1. HIV and induced abortion among migrants from sub-Saharan Africa living in Île-de-France: Results of the PARCOURS study

Abstract

Introduction: HIV heavily affects sub-Saharan African women living in France and can impact reproductive decisions. It was investigated whether HIV was associated with induced abortion in pregnancies held after migration by women from sub-Saharan Africa living in Île-de-France. Methods: We used data on ANRS Parcours, a retrospective life event survey conducted in health facilities in the metropolitan region of Paris, between February 2012 and May 2013, with migrants from sub-Saharan Africa. Data on the history of pregnancies were collected among women living with HIV (HIV group) and those attending primary care centers (reference group). We investigated 242 women in the reference group, who had 729 pregnancies, and the 277 women in the HIV group, who had 580 pregnancies. The association between abortion and HIV was evaluated using clustered logistic models, successively adjusted for women and pregnancy characteristics, for the whole sample, and stratified by pregnancy intendedness. Results: In the reference group, 11.0 % of pregnancies were terminated in abortion, the same situation as 14.1 % in the HIV group (p = 0.124). HIV was not associated with abortion in the crude and adjusted models. However, after adjustments, HIV exhibited a non-significant trend towards reducing the likelihood of abortion, particularly when considering the intendedness of pregnancy variable. Conclusions: Factors that shape the overall context of women's lives and pregnancies, which are shared with the reference group, may have a more significant impact on reproductive decision-making than HIV alone. Health services must pay attention to the intendedness of pregnancies, providing advice and support on the prevention of mother-to-child transmission to women living with HIV who intend to become pregnant, in addition to strengthening the provision of family planning and the prevention of unintended pregnancies.

Abstract

While breast cancer is the most common cancer globally, not all women have access to quality care. Long considered a disease of high-income countries, low- and middle-income countries are now facing a major public health issue regarding cancer. This is the case in sub-Saharan Africa, where access to quality breast cancer treatment is often lacking. This context leads some women to travel abroad, particularly to France, hoping to save their lives. This article aims to examine the experiences, the social cost and the perceptions associated with therapeutic mobility for breast cancer in France. To this end, this study uses a qualitative method based on nineteen biographical interviews with women from sub-Saharan Africa with breast cancer who have migrated to France seeking for treatment. It shows that these women often face significant difficulties throughout their care and life paths in France. Main issues lay in accessing care, housing insecurity, loneliness, and administrative instability. This reflects how the double biographical disruption, caused by cancer and migration, impact entire disease trajectories, from entry to care to post-breast cancer period. With this research, we suggest extending the concept of social death to the issue of women who have undergone therapeutic mobility for their breast cancer in France. Highlighting the experiences of women who remain almost invisible in the public arena, this article analyses poorly heard realities and shows how social inequalities in health can be found even in reconstructing oneself after breast cancer.

Abstract

Introduction: Biomedical HIV prevention tools are available in France to prevent new infections. However, evidence suggests a lack of knowledge of these tools among sub-Saharan African immigrants, who are particularly affected by HIV due to social hardship, an indirect factor of HIV acquisition. We analysed the impact of an empowerment-based intervention on the knowledge of treatment as prevention (TasP), pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis (PEP) in a population of precarious sub-Saharan African immigrants. Methods: Data were collected throughout the MAKASI project. Following an outreach approach, participants were recruited in public places based on their precarious situations and followed for six months (0, 3, 6 months) between 2018 and 2021. Participants were randomized into two groups and received an empowerment intervention sequentially (stepped wedge design). We used random-effects logistic regression models to evaluate the intervention effect on the knowledge of biomedical HIV prevention tools. ClinicalTrials.gov Identifier: NCT04468724. Results: The majority of the participants were men (77.5%), and almost half of them had arrived in France within 2 years prior to inclusion (49.3%). Abstract

Abstract Background The management of chronic diseases such as diabetes, obesity and high blood pressure is a major global health challenge, particularly among the most disadvantaged populations. Beyond the biomedical management of these diseases, comprehensive support that takes into account the peoples’ economic and social situation is fundamental. The objective of this scoping review is to create an inventory and an analysis of the different types of support for these chronic diseases among disadvantaged, immigrant or minority populations to contribute to a better definition and characterization of what should be global support for these vulnerable populations suffering from these diseases. Methods A search of PubMed, PsycINFO, Sages Journals and Web of Science was conducted (between March and May 2021) for articles published between January 2000 and May 2021. Articles were selected after screening titles, abstracts and full texts according to our 5 inclusion criteria. Results We included 16 articles. The diabetes, obesity and high blood pressure support programs described in these articles operate to improve physical and mental health and access to care. The approaches of these interventions are focused on the training and participation of people and the implementation of support actions adapted to the person. The majority of these interventions have a real attachment to the community. Conclusions This review of the literature shows that support for people with chronic diseases such as diabetes, obesity or high blood pressure is based on three pillars: empowerment, peer mediation and holistic and tailor-made support for the individual. The empowerment approach, which considers the capacities and resources of individuals and whose goal is to strengthen their ability to act on their health, appears to be entirely suited to the support of these chronic diseases. This review underlines the importance of moving away from a biomedical approach to a holistic approach truly focused on the person, their capacities and their needs.

5. Moderation of the association between COVID-19-related income loss and depression by receipt of financial support: Repeated cross-sectional surveys of young adults in Canada and France (2020–2021)

Abstract

Background: To mitigate the adverse effects of the COVID-19 pandemic on financial resources, governments and family/friends mobilized financial support interventions (e.g., emergency aid funds) and assistance. However, little is known about how financial assistance alleviated mental health problems. This study aimed to investigate the moderating effect of financial support from the government or from family/friends on the association between income loss and depression among young adults. Methods: Two online cross-sectional surveys among young adults ages 18–29 living in Canada and France were conducted in 2020 (n = 4,511) and 2021 (n = 3,329). Moderate-to-severe depressive symptoms were measured using the Patient Health Questionnaire-9 (cut-off score: ≥10). Two logistic regression models were performed for each survey with an interaction term between income loss and financial support (government or family/friends modeled separately), controlling for demographics. Results: Overall, half reported depressive symptoms (2020/2021: 53.5%/45.6%), and over a third lost income (2020/2021: 10.2%/11.6% all income, 37.7%/21.6% some income). In 2020, 40.6% received government financial support (17.7% in 2021) while family/friends support was received by 12% (in both surveys). In both surveys, among those who received governmental financial support, income loss was associated with depression, whether participants lost all their income (e.g., 2020: Adjusted Odds Ratios (AOR) 1.75, 95% Confidence Interval [1.29–2.44]), or some of their income (e.g., 2020: AOR 1.45 [1.17–1.81]). However, among those who received family/friends financial support, income loss was no longer significantly associated with depression in both cycles, whether participants lost all their income (e.g., 2020: AOR 1.37 [0.78–2.40]), or some of their income (e.g., 2020: AOR 1.31 [0.86–1.99]). Conclusions: Association between income loss and depression was moderated by receipt of family/friends financial support but not by receipt of government financial support. Financial support interventions may help to mitigate the negative effects of income loss on young adults mental health during periods of economic crisis.

Abstract

Abstract Background Since climate change, pandemics and population mobility are challenging healthcare systems, an empirical and integrative research to studying and help improving the health systems resilience is needed. We present an interdisciplinary and mixed-methods research protocol, ClimHB, focusing on vulnerable localities in Bangladesh and Haiti, two countries highly sensitive to global changes. We develop a protocol studying the resilience of the healthcare system at multiple levels in the context of climate change and variability, population mobility and the Covid-19 pandemic, both from an institutional and community perspective. Methods The conceptual framework designed is based on a combination of Levesque’s Health Access Framework and the Foreign, Commonwealth and Development Office’s Resilience Framework to address both outputs and the processes of resilience of healthcare systems. It uses a mixed-method sequential exploratory research design combining multi-sites and longitudinal approaches. Forty clusters spread over four sites will be studied to understand the importance of context, involving more than 40 healthcare service providers and 2000 households to be surveyed. We will collect primary data through questionnaires, in-depth and semi-structured interviews, focus groups and participatory filming. We will also use secondary data on environmental events sensitive to climate change and potential health risks, healthcare providers’ functioning and organisation. Statistical analyses will include event-history analyses, development of composite indices, multilevel modelling and spatial analyses. Discussion This research will generate inter-disciplinary evidence and thus, through knowledge transfer activities, contribute to research on low and middle-income countries (LMIC) health systems and global changes and will better inform decision-makers and populations.

IntroductionIn France, combination prevention tools, particularly antiretroviral treatment for HIV prevention has been available for several years. We described the knowledge of these antiretroviral treatments among immigrants from sub-Saharan Africa, who are particularly affected by HIV, and the factors associated with this knowledge.MethodsThe data come from the Makasi study, which was conducted between 2019 and 2020 among precarious immigrants from sub-Saharan Africa recruited through a community-based outreach approach in the greater Paris area (n = 601). We described levels of knowledge of HIV treatment effectiveness (HTE), treatment as prevention (TasP), post-exposure prophylaxis (PEP), and pre-exposure prophylaxis (PrEP), by sex with chi2 test. We investigated factors associated with their knowledge with logistic regressions adjusted for sociodemographic characteristics, living conditions and sexual behaviors (p≤0.2).ResultsRespondents were mostly men (76%), from West Africa (61%), in precarious situation: 69% were unemployed, 74% were undocumented and 46% had no health coverage. Among this population, knowledge of HIV preventive treatments was heterogeneous. While HTE was well known (84%); TasP was known by only half of the respondents (46%), and PEP and PrEP were hardly known: 6% and 5%, respectively. Multivariate regressions models showed that these antiretroviral treatments for the prevention of HIV was better known by people with a higher level of education (PEP: aOR = 3.33 [1.09-10.20], p = 0.03; HTE: aOR = 4.33 [1.87-10.04], pConclusionsThere is a need for specific communication on antiretroviral treatment for HIV prevention that targets sub-Saharan immigrants, particularly those who have no access to the health-care system and those who are less educated.

Abstract

Abstract Background Many respondent-driven sampling (RDS) methodologies have been employed to investigate hard-to-reach populations; however, these methodologies present some limits. We describe a minimally investigated RDS methodology in which peer recruitment and interviewing are phone-based. The feasibility of the methodology, field experiences, validity of RDS assumptions and characteristics of the sample obtained are discussed. Methods We conducted a phone-based RDS survey among men who have sex with men (MSM) aged 18 or above and living in Côte d’Ivoire. Eight initial MSM across Côte d’Ivoire were selected. Participants were asked to call a hotline to be registered and interviewed by phone. After the participants completed the questionnaire, they were asked to recruit a maximum of 3 MSM from their acquaintances. Results During the 9 months of the survey, 576 individuals called the hotline, and 518 MSM completed the questionnaire. The median delay between the invitation to participate and the completion of the questionnaire by peer-recruited MSM was 4 days [IQR: 1–12]. The recruitment process was not constant, with high variation in the number of people who called the hotline during the survey period. RDS chain convergence to equilibrium was reached within 6 waves for most of the selected variables. For the network size estimation assumption, participants who incorrectly estimated their network size were observed. Regarding the sample obtained, MSM were recruited from all the regions of Côte d’Ivoire with frequent interregional recruitment; 23.5% of MSM were recruited by someone who does not live in the same region. Compared to the MSM who participated in two other surveys in Côte d’Ivoire, the MSM in our sample were less likely to know about an MSM non-governmental organisation. However, MSM aged 30 years old and above and those with a low level of education were underrepresented in our sample. Conclusion We show that phone-based RDS surveys among MSM are feasible in the context of sub-Saharan Africa. Compared to other classical RDS survey methodologies, the phone-based RDS methodology seems to reduce selection bias based on geography and proximity with the MSM community. However, similar to other methodologies, phone-based RDS fails to reach older and less-educated MSM.

Abstract Background Sub-Saharan and Caribbean immigrants are particularly affected by HIV in Europe, and recent evidence shows that a large portion of them acquired HIV after arrival. There is a need for efficient interventions that can reduce immigrants’ exposure to HIV. We describe the pilot phase of a community-based empowerment outreach intervention among sub-Saharan and Caribbean immigrants in the greater Paris area aimed at 1) constructing the intervention, 2) assessing its feasibility, and 3) assessing the feasibility of its evaluation based on a stepped-wedge approach. Methods 1) To develop the intervention, a literature review was conducted on existing interventions and participatory approaches developed, including the constitution of peer groups. 2) To assess the intervention’s feasibility, a pilot was conducted between April 2018 and December 2018. A daily register was used to collect data on sociodemographic characteristics of all persons who visited the mobile team to assess eligibility and acceptability. 3) To assess the feasibility of performing a stepped-wedge trial to evaluate the intervention, we compared eligibility, enrolment and retention at 3 months in two arms (immediate vs deferred). Chi-squared tests were used to compare reach and retention between the two arms. Results Intervention development. The Makasi intervention was designed as an outreach intervention that starts with the persons’ capacities and helps them appropriate existing resources and information and obtain knowledge about sexual health, based upon motivational interviewing techniques. Intervention Feasibility . Between April 2018 and December 2018, a total of 485 persons were identified as eligible. Participation in the intervention was proposed to 79% of eligible persons. When proposed, the persons enrolled in the intervention with a response rate of 69%. Some were lost to follow-up, and 188 persons were finally included. Evaluation Feasibility. The proportions of eligible (45 and 42%) individuals and of enrolled individuals (65 and 74%) were similar and not significantly different in the immediate and deferred arms, respectively. Conclusions A community-based outreach intervention aimed at improving sub-Saharan and Caribbean immigrants’ empowerment in sexual health is feasible. The pilot phase was key to identifying challenges, designing a relevant intervention and validating the stepped-wedge protocol for evaluation.

Abstract

In order to limit the spread of the SARS-CoV-2 virus, the majority of governments have introduced population containment. Certain population groups, including immigrants in precarious situations, are experiencing the impact of this measure in a brutal manner. This article is based on accounts of containment experiences collected by telephone within the framework of a pre-existing intervention research carried out among immigrants to France from Sub-Saharan Africa who are in a precarious situation. It highlights certain social effects of containment and the logics at work in the precarious situations. This research shows how this a priori unprecedented situation affects individual capacities to act and generates a ‘disaffiliation process’ causing individuals to shift towards ‘social non-existence’, repeating lived experiences and exacerbating pre-existing logics. The ordeal of containment proves to be an ordinary experience for these individuals.

In Europe, migrants are at higher risk of common mental disorders or psychological distress than are natives. Little is known regarding the social determinants of migrant mental health, particularly the roles played by migration conditions and transnational practices, which may manifest themselves in different ways for men and for women. The goal of this paper was to understand the gendered roles of migration paths and transnational ties in mental health among sub-Saharan African migrants residing in the Paris, France, metropolitan area. This study used data from the Parcours study conducted in 2012–2013, which employed a life-event approach to collect data from a representative sample of migrants who visited healthcare facilities (n = 2468). We measured anxiety and depressive symptoms at the time of data collection with the Patient Health Questionnaire-4 (PHQ-4). Reasons for migration, the living conditions in the host country and transnational ties after migration were taken into account by sex and after adjustment. Our study demonstrates that among sub-Saharan African migrants, mental health is related to the migratory path and the migrant’s situation in the host country but differently for women and men. Among women, anxiety and depressive symptoms were strongly related to having left one’s home country because of threats to one’s life. Among men, residing illegally in the host country was related to impaired mental health. For both women and men, cross-border separation from a child less than 18 years old was not independently associated with anxiety and depressive symptoms. In addition, social and emotional support from relatives and friends—both from the society of origin and of destination—were associated with lower anxiety and depressive symptoms. Migrant mental health may be impaired in the current context of anti-migrant policies and an anti-immigrant social environment in Europe.

Les personnes nées en Afrique subsaharienne constituent en France l’une des populations les plus touchées par l’hépatite B et par l’infection VIH. Pour ajuster les stratégies de santé publique aux besoins propres de ces migrants, il est nécessaire de mieux comprendre ce qui accroît leur vulnérabilité face à ces infections, en particulier les expériences de précarité qui peuvent accompagner l’installation en France. Si la précarité des migrants est souvent évoquée, celle-ci est en réalité relativement mal mesurée ; en particulier la fréquence des précarités et leur place dans la trajectoire individuelle sont mal connues.Afin d’étudier comment les trajectoires de santé, les trajectoires sociales, familiales, migratoires et administratives s’entrecroisent et se construisent les unes par rapport aux autres parmi les Africains vivant en France, une enquête a été menée en 2012-2013 en Île-de-France, l’enquête Parcours pour « Parcours de vie, VIH et hépatite B chez les migrants subsahariens vivant en Île-de-France ». Cette enquête biographique, réalisée dans 70 services de santé, est la première en France à utiliser la méthode de collecte biographique démographique pour des recherches dans le champ de la santé, en impliquant des professionnels de santé.Nous présentons ici la méthodologie et la collecte de l’enquête Parcours et nous discutons des leçons tirées de cette expérience d’une enquête biographique en structures de santé et auprès de migrants. Puis nous présentons comment deux types de précarité vécues en France, la précarité résidentielle et la précarité administrative, ont été caractérisées sur l’ensemble des trajectoires biographiques. Enfin nous estimons le niveau de ces précarités lors de l’arrivée en France, leur évolution au long du séjour en France et leurs déterminants.

Using data from the ANRS-PARCOURS study, we aimed to assess the prevalence of forced sex after migration and its association with post-migration acquisition of HIV as well as the circumstances of forced sex after migration, including housing and administrative insecurity, among sub-Saharan African migrant women living in the Paris Region, France. Methods: The ANRS-PARCOURS study was a retrospective life-event survey done between February, 2012, and May, 2013, in health-care facilities in the Paris region of France. Women were eligible if they were born in sub-Saharan Africa, aged between 18 and 59 years, and had been diagnosed with HIV infection at least 3 months earlier for women receiving HIV care or not diagnosed with HIV. In this analysis, we used ANRS-PARCOURS study data to compare the incidence of forced sex after migration in three groups of sub-Saharan African migrant women: those who acquired HIV after migrating, those who acquired HIV before migrating, and those without HIV. We assessed the associations between forced sex, sexual partnerships, and living conditions after migration with mixed-effects logistic regression and generalised structural equation models. The study is registered with ClinicalTrials.gov, number NCT02566148. Findings: We obtained data from 980 eligible individuals who participated in the ANRS-PARCOURS study (407 without HIV and 573 HIV-positive) from 54 randomly selected health-care facilities. We excluded 20 women whose HIV infection could not be dated and eight women with missing data from the analyses, for a total of 405 women in the reference group (without HIV) and 547 women in the HIV group (156 with post-migration HIV acquisition, 391 with pre-migration HIV). Women who acquired HIV after migration experienced forced sex after migration more frequently than women without HIV (24 [15%] vs 18 [4%]; p=0·001). Forced sex after migration was associated with being hosted by family or friends (β=0·95, 95% CI 0·19–1·72) and lack of stable housing (β=1·10, 0·17–2·03). Lack of a residence permit was also associated with forced sex after migration. Interpretation: The social hardships faced by sub-Saharan African migrant women after migration, especially a lack of housing or lack of a residence permit, increases their exposure to sexual violence and to HIV infection. Abstract

Migrants' access to care depends on their health insurance coverage in the host country. We aimed to evaluate in France the dynamic and the determinants of health insurance coverage acquisition among sub-Saharan migrants.In the PARCOURS life-event retrospective survey conducted in 2012-2013 in health-care facilities in the Paris region, data on health insurance coverage (HIC) each year since arrival in France has been collected among three groups of sub-Saharan migrants recruited in primary care centres (N = 763), centres for HIV care (N = 923) and for chronic hepatitis B care (N = 778). Year to year, the determinants of the acquisition and lapse of HIC were analysed with mixed-effects logistic regression models.In the year of arrival, 63.4% of women and 55.3% of men obtained HIC. But three years after arrival, still 14% of women and 19% of men had not obtained HIC. HIC acquisition was accelerated in case of HIV or hepatitis B infection, for migrants arrived after 2000, and for women in case of pregnancy and when they were studying. Conversely, it was slowed down in case of lack of a residency permit and lack of financial resources for men. In addition, women and men without residency permits were more likely to have lost HIC when they had one.In France, the health insurance system aiming at protecting all, including undocumented migrants, leads to a prompt access to HIC for migrants from sub-Saharan Africa. Nevertheless, this access may be impaired by administrative and social insecurities.

Abstract

Migrants account for 35% of HIV diagnoses in the European Union (ECDC/WHO 2014). Little is known about the impact of such a lifelong infection diagnosis on lives that are already disrupted by migration. In this paper, we assess the impact of HIV diagnosis on activity, union, well-being among African migrants living in France, the second group most affected by HIV after MSM. We compare it with the impact of the diagnosis of Hepatitis B, another lifelong infection affecting African migrants.We use the ANRS PARCOURS survey, a retrospective life-event survey led in 2012-2013 in 74 health structures in Paris greater area which collected 926 life histories of Sub-Saharan migrants living with HIV and 779 with Hepatitis B. We modelled the probability year by year since 18 years of age until data collection to lose one's activity, to experience a conjugal break up and degradation of well-being and we estimated the impact of migration and of HIV and Hepatitis B diagnoses on these probabilities, after adjustment on other factors, thanks to discrete-time logistic regressions.Migration entailed loss of activity and conjugal break up, though HIV diagnosis after migration did not statistically impact on these outcomes. Yet HIV diagnosis had a massive negative impact on well-being (aOR = 11.31 [4.64-27.56] for men and 5.75 [2.79-11.86] for women). This negative impact on well-being tended to diminish for persons diagnosed after 2004. The negative impact of HIV diagnosis on African migrants' well-being seems to be attenuated in the last decade, which hints at a normalization of the subjective experience of HIV diagnosis.

Abstract

HIV testing is an important tool in the management of the HIV epidemic among key populations. We aimed to explore the dynamic of first-time HIV testing in France for sub-Saharan migrants after their arrival.ANRS-Parcours is a retrospective life-event survey conducted from 2012 to 2013 in healthcare facilities in the Paris region, among 926 sub-Saharan HIV-infected migrants and 763 non-infected migrants. After describing the time to first HIV test in France and associated circumstances, we performed a discrete-time logistic regression to analyze the influence of socioeconomic position, contact with the healthcare system and sexual behaviors, on first-time HIV testing in France in migrants who arrived after 2000.Median first-time HIV testing occurred during the second year spent in France for non-infected men and women in both groups, and during the first year for men of the HIV group. The probability of testing increased with hospitalization and pregnancy for women of both groups. For non-infected men unemployment and absence of a residence permit were associated with an increased probability of HIV testing [respectively, OR = 2.2 (1.2-4.1) and OR = 2.0 (1.1-3.5)]. Unemployment was also associated with an increased probability of first-time HIV-testing for women of the HIV group [OR: 1.7 (1.0-2.7)]. Occasional and multiple sexual relationships were associated with an increased probability of first-time testing only for HIV-infected women [OR: 2.2 (1.2-4.0) and OR = 2.4 (1.3-4.6)].Access to first HIV testing in France is promoted by contact with the health care system and is facilitated for unemployed and undocumented migrants after arrival.However, testing should be offered more systematically and repeated in order to reduce time between HIV infection and diagnosis, especially for deprived people which are particularly vulnerable regarding HIV infection.

In European countries, chronic hepatitis B (CHB) disproportionately affects migrants from medium- and high-endemic areas and is largely underdiagnosed. To inform policy and improve screening strategies, we measured the timing of CHB diagnosis after migration and its determinants among sub-Saharan migrants living in the Paris metropolitan area (France).The PARCOURS study is a retrospective life-event history survey conducted in health care services in 2012-2013 among 779 migrants from sub-Saharan Africa who were receiving care for CHB. We investigated the timing of CHB diagnosis from the time of arrival in France using the Kaplan-Meier method and characteristics associated with CHB diagnosis since the time of arrival in France using discrete-time multivariate logistic regression models.The median CHB diagnosis occurred during the fourth year spent in France for men and during the second year spent in France for women. Among men, the probability of CHB diagnosis increased during years with (versus without) a temporary resident permit (aOR: 1.6, 95%CI: 1.1-2.2), a precarious accommodation (aOR: 1.7, 95%CI: 1.1-2.6), and hospitalization (aOR: 7.7, 95%CI: 3.4-15.1). Among women, CHB diagnosis was more likely to occur during years with unemployment (aOR: 1.9, 95%CI: 1.1-3.94), pregnancy (aOR: 6.6, 95%CI: 3.5-12.5) and hospitalization (aOR: 9.0, 95%CI: 2.95-32.3). For both sexes, the probability of CHB diagnosis was higher among those who migrated to France because they were threatened in their country.This study shows that social hardships (residential, economic, administrative) and contact with the health care system after arrival in France hasten access to a CHB diagnosis.

Abstract

BACKGROUND: Men who have sex with other men (MSM) are a vulnerable population in Africa that has been insufficiently explored. Given the high rate of bisexuality among MSM (73% in the past year), it is important to understand their risk-taking behaviors regarding both men and women. METHODOLOGY/PRINCIPAL FINDINGS: A socio-behavioral survey was carried out in 2007 among 501 MSM recruited using the snowball sampling method. We explore in this article why a condom was not used during last sexual intercourse with a man and with a woman, taking into account the respondent's characteristics, type of relationship and the context of the sexual act. In the survey, 489 men reported that they had had sexual intercourse at least once with another man during the previous year, and 358 with a man and with a woman. The main risk factors for not using a condom at last sexual intercourse with another man were having sex in a public place (aOR = 6.26 [95%CI: 2.71-14.46]), non-participation in an MSM prevention program (aOR = 3.47 [95%CI: 2.12-5.69]), a 19 years old or younger partner (aOR = 2.6 [95%CI: 1.23-4.53]), being 24 years or younger (aOR = 2.07 [95%CI: 1.20-3.58]) or being 35 years or over (aOR = 3.08 [95%CI:1.11-8.53]) and being unemployed (aOR = 0.36 [95%CI: 0.10-1.25]). The last sexual intercourse with the respondent's wife was hardly ever protected (2%). With women, the other factors were a 15 years or younger partner (aOR = 6.45 [95%CI: 2.56-16.28]), being educated (primary: aOR = 0.45 [95%CI: 0.21-0.95], secondary or higher: aOR = 0.26 [95%CI: 0.11-0.62]), being a student (aOR = 2.20 [95%CI: 1.07-4.54]) or unemployed (aOR = 3.72 [95%CI: 1.31-10.61]) and having participated in a MSM prevention program (aOR = 0.57 [95%CI: 0.34-0.93]). CONCLUSION: Having participated in a prevention program specifically targeting MSM constitutes a major prevention factor. However, these programs targeting MSM must address their heterosexual practices and the specific risks involved.

23. The MAKASI empowerment randomized intervention and depression and loneliness among sub-Saharan immigrants living in the greater Paris area

Abstract

Purpose The MAKASI intervention aimed to empower sub-Saharan African immigrants living in precarious situations in the Paris metropolitan area. Because there are factors specifically related to immigration that may increase the risk for common mental disorders, the present study aimed to examine participants' levels of depression and loneliness and analyze the effect of the intervention on depression and loneliness. Methods The MAKASI study was designed as a stepped-wedge randomized intervention trial. Study participants were recruited through an outreach program led by a nongovernmental organization and randomly assigned to two groups, with an intervention delay of three months between groups. Both groups were assessed for six months after inclusion and the effect of the intervention on depression and loneliness was assessed using generalized linear mixed models. The study was conducted from 2018 to 2021 and we took in consideration whether being interviewed during one of the Covid-19 confinement had an effect on the results. Results A total of 821 subjects participated in the Makasi study. High levels of depression and loneliness were found in the study population. We found no effect of the intervention on depression [95%CI 0.77 to 2.40]. Similarly, no effect of the intervention was found on loneliness [95%CI 0.87 to 2.54]. Conclusions The intervention we tested did not appear to improve the level of depression and loneliness among participants. However, the high prevalence of mental and emotional problems in the study population suggests a public health crisis among immigrants in the greater Paris area. Abstract

BackgroundThe often-precarious life circumstances of undocumented migrants are likely to heighten the detrimental impact of the COVID-19 pandemic on their lives. Given the paucity of research exploring how undocumented migrants are affected by the COVID-19 pandemic, we set out to explore the association between being an undocumented migrant and a range of social and mental health measures. MethodsOur study draws on three complementary surveys conducted among migrants in France between April 1st and June 7th 2020 (APART TOGETHER, MAKASI, ECHO; n = 716). We tested associations between eight outcome measures, covering health literacy, prevention behaviours, perceptions of government responses, livelihoods and mental health (PHQ-9 score), and the participants legal status as either undocumented or documented. We modelled the probability of food insecurity increase, job loss, depression, and responses to SARS-COV-2 symptoms with logistic regression models, adjusted for age, gender and legal status. ResultsUndocumented migrants had a higher probability of experiencing food insecurity increase (aORs=10.40 [3.59, 30.16], and 2.19 [1.39, 3.50] in APART TOGETHER and ECHO), a higher probability of depression (aOR=2.65 [1.01, 6.97] in MAKASI). In all three surveys, undocumented migrants were more likely to lose their job (aORs=6.51 [1.18, 36.00], 8.36 [1.08, 64.70] and 3.96 [1.79, 9.16] in APART TOGETHER, MAKASI and ECHO respectively). ConclusionOur results suggest that the lives of undocumented migrants have been dramatically worsened by the COVID-19 pandemic, exposing and amplifying the inequalities facing this group. There is an urgent need for action to address these inequalities. Abstract

Immigrants in Western countries have been particularly affected by the COVID-19 crisis.We analysed excess mortality rates among the foreign-born population and changes in their distinctive mortality profiles ("migrant mortality advantage") during the first pandemic wave in France.Deaths from all causes in metropolitan France from March 18 to May 19, 2020 were used, with information on sex, age, region of residence and country of birth. Similar data from 2016 through 2019 were used for comparisons.During the pre-pandemic period (2016-2019), immigrant populations (except those from Central and Eastern Europe) had lower standardized mortality rates than the native-born population, with a particularly large advantage for immigrants from sub-Saharan Africa. In the regions most affected by COVID-19 (Grand-Est and Île-de-France), the differences in excess mortality by country of birth were large, especially in the working-age groups (40-69 years), with rates 8 to 9 times higher for immigrants from sub-Saharan Africa, and about 3 to 4 times higher for immigrants from North Africa, from the Americas and from Asia and Oceania relative to the native-born population. The relative overall mortality risk for men born in sub-Saharan Africa compared to native-born men, which was 0.8 before the pandemic, shifted to 1.8 during the first wave (0.9 to 1.5 for women). It also shifted from 0.8 to 1.1 for men from North Africa (0.9 to 1.1 for women), 0.7 to 1.0 for men from the Americas (0.9 to 1.3 for women), and 0.7 to 1.2 for men from Asia and Oceania (0.9 to 1.3 for women).Our findings shed light on the disproportionate impact of the first wave of the pandemic on the mortality of populations born outside Europe, with a specific burden of excess mortality within the working-age range, and a complete reversal of their mortality advantage.

Abstract

Background Many respondent-driven sampling (RDS) methodologies have been employed to investigate hard-to-reach populations; however, these methodologies present some limits. We describe a minimally investigated RDS methodology in which peer recruitment and interviewing are phone-based. The feasibility of the methodology, field experiences, validity of RDS assumptions and characteristics of the sample obtained are discussed. Methods We conducted a phone-based RDS survey among men who have sex with men (MSM) aged 18 or above and living in Côte d’Ivoire. Eight initial MSM across Côte d’Ivoire were selected. Participants were asked to call a hotline to be registered and interviewed by phone. After the participants completed the questionnaire, they were asked to recruit a maximum of 3 MSM from their acquaintances. Results During the 9 months of the survey, 576 individuals called the hotline, and 518 MSM completed the questionnaire. The median delay between the invitation to participate and the completion of the questionnaire by peer-recruited MSM was 4 days [IQR: 1–12]. The recruitment process was not constant, with high variation in the number of people who called the hotline during the survey period. RDS chain convergence to equilibrium was reached within 6 waves for most of the selected variables. For the network size estimation assumption, participants who incorrectly estimated their network size were observed. Regarding the sample obtained, MSM were recruited from all the regions of Côte d’Ivoire with frequent interregional recruitment; 23.5% of MSM were recruited by someone who does not live in the same region. Compared to the MSM who participated in two other surveys in Côte d’Ivoire, the MSM in our sample were less likely to know about an MSM non-governmental organisation. However, MSM aged 30 years old and above and those with a low level of education were underrepresented in our sample. Conclusion We show that phone-based RDS surveys among MSM are feasible in the context of sub-Saharan Africa. Compared to other classical RDS survey methodologies, the phone-based RDS methodology seems to reduce selection bias based on geography and proximity with the MSM community. However, similar to other methodologies, phone-based RDS fails to reach older and less-educated MSM.

Abstract

International audience; Practices of Provider-Initiated HIV Testing and Counseling (PITC) remains suboptimal in Côte d’Ivoire. The aim of this survey was to identify the practices and obstacles to PITC among healthcare professionals in Côte d'Ivoire. A nationally representative cross-sectional survey was conducted in 2018 by telephone among three separate samples of midwives, nurses and physicians practicing in Côte d'Ivoire. The number of HIV tests proposed during consultation in the month preceding the survey was collected for each professional. Factors associated with the number of proposed tests were identified through ordinal logistic regression models. A total of 298 midwives, 308 nurses and 289 physicians were interviewed. Midwives proposed the test more frequently, followed by nurses and physicians. Among midwives, a higher number of proposed tests was associated with the perception that HIV testing does not require specific consent compared to other diseases (aOR 4.00 [95% CI 1.37–14.29]). Among nurses, having received HIV training and the presence of community HIV counselors were associated with a higher number of proposed tests (aOR 2.01 [1.31–3.09] and aOR 1.75 [1.14–2.70], respectively). For physicians, the presence of a voluntary testing center was associated with a higher number of proposed tests (aOR 1.69 [1.01–2.86]). PITC practices and barriers differed across professions. Beyond improving environmental opportunities such as dedicated staff or services, strengthening the motivations and capabilities of healthcare professionals to propose testing could improve PITC coverage.; Las prácticas de Asesoramiento y Pruebas de VIH Iniciadas por el Proveedor (APIP) siguen siendo subóptimas en Côte d'Ivoire. El objetivo de este estudio fue identificar las prácticas y los obstáculos a las APIP entre los profesionales de la salud en Côte d'Ivoire. En 2018 se llevó a cabo una encuesta transversal representativa a nivel nacional por teléfono entre tres muestras separadas de parteras, enfermeras y médicos que ejercen en Côte d'Ivoire. Se recogió para cada profesional el número de pruebas de VIH propuestas durante la consulta en el mes anterior a la encuesta. Los factores asociados con el número de pruebas propuestas se identificaron mediante modelos ordinales de regresión logística. Se entrevistó a un total de 298 parteras, 308 enfermeras y 289 médicos. Las parteras propusieron la prueba con mayor frecuencia, seguidas de las enfermeras y los médicos. Entre las parteras, un mayor número de pruebas propuestas se asoció a la percepción de que la prueba del VIH no requiere un consentimiento específico en comparación con otras enfermedades (aOR 4,00 [IC del 95%: 1,37-14,29]). Entre las enfermeras, el haber recibido capacitación sobre el VIH y la presencia de consejeros comunitarios sobre el VIH se asociaron con un mayor número de pruebas propuestas (aOR 2,01 [1,31-3,09] y aOR 1,75 [1,14-2,70], respectivamente). Para los médicos, la presencia de un centro de pruebas voluntarias se asoció con un mayor número de pruebas propuestas (aOR 1,69 [1,01-2,86]). Las prácticas y barreras del APIP difieren entre las profesiones. Más allá de mejorar las oportunidades del entorno, como el personal o los servicios dedicados, el fortalecimiento de las motivaciones y capacidades de los profesionales de la salud para proponer pruebas podría mejorar la cobertura del APIP.

Abstract

Highlights • The precarious immigrant population is often invisible because it is at the interstices of care devices and, in the current epidemic context, isolation facilities. • Confinement affects individuals' abilities to act, and generates a "disaffiliation process" to placing this precarious immigrant population even more on the margins of society. • Covid-19 crisis exacerbates pre-existing difficulties, generating not so new situations for these precarious immigrant population. • Need to formulate specifics interventions : targeted outreach intervention, participatory approach and deliberative spaces ; promote health promotion intervention; right based intervention., In order to limit the spread of the SARS-CoV-2 virus, the majority of governments have introduced population containment. Certain population groups, including immigrants in precarious situations, are experiencing the impact of this measure in a brutal manner. This article is based on accounts of containment experiences collected by telephone within the framework of a pre-existing intervention research carried out among immigrants to France from Sub-Saharan Africa who are in a precarious situation. It highlights certain social effects of containment and the logics at work in the precarious situations. This research shows how this a priori unprecedented situation affects individual capacities to act and generates a ‘disaffiliation process’ causing individuals to shift towards ‘social non-existence’, repeating lived experiences and exacerbating pre-existing logics. The ordeal of containment proves to be an ordinary experience for these individuals.

Abstract

Migration can affect reproductive outcomes due to different socioeconomic and cultural contexts before and after migration, to changes in the affective and conjugal status of women and to their life conditions. The aim of this study is to investigate the association between international migration and abortion. The data came from a retrospective life-event survey from sub-Saharan African women living in Ile-de-France. Differences in abortion distribution before and after migration were assessed using the Pearson chi-square test, and the association between the predictor and the outcome was investigated using Generalized Estimating Equations. A total of 363 women and 1377 pregnancies were investigated. Among these pregnancies, 15.6% that occurred before and 11.0% that occurred after migration was reported as ended in abortion (p = 0.011). The odds of reporting having had an abortion was lower after migration (OR 0.59, 95% CI 0.42–0.84), even after adjustment. However, after including intendedness of pregnancy in the model, this association lost its significance. The difference in induced abortion occurrence between before and after migration is almost entirely due to a change in the intendedness of pregnancy. Thus, socioeconomic and cultural issues have a greater weight in the decision to abort than the legal interdiction of this practice.

Abstract

Background Despite the implementation of Provider Initiated Testing and Counselling (PITC) in 2009, PITC coverage remains low in Cote d'Ivoire. The purpose of this study is to determine whether an human immunodeficiency virus (HIV) test was offered and performed at specific life events where PITC is recommended by national guidelines. Methods In 2017, a cross-sectional telephone survey was conducted among a representative sample of 3,867 adults from the general population in Cote d'Ivoire. The occurrences of the following events over the past 5 years were documented: pregnancy (event A) or partner's pregnancy (event B) of the last child, sexually transmitted infection (event C) and marriage (event D). For each of these events, participants were asked (i) if they consulted a health care professional, (ii) if they were offered an HIV test during that consultation and (iii) if they accepted it. Results Consulting a health care provider was reported by 94.9%, 58.3%, 70.3% and 19.1% of those who reported events A, B, C and D respectively. In case of medical consultations following events A, B, C and D, respectively 70.1%, 33.1%, 28.1%, and 78.8% of individuals were offered an HIV test. The testing acceptance was high regardless of the event. Overall, testing coverage was 63.7%, 16.9%, 13.4% and 14.5% for events A, B, C and D respectively. Conclusions Increasing HIV testing coverage in Cote d'Ivoire requires (i) facilitating attendance to health services in case of sexually transmitted infections, marriage and pregnancy—for men—and (ii) strengthening routine testing offer on these occasions.

Abstract

Les migrations constituent un des moteurs majeurs des dynamiques de population. Elles sont pourtant encore trop peu étudiées sous l’angle de la santé publique, bien que celle-ci s’intéresse à tout ce qui est en lien avec le bien-être et la santé des populations. En France, cela vient en partie de notre modèle d’intégration républicaine, qui ne distingue pas les individus ou les groupes selon des critères d’origine ethnique ou de religion. Cependant, certaines maladies comme le VIH/sida ont montré qu’il pouvait être nécessaire de prendre en compte l’origine des personnes pour améliorer la prévention et l’accès aux soins. Améliorer l’accueil et la prise en charge des immigrés dans le système de santé français passe en effet par une meilleure connaissance de leurs besoins spécifiques.

Abstract

BackgroundLife course epidemiology is now an established field in social epidemiology; in sociodemography, the quantitative analysis of biographies recently experienced significant trend from event history analysis to sequence analysis. The purpose of this article is to introduce and adapt this methodology to a social epidemiology question, taking the example of the impact of HIV diagnosis on Sub-Saharan migrants’ residential trajectories in the Paris region.MethodsThe sample consists of 640 migrants born in Sub-Saharan Africa receiving HIV care. They were interviewed in healthcare facilities in the Paris region within the PARCOURS project, conducted from 2012 to 2013, using life event history calendars, which recorded year by year their health, family and residential histories. We introduce a two-step methodological approach consisting of (1) sequence analysis by optimal matching to build a typology of migrants’ residential pathways before and after diagnosis, and (2) a Cox model of the probability to experience changes in the residential situation.ResultsThe seven-clusters typology shows that for a majority, the HIV diagnosis did not entail changes in residential situation. However 30% of the migrants experienced a change in their residential situation at time of diagnosis. The Cox model analysis reveals that this residential change was in fact moving in with one’s partner (HR 2.99, PConclusionThis original combination of sequence analysis and Cox models is a powerful process that could be applied to other themes and constitutes a new approach in the life course epidemiology toolbox.Trial registration numberNCT02566148.

Abstract

International audience; Objectives : The aim of this study is to provide information on changes in mental health among disadvantaged immigrants from Sub-Saharan Africa in the Greater Paris area and their level of information about Covid-19.MethodsPrior to the Covid-19 epidemic, the Makasi community-based cohort followed 850 immigrants from sub-Saharan Africa in the Greater Paris area. Between the 1st of April and the 7th of June 2020, all participants scheduled for a follow-up survey were systematically included into an additional COVID-19-related wave of data collection (N = 100). We compared participants' type of housing, level of food insecurity, work and mental health (PHQ9) before and during the first COVID-19-related lockdown, using paired-Mc Nemar chi-2 tests. We next described their level of information on Covid-19 and policy measures, broken down by sex.ResultsAmong the 100 participants, 68% had no legal residence permit. Food insecurity was more often reported during lockdown than before (62% vs 52%). 9% of participants had a score indicative of severe depression (PHQ9) before lockdown and 17% afterwards (p = 0.17). Only 51% knew about the possibility of asymptomatic transmission of the COVID-19 virus. Conclusions : This study brings original information on a hard-to-reach population group. Our results suggest that the lockdown had a detrimental impact on various economic and mental health aspects among disadvantaged migrants residing in the Greater Paris area.

Objectif : L’objectif de cette etude est d’analyser l’acces aux soins des personnes originaires d’Afrique subsaharienne (ASS) vivant avec une hepatite B chronique (HBC) en France.Methodes : L’enquete ANRS-Parcours est une etude biographique realisee en 2012-2013 aupres de personnes originaires d’ASS recrutees dans des services de prise en charge de l’HBC en Ile-de-France. Les donnees ont ete recueillies en face-a-face a l’aide d’une grille biographique et d’un questionnaire standardise.Resultats : Parmi les 619 participants, 96,4 % ont une couverture maladie de base dont 18,6 % la Couverture maladie universelle (CMU) et 23,4 % l’Aide medicale d’etat (AME). Un tiers des beneficiaires de l’Assurance maladie n’ont pas de complementaire sante et 75,7 % sont couverts au titre d’une Affection longue duree. L’obtention d’une couverture maladie apres l’arrivee en France a lieu en mediane la premiere annee. Parmi les participants, 22,0 % rapportent avoir renonce aux soins pour raisons financieres depuis l’arrivee en France et 9,7 % avoir vecu un refus de soins le plus souvent par refus de la CMU ou l’AME. Une fois diagnostique, l’entree en soins a lieu en mediane l’annee-meme du diagnostic. Le retard a l’entree en soins est plus frequent chez les personnes sans couverture maladie l’annee du diagnostic. Les ruptures de suivi de plus de 12 mois sont rares.Conclusion : Les personnes originaires d’ASS vivant avec une HBC accedent rapidement a une couverture maladie et aux soins. Cependant, des obstacles a l’acces aux soins persistent pour certaines du fait notamment de l’absence ou de l’incompletude d’une couverture maladie et des refus de soins aux beneficiaires de l’AME ou de la CMU.

Abstract

The intensification of international migrations questions the national health system: do people who come from elsewhere have specific health needs? Answering this question is difficult on the one hand because of the very great heterogeneity of situations that can cover this "coming from elsewhere", and on the other hand because of the scarcity of available health data depending on the origin. This article brings some elements to answer these questions, starting by defining who are the populations concerned and what is their weight in the French population. From the available scientific literature, it shows that if immigrants are sometimes healthier than natives when they arrive in France, because good health is one of the conditions to "try the adventure elsewhere", this initial robustness can be undermined by the difficult living conditions on arrival in France. In addition, people from countries with limited resources know during their lives in France an "accelerated health transition", chronic diseases and mental disorders taking precedence over infectious diseases.SANTÉ DES IMMIGRÉS : LA TRANSITION ÉPIDÉMIOLOGIQUE À L’ÉCHELLE INDIVIDUELLE L’intensification des circulations internationales interroge le système national de santé : les personnes qui arrivent d’ailleurs ont-elles des besoins de santé spécifiques ? Répondre à cette question est difficile d’une part du fait de la très grande hétérogénéité de situations que peut recouvrir ce « venir d’ailleurs », et d’autre part du fait de la rareté des données de santé disponibles selon l’origine. Cet article apporte quelques éléments d’éclairage, en commençant par définir qui sont les populations concernées et quel est leur poids dans la population française. À partir de la littérature scientifique disponible, il montre ensuite que si les immigrés sont parfois en meilleure santé que les natifs lorsqu’ils arrivent en France, car être en bonne santé est une des conditions pour « tenter l’aventure ailleurs », cette robustesse initiale peut être entamée par les conditions de vie difficiles à l’arrivée en France. Par ailleurs, les personnes venant de pays à ressources limitées connaissent au cours de leur vie en France une « transition sanitaire accélérée », les maladies chroniques et les troubles mentaux prenant le pas sur les maladies infectieuses.

Abstract

ObjectiveRecent studies highlighted that many HIV-positive migrants in Europe acquired their infections post-migration. However, the timing of these infections is not always known. This study aims to estimate the timing of post-migration HIV acquisition among sub-Saharan migrants in France and to understand the correlates of post-migration infection.MethodsWithin the PARCOURS retrospective survey conducted in 2012–2013 in 74 healthcare facilities in the Paris region, life-event data were collected among a random sample of 926 patients living with HIV in HIV services and 763 patients undiagnosed with HIV in primary care centres born in sub-Saharan Africa (reference group). Based on previous analysis, we considered the first 6 years in France after migration as a settlement period. Among the persons who acquired HIV after migration, we estimated the proportion of persons infected during settlement (0–6 years after migration) and after settlement (>6 years after migration) by using an algorithm that combines life-event data and a modelisation of CD4+T-cell count decline. We then assessed the determinants of HIV acquisition during settlement and after settlement using bivariate logistic regression models.ResultsOverall, 58% of sub-Saharan migrants who acquired HIV after migration were infected during the first 6 years in France. HIV acquisition during settlement was found to be linked to short/transactional partnerships and lack of a resident permit. 42% of migrants had contracted HIV after settlement. After settlement, HIV acquisition was associated with short/transactional but also with concurrent partnerships and not with social hardship.ConclusionTwo profiles of HIV post-migration acquisition emerged. The majority of HIV post-migration acquisition occurs during the settlement period: comprehensive combination prevention programmes among recently arrived migrants are needed. However, long-term migrants are also at risk for HIV through multiple partnerships. Prevention programmes should address the different profiles of migrants at risk for post-migration HIV acquisition.

42. Immigration et santé: de l’étude des différences à celle des inégalités

Abstract

Les disparités de santé entre populations immigrées et non-immigrées ont mis du temps à émerger comme objet de recherche légitime. Elles ont d’abord été explorées en lien avec les pratiques culturelles et les conditions sanitaires (présence d’épidémies, d’agents pathogènes) dans les pays d’émigration. Ce chapitre présente, à partir d’une étude sur le saturnisme infantile en France et les grands résultats de l’enquête PARCOURS, en quoi les inégalités de santé entre populations immigrées et natives sont liées aux inégalités sociales et notamment aux conditions de logement. L’accès aux soins pour les personnes immigrées et la prise en charge qui leur est offerte va, selon les cas, minimiser ou aggraver ces inégalités sociales et de santé.

Abstract

Sub-Saharan African migrants living in France are particularly affected by HIV. Due to the fear of sexual transmission of the virus, those in a relationship could experience a union break-up after an HIV diagnosis. Based on data from the time-event ANRS-Parcours survey that was conducted among a representative sample of sub-Saharan migrants living with HIV (France, 2012-2013), we studied union break-ups after HIV diagnosis among people who were in a relationship at the time of their diagnosis. Women experienced a more rapid union break-up after HIV diagnosis than did men. The living conditions of men were not correlated to their risk of union break-up while among women, having a personal dwelling reduced the risk of union break-up. For both sexes, less established relationships were more likely to break up after HIV diagnosis. Having disclosed the HIV status to a partner was associated with a reduced risk of separation between partners, after adjustment on socio-economic conditions and migration characteristics.

Abstract

BackgroundOne of the classic challenges for prevention programmes is reaching the populations they serve. In France, a substantial number of African migrants living with HIV acquired their infection after migrating. The aim of this paper is to better understand the characteristics of the population reached by a community-based outreach approach.MethodsWe compared sociodemographic characteristics across three different groups in the Paris greater area: (1) the general African migrant population (Population census), (2) the African migrant population using either the regular healthcare system or the system for vulnerable populations (PARCOURS Survey) and (3) the African migrant population reached through a community-based mobile unit (Afrique Avenir). Comparisons were conducted according to sex, age, region of origin, duration of residence and occupational and legal statuses using χ2 tests.ResultsThe migrants reached by the mobile unit were mostly men (69%), 52% of whom were younger than 35 years old. They more often lived in precarious situations than did the general sub-Saharan population (49% vs 35% were unemployed, respectively, pConclusionOur study shows that the outreach approach can constitute a missing link in the prevention chain among sub-Saharan African migrants, reaching a group that differs from the general migrant population and from the migrant population in healthcare services—not only the newly arrived migrants who live in more precarious situations but also those who have been in France for several years and are still affected by social hardship.

Abstract

International audience; Worldwide, concerted efforts are made to stimulate the use of information and communication technology (ICT) in health care. From online Personal Health Record (PHR), treatment and appointment reminders by SMS, to the e-mailing of confidential clinical data, healthcare systems are increasingly using technologies to provide better services with less financial and human resources. Among ICTs for health, eHealth-i.e. health on the Internet-has been extensively studied whereas the use of mobile technologies for health called "mhealth" or mobile Health has been less documented so far. In 2013, almost 6.8 billion people were mobile phone users, mobile-cellular penetration rates stand at 96% globally (89% in developing countries). Access to the Internet, by comparison, is a reality for only 39% of the world population (77% of the developed world population is online). During the last three years, many health stakeholders and mobile technology providers have understood the high potential of mHealth and launched more than 600 operational projects in developing countries. In this article we are discussing this issue first by assessing the current state of knowledge on mobile health in developing countries, as well as the issues and challenges raised by mobile health. We then focus our discussion on maternal health to discuss the potential applications of these new techniques in this specific field. Finally we suggest several avenues for further analysis on the implications of using mobile phones as a tool for women's health.

The fertility of populations living in the Palestinian Territories remained exceptionally high for many years. Yet recent changes in their reproductive behaviours, and notably the much broader diffusion of contraception in the Territories than elsewhere in the region, raise questions about the factors driving fertility decline. In this article, Sarah Memmi and Annabel Desgrees du Lou combine two types of data – a statistical survey conducted in 2006 and in-depth interviews with Palestinian couples – to analyse the role of conjugal relationships in determining contraceptive use. Three categories of couples are defined by establishing a typology of gender relations within the couple relative to fertility control. While for some sections of the population fertility behaviours are still structured by patriarchal gender norms, for others the couple is becoming a unit of dialogue and shared decision-making where contraceptive choices are made jointly. Gender Relations and Contraceptive Practices of Palestinian Couples: Through the prism of gender relations within the couple, this article analyses how married couples manage birth control in the Palestinian Territories, where fertility remains high despite women’s high level of education. Using data from the Palestinian Family Health Survey conducted in 2006 and from detailed interviews of married men and women conducted in 2010-2011, we analyse the prevalence of contraceptive use in the Palestinian Territories and its determinants, as well as the influence of each spouse in contraceptive choices according to their type of conjugal relationship, and the link between gender relations within the couple and the type of contraception used. Although contraceptive use is widespread, it remains subordinate to the powerful social norm of having many children, especially sons, which prevails irrespective of the woman’s socio-demographic characteristics. The organization of fertility control varies according to the type of conjugal relationship, and depends on individual reproductive histories. Contraception and reproduction nevertheless always involve both spouses, and the couple is emerging as the main decision-making unit. Los datos provienen de la encuesta Palestinian Family Health Survey de 2006 y de entrevistas en profundidad realizadas en 2010-2011 con hombres y mujeres casados. Analizamos el nivel del recurso a la contracepcion en los Territorios palestinos asi como sus determinants, la influencia respectiva de los conyuges en la eleccion de las practicas contraceptivas segun el modo de relaciones, y la articulacion entre las relaciones de genero en la pareja y el tipo de contracepcion utilizada. La practica de la contracepcion es general pero queda subordinada a una fuerte valorizacion social de los nacimientos, en particular masculinos, y ello cualesquiera que sean las caracteristicas socio-demograficas de las mujeres. Los modos de de organizacion conyugal del control de la fecundidad son diferentes segun el tipo de relaciones conyugales, y dependen de las biografias reproductivas de los individuos. Sin embargo, contracepcion y procreacion implican siempre a los dos conyuges, la pareja apareciendo como principal unidad de decision.

49. [Health care access of Sub-Saharan African migrants living with chronic hepatitis B]

Abstract

Objective: The objective of this study was to analyse health care access of Sub-Saharan African migrants living with chronic hepatitis B (CHB) in France. Methods: The ANRS-Parcours survey was a life-event survey conducted in 2012-2013 among Sub-Saharan African migrants recruited by health care facilities managing CHB in the Paris region. Data were collected by face-to-face interview using a biographical grid and a standardized questionnaire. Results: 96.4% of the 619 participants basic health insurance coverage with CMU universal health insurance coverage in 18.6% of cases and AME state medical assistance in 23.4% of cases. One-third of basic health insurance beneficiaries did not have any complementary health insurance and 75.7% had long-term disease status. The median time to acquisition of health insurance cover after arrival in France was one year. 22.0% of participants reported delaying health care for financial reasons since their arrival in France and 9.7% reported being refused health care usually due to refusal of CMU or AME. Health care access was effective within one year of the diagnosis. Delayed health care access was more common among people without health insurance coverage in the year of diagnosis. Patients lost to follow-up for more than 12 months were rare. Conclusion: Sub-Saharan African migrants living with chronic hepatitis B rapidly access health insurance coverage and health care. However, barriers to health care access persist for some people, essentially due to absent or incomplete health insurance cover and refusal of care for AME or CMU beneficiaries.

50. [Barriers to tuberculosis awareness and screening: a qualitative study in a French department]

Abstract

The incidence of tuberculosis in Seine-Saint-Denis is considerably higher than the national average. The north-west part of the department is particularly concerned. Screening teams encounter difficulties communicating on the disease, reaching the populations concerned and obtaining their adhesion to the screening proposal. The objective of this study was to identify and elucidate the obstacles to tuberculosis prevention and screening. A qualitative study was conducted based on observation of screening actions in Seine-Saint-Denis and semi-directive interviews with health professionals in charge of screening, community representatives and associations, and individuals from the population concerned. Obstacles to tuberculosis awareness and screening appear to be linked to the way in which screening is organized and implemented, and communication difficulties in relation to this disease. Three major obstacles were identified: the gap between the little attention paid to this disease and the sanitary crisis it can trigger in the event of an epidemic; the unsuitability of communication tools for the target population and their lack of attractiveness; maladjustment of screening actions to the local context. The study highlights the individual and social marginalization of the tuberculosis issue and the mechanisms producing this marginalization. It encourages the development of more interactive communication tools and modes, and increased awareness of the professionals involved in screening about field realities.
