Döner dengesizlik etkisindeki yatay tamburlu çamaşır makinelerinin iki ve üç boyutluve doğrusal olmayan matematiksel modeli çıkarılacaktır. Several case studies are performed in order to observe the effects oforientation angles of springs and dampers between the drum and the cabinet. In orderto reduce the steady-state vibration amplitude of the drum, suitable orientation anglesof the springs and dampers and their implementation locations are identified.Furthermore, to further reduce the vibration amplitude of the drum at high frequencies,instead of viscous dampers, dry friction dampers are installed in the system. Introduction: Accessory breast tissue is a very rare finding in the general population with an incidence of one to two percent. An even rarer occurrence is accessory mammary-like tissue which developed breast carcinoma. The authors present a case of aggressive and metastatic carcinoma of vulvar originating from mammary-like tissue. Case: A 73-year-old Caucasian female presented with a lesion in her left vulva. The lesion was ulcerated and fragile. A dermatologist had evaluated the lesion and took a punch biopsy. Result was vulvar carcinoma. She was admitted to the gynecologic oncology clinic then after and was operated. After a radical vulvectomy and bilateral inguinal lymphadenectomy she received adjuvant radiotherapy because of lymph node metastasis. One year after the finish of radiotherapy patient was found to have lung and femur metastasis. She began to receive systemic chemotherapy for metastasis. Conclusion: Primary mammary-like adenocarcinoma of the vulva is exceedingly rare. There is no consensus about the diagnosis, treatment, and follow up of these patients in literature. However, given that histological data confirms these cancers are behaving like breast cancers instead of known patterns of vulva cancer, the best treatment practices for breast cancer may be applied to treat these vulvar carcinoma patients.

WOS: 000269782800005, PubMed: 19541303, Objective: To evaluate retrospectively the prevalence of cervical cytological abnormalities in patient records obtained from healthcare centers in Turkey. Method: Demographic characteristics and data on cervical cytological abnormalities were evaluated from patients who underwent flap tests in healthcare centers in 2007. Results: Data were collected from 33 healthcare centers totaling 140 334 patients. Overall, the prevalence of cervical cytological abnormalities was 1.8%; the prevalence of ASCUS, ASC-H, LSIL, HSIL, and AGC was 1.07%, 0.07%, 0.3%, 0.17%, and 0.08%, respectively. The prevalence of preinvasive cervical neoplasia was 1.7% and the prevalence of cytologically diagnosed invasive neoplasia was 0.06%. Conclusion: The abnormal cervical cytological prevalence rate in Turkey is lower than in Europe and North America. This might be due to sociocultural differences, lack of population-based screening programs, or a lower HPV prevalence rate in Turkey. (C) 2009 International Federation of Gynecology and Obstetrics. Amaç: Malign mikst müllerian tümörler (MMMT) epitelyal ve mezenkimal komponentler içeren çok agresif bifazik neoplazmlardır. Kadın genital sisteminde tubada MMMT’ler nadiren görülürler. Olgu Sunumu: Bu bildiride tuba fimbrial uç yerleşimli iki MMMT olgusu sunulmaktadır. Her iki hastada FIGO evresi IIIC (T3c N0 M0) idi. Tümörlerden biri homolog, diğeri heterolog tipteydi. Tartışma: Bu nadir tümörlerin prognozu kötüdür. Hastalarımızdan biri tanı konduktan 6 ay sonra yaşamını yitirmiştir. Bu tümörler için sağkalımı artırmada tam cerrahi, kemoterapi ve eksternal radyoterapi zorunlu görünmektedir., Objective: Malignant mixed müllerian tumors (MMMT) are highly aggressive biphasic neoplasms consisting of both epithelial and mesenchymal components. In female genital tract, MMMTs are rarely seen in fallopian tube. Case Report: We will present two cases of MMMT of fallopian tube located at the fimbrial end. Both patients were FIGO stage IIIC (T3c N0 M0). One of the tumor was homologous and the other one heterologous variety. Conclusion: These rare tumors have poor prognosis, as one of the patients died only in 6 months after the diagnosis. For these tumors complete surgery, chemotherapy and external radiation seemed to be necessary in order to improve survival.

Amaç: Jinekolojik kanserler nedeniyle tedavi planlanan hastalarda fertilitenin korunmasının koşul ve yöntemlerini ve yardımcı üreme tekniklerinin (YÜT) bu konudaki katkılarının literatür ışığında incelenmesi ve derlenmesi. Materyal ve Metod: İngilizce tıp 1 iteratürüne girmiş bulunan vaka sunumu, makale ve derlemelerin özetlenmesi. Sonuç ve Yorum: Jinekolojik onkoloji cerrahisinde fertilite koruyucu cerrahi gün geçtikçe daha da öncelikli hale gelmektedir. Değişik kanser bölgelerine ve hastanın özelliklerine göre bu tip cerrahiye başvurulması ve bu hastalarda yardımcı üreme tekniklerinin kullanımı gereğinde özendirilmelidir. Objective: This review aims to summarize the conditions and methods of fertility sparing surgery in gynecologic oncology and the role of assisted reproductive techniques in this entity. Method: Case reports, clinical trials and reviews about the relevant topic in English literature are summarized. Result and Conclusion: Fertility sparing surgery is getting more essential in gynecologic oncologic treatment modalities day by day. This type of surgery must be promoted in different cancer types and localizations. Use of assisted reproductive techniques must also be advised in these patients whenever necessary.

Amaç: Profilaktik HPV aşılarının ülkemizde de onay alması sonrası klinisyenlerin hizmetinde olacağı bugünlerde kullanım ve toplum bilgilendirilmesi önemli bir görev olarak belirmektedir. Aşının özellikleri ve kullanım önerilerinin yanında serviks kanseri, bu hastalığın özellikleri, serviksin preinvazif hastalıkları, HPV ye bağlı hastalıklar ve kanserdeki rolü tıp mensupları arasında ve toplum bazında önemli bir konu olacaktır. Toplum özellikleri açısından değerlendirildiğinde tıp mensuplarının aileler ve diğer toplum üyelerince yöneltilecek soru ve taleplerle karşı karşıya kalacağı bilinmektedir. Bu derlemenin amacı bütün bu sorularla birlikte serviks kanserinden korunmada aşının etkinliğini ve yerini olası problemlerle birlikte değerlendirmektir. Objective: After the recent approval of prophylactic vaccines for HPV infection in our country, the utilisation of the vaccine and notification of the community have become an important responsibility for the clinicians. The characteristics of the vaccine and suggestions for usage will come up as important topics as well as cervical cancer and the characteristics of the disease itself, preinvasive cervical lesions, the diseases associated with HPV and the role of HPV in cancer. Furthermore, it is not unexpected for the medical professionals to be faced with many questions and demands from families and other members of the society. The aim of this review is to evaluate the efficacy and role of vaccination in the prevention of cervical cancer along with the possible problems.

Enerji talep ve tüketiminin hızla artması, ülkemizin enerji konusunda yüksek oranda dışa bağımlılığı ve enerji tüketimi sonucu ortaya çıkan çevresel faktörler sebebiyle, ülkemizde enerjinin kullanılması konusunda çeşitli düzenlemelerin yapılması gerekliliği ortaya çıkmıştır. Ülkemizde kış şartları göz önüne alındığında binalarda tüketilen enerjinin büyük bir kısmı ısıtma amaçlı kullanılmaktadır. Binalarda enerji tüketimi, yapı elemanlarının optimum kalınlıkta ve uygun yalıtım malzemesi ile yalıtımı, cam ve doğramaların iyileştirilmesi, ve verimli ısıtma sistemleri kullanılmasıyla önemli ölçüde azaltılabilir. Ülkemizde enerji tüketimini minimize eden doğrultuda çözümler sunmak amacıyla 2000 yılında `TS 825 Binalarda Isı Yalıtım Kuralları` yürürlüğe konulmuştur. Isı yalıtımı, binalarda ilk yatırım giderlerini arttıran bir etken olmasına rağmen, enerji tüketiminde ve işletme maliyetlerinde sağlanan tasarruf ile kısa sürede geri dönüşümü sağlamaktadır. Bunun için ısı yalıtımında doğru malzeme seçimi ve optimum yalıtım kalınlığının belirlenmesi önemli bir konudur.Bu tez kapsamında İstanbul Beykoz'da bulunan bir binada P1-P2 ekonomik analiz yöntemi esas alınarak yönlere göre optimum yalıtım kalınlıkları belirlenmiştir. Binanın dört yönünde dış duvarın iç ve dış yüzey sıcaklıkları, dış ortam ve iç ortam sıcaklığı yıl boyunca veri kaydedici aracılığıyla ölçülerek kaydedilmiştir. Yıllık dış duvar iç yüzey, dış duvar dış yüzey, dış ortam sıcaklıkları ölçüm değerleri grafik olarak verilmiştir. Bir sene boyunca binanın tüm cepheler için ölçülen iç yüzey ve dış yüzey sıcaklıkları kullanılarak TS 825 standardına uygun bir model geliştirilmiş ve yönlere göre optimum yalıtım kalınlıklarının belirlenmesi ile beraber enerji tasarrufu ve geri ödeme süreleri hesaplanmıştır. Hesap sonuçlarına göre optimum yalıtım kalınlıkları kuzey, güney, batı doğu ve yönden bağımsız durum için sırasıyla; 6,47 cm, 3 cm, 6,92 cm, 7 cm, 5,25 cm olarak belirlenmiştir. Enerji tasarrufu ve geri ödeme süreleri ise kuzey, batı, doğu ve yönden bağımsız durum için sırasıyla; 89,38 TL/m² ve 1,64 yıl, 74,05 TL/m² ve 2,02 yıl, 73,91 TL/m² ve 2,04 yıl, 68,32 TL/m² ve 2,14 yıl olarak hesaplanmıştır.Anahtar Kelimeler: Bina dış duvar optimum yalıtım kalınlığı, Enerji tasarrufu, geri ödeme süresi. The necessity of the various arrangements emerged for the use of energy in consequences of increasing the energy demand and energy consumption, high percentage of dependency on foreign sources and environmental factors caused by energy consumption in our country.Considering the winter period, given a large part of the energy consumed in buildings used for heating purposes in our country. There are some significant point that buildings energy consumption can be dramatically reduced such as insulation with optimum insulation thickness and appropriate insulation material of building elements, improvement of glazing and joinery and with using of efficient heating systems. In our country, in order to provide solutions that minimize energy consumption, `TS 825 Thermal Insulation Rules in Buildings` was put in to effect in 2000. Thermal insulation in buildings is a factor that increases the initial investment cost although the saving in energy consumption and operation cost it's provide the recycling in a short term period. Thus, choose the right materials for the thermal insulation and the determination of the optimum insulation thickness is an important issue. In this study, optimum insulation thickness is determined for different external wall orientation based on the P1-P2 economic analysis method in a building located in İstanbul, Beykoz. In four aspects of the building, interior and exterior surface temperatures for the outer wall and the ambient and indoor temperatures has been measured by means of the data logger throughout the year. Annual inner surface and outer surface temperature of external wall and ambient-indoor temperature which measured throughout the year is shown as graphics. According to the TS 825 standard, a suitable model was developed using the measured inner surface and outer surface temperature of external wall. Then in developed model, optimum insulation thickness for different wall orientation, energy savings and pay back periods was calculated. As a result, optimum insulation thickness for the orientation of north, south, west, east and situation of independent from orientation were found 6,47 cm, 3 cm, 6,92 cm, 7 cm and 5,25 cm, respectively. Energy savings and pay back periods are also being calculated as 89,38 TL/m² and 1,64 year, 74,05 TL/m² and 2,02 year, 73,91 TL/m² and 2,04 year, 68,32 TL/m² and 2,14 year in north, west, east and situation of independent from orientation, respectively. Apert syndrome is characterized by craniosynostosis, midfacial hypoplasia and symmetric cutaneous and bony syndactily of the limbs. We report a rare case of Apert syndrome with cloverleaf skull deformity, prenatally diagnosed at 20 weeks' gestation in which the ultrasonographic features of a characteristic trilobed skull shape, abnormal biparietal diameter and head circumference, as well as malformations of the all extremities confirmed the diagnosis. Ancak, şu anda, vulvada meme-benzeri bezler olduğu için, memede görülen herhangi bir patolojinin bu bezlerde görülebileceği kabul edilmektedir. Vulvanın adenokarsinomu nadir olup, meme-benzeri adenokarsinomları literatürde olgu sunumları şeklinde yer almaktadır. Vulvar kitle ve kanama şikayetiyle başvuran 70 yaşındaki hastada tanımladığımız vulvanın primer meme benzeri karsinomu olgusunu sunuyoruz. İnsizyonel biyopsi vulvar adenokarsinom olarak raporlandı. İntraoperatif sentinal lenf düğümü incelemesi adenokarsinom metastazı olan hastaya hemivulvektomi ve bilateral inguinal lenfadenektomi uygulandı. Vulvanın adenokarsinomları nadir olup, prognoz ve tedavi primer meme kanseri ile benzerlik gösterdiği için bu olgularda meme benzeri karsinom akılda tutulmalıdır., Breast like carcinomas of the vulva was thought to be originated from ectopic breast tissue or metastasis from breast. It is now accepted that, as mammary-like glands are present in the vulva, any pathology that is found in the breast can be seen at these glands. Adenocarcinoma of the vulva is rare and the breast-like adenocarcinomas are presented as case reports in the literature. We present a primary breast-like carcinoma of the vulva in a 70 year old patient, who had complaint of vulvar mass and bleeding from it. The incisional biopsy was reported as vulvar adenocarcinoma. The intraoperative diagnosis of sentinel lymph node was adenocarcinoma metastasis and hemivulvectomy together with bilateral inguinal lymphadenectomy was carried out.
