Abstract

Background: Valvular heart disease (VHD) is an important cause of mortality and morbidity and has been subject to important changes in management. The VHD II survey was designed by the EURObservational Research Programme of the European Society of Cardiology to analyze actual management of VHD and to compare practice with guidelines. Methods: Patients with severe native VHD or previous valvular intervention were enrolled prospectively across 28 countries over a 3-month period in 2017. Indications for intervention were considered concordant if the intervention was performed or scheduled in symptomatic patients, corresponding to Class I recommendations specified in the 2012 European Society of Cardiology and in the 2014 American Heart Association/American College of Cardiology VHD guidelines. Results: A total of 7247 patients (4483 hospitalized, 2764 outpatients) were included in 222 centers. Median age was 71 years (interquartile range, 62–80 years); 1917 patients (26.5%) were ≥80 years; and 3416 were female (47.1%). Severe native VHD was present in 5219 patients (72.0%): aortic stenosis in 2152 (41.2% of native VHD), aortic regurgitation in 279 (5.3%), mitral stenosis in 234 (4.5%), mitral regurgitation in 1114 (21.3%; primary in 746 and secondary in 368), multiple left-sided VHD in 1297 (24.9%), and right-sided VHD in 143 (2.7%). Two thousand twenty-eight patients (28.0%) had undergone previous valvular intervention. Intervention was performed in 37.0% and scheduled in 26.8% of patients with native VHD. The decision for intervention was concordant with Class I recommendations in symptomatic patients with severe single left-sided native VHD in 79.4% (95% CI, 77.1–81.6) for aortic stenosis, 77.6% (95% CI, 69.9–84.0) for aortic regurgitation, 68.5% (95% CI, 60.8–75.4) for mitral stenosis, and 71.0% (95% CI, 66.4–75.3) for primary mitral regurgitation. Valvular interventions were performed in 2150 patients during the survey; of them, 47.8% of patients with single left-sided native VHD were in New York Heart Association class III or IV. Transcatheter procedures were performed in 38.7% of patients with aortic stenosis and 16.7% of those with mitral regurgitation. Conclusions: Despite good concordance between Class I recommendations and practice in patients with aortic VHD, the suboptimal number in mitral VHD and late referral for valvular interventions suggest the need to improve further guideline implementation.

Abstract

The role of coronary-artery bypass grafting (CABG) in the treatment of patients with coronary artery disease and heart failure has not been clearly established.Between July 2002 and May 2007, a total of 1212 patients with an ejection fraction of 35% or less and coronary artery disease amenable to CABG were randomly assigned to medical therapy alone (602 patients) or medical therapy plus CABG (610 patients). The primary outcome was the rate of death from any cause. Major secondary outcomes included the rates of death from cardiovascular causes and of death from any cause or hospitalization for cardiovascular causes.The primary outcome occurred in 244 patients (41%) in the medical-therapy group and 218 (36%) in the CABG group (hazard ratio with CABG, 0.86; 95% confidence interval [CI], 0.72 to 1.04; P=0.12). A total of 201 patients (33%) in the medical-therapy group and 168 (28%) in the CABG group died from an adjudicated cardiovascular cause (hazard ratio with CABG, 0.81; 95% CI, 0.66 to 1.00; P=0.05). 21. [Multichamber pacing--a new non-pharmacologic method for the treatment of severe heart failure]

Abstract

According to initial clinical results biventricular pacing seems to be effective in the treatment of patients suffering from drug refractory severe heart failure combined with intraventricular conduction disturbance. Biventricular cardioverter defibrillators and biventricular pacemakers were implanted in patients suffering from drug refractory severe heart failure in 10 and in 15 cases, respectively. NYHA III-IV functional class, low left ventricular ejection fraction (24.2 +/- 6%), wide QRS (150 ms) with left bundle branch block and lateral dyssynchrony were present in each case. The left ventricle was enlarged in each patient (endodiastolic/endsystolic diameter: 78.6 +/- 9.2/68.2 +/- 8.3). The indications of cardioverter defibrillator implantations were both sustained ventricular tachycardia and ventricular fibrillation, nonsustained ventricular tachycardia combined with syncope in 7 and in 3 case, respectively. The duration of QRS decreased (187 +/- 32 vs. 136 +/- 19 ms, p = 0.012) and wall movement disorder disappeared. At the last follow up every patients were in NYHA II functional class and a decrease in left ventricular diameter could be observed (endodiastolic: 70.3 +/- 9.1 mm, p = 0.04; endosystolic: 61.9 +/- 8.8 mm, p = 0.04). During the follow up period (8.8 +/- 5.1 months) 18 episodes of ventricular arrhythmias were detected in the same patient, 2 patients died (1 arrhythmia death, 1 sudden cardiac death). Biventricular pacemakers and cardioverter defibrillators were implanted and applied successfully in the treatment of congestive heart failure for the first time in Hungary.

Abstract

The neuroprotective effect of intraperitonally administered GYKI 52466 (2,3-benzodiazepine derivate) was investigated on AMPA (alpha-amino-3-hydroxy-5-methyl-4-isoxalon-propionic acid)-induced neuronal degeneration in the striatum of adult rats. The dose-dependent neurotoxic effect of AMPA was evaluated by the decrease in the activity of choline acetyltransferase (ChAT), due to degeneration of cholinergic neurons. An injection of 25 mg/kg GYKI 52466 30 min prior to the striatal application of 50 nmol AMPA, followed by repeated application of GYKI 52466 (10 times 5 mg/kg at 10 min intervals, reaching a final dose of 75 mg/kg) was able to prevent neuronal damage monitored by ChAT activity. Conversely, co-injection of GYKI 52466 (50 and 75 mg/kg) with AMPA (50 and 100 nmol) did not elicit any significant protection against the neuronal loss as measured by the ChAT enzyme activity. Therefore, one dose of agonist decreasing ChAT activity by about 40% (50 nmol) was tested on [3H]girisopam binding sites and on the immunoreactivity of glial fibrillary acid protein. The lesions were measured on methylene blue-stained serial sections with a computer assisted image analysis program (NIH Image 1.60). As a result of the AMPA treatment [3H]girisopam binding sites became depleted, and the immunoreactivity of glial fibrillary acid protein increased and on the site of the injection in the striatum a lesion developed. In the presence of AMPA (50 nmol) administered intrastriatally, GYKI 52466 (75 mg/kg i.p.) was able to make the radioactive signal of [3H]girisopam reappear. The volume of AMPA induced neuronal damage in the striatum and the extent of gliosis was reduced. These data provide evidence for the neuroprotective effect of GYKI 52466, and suggest a potential therapeutic value in some neurological disorders.

Abstract

Introduction: Hip fracture is a common injury among older people and is associated with increased risk of morbidity and death. Prolonged waiting for hip fracture surgery is considered detrimental, but recommendations regarding timing of surgery vary. We used the Swedish hip fracture registry RIKSHOFT and two administrative registries to explore the association between time to surgery and postoperative morbidity, as well as between time to surgery and time spent admitted to hospital, up to 120 days after the initial hospital discharge. Methods: 63998 patients ≥65 years old, admitted to a hospital between January 1st, 2012 and August 31st, 2017 were included. Primary outcomes were atrial fibrillation/flutter (AF), congestive heart failure (CHF), pneumonia and a combination of stroke/intracranial bleeding, myocardial infarction and acute kidney injury. Time to surgery was divided into <12 h, 12-24 h and >24 h. Crude and adjusted survival analyses were performed. Differences in time admitted to a hospital were tested using chi² tests. Results: For those waiting >24 hours, there was an increased risk of AF and CHF, and the risk of having CHF was slightly higher for women than for men. Overall, the patients who had waited 12-24 h had no increased risks compared with the patients operated <12 hours. Those waiting longer for surgery spent more time admitted to a hospital than those with shorter waiting times, but most patients of all groups had no readmissions. Conclusion: Waiting >24 hours for hip fracture surgery is associated with worse outcomes, which should be considered when creating new guidelines. [ABSTRACT FROM AUTHOR]

Abstract

Following the development of new techniques in echocardiography and especially of Doppler color flow imaging, a new dimension of cardiac surgery has been brought about. The use of transesophageal probes facilitates routine surgery to be performed without interferences and, also, comparability between repeated ultrasonic measurements. Originally, echocardiography was mainly applied for quality control by comparing pre- and postoperative echocardiograms. Now it is equally important in perioperative strategy planning guiding the surgeon's decision throughout the operation as well as in cardiac emergencies. Using intraoperative echocardiography routinely in cardiac surgery establishes a more and more close cooperation between surgeons, cardiologists, and anesthesiologists. In order to evaluate the clinical relevance of echo findings, they have to be repeatedly correlated to surgical realities and to long term results of clinical outcome.

36. Cost-effectiveness analysis of goal-directed hemodynamic treatment of elderly hip fracture patients: before clinical research starts.

Abstract

BACKGROUND: Health economic evaluations are increasingly used to make the decision to adopt new medical interventions. Before such decisions, various stakeholders have invested in clinical research. But health economic factors are seldom considered in research funding decisions. Cost-effectiveness analyses could be informative before the launch of clinical research projects, particularly when a targeted intervention is resource-intensive, total cost for the trial is very high, and expected gain of health benefits is uncertain. This study analyzed cost-effectiveness using a decision analytic model before initiating a large clinical research project on goal-directed hemodynamic treatment of elderly patients with hip fracture. METHODS: A probabilistic decision analytic cost-effectiveness model was developed; the model contains a decision tree for the postoperative short-term outcome and a Markov structure for long-term outcome. Clinical effect estimates, costs, health-related quality-of-life measures, and long-term survival constituted model input that was extracted from clinical trials, national databases, and surveys. Model output consisted of estimated medical care costs related to quality-adjusted life-years. RESULTS: In the base case analysis, goal-directed hemodynamic treatment reduced average medical care costs by 1,882 and gained 0.344 quality-adjusted life-years. In 96.5% of the simulations, goal-directed hemodynamic treatment is less costly and provides more quality-adjusted life-years. The results are sensitive to clinical effect size variations, although goal-directed hemodynamic treatment seems to be cost-effective even with moderate clinical effect. CONCLUSION: This study demonstrates that cost-effectiveness analysis is feasible, meaningful, and recommendable before launch of costly clinical research projects. [ABSTRACT FROM AUTHOR]

Researchers induced sleep in a collection of rats with an injection of a chemical mixture. They found that in rats who were fed an optimal diet, prolonged sleep led to a higher eruption rate of incisors.

Abstract

Purpose. - This work evaluated the interobserver variability in cone beam computed tomography (CBCT) registration for prostate cancers treated with intensity-modulated radiotherapy. Material and methods. - Twelve technologists realized 286 CBCT/CT registrations (bone registration followed by prostate to prostate registration). The registration results were compared to those obtained by two radiation oncologists (reference). Each technologist reported the shifts calculated by the software in all three axes. A statistical analysis allowed us to calculate the minimum threshold under which 95% of the observers found similar values. A variance analysis followed by the post hoc test were used to find differences in interobserver registration variability and determine whether any individual users performed registrations which differed significantly from those of the other users. Results. - The registration differences compared to the reference in the three directions in terms of 95th percentile are: 2.1 mm left-right, 3.5 mm target-gun, 7.3 mm anterior-posterior. In the posterior direction, 4% of the observers have found differences superior to 8 mm, margin used in routine without the use of a daily CBCT. The variance test revealed a P-value

Abstract

Background: Aortic dissection occurs rarely during pregnancy but carries a significantly high vital risk for both the mother and the fetus. Early diagnosis and treatment are critical for a successful outcome., Case Presentation: A 32-year-old pregnant woman at 31 weeks of gestation began experiencing shortness of breath, chest pain, and palpitations, which were attributed to an anxiety disorder she had been previously diagnosed with. The symptoms continued to worsen following the delivery when a chest computed tomography investigation revealed signs of chronic type A aortic dissection and an 80 mm post-dissection aneurysm of the ascending aorta. Aortic repair via composite aortic root replacement surgery (Bentall procedure) and partial aortic arch replacement was performed. The patient's postoperative evolution was notable for a series of complications., Conclusions: Our report highlights the importance of thorough maternal examination during pregnancy. The high mortality rates associated with aortic dissection occurring in pregnant women and the possibility of missed intervention due to atypical clinical presentation warrant the need for standardized international protocols aimed at the prevention and timely diagnosis of prenatal aortic disease., Competing Interests: Declarations. Abstract

Background: Postoperative mortality might be influenced by postoperative care, vigilance, and competence to rescue. This study aims to describe the course of events preceding death in a high-risk surgical cohort., Methods: We analyzed hospital records of patients who died within 30 days after surgery in 4 high volume hospitals using (1) reflective narrative thematic approach to identify recurring themes reflecting issues with conduct of care and (2) Global Trigger Tool to describe incidence, timing, and types of adverse events (AEs) leading to harm., Results: Preoperative predicted median risk of death in the studied group was 9%/13% according to SORT/P-POSSUM, respectively. Nine recurring themes were identified. Prominent themes were "consensus concerning aim and/or risk with planned surgery," "level of (intraoperative) competence and monitoring," and in the postoperative period "level of care and vigilance" on signs of deterioration. We found a total of 303 AEs, with only three patients (5%) having no adverse events. Most common severity category was "I," that is "contributed to patient's death" (n = 110, 36% of all AEs). Of these, 60% were classified as preventable or probably preventable. The peak incidence of AEs was seen on the day of index surgery. Most common types of AEs were "failure of vital functions" (n = 79, 26%), followed by infections (n = 45, 15%)., Conclusions: A high predicted risk of death and a peak of adverse events on the day of index surgery were detected. Abstract

Background: Feasible estimations of perioperative changes in oxygen consumption (VO2) could enable larger studies of its role in postoperative outcomes. Current methods, either by reverse Fick calculations using pulmonary artery catheterisation or metabolic by breathing gas analysis, are often deemed too invasive or technically requiring. In addition, reverse Fick calculations report generally lower values of oxygen consumption., Methods: We investigated the relationship between perioperative estimations of VO2 (EVO2), from LiDCO™plus-derived (LiDCO Ltd, Cambridge, UK) cardiac output and arterial-central venous oxygen content difference (Ca-cvO2), with indirect calorimetry (GVO2) by QuarkRMR (COSMED srl. Italy), using data collected 2017-2018 during a prospective observational study on perioperative oxygen transport in 20 patients >65 years during epidural and general anaesthesia for open pancreatic or liver resection surgery. Eighty-five simultaneous intra- and postoperative measurements at different perioperative stages were analysed for prediction, parallelity and by traditional agreement assessment., Results: Unadjusted bias between GVO2 and EVO2 indexed for body surface area was 26 (95% CI 20 to 32) with limits of agreement (1.96SD) of -32 to 85 ml min-1m-2. Correlation adjusted for the bias was moderate, intraclass coefficient(A,1) 0.51(95% CI 0.34 to 0.65) [F (84,84) = 3.07, P<0.001]. There was an overall association between GVO2 and EVO2, in a random coefficient model [GVO2 = 73(95% CI 62 to 83) + 0.45(95% CI 0.29 to 0.61) EVO2 ml min-1m-2, P<0.0001]. GVO2 and EVO2 changed in parallel intra- and postoperatively when normalised to their respective overall means., Conclusion: Based on this data, estimations from LiDCO™plus-derived cardiac output and Ca-cvO2 are not reliable as a surrogate for perioperative VO2. Results were in line with previous studies comparing Fick-based and metabolic measurements but limited by variability of data and possible underpowering.
