1. Effect of flow adjustment dialysate (Qd) hemodialysis on effectiveness of underweight patients

Abstract

KDOQI guidelines in 2006 using standard dialysis adequacy Kt/V, where V is volume of distributionof urea, underweight patients have lower total body water, lower V, which couldreduce the requirement without affecting Qd the efficiency of dialysis. Objective: to evaluatethe effect on the adequacy of dialysis Qd reduction occurs in patients weighing less than orequal to 60 kg who are on hemodialysis. Methodology: patients with chronic kidney diseaseon hemodialysis regularly with weight less than or equal to 60 kg of a renal clinic to evaluatetwo periods I and II, were continued therapy parameters with decrease of Qd for the secondperiod. The variables were collected directly by the researchers of the history. The values thusobtained would be compared using t test or paired variables, and statistical significance of thetest below 0,05. Results: we included 61 patients, 60.7% female, mean age 57,3 years (SD 14,8).Average age of men 60.1 (SD 13,9) and women was 55,9 (SD 15,4). There were no statisticallysignificant differences for the variables Kt/V, Hemoglobin and there was a significant reductionin the phosphorus levels. Conclusions: this study demonstrates that adequate therapy is achievedwith less than Qd traditional standards, with 400 ml/min in patients with low weight as long asyou keep the other parameters of renal substitution.

Abstract

Las guías KDOQI del año 2006 utilizan como estándar de adecuación para la diálisis el parámetroKt/V, donde V es el volumen de distribución de urea. Los pacientes con bajo peso tienen menorcantidad total de agua corporal total (menor V), de modo que es posible reducir la cantidad de Qdsin alterar la eficacia de la diálisis. Objetivo: evaluar el efecto de la reducción del Qd sobre la adecuaciónde la diálisis en pacientes con pesos menores o iguales a 60 kg sometidos a hemodiálisis.Metodología: estudio observacional con alcance comparativo realizado en la unidad de diálisis deuna clínica especializada de Bogotá. Se evaluaron dos valores de Qd (400 ml/min y 500 ml/min)dentro del proceso de diálisis de individuos con peso menor o igual a 60 kg. Se hizo un seguimientode tres meses para cada forma de diálisis y al final de cada mes se midieron los niveles de Kt/V,hemoglobina y fósforo. Resultados: fueron incluidos 61 pacientes, 60,7% de sexo femenino. Laedad media fue de 55,9 años (DE 14,8) para las mujeres y 60,1 años (DE 13,9) para los hombres.No se observaron diferencias estadísticamente significativas entre los niveles medios de las variablesevaluadas, en los momentos de seguimiento ni al final del estudio. Conclusiones: es posibletener una adecuada terapia de diálisis utilizando un valor menor al establecido por los estándarestradicionales cuando se tienen pacientes con bajo peso, siempre y cuando se mantengan los demásparámetros de suplencia renal.

El proyecto Ecología Gamificada (ECOGAMI) se orienta al diseño, desarrollo e implementación de productos educativos digitales y no-digitales empleando el concepto de gamificación. El objetivo es proponer una alternativa educativa de complemento a la iniciativa - Mi Escuela está cerca del bosque - desarrollada por el Parque Natural Metropolitano (PNM), la cual procura fomentar y mejorar la enseñanza de la cultura ambiental en estudiantes de primaria. Los resultados obtenidos indican un impacto positivo de esta propuesta como un reforzamiento en el proceso de enseñanza y aprendizaje de los estudiantes.

Abstract

Isolated limb perfusion (ILP) is a method for treating unresectable lesions of limbs in patients with melanoma or sarcoma by using high doses of tumor necrosis factor alpha and melphalan. This high doses can result in high systemic toxicity if there is a drug leak from the isolated circulation of the limb to the systemic. This makes it imperative to monitor the leakage rate (F(%)) during the infusion, currently performed with radiotracers. The objective of this work was to develop a leakage monitoring protocol as accurate as possible to ensure safe ILP. Material and method We built a phantom with three compartments (body, limb and precordial area) and a high sensitivity collimator fitted to a portable gamma camera. We simulate ILP with scheduled leaks every 10 min from 1% to 9% (theorical F(%)). We mesured F(%) using two equation: one is the proposed in the literature and another corrected by decay of the radioisotope. We test the optimal radiopharmaceutical doses to minimize the detector dead time error and compare F(%) mesured by both equations regarding the theoretical F(%). The leakage monitoring protocol was used in 17 ILP of 16 patients and an analysis of the recorded data was performed. Results We found significant differences between F(%) mesured using the first equation and theoretical F(%), obtaining results very adjusted to the theorical after applying the decay correction. Conclusions the decay correction of the radioisotope is a simple manner to carry out the procedure more safely, reducing the error in the calculation of F(%).

Abstract

Resumen La perfusion de miembro aislado (PMA) es un metodo para el tratamiento de extremidades con lesiones irresecables de pacientes con melanoma o sarcoma. Se usan altas dosis de factor de necrosis tumoral α y melfalan, que pueden provocar toxicidad sistemica severa en caso de fuga del farmaco desde la circulacion aislada de la extremidad a la sistemica. Esto hace imperativo monitorizar el porcentaje de fuga (F[%]) durante la perfusion, actualmente realizado con radiotrazadores. El objetivo de este trabajo fue elaborar un protocolo de monitorizacion de fuga lo mas exacto posible que garantizara una PMA segura. Material y metodo Construimos un fantoma con 3 compartimentos (cuerpo, extremidad y area precordial) y un colimador de alta sensibilidad adaptable a una gammacamara portatil. Simulamos PMA con fugas programadas cada 10 min del 1 al 9% (F[%] teoricos). Medimos estos F(%) mediante 2 ecuaciones: una propuesta en la literatura y otra corregida por decaimiento del radioisotopo. Testamos las dosis de radiofarmaco optimas para minimizar el error por tiempo muerto del detector y comparamos los F(%) medidos segun las 2 ecuaciones con los F(%) teoricos. El protocolo de monitorizacion de fuga se utilizo en 17 PMA de 16 pacientes y se analizaron los datos registrados. Resultados Encontramos diferencias significativas entre los F(%) medidos con la primera ecuacion y los F(%) teoricos, con unos resultados muy ajustados a los teoricos al aplicar la correccion por decaimiento. Conclusiones La correccion por decaimiento del radioisotopo es una manera simple de realizar el procedimiento de forma mas segura y de disminuir el error en el calculo de F(%).

7. Reported adverse events related to use of hepatitis C virus direct-acting antivirals with opioids: 2017–2021

Abstract

Abstract Introduction Due to concerns over potential interactions between some hepatitis C direct-acting antivirals (DAAs) and opioids, we describe adverse event (AE) reports of concomitant use of opioids and DAAs. Methods AEs reported (July 28, 2017–December 31, 2021) with the administration of the DAAs glecaprevir/pibrentasvir, sofosbuvir/velpatasvir, ledipasvir/sofosbuvir, sofosbuvir/velpatasvir/voxilaprevir, and elbasvir/grazoprevir as suspect products were downloaded from the US Food and Drug Administration AE Reporting System Public Dashboard. The number of AE reports containing opioids (fentanyl, hydrocodone, oxycodone) as co-suspect products/concomitant products were counted and summarized by severity, reporting country and whether an outcome of death was reported. Overdose AEs were counted irrespective of opioid use, and changes over time were assessed. Results In total, 40 AEs were reported for DAAs and concomitant fentanyl use, 25 (62.5%) were in the USA, 35 (87.5%) were considered serious, and 14 (35.0%) resulted in death; and 626 were reported with concomitant oxycodone/hydrocodone use, 596 (95.2%) were in the USA, 296 (47.3%) were considered serious, and 28 (4.5%) resulted in death. There were 196 overdose AEs (32 [16%] deaths) declining from 2018 (N = 56) to 2021 (N = 29). Conclusions Treating people with hepatitis C virus (HCV) infection who use drugs is key to achieving HCV elimination. Low numbers of DAA AE reports with opioids may provide reassurance to prioritize HCV treatment in this population. These data contribute to evidence supporting the continued scale-up of DAA treatment among people who use drugs to achieve HCV elimination goals.

Abstract

Minimization of Qd is a matter of interest for our group not only in terms of dialysis efficiency and patient safety but also in terms of reduction of wastage in water. Since 2012, we have been prescribing Qd 400 mL/min for small patients, and we have already shared our experience [1, 2].We found no differences in Kt/V, phosphate, hemoglobin, potassium, and interdialytic gain between our Qd groups Table 1 [3]. Recently we have successfully finished our 5-year analysis of mortality between Qd 400 and 500 mL/min in a chronic HD population in Colombia. Our results were shared in the ISN Congress next April in Melbourne in the poster session on the topic green nephrology as one of our Blue Planet dialysis initiative

9. Assessment of Drug–Drug Interaction Risk Between Intravenous Fentanyl and the Glecaprevir/Pibrentasvir Combination Regimen in Hepatitis C Patients Using Physiologically Based Pharmacokinetic Modeling and Simulations

Abstract

Abstract Introduction An unsafe injection practice is one of the major contributors to new hepatitis C virus (HCV) infections; thus, people who inject drugs are a key population to prioritize to achieve HCV elimination. The introduction of highly effective and well-tolerated pangenotypic direct-acting antivirals, including glecaprevir/pibrentasvir (GLE/PIB), has revolutionized the HCV treatment landscape. Glecaprevir is a weak cytochrome P450 3A4 (CYP3A4) inhibitor, so there is the potential for drug–drug interactions (DDIs) with some opioids metabolized by CYP3A4, such as fentanyl. This study estimated the impact of GLE/PIB on the pharmacokinetics of intravenous fentanyl by building a physiologically based pharmacokinetic (PBPK) model. Methods A PBPK model was developed for intravenous fentanyl by incorporating published information on fentanyl metabolism, distribution, and elimination in healthy individuals. Three clinical DDI studies were used to verify DDIs within the fentanyl PBPK model. This model was integrated with a previously developed GLE/PIB PBPK model. After model validation, DDI simulations were conducted by coadministering GLE 300 mg + PIB 120 mg with a single dose of intravenous fentanyl (0.5 µg/kg). Results The predicted maximum plasma concentration ratio between GLE/PIB + fentanyl and fentanyl alone was 1.00, and the predicted area under the curve ratio was 1.04, suggesting an increase of only 4% in fentanyl exposure. Conclusion The administration of a therapeutic dose of GLE/PIB has very little effect on the pharmacokinetics of intravenous fentanyl. This negligible increase would not be expected to increase the risk of fentanyl overdose beyond the inherent risks related to the amount and purity of the fentanyl received during recreational use.

10. Kinematics and energetics of foraging behavior in Rice’s whales of the Gulf of Mexico

Abstract

Abstract Rorqual foraging behavior varies with species, prey type and foraging conditions, and can be a determining factor for their fitness. Little is known about the foraging ecology of Rice’s whales (Balaenoptera ricei), an endangered species with a population of fewer than 100 individuals. Suction cup tags were attached to two Rice’s whales to collect information on their diving kinematics and foraging behavior. The tagged whales primarily exhibited lunge-feeding near the sea bottom and to a lesser extent in the water-column and at the sea surface. During 6–10 min foraging dives, the whales typically circled their prey before executing one or two feeding lunges. Longer duration dives and dives with more feeding-lunges were followed by an increase in their breathing rate. The median lunge rate of one lunge per dive of both animals was much lower than expected based on comparative research on other lunge-feeding baleen whales, and may be associated with foraging on fish instead of krill or may be an indication of different foraging conditions. Both animals spent extended periods of the night near the sea surface, increasing the risk for ship strike. Furthermore, their circling before lunging may increase the risk for entanglement in bottom-longline fishing gear. Overall, these data show that Rice’s whale foraging behavior differs from other lunge feeding rorqual species and may be a significant factor in shaping our understanding of their foraging ecology. Efforts to mitigate threats to Rice’s whales will benefit from improved understanding of patterns in their habitat use and fine-scale ecology.

11. Real-World Effectiveness of 8-Week Glecaprevir/Pibrentasvir in Treatment-Naïve, Compensated Cirrhotic HCV Patients

Abstract

Abstract Introduction The EXPEDITION-8 clinical trial has demonstrated that treatment-naïve patients with compensated cirrhosis (TN/CC) of HCV genotypes 1–6 can achieve a 98% intent-to-treat sustained virologic response rate 12 weeks post-treatment with an 8-week glecaprevir/pibrentasvir (G/P) regimen. Further real-world evidence is needed to support the effectiveness of 8-week G/P in a clinical practice setting and to consolidate these treatment recommendations. The aim of this study is to contribute real-world evidence for the effectiveness of an 8-week G/P treatment in TN/CC patients with HCV genotypes 1–6. Methods Retrospective real-world data from 494 TN/CC patients with HCV genotypes 1–6 were collected between August 2017 to December 2020 from the Symphony Health Solutions administrative claims database. Demographic and clinical characteristics were collected at baseline. Patients were required to have a follow-up HCV ribonucleic acid level at least 8 weeks or more after the end of treatment. The percentage of patients achieving a sustained virologic response (SVR) is reported. Results The majority of patients were male (58%) and Caucasian (40%), with a mean age of 58 years; 74%, 12%, 12%, and 1% of patients were HCV genotype 1, 2, 3, and 4–6 infected, respectively. SVR was achieved in 95.5% of all patients. Across patient subgroups, SVR was achieved in 95.6% of patients with HCV genotype 3 and in 93% of HCV patients with a recent diagnosis of illicit drug use or abuse (within 6 months prior to G/P initiation). Conclusion Early real-world evidence indicates high effectiveness of the 8-week G/P regimen in TN/CC patients of HCV genotypes 1–6 from a large US claims database.

Resumen Objetivo Valorar la utilidad de los estudios PET-18F-FDG en el seguimiento de los pacientes diagnosticados de carcinoma diferenciado de tiroides que presentan riesgo de enfermedad y niveles de tiroglobulina (Tg) no interpretables por la presencia de anticuerpos antitiroglobulina (AbTg). Material y metodos Estudio retrospectivo de 7 mujeres con edad media de 40 anos y diagnostico histologico de cancer diferenciado de tiroides (7 carcinomas papilares) que fueron remitidos a nuestro servicio para la realizacion de estudio PET-18F-FDG por sospecha de enfermedad, debido a rastreo 131I negativo y niveles de AbTg elevados, entre los anos 2002 y 2007. Se les realizaron 11 exploraciones PET tras la inyeccion de 370-434 MBq de 18F-FDG en condiciones de normoglucemia y previa administracion de relajante muscular, hidratacion y furosemida. Los resultados de la PET se confirmaron por histologia y/o por evolucion clinico radiologica, con un periodo de seguimiento minimo de 24 meses. Resultados La prevalencia de la recurrencia de la enfermedad en la poblacion estudiada fue del 57,14%. Todas las pacientes presentaban niveles de Tg inferiores a 3 ng/dl y AbTg superiores a 200 UI/ml. Tres pacientes presentaban antecedentes de tiroiditis. De los 11 estudios realizados la PET fue negativa en tres y positiva en 8. Se descarto la existencia de enfermedad en 3 pacientes mientras que en 4 pacientes la PET permitio localizar con exito la existencia de recidiva. Conclusion La PET-18F-FDG es una tecnica util para localizar enfermedad, asi como para descartar la existencia de recurrencia con una elevada seguridad diagnostica, en pacientes con antecedentes de carcinoma diferenciado de tiroides con rastreo 131I negativo, pero con elevacion patologica de anticuerpos AbTg.

Abstract

Aim To evaluate the utility of 18F FDG-PET for patients diagnosed of differentiated thyroid carcinoma who present risk of disease and invaluable levels of thyroglobulin (Tg) by the presence of antibodies antithyroglobulin (AbTg). Material and methods Retrospective study of 7 women of 40 years old and histological diagnosis of differentiated thyroid cancer (7 papillary tumours) that were sent to our department for the accomplishment of 18F FDG-PET study because of suspicion of disease, due to 131 I negative and high levels of AbTg, between the year 2002 and 2007. 11 PET scans were obtained after the intravenous injection of 370–434 MBq of 18F FDG in normoglycemia conditions and previous administration of muscle relaxant, hydration and diuretic. The results of 18F FDG-PET scans were confirmed by pathologic examination or clinical outcome and radiographic examination for more than 24 months. Results The prevalence of recurrence in our population was 57.14%. All patients presented levels of Tg lower than 3 ng/dl and AbTg superior to 200 UI/ml. Three patients had precedent thyroiditis. Out of 11 scans performed 3 of them were negative and 8 cases were found positive. It ruled out the existence of disease in three patients and localized the presence of recurrence in 4 patients. Conclusion 18F-FDG-PET is a useful diagnostic tool for the detection of recurrence as well as to rule out the existence of disease with a high accuracy, in patients with differentiated thyroid carcinoma with 131 I whole body scan negative but with pathological elevation of antithyroglobulin antibodies.

Abstract

IMPORTANCE:. Initial Society of Critical Care Medicine Discovery Viral Infection and Respiratory illness Universal Study (VIRUS) Registry analysis suggested that improvements in critical care processes offered the greatest modifiable opportunity to improve critically ill COVID-19 patient outcomes. OBJECTIVES:. The Structured Team-based Optimal Patient-Centered Care for Virus COVID-19 ICU Collaborative was created to identify and speed implementation of best evidence based COVID-19 practices. DESIGN, SETTING, AND PARTICIPANTS:. This 6-month project included volunteer interprofessional teams from VIRUS Registry sites, who received online training on the Checklist for Early Recognition and Treatment of Acute Illness and iNjury approach, a structured and systematic method for delivering evidence based critical care. Collaborators participated in weekly 1-hour videoconference sessions on high impact topics, monthly quality improvement (QI) coaching sessions, and received extensive additional resources for asynchronous learning. MAIN OUTCOMES AND MEASURES:. Outcomes included learner engagement, satisfaction, and number of QI projects initiated by participating teams. RESULTS:. Eleven of 13 initial sites participated in the Collaborative from March 2, 2021, to September 29, 2021. A total of 67 learners participated in the Collaborative, including 23 nurses, 22 physicians, 10 pharmacists, nine respiratory therapists, and three nonclinicians. Site attendance among the 11 sites in the 25 videoconference sessions ranged between 82% and 100%, with three sites providing at least one team member for 100% of sessions. The majority reported that topics matched their scope of practice (69%) and would highly recommend the program to colleagues (77%). A total of nine QI projects were initiated across three clinical domains and focused on improving adherence to established critical care practice bundles, reducing nosocomial complications, and strengthening patient- and family-centered care in the ICU. Major factors impacting successful Collaborative engagement included an engaged interprofessional team; an established culture of engagement; opportunities to benchmark performance and accelerate institutional innovation, networking, and acclaim; and ready access to data that could be leveraged for QI purposes. CONCLUSIONS AND RELEVANCE:. Use of a virtual platform to establish a learning collaborative to accelerate the identification, dissemination, and implementation of critical care best practices for COVID-19 is feasible. Our experience offers important lessons for future collaborative efforts focused on improving ICU processes of care.

Today we see how prevalent the need for updated information representing the experience and skills of an organization, educational institution, company or group of experts on a topic with the aim of which are documented both explicit knowledge (which is simple record and report) and tacit knowledge (which can be understood only by one person and therefore is not easy to record or document). Here's where we consider appropriate to set up a smart manager to apply scientific knowledge gamification techniques in order to manage information and provide a mechanism to create, store and access it. Abstract

OBJECTIVES:. To describe the prevalence, associated risk factors, and outcomes of serious neurologic manifestations (encephalopathy, stroke, seizure, and meningitis/encephalitis) among patients hospitalized with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. DESIGN:. Prospective observational study. SETTING:. One hundred seventy-nine hospitals in 24 countries within the Society of Critical Care Medicine Discovery Viral Infection and Respiratory Illness Universal Study COVID-19 Registry. PATIENTS:. Hospitalized adults with laboratory-confirmed SARS-CoV-2 infection. INTERVENTIONS:. None. RESULTS:. Of 16,225 patients enrolled in the registry with hospital discharge status available, 2,092 (12.9%) developed serious neurologic manifestations including 1,656 (10.2%) with encephalopathy at admission, 331 (2.0%) with stroke, 243 (1.5%) with seizure, and 73 (0.5%) with meningitis/encephalitis at admission or during hospitalization. Patients with serious neurologic manifestations of COVID-19 were older with median (interquartile range) age 72 years (61.0–81.0 yr) versus 61 years (48.0–72.0 yr) and had higher prevalence of chronic medical conditions, including vascular risk factors. Adjusting for age, sex, and time since the onset of the pandemic, serious neurologic manifestations were associated with more severe disease (odds ratio [OR], 1.49; p < 0.001) as defined by the World Health Organization ordinal disease severity scale for COVID-19 infection. Patients with neurologic manifestations were more likely to be admitted to the ICU (OR, 1.45; p < 0.001) and require critical care interventions (extracorporeal membrane oxygenation: OR, 1.78; p = 0.009 and renal replacement therapy: OR, 1.99; p < 0.001). Hospital, ICU, and 28-day mortality for patients with neurologic manifestations was higher (OR, 1.51, 1.37, and 1.58; p < 0.001), and patients had fewer ICU-free, hospital-free, and ventilator-free days (estimated difference in days, –0.84, –1.34, and –0.84; p < 0.001). CONCLUSIONS:. Encephalopathy at admission is common in hospitalized patients with SARS-CoV-2 infection and is associated with worse outcomes. While serious neurologic manifestations including stroke, seizure, and meningitis/encephalitis were less common, all were associated with increased ICU support utilization, more severe disease, and worse outcomes.

Abstract

OBJECTIVES:. To describe hospital variation in use of “guideline-based care” for acute respiratory distress syndrome (ARDS) due to COVID-19. DESIGN:. Retrospective, observational study. SETTING:. The Society of Critical Care Medicine’s Discovery Viral Infection and RESPIRATORY ILLNESS UNIVERSAL STUDY COVID-19 REGISTRY. PATIENTS:. Adult patients with ARDS due to COVID-19 between February 15, 2020, and April 12, 2021. INTERVENTIONS:. Hospital-level use of “guideline-based care” for ARDS including low-tidal-volume ventilation, plateau pressure less than 30 cm H2O, and prone ventilation for a Pao2/Fio2 ratio less than 100. MEASUREMENTS AND MAIN RESULTS:. Among 1,495 adults with COVID-19 ARDS receiving care across 42 hospitals, 50.4% ever received care consistent with ARDS clinical practice guidelines. After adjusting for patient demographics and severity of illness, hospital characteristics, and pandemic timing, hospital of admission contributed to 14% of the risk-adjusted variation in “guideline-based care.” A patient treated at a randomly selected hospital with higher use of guideline-based care had a median odds ratio of 2.0 (95% CI, 1.1–3.4) for receipt of “guideline-based care” compared with a patient receiving treatment at a randomly selected hospital with low use of recommended therapies. Median-adjusted inhospital mortality was 53% (interquartile range, 47–62%), with a nonsignificantly decreased risk of mortality for patients admitted to hospitals in the highest use “guideline-based care” quartile (49%) compared with the lowest use quartile (60%) (odds ratio, 0.7; 95% CI, 0.3–1.9; p = 0.49). CONCLUSIONS:. During the first year of the COVID-19 pandemic, only half of patients received “guideline-based care” for ARDS management, with wide practice variation across hospitals. Strategies that improve adherence to recommended ARDS management strategies are needed.

Abstract

IMPORTANCE:. At the start of the coronavirus disease 2019 pandemic, medications repurposed for management of coronavirus disease 2019 were used in the absence of clinical trial evidence. OBJECTIVES:. To describe the variation and evolution in use of repurposed medications for coronavirus disease 2019. DESIGN, SETTING, AND PARTICIPANTS:. Observational cohort study of adults hospitalized with coronavirus disease 2019 between February 15, 2020, and April 12, 2021, across 76 United States and international hospitals within the Society of Critical Care Medicine’s Discovery Viral Infection and Respiratory Illness Universal Study coronavirus disease 2019 registry. MAIN OUTCOMES AND MEASURES:. Hospital variation was quantified using multivariable adjusted random effects logistic regression models and unsupervised clustering. Repurposed medications included antivirals, corticosteroids, hydroxychloroquine, immunomodulators, and therapeutic dose anticoagulants. RESULTS:. Among 7,069 adults hospitalized with coronavirus disease 2019, 1,979 (28%) received antivirals, 2,876 (41%) received corticosteroids, 1,779 (25%) received hydroxychloroquine, 620 (9%) received immunomodulators, and 2,154 (31%) received therapeutic dose anticoagulants. Contribution of hospital site to risk-adjusted variation was 46% for antivirals, 30% for corticosteroids, 48% for hydroxychloroquine, 46% for immunomodulators, and 52% for therapeutic dose anticoagulants. Compared with the early pandemic, the later pandemic practice phenotypes converged with increased use of antivirals (odds ratio, 3.14; 95% CI, 2.40–4.10) and corticosteroids (odds ratio, 5.43; 95% CI, 4.23–6.97), with decreased use of hydroxychloroquine (odds ratio, 0.02; 95% CI, 0.01–0.04) and immunomodulators (odds ratio, 0.49; 95% CI, 0.34–0.70). There was no clinically significant change in the use of therapeutic dose anticoagulants (odds ratio, 1.01; 95% CI, 1.01–1.02). There were no differences in risk-adjusted mortality between hospitals with high rates of repurposed medication use compared with hospitals with low rates of use. CONCLUSIONS AND RELEVANCE:. Hospital variation in the use of repurposed medications varied widely across hospitals early in the pandemic and later converged with the emergence of randomized clinical trials. Platforms developed for rapid activation and enrollment in clinical trials of repurposed medications are needed prior to the next pandemic to expedite effective, evidence-based practice.

Abstract

IMPORTANCE:. Even with its proclivity for older age, coronavirus disease 2019 has been shown to affect all age groups. However, there remains a lack of research focused primarily on the young adult population. OBJECTIVES:. To describe the epidemiology and outcomes of coronavirus disease 2019 and identify the risk factors associated with critical illness and mortality in hospitalized young adults. DESIGN, SETTINGS, AND PARTICIPANTS:. A retrospective cohort study of the Society of Critical Care Medicine’s Viral Infection and Respiratory Illness Universal Study registry. Patients 18–40 years old, hospitalized from coronavirus disease 2019 from March 2020 to April 2021, were included in the analysis. MAIN OUTCOMES AND MEASURES:. Critical illness was defined as a composite of mortality and 21 predefined interventions and complications. Multivariable logistic regression was used to assess associations with critical illness and mortality. RESULTS:. Data from 4,005 patients (152 centers, 19 countries, 18.6% non-U.S. patients) were analyzed. The median age was 32 years (interquartile range, 27–37 yr); 51% were female, 29.4% Hispanic, and 42.9% had obesity. Most patients (63.2%) had comorbidities, the most common being hypertension (14.5%) and diabetes (13.7%). Hospital and ICU mortality were 3.2% (129/4,005) and 8.3% (109/1,313), respectively. Critical illness occurred in 25% (n = 996), and 34.3% (n = 1,376) were admitted to the ICU. Older age (p = 0.03), male sex (adjusted odds ratio, 1.83 [95% CI, 1.2–2.6]), and obesity (adjusted odds ratio, 1.6 [95% CI, 1.1–2.4]) were associated with hospital mortality. In addition to the above factors, the presence of any comorbidity was associated with critical illness from coronavirus disease 2019. Multiple sensitivity analyses, including analysis with U.S. patients only and patients admitted to high-volume sites, showed similar risk factors. CONCLUSIONS:. Among hospitalized young adults, obese males with comorbidities are at higher risk of developing critical illness or dying from coronavirus disease 2019.

Abstract

In computer science, stochastic processes, and industrial engineering, stationarity is often taken to imply a stable, predictable flow of events and non-stationarity, consequently, a departure from such a flow. Efficient detection and accurate estimation of non-stationarity are crucial in understanding the evolution of the governing dynamics. Pragmatic considerations include protecting human lives and property in the context of devastating processes such as earthquakes or hurricanes. Cumulative Sum (CUSUM) charting, the prevalent technique to weed out such non-stationarities, suffers from assumptions on a priori knowledge of the pre and post-change process parameters and constructs such as time discretization. In this paper, we have proposed two new ways in which non-stationarity may enter an evolving system - an easily detectable way, which we term strong corruption, where the post-change probability distribution is deterministically governed, and an imperceptible way which we term hard detection, where the post-change distribution is a probabilistic mixture of several densities. In addition, by combining the ordinary and switched trend of incoming observations, we develop a new trend ratio statistic in order to detect whether a stationary environment has changed. Surveying a variety of distance metrics, we examine several parametric and non-parametric options in addition to the established CUSUM and find that the trend ratio statistic performs better under the especially difficult scenarios of hard detection. Simulations (both from deterministic and mixed inter-event time densities), sensitivity-specificity type analyses, and estimated time of change distributions enable us to track the ideal detection candidate under various non-stationarities. Applications on two real data sets sampled from volcanology and weather science demonstrate how the estimated change points are in agreement with those obtained in some of our previous works, using different methods. Incidentally, this study sheds light on the inverse nature of dependence between the Hawaiian volcanoes Kilauea and Mauna Loa and demonstrates how inhabitants of the now-restless Kilauea may be relocated to Mauna Loa to minimize the loss of lives and moving costs.

Abstract

Soft tissue sarcomas (STS) have minimal expression of PD-L1, a biomarker for PD-1 therapy efficacy. Radiotherapy (RT) has been shown to increase PD-L1 expression pre-clinically. We examined the expression of PD-L1, pre- and post-RT, in 46 Stage II-III STS patients treated with pre-operative RT (50–50.4 Gy in 25–28 fractions) followed by resection. Five additional patients who did not receive RT were utilized as controls. PD-L1 expression on biopsy and resection samples was evaluated by immunochemistry using the anti PD-L1 monoclonal antibody (E1L3 N clone; Cell Signaling). Greater than 1% membranous staining was considered positive PD-L1 expression. Changes in PD-L1 expression were analyzed via the Fisher exact test. Kaplan-Meier statistics were used to correlate PD-L1 expression to distant metastases (DM) rate. The majority of STS were T2b (87.0%), high-grade (80.4%), undifferentiated pleomorphic histology (71.7%), and originated from the extremities (84.6%). Zero patients demonstrated PD-L1 tumor expression pre-RT. Post-RT, 5 patients (10.9%) demonstrated PD-L1 tumor expression (p = 0.056). Tumor associated macrophages (TAM) expression of PD-L1 increased after RT: 15.2% to 45.7% (p = 0.003). Samples from controls demonstrated no baseline (0%) or change in tumor PD-L1 expression. Freedom from DM was lower for patients with PD-L1 TAM expression post-RT (3 years: 49.7% vs. 87.8%, log-rank p = 0.006); TAM PD-L1 positivity remained an independent predictor for DM on multivariate analyses (Hazard ratio – 0.16, 95% confidence interval: 0.034–0.721, p = 0.042). PD-L1 expression on human STS tumor and TAM appears to elevate after pre-operative RT. Expression of PD-L1 on TAM after RT was associated with a higher rate of DM.

Abstract

We demonstrate transmission of a 200-Gb/s superchannel over 3-km single-mode optical fiber (SMF) targeting intra-data center interconnect applications using a novel single-section quantum-well passive-mode-locked laser that generates an optical frequency comb. The transmitted optical signal consists of 16 subcarriers filtered from the optical frequency comb of the laser and modulated with an advanced modulation format of Baudrate-pilot-aided quadrature phase-shift keying (QPSK) modulation that enables direct detection. Bit error rates below the forward error correction limit have been confirmed for all subcarriers, and the detailed noise characteristics of the passive-mode-locked laser have been investigated to support the understanding of the system performance.

Abstract

There is evidence that forest resiliency is declining in the western US due to recent increases in both areas burned by wildfire and the number of large fires. Fire refugia may increase forest resiliency; however, for land managers to incorporate fire refugia into their management plans, methods need to be developed to identify and rank criteria for what make fire refugia important. As part of a larger effort to build a spatially explicit ranking model for unburned islands in the inland northwestern US, we investigated the perceived importance of criteria used to inform a ranking model to identify high-value fire refugia. We developed a survey targeting land managers within the inland northwestern US. Participants were asked to score a predetermined list of criteria by their importance for determining the value of fire refugia. These scores were analyzed to identify trends among respondents that could be used to develop a fire refugia ranking model. The results indicate that respondents generally organized criteria into two groups: Human infrastructure and wildlife habitat. However, there was little consensus among respondents in their scoring of fire refugia importance criteria, suggesting that a single region-wide fire refugia ranking model may not be feasible. More research with a larger sample size is needed to develop targeted ranking models.

Abstract

The small ubiquitin-like modifier (SUMO) is a protein that regulates a wide variety of cellular processes by covalent attachment of SUMO moieties to a diverse array of target proteins. Sumoylation also plays an important role in the replication of many viruses. Previously, we showed that Kaposi's sarcoma-associated herpesvirus (KSHV) encodes a SUMO-ligase, K-bZIP, which catalyzes sumoylation of host and viral proteins. We report here that this virus also encodes a gene that functions as a SUMO-targeting ubiquitin-ligase (STUbL) which preferentially targets sumoylated proteins for degradation. K-Rta, the major transcriptional factor which turns on the entire lytic cycle, was recently found to have ubiquitin ligase activity toward a selected set of substrates. We show in this study that K-Rta contains multiple SIMs (SUMO interacting motif) and binds SUMOs with higher affinity toward SUMO-multimers. Like RNF4, the prototypic cellular STUbL, K-Rta degrades SUMO-2/3 and SUMO-2/3 modified proteins, including promyelocytic leukemia (PML) and K-bZIP. PML-NBs (nuclear bodies) or ND-10 are storage warehouses for sumoylated proteins, which negatively regulate herpesvirus infection, as part of the intrinsic immune response. Herpesviruses have evolved different ways to degrade or disperse PML bodies, and KSHV utilizes K-Rta to inhibit PML-NBs formation. This process depends on K-Rta's ability to bind SUMO, as a K-Rta SIM mutant does not effectively degrade PML. Mutations in the K-Rta Ring finger-like domain or SIM significantly inhibited K-Rta transactivation activity in reporter assays and in the course of viral reactivation. Finally, KSHV with a mutation in the Ring finger-like domain or SIM of K-Rta replicates poorly in culture, indicating that reducing SUMO-conjugates in host cells is important for viral replication. To our knowledge, this is the first virus which encodes both a SUMO ligase and a SUMO-targeting ubiquitin ligase that together may generate unique gene regulatory programs.

Abstract

Recent studies identifying putative truncated androgen receptor isoforms with ligand-independent activity have shed new light on the acquisition of androgen depletion independent (ADI) growth of prostate cancer. In this study, we present a model system in which a C-terminally truncated variant of androgen receptor (TC-AR) is inducibly expressed in LNCaP, an androgen-dependent cell line, which expresses little truncated receptor. We observed that when TC-AR is overexpressed, the endogenous full length receptor (FL-AR) is transcriptionally downmodulated. This in essence allows us to "replace" FL-AR with TC-AR and compare their individual properties in exactly the same genetic and cellular background, which has not been performed before. We show that the TC-AR translocates to the nucleus, activates transcription of AR target genes in the absence of DHT and is sufficient to confer ADI growth to the normally androgen dependent LNCaP line. We also show that while there is significant overlap in the genes regulated by FL- and TC-AR there are also differences in the respective suites of target genes with each AR form regulating genes that the other does not. Among the genes uniquely activated by TC-AR is RHOB which is shown to be involved in the increased migration and morphological changes observed in LN/TC-AR, suggesting a role of RHOB in the regulation of androgen-independent behavior of prostate cancer cells.
