- Abstract
Objectives Left-sided methicillin-susceptible Staphylococcus aureus (MSSA) endocarditis treated with cloxacillin has a poorer prognosis when the vancomycin minimum inhibitory concentration (MIC) is >=1.5 mg/L. We aimed to validate this using the International Collaboration on Endocarditis cohort and to analyse whether specific genetic characteristics were associated with a high vancomycin MIC (>=1.5 mg/L) phenotype. Methods All patients with left-sided MSSA infective endocarditis treated with antistaphylococcal beta-lactam antibiotics between 2000 and 2006 with available isolates were included. Vancomycin MIC was determined by Etest as either high (>=1.5 mg/L) or low (<1.5 mg/L). Isolates underwent spa typing to infer clonal complexes and multiplex PCR for identifying virulence genes. Univariate analysis was performed to evaluate the association between in-hospital and 1-year mortality, and vancomycin MIC phenotype. Results Sixty-two cases met the inclusion criteria. Vancomycin MIC was low in 28 cases (45%) and high in 34 cases (55%). No significant differences in patient demographic data or characteristics of infection were observed between patients with infective endocarditis due to high and low vancomycin MIC isolates. Isolates with high and low vancomycin MIC had similar distributions of virulence genes and clonal lineages. In-hospital and 1-year mortality did not differ significantly between the two groups (32% (9/28) vs. 27% (9/34), p 0.780; and 43% (12/28) vs. 29% (10/34), p 0.298, for low and high vancomycin MIC respectively). Conclusions In this international cohort of patients with left-sided MSSA endocarditis treated with antistaphylococcal beta-lactams, vancomycin MIC phenotype was not associated with patient demographics, clinical outcome or virulence gene repertoire.Copyright © 2017 European Society of Clinical Microbiology and Infectious Diseases

The article is devoted to defining theoretical principles, tools, and mechanisms of fiscal policy; compliance of Ukrainian fiscal policy with existing theoretical principles and practice in this area of government regulation in developed countries.The main stages in the evolution of Keynesian strategies of macro regulation are highlighted.We analyzed current phase, which is defined as a phase of fiscal consolidation that started at the theoretical level with discussions on what is more effective: a policy based on stringent rules and laws or policy based on circumstances. It is emphasized that practical importance of theoretical discussions was increased with side effects, that have arisen during the economic policies to overcome the global financial crisis of 2008-2009. Overview of the basic measures stimulating fiscal policy pursued by developed countries to overcome the financial crisis of 2008-2009 is provided. It is determined that the leading countries adhere to common tactic of stimulating fiscal policy and concluded that despite some modifications, fiscal policy is based on the basic postulates of Keynesian theory of macroeconomic regulation and experience of anti-crisis policy of recent years proves its efficiency in modern conditions of information and network economy. The content of Ukrainian fiscal policy in modern terms is revealed. Factors that are crucial for fiscal policy are systematized. It was found that in recent years a contractionary policy was implemented, due to the need for fiscal consolidation, and despite the economic downturn, rising unemployment, reduced total expenditures.The following features of fiscal policy are determined: reaction is not systematic, but rather pointed; discretionary fiscal policy mechanism prevails; there is a discrepancy between the versions of the fiscal policy and the phases of the economic cycle, due to both objective factors such as the existence of military conflict and the lack of necessary funds and government implementation of quasi-fiscal operations, and focus on the action of purely economic factors to improve economic situation in the country., Статья посвящена определению теоретических основ, инструментов, механизмов бюджетно-налоговой политики и соответствия мер бюджетно-налоговой политики Украины существующим теоретическим положениям и практике проведения данного направления государственного регулирования экономики в развитых странах. Выделено основные этапы эволюции кейнсианской стратегии макрорегулирования. Обобщенно основные меры стимулирующей бюджетно-налоговой политики, которую проводили развитые страны для преодоления последствий финансово-экономического кризиса 2008-2009 гг. Раскрыто содержание бюджетно-налоговой политики Украины в современных условиях, систематизировано факторы, которые являются определяющими для её проведения., Стаття присвячена визначенню теоретичних засад, інструментів, механізмів бюджетно-податкової політики та відповідності заходів бюджетно-податкової політики України існуючим теоретичним положенням і практиці проведення даного напряму державного регулювання економіки у розвинутих країнах. Виділено основні етапи еволюції кейнсіанської стратегії макрорегулювання. Узагальнено основні заходи стимулюючої бюджетно-податкової політики, яку проводили розвинуті країни для подолання наслідків фінансово-економічної кризи 2008-2009 рр. Ukrainian agriculture creates 12-14% of GDP. Ensuring the conditions for sustainable economic development implies the use of adequate mechanisms for regulating economic processes by the government. In the process of formation and implementation of the organizational and economic mechanism of regulatory policy, a system of indicators plays an important role in assessing the impact of such policy on the participants, monitoring and, in case of the deviation from planned results, adjusting regulatory measures. This research analyzes and systematizes the indicators that determine organizational and economic mechanism of regulatory policy effectiveness in agriculture. The systematization of indicators to evaluate organizational and economic mechanism of regulatory policy effectiveness in agriculture has allowed to substantiate the methodological principles of its integrated assessment. Application of the proposed methodological approach to assessing the organizational and economic mechanism of regulatory policy effectiveness in agriculture in Ukraine was performed during the 2010, 2016-2017 period. It revealed a slow progress in the organizational and economic mechanism of regulatory policy efficiency during the investigated period. This result is primarily attributed to the lack of purposeful, systematic change management, the lack of mid-term planning, the strictness of most programs, imperfect financial instruments of the organizational and economic mechanism, and procedures for application. - Abstract
The article is devoted to defining theoretical principles, tools, and mechanisms of fiscal policy; compliance of Ukrainian fiscal policy with existing theoretical principles and practice in this area of government regulation in developed countries.The main stages in the evolution of Keynesian strategies of macro regulation are highlighted.We analyzed current phase, which is defined as a phase of fiscal consolidation that started at the theoretical level with discussions on what is more effective: a policy based on stringent rules and laws or policy based on circumstances. It is emphasized that practical importance of theoretical discussions was increased with side effects, that have arisen during the economic policies to overcome the global financial crisis of 2008-2009. Overview of the basic measures stimulating fiscal policy pursued by developed countries to overcome the financial crisis of 2008-2009 is provided. It is determined that the leading countries adhere to common tactic of stimulating fiscal policy and concluded that despite some modifications, fiscal policy is based on the basic postulates of Keynesian theory of macroeconomic regulation and experience of anti-crisis policy of recent years proves its efficiency in modern conditions of information and network economy. The content of Ukrainian fiscal policy in modern terms is revealed. Factors that are crucial for fiscal policy are systematized. It was found that in recent years a contractionary policy was implemented, due to the need for fiscal consolidation, and despite the economic downturn, rising unemployment, reduced total expenditures.The following features of fiscal policy are determined: reaction is not systematic, but rather pointed; discretionary fiscal policy mechanism prevails; there is a discrepancy between the versions of the fiscal policy and the phases of the economic cycle, due to both objective factors such as the existence of military conflict and the lack of necessary funds and gover, Статья посвящена определению теоретических основ, инструментов, механизмов бюджетно-налоговой политики и соответствия мер бюджетно-налоговой политики Украины существующим теоретическим положениям и практике проведения данного направления государственного регулирования экономики в развитых странах. - Abstract
Background. The impact of early valve surgery (EVS) on the outcome of Staphylococcus aureus (SA) prosthetic valve infective endocarditis (PVIE) is unresolved. The objective of this study was to evaluate the association between EVS, performed within the first 60 days of hospitalization, and outcome of SA PVIE within the International Collaboration on Endocarditis-Prospective Cohort Study. Methods. Participants were enrolled between June 2000 and December 2006. Cox proportional hazards modeling that included surgery as a time-dependent covariate and propensity adjustment for likelihood to receive cardiac surgery was used to evaluate the impact of EVS and 1-year all-cause mortality on patients with definite left-sided S. aureus PVIE and no history of injection drug use. Results. EVS was performed in 74 of the 168 (44.3%) patients. One-year mortality was significantly higher among patients with S. aureus PVIE than in patients with non-S. aureus PVIE (48.2% vs 32.9%; P = .003). Staphylococcus aureus PVIE patients who underwent EVS had a significantly lower 1-year mortality rate (33.8% vs 59.1%; P = .001). In multivariate, propensity-adjusted models, EVS was not associated with 1-year mortality (risk ratio, 0.67 [95% confidence interval, .39-1.15]; P = .15). Conclusions. In this prospective, multinational cohort of patients with S. aureus PVIE, EVS was not associated with reduced 1-year mortality. The decision to pursue EVS should be individualized for each patient, based upon infection-specific characteristics rather than solely upon the microbiology of the infection causing PVIE.Copyright © The Author 2014.

- Abstract
Background. The timing of cardiac surgery after stroke in infective endocarditis (IE) remains controversial. We examined the relationship between the timing of surgery after stroke and the incidence of in-hospital and 1-year mortalities. Methods. Data were obtained from the International Collaboration on Endocarditis-Prospective Cohort Study of 4794 patients with definite IE who were admitted to 64 centers from June 2000 through December 2006. Multivariate logistic regression and Cox regression analyses were performed to estimate the impact of early surgery on hospital and 1-year mortality after adjustments for other significant covariates. Results. Of the 857 patients with IE complicated by ischemic stroke syndromes, 198 who underwent valve replacement surgery poststroke were available for analysis. Overall, 58 (29.3%) patients underwent early surgical treatment vs 140 (70.7%) patients who underwent late surgical treatment. After adjustment for other risk factors, early surgery was not significantly associated with increased in-hospital mortality rates (odds ratio, 2.308; 95% confidence interval[CI], .942-5.652). Overall, probability of death after 1-year follow-up did not differ between 2 treatment groups (27.1% in early surgery and 19.2% in late surgery group, P = .328; adjusted hazard ratio, 1.138; 95% CI, .802-1.650). Conclusions. There is no apparent survival benefit in delaying surgery when indicated in IE patients after ischemic stroke. Further observational analyses that include detailed pre- and postoperative clinical neurologic findings and advanced imaging data (eg, ischemic stroke size), may allow for more refined recommendations on the optimal timing of valvular surgery in patients with IE and recent stroke syndromes.Copyright © The Author 2012.

- Abstract
The HACEK organisms (Haemophilus species, Aggregatibacter species, Cardiobacterium hominis, Eikenella corrodens, and Kingella species) are rare causes of infective endocarditis (IE). The objective of this study is to describe the clinical characteristics and outcomes of patients with HACEK endocarditis (HE) in a large multi-national cohort. Patients hospitalized with definite or possible infective endocarditis by the International Collaboration on Endocarditis Prospective Cohort Study in 64 hospitals from 28 countries were included and characteristics of HE patients compared with IE due to other pathogens. Of 5591 patients enrolled, 77 (1.4%) had HE. HE was associated with a younger age (47 vs. 61 years; p<0.001), a higher prevalence of immunologic/vascular manifestations (32% vs. 20%; p<0.008) and stroke (25% vs. 17% p = 0.05) but a lower prevalence of congestive heart failure (15% vs. 30%; p = 0.004), death in-hospital (4% vs. 18%; p = 0.001) or after 1 year follow-up (6% vs. 20%; p = 0.01) than IE due to other pathogens (n = 5514). On multivariable analysis, stroke was associated with mitral valve vegetations (OR 3.60; CI 1.34-9.65; p<0.01) and younger age (OR 0.62; CI 0.49-0.90; p<0.01). The overall outcome of HE was excellent with the in-hospital mortality (4%) significantly better than for non-HE (18%; p<0.001). Prosthetic valve endocarditis was more common in HE (35%) than non-HE (24%). The outcome of prosthetic valve and native valve HE was excellent whether treated medically or with surgery. Current treatment is very successful for the management of both native valve prosthetic valve HE but further studies are needed to determine why HE has a predilection for younger people and to cause stroke. The small number of patients and observational design limit inferences on treatment strategies. Self selection of study sites limits epidemiological inferences. © 2013 Chambers et al.

- Abstract
The HACEK organisms (Haemophilus species, Aggregatibacter species, Cardiobacterium hominis, Eikenella corrodens, and Kingella species) are rare causes of infective endocarditis (IE). The objective of this study is to describe the clinical characteristics and outcomes of patients with HACEK endocarditis (HE) in a large multi-national cohort. Patients hospitalized with definite or possible infective endocarditis by the International Collaboration on Endocarditis Prospective Cohort Study in 64 hospitals from 28 countries were included and characteristics of HE patients compared with IE due to other pathogens. Of 5591 patients enrolled, 77 (1.4%) had HE. HE was associated with a younger age (47 vs. 61 years; p<0.001), a higher prevalence of immunologic/vascular manifestations (32% vs. 20%; p<0.008) and stroke (25% vs. 17% p = 0.05) but a lower prevalence of congestive heart failure (15% vs. 30%; p = 0.004), death in-hospital (4% vs. 18%; p = 0.001) or after 1 year follow-up (6% vs. 20%; p = 0.01) than IE due to other pathogens (n = 5514). On multivariable analysis, stroke was associated with mitral valve vegetations (OR 3.60; CI 1.34-9.65; p<0.01) and younger age (OR 0.62; CI 0.49-0.90; p<0.01). The overall outcome of HE was excellent with the in-hospital mortality (4%) significantly better than for non-HE (18%; p<0.001). Prosthetic valve endocarditis was more common in HE (35%) than non-HE (24%). The outcome of prosthetic valve and native valve HE was excellent whether treated medically or with surgery. Current treatment is very successful for the management of both native valve prosthetic valve HE but further studies are needed to determine why HE has a predilection for younger people and to cause stroke. The small number of patients and observational design limit inferences on treatment strategies. Self selection of study sites limits epidemiological inferences.

- Abstract
Purpose: To evaluate the progression of keratoconus in 932 eyes of 659 patients through visual, refractive and topographic data after intracorneal ring segment (ICRS) implantation., Methods: Retrospective review of 659 patients who underwent ICRS (Intacs®) implantation for keratoconus between September 1997 and November 2017. Demographics, preoperative and postoperative uncorrected visual acuity (UCVA) and best-corrected visual acuity (BCVA) in LogMAR, corneal topography parameters (thinnest pachymetry, Kmax), central corneal pachymetry and total follow-up time were evaluated. Statistical analysis was performed using IBM SPSS Statistics 24.0 for windows., Results: Nine hundred and thirty-two eyes of 659 patients, with a mean age of 30.41 years (range 11-76 years), were evaluated. Mean total follow up time was 3.02 years. Forty-one eyes had a total follow up of over 10 years. Both UCVA and BCVA improved significantly after ICRS implantation (P<0.01). Only 18 eyes (2.66% of eyes of patients under 35 years of age) were found to have progression of keratoconus based on postoperative topographic data (Mean age 23.00 years, 55.6% female, total follow-up 2 to 10 years) CONCLUSION: ICRS implantation showed long-term improvement and stability in visual and topographic results in a large case series of patients with keratoconus. Only a minimal rate of progression was detected in young patients. However, further prospective studies need to be conducted to better define predictability of postoperative visual outcomes and progression., (Copyright © 2020 Elsevier Masson SAS. - Abstract
We describe a case series of four eyes of four keratoconus patients with extruded intracorneal ring segments (ICRS) in whom donor corneal tissue was used to substitute for the extruded ICRS in an attempt to preserve its effect. In all patients, the extruded intracorneal ring was removed through the exposed area. Donor corneal tissue (tailored to the size and shape of the ICRS) was inserted into the empty ICRS tunnel and fixed in place using 10-0 nylon sutures. At three months follow-up, all patients had a healed graft and displayed a stable Kmax, best corrected visual acuity (BCVA) and corneal topography comparable to the preoperative values. Replacing extruded intracorneal rings with donor tissue grafts in keratoconus patients appears to be a promising technique in preserving the effect of the corneal ring and possibly in treating primary keratoconus with donor tissue segments. - Abstract
Although rare, infective endocarditis (IE) continues to cause significant morbidity and mortality. Previous data from the American University of Beirut Medical Center (AUBMC) had shown predominance of streptococcal infection. As worldwide studies in developed countries show increasing trends in Staphylococcus aureus endocarditis, it becomes vital to continually inspect local data for epidemiological variations. We reviewed all IE cases between 2001 and 2014, and we performed a comparison to a historical cohort of 86 IE cases from 1987 to 2001. A total of 80 patients were diagnosed with IE between 2001 and 2014. The mean age was 61 years. The most commonly isolated organisms were streptococci (37%), compared to 51% in the previous cohort. S. aureus accounted for 11%. Only one S. aureus isolate was methicillin-resistant. In the historical cohort, 26% of cases were caused by S. aureus. Enterococci ranked behind staphylococci with 22% of total cases, while in the previous cohort, enterococcal IE was only 4%. Compared to previous data from AUBMC, the rates of streptococcal and staphylococcal endocarditis have decreased while enterococcal endocarditis has increased. This study reconfirms that in Lebanon, a developing country, we continue to have a low predominance of staphylococci as etiologic agents in IE., (Copyright © 2017 The Authors. Purpose: To evaluate the effect of femtosecond laser and excimer laser on an intracorneal inlay (KAMRA
® ) implanted in animal models., Methods: Femtosecond laser was used to create corneal intrastromal pockets at 250μm depth in five porcine eyes. Four intact KAMRA inlays, examined by slit-lamp biomicroscopy and light microscopy, were implanted in the pocket of four eyes. A standard LASIK flap was created above each implanted inlay in the four eyes using a femtosecond laser with flap thicknesses of 150μm, 130μm, 110μm and 90μm. In the fifth porcine eye, a LASIK flap was created using femtosecond laser at 110μm depth, and a fifth inlay was then implanted in the 250μm pocket. Excimer laser ablation was performed under the flap targeting a -3.00 refraction. The inlay was then explanted, examined and reimplanted in the same pocket followed by a second similar excimer laser ablation., Results: Significant burn, shrinkage and distortion of microholes were noted in all the first four inlays following the femtosecond laser flap creation at all the various flap thicknesses. The damage was noted to be more prominent as the distance between the flap and inlay decreased. No apparent effect was noted on the fifth inlay following repeated excimer laser ablations., Conclusion: Unlike excimer laser, femtosecond laser appears to be hazardous and damaging to the intracorneal KAMRA inlay when applied above it., (Copyright © 2017 Elsevier Masson SAS. - Abstract
The objective of this study was to examine the epidemiology and the clinical manifestations of typhoid fever as well as the susceptibility and strain relatedness of Salmonella typhi isolates in Lebanon from 2006 to 2007. A total of 120 patients with typhoid fever were initially identified from various areas of the country based on positive culture results for S. typhi from blood, urine, stools, bone marrow and/or positive serology. Clinical, microbiological and molecular analysis was performed on cases with complete data available. These results indicated that drinking water was an unlikely mode of transmission of the infection. Despite increasing reports of antimicrobial resistance among S. typhi isolates, the vast majority of these isolates were susceptible to various antibiotic agents, including ampicillin, cephalosporins, quinolones, and trimethoprim/sulfamethoxazole. Molecular analysis of the isolates revealed a predominance of one single genotype with no variation in distribution across the geographical regions., (Copyright © 2014 Ministry of Health, Saudi Arabia.
