Abstract

Introduction: Although pregnancy is a physiological condition, the secretion of diabetogenic hormones such as growth hormone, corticotropin-releasing hormone, placental lactogen hormone, prolactin and progesterone from the placenta could lead to insulin resistance (IR). In Metabolic Syndrome, obesity and Type1&2 diabetes, a shift in the kynurenine pathway (KP) towards IDO activation is observed. The activation of the IDO leads to the activation of the Aryl hydrocarbon receptor (AhR) and Interleukin-6 (IL-6) also, which may also induce some effects like insulin resistance, β-cell disfunction and increased gluconeogenesis. We hypothesized that the overactivation of IDO and some KP enzymes would be observed in GDM patients, in a similar manner to metabolic syndrome, prediabetes, and diabetes patients.

Abstract

Background: Fractalkine (FKN) is an inflammatory cytokine that has been shown with increased serum levels in diabetic patients and is considered to contribute to the adipose tissue inflammation by supporting monocyte adhesion to adipocytes which has an important role in the pathogenesis of type 2 diabetes mellitus (T2DM). Our aim was to evaluate the effects of glucose ingestion on the serum fractalkine levels in healthy subjects with normal glucose tolerance (NGT) and newly diagnosed T2DM patients. Methods: A total of 67 patients were included in this study, and they were divided into NGT (n= 34) and T2DM (n= 33) groups according to their oral glucose tolerance test (O G TT) results. The serum FKN and C-reactive protein (CRP) levels were measured at 0 and 120 minutes during an O G TT following overnight fasting. Results: The 0-minute (basal) and 120-minute O G TT FKN levels were found to be significantly higher in the T2DM group when compared to the NGT group (p= 0.012 and p= 0.001, respectively). However, no significant differences were observed in terms of the changes in the basal and 120-minute O G TT FKN levels in the T2DM and NGT groups (p= 0.433 and p= 0.06, respectively). A significant positive correlation was observed between the 120-minute O G TT FKN and glucose levels in the study group consisting of all of the patients (r= 0.331, p= 0.006). Conclusions: In this study, basal and post-glycemic load FKN levels were found to be higher in newly diagnosed T2DM patients than those with NGT; however, there was no additional change in FKN levels by glycemic load.

Abstract

Aim: This study aimed to investigate the rate of carpal tunnel syndrome (CTS) and polyneuropathy in patients with new-diagnosed hypothyroidism and the relationship between median nerve conduction and the cross-sectional area of the median nerve by ultrasonography. Material and Methods: It was a prospective, cross-sectional and case-control study. This study included thirty-five new-diagnosed hypothyroidism cases and thirty-five healthy controls. Bilateral sensory and motor nerve potentials were noted in the lower and upper extremities. The cross-sectional area of the median nerve was examined at the entrance of the CT with the axial plan by ultrasonography. The relationship between the cross-sectional areas of the median nerve and nerve conduction parameters was investigated. Results: CTS was determined electrophysiologically in 8 (22.9%) patients. The control group had no CTS. There was no significant electrophysiological finding to support polyneuropathy in the patients. The cross-sectional areas of the median nerve were higher in the patient group but did not reach statistically significant (p>0.05). There was a positive correlation between the right and left cross-sectional area of median nerves and body mass index (BMI) (p<0.05). Conclusion: The rate of CTS is high in patients with newly diagnosed hypothyroidism. There is a positive correlation between crosssectional nerve areas and BMI. There was no significant correlation between nerve conductions and median nerve ultrasonographic cross-sectional areas. [ABSTRACT FROM AUTHOR]

Abstract

The purpose of this study was to determine whether or not Helicobacter pylori (H. pylori) infection affects the hypothalamic-pituitary-adrenal (HPA) axis and to evaluate the association between vitamin D levels with H. pylori infected and eradicated patients. The glucagon stimulation test (GST) was used to assess the HPA gland axis. An H. pylori infection was diagnosed based on the rapid urease test and histology. All infected patients received triple eradication therapy. Three months after the treatment, 14C urea breath test was reinstituted, and GST was repeated in patients who were negative for H. pylori. Of the 43 patients, 20 (46%) were found to have a lower mean value of peak cortisol than normal responses to the GST in the H. pylori-infected subjects. In 29 subjects, H. pylori infection was successfully eradicated. Twelve (41%) out of 29 subjects were defined as having a blunted GST response and 17 (%59) subjects had normal HPA axis response to GST in the H. pylori-eradicated subjects. A significant correlation between low 25-hydroxyvitamin D3 (25(OH)D3) level and low peak cortisol response to GST was identified in the H. pylori-infected subjects. After H. pylori eradication, the positive correlation between 25(OH)D3 level and high peak cortisol response to GST was also identified. Mean 25(OH)D3 levels were lower in the H. pylori-infected subjects than in the H. pylori-eradicated subjects. Our results indicate an increased prevalence of blunted glucocorticoid response to GST in patients with infected H. pylori. Vitamin D deficiency is also common in the H. pylori-infected subjects and associated with blunted glucocorticoid response. These findings suggest that H. pylori eradication increases the cortisol response to GST. Vitamin D supports adrenal/cortisol production whereby a deficiency can result blunted glucocorticoid response to GST in patients with infected H. pylori.

Abstract

Hypersecretion of PTHrP is a relatively common cause of malignancy-related hypercalcemia. However, there is only one case report of letrozole induced hypercalcemia. A 52-year-old female patient was referred to our clinic because of the recent discovery of hypercalcemia (11.0 mg/dL). The patient had a history of left breast carcinoma. She had started a course of letrozole (aromatase inhibitor; 2.5 mg dose/day) ten months earlier. Patient’s parathyroid hormone-related protein levels were normal and a bone scintigram revealed no evidence of skeletal metastasis. Other potential causes of high calcium levels were ruled out. We recognized that, when letrozole was taken at one dose daily (2.5 mg), she had recurrent hypercalcemia. Our experience suggests that letrozole may precipitate hypercalcemia in a patient with breast cancer.

A 59-year-old female patient was admitted to the emergency service with complaints of hematemesis and melena for the last few days. In laboratory tests, the platelet count was found to be 6×109/L. Intravenous or oral corticosteroid treatment was thought to be given for ITP but disclaimed due to upper GIS bleeding. On the 5th day of treatment, Brucella melitensis was isolated from blood culture before the results of Wright tube agglutination tests were reported positive as 1 : 80. On the second day of the anti-brucellosis treatment, the thrombocyte count was raised from 6000/mm3 to 110000/mm3, and on the 3rd day to 225000/mm3.

Abstract

Aim of the study: To determine the parenchymal vascularity of the thyroid gland with color superb microvascular imaging in patients with Graves’ disease, and compare the vascularization index values with healthy subjects. Materials and methods: The thyroid glands of 37 patients whose laboratory and clinical findings were consistent with Graves’ disease, and 40 asymptomatic subjects with normal laboratory values, were examined using color superb microvascular imaging. Measurements of the vascularization index were performed with a free region of interest which was drawn along the outer margin of the gland on the color superb microvascular imaging mode. The vascularization index values obtained in the Graves’ disease and control groups were compared. A correlation analysis was performed between the vascularization index values and laboratory and grayscale US parameters. Abstract

Introduction: Gestational diabetes is defined as various degrees of glucose intolerance diagnosed or detected for the first time during pregnancy and is the most common metabolic complication of pregnancy. Early diagnosis and adequate treatment are important to prevent complications. Preeclampsia, polyhydramnios, fetalmacrosomia, and operative delivery are some of the complications seen in pregnant women diagnosed with Gestational Diabetes Mellitus (GDM). Aim: The present study was designed to determine whether there was an association between Mean Platelet Volume (MPV) in predicting poor fetal outcome, insulin resistance, neonatal Apgar scores and gestational age for women with GDM. Materials and Methods: In this retrospective study, we enrolled 101 pregnant women with GDM together with a group of 138 healthy controls. MPV, insulin and homeostatic model assessment (HOMA-IR) values were measured at 24-28 weeks of the pregnancy. An independent samples t-test was used to compare MPV values. Multivariate linear regression models were used to establish relations between MPV values, HOMAIR, insulin levels and Apgar score. Results: There was a significant positive correlation between MPV values, HOMA-IR and Insulin levels and a negative correlation with Apgar score at 1 min and 5 min in the GDM group (r=0.227, p=0.02; r=0.206, p=0.03; r=-0.485, p<0.001; and r=-0.399, p<0.001, respectively). In the multivariate logistic regression analysis, a high MPV value was most consistently associated with a low Apgar 1 min score (β=-0.387, p=0.003) in the GDM group. An MPV of >8.0 fL had a sensitivity of 82% and a specificity of 75% for the prediction of GDM.
