Abstract: Background The impact of disease activity and treatment on fertility outcomes in patients with spondyloarthritis (SpA) has been little explored. This study aimed to describe median time to pregnancy (TTP) in women with SpA and the factors influencing TTP in this population.Methods This prospective observational multicentre (63 centres) French cohort (GR2 study—NCT02450396) included consecutive women with a diagnosis of SpA (according to their rheumatologist) who wanted to become pregnant between 2015 and 2021. TTP (in months) was the main outcome criterion, prospectively calculated from the date of study inclusion to the date of conception. Data on demographics, disease characteristics, disease activity, severity and treatment were prospectively collected at inclusion and each year thereafter until pregnancy occurred. TTP and its associated factors were estimated by survival analysis (Shared Frailty Cox models), with a random centre effect and multiple imputation to address missing data.Results We analysed 88 women included before conception. Among them, 56 (63.6%) became pregnant during follow-up. Median TTP was 16.1 (95% CI (12.2 to 25.3)) months. Mean preconceptional Bath Ankylosing Spondylitis Disease Activity Index at inclusion was 2.9 (±SD 2.1). Patients were treated with TNF inhibitors, non-steroidal anti-inflammatory drugs (NSAIDs), conventional synthetic disease-modifying antirheumatic drugs and glucocorticoids in 61 (69.3%), 23 (26.1%), 12 (13.6%) and 8 (9.1%) cases, respectively. 2. Training Family Medicine Residents in Dermoscopy Using an e-Learning Course: Pilot Interventional Study

Abstract: BackgroundSkin cancers are the most common group of cancers diagnosed worldwide. Aging and sun exposure increase their risk. The decline in the number of dermatologists is pushing the issue of dermatological screening back onto family doctors. Dermoscopy is an easy-to-use tool that increases the sensitivity of melanoma diagnosis by 60% to 90%, but its use is limited due to lack of training. The characteristics of “ideal” dermoscopy training have yet to be established. We created a Moodle (Moodle HQ)-based e-learning course to train family medicine residents in dermoscopy. ObjectiveThis study aimed to evaluate the evolution of dermoscopy knowledge among family doctors immediately and 1 and 3 months after e-learning training. MethodsWe conducted a prospective interventional study between April and November 2020 to evaluate an educational program intended for family medicine residents at the University of Montpellier-Nîmes, France. They were asked to complete an e-learning course consisting of 2 modules, with an assessment quiz repeated at 1 (M1) and 3 months (M3). The course was based on a 2-step algorithm, a method of dermoscopic analysis of pigmented skin lesions that is internationally accepted. The objectives of modules 1 and 2 were to differentiate melanocytic lesions from nonmelanocytic lesions and to precisely identify skin lesions by looking for dermoscopic morphological criteria specific to each lesion. Each module consisted of 15 questions with immediate feedback after each question. ResultsIn total, 134 residents were included, and 66.4% (n=89) and 47% (n=63) of trainees fully participated in the evaluation of module 1 and module 2, respectively. 3. Efficacy and tolerance of photodynamic therapy with methyl‐aminolevulinic acid in early‐stage mycosis fungoides: A real‐life retrospective study

Abstract: Abstract Background Methyl‐aminolevulinate‐based photodynamic therapy (MAL‐PDT) has been successfully used in early‐stage mycosis fungoides (MF) lesions. However, the long‐term outcome of initially responding lesions has not been systematically investigated to date. To address this issue, we hereby report a large series of early‐stage MF lesions treated with MAL‐PDT and followed up for 1 year after initial treatment. Objectives The objective of this study was to retrospectively evaluate short‐ and middle‐term clinical results of MAL‐PDT on early‐stage MF lesions (patches and plaques) and to identify characteristics possibly predictive of effectiveness. Methods Data from all early‐stage MF lesions (plaques and patches) treated with MAL‐PDT between 2010 and 2017 in a tertiary referral centre were retrospectively collected. Primary endpoints were initial clinical response rated as complete response (CR) or partial response (PR), and the rate of relapse of initially responding lesions was further evaluated 1 year after the last PDT session. Secondary endpoints addressed tolerance and the relevancy of predefined parameters possibly predictive of CR achievement. Results A total of 62 lesions from 30 patients with early‐stage MF (21 Ia, nine Ib) were treated and analysed. At the 6‐week evaluation, the overall clinical response rate (RR) was 87.25% (55% CR and 32.25% PR) with a mean of 2.7 PDT sessions per lesion. Twenty‐five percent of initially responding lesions relapsed locally within 1 year. Transient, although significant, pain occurred at least once in 30% of patients. In univariate analysis, the location of lesions on sun‐protected areas was the only parameter statistically related to CR, a result confirmed in multivariate analysis. Conclusions Overall, these data confirm the interest in the use of MAL‐PDT in early‐stage MF even for deep (folliculotropic) lesions with a higher efficiency on sun‐protected areas. Furthermore, this study demonstrates for the first time that the response is often sustained over time. Accordingly, MAL‐PDT should be considered in the management of early‐stage MF, especially in oligolesional forms or in residual lesions refractory to systemic treatment.

Abstract: The occurrence or the worsening of cutaneous T-cell lymphoma (CTCL) during or after a treatment with dupilumab has recently been reported in a limited number of patients including a small series of 7 patients, of whom 4 had been previously diagnosed with atopic dermatitis (AD).1 We hereby describe a previously unreported observation of systemic CD30+ lymphoproliferation occurring during treatment with dupilumab, adding to the literature of new-onset cutaneous lymphomas associated with dupilumab therapy.

Abstract: Background: Hereditary angioedema (HAE) is characterized by unpredictable and potentially life-threatening attacks of cutaneous and submucosal swelling. Over the past decade, new agents, based on a better understanding of the underlying biologic mechanisms of HAE, have changed the face of long-term prophylaxis (LTP). Objective: The objective was to describe current practices and unmet needs with regard to LTP for HAE in expert centers in France. Methods: The study was conducted in France in 2020. Based on their experience with patients with HAE who had visited their center at least once in the past 3 years, physicians from 25 centers who are expert in the management of HAE were requested to fill in a questionnaire that encapsulated their active patient list, criteria for prescribing LTP, and medications used. They were asked about potential unmet needs with currently available therapies. They were asked to express their expectations with regard to the future of HAE management. Results: Analysis was restricted to 20 centers that had an active patient file and agreed to participate. There were 714 patients with C1 inhibitor (C1-INH) deficiency, of whom 423 (59.2%) were treated with LTP. Altered quality of life triggered the decision to start LTP, as did the frequency and severity of attacks. Ongoing LTP included androgens (28.4%), progestins (25.8%), lanadelumab (25.3%), tranexamic acid (14.2%), intravenous C1-INHs (5.6%), and recombinant C1-INH (0.7%). Twenty-nine percent of the patents with LTP were considered to still have unmet needs. Physicians' concerns varied among therapies: poor tolerability for androgens and progestins, a lack of efficacy for tranexamic acid and progestins, dosage form, and high costs for C1-INHs and lanadelumab. Physicians' expectations encompassed more-efficacious and better-tolerated medications, easier treatment administration for the sake of improved quality of life of patients, and less-expensive therapies. Conclusion: Despite the recent enrichment of the therapeutic armamentarium for LTP, physicians still expressed unmet needs with currently available therapies.

Abstract: Dupilumab is a therapeutic antibody targeting IL-4 and IL-13 receptor subunit alpha used for the treatment of atopic dermatitis (AD) patients. To understand the pathogenesis of DI-Pso, we performed gene expression profiling studies on skin biopsies of DI-Pso (n=7) compared with plaque psoriasis (PSO), AD and healthy controls (HC) (n=4 each). Differential gene expression was performed using enrichment and gene ontology analysis. Gene expression was validated by qPCR and protein levels assessed by immunohistochemistry. Transcriptomic and protein analysis of DI-Pso compared with HC, PSO and AD skins revealed an activation of Th17/IL-23 pathways associated with a significant expression of IL-36, surrogate marker of pustular psoriasis (PP). By contrast, Th2 representative genes' expression were strongly decreased in DI-Pso across comparison. Matching analysis with public data of PP skin corroborated that DI-Pso and PP upstream regulators overlap, greater than with PSO. Furthermore, DI-Pso showed strongly decreased expression of many barrier skin genes compared with HC, PSO and AD. Our data indicate that pathogenesis of DI-Pso relied on a shift of skin immune responses from a Th2 to an IL-36 and Th17 polarization and on intensified skin barrier alterations.

Abstract: Human immunoglobulins are used for treating diverse inflammatory and autoimmune disorders. Eczema is an adverse event reported but poorly described.To describe the clinical presentation, severity, outcome, and therapeutic management of immunoglobulin-associated eczema.This retrospective and descriptive study included a query of the French national pharmacovigilance database, together with a national call for cases among dermatologists.We included 322 patients. Eczema occurred preferentially in men (78.9%) and in patients treated for neurological pathologies (76%). The clinical presentation consisted mainly of dyshidrosis (32.7%) and dry palmoplantar eczema (32.6%); 5% of cases exhibited erythroderma. Sixty-two percent of the eczema flares occurred after the first immunoglobulin course. Eczema was observed with 13 intravenous or subcutaneous immunoglobulin types and recurred in 84% of patients who maintained the same treatment and in 68% who switched the immunoglobulin type. After immunoglobulin discontinuation, 30% of patients still had persistent eczema.Retrospective study, with possible missing data or memory bias.Immunoglobulin-associated eczema occurred with all immunoglobulin types, preferentially in patients with neurologic diseases who required prolonged immunoglobulin treatment. Recurrence was frequent, even after switching the immunoglobulin type, which can lead to a challenging therapeutic situation when immunoglobulin maintenance is required.

16. Evolution of Bullous Pemphigoid Clinical Pattern over Time: A 17-Year Single-Centre Retrospective Survey of 312 Cases

Abstract: Background and Objectives: Bullous pemphigoid (BP) clinical profile may have evolved during the last 2 decades. A retrospective, single-centre analysis investigated a possible shift of clinical presentation of the disease over time regarding both lesions’ clinical pattern and locations and more particularly an increased frequency of characteristics considered as less classical regarding the usual clinical description of BP. Patients and Methods: Initial clinical data from all BP patients treated between January 2001 and April 2017 in a reference centre were collected and compared between four 4-year successive chronological subsets (G1 to G4). Results: 213/312 patients retained for final analysis (68.3%) displayed at least one initial non-classical characteristic, mainly head and neck, palmo-plantar, and/or mucosal involvement. Chronological analysis confirmed a significant increase over time of the percentage of patients displaying such features (G1 57.9% vs. G4 73.7%, p = 0.041). Conclusion: Changes in BP clinical pattern may have occurred over the last two decades with the progressive emergence of forms with a number of less classical features. No significant clinical difference was observed between patients receiving or not DPP4 inhibitors at the time of diagnosis.

Abstract: Background and Objective: Chronic spontaneous urticaria (CSU) is a distressing disease. We report real-world data from the global Chronic Urticaria Registry (CURE) about associations between various CSU states and sleep impairment, plus important health-related quality-of-life (HRQoL) outcomes and compared different methods to assess CSU states. Methods: CURE data were collected at baseline and 6-monthly follow-ups (FU). Assessments included CSU states using the Urticaria Control Test (UCT), weekly Urticaria Activity Score (UAS7), and Physician Global Assessment (PhyGA) of treatment response. Complete response to treatment (CR, UAS7 = 0), complete control of disease (CC, UCT = 16), and PhyGA = CR were assessed, plus the Dermatology Life Quality Index and the Chronic Urticaria Quality-of-Life Questionnaire (CU-Q2oL) sleep domain. Results: Overall, 2078 patients were included. At baseline, 9.8%, 17.9%, and 42.3% of patients had UCT = 16, UAS7 = 0, or PhyGA = CR, respectively, which increased at FU1 and FU2. Abstract: Background The number of patients with a history of melanoma who are waiting a solid organ transplantation (SOT) is increasing. Few recommendations exist on the timing to transplantation after melanoma diagnosis. The aim of this study was to assess the melanoma recurrence-free survival after pretransplant melanoma (PTM). Methods We conducted a multicenter ambispective observational study. Organ transplant recipients (OTR) with a history of PTM and complete AJCC staging were included. Results Thirty-seven patients (predominantly men with a renal allograft) were included. Five melanomas were in situ, 21 stage IA, 4 stage IB, 5 stage II and 2 stage IIIB. The median post-transplantation follow-up time was 4 years. 62% of patients were followed more than 2 years. Recurrence-free survival since melanoma reached 89.9%, but varied significantly according to AJCC staging (p=0.0129). Three patients presented a recurrence. Conclusion Despite the rather limited sample size and a wide range of follow-up, our findings concerning the recurrence-free survival appear reassuring for in situ and stage IA PTM; accordingly we suggest that a waiting time to transplantation is not mandatory in patients with in situ or stage IA PTM, especially whenever SOT I s urgently needed. Caution is however needed for patients with higher stage.

Abstract: International audience; Acquired cold contact urticaria (ACU) is a putatively serious condition, because of the risk of anaphylactic shock whenever patients are massively exposed to cold atmosphere/water, raising the question of the prescription of an "emergency kit" with oral antihistamines and epinephrine auto-injector. We performed an online survey to evaluate how French-speaking urticaria experts manage ACU. According to the 2016 consensus recommendations on chronic inducible urticarias, all the participants perform at least 1 of the available provocation tests and 84.2%, 77.8%, and 88.9% prescribe on-label use of second generation anti-H1 antihistamines (2GAH1) as a first line treatment, updosed 2GAH1 as a second line treatment, and omalizumab as a third line treatment, respectively. Interestingly, 44.4% of the practitioners always prescribe a continuous background treatment, versus 11.1% prescribing only on-demand therapy. Also, 11.7% of participants always prescribe an epinephrine auto-injector, 70.6% sometimes do, and 17.6% never do. Finally, 89.5% authorize swimming under strict conditions but 36.8% and 68.4% contra-indicate other water sports and occupational cold exposure, respectively.

Materiel et methodes Des adultes presentant une DA moderee a severe ont recu l’abrocitinib (200 mg/100 mg), le dupilumab 300 mg ou un placebo plus un traitement topique. Le nombre d’eosinophiles a ete evalue a l’inclusion (BL) et aux semaines (S) 2, 4, 8, 12 et 16. Les numerations sanguines correspondant a une eosinophilie et une hypereosinophilie etaient respectivement > 500 a ≤ 1500/mm3 (0,5–1,5 g/L) et > 1500/mm3 (> 1,5 g/L). Les relations entre la variation par rapport a BL (CfB) du nombre d’eosinophiles a S2 et S16 et la CfB des criteres d’evaluation de l’efficacite clinique IGA, EASI et echelle visuelle analogique du score de gravite de la dermatite atopique [SCORAD-VAS pour les troubles du sommeil et le prurit] a S16 ont ete evaluees par les coefficients de correlation de Pearson. BL a ete definie comme la derniere mesure avant la premiere dose. Resultats Sur les 837 patients de JADE COMPARE, 187 (22,3 %) avaient une eosinophilie et 23 (2,7 %) une hypereosinophilie a BL. L’eosinophilie et l’hypereosinophilie ont diminue a S2 de 18,8 % avec l’abrocitinib 200 mg (BL : 27,0 % ; S2 : 8,2 %), 6,6 % avec l’abrocitinib 100 mg (23,9 % ; 17,3 %) et 2,8 % avec le dupilumab (23,6 % ; 20,8 %). A S16, l’eosinophilie et l’hypereosinophilie ont diminue de 17,7 % par rapport a BL avec l’abrocitinib 200 mg (BL : 27,0 % ; S16 : 9,3 %) et de 3 % avec l’abrocitinib 100 mg (23,9 % ; 20,9 %), tandis que le groupe dupilumab retrouvait les taux BL (23,6 % ; 23,8 %). La CfB du nombre d’eosinophiles a S16 a montre une correlation positive avec les ameliorations de la severite de la DA mesurees par la CfB du score EASI a S16 dans le groupe abrocitinib 200 mg (correlation : 0,32 [IC95 % : 0,19–0,44]) et avec les ameliorations des troubles du sommeil mesurees par la CfB du score SCORAD-VAS a S16 dans le groupe abrocitinib 100 mg (0,30 [0,17–0,42]). L’eosinophilie n’a ete associee a aucun evenement indesirable. Discussion L’abrocitinib a ete associe a une reduction dose–dependante prolongee de la proportion de patients avec une eosinophilie ou hypereosinophilie a S16 dans l’etude JADE COMPARE avec disparition des cas d’hypereosinophilie a la dose de 200 mg Par ailleurs, la diminution du nombre d’eosinophiles avec l’abrocitinib est correlee positivement avec l’amelioration de la severite de la DA et des troubles du sommeil a S16.

Abstract: Introduction Les angioedemes hereditaires par mutation du facteur XII representent, selon les series, 20 a 25 % des angiœdemes hereditaires bradykiniques a C1 inhibiteur normal. Ils surviennent preferentiellement chez les femmes apres la puberte, sous contraception contenant des œstrogenes ou pendant la grossesse. Cependant les hormones ne semblent pas etre le seul facteur declenchant ce type d’angiœdeme, en effet certaines patientes developpent des angiœdemes en dehors des grossesses et de la prise de contraception oestroprogestative et quelques series rapportent des cas de sujets de sexe masculin symptomatiques. L’objectif principal de notre etude est d’etudier la prevalence des symptomes d’angioedeme chez les sujets de sexe masculin et de les caracteriser en decrivant leur localisation, leur frequence, leur duree et leur reponse aux traitements eventuellement administres. Materiels et methodes Nous avons realise une etude observationnelle retrospective des manifestations d’angiœdeme chez les sujets de sexe masculin porteurs de la mutation du FXII au sein du centre de reference des angiœdemes de l’hopital Saint Antoine a Paris, et chez ceux depistes par le laboratoire de genetique de Grenoble entre juin 2006 et mai 2020. Resultats 168 sujets de sexe masculin ont ete recenses au sein du centre de reference de l’hopital Saint Antoine, 21 sont porteurs de la mutation du FXII. Nous avons pu obtenir les informations concernant 22 autres sujets masculins porteur de la mutation suivis dans d’autres centres de reference des angiœdemes bradykiniques. Parmi ces sujets, 19 % (8/43) sont symptomatiques. L’âge moyen des premiers symptomes est de 29,5 ans (6 a 68 ans). Les localisations principales sont des œdemes de la face (50 %) et des douleurs abdominales (50 %), moins frequemment les œdemes siegent au niveau des extremites (12 %), du larynx (12 %) et de la langue (12 %). La duree des crises est de 48 a 72 heures, avec une frequence moyenne de 2,25 crises par an. La reponse au traitement n’a pu etre evaluee chez tous les patients ; la moitie des patients ont repondu a l’icatibant ou aux concentres de C1 Inhibiteur Conclusion Notre travail a permis de recenser les sujets masculins symptomatiques porteurs de la mutation du facteur XII depistes en France entre 2006 et 2021. Les manifestations cliniques sont rares ; la localisation et la duree des crises sont similaires a celles observees chez les femmes, mais leurs crises semblent moins frequentes chez ces derniers. Le mecanisme bradykinique n’a cependant pas pu etre confirme chez tous les sujets. Le suivi de ces patients est donc necessaire ainsi que leur information sur la maladie, le mode de transmission genetique et sa prise en charge.

Abstract: International audience; Acquired angioedema with C1-inhibitor deficiency is a rare and peculiar entity belonging to the spectrum of bradykinin angioedemas. It usually occurs in subjects over 60 years old, and is mostly associated with a B-cell lymphoid hemopathy or a monoclonal gammopathy.The diagnosis relies on at least one angioedema episode, lasting more than 24 h, and on the decrease of functional C1-inhibitor. Low C1q is observed in 90% of patients, and an anti C1-inhibitor antibody is found in 50% of patients.The treatment of severe attacks relies on icatibant or C1-inhibitor perfusions. Long term prophylaxis in patients with frequent attacks requires treatment of the associated hemopathy if so. In case of idiopathic angioedema, tranexamic acid and danazol may be used, provided that there is-no thrombophilia; as well as rituximab as second-line treatment. Inhibitors of kallikrein still need to be evaluated in this therapeutic indication.; Les angiœdèmes par déficit acquis en C1-inhibiteur représentent une entité rare au sein des angiœdèmes bradykiniques. Ils diffèrent des formes héréditaires essentiellement par le terrain, car ils surviennent chez des sujets âgés de 60 ans en moyenne, et sont, le plus souvent, associés à une hémopathie lymphoïde B de bas grade ou à une gammapathie monoclonale.Leur diagnostic repose sur la survenue d'au moins un épisode d'angiœdème durant plus de 24 heures et sur la diminution de C1-inhibiteur fonctionnel. Une baisse du C1q est présente dans 90 % des cas et un anticorps anti C1-inhibiteur chez 50 % des patients.Le traitement des crises sévères repose sur l'icatibant ou l'administration de C1-inhibiteur. Le traitement préventif, indiqué en cas de crises fréquentes, nécessite le traitement de l'hémopathie identifiée. En l'absence de pathologie associée, l'acide tranexamique ou le danazol sont des options à envisager en l'absence de facteur de risque de thrombose, et le rituximab peut être proposé en cas d'échec. La place des inhibiteurs de la kallicréine sera à définir dans cette indication.

Abstract: International audience; Hemophagocytic lymphohistiocytosis (HLH) has been only rarely reported in patients with BRAF-mutated advanced melanoma treated with targeted therapies and never with first-line dabrafenib/trametinib combination thus far. Two patients treated with first-line dabrafenib and trametinib combination therapy for metastatic melanoma presented with sudden occurrence of fever, cytopenia, rhabdomyolysis, hepatic cytolysis, hypertriglyceridemia and very high ferritin levels after few weeks of treatment, associated with concomitant epstein-barr virus (EBV) reactivation in one patient. In both cases, drug-induced HLH was primarily considered owing to a high H-score and the absence of other etiology. Patients rapidly improved after treatment discontinuation associated with oral steroids in one patient and did not relapse after subsequent treatment resumption with a concurrent anti-BRAF/anti-MEK combination. In metastatic melanoma HLH may occur either spontaneously in the absence of any treatment as a paraneoplastic condition, related to an intercurrent infection or drug-induced mainly with various immunotherapy or with dabrafenib and trametinib following immunotherapy. However, such observations are scarce and these are the first cases of HLH occurring during first-line treatment with dabrafenib and trametinib in advanced melanoma to our knowledge. Pathomechanisms remain to be elucidated since triggering factors may encompass the treatment itself but also other significant actors including viral reactivation along with the underlying disease. The liability of treatment should be considered in cases of HLH occurring in patients with advanced melanoma successfully treated with a combined targeted therapy. A rechallenge with a concurrent anti-BRAF/anti-MEK can be proposed in this setting.

Abstract: Lenalidomide (LND), a synthetic thalidomide analog mainly used in haematological malignancies has proved efficient and well-tolerated in small case series of refractory Cutaneous Lupus Erythematosus (CLE). A retrospective survey of CLE patients treated with low-dose LND in three institutional Departments of Dermatology was conducted to further assess the benefit/risk ratio of this molecule in this setting (Institutional Review Board for ethical issues of University Hospital of Montpellier 2018_IRB-MTP_11-01). Abstract: This document summarizes the aims of GA2 LEN/HAEi Angioedema Centers of Reference and Excellence (ACAREs) and elaborates the requirements that ACAREs must fulfill to become certified. It also provides (see Appendix S1) background information on GA2LEN and HAEi, including HAEi member organizations and regional patient advocates, on why we need an Angioedema Center of Reference and Excellence (ACARE) program and network, and on the accreditation and certification process, governance and funding, and on the interaction with other GA2LEN networks of centers of reference and excellence. The protocols, aims, requirements, and provisions related to becoming a certified CARE are based on (a) the experience of the GA2LEN UCARE network and (b) input from angioedema patients, general practitioners, and angioedema specialists.

Abstract: Optimal doses of total skin electron beam therapy for mycosis fungoides remain to be established. Clinical efficiency and adverse effects of middle-dose (25 Gy) vs. low-dose (10-12 Gy) total skin electron beam therapy were retrospectively compared in a series of 14 and 12 mycosis fungoides, respectively. Overall skin response rate was 96.2% (92.9% middle-dose and 100% low-dose; not significant (NS)). Overall complete and partial skin response rates were 57.7% (42.9% middle-dose and 75% low-dose; NS) and 38.5% (50% middle-dose and 25% low-dose; NS), respectively. All responding patients relapsed after an overall median time of 5 months (7 months middle-dose vs. 4 months low-dose; p = 0.164, NS). Tolerance was equally fair in both groups, with only grade 1 and 2 adverse events observed in 100% vs. 66.7% of patients in middle-dose and low-dose groups (NS). Although no significant difference was observed, middle-dose protocol may be recommended owing to a longer relapse-free survival for a similar tolerance.

Abstract: Background Essure is an effective method for hysteroscopic sterilization. Reports of adverse effects, the underlying mechanisms of which are unknown, have increased in recent years. Objective The aim of the study was to determine whether there is a relationship between adverse events attributed to Essure and nickel sensitization. Methods Patients presenting alleged adverse reactions to Essure were referred for nickel patch testing before removal. Data regarding medical history of nickel sensitization and symptoms attributed to Essure were collected. Dimethylglyoxime spot tests were performed on the explanted Essure. There was a follow-up at 3 months to evaluate whether there is improvement of the symptoms after Essure removal. Conclusions Nickel sensitization via the classic delayed hypersensitivity pathway did not seem to be responsible for adverse events attributed to Essure. Among systemic symptoms reported, extracutaneous symptoms had the highest prevalence. Systemic contact dermatitis to nickel could not be ruled out in one case.

Abstract: Introduction Les angioedemes bradykiniques (AEB) sous antagonistes des recepteurs de l’angiotensine II (ARAII) sont rares et peuvent mimer les angioedemes histaminiques (AEHi) qui sont plus frequents ce qui mene alors a tort a l’arret du medicament. L’objectif de cette etude etait alors d’evaluer les caracteristiques cliniques, therapeutiques et d’evolution d’une cohorte de patients suspects d’AEB secondaires aux ARAII. Materiel et methodes Cette etude multicentrique nationale, observationnelle et retrospective a inclus les cas consecutifs d’angioedemes acquis apparus chez des patients sous ARAII et suivi dans un centre de Reference associe sur les angioedemes a kinines (CReAk). Discussion Cinquante-cinq patients adresses dans les services specialises pour AE sous ARAII ont ete inclus. Le diagnostic final etait un AEB dans 30 (55 %) cas et un AEHi dans 25 (45 %) cas a la fin du suivi. Les caracteristiques cliniques des AE n’etaient pas significativement differentes dans les deux groupes. Abstract: Introduction Le dupilumab est la premiere biotherapie disponible pour le traitement de la dermatite atopique (DA) moderee a severe de l’enfant âge de 6 a 11 ans, necessitant un traitement systemique. Sa prescription en France a ete possible par l’intermediaire d’une ATU depuis aout 2020. L’objectif de cette etude etait d’evaluer son efficacite et sa tolerance en vie courante a partir des donnees observationnelles des centres prescripteurs francais. Materiel et methodes Il s’agit d’une cohorte retrospective multicentrique. Les enfants ( Resultats 80 enfants (âge moyen : 9,6 ± 2,0 ans ; fille, n = 46, 57,5%) ont ete inclus. A M3, il existait une diminution significative du SCORAD moyen : 21,8 ± 13,8 a M3 vs 54,0 ± 17,8 a J0 ; p 500/mm3) n’etait significativement pas plus elevee a M3 (n = 40) qu’avant le traitement (n = 39), cependant on notait une majoration de l’hypereosinophilie a plus de 2500/mm3 chez 3 patients a 3 mois. 31 patients ont presente des poussees de DA sous traitement dont 3 avec une localisation « tete et cou ». 14 (17%) ont signale des douleurs au site d’injection ; et 5 (6%) ont arrete le traitement, dont 2 du fait des douleurs et contraintes liees a l’injection. Discussion Cette etude rapporte les premieres donnees en vie courante d’enfants de moins de 12 ans traites par dupilumab. Les limites sont le caractere retrospectif et le nombre de donnees manquantes. Les resultats suggerent une efficacite a M3, superposable aux donnees des essais chez l’enfant. Les effets indesirables telles que les conjonctivites et l’hypereosinophilie semblent etre identiques a ceux de l’adulte, par contre les poussees « tete et cou » semblent etre moins frequentes chez l’enfant. Les douleurs et contraintes de l’injection peuvent etre un motif d’arret de traitement plus frequent a prendre en compte. Ces premieres donnees en vie courante confirment l’efficacite du dupilumab chez l’enfant atteint de DA moderee a severe avec une bonne tolerance. Cependant, le mode d’administration sous forme injectable peut etre un frein a cet âge.

Abstract: Treatment of benign familial pemphigus or Hailey-Hailey disease (HHD), a rare inherited condition associated with a significant impairment of quality of life, is often challenging and disappointing with frequent relapses and infectious complications. Topical photodynamic therapy (PDT) may offer new perspectives in this difficult setting.Eight patients with long-lasting HHD lesions refractory to multiple treatments were treated on at least one involved site with PDT using methyl-amino levulinate with a standardized protocol of three sessions of irradiation separated by 3-week intervals.A complete or partial clearing was achieved in all treated areas, and the result was satisfactorily maintained in all cases after a follow-up period ranging from 3 to 36 months. Results were of higher quality in non-inguinal areas. Tolerance was overall acceptable with local pain during and shortly after irradiation being the main limiting factor.Our series, although limited in size, emphasizes the interest of PDT in this difficult condition even though results may be incomplete. Treatment-related pain can be adequately managed by prior analgesics, cooling with sprayed water and local tumescent anesthesia. Overall, PDT appears as a relevant option in refractory HHD management with a favorable benefit/risk ratio.

Abstract: Introduction L’hypereosinophilie (HE, taux d’eosinophiles sanguins (Eos) ≥ 1500/mm3) sous dupilumab dans la dermatite atopique (DA) semble plus importante dans notre etude de vraie vie menee par le GREAT (Groupe de Recherche sur l’Eczema ATopique) en 2019 que dans les essais cliniques. Toutefois, tres peu de donnees sont disponibles concernant la cinetique et l’impact clinique de cette HE. Materiel et methodes Nous avons conduit au sein du GREAT une etude retrospective incluant tous les patients avec DA moderee a severe et presentant une HE sous dupilumab, quel que soit le taux d’Eos avant traitement et la date de declaration de l’HE apres debut du dupilumab. Nous avons collecte les donnees demographiques, date d’introduction du dupilumab, score IGA (Investigator Global Assessment), taux d’Eos avant et pendant le traitement et l’impact clinique de ces eosinophiles. Resultats Sur 653 patients traites entre mars 2017 et mars 2020, 59 presentaient une HE sous dupilumab (prevalence de 9 %). 50/59 (85 %) presentaient des Eos entre 500–1500/mm3 avant traitement, et 4 une HE prealable. L’HE apparaissait rapidement apres introduction du dupilumab (delai moyen de 2,5 mois). Sur 15 patients chez lesquels le dupilumab etait arrete, l’HE etait le motif d’arret chez 10. Aucun ne presentait d’atteinte d’organe en lien avec l’HE. Les donnees evolutives disponibles a 6 mois montraient une persistance de l’HE chez 12/15 (80 %) patients ayant poursuivi le traitement, contre 5/8 (62,5 %) dans le groupe avec arret du dupilumab. Les donnees a plus long terme montraient une tendance vers la normalisation du taux d’Eos chez les patients pour lesquels des donnees etaient disponibles. Discussion La majorite des patients avec HE sous dupilumab presentait dans notre etude un taux d’Eos augmente avant traitement (500–1500/mm3), comme cela a ete rapporte dans l’asthme. Une augmentation rapide du taux d’Eos sous dupilumab etait observee mais le mecanisme physiopathologique n’est pas completement elucide. Il a ete montre que le blocage de l’IL-4 et de l’IL-13 par le dupilumab entraine une diminution rapide de la production d’eotaxine-3, chimiokine a l’origine de la migration des eosinophiles dans les tissus. Les eosinophiles se trouveraient donc « bloques » dans le sang, et ce jusqu’a regulation des taux de cytokines, expliquant ainsi le caractere transitoire de l’HE. A la difference de ce qui a ete decrit chez quelques patients dans les essais du dupilumab dans l’asthme, aucune atteinte d’organe severe en lien avec l’HE n’a ete rapportee dans notre etude. Ces donnees rassurantes nous incitent a ne pas arreter systematiquement le dupilumab lorsqu’il est responsable d’HE. Toutefois, en l’absence d’etudes prospectives et mecanistiques dediees, il est recommande de surveiller le taux d’Eos avant et au moins pendant les premieres semaines du traitement.

Abstract: Therapeutic monoclonal antibodies are henceforth commonly used in the management of psoriasis but have been also used more recently in chronic spontaneous urticaria and atopic dermatitis. Three examples are developed herein: dupilumab, omalizumab and lanadelumab. The specificity of their mechanism of action results from a better understanding of the inflammatory pathways in these chronic diseases, which previously shared either the same targeted topical or systemic treatments. However, their high costs should be put into perspective with the epidemiology, the precise evaluation of the severity, the optimization of first line treatments and the long-term benefit/risk ratio.Anticorps monoclonaux thérapeutiques - En dermatologie aussi !Les anticorps monoclonaux thérapeutiques en dermatologie ont d’abord été utilisés pour la prise en charge du psoriasis, puis, plus récemment, de l’urticaire chronique spontanée et de la dermatite atopique. Trois exemples seront abordés dans cette revue: le dupilumab, l’omalizumab et le lanadélumab. Leur mode d’action spécifique résulte d’une meilleure compréhension des mécanismes inflammatoires de ces maladies chroniques qui partageaient autrefois des traitements topiques ou systémiques plus ou moins ciblés.
