required to be effective in their classrooms. Descriptive statistics, ANOVA and t-tests were used to analyse the data. The results of this study showed that participants identified adaptive instructional competencies (including setting curricula goals, modifying learning content and teaching to the different learning styles of students) as most important. Human resources (teacher aides, specialist teachers,) followed by material resources were seen to be key support resources required for them to be able to implement IE. This study could be used as a baseline for future research on key competencies and resources for primary school teachers for inclusive education.

This paper explores social inclusion approaches implemented by ten secondary schools in Victoria, Australia, to support refugee students' well-being, as articulated in their policies, reports, and other published documents. Using an exploratory, qualitative research design, we found that all schools employed a holistic approach to implementing social inclusion programs for refugee students. This paper reports on the best practices and unique examples of social inclusion programs from all schools involved in the study.

Gender equity is an area of concern within STEM, where women are underrepresented in education and career roles. Increasing the representation of women in STEM and removing negative gender stereotypes are necessary actions, both of which can be fostered during formal education through employability initiatives and in the workplace. This study focused on the university setting and explores the perceived employability beliefs of students in STEM (including medicine), particularly their career identity and commitment, learning mindset, awareness of career and how their learning relates to their future career. The 12,708 student responses to an online questionnaire illustrate significant gendered differences in employability beliefs. Female students in STEM have significantly higher confidence in career identity and commitment, and relevance of learning compared to males, although they feel less confident in their reconsideration of career commitment and occupational mobility. In comparison to non-STEM students, women in STEM report higher confidence in career identity and commitment, reconsideration of career commitment, willingness and ability to learn, relevance of learning, career exploration and awareness and occupational mobility. Employability interventions are needed within the STEM educational context to promote long-term equal opportunities for all genders in the workplace.

Introduction The global measles incidence has decreased from 145 to 49 cases per 1 million population from 2000 to 2018, but evaluating the economic benefits of a second measles-containing vaccine (MCV2) is crucial. This study reviewed the evidence and quality of economic evaluation studies to guide MCV2 introduction.Methods The systematic review of model-based economic evaluation studies was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. The search yielded 2231 articles, with 876 duplicates removed and 1355 articles screened, with nine studies included for final analysis.Results Six studies reported a positive benefit–cost ratio with one resulting in net savings of $11.6 billion, and two studies estimated a 2-dose MMR vaccination program would save $119.24 to prevent one measles case, and a second dose could prevent 9,200 cases at 18 months, saving $548.19 per case. The most sensitive variables were the discount rate and vaccination administration cost.Conclusions Two MCV doses or a second opportunity with an additional dose of MCV were highly cost-beneficial and resulted in substantial cost savings compared to a single routine vaccine. But further research using high-quality model-based health economic evaluation studies of MCV2 should be made available to decision-makers.PROSPERO Registration CRD42020200669

Abstract We have recently identified the uncharacterized ZNF555 protein as a component of a productive complex involved in the morbid function of the 4qA locus in facioscapulohumeral dystrophy. Subsequently named DiPRO1 (Death, Differentiation, and PROliferation related PROtein 1), our study provides substantial evidence of its role in the differentiation and proliferation of human myoblasts. DiPRO1 operates through the regulatory binding regions of SIX1, a master regulator of myogenesis. Its relevance extends to mesenchymal tumors, such as rhabdomyosarcoma (RMS) and Ewing sarcoma, where DiPRO1 acts as a repressor via the epigenetic regulators TIF1B and UHRF1, maintaining methylation of cis-regulatory elements and gene promoters. Loss of DiPRO1 mimics the host defense response to virus, awakening retrotransposable repeats and the ZNF/KZFP gene family. This enables the eradication of cancer cells, reprogramming the cellular decision balance towards inflammation and/or apoptosis by controlling TNF-α via NF-kappaB signaling. Finally, our results highlight the vulnerability of mesenchymal cancer tumors to si/shDiPRO1-based nanomedicines, positioning DiPRO1 as a potential therapeutic target.

Objective: The objective is to evaluate dental auxiliaries’ knowledge, attitudes, and practices for the prevention of noma disease. Materials and Methods: This was a cross-sectional study conducted over 6 months. After obtaining ethical clearance from the Sokoto and Zamfara State Ministries of Health, all consenting dental assistant staff who met the selection criteria were administered a validated questionnaire containing 17 questions assessing the respondents’ knowledge, attitudes, and practices. They were recruited using a self-administered questionnaire. The overall score was classified as “low” if the average score was 50% or less (50% or less), satisfactory (51%–69%), or good (70% or more). Data were analyzed using IBM SPSS version 20 software. Results: A total of 149 respondents returned the questionnaire, of which 80 (53.7%) were men and 69 (46.3%) were women. The mean ± standard deviation age of respondents was 27.37 ± 7.00 years, with a range of 20–58 years. The age of respondents was divided into four groups, and the majority of 112 respondents (75.2%) were in their 30s. The majority of respondents, 141 (94.6%), know about the prevention of noma disease. The total respondent score was 1827 points (72.1%), which was evaluated as “good.” Conclusion: Knowledge, attitudes, and practices of dental auxiliaries in the field of noma prevention are good. Dental auxiliaries, if supervised and appropriately trained, can play a role in noma prevention by actively participating in and disseminating information on specific preventive measures such as scaling and polishing.

Rhabdomyolysis can lead to variable clinical presentation and complications putting life at risk. Patients with preexisting low serum 25(OH) Vitamin D are at high risk of exertional rhabdomyolysis during strenuous activities. This aimed to study the pathophysiology of rhabdomyolysis associated with sedentary lifestyle, strenuous exercise, and Vitamin D3 deficiency. A 26-year-old male with chief complaints of cramps in thigh muscles, reduced urine output, and dark coffee-colored urine was presented at Bawa Hospital, Ludhiana. The patient has a history of strenuous exercise and Vitamin D deficiency. Serum creatinine kinase, liver enzymes, and myoglobin in urine were evaluated. The patient was treated with intravenous fluids, sodium bicarbonate, and diuretic. The case was diagnosed as rhabdomyolysis and was treated accordingly. The recovery after 5 days was good following early treatment with fluids and forced alkaline diuresis. The kidney function tests remained normal throughout the treatment period. Aggressive fluid resuscitation is the key management for rhabdomyolysis.

Abstract Background Trigeminal trophic syndrome is a rare cranial and facial condition caused by damage to the central or peripheral branches of the trigeminal nerve. This syndrome consists of a triad of anesthesia, paresthesia, and crescent-shaped facial ulcer involving the ala nasi and sometimes extending to the upper lip. Although previous screening for human immunodeficiency virus in some patients with trigeminal trophic syndrome was negative, we present a unique case of trigeminal trophic syndrome who tested positive for human immunodeficiency virus with eye complications. Case presentation We present a rare case of trigeminal trophic syndrome in a 44-year-old Black African woman who tested positive for human immunodeficiency virus. She presented with a 6-week history of progressive, persistent, and painless left sided facial and scalp ulcerations that started as small skin erosion. Diagnosis of trigeminal trophic syndrome was made on clinical grounds based on the triad of anesthesia, paresthesia, and unilateral crescent-shaped ulcer in the trigeminal dermatome and her past medical history. The ulcer healed completely after counseling and pharmacological therapy, but she later developed left periorbital cellulitis and left upper eyelid full-thickness defect. Conclusion This is by far the first documented case of trigeminal trophic syndrome with a positive human immunodeficiency virus test. Testing for human immunodeficiency virus in patients with trigeminal trophic syndrome is necessary as this can help improve clinical management and treatment outcomes. Seeking the services of specialists remotely in resource constraint settings is beneficial for managing complications associated with trigeminal trophic syndrome.

Abstract Introduction Antimicrobial resistance (AMR) is a global public health concern and irrational use of antibiotics in hospitals is a key driver of AMR. Even though it is not preventable, antimicrobial stewardship (AMS) programmes will reduce or slow it down. Research evidence from Sierra Leone has demonstrated the high use of antibiotics in hospitals, but no study has assessed hospital AMS programmes and antibiotic use specifically among children. We conducted the first-ever study to assess the AMS programmes and antibiotics use in two tertiary hospitals in Sierra Leone. Methods This was a hospital-based cross-sectional survey using the World Health Organization (WHO) point prevalence survey (PPS) methodology. Data was collected from the medical records of eligible patients at the Ola During Children’s Hospital (ODCH) and Makeni Regional Hospital (MRH) using the WHO PPS hospital questionnaire; and required data collection forms. The prescribed antibiotics were classified according to the WHO Access, Watch, and Reserve (AWaRe) classification. Ethics approval was obtained from the Sierra Leone Ethics and Scientific Review Committee. Statistical analysis was conducted using the SPSS version 22. Results Both ODCH and MRH did not have the required AMS infrastructure; policy and practice; and monitoring and feedback mechanisms to ensure rational antibiotic prescribing. Of the 150 patients included in the survey, 116 (77.3%) were admitted at ODCH and 34 (22.7%) to MRH, 77 (51.3%) were males and 73 (48.7%) were females. The mean age was 2 years (SD=3.5). The overall prevalence of antibiotic use was 84.7% (95% CI: 77.9% – 90.0%) and 77 (83.8%) of the children aged less than one year received an antibiotic. The proportion of males that received antibiotics was higher than that of females. Most (58, 47.2 %) of the patients received at least two antibiotics. The top five antibiotics prescribed were gentamycin (100, 27.4%), ceftriaxone (76, 20.3%), ampicillin (71, 19.5%), metronidazole (44, 12.1%), and cefotaxime (31, 8.5%). Community-acquired infections were the primary diagnoses for antibiotic prescription. Conclusion The non-existence of AMS programmes might have contributed to the high use of antibiotics at ODCH and MRH. This has the potential to increase antibiotic selection pressure and in turn the AMR burden in the country. There is need to establish hospital AMS teams and train health workers on the rational use of antibiotics.
