Objective: To analyze nurses' perception of autonomy to exercise their practices in the context of primary health care. Methods: A descriptive study with a qualitative approach that analyzed 108 interviews carried out with nurses from the four capitals of the southeastern region of Brazil. The interviews, guided by a semi-structured script, were recorded and transcribed. The data produced were processed and explored with the help of the NVIVO® software. Results: Most of the participants were women, white, lived in the same city where they work, and graduated from private institutions. Two categories were organized: repercussions of autonomy and its consequences for solving users' health needs; and (lack of) knowledge about the regulation of nursing practices: challenges for autonomy. Conclusion: Nurses understand the importance of autonomy for their practices, but they face interference in their daily work, either by management, the structure of services or the need for protocols that guarantee the exercise of their professional activity. Analyze in the scientific literature what has been produced about the role of nurses in the treatment of chronic wounds. Literature review with a qualitative approach, carried out in the databases Scientific Electronic Library Online (SCIELO), Latin American and Caribbean Literature in Health Sciences (LILACS), Medical Literature Analysis and Retrieval System Online (Medline), Nursing Database (BDEnf). Ten articles were part of the sam-ple. We found technical weaknesses and general knowledge about the prevention, treatment and recovery of chronic wounds linked to academic training and some challenges for professionals to preserve their autonomy. If it is necessary to train nurses to make decisions, carry out procedures and manage their team, this preparation should start during graduation, making the professional develop skills to deal with the management of chronic skin lesions and improve critical capacity reflective. El proceso de digitalización es un fenómeno consolidado en los sistemas educativos contemporáneos. En su fase actual, este se caracteriza por el despliegue de la denominada “plataformización educativa”, escena liderada por las grandes corporaciones tecnológicas o big tech. En consideración de este contexto, el objetivo del artículo es explorar algunas de las tensiones y nudos críticos que dicho proceso entraña para la profesionalidad docente, particularmente en uno de sus ejes estructurantes: la autonomía profesional. Para dar cuenta de ello, se utilizan los registros recabados en una investigación cualitativa realizada en seis establecimientos educativos de Barcelona. Del análisis de las voces de los equipos directivos y del profesorado emergen tres grandes dimensiones de tensión: i) la necesidad de una alfabetización digital; ii) el monocultivo de las plataformas y su naturalización; y iii) los limitados márgenes de autonomía en el uso de herramientas digitales para la interacción pedagógica. Se concluye vinculando estas dimensiones con una reflexión en torno a la necesidad de desarrollar una alfabetización digital crítica en los contextos educativos.

Este artículo asume como objetivo el de proponer ejes de análisis para la investigación sobre autonomía profesional periodística en el contexto cubano. Se argumenta que, si bien la investigación sobre periodismo en Cuba ha abordado a la autonomía profesional de manera tangencial fundamentalmente, se hace necesario asumir a esta como objeto de estudio central en indagaciones orientadas al análisis de los medios, ante las evidentes afectaciones y reconfiguraciones de la prensa de la Isla. Se partió de la investigación bibliográfico-documental sobre textos asociados a tres áreas: sociología de las profesiones y conceptualización de autonomía profesional, indagaciones sobre autonomía profesional periodística y aproximaciones a la autonomía profesional periodística en Cuba. Se atendió, además, a entrevistas realizadas a especialistas. Se proponen cuatro ejes de análisis para la investigación futura: a) conceptualización de la autonomía profesional, b) atención a las percepciones de los periodistas en torno al modo en que se concretan las libertades en su ejercicio profesional, c) atención a las condiciones que atraviesan el desempeño de los periodistas de medios oficiales con posibilidad de incidir en la autonomía profesional, y d) atención a las condiciones que atraviesan el desempeño de los periodistas de medios independientes de la estructura mediático- partidista con posibilidad de incidir en la autonomía profesional.

Introduction: chronic kidney disease is a pathology that reduces the expectancy and quality of life of patients. Doctors are primarily responsible for preserving the patient's life and are responsible for shared decision making. Objective: to analyze the perception of medical personnel regarding the prolongation of renal replacement treatment in patients with chronic kidney disease in the terminal phase in the dialysis unit at the General Teaching Hospital of Ambato, Ecuador. Method: a qualitative research was carried out, with a semi-structured interview that consisted of questions guided by the theory-hypothesis of the topic. The study sample was made up of doctors from the Dialysis Unit, responsible for the clinical management of patients with chronic kidney disease undergoing dialysis treatment. The protocol was approved by the Human Research Ethics Committee of the Pontificia Universidad Católica del Ecuador, with approval code CEI-88-2020. Results: the thematic analysis showed that doctors need to comprehensively ethically and clinically evaluate the patient before maintaining or terminating hemodialysis; The level of knowledge and ethical-clinical skills influence decision making. Furthermore, the application of Beauchamp and Childress' bioethical principles to decision making is vague, morbidly confusing, and therefore irrelevant to this type of decision. Conclusions: doctors perceive the need to comprehensively evaluate the patient, taking into account not only the physical but also the psychological, social and economic condition of the patient. They consider it unnecessary to maintain hemodialysis treatment in a patient with a deteriorated quality of life, with reduced autonomy and whose prolonged treatment could cause more pain than benefit.

Introducción. Al declararse la pandemia por SARS-CoV-2, se establecieron múltiples cambios en los sistemas de salud y en las instituciones hospitalarias, influyendo en la actividad quirúrgica global. El objetivo de este estudio fue evaluar el impacto de la pandemia en los niveles de autonomía y supervisión operatorias de los residentes de cirugía. Métodos. Estudio analítico cuasi-experimental, que incluyó los procedimientos quirúrgicos registrados por residentes de cirugía general de la Universidad de La Sabana, de febrero de 2019 a agosto de 2021. Se analizaron la autonomía y la supervisión mediante la escala Zwisch en los periodos prepandemia y pandemia. Resultados. Se recolectaron datos de 10.618 procedimientos en el periodo establecido, la mayoría realizados con abordaje abierto (57,4 %) y en rotaciones tronculares de cirugía general (65 %). Los procedimientos realizados más frecuentes fueron apendicectomía (18,6 %), colecistectomía (18,4 %) y herniorrafías (8,6 %). The emancipatory pattern has allowed the foundation and development of the nursing discipline, towards a way of thinking from critical theory. The purpose of the study is to reflect on the possibility that the nursing professional has to develop through the personal pattern a strategy for the emancipation of consciousness in heteronormative-patriarchal structures within the control state apparatuses (health institutions) where technification, medicalization and pathologization of life is above self-care and health care as a human experience. To address the issue, the analytical category of Gender was used in a transversal way, which allows to make visible the power relations that generate inequalities based on sexual difference. Then, it is understood that all things, people or professions perceived as feminine (as in the case of the nursing profession) are subordinated and oppressed compared to those perceived as masculine. Caring and knowledge patterns for self-care are proposed as resistance to inequalities generated by the integration of the biomedical-heteropatriarchal model in nursing thought and practice. Objetivo: Identificar la influencia del sistema de salud colombiano sobre la autonomía profesional de odontólogos que ejercen en IPS públicas y privadas de Bogotá. Metodología: Estudio cualitativo comprensivo, llevado a cabo con herramientas etnográficas como la entrevista semiestructurada. Las categorías de análisis fueron: autonomía profesional, modelos de atención, toma de decisiones y sistema contractual. La información se analizó de manera categorial, contextual y de contenido. Resultados: Se evidenció que la autonomía profesional está limitada por el tiempo de atención para los pacientes (veinte minutos). Otra limitante es la restricción en la realización de procedimientos y, en ocasiones, los protocolos de atención. Por estas limitaciones, el odontólogo pierde habilidades y conocimientos adquiridos en la academia, sus habilidades y la seguridad en sí mismo. Discusión: El sistema de salud colombiano limita la autonomía profesional de los odontólogos que ejercen en IPS públicas y privadas. La autonomía profesional, relacionada con la toma de decisiones, es un eje importante del ejercicio asistencial. Ejercer la autonomía profesional permite lograr razonamientos ético-clínicos, desarrollar destrezas y promover el bienestar de los pacientes, es decir, el compromiso bioético (beneficencia, no maleficencia y justicia), y el respeto.

This article presents the findings of an activity that combined agro-ecological, multidisciplinary, and inclusive aspects, undertaken with the aim of disseminating the goals of sustainable development. The work was carried out in 2019 by the Education for Transformation Extension Project: Environment and Health, of the University of Vale do Itajaí (Univali), Santa Catarina State, Brazil. The target groups were female agriculturalists from the Vale do Itajaí region, as well as academics, teachers, and students from state schools. The work received the support of the Coordination for Regional Education (CRE) of Itajaí and the community in general, reaching approximately 2,500 people directly and 12,000 people indirectly. The methodological approach was based on Freire's culture circle system employing discussion wheels. The activities were evaluated to elucidate the impacts of the different activities, with the aim of validating the various strategies adopted. It could be inferred that they contributed to the strengthening of autonomy, stimulating active participation of the public involved in the construction of knowledge concerning sustainable agriculture. Organizing and managing the sales of agroecological products from the cultivations of women farmers in the Solidarity Economy Fair, as well as in product baskets based on the model of the Community that Sustains Agriculture (CSA), provided food security and independence, financial emancipation, and social inclusion. The management of wastes reduced solid organic waste by more than ninety tons, minimizing environmental impacts. The agricultural system based on agroecological principles provides high levels of diversity and resilience, with excellent potential for the local production of healthy foods that are essential for rural and urban communities. Se aborda la ideología de las y los profesionales de la medicina, desde la teoría de Thompson, para entender los significados de la profesión médica, a partir de las categorías: autonomía, normas y valores, y reconocimiento social. Se realizaron 24 entrevistas semiestructuradas a profesionales de la medicina en cuatro ámbitos de trabajo: institucional público, institucional privado, ejercicio privado y ejercicio mixto, en Amecameca, municipio del Estado de México, en el período comprendido de marzo a octubre de 2019. El análisis de los significados expresados por las y los profesionales de la medicina en cada una de las categorías de interés muestra una base condicionada por la estructura del control estatal y por la economía mercantilizada, que valora con mayor significación formas de práctica médica privada. En la conformación ideológica de la profesión médica subsiste un componente práctico inmerso en un romanticismo liberal en un escenario casi extinto que subsiste en la actualidad, sin articularlo a los procesos sociales más generales.

Objetivo: identificar la influencia del sistema de salud colombiano sobre la autonomía profesional de odontólogos que ejercen en IPS públicas y privadas de Bogotá. Metodología: estudio cualitativo comprensivo, llevado a cabo con herramientas etnográficas como la entrevista semiestructurada. Las categorías de análisis fueron: autonomía profesional, modelos de atención, toma de decisiones y sistema contractual. La información se analizó de manera categorial, contextual y de contenido. Resultados: se evidenció que la autonomía profesional está limitada por el tiempo de atención para los pacientes (veinte minutos). Otra limitante es la restricción en la realización de procedimientos y, en ocasiones, los protocolos de atención. Por estas limitaciones, el odontólogo pierde habilidades y conocimientos adquiridos en la academia, sus habilidades y la seguridad en sí mismo. Discusión: el sistema de salud colombiano limita la autonomía profesional de los odontólogos que ejercen en IPS públicas y privadas. La autonomía profesional, relacionada con la toma de decisiones, es un eje importante del ejercicio asistencial. Ejercer la autonomía profesional permite lograr razonamientos ético-clínicos, desarrollar destrezas y promover el bienestar de los pacientes, es decir, el compromiso bioético (beneficencia, no maleficencia y justicia), y el respeto.

El estudio busca comprender los significados elaborados por el profesorado en jefe respecto a la política educativa en Chile, a partir del análisis de la implementación del Programa de Acompañamiento y Acceso Efectivo a la Educación Superior (PACE), el cual otorga un ingreso alternativo a la educación superior a estudiantes en condición de vulnerabilidad. La subjetividad del profesorado jefe se articuló como eje central para comprender el rol que ejerce en el diseño, implementación y evaluación de la política educativa; de su participación en la implementación del PACE y de las particularidades de su función. La investigación se planteó desde un enfoque cualitativo, como un estudio de caso, aplicando la entrevista en profundidad y el grupo focal como técnicas de recolección de datos. Los resultados muestran al PACE como ejemplo de políticas educativas focalizadas en actores específicos de la comunidad educativa; subsidiándoles derechos sociales sin alterar la estructura del sistema educativo. Además, se caracteriza por la escasa participación docente, en este caso de profesores jefes, pese al rol preponderante que cumplen en el tránsito hacia la educación superior del estudiantado.

Background: The recognition of nurses autonomy can only be achieved if it is correctly measured and validated. In this sense, it is imperative to evaluate their knowledge, recognizing their decisive role. Objective: To validate and evaluate the psychometric properties of the Knowledge Questionnaire about Autonomous Nursing Interventions for patients with Acute Coronary Syndrome. Methodology: Methodological, descriptive, analytical-correlational and cross-sectional study. Internal consistency was assessed using Cronbach's alpha and confirmatory factor analysis in a sample of 738 Portuguese nurses enrolled in the Ordem dos Enfermeiros. Results: The average age was 37.57 years, 76.0% of which were women. After the refinement of the questionnaire, the confirmatory factor analysis showed an acceptable quality of adjustment of the factor structure composed of 9 factors with 44 items (x²/gl = 2.042; CFI = 0.846; GFI = 0.826; RMSEA = 0.053; RMR = 0.069). The overall internal consistency was α = 0.930, showing high reliability, explaining 56.739% of total variance. Conclusion: The questionnaire is reliable and valid, revealing the potential for investigation, monitoring and evaluation of nurses' knowledge in a training context and clinical practice. In the last decades, medicine has experienced intense technological, political and cultural changes that have reconfigured the role of the physician. To characterize the actual status of the medical profession, this article discusses the crisis in the medical profession through the identification and analysis of 6 macrosocial phenomena that can be traced through the sociological perspective including commercialism, managerialism and medicalization. Aside of these tendencies, it seems that it is in the daily encounter of the physician and the new role of the patient where these changes are reproduced and strengthened. The analysis nourishes from a sociological perspective by eliciting the classical attributes of the profession: monopoly of knowledge, auto-regulation, mandate and license. In the light of this concept, it becomes clear that the first three attributes suffer an involution, which offers insight to reexamine the grounds in which the concept was constructed, and to reflect on the implications for medical education. This article deals with the ideology of medical professionals in order to understand the meanings attributed to their profession, taking Thompson's theory as a starting point and centering on the categories of autonomy, norms and values, and social recognition. Twenty-four semi-structured interviews were conducted with medical professionals in Amecameca, a municipality in the State of Mexico, from March to October 2019, in four sectors of service provision: public institutional, private institutional, private practice, and mixed practice. The analysis of meanings expressed by medical professionals with respect to each category of interest reveals that medical professionals are conditioned by the structure of state control and by the commodified economy, where private forms of medical practice are more highly regarded. In the ideological conformation of the medical profession, there remains a practical component imbued with a liberal romanticism in an extinct scenario that still exists today, without being articulated to more general social processes. Objective: To analyze the meaning of nursing practice in an Intensive Care Unit. Method: Qualitative study, conducted in an Intensive Care Unit in the south of Brazil, with nine nurses. The data collection took place through a discussion group, having as a triggering question: What does the practice of Nursing History in the Intensive Care Unit? The data were analyzed through the Collective Subject Discourse. Results: The study gave rise to three discourses with the central ideas: Nursing history, nursing qualification and empowerment; Information as a care tool; The nursing history as a collective construction. Introduction: Professional nurse autonomy is considered a complex issue with little attention, especially in fields that demand highly complex actions such as the operating room. Objective: To analyze the social representations made by nurses about their professional autonomy in the operating room. Materials and Methods: An exploratory descriptive qualitative study was conducted based on the theoretical underpinning of the Theory of Social Representations. Nine nurses, who work in the operating room of a university hospital in the state of Rio de Janeiro, participated in the study. The data were collected between June and October 2017 through a semi-structured interview and subsequently subjected to thematic analysis. Results: Based on the data analysis made, the following categories were created: the professional autonomy of the operating room nurse: knowledge, freedom of action and decision-making power; the constraints to professional nurse autonomy in the operating room: hierarchical and power relationships and knowledge. Discussion: Professional nurse autonomy in the operating room is an object of representation with different cognitive and practical dimensions: the first one is mainly based on technical-scientific knowledge, while the second one is based on the freedom of action and the decision-making power required to exercise full autonomy. Conclusions: Professional nurse autonomy is an object of social representation conceived as a body that generates knowledge, know-how, reflection and professional identity that can arouse attitudes and practices. Introducción: la enfermedad renal crónica es una patología que disminuye la expectativa y calidad de vida de los pacientes. Los médicos son los principales encargados de preservar la vida del paciente y son responsables en la toma de decisión compartida. Objetivo: analizar la percepción del personal médico sobre la prolongación del tratamiento sustitutivo renal en los pacientes con enfermedad renal crónica en fase terminal en la unidad de diálisis en el Hospital General Docente de Ambato, Ecuador. Método: se realizó una investigación cualitativa, con una entrevista semiestructurada que constó de preguntas guiadas por la teoría-hipótesis del tema. La muestra de estudio estuvo compuesta por los médicos de la unidad de diálisis, responsables del manejo clínico de los pacientes con enfermedad renal crónica en tratamiento de diálisis. El protocolo fue aprobado por el Comité de Ética de investigación en Seres Humanos de la Pontificia Universidad Católica del Ecuador, con código de aprobación CEI-88-2020. Resultados: el análisis temático evidenció que los médicos necesitan evaluar ética y clínicamente de manera integral al paciente antes de mantener o dar por terminada la hemodiálisis; el nivel de conocimiento y habilidades ético-clínicas influyen en la toma de decisiones. Además, la aplicación de los principios bioéticos de Beauchamp y Childress para la toma de decisiones es vaga, moramente confusa y por tanto irrelevante para este tipo de decisiones. Conclusiones: los médicos perciben la necesidad de evaluar integralmente al paciente, al tener en cuenta no solo la condición física sino psicológica, social y económica del paciente. Consideran innecesario mantener un tratamiento de hemodiálisis en un paciente con una calidad de vida deteriorada, con autonomía reducida y cuyo tratamiento prolongado podría causarle más dolor que beneficio.

33. Professional care in social intervention contexts: an organisational approach

Professions linked to social intervention are subject to strong pressure that causes high levels of stress and professional burnout; therefore, the care of professionals becomes a strategic area of management. Organisational culture has been studied as an intensifier of this professional stress and has been scarcely addressed as a palliative element. The aim of this article is to analyse how the organisational culture of the institutions where social intervention professionals work can contribute to professional care. The research focuses on an exploratory case study in Andalusia where, through fieldwork in 14 community social services centres, 80 in-depth interviews were conducted with professionals working in these centres (social workers, psychologists, and social educators). Based on these interviews, it was possible to identify organisational strategies that contribute to professional care: leadership, collective construction or professional autonomy. The results show the importance of the organisational culture of care which, combined with the classic self-care approach, can facilitate professional performance in the field of social intervention and, therefore, improve citizen care., Las profesiones vinculadas a la intervención social están sometidas a una fuerte presión que provoca altos índices de estrés y desgaste profesional; por tanto, el cuidado de los profesionales se convierte en un ámbito estratégico de gestión. La cultura organizativa ha sido estudiada como elemento intensificador de ese estrés profesional y escasamente abordada como elemento paliativo del mismo. El objetivo de este artículo es analizar cómo la cultura organizativa de las instituciones donde desempeñan su labor los profesionales de la intervención social puede aportar al cuidado profesional. La investigación se centra en un estudio de caso exploratorio en Andalucía donde, mediante el trabajo de campo en 14 centros de servicios sociales comunitarios, se han desarrollado 80 entrevistas en profundidad a profesionales que trabajan en estos centros (trabajadoras sociales, psicólogas y educadoras sociales). A partir de las mismas se han podido identificar estrategias organizativas que contribuyen al cuidado profesional: liderazgo, construcción colectiva o autonomía profesional. Los resultados muestran la importancia de la cultura organizativa sobre los cuidados que, combinado con el enfoque clásico de autocuidados, puede facilitar el desempeño profesional en el campo de la intervención social, y, por tanto, mejorar la atención a la ciudadanía.

Introducción: la enfermería de práctica avanzada implica el desarrollo de nuevos roles en la formación y práctica enfermera a partir del nivel de maestría, cuya esencia es la ampliación de los dominios de actuación con autonomía e independencia. Metodología: revisión temática. Se realizó búsqueda en bases de datos Lilacs, Pubmed y EBSCO. Se utilizó como palabra clave enfermería de práctica avanzada, se limitó a los artículos publicados desde el año 2010 hasta el año 2016 en idioma inglés, español y portugués. Análisis: Resultados: La ley 266 de 1996 define la enfermería como como una profesión liberal. De otro lado, varias políticas, planes y programas en salud sexual y reproductiva manifiestan la necesidad de personal formado en estas temáticas y en componentes adicionales como la investigación y el liderazgo. Una oportunidad para el desarrollo de la autonomía y la independencia en enfermería es la práctica avanzada que favorezca la ampliación de los dominios de desempeño profesional y ayude a visibilizar el quehacer enfermero. Conclusión: la salud sexual y reproductiva puede ser un nicho propicio para el desarrollo de la enfermería de práctica avanzada en el marco de la atención primaria en salud siempre y cuando exista un marco político y regulatorio.

Introduction: Changes in knowledge and practice have consolidated physiotherapy as an emerging fast-growing profession. However, the market dynamics and the configuration of health systems have transformed the characteristics of human resources, as well as the working conditions, advances and trends of professional practice. Objective: To characterize the current state of the professional practice of physiotherapy in the organizational framework of the Colombian health system, since its transformations have impacted the professional practices of the health sector. Materials and methods: An online survey was applied to physiotherapists graduated from Universidad Nacional de Colombia between 1980 and 2012. The survey evaluates the fields and areas of performance, working conditions, professional autonomy, social assessment and morbidity profile. Results: Physiotherapy is conceived as a profession required by society, with fields and areas of performance projected beyond the health sector. However, the deteriorating work conditions are worrying, particularly recruitment, low wages and restrictions for autonomous professional practice, especially those related to healthcare services. Conclusions: It is necessary to ensure working conditions that promote a suitable professional practice and the permanence of professionals, particularly in clinical practice.

Methodology: comprehensive qualitative study, carried out with ethnographic tools such as the semi-structured interview. The analysis categories were: professional autonomy, health care models, decision making and contract system. The information was analyzed according to its category, context and content. Results: it was found that professional autonomy is limited due to the time given per patient (twenty minutes). Another limitation is the restriction to carry out procedures and, sometimes, the care protocols. Due to these limitations, dentists lose the skills and knowledge acquired in the academia, their abilities and self-confidence. Discussion: the Colombian health system limits the professional autonomy of dentists who work in public and private IPS. Professional autonomy, related to decision making, is an important axis in the healthcare practice. Exercising professional autonomy allows achieving ethical-clinical reasonings, developing skills and promoting the patients´ well-being, that is, the bioethical commitment (beneficence, non-maleficence and justice), and respect. Objective: to identify the perception of obstetric nurses about the healthcare model and practice in a philanthropic maternity hospital. Method: qualitative study of 13 obstetric nurses working in a Minas Gerais maternity hospital. Researchers used semi-structured interviews for data collection which took place from September 2015 to February 2016. Data analysis used content analysis. Results: the following categories emerged: Performance based on humanization and good practices; Professional autonomy; Professional mentoring during decision making; Teamwork and Performance Model. Discussion: research revealed that nurses are autonomous and work as a team; they did not single out a healthcare model and identified work demand and technocratic model as obstacles. Final considerations: management support is important for the professionals' performance since it contributes to autonomy at work. Despite the progress made, there are still challenges both in terms of staff and communication, as well as management support. [ABSTRACT FROM AUTHOR]

ABSTRACT Objective To explore potential associations between nursing workload and professional satisfaction among nursing personnel (NP) in Greek Coronary Care Units (CCUs). Method A cross-sectional study was performed involving 66 members of the NP employed in 6 randomly selected Greek CCUs. Job satisfaction was assessed by the IWS and nursing workload by NAS, CNIS and TISS-28. Results The response rate was 77.6%. The reliability of the IWS was α=0.78 and the mean score 10.7 (±2.1, scale range: 0.5-39.7). The most highly valued component of satisfaction was “Pay”, followed by “Task requirements”, “Interaction”, “Professional status”, “Organizational policies” and “Autonomy”. NAS, CNIS and TISS-28 were negatively correlated (p≤0.04) with the following work components: “Autonomy”, “Professional status”, “Interaction” and “Task requirements”. Night shift work independently predicted the score of IWS. Conclusion The findings show low levels of job satisfaction, which are related with nursing workload and influenced by rotating shifts.

Resumo O artigo caracteriza a “objeção de consciência” – cercada por controvérsias e marcada pela ausência de definição unificada – e os limites de seu exercício. O objetivo da pesquisa, baseada na abordagem de revisão crítica de literatura, é propor uma definição para o termo. Para isso, identificaram-se situações em que a objeção de consciência é erroneamente invocada ou serve de pretexto para comportamentos antiéticos, e se procurou estabelecer os elementos que verdadeiramente compõem tal objeção. O conceito proposto pretende contribuir para esclarecer o assunto e estabelecer limites justos ao exercício ético desse direito. Abstract This article characterizes “conscientious objection” – surrounded by controversies and marked by the absence of a unified definition – and the limits of its exercise. From a critical literature review approach, the objective is to propose a definition for the term. For such, situations where conscientious objection is wrongly invoked or serves as a pretext for unethical behavior were identified, and an attempt to establish the elements that truly compose such objection was made. The proposed concept intends to contribute to clarifying the matter and establishing fair limits to the ethical exercise of this right. Resumen El artículo caracteriza la “objeción de conciencia”, rodeada de controversias y marcada por la ausencia de una definición unificada, y los límites de su ejercicio. El objetivo de la investigación, basada en el enfoque de la revisión crítica de la literatura, es proponer una definición para el término. Para ello, se identificaron situaciones en las que la objeción de conciencia se invoca erróneamente o sirve de pretexto para conductas poco éticas, y se intentó establecer los elementos que verdaderamente componen dicha objeción. Educational systems across the world are applying measures addressed to the improvement of education quality. These measures have as a consequence, the shaping of a rhetoric that works as a framework for the interpretation of educational agents’ action. The article deals with the analysis of the impact of global educational policies on the notions of educational agents’−teachers and parents−autonomy, freedom and participation, within the Madrilenian educational system. To this end, it takes as a context the meaning given to these notions within the Spanish educational system, through the analysis of its evolution over the national education laws. Next, it presents the reform applied to the Madrilenian educational system through facing the policy makers’ speeches and the educational legislation that they have enacted. The results show that the impact of the reform leads to the limitation of teachers’ autonomy, the erosion of parents’ participation and the narrowing of parents’ freedom.

Este artículo muestra los resultados obtenidos sobre el nivel de autonomía profesional que los periodistas ecuatorianos perciben en su trabajo diario y los principales limitantes que encuentran basados en los niveles o factores de influencia. Este estudio se enmarca dentro del proyecto Cultura Periodística de Ecuador (CPE) en el que se realizaron 31 entrevistas en profundidad a periodistas en activo de 6 medios de comunicación nacionales (públicos, privados y comunitarios). La autonomía profesional es uno de los principales valores que determinan el nivel de profesionalización del periodismo y el profesionalismo de los periodistas al estar directamente vinculada a los conceptos de autorregulación, libertad de expresión e independencia. Esta situación provoca que las influencias percibidas por los periodistas —procedentes de los niveles de actor, institución y sistemas (Oller y Meier, 2012)— afecten directamente a su libertad de acción profesional y, por lo tanto, a sus índices de autonomía. Los resultados muestran que la mayor parte de los periodistas entrevistados considera que posee total libertad en su medio de comunicación y que los principales limitantes proceden del nivel del sistema, concretamente de la actual legislación en comunicación.

O estudo apresenta práticas humanizadoras instituídas em um hospital público no atendimento às vítimas da tragédia de Santa Maria, RS, Brasil. O relato de experiência foi construído por intermédio de depoimentos dos trabalhadores envolvidos no acolhimento dos familiares de vítimas da tragédia. A articulação e parcerias entre os diferentes serviços da instituição; o acionamento de Protocolos de Catástrofes; a criação de espaço de escuta, reordenação de fluxos e processos assistenciais foram práticas adotadas para exercer trocas solidárias empenhadas na produção de saúde. Apontamos, por meio das diretrizes da Política Nacional de Humanização (PNH), os mecanismos utilizados e como se produziu, nesse hospital, o acolhimento às vitimas da catástrofe. A experiência de contágio da PNH desenvolvida na instituição, na lógica do apoio institucional, possibilitou intervenções e novos modos de trabalho adaptados ao contexto e alinhados à proposta da humanização na atenção/gestão em saúde.
