Background: This study assessed the prevalence of gastroesophageal reflux disease (GERD) in a general adult population in Iran. The association between GERD and various factors was also evaluated. Methods: We performed a cross-sectional study on 163,018 individuals aged over 35 who were enrolled in the PERSIAN cohort. GERD was defined as the occurrence of heartburn and/or regurgitation symptoms at least several days a month. Survey design analysis for pooled data was performed and multiple regression analysis was conducted to determine the independent risk factors for GERD. Results: The prevalence of GERD in our study was estimated at 21.86% (95% confidence interval:17.4%-36.4%). The mean age of the participants was 49.84 years±9.25 (35–70) and 44.75% of the participants were male. Symptoms of heartburn and regurgitation were reported in 18.65% (n: 29,170) and 6.06% (n: 9,717) of participants, respectively. In the multivariate analysis, several factors were found to be associated with a higher prevalence of GERD: female sex, age >50, current smoking, opium use, weekly consumption of fried foods, frequent consumption of hot tea, less than 6 hours of sleep per night, psychiatric disorders, usage of NSAIDs, and poor oral hygiene, were associated with a higher prevalence of GERD. Conversely, higher education levels and average physical activity were found to be less commonly associated with GERD. Conclusion: We found a relatively high prevalence of GERD (21.86%) in this population-based study in Iran. By identifying modifiable risk factors, this research offers opportunities for targeted interventions and lifestyle modifications to reduce the burden of GERD. [ABSTRACT FROM AUTHOR]

Rheumatoid arthritis (RA) has been linked to an increased risk of osteoporosis as well as fractures. Patients diagnosed with RA had a 25% increased risk of osteoporotic fracture, according to a recent population‐based cohort study that compared them to people without RA. Several studies have found a correlation between osteoporosis and the presence of pro‐inflammatory cytokines, such as tumor necrosis factor (TNF)‐α, interleukin (IL)‐1, and 6. These cytokines play a crucial part in the process of bone resorption by boosting osteoclast activation and encouraging osteoclast differentiation. Based on the correlation between RA, osteoporosis, and inflammation, it is possible that systemic immunosuppression with disease‐modifying antirheumatic drugs (DMARDs) can help individuals with RA have a lower chance of developing osteoporosis and osteoporotic fractures. There is little information on how different DMARDs, biologic or non‐biologic, affect RA patients' bone metabolism. In this study, we present an overview of the influence that targeted therapies, such as biologics, non‐biologics, and small molecule inhibitors, have on bone homeostasis in RA patients. Background and purpose: Vitamin D deficiency is highly prevalent in Iran. Death toll due to the Coronavirus Disease 2019 (COVID-19) is high in this country. The purpose of the present investigation was to provide more information on the relationship between serum levels of vitamin D in confirmed cases of COVID-19 in north of Iran. Materials and methods: This cross-sectional descriptive study was performed in COVID-19 patients (n=80) attending Sari Imam Khomeini Hospital, Iran 2020, who were confirmed for COVID-19 infection with CT-Scan or RT-PCR. Seventy healthy individuals were also examined as the control group. SPSS V19 was applied for data analysis. Results: In this study, 80 patients with COVID-19 (mean age: 57.22 ± 17.44 years old) and 70 healthy individuals (mean age: 48.69 ± 12.99 years) were studied. There were 38 and 35 males in these two groups, respectively. Serum Vitamin D levels were significantly lower in patients with COVID-19 than those in the control group (18.72 ± 12.53 vs. 30.86 ± 15.19 ng/ml, P< 0.001). Conclusion: This study showed that serum levels of Vitamin D were lower in patients with COVID-19 than healthy individuals and frequency of Vitamin D deficiency was higher in these patients. Background: Pregnancy is a phenomenon that antigens of semi allogenic fetus are in direct contact with mother's immune system. Immune dysregulation can cause fetus rejection by mother's immune system responses. Human leukocyte antigen-G1, as an immunotolerant molecule has a major role to induce tolerance during pregnancy by suppression of natural killer cells through inhibitor receptors on these cells. Natural killer cells have an important role in immune surveillance and these cells can be reaction with HLA-G molecules on the trophoblast cells surface. This function prevents natural killer cell invasion against fetus trophoblast cells. The purpose of this study was determination of natural killer cells percent and human leukocyte antigen-G1 expression in peripheral blood of threatened-abortion pregnant women in comparison with control group. Methods: This case-control study was conducted from, February 2014 to October, 2014 in Baghban Clinic in Sari City, Mazandaran province. We investigated 21 threatenedabortion women with light bleeding or spotting less than twenty weeks of pregnancy in comparison with 21 normal pregnant women as control group. Peripheral blood mononuclear cell was isolated by ficoll histopaque (1.077) and natural killer cells percent were evaluated by flow cytometry. Furthermore, we assessed the human leukocyte antigen- G1 isoforms expression by real-time polymerase chain reaction (PCR) in case and control groups. Results: The results of this study was shown that natural killer cells percent in threatened- abortion pregnant women was significantly higher than normal pregnant women (P=0.03). In addition, human leukocyte antigen-G1 isoform had a lower expression in threatened-abortion pregnant women in comparison with control group (P=0.004). Conclusion: Decreasing of human leukocyte antigen-G1 expression with increasing of natural killer cells level in threatened-abortion pregnant women is an indicator of mother's immune system dysregulation in comparison with control group. Therefore, it is concluded that in the threatened-abortion pregnant women, human leukocyte antigen- G1 expression level with natural killer cells percent as diagnostic marker must be determine. Background and purpose: Iron deficiency is the most common nutritional deficiency in both developing and developed Countries. Among its symptoms, pica is the most common and may appear in various forms even before anemia. It causes problems such as poisoning, intestinal problems, damage to the teeth, and parasitic fungal and bacterial infections, etc. The aim of this study was an epidemiological survey of pica disorder in women with iron deficiency and the associated factors. Materials and methods: This was a cross-sectional study in which 260 women with iron deficiency anemia were enrolled during one year based on low hemoglobin, its percent saturation and low ferritin levels. A questionnaire was employed to obtain information such as age, iron supplement use, family history, type of pica, etc. Results: Among the participants, 177 patients (68.1 %) had one or more types of pica and 83 (31.9 %) had none. Moreover, forty four patients (17 %) had pica positive family history. The pica type, from high to low prevalence, consisted of ingesting sugary and starchy materials, ice, curd, plaster, soil, etc. Conclusion: Considering the diversity, relatively high prevalence, serious and dangerous complications caused by pica, early treatment of anemia and promotion of public awareness seems necessary [ABSTRACT FROM AUTHOR]

The article describes the case of a 64 year old lady presented to the rheumatology clinic with sudden onset of painful diffuse bilateral hand arthropathy and complaining of impaired function and burning sensation of the hands and gradually increasing fatigue and mild intermittent pain of hand joints. A clinical picture of the patient was suggestive of systemic amyloidosis. A combination regiment of bortezomib, cyclophosphamide and dexamethasone was started to which she responded favorably.

Objective: We aimed to assess the relationship between major dietary patterns and risk of diabetes type 2 among Iranian adults. Methods: In this population-based case-control study in Mazandaran province, we enrolled 332 subjects (110 newly diagnosed cases and 222 controls) aged 43 - 77 years. Dietary intakes were collected using a validated semi-quantitative food frequency questionnaire (FFQ). Data on demographic, anthropometric, socioeconomic characteristics and other covariates were collected using structured lifestyle questionnaires. Factor analysis was used to identify major dietary patterns. Odds ratios (OR) were calculated for risk of type 2 diabetes across quartiles of dietary pattern scores. Results: Three major dietary patterns were identified, including: "healthy", "transitional" and "traditional". A significant direct association was found between the transitional dietary pattern and risk of diabetes type 2 after adjustment of potential confounders (OR = 2.17; 95% CI: 1.0, 4.50; Ptrend = 0.02). The traditional dietary pattern was significantly associated with the increased risk of diabetes type 2 after controlling for confounders (OR = 2.13; 95% CI: 1.03, 4.41; Ptrend = 0.01). There was no significant relationship between healthy dietary pattern and risk of diabetes type 2. Conclusions: In conclusion, transitional dietary pattern characterized by high consumption of salt, organ meats, dried fruits, poultry, tea, low- fat dairy and other vegetables. Traditional dietary pattern characterized by high intakes of garlic, dough, high- fat dairy, dried fruits, red meats, grains, as well as animal and hydrogenated fats were associated with an increased risk of type 2 diabetes. Organophosphorus agents are easily absorbed via respiratory, gastrointestinal, and dermal routes, and inhibit the acetylcholine transferase enzyme (AChE), which is responsible for the majority of toxicity caused by organophosphates in the body. A comprehensive search was conducted across three prominent databases, namely Google Scholar, PubMed, and Science Direct, to identify relevant articles published. The search focused on the keywords "MgSO4" or "magnesium sulfate" in conjunction with "organophosphate" or "organophosphate poisoning." Inhibition of AChE results in the accumulation of acetylcholine (ACh) in synapses and stimulation of cholinergic receptors. Considering that several studies have shown the use of magnesium sulfate (MgSO4) in inhibiting the release of ACh in the central and peripheral sympathetic and parasympathetic synapses, this study was conducted to review the role of MgSO4 in the treatment of OP. The intravenous administration of MgSO4 exhibits favorable tolerability and clinical efficacy in alleviating cardiac toxicity associated with OP exposure.

Objectives: This study aims to design a conceptual model for the effect of various factors on walking ability, participation, and quality of life in children with spastic diplegic cerebral palsy (SDCP) and test it based on field data using path analysis ., Materials & Methods: This cross-sectional study was performed on 181 children with SDCP. The following were used to measure each of the variables: the Modified Ashworth Scale, the Micro Manual Muscle Tester, the Timed Up and Go Test (TUG), the Boyd and Graham test, the goniometer, weight and height, the Gross Motor Function Classification System, the Life Habits Questionnaire, and cerebral palsy Quality of Life Questionnaire for Children. The structural model was tested in Amos 17., Results: All paths of the proposed model were significant (P <0.05). Among evaluated variables, muscle strength (B = -0.466), balance (B = 0.326), and spasticity (B = 0.143) affected walking ability. Moreover, as an intermediate factor, walking ability affected the subjects' participation (B = -0.819) and quality of life (B = -0.183)., Conclusion: Muscle strength, balance, and spasticity are the most influential factors in the walking ability of children with SDCP. Background: Dendritic cells (DCs) are professional antigen presenting cells that have an important role in the initiation of immune response. The use of maturation factors in dendritic cell differentiation provides a promising approach in immunotherapy., Objective: In this study, we compared tumor necrosis factor-α, polyribocytidylic acid, lipopolysacharide and CpG oligonucleotides in inducing dendritic cell maturation., Methods: We generated immature dendritic cells with GM-CSF in combination with IL-4 from peripheral blood mononuclear adherent cells and used tumor necrosis factor-α, polyribocytidylic acid, lipopolysacharide and CpG for the induction of dendritic cell maturation. CD83 maturation marker on the dendritic cells was analyzed by flowcytometry after 7 days. In addition, mixed leukocyte reaction between dendritic cells and T cells was performed by MTT proliferation assay., Results: Flow cytometry results demonstrated a comparable high level of CD83 expression on the mature dendritic cells generated by TNF-α, CpG, Poly I:C, and LPS treatment of the immature dendritic cells. However, a significantly poorer proliferation of lymphocytes cocultured with the Poly I:C-treated DCs was observed compared to the CpG-treated DCs in mixed leukocyte reaction (p=0.026). Conversely, a significantly stronger proliferation of lymphocytes was observed when cocultured with TNF-α-treated DCs compared to the LPS-treated DCs (p=0.025)., Conclusion: Our results indicated that all of studied maturation inducing factors can be used in DC maturation but TNF-α and CpG were the preferred in vitro maturation factors. It is concluded that maturation of dendritic cells by CpG motif and TNF-α can be used to regulate immune responses.
