Zusammenfassung Hintergrund Seitdem im Juni 2019 Elektro-Scooter als urbanes Fortbewegungsmittel in Hamburg eingeführt wurden, wurde eine hohe Anzahl von Verstößen gegen die geltenden Gesetze im Straßenverkehr (StVG, StGB) bezüglich Alkoholkonsums durch E‑Scooter-Fahrer festgestellt. Ziel der Arbeit Diese Studie hat zum Ziel, einen Überblick über Gefährdung des Straßenverkehrs von E‑Scooter-Fahrern unter Alkoholeinfluss zu erlangen, deren Relevanz hinsichtlich anderer Verkehrsteilnehmergruppen aufzuzeigen und eine erste Zwischenbilanz in Bezug auf deren Frequenz nach eineinhalb Jahren zu ziehen. Material und Methoden Die Daten der durch das Institut für Rechtsmedizin des Universitätsklinikums Hamburg-Eppendorf zwischen dem 15.06.2019 und dem 31.12.2020 hinsichtlich ihrer Blutalkoholkonzentration untersuchten alkoholisierten E‑Scooter-Fahrer (n = 342) wurden bezüglich ihrer demografischen Informationen sowie der ärztlichen Untersuchungsergebnisse retrospektiv ausgewertet und in den Kontext mit der Gesamtzahl der Verstöße gegen die Gesetze im Straßenverkehr mit anschließender Blutalkoholmessung gebracht. Ergebnisse Insgesamt wurden 9,6 % der Gesamtzahl der Verstöße gegen die Gesetze im Straßenverkehr in Verbindung mit anschließender Bestimmung der Blutalkoholkonzentration wurden von E‑Scooter-Fahrern verübt. 87,7 % der Untersuchten waren männlich. Die Blutalkoholkonzentration lag bei 76,9 % der Untersuchten über der für eine absolute Fahruntüchtigkeit beim Benutzen eines Pkw gültigen Grenze von 1,10 ‰. Eine Häufung der Fälle war v. a. in den Nachtstunden sowie an den Wochenenden auffällig. Aufgrund unpräziser Aufzeichnungen ist eine gewisse Dunkelziffer von E‑Scooter-Vorfällen unter den nicht näher bezeichneten Kraftfahrzeugen anzunehmen. Diskussion Da E‑Scooter-Fahrer einen bedeutenden Anteil unter den alkoholisierten Verkehrsteilnehmern einnehmen und die Vorfälle meist nachts und am Wochenende stattfanden, scheinen eine vermehrte Aufklärung über Gefahren bei Nutzung der E‑Scooter unter Alkoholeinfluss und ggf. ein Fahrverbot zu diesen Zeiten sinnvoll zu sein.

BACKGROUND: In the absence of lung biopsy there are various algorithms for the diagnosis of invasive pulmonary aspergillosis (IPA) in intensive care unit (ICU) patients that rely on clinical signs, underlying conditions, radiological features, and mycology. The aim of the present study was to compare four diagnostic algorithms in their ability to differentiate between probable IPA (i.e. requiring treatment) and colonization in critically ill patients. In addition, we determined their diagnostic accuracy to detect proven IPA.METHODS: For this diagnostic accuracy study, we included a mixed ICU population with a positive Aspergillus culture from respiratory secretions that were treated in a German quarternary care hospital between 2005 and 2020. Four different diagnostic algorithms were applied to all patients. We compared agreement among the four algorithms to discriminate probable IPA from colonization. In a subgroup of patients with lung tissue histopathology available we determined the sensitivity and specificity of the single algorithms to detect proven IPA. RESULTS: A total number of 684 critically ill patients were included. The mean age of the study population was 63 years (IQR 52-73). Sixty-nine percent (n=472) were medical and 31% (n=212) were surgical ICU patients. Overall, 79% (n=543) of patients fulfilled the criteria for probable IPA according to at least one diagnostic algorithm. Only 4% of patients (n=29) fulfilled the criteria for probable IPA according to all four algorithms. Agreement among the four diagnostic criteria was low (Cohen’s kappa 0.07-0.29). From 85 patients with histopathological examination of lung tissue 40% (n=34) had confirmed IPA. The new EORTC/MSGERC ICU working group criteria had high specificity (0.59 [0.41-0.75]) and sensitivity (0.73 [0.59-0.85]).CONCLUSIONS:In a cohort of mixed ICU patients, the agreement among four algorithms for the diagnosis of IPA was low. The 2021 EORTC/MSGERC ICU working group criteria achieved the highest specificity and sensitivity to detect proven IPA. Although improved by the latest diagnostic criteria, the discrimination of invasive fungal infection from Aspergillus colonization in critically ill patients remains challenging and may require further optimization.

We investigated the infectivity of 128 severe acute respiratory disease coronavirus 2-associated deaths and evaluated predictive values of standard diagnostic procedures. Maintained infectivity (20%) did not correlate with viral RNA loads but correlated well with anti-S antibody levels. Sensitivity >90% for antigen-detecting rapid diagnostic tests supports their usefulness for assessment.

The aim of this study is to investigate the ability of postmortem computed tomography (PMCT) to distinguish intraperitoneal decomposition gas from pneumoperitoneum due to intestinal perforation. This retrospective study investigated the factors affecting intraperitoneal gas in two groups of 14 decedents as detected by postmortem CT performed in the Department of Legal Medicine of Hamburg University. The first group died with a cause of death associated with intestinal perforation, and the second group with other different natural causes of death. These factors include postmortem interval, gas volume, gas distribution, radiology alteration index (RAI), and pneumoperitoneum-associated pathology. The findings of this study showed the appearance of specific gas distribution patterns and a significant increase in gas volumes in the cases of intestinal perforation. Moreover, postmortem interval and the pneumoperitoneum-associated pathology could help distinguish postmortem-generated gas from pneumoperitoneum. Based on the findings of this study, we propose that these findings can improve the proper detection of intestinal perforation cases in the future.

Understanding the underlying pathology in different tissues and organs is crucial when fighting pandemics like COVID-19. During conventional autopsy, large tissue sample sets of multiple organs can be collected from cadavers. However, direct contact with an infectious corpse is associated with the risk of disease transmission and relatives of the deceased might object to a conventional autopsy. To overcome these drawbacks, we consider minimally invasive autopsies with robotic needle placement as a practical alternative. One challenge in needle based biopsies is avoidance of dense obstacles, including bones or embedded medical devices such as pacemakers. We demonstrate an approach for automated planning and visualising suitable needle insertion points based on computed tomography (CT) scans. Needle paths are modeled by a line between insertion and target point and needle insertion path occlusion from obstacles is determined by using central projections from the biopsy target to the surface of the skin. We project the maximum and minimum CT attenuation, insertion depth, and standard deviation of CT attenuation along the needle path and create two-dimensional intensity-maps projected on the skin. A cost function considering these metrics is introduced and minimized to find an optimal biopsy needle path. Furthermore, we disregard insertion points without sufficient room for needle placement. For visualisation, we display the color-coded cost function so that suitable points for needle insertion become visible. We evaluate our system on 10 post mortem CTs with six biopsy targets in abdomen and thorax annotated by medical experts. For all patients and targets an optimal insertion path is found. The mean distance to the target ranges from (49.9 ± 12.9)mm for the spleen to (90.1 ± 25.8)mm for the pancreas.

Coronavirus disease 19 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has become a global pandemic with significant mortality. Accurate information on the specific circumstances of death and whether patients died from or with SARS-CoV-2 is scarce. To distinguish COVID-19 from non-COVID-19 deaths, we performed a systematic review of 735 SARS-CoV-2-associated deaths in Hamburg, Germany, from March to December 2020, using conventional autopsy, ultrasound-guided minimally invasive autopsy, postmortem computed tomography and medical records. Statistical analyses including multiple logistic regression were used to compare both cohorts. 84.1% (n = 618) were classified as COVID-19 deaths, 6.4% (n = 47) as non-COVID-19 deaths, 9.5% (n = 70) remained unclear. Median age of COVID-19 deaths was 83.0 years, 54.4% were male. In the autopsy group (n = 283), the majority died of pneumonia and/or diffuse alveolar damage (73.6%; n = 187). Thromboses were found in 39.2% (n = 62/158 cases), pulmonary embolism in 22.1% (n = 56/253 cases). In 2020, annual mortality in Hamburg was about 5.5% higher than in the previous 20 years, of which 3.4% (n = 618) represented COVID-19 deaths. Our study highlights the need for mortality surveillance and postmortem examinations. The vast majority of individuals who died directly from SARS-CoV-2 infection were of advanced age and had multiple comorbidities.

Zusammenfassung Hintergrund Im Rahmen der COVID-19-Pandemie sind Inzidenz und Mortalität entscheidende Determinanten, um Ausbreitungsdynamik und Gefahrenpotenzial zu beurteilen. Untersucht wird, ob ein systematisches mikrobiologisches Monitoring von Todesfällen eine relevante Untererfassung der Mortalität aufzeigen kann, und ob sich ggf. eine Sterbekohorte zuvor nicht erfasster Fälle von einer Hellfeldkohorte unterscheidet (Soziografie, Todesursache). Methode Es erfolgte eine systematische Reverse-Transkriptase(RT)-qPCR(quantitative Polymerasekettenreaktion)-Testung von Verstorbenen in zentralen Leichenhallen in Hamburg (Institut für Rechtsmedizin, Krematorium) auf eine SARS-CoV‑2-Infektion mittels Nasen-Rachen-Abstrich über 8 Wochen ab Auftreten pandemiebezogener Todesfälle mit vergleichender Analyse der Hell- und Dunkelfeldkollektive. Ergebnisse Unter insgesamt 1231 verdachtsunabhängig getesteten Verstorbenen lag bei 29 Fällen (2,4 %) eine zuvor nicht bekannte SARS-CoV‑2-Infektion vor. In der ersten Phase der Pandemie überwogen Zufallsentdeckungen unter unklaren und nichtnatürlichen Todesfällen in der Rechtsmedizin, die vermehrt jüngere Altersgruppen, v. a. aus häuslicher Umgebung, umfassten. Im Krematorium zeigten sich mit weiterem Verlauf zunehmend Zufallsentdeckungen bei Todesfällen aus stationären Pflegeinstitutionen. Das Gesamtkollektiv wies soziodemografisch keine signifikanten Unterschiede zu einem Vergleichskollektiv bekannter/registrierter SARS-CoV‑2-assoziierter Todesfälle auf. In der Dunkelfeldkohorte war die Todesursache COVID-19 signifikant seltener. Schlussfolgerung Ein systematisches verdachtsunabhängiges PCR-basiertes Monitoring von Todesfällen ermöglicht eine vollständigere Erfassung von SARS-CoV‑2-positiven Sterbefällen insbesondere im nichtklinischen Sektor. Durch die Erfassung eines Dunkelfelds, das einer Routineleichenschau bislang entgeht, kann ein präventiver epidemiologischer Beitrag geleistet werden.

Due to the development of novel functionalities, distinct SARS-CoV-2 variants such as B.1.1.7 fuel the current pandemic. B.1.1.7 is not only more transmissible, but may also cause an increased mortality compared to previous SARS-CoV-2 variants. Human tissue analysis of the SARS-CoV-2 lineage B.1.1.7 is urgently needed, and we here present autopsy data from 7 consecutive SARS-CoV-2 B.1.1.7 cases. The initial RT-qPCR analyses from nasopharyngeal swabs taken post mortem included typing assays for B.1.1.7. We quantitated SARS-CoV-2 B.1.1.7 viral load in autopsy tissue of multiple organs. Highest levels of SARS-CoV-2 B.1.1.7 copies normalized to ß-globin were detected in the respiratory system (lung and pharynx), followed by the liver and heart. Importantly, SARS-CoV-2 lineage B.1.1.7 was found in 100% of cases in the lungs and in 85.7% in pharynx tissue. Detection also in the kidney and brain highlighting a pronounced organ tropism. Comparison of the given results to a former cohort of SARS-CoV-2 deaths during the first wave in spring 2020 showed resembling organ tropism. Our results indicate that also SARS-CoV-2 B.1.1.7 has a relevant organ tropism beyond the respiratory tract. We speculate that B.1.1.7 spike protein’s affinity to human ACE2 facilitates transmission, organ tropism, and ultimately morbidity and mortality. Further studies and larger cohorts are obligatory to proof this link.

Background The new coronavirus, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has caused more than 210 000 deaths worldwide. However, little is known about the causes of death and the virus's pathologic features. Objective To validate and compare clinical findings with data from medical autopsy, virtual autopsy, and virologic tests. Design Prospective cohort study. Setting Autopsies performed at a single academic medical center, as mandated by the German federal state of Hamburg for patients dying with a polymerase chain reaction-confirmed diagnosis of COVID-19. Patients The first 12 consecutive COVID-19-positive deaths. Measurements Complete autopsy, including postmortem computed tomography and histopathologic and virologic analysis, was performed. Clinical data and medical course were evaluated. Results Median patient age was 73 years (range, 52 to 87 years), 75% of patients were male, and death occurred in the hospital (n = 10) or outpatient sector (n = 2). Coronary heart disease and asthma or chronic obstructive pulmonary disease were the most common comorbid conditions (50% and 25%, respectively). Autopsy revealed deep venous thrombosis in 7 of 12 patients (58%) in whom venous thromboembolism was not suspected before death; pulmonary embolism was the direct cause of death in 4 patients. Postmortem computed tomography revealed reticular infiltration of the lungs with severe bilateral, dense consolidation, whereas histomorphologically diffuse alveolar damage was seen in 8 patients. In all patients, SARS-CoV-2 RNA was detected in the lung at high concentrations; viremia in 6 of 10 and 5 of 12 patients demonstrated high viral RNA titers in the liver, kidney, or heart. Limitation Limited sample size. Conclusion The high incidence of thromboembolic events suggests an important role of COVID-19-induced coagulopathy. Further studies are needed to investigate the molecular mechanism and overall clinical incidence of COVID-19-related death, as well as possible therapeutic interventions to reduce it. The body as a whole, along with individual organs and tissues, is the source for many examinations and measures relating to both teaching and research in medicine. There is no possibility of having an alternative model for the body in the near future. When it comes to organ and tissue donation activities, the human body gives other people another chance in life and indicates a better quality of life for many. Therefore, interventions on the deceased are carried out at the Institute of Legal Medicine in Hamburg (with the declaration of consent being taken from the person during lifetime or the relatives) for teaching, research, and clinical practices other than judicial autopsies. This includes scientific investigations in clinical research projects and surgical interventions within the framework of the Post-mortem Learning Centre and tissue donation, especially cornea transplantation in Germany and abroad. For clinical research projects, especially regarding quality assurance and innovation, post-mortem procedures on corpses are increasingly becoming the focus of attention.

Die durch das neue Coronavirus (SARS-CoV-2) bedingte Erkrankung Coronavirus-Krankheit 2019 (COVID-19) stellt eine Gefahrdung fur Altere dar. Ende Marz 2020 kam es zu einem ersten, bislang grosten Infektionsausbruch in einem Seniorenpflegewohnheim in Hamburg. Analyse von Verfahrensablaufen in einer von SARS-Coronavirus-2 (SARS-CoV‑2) betroffenen Wohneinheit mit 60 demenziell vorerkrankten Senioren sowie umfassende Darstellung der Obduktionsbefunde SARS-CoV-2-positiv getesteter dortiger Sterbefalle. In 39 von 60 Fallen infizierten sich Pflegeheimbewohner mit SARS-CoV‑2. Eine Ausbreitung der Infektionswelle auf andere Wohnbereiche lies sich verhindern. In allen 8 Sterbefallen wurde durch die Obduktion die Diagnose eines COVID-19-bedingten Todes gestellt. Autoptisch fanden sich eine todesursachliche (Broncho‑)Pneumonie sowie Korrelate relevanter kardialer, renaler und pulmonaler Vorerkrankungen in allen Fallen. In 75 % (n = 6) der Falle waren frische Unterschenkelvenenthrombosen, in 66,7 % (n = 4) kombiniert mit peripheren Lungenarterienthromboembolien, nachweisbar. Das im deutschsprachigen Raum erstmals systematisch beschriebene Kollektiv von SARS-CoV-2-Infizierten in einer Pflegeeinrichtung ist charakteristisch fur die bislang bekannten klinischen und epidemiologischen Besonderheiten der neuen Coronainfektion. Durch die zentralisierte Evaluation aller Todesfalle in Hamburg war eine gezielte Aufarbeitung der SARS-CoV-2-positiven Sterbefalle unter den betroffenen Heimbewohnern moglich. Der deutlich erhohten Letalitat von ca. 20 % konnten in allen Fallen relevante Komorbiditaten multipler Organsysteme zugeordnet werden – die mit den klinischen Daten im Einklang standen.

Male sex belongs to one of the major risk factors for severe COVID-19 outcome. However, underlying mechanisms that could affect sex dependent disease outcome are yet unknown. Here, we identified the CYP19A1 gene encoding for the testosterone-to-estradiol metabolizing enzyme CYP19A1 (alias aromatase) as a male abundant host factor that contributes to worsened disease outcome in SARS-CoV-2 infected male hamsters. Pulmonary CYP19A1 transcription is further elevated upon viral infection in males correlating with reduced testosterone and increased estradiol levels. Dysregulated circulating sex hormone levels in male golden hamsters are associated with reduced lung function compared to females. Treatment of SARS-CoV-2 infected hamsters with letrozole, a clinically approved CYP19A1 inhibitor, supported recovery of dysregulated plasma sex hormone levels and was associated with improved lung function and health in male but not female animals compared to placebo controls. Whole human exome sequencing data analysis using a Machine Learning approach revealed a CYP19A1 activity increasing mutation being associated with the development of severe COVID-19 for men. In human autopsy-derived lungs CYP19A1 was expressed to higher levels in men who died of COVID-19, at a time point when most viral RNA was cleared. Our findings highlight the role of the lung as a yet unrecognized but critical organ regulating metabolic responses upon respiratory virus infection. Furthermore, inhibition of CYP19A1 by the clinically approved drug letrozole may pose a new therapeutic strategy to reduce poor long-term COVID-19 outcome.

Objective: Soft tissue deformation and ruptures complicate needle placement. However, ruptures at tissue interfaces also contain information which helps physicians to navigate through different layers. This navigation task can be challenging, whenever ultrasound (US) image guidance is hard to align and externally sensed forces are superimposed by friction. Methods: We propose an experimental setup for reproducible needle insertions, applying optical coherence tomography (OCT) directly at the needle tip as well as external US and force measurements. Processing the complex OCT data is challenging as the penetration depth is limited and the data can be difficult to interpret. Using a machine learning approach, we show that ruptures can be detected in the complex OCT data without additional external guidance or measurements after training with multi-modal ground-truth from US and force. Results: We can detect ruptures with accuracies of 0.94 and 0.91 on homogeneous and inhomogeneous phantoms, respectively, and 0.71 for ex-situ tissues. Conclusion: We propose an experimental setup and deep learning based rupture detection for the complex OCT data in front of the needle tip, even in deeper tissue structures without the need for US or force sensor guiding. Significance: This study promises a suitable approach to complement a robust robotic needle placement.

Post-mortem computed tomography (PMCT) is a rapid and noninvasive diagnostic tool for important contributions to the identification of pulmonary findings in the deceased with pneumonia, including cases of coronavirus disease 2019 (COVID-19). Although computed tomography (CT) shows a high sensitivity for pneumonia in living persons, it is relatively unspecific for COVID-19 pneumonia clinically. Typical CT findings for viral pneumonia therefore require confirmation by PCR tests (polymerase chain reaction tests), even if lung infections with severe acute respiratory syndrome coronavirus type 2 (SARS-CoV-2) show characteristic patterns, most frequently ground glass opacities (GGO) and a combination of GGO and air space consolidations. In the consecutive autopsy series of SARS-CoV‑2 deaths from Hamburg, Germany, the most frequent cause of death was and still is COVID-19 pneumonia. Typical findings were frequently found in the PMCT in SARS-CoV-2-associated deaths, which were taken into account when classifying the death as COVID-19.Die Post-mortem-Computertomographie (PMCT) ist ein schnelles und nicht-invasives Diagnoseinstrument, das einen wichtigen Beitrag zur Identifizierung von Lungenbefunden bei Verstorbenen mit Pneumonie leistet – auch bei Fällen der Coronaviruskrankheit (COVID -19). Obwohl die Computertomografie (CT) eine hohe Sensitivität für Lungenentzündungen bei lebenden Personen aufweist, ist sie für eine COVID-19-Pneumonie klinisch recht unspezifisch. Typische CT-Befunde für virale Pneumonien bedürfen daher der Bestätigung durch PCR-Tests (Polymerase-Ketten-Reaktion-Tests), auch wenn die Lungeninfektion mit SARS-CoV‑2 charakteristische Muster zeigt – am häufigsten Milchglastrübungen (GGO) und eine Kombination aus GGO und Konsolidierungen.In der konsekutiven Autopsieserie von SARS-CoV-2-Todesfällen aus Hamburg war und ist die häufigste Todesursache die COVID-19-Pneumonie. In der PMCT fanden sich bei SARS-CoV-2-assoziierten Todesfällen häufig typische Befunde, die bei der Klassifizierung des Todes als COVID-19 berücksichtigt wurden.

Analyses of infection chains have demonstrated that severe acute respiratory syndrome coronavirus 2 is highly transmissive. However, data on postmortem stability and infectivity are lacking. Our finding of nasopharyngeal viral RNA stability in 79 corpses showed no time-dependent decrease. Maintained infectivity is supported by virus isolation up to 35 hours postmortem.

The body of a deceased with Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) infection is considered infectious. In this study, we present the results of infectivity testing of the body and testing of mortuary staff for SARS-CoV-2. We performed real-time quantitative polymerase chain reaction (RT-qPCR) for SARS-CoV-2 on 33 decedents with ante mortem confirmed SARS-CoV-2 infection. Swabs of the body surface from five different body regions and from the body bag or coffin were examined. A subset of the swabs was brought into cell culture. In addition, screening of 25 Institute of Legal Medicine (ILM) personnel for ongoing or past SARS-CoV-2 infection was performed at two different time points during the pandemic. Swabs from all locations of the body surface and the body environment were negative in cases of negative post mortem nasopharyngeal testing (n=9). When the post mortem nasopharyngeal swab tested positive (n=24), between 0 and 5 of the body surface swabs were also positive, primarily the perioral region. In six of the cases, the body bag also yielded a positive result. The longest postmortem interval with positive SARS-CoV-2 RT-qPCR at the body surface was nine days. In no case viable SARS-CoV-2 was found on the skin of the bodies or the body bags. One employee (autopsy technician) had possible occupational infection with SARS-CoV-2; all other employees were tested negative for SARS-CoV-2 RNA or antibody twice. Our data indicate that with adequate management of general safety precautions, transmission of SARS-CoV-2 through autopsies and handling of bodies is unlikely.

Cocain gehort in Europa zu den am haufigsten konsumierten Stimulanzien. Cocainassoziierte Todesfalle sind weit uberwiegend Mischintoxikationen mit Opiatbeteiligung oder im Rahmen von Traumamechanismen unter Einwirkung von Cocain indirekt substanzbezogen. Der toxikologische Stellenwert des Cocains fur die Todesursachlichkeit ist deshalb oft schwer zu bestimmen. Es sollte gepruft werden, ob die Cocainkonsumintensitat und das Auftreten kardialer pathologischer Veranderungen bezuglich des Todesmechanismus im Zusammenhang stehen. Es wurde die Pravalenz autoptisch/histologisch festgestellter kardialer pathologischer Veranderungen bei Drogentodesfallen mit positivem Cocainnachweis in der Haaranalyse (n = 195, davon n = 43 mit histologisch nachgewiesener kardialer Storung) korreliert mit quantitativen Unterschieden der Cocain‑/Benzoylecgoninkonzentrationen in den Haarproben bzw. gegenuber einer Vergleichsgruppe (n = 151). Bei Verstorbenen zwischen 20 und 40 Jahren zeigten sich zwischen Untersuchungs- und Vergleichskollektiv keine signifikanten Unterschiede der Herzgewichte. Es fanden sich signifikante Differenzen zwischen Herzgewichten, die gemas dem ermittelten Body-Mass-Index (BMI) erwartet wurden, und den tatsachlich gemessenen. Besonders hohe Cocain‑/Benzoylecgonin-Haar-Konzentrationen (>10 ng/mg) waren jedoch nicht signifikant haufiger mit Epi‑, Peri‑, Myokarditis, Koronarsklerose oder Myokardhypertrophie assoziiert. Die relative Bewertung der Kausalitat eines Cocainkonsums bei Todesfallen ist nur mithilfe einer interdisziplinaren Fallanalyse moglich. Diese hat unter Hinzuziehung aller diagnostischen Optionen zu erfolgen, mit deren Hilfe der Einfluss eines akuten/chronischen Cocainkonsums auf kardiale und zentralnervose Funktionen eingeschatzt werden kann.

Objectives Conventional autopsies remain the gold standard of postmortem healthcare quality assurance and help gathering extended knowledge on diseases. In answer to constantly declining autopsy rates non- or minimally invasive autopsy methods were introduced. Ultrasound is a well-established tool for imaging commonly used in clinical practice. This narrative review aims to summarize the current literature regarding the feasibility and validity of ultrasound in a forensic context. Material and methods A PubMed database search was carried out. Abstracts were scanned for pre-defined ex- and inclusion criteria, followed by a snowball search procedure applied to the primarily included articles. Results Forty-five publications met our inclusion criteria. The selected articles concern the feasibility of ultrasound in pre- or postmortem settings, forensic age estimation, and minimally invasive approaches. For imaging, ultrasound was deemed a reliable tool for the examination of epiphyses und superficial wounds, with limitations regarding internal organs and image quality due to postmortem changes. Ultrasound-guided minimally invasive approaches yielded higher success rates for adequate tissue sampling. Many investigations were carried out in low- and middle-income countries focusing on infectious diseases. Conclusion Ultrasound seems a promising but underutilized imaging tool in legal medicine to date. Promising approaches on its feasibility have been conducted. Especially for minimally invasive methods, ultrasound offered significant improvements on qualified biopsy sampling and thus appropriate diagnostics. Moreover, ultrasonic evaluation of epiphyses for age estimation offered valuable results. Nevertheless, further assessment of ultrasonic feasibility in forensic contexts is needed.

A detailed knowledge on the exact morphology of pelvic injuries provided crucial information in understanding the mechanisms of injury and has influence on the natural course and subsequent mortality. However, forensic medical literature investigating pelvic fractures in detail is scarce to date. This case series aims to compare the accuracy in detecting pelvic injuries using autopsy and ante mortem computed tomography (CT). Nineteen deceased patients with CT scans of pelvic fractures were included retrospectively. Pelvic injuries were independently assessed by a board-certified radiologist (R) and a board-certified trauma surgeon (T), both using the ante mortem CT scans, and by a board-certified forensic pathologist using autopsy (A) results without knowledge of the CT scan findings. No patient had died causatively from a pelvic fracture. Most injuries of the pelvis were present in the pubic rami (16/18) and sacral bone (13/18), followed by the sacroiliac joint (9/18) and iliac bone fractures (8/18). Ilium fractures (A:100%;R:67%;T:67%) and injuries of the sacroiliac joint (A:83%;R:50%;T:42%) were best detected via autopsy. The diagnosis of sacral fractures (A:19%;R:94%;T:88%) and fractures of the pubic rami (A:67%;R:96%;T:96%) were most often missed in autopsy. The results show deficits in the assessment of the pelvic injury for both CT and autopsy. Autopsy was superior in detecting injuries of the sacroiliac joint, but inferior in detecting sacral and pubic bone fractures. For an encompassing evaluation of ligamento-skeletal pelvic injuries, the complementary use of both CT and autopsy is recommended.

Background and ObjectiveCapturing the population variability of bone properties is of paramount importance to biomedical engineering. The aim of the present paper is to describe variability and correlations in bone mineral density with a spatial random field inferred from routine computed tomography data.MethodsRandom fields were simulated by transforming pairwise uncorrelated Gaussian random variables into correlated variables through the spectral decomposition of an age-detrended correlation matrix. The validity of the random field model was demonstrated in the spatiotemporal analysis ofbone mineral density. The similarity between the computed tomography samples and those generated via random fields was analyzed with theenergy distancemetric.ResultsThe random field of bone mineral density was found to be approximately Gaussian/slightly left-skewed/strongly right-skewed at various locations. However, average bone density could be simulated well with the proposed Gaussian random field for which the energy distance, i.e., a measure that quantifies discrepancies between two distribution functions, is convergent with respect to the number of correlation eigenpairs.ConclusionsThe proposed random field model allows the enhancement of computational biomechanical models with variability in bone mineral density, which could increase the usability of the model and provides a step forward inin-silicomedicine.

Summary Background Prominent clinical symptoms of COVID-19 include CNS manifestations. However, it is unclear whether severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), the causative agent of COVID-19, gains access to the CNS and whether it causes neuropathological changes. We investigated the brain tissue of patients who died from COVID-19 for glial responses, inflammatory changes, and the presence of SARS-CoV-2 in the CNS. Methods In this post-mortem case series, we investigated the neuropathological features in the brains of patients who died between March 13 and April 24, 2020, in Hamburg, Germany. Inclusion criteria comprised a positive test for SARS-CoV-2 by quantitative RT-PCR (qRT-PCR) and availability of adequate samples. We did a neuropathological workup including histological staining and immunohistochemical staining for activated astrocytes, activated microglia, and cytotoxic T lymphocytes in the olfactory bulb, basal ganglia, brainstem, and cerebellum. Additionally, we investigated the presence and localisation of SARS-CoV-2 by qRT-PCR and by immunohistochemistry in selected patients and brain regions. Findings 43 patients were included in our study. Patients died in hospitals, nursing homes, or at home, and were aged between 51 years and 94 years (median 76 years [IQR 70–86]). We detected fresh territorial ischaemic lesions in six (14%) patients. 37 (86%) patients had astrogliosis in all assessed regions. Activation of microglia and infiltration by cytotoxic T lymphocytes was most pronounced in the brainstem and cerebellum, and meningeal cytotoxic T lymphocyte infiltration was seen in 34 (79%) patients. SARS-CoV-2 could be detected in the brains of 21 (53%) of 40 examined patients, with SARS-CoV-2 viral proteins found in cranial nerves originating from the lower brainstem and in isolated cells of the brainstem. The presence of SARS-CoV-2 in the CNS was not associated with the severity of neuropathological changes. Interpretation In general, neuropathological changes in patients with COVID-19 seem to be mild, with pronounced neuroinflammatory changes in the brainstem being the most common finding. There was no evidence for CNS damage directly caused by SARS-CoV-2. The generalisability of these findings needs to be validated in future studies as the number of cases and availability of clinical data were low and no age-matched and sex-matched controls were included. PurposeFeasibility of a highly reduced-dose lumbar spine CT protocol using iterative reconstruction (IR) in a human cadaver model.Materials and methodsThe lumbar spine of 20 human cadavers was repeatedly examined using three different reduced-dose protocols (RDCT) with decreasing reference tube current-exposure time products (RDCT-1: 50 mAs; RDCT-2: 30 mAs; RDCT-3: 10 mAs) at a constant tube voltage of 140 kV. A clinical standard-dose protocol (SDCT) served as the reference (reference tube current-exposure time product: 70 mAs; tube voltage: 140 kV). Images were reconstructed using filtered back projection (FBP) and two increasing levels of IR: IRL4 and IRL6. A five-point scale was used by two observers to assess the diagnostic quality of anatomical structures (cortical and trabecular bone, intervertebral foramina, pedicles and intervertebral joints, spinous and transverse processes). Objective image noise (OIN) was measured. Results were interpreted using a linear mixed-effects regression model.ResultsRDCT-2 with IRL6 (1.2 ± 0.5mSv) was the lowest reduced-dose protocol which provided diagnostically acceptable and equivalent image quality compared to the SDCT (2.3 ± 1.1mSV) with FBP (p > 0.05). All RDCT protocols achieved a significant reduction of the mean (±SD) effective radiation doses (RDCT-1: 1.7±0.9mSv; RDCT-2: 1.2±0.5mSv; RDCT-3: 0.4±0.2mSv; p < 0.05) compared to SDCT. OIN was lower in all RDCT protocols with the application of IRL4 and IRL6, compared to the SDCT with FBP (p < 0.05).ConclusionHighly reduced-dose lumbar spine CT providing diagnostically acceptable image quality is feasible using IR in this cadaver model and may be transferred into a clinical setting.

Clinical characterization of COVID-19 (Corona Virus Disease 2019) is being performed worldwide to gain insights into the pathogenesis and course of the disease. Little is known regarding morphological findings, which are essential to understanding the unique features and pathomechanisms of the disease, from which one can identify possible new treatments. It has been shown that diffuse alveolar damage, signifying acute respiratory distress syndrome, is present together with atypical multinucleated cells in reported cases of the disease by Tian et al. (J Thorac Oncol 15:700-704, 2020). Macroscopic morphological findings in COVID-19 remain elusive to this day. Here, we report the case of the first German to die due to COVID-19. A detailed examination consisting of full-body computed tomography, autopsy, histology assessment, and viral assessment has been performed. The lungs of the deceased contained high concentrations of SARS-CoV-2 RNA and displayed the typical radiological signatures of COVID-19. Furthermore, a morphological pattern was found displaying hyperaemic areas interspersed by normally perfused areas affecting the whole lung. We also report a finding suggestive of micro-thrombotic events in the lung, which is compatible with the recently described coagulation changes and increased incidence of pulmonary artery embolisms seen in COVID-19 patients as reported by Wichmann et al. (Ann Intern Med, 2020). A broader study is needed to confirm these findings.

Furthermore, autopsies are essential for the correct statistical recording of the coronavirus disease 2019 (COVID-19) deaths. In the northern German Federal State of Hamburg, all deaths of Hamburg citizens with ante- or postmortem PCR-confirmed SARS-CoV-2 infection have been autopsied since the outbreak of the pandemic in Germany. Our evaluation provides a systematic overview of the first 80 consecutive full autopsies. A proposal for the categorisation of deaths with SARS-CoV-2 infection is presented (category 1: definite COVID-19 death; category 2: probable COVID-19 death; category 3: possible COVID-19 death with an equal alternative cause of death; category 4: SARS-CoV-2 detection with cause of death not associated to COVID-19). In six cases, SARS-CoV-2 infection was diagnosed postmortem by a positive PCR test in a nasopharyngeal or lung tissue swab. In the other 74 cases, SARS-CoV-2 infection had already been known antemortem. The deceased were aged between 52 and 96 years (average 79.2 years, median 82.4 years). In the study cohort, 34 deceased were female (38%) and 46 male (62%). Overall, 38% of the deceased were overweight or obese. All deceased, except for two women, in whom no significant pre-existing conditions were found autoptically, had relevant comorbidities (in descending order of frequency): (1) diseases of the cardiovascular system, (2) lung diseases, (3) central nervous system diseases, (4) kidney diseases, and (5) diabetes mellitus. A total of 76 cases (95%) were classified as COVID-19 deaths, corresponding to categories 1–3. Four deaths (5%) were defined as non-COVID-19 deaths with virus-independent causes of death. In eight cases, pneumonia was combined with a fulminant pulmonary artery embolism. Peripheral pulmonary artery embolisms were found in nine other cases. Overall, deep vein thrombosis has been found in 40% of the cases. This study provides the largest overview of autopsies of SARS-CoV-2-infected patients presented so far.

Forensic autopsies include a thorough examination of the corpse to detect the source or alleged manner of death as well as to estimate the time since death. However, a full autopsy may be not feasible due to limited time, cost or ethical objections by relatives. Hence, we propose an automated minimal invasive needle biopsy system with a robotic arm, which does not require any online calibrations during a procedure. The proposed system can be easily integrated into the workflow of a forensic biopsy since the robot can be flexibly positioned relative to the corpse. With our proposed system, we performed needle insertions into wax phantoms and livers of two corpses and achieved an accuracy of 4.34 ± 1.27 mm and 10.81 ± 4.44 mm respectively.

Background According to the as low as reasonably achievable (ALARA) principle, radiation exposure in computed tomography (CT) should be minimized while maintaining adequate image quality. Dedicated CT protocols combined with iterative reconstruction (IR) can reduce radiation dose and/or improve image quality. Purpose To investigate the feasibility of extremely reduced-dose (RD) CT of the thoracic spine in human cadavers using a standard-dose (SD) and three different RDCT protocols reconstructed with filtered back projection (FBP) and IR. Material and Methods The thoracic spines of 11 cadavers were examined using different RDCT protocols with decreasing reference tube currents (RDCT-1: 50 mAs; RDCT-2: 30 mAs; RDCT-3: 10 mAs) at 140 kV. A clinical SDCT (70 mAs, 140 kV) served as reference. Raw data were reconstructed using FBP and two increasing levels of IR (IRL4 and IRL6). Images were evaluated for image quality, diagnostic acceptability, and visibility of anatomical structures according to a 5-point-scale. Results Regardless of the reconstruction technique, image quality was rated as diagnostically acceptable for all cadavers in SDCT and RDCT-1. Image quality of reconstructions with FBP were generally rated lower. Application of IR improved image quality ratings in SDCT and RDCT. RDCT-2 with IR was the most reduced-dose CT protocol which enabled diagnostically acceptable image quality in all cadavers. Compared to SDCT, RDCT protocols resulted in significantly reduced effective radiation doses (SDCT: 4.1 ± 1.5 mSv; RDCT-1: 2.9 ± 1.1 mSv; 2:1.7 ± 0.6 mSv; 3:0.6 ± 0.1 mSv; P = 0.001). Conclusion Diagnostically acceptable RDCT of the thoracic spine with 1.7 mSv is feasible using IR.
