Developing a clear understanding of the effect that bacterial translocation has on initiating apoptotic pathways that induce multiple organ dysfunction syndrome (MODS) is essential for developing effective treatment modalities. Translocation does not occur naturally and apoptotic pathways remain unclear. The aim of this study is to investigate the effect of bacterial translocation on apoptotic pathways in the spleen, small intestine, colon, liver, and mesenteric lymph nodes (MLN) of rats. We randomly divided 12 healthy Wistar-Albino rats into two groups and induced bacterial translocation in the experiment group by clamping the superior mesenteric arteries (SMA) for comparison of test results to the control group. Samples from both groups were collected under sterile conditions and an inoculation procedure was performed. Caspase 3, 8, 9 and p53 were evaluated by immunohistochemistry, and cell expressions were counted. Klebsiella and E. coli colonies were observed in the bacteria cultures associated with the experiment group. Bacterial translocation-activated caspase 8 and 3 were found in all tissues of the experiment group; however, activated p53 was identified only in the colon and liver and activated caspase 9 was seen in small intestine, mesenteric lymph nodes and liver. We concluded that translocated bacteria stimulated extrinsic and intrinsic signaling pathway in gastrointestinal systems. © 2017, Scientific Publishers of India. All rights reserved.

The first results of the use of pH-monitoring of the myocardium with a Khuri monitor (Terumo Cardiovascular Systems Corporation) are presented. Before aorta clamping, the pH changes depended on the initial degree of damage to the examined myocardial segment. pH-Monitoring enabled urgent modification of the cardioplegia protocol, which helped to normalize pH and to prevent severe postoperative complications. The thermometric and pH-metric data were compared with the results of clinical and morphological studies. pH-monitoring made it possible to control myocardial vitality during the surgical intervention.

In the present study of mitral valve replacement, we investigated whether complete preservation of both leaflets (that is, the subvalvular apparatus) is superior to preservation of the posterior leaflet alone. Seventy patients who underwent mitral valve replacement in our clinic were divided into 2 groups: MVR-B (n=16), in whom both leaflets were preserved, and MVR-P (n=54), in whom only the posterior leaflet was preserved. The preoperative and postoperative clinical and echocardiographic findings were evaluated retrospectively. No signs of left ventricular outflow tract obstruction were observed in either group. In the MVR-B group, no decrease was observed in left ventricular ejection fraction during the postoperative period, whereas a significant reduction was observed in the MVR-P group (P=0.003). No differences were found between the 2 groups in their need for inotropic agents or intra-aortic balloon pump support, or in cross-clamp time, duration of intensive care unit or hospital stays, postoperative development of new atrial fibrillation, or mortality rates. Bileaflet preservation prevented the decrease in left ventricular ejection fraction that usually followed preservation of the posterior leaflet alone. However, posterior leaflet preservation alone yielded excellent results in terms of decreased left ventricular diameter. Bileaflet preservation should be the method of choice to prevent further decreases in ejection fraction and to avoid death in patients who present with substantially impaired left ventricular function.

Despite high mortality rates for penetrating heart injuries, developments in transport, diagnosis, and surgical interventions have increased survival rates. In some cases, life-threatening complications may be misdiagnosed or remain asymptomatic and lead to loss of life. Herein, we report a patient with aortic valve regurgitation because of noncoronary cusp perforation and ventricular septal defect that remained asymptomatic and diagnosed 5 years after a penetrating heart injury. © 2014 Georg Thieme Verlog KG Stuttgart . Objective The aim of this study is to assess mid-term outcomes of reduction aortoplasty and wrapping technique which is an alternative method to replacement of the aorta in ascending aortic aneurysms. We aimed to show the efficacy of this technique especially when used for patients with increased perioperative mortality or morbidity due to their concomitant pathologies. Materials and Methods From March 2009 to May 2011, 22 patients underwent reduction aortoplasty and wrapping for ascending aortic aneurysm. The study group consisted of 16 male and six female patients with a mean age of 68 ± 13.1 years. Linear plication with wrapping was performed as concomitant surgery in 20 patients and as a primary procedure in two patients. The diameter of the aorta was measured using a computed tomography scan 12 months after surgery and was compared with the preoperative value. Results Mean aortic clamp and cardiopulmonary bypass times were 91.0 ± 58.1 and 150.7 ± 80.9 minutes, respectively. Mortality was 9.1%. In no case was the postoperative death related to the aortoplasty procedure. The mean postoperative ascending diameter was 29.5 ± 1.04 mm (vs. preoperative 49.1 ± 4.1 mm, p < 0.001). Mean follow-up time was 17.2 ± 8.88 months. During the follow-up period we observed that there were no findings to suggest redilatation. Conclusion Linear plication with external wrapping is a therapeutic option with promising mid-term results, in carefully selected, high surgical risk patients with an ascending aortic aneurysm. doi:10.1111/jocs.12139 (J Card Surg 2013;28:421-426) © 2013 Wiley Periodicals, Inc.

Background: Abdominal aortic surgery can cause ischemic/reperfusion (I/R) injury not only in the lower limbs but also in remote organs such as kidneys. Venous blood volume exclusion from the inferior vena cava (phlebotomy) or/and mannitol are used as a treatment for I/R injury of kidney in humans, despite the fact that the effectiveness of these treatments is still debated. The aim of this study was to evaluate the effects of phlebotomy or/and mannitol on rat kidneys in a model of lower limbs I/R-induced acute renal injury (ARI). Material and Methods: Thirty male Wistar albino rats were used and divided into five groups: (I) sham-operated group, laparotomy without I/R injury (group [S], n = 6); (II) I/R group, infrarenal aortic cross-clamp was used for lower limbs I/R, 3 hours of ischemia followed by 2 hours of reperfusion (group [I/R], n = 6); (III) I/R + phlebotomy group, identical to group [I/R] except for 1 mL of blood aspiration from the inferior caval vein just after ischemia (group [P], n = 6); (IV) I/R + mannitol-treated group, these rats were subjected to I/R and received a bolus injection of mannitol (group [M], n = 6); and (V) I/R + phlebotomy + mannitol-treated group (group [P + M], n = 6), the same procedures were performed as those described for previous groups. At the end of 2-hour reperfusion, all rats were sacrificed. Both kidneys were harvested for biochemical assay (myeloperoxidase [MPO] and superoxide dismutase [SOD] activities, and malondialdehyde [MDA] and reduced glutathione levels) and for histopathological examination (tubular necrosis and acute inflammation on kidney [ARI score]). Results: Aortic I/R significantly increased the level of MDA (reflecting lipid peroxidation), SOD (enzymatic endogenous antioxidant), and MPO (reflecting neutrophil infiltration) activity (p < 0.05). Phlebotomy or/and mannitol treatments significantly decreased the level of MDA, SOD, and MPO activity and increased glutathione level (nonenzymatic antioxidant in the kidney tissues) (p < 0.05). Histological evaluation of ARI score showed that aortic I/R significantly increased (p value for group [S] versus group [I/R] was 0.012), whereas phlebotomy or/and mannitol treatments significantly decreased tubular necrosis and inflammatory infiltration (p values for group [I/R] versus group [P], [M], and [P + M] were 0.043, 0.043, and 0.003, respectively). Conclusion: This experiment clearly indicated that the lower limbs I/R-induced ARI attenuated significantly by phlebotomy or/and mannitol treatments. Phlebotomy plus mannitol is more effective treatment than phlebotomy or mannitol alone in preventing lower limbs I/R-induced ARI in rats. Further clinical studies are required to clarify whether phlebotomy or/and mannitol treatments are beneficial in alleviating of ARI during abdominal aortic surgery. © 2011 Annals of Vascular Surgery Inc.

Pseudoaneurysms of abdominal aorta after blunt injury are uncommon. These events may present months and even many years after an injury. Such a pseudoaneurysm may rupture at any time which is a life threatening condition. We reported a case of a 33 years old man with a ruptured abdominal aorta pseudoaneurysm which had formed two months earlier following an automobile accident. Just after the accident the patients had admitted to the hospital. Computed tomography (CT) had revealed fractures of T9, L3 vertebral body and C6 vertebral fascet. Posterior transpedicular rod stabilization had been performed for T9 vertebral body fracture. After the operation, he had been started heparin therapy for paraplegia and immobilization. Neither CT nor abdominal USG had showed any sign of retroperitoneal hematoma or extravasations. Two months after the initial surgery, the patient was consulted to us because of his increasing abdominal pain, tenderness, sudden hemoglobin reduction and hemodynamic instability. CT of the abdomen and pelvis showed a large infrarenal aortic pseudoaneurysm. Because of the presence of huge retroperitoneal hematoma and abdominal compression signs, a surgical management was decided to be performed. The pseudoaneurysm was resected, the aortic defect was repaired with a Dacron patch, and the organized hematomas were evacuated. In such patients, even in the late period after trauma, a sudden onset of symptoms like abdominal pain, distention and hemodynamic instability should be carefully evaluated and pseudoaneurysm should be suspected.

Background. During off-pump coronary bypass grafting, local vascular control of the target vessel and a bloodless field are crucial. The aim of this study is to asses the histopathological outcomes of intracoronary shunts and bulldog clamping on the beating heart in a canine model. Methods. Twelve healthy adult mongrel dogs weighing between 15 to 25 kg were included in the study. Following left thoracotomy, proximal left anterior descending artery segment 1 cm to distal of diagonal branch was marked. Arteriotomy at this site was performed and a shunt was inserted for 10 min in the shunt group. The bulldog clamp was applied 3 cm distal to the mark for 10 min after heparinization in the bulldog group. Thirty days after the procedure, the specimens of left anterior descending artery from both regions were collected and were examined. Vascular damage, presence of intimal hyperplasia, and denudation were noted. Results. Only intimal denudation was found significantly higher in the shunt group (P < 0.05). In this group, only one case had grade 0 endothelial damage. In the bulldog group, all cases had endothelial damage of various grades. Conclusion. The proven advantages of temporary intracoronary shunts are well-known, e.g., preserving the ventricular functions. Despite these advantages, our study revealed an ultimate bad result for an off-pump coronary by pass patient: intimal denudation. We conclude that further studies with a larger number of subjects are needed to decide whether routine shunt insertion into coronary arteries during off-pump coronary bypass surgery is appropriate or not.

The objectives of this study were to compare the protective effects of ascorbic acid and iloprost on lung injury caused by ischemia reperfusion (I/R) of the lower extremities of rats. Wistar albino rats (n = 34) were divided into five groups. In the I/R group (n = 6), the aorta was cross-clamped for 3 hr, followed by 1 hr of reperfusion. In the vitamin C group (n = 8), animals were pretreated with 100 mg/kg ascorbic acid via the left jugular vein before aortic cross-clamping. In the iloprost group (n = 8), animals were pretreated with 20 ng/(kg·min) iloprost by constant intravenous infusion via the left jugular venous cannula. In the sham group (n = 6), the abdomen was left open at the same period and a juguler venous line was established. In the control group (n = 6), lungs were removed and blood samples taken immediately after sternotomy. No treatment was given in this group. After both lungs were removed, biochemical parameters were measured and histopathological evaluation was made. Although the arterial blood pO2 and HCO3 levels were stastistically significantly high in both the vitamin C and iloprost groups compared to the I/R group, plasma malondialdehyde (MDA) levels were significantly low. Meanwhile, the MDA levels in the lung tissue were significantly low in the vitamin C group compared to the I/R group. The MDA level in the lung tissue in the iloprost group was also low compared to the I/R group, but it was not statistically significant. The lungs of the I/R group displayed intense interstitial leukocytic infiltration in histopathological examination compared to the other groups. Pretreatment of animals with iloprost and vitamin C significantly decreased the pulmonary injury characterized by decreased plasma leukocyte sequestration. The results suggest that both vitamin C and iloprost are useful agents for attenuating the lung injury caused by increased oxidative stress and neutrophil accumulation after a period of I/R of the lower extremities. © Annals of Vascular Surgery Inc.

A 4-year-old boy with a history of acute lymphoblastic leukemia was referred for evaluation of cardiac function due to progressive decline in exercise tolerance and to cardiomegaly on chest radiography. Six months earlier, there had been no such finding on the chest film. Echocardiography, also performed at the earlier visit, had yielded a normal result.

Objective: To determine whether glutamat and aspartat enriched cold crystalloid cardioplegia which was given in antegrade way has any effect on the myocardial protection during cardiopulmonary bypass. Material and Methods: Thirty-four patients who were electively undergone open heart surgery at Osmangazi University Faculty of Medicine, thoracic and cardiovascular surgery department, between March 2001 and May 2001 were included in this study. The patients were divided in two groups, each consisting of 17 patients. In group 1 coronary artery bypass surgery (CABG) was performed in 11 patients, mitral valve replacement (MVR) in 3 patients, aortic valve replacement (AVR) in 1 patient and AVR and MVR in 2 patients. While in group 2 CABG was performed in 13 patients and MVR was done in 4 patients. Group 1 patients received antegrade glutamat and aspartat (15 mmol/L) enriched cold crystalloid cardioplegia and group 2 patients were given cold crystalloid cardioplegia by antegrade route. Age, gender, diabetes mellitus, hypertension, preoperative myocardial infarction, smoking, ejection fraction, aortic cross-clamp time, need to defibrillation, inotropic support, and intraaortic balloon pump were recorded. The levels of cardiac troponin I (cTI) and creatine kinase myocardial band fraction (CK-MB) were measured in arterial blood samples at five different times. Statistical analysis was performed using Student's t-test and Chi-square test. Results: There were no statistically significant differences in cTI and CK-MB values in blood samples taken at 5 different times pre and postoperatively between group 1 and group 2. Evoked spinal cord potentials (ESCPs) were monitored in 12 patients who underwent repair of thoracoabdominal aortic aneurysm with a high risk of spinal ischemia. A pair of bipolar catheter electrodes were introduced into the epidural space, one at the level of the C5-T2 vertebrae and the other at the level of T11-L2. Conductive mixed ESCP in seven patients, conductive sensory ESCP in one patient, and segmental descending ESCP in three patients were observed by applying a rectangular electric current to one of each pair of epidural electrodes and recording through the other. Segmental ESCP in response to posterior tibial nerve stimulation was observed in one patient. Following aortic cross-clamping, the I wave of conductive mixed ESCPs gradually decreased in amplitude with latency prolongation in five of the seven patients and disappeared in one of these five; transient augmentation of amplitude was observed before eventual decline in four of these five patients. The N wave of segmental descending ESCP subsequently flattened in two of the three patients and the N1 wave of segmental ESCP in the one patient. Three of the four patients in whom the ESCPs disappeared during aorta clamping recovered the ESCPs after declamping and showed no neurological disorders postoperatively. Intraoperative ESCP monitoring appears to be useful to detect spinal cord ischemia in the early stage and to alert surgeons and anesthesiologists so that timely resuscitative steps can be taken.

An original procedure of extensive bloodless hepatectomy is described in the pig. Complete vascular liver isolation with aorta clamping up to 25 min allows 60-75% bloodless hepatectomy without any portal triad dissection. Perfect hemostasis and shortening of liver ischemia is obtained with the use of a gelatin-resorcin-formaldehyde adhesive. Hemodynamic tolerance is good, and liver regeneration appears to be as fast as in minor species.

In this paper, we propose the use of black box models for the system identification of the cardiopulmonary baroreflex control of arterial resistance and of ventricular contractility and of arterial baroreflex control of heart rate (HR) from invasive, continuous measurements of arterial blood pressure (ABP) and central venous pressure (CVP), and non invasive, continuous recordings of ECG and respiration. Two crucial phases of the abdominal aortic aneurism (AAA) repair were investigated: the clamping and declamping of aorta. The objective of the present work is to evaluate and to test the ability to monitor baroreflex responses to clamping and declamping maneuvers preceding and following aneurism removal.
