Background: There are almost no validated tools in German that assess dementia knowledge, attitude toward dementia, and confidence in the general population. Objective: Translation and validation of the German version of the Dementia Knowledge Assessment Tool 2 (DKAT2), the Dementia Attitude Scale (DAS), and the Confidence in Dementia Scale (CODE). Methods: Instruments were translated into German and adapted for the general public. A convenience sample of 263 persons was recruited via an online platform. Validation of the tools’ psychometric properties consisted of an assessment of its reliability (internal consistency and 4-week test-retest reliability of a subgroup with n = 110), an analysis of its construct validity through principal component analysis and known-group analysis, convergent validity, and an item analysis for DKAT2-D. This study used the STROBE checklist for reporting. Results: Acceptable to excellent internal reliability was found for DAS-D (α= 0.90), DKAT2-D (α= 0.78), and CODE-D (α= 0.93). The principal component analysis confirmed the two-factor model for the DAS-D as well as the one-factor solution for CODE-D. The intra-class correlation coefficient between the first and the 4-week retest was good (CODE-D: 0.897; 0.849–0.929) to excellent (DKAT2-D: 0.918; 0.879–0.945 and DAS-D: 0.940; 0.910–0.960). Known-group analysis revealed that DAS-D, DKAT-D, and CODE-D could distinguish between individuals with or without experience with people with dementia and with or without participation in a dementia course. Conclusion: The German versions DAS-D, DKAT2-D, and CODE-D are reliable and valid tools to measure knowledge, attitude, and confidence in dementia in the German-speaking general population.

Background: Dementia is rapidly increasing worldwide due to demographic aging. More than two-thirds of patients are cared by family members. The quality of care depends on the caregivers’ attitude toward dementia influencing patient care decisions. Objective: The aim of this study is to examine the factors that influence the caregivers’ attitude and whether there is an association between participation in a psycho-educational program and attitude. Methods: We performed a cross-sectional study using a structured closed-ended questionnaire to retrieve socio-demographic information from caregivers and the persons with dementia (N = 86). The study included validated scales such as the Dementia Attitude Scale, the Dementia Knowledge Assessment Tool 2, the Positive Aspects of Caregiving, the Zarit Burden Interview, the Confidence in Dementia Scale, and Spielberger’s State-Trait Anxiety Inventory, as well as a neuropsychological battery to assess the condition of people with dementia. Results: Our final model explains 55.6% of the total variance and shows a significant correlation of five factors with attitude toward dementia: confidence, behavioral and psychological symptoms of dementia, anxiety as a trait, positive aspects of caregiving, and dementia knowledge. The caregivers who participated in a psycho-educational program showed a significantly more positive attitude toward dementia, better dementia knowledge, higher confidence in dementia care, and lower anxiety as a state. Conclusion: The strong correlation of attitude and knowledge, as well as confidence in dementia care, supports the tripartite model of attitude, which hypothesizes the interrelation of affect, cognition, and behavior.

The NMDA-R hypofunction model of schizophrenia started with the clinical observation of the precipitation of psychotic symptoms in patients with schizophrenia exposed to PCP or ketamine. Healthy volunteers exposed to acute low doses of ketamine experienced mild psychosis but also negative and cognitive type symptoms reminiscent of the full clinical picture of schizophrenia. In rodents, acute systemic ketamine resulted in a paradoxical increase in extracellular frontal glutamate as well as of dopamine. Similar increase in prefrontal glutamate was documented with acute ketamine in healthy volunteers with 1H-MRS. Furthermore, sub-chronic low dose PCP lead to reductions in frontal dendritic tree density in rodents. In post-mortem ultrastructural studies in schizophrenia, a broad reduction in dendritic complexity and somal volume of pyramidal cells has been repeatedly described. This most likely accounts for the broad, subtle progressive cortical thinning described with MRI in- vivo. Additionally, prefrontal reductions in the obligatory GluN1 subunit of the NMDA-R has been repeatedly found in post-mortem tissue. The vast 1H-MRS literature in schizophrenia has documented trait-like small increases in glutamate concentrations in striatum very early in the illness, before antipsychotic treatment (the same structure where increased pre-synaptic release of dopamine has been reported with PET). The more recent genetic literature has reliably detected very small risk effects for common variants involving several glutamate-related genes. The pharmacological literature has followed two main tracks, directly informed by the NMDA-R hypo model: agonism at the glycine site (as mostly add-on studies targeting negative and cognitive symptoms); and pre-synaptic modulation of glutamatergic release (as single agents for acute psychosis). Unfortunately, both approaches have failed so far. There is little doubt that brain glutamatergic abnormalities are present in schizophrenia and that some of these are related to the etiology of the illness. The genetic literature directly supports a non- specific etiological role for glutamatergic dysfunction. Whether NMDA-R hypofunction as a specific mechanism accounts for any important component of the illness is still not evident. However, a glutamatergic model still has heuristic value to guide future research in schizophrenia. New tools to jointly examine brain glutamatergic, GABA-ergic and dopaminergic systems in-vivo, early in the illness, may lay the ground for a next generation of clinical trials that go beyond dopamine D2 blockade.

Der Beitrag stellt Überlegungen an und nennt Befunde zu der Frage, inwiefern die Coronavirus-Pandemie besondere Auswirkungen auf ältere Menschen hat und inwiefern in der Gruppe älterer Menschen eine besondere Risikogrup­pe zu sehen ist. Außerdem beschäftigt sich Andreas Kruse mit der Frage, wie sich ältere Menschen mit der Pandemie auseinandersetzen und ob man viel­leicht die Aussage wagen dürfe, dass ältere Menschen mit ihren vielfach hochreflektier­ten Lebenserfahrungen, mit ihrem Lebenswissen ein besonderes Potenzial haben, eine solche Situation in einer für sie selbst, aber auch für andere Menschen verantwortlichen Art und Weise zu verarbeiten. The paper discusses the question to what extent the coronavirus pandemic has a special impact on older people and to what extent a special risk group is to be seen in the group of older people. In addition, Andreas Kruse deals with the question of how older people deal with the pandemic and whether it is possible to venture the statement that older people, with their often highly reflective life experiences, with their life knowledge, have a special potential to process such a situation in a way that is responsible for themselves, but also for other people.

Background: People with Dementia (PwD) are frequently admitted to hospital settings. The lack of proper dementia knowledge, poor communication skills, negative attitudes toward dementia, and lack of confidence affects the quality of care, thus development of dementia trainings has increased. Nevertheless, literature regarding the effectiveness of training implementation is limited. Objective: The aim of this narrative synthesis is to 1) identify the characteristics of training programs and 2) explore the effectiveness of these training programs in everyday clinical practice. Methods: A systematic search in PubMed, PsycINFO, CINAHL, and Cochrane was conducted, including qualitative and quantitative peer-reviewed studies. Holton’s evaluation model with its three outcome levels (learning, individual performance, and organizational results) was adopted. 14 studies were included. Results: The synthesis of the results was divided into two parts: 1) to describe the characteristics and content of trainings 2) to evaluate the effectiveness of training programs according to the three outcome levels of Holton’s model, taking into consideration its construct domains: ability, motivation, and environment. Learning outcomes were assessed in all selected studies: 13 studies observed changes in individual performance, four studies reported changes within the organizational level, and only five showed sustainable changes over time. Conclusion: Person-centered care (PCC) approaches, interactive and varied teaching methods, supporting conditions like champions, action plans, and setting care policies, are all characteristics of effective trainings. Successful programs should be sustainable over time, demonstrating positive outcomes across the organization. Based on current findings, there is a lack of adequate evaluation with regard to training programs on the organizational level.

Der Deutsche Ethikrat hat im März 2020 die Stellungnahme „Robotik für gute Pflege“ veröffentlicht. Andreas Kruse, Mitglied des Deutschen Ethikrats, fasst die wichtigsten Aspekte zusammen.

Die Gesellschaft steht in der Verantwortung der Starkung „Angehorigen-sensibler“ Kommunen, die auf dem Prinzip der Daseinsvorsorge basieren und auf Wertschatzung pflegender Angehoriger (pA) von Menschen mit Demenz (MmD), deren Befahigung zur Selbstsorge und auf Partizipation ausgerichtet sind. Es braucht Methoden, durch die informelle Pflege- und Unterstutzungsleistungen samt ihrer Herausforderungen und Rahmenbedingungen differenziert abgebildet werden. Der Beitrag verfolgt das Ziel, die Methode der „Rathausgesprache“ (RG) vorzustellen. Deutschlandweit wird in 20 Kommunen jeweils ein offentliches RG durchgefuhrt, in dem pA und kommunale Akteure (kA) in den Dialog treten. In 3 Diskussionsgruppen wird u. a. die Frage nach Moglichkeiten der Ressourcenbereitstellung zur Forderung der Selbstsorgefahigkeit pA thematisiert. Um Aussagen zum Gelingen der Projektplanung zu treffen, wurden 3 Pilot-RG evaluiert. Insbesondere die Rekrutierung der Stichprobe und die Organisation der RG sind administrativ und logistisch anspruchsvoll. Das Projektvorhaben kann nur durch intensive Zusammenarbeit zwischen den wissenschaftlichen Akteuren und den Kommunen erfolgreich sein. Die Pilot-RG zeigen, dass die Projektplanung inhaltlich und logistisch gelungen ist. Die Methode erhielt mehrheitlich sehr positive Bewertungen. Die RG versprechen mit Blick auf die Gestaltung Angehorigen-sensibler Kommunen groses Potenzial. Welchen konkreten Nutzen die RG fur pA und die kA aufweisen, wird noch untersucht.

Background: The outcomes of hospitalized People with Dementia (PwD) are likely to be negative due to, among other key causes, negative staff attitudes and limited staff knowledge regarding dementia. Targeted interventions have been shown to positively change the attitudes of the hospital staff while also increasing their overall knowledge of dementia. However, training effects are often short-lived and frequently long-term effects are not examined in studies. Objective: To examine whether attending a dementia training program changes the attitudes of hospital staff toward PwD and/or increases their knowledge levels about dementia, and whether or not these changes are stable. Methods: The training program lasted two days and N = 60 attending hospital staff members agreed to participate in the study. Data were assessed with questionnaires prior to the training, 3 months, and 6 months after the training. German versions of the Dementia Attitude Scale (DAS-D) and the Knowledge in Dementia (KIDE) scale were used. Additionally, data about perception of PwD and confidence in dealing with challenging behavior were collected and analyzed. Results: After the training program, participants showed a significantly better attitude toward PwD as measured by DAS-D. These time-effects occurred in both DAS-D subscales (“dementia knowledge” and “social comfort”). Although a positive trend could be seen in the KIDE scale, no statistically significant increase occurred over time. Conclusion: Specialist training programs seem to be promising in positively changing attitudes toward and increasing knowledge about PwD with long-term effects. Further research should address the effects of attitude change in patient care.

Zusammenfassung Ein Teilprojekt der von der Deutschen Bischofskonferenz in Auftrag gegebenen Studie »Sexueller Missbrauch an Minderjährigen durch katholische Priester, Diakone und männliche Ordensangehörige im Bereich der Deutschen Bischofskonferenz« befasst sich mit der Analyse von Strafakten. Es wurden Strafverfahren wegen sexueller Missbrauchsdelikte gegen Minderjährige durch Kleriker der katholischen Kirche und durch Mitglieder anderer Institutionen, wie z. B. Schulen, ausgewertet. In diesem Beitrag wird untersucht, ob die besonderen Rahmenbedingungen in der katholischen Kirche, insbesondere das Kirchenrecht und der Klerikalismus, einen Einfluss auf Taten, Täter und institutionelle Reaktionen bei Delikten des sexuellen Missbrauchs von Minderjährigen haben. Die Analyse ergab, dass die Besonderheiten in der katholischen Kirche mit spezifischen Tat- und Tätermerkmalen korrespondierten und die Institutionen unterschiedlich auf Missbrauchsvorwürfe reagierten. Obwohl das Normensystem, das einen sexuellen Missbrauch verhindern soll, in der katholischen Kirche viel ausgeprägter ist als in anderen Institutionen, sind keine Unterschiede im Verhalten der Akteure ersichtlich.

ZusammenfassungDas hohe Alter soll nachfolgend im Sinne von Grenzgängen gedeutet werden, was zum einen bedeutet, die zwischen psychologischen Bereichen liegenden Grenzen immer wieder zu überschreiten und in diesem Überschreiten ein hohes Maß an psychologischer Komplexität zu verwirklichen. Zum anderen soll mit den Grenzgängen angedeutet werden, dass es alten Menschen gelingen kann, in der Auseinandersetzung mit Grenzen – und das Erleben zunehmender Verletzlichkeit lässt die Grenzen der eigenen Existenz immer deutlicher in das Zentrum des Bewusstseins treten – zu neuen Erlebens-, Verhaltens- und Lebensqualitäten zu gelangen.

ZusammenfassungDer Beitrag konzentriert sich auf die Lebenssituation von Menschen mit geistiger Behinderung. Er geht der Frage nach, inwieweit sich das fachliche und gesellschaftliche Verständnis für Menschen mit geistiger Behinderung gewandelt (oder eben nicht gewandelt) hat, wobei zwei Aspekte im Zentrum stehen: (a) Die Wahrnehmung und ausdrückliche Anerkennung der Ressourcen und Kompetenzen, (b) die Umsetzung der Rehabilitations- und produktiven Veränderungspotenziale. Vor dem Hintergrund dieser beiden Bereiche der Analyse diskutiert der Beitrag Fragen der Teilhabe wie auch des gleichberechtigten Zugangs zur gesundheitlichen – und dies heißt auch: rehabilitativen – Versorgung sowie der gegebenen (vs. mangelnden) Expertise von Mitarbeiterinnen und Mitarbeitern des Versorgungssystems mit Blick auf Bedarfe und Bedürfnisse, auf Kompetenzen und Vulnerabilitäten von Menschen mit geistiger Behinderung. Es wird auf die Notwendigkeit hingewiesen, die medizinisch-rehabilitative und pflegerisch-rehabilitative Versorgung systematisch auszubauen, wobei die in den beiden vergangenen Jahrzehnten erzielten Fortschritte in der differenzierten Einschätzung von Rehabilitations- und produktiven Veränderungspotenzialen nicht übersehen werden dürfen. Der Beitrag legt – in seiner ethischen Rahmung – großes Gewicht auf die Explikation des Person-Verständnisses und – daraus folgend – auf die Wahrnehmung und unbedingte Anerkennung der Personalität von Menschen mit geistiger Behinderung, die ihrerseits auf Konzepten wie jenen der Selbstverantwortung und Autonomie fundiert.

Background The number of dementia training programs in hospital settings is steadily increasing. The way training sessions are designed influences the way the learning content is implemented in practice. To develop a successful training it is important to meet the needs of the target group; however, not much is known about staff preferences and expectations relevant to future dementia training programs in hospitals in Germany and Greece. Objective The aim of this survey was to explore staff training needs relevant to the topic of dementia, in general hospitals in Germany and Greece. This study analyzed the interests of staff members, preferences and expectations with respect to dementia training. Material and methods This was a descriptive survey based on a 54-item questionnaire conducted with 61 nursing staff, head nurses and physicians (Germany: n = 25, Greece: n = 36) recruited from 5 hospitals (Germany: n = 3, Greece: n = 2). Parts of the questionnaire explored participants’ previous education regarding dementia and their expectations towards future dementia programs. Results Although staff attendance in educative programs was high in the last 5 years for both countries, participation in dementia training programs was low (Germany 24%, Greece 5.5%). Additionally, the great majority of participants were willing to be trained in future dementia training programs (Germany 96%, Greece 100%). Employees from both countries expect increased clinical skills as a result of participation in such training programs. In Greece, staff members hope for better handling of people with dementia, while in Germany, concrete practical advice is preferred. Conclusion There seems to be a strong willingness to participate in further dementia training programs where not only theoretical knowledge is provided but also practical advice.

Zusammenfassung Hintergrund Seit 2015 definiert das Hospiz- und Palliativgesetz einen Anspruch auf Vorsorgeplanung in der letzten Lebensphase für Pflegeheimbewohner. Dennoch ist das Konzept Advance Care Planning (ACP) in Deutschland bislang unzureichend beforscht. Die defizitäre Evidenzlage erscheint zu diesem Thema in Verbindung mit dem Krankheitsbild Demenz noch gravierender. Die Bedeutung eines solchen Gesprächsprozesses steigt mit dem Umstand der Diagnosestellung Demenz für den Betroffenen, aber auch für seine An- und Zugehörigen und medizinisch-pflegerische Begleitung erheblich. Ziel der Arbeit Im Hinblick auf Menschen mit Demenz wurden Dokumente eines prominenten deutschen ACP-Anbieters von den Autoren als unzureichend eingeschätzt. Ziel der hier dargelegten Pilotstudie war es, einen im nationalen Kontext vielfach verwendeten Fragebogen (Werteanamnese) zu modifizieren, um ihn bei dieser vulnerablen Gruppe anwenden zu können. Material und Methoden Die Werteanamnese wurde in 11 Schritten modifiziert und im Pretest mit früherkrankten Menschen mit Demenz erprobt. Der Fokus des Dokuments liegt auf Einstellungen zu Leben, Sterben und lebensverlängernden Maßnahmen. Der Modifikationsprozess erfolgte in einem iterativen Prozess, angelehnt an die Grounded Theory nach Strauss und Corbin. Ergebnisse und Diskussion Die schrittweise Anpassung des Dokuments hat einen praktikablen und niedrigschwelligen Zugang geschaffen, der es auch der vulnerablen Gruppe der Menschen mit Demenz ermöglicht, sich mit existenziellen Fragen umfassend auseinanderzusetzen und sich darüber auszutauschen. Dieser Fragenkatalog dient als Basisdokument für die anschließende Hauptstudie – hier wird nach der Lebensbindung von Menschen mit Demenz gefragt und somit eine neue Perspektive auf ACP gelegt.

Der vorliegende Beitrag stellt Befunde einer Strafaktenanalyse dar, die sich mit Strafverfahren wegen sexueller Missbrauchsdelikte an Minderjahrigen in der katholischen Kirche und in anderen Institutionen, wie z. B. Schulen, befasst. Die Analyse ergab, dass die Tatvorwurfe teilweise sehr schwer waren. Vielen Beschuldigten wurde nicht nur ein Ubergriff, sondern eine Mehrzahl von Delikten vorgeworfen. Viele Betroffene mussten mehrfach einen sexuellen Missbrauch erleiden. Die Delikte in der katholischen Kirche und in den anderen Institutionen weisen viele Ahnlichkeiten auf, es bestehen aber auch Unterschiede. Von den Strafverfahren, welche die katholische Kirche betrafen, fuhrten weniger als ein Drittel zu einer Verurteilung. Das lag insbesondere daran, dass viele dieser Verfahren wegen Verjahrung eingestellt werden mussten.

Die Tragfähigkeit geklebter Anschlüsse zu bestimmen, ist eine zum Teil immer noch offene Frage. Insbesondere der oft unterstellte direkte Zusammenhang zwischen Spannungen und Traglast ist komplizierter, als es die klassische Mechanik impliziert. In diesem Zusammenhang erscheint es sinnvoll, auch die konstruktiven Details einer Klebverbindung zu betrachten.

ZusammenfassungDer nachfolgende Beitrag konzentriert sich auf das Zusammenwirken von menschlicher und künstlicher Intelligenz im Alter. Er untersucht dieses Zusammenwirken im Kontext von Selbstgestaltung, Weltgestaltung und Verletzlichkeit. Selbst- und Weltgestaltung werden als zwei grundlegende Motive des hohen Alters verstanden: es wird diskutiert, in welcher Hinsicht künstliche Intelligenz in ihrem Zusammenwirken mit menschlicher Intelligenz zur Verwirklichung dieser beiden Motive beizutragen vermag. Weiterhin werden die kompensatorischen Funktionen dieses Zusammenwirkens im Kontext der Verletzlichkeit diskutiert, wobei Verletzlichkeit als bedeutender Aspekt der Conditio humana interpretiert wird, auf den Menschen antworten müssen. Die Nennung von Prinzipien der ethischen Betrachtung digitaler Technologien soll als allgemeiner ethischer Orientierungsrahmen des Zusammenwirkens von menschlicher und künstlicher Intelligenz dienen; Dilemmata der Nutzung digitaler Technologien werden aufgezeigt und diskutiert. Der fachlich und ethisch verantwortliche Umgang mit künstlicher Intelligenz wird als eine Chance für eingutes Lebenim Alter gewertet.

Society is responsible for strengthening "caregiver-sensitive" communities based on the principle of social services to the public and are thus oriented towards the appreciation of caring relatives (CR) of people with dementia (PwD), their capability for self-care and participation. This requires methods to emphasize the social value of informal care and to re-evaluate existing support services. The article aims to present the method of "town hall talks".In 20 communities throughout Germany a public town hall talk in which CR, actors in the community (AC) and the citizens enter into a dialogue, was conducted. In three moderated discussion groups the question of communal responsibility for the provision of resources was addressed. These resources should encourage the CR ability of self-care as well as strategies for their support. In order to make a statement regarding the success of the project planning, three pilot town hall talks were evaluated.The recruitment of the population sample and the organization of the town hall talks in particular are administratively and logistically challenging. The project can only be successful through networking and intensive cooperation between scientists and the communities. In terms of content and logistics, the project planning of the pilot town hall talks was successful. The method received excellent ratings from the majority of the CR and AC.Regarding the creation of caregiver-sensitive communities the town hall talks promise great potential. ZusammenfassungDaten zum sexuellen Missbrauch an Kindern können zum einen über die Hellfeldstatistiken gewonnen und zum anderen durch Dunkelfeldforschung erhoben werden. Der vorliegende Beitrag befasst sich mit der längsschnittlichen Entwicklung der Delikte in den vergangenen 63 Jahren in Deutschland. Die Straftaten des sexuellen Missbrauchs von Kindern gemäß § 176 StGB weisen in der Polizeilichen Kriminalstatistik (PKS) seit Veröffentlichung der PKS 1953–2016 einen Rückgang der Häufigkeitszahlen auf. Die Ergebnisse aus der Dunkelfeldforschung sind uneinheitlich, sprechen aber teilweise auch für einen Rückgang. Die Bedeutung dieser Befunde für die Präventionsarbeit und gesundheitspolitische Aktivitäten wird diskutiert.

Zusammenfassung Hintergrund Die palliative Begleitung schwer kranker und sterbender Menschen in ambulanten und stationären Einrichtungen der Alten- und Krankenpflege wie auch der spezialisierten palliativen Versorgung erfordert multiprofessionelle Zusammenarbeit. Aufgrund der besonderen Belastungssituationen sowie der Charakteristika verschiedener Sterbeorte sind die Beteiligten mit zahlreichen Kommunikationsschwierigkeiten und damit verbundenen Konfliktfeldern konfrontiert. Methodik Ziel war es, die Bedürfnisse und Erwartungen von Menschen in Palliativsituationen in der Interaktion mit Pflegenden zu erheben und herauszufinden, inwieweit Konfliktsituationen auftreten und wie diesen begegnet wird. Im Rahmen eines qualitativen Forschungsansatzes wurden 58 problemzentrierte Interviews mit Menschen in einer Palliativsituation in fünf unterschiedlichen Versorgungssettings (Palliativstation, Hospiz, Onkologie, stationäre Einrichtung der Altenhilfe, ambulante Versorgung) geführt und mittels qualitativer Inhaltsanalyse ausgewertet. Ergebnisse Die Mehrheit der Befragten beschreibt institutionelle und personelle Rahmenbedingungen sowie das Verhalten der Pflegenden als konfliktursächlich. Im Bereich von Informationsasymmetrien zeigen sich Konflikte, die sich primär auf eine wechselseitige fehlende Offenheit, Ehrlichkeit und Transparenz beziehen. Schlussfolgerung Gestützt auf Erkenntnisse des explorativen Forschungsprojekts soll im Rahmen eines geplanten Theorie-Praxis-Transferprojekts ein Beitrag zur Verbesserung der Versorgungsqualität am Lebensende geleistet werden.

In the project I-CARE a technical system for tablet devices is developed that captures the personal needs and skills of people with dementia. The system provides activation content such as music videos, biographical photographs and quizzes on various topics of interest to people with dementia, their families and professional caregivers. To adapt the system, the activation content is adjusted to the daily condition of individual users. For this purpose, emotions are automatically detected through facial expressions, motion, and voice. The daily interactions of the users with the tablet devices are documented in log files which can be merged into an event list. In this paper, we propose an advanced format for event lists and a data analysis strategy. A transformation scheme is developed in order to obtain datasets with features and time series for popular methods of data mining. The proposed methods are applied to analysing the interactions of people with dementia with the I-CARE tablet device. We show how the new format of event lists and the innovative transformation scheme can be used to compress the stored data, to identify groups of users, and to model changes of user behaviour. As the I-CARE user studies are still ongoing, simulated benchmark log files are applied to illustrate the data mining strategy. We discuss possible solutions to challenges that appear in the context of I-CARE and that are relevant to a broad range of applications.

In the first part of this article a comprehensive understanding of political advice is explicated. In this context reference is made to the value judgement dispute in German sociology and a differentiation between an enlightening and a critical function of scientific policy advice. Development of social scientific policy advice is considered against the background of the distinction between technocratic, decisionistic and pragmatic models of counseling. In the second part of the article, the committee of inquiry on demographic change, the national reports on aging and the reviews on the development of the healthcare system are considered as practical examples of policy advice.

Sexual violence against children remains a global public health problem. The health sector has an opportunity and responsibility to be part of the multisector collaboration to prevent and respond to sexual abuse of minors. The following article presents a critical analysis of hitherto published empirical studies that examine the extent and variety of sexual abuse of minors within the Catholic Church and other institutions. The analysis consists of 40 studies concerning the Catholic Church and 13 studies concerning other institutions not belonging to the Catholic Church. We report the characteristics of the offenders and the offenses. Furthermore, the devastating consequences for children abused by members of powerful institutions are discussed. Knowledge on the role of institutions in sexual abuse of minors and psychological distress following these experiences is necessary to point the way to improvements in prevention and intervention strategies. This literature review is part of a larger research project on the sexual abuse of minors in the context of the Catholic Church in Germany (MHG Study) that is conducted by an interdisciplinary team of psychologists, psychiatrists, criminologists, and sociologists and is funded by the German Bishops' Conference.

Lignocellulosic biomasses are strongly connected composites of cellulose, hemicelluloses, and lignin. A pretreatment is required in order to make these components available for their later conversion into chemicals. At this point, two strategies have to be considered: to either produce chemicals via microorganism or enzymes (1), or by chemical conversion (2). The focus of this article is the second strategy, which is chemical conversion, performed in water to produce the final products furfural and 5-hydroxymethylfurfural (HMF). Reviewed first is the composition of cellulose and hemicelluloses as well as their degradation chemistry in water. Then, fundamental modes of action and process parameters of pretreatment methods in aqueous solution are summarized. The pretreatment methods discussed here are steam explosion, treatment with hot liquid water, diluted and concentrated acids, as well as alkaline solutions. Finally, the advantages and disadvantages of these pretreatments are discussed for lignocellulosic biomass.

This study explores the extent of sexual abuse of minors by members of the Catholic Church in Germany. It is the first comprehensive study to examine this extent in a European country. The goals of this study are as follows: (a) to analyze whether the extent and characteristics of sexual abuse in a European country are comparable to those in the United States and Australia and (b) how discrepancies can be explained. The personnel files of 38,156 Catholic Priests, deacons, and male members of religious orders in the authority of the German Bishops’ Conference were analyzed. The study period lasted from 1946 to 2014. All 27 German dioceses took part in this study. A total of 4.4% of all clerics ( N = 1,670) from 1946 to 2014 were alleged to have committed sexual abuse, and 3,677 children or adolescents were identified as victims. These results are similar to those from comparable studies in the United States. Sexual abuse of minors within the authority of the Catholic Church seems to be a worldwide phenomenon.

Over 40% of patients over 65 years old in general hospitals show cognitive decline with nearly 1 in 5 suffering from dementia, while dementia is often not detected as a secondary diagnosis at hospital admission [6]. The situation is similar worldwide and will not ease in the next few years due to demographic change, despite a slightly lower incidence of dementia [20, 22]. Due to the heterogeneity of healthcare systems internationally, there is no single solution to improving the situation for people with dementia and hospital staff. To date, 29 of the 194 member states of the World Health Organization (WHO) have adopted their own dementia strategies [3]. In Germany, the participants took up their work in 2019 [8]. Since there are so far no national guidelines for the care of people with dementia (PwD) in Germany, numerous model projects have been carried out that aim to solve specific problems faced by a particular hospital and which were specific for this hospital. Most of these projects have not received scientific support and that is why they are not available for research or for “imitators”. Research projects can only partially contribute to the nationwide improvement of the care for PwD. Although innovative work has already been carried out, e.g. on delirium prevention, communication with PwD, hospital staff training and transformation into dementia-sensitive hospitals, healthcare organizations often fail to transfer theoretical knowledge into practical skills. There is still a general lack of sustainability and the need for a conceptual framework designed to mandatorily implement established interventions, concepts and guidelines into practice for acute-care hospitals.

Since 2015, the Hospice and Palliative Care Act has defined the entitlement of nursing home residents to preventive planning in the final phase of their lives. Nevertheless, the advance care planning (ACP) concept has not yet been sufficiently researched in Germany. The deficient evidence on this topic appears to be even more considerable in connection with dementia. The necessity of such a discussion increases exponentially when dementia is diagnosed, not only for the person affected but also for their relatives and other caregivers and companions.With respect to people with dementia, documents by a prominent German ACP provider were assessed as being insufficient by the authors. The aim of the pilot study presented here was to modify a questionnaire (value anamnesis) frequently used in the national context in order to apply it to this vulnerable group.The value anamnesis was modified in 11 steps and pre-tested on people with dementia in an early stage. The focus of the document is on attitudes towards life, death and life-prolonging measures. The modification process was carried out in an iterative process based on the grounded theory according to Strauss and Corbin.The stepwise adaptation of the document has created a practicable and low-threshold approach that also enables the vulnerable group of people with dementia to comprehensively deal with existential issues and to exchange views on them. This catalogue of questions is used as a basis document for the subsequent main study: here the life attachment of people with dementia is explored and thus a new perspective is placed on ACP.HINTERGRUND: Seit 2015 definiert das Hospiz- und Palliativgesetz einen Anspruch auf Vorsorgeplanung in der letzten Lebensphase für Pflegeheimbewohner. Dennoch ist das Konzept Advance Care Planning (ACP) in Deutschland bislang unzureichend beforscht. Die defizitäre Evidenzlage erscheint zu diesem Thema in Verbindung mit dem Krankheitsbild Demenz noch gravierender. Die Bedeutung eines solchen Gesprächsprozesses steigt mit dem Umstand der Diagnosestellung Demenz für den Betroffenen, aber auch für seine An- und Zugehörigen und medizinisch-pflegerische Begleitung erheblich.Im Hinblick auf Menschen mit Demenz wurden Dokumente eines prominenten deutschen ACP-Anbieters von den Autoren als unzureichend eingeschätzt. Ziel der hier dargelegten Pilotstudie war es, einen im nationalen Kontext vielfach verwendeten Fragebogen (Werteanamnese) zu modifizieren, um ihn bei dieser vulnerablen Gruppe anwenden zu können.Die Werteanamnese wurde in 11 Schritten modifiziert und im Pretest mit früherkrankten Menschen mit Demenz erprobt. Der Fokus des Dokuments liegt auf Einstellungen zu Leben, Sterben und lebensverlängernden Maßnahmen. Der Modifikationsprozess erfolgte in einem iterativen Prozess, angelehnt an die Grounded Theory nach Strauss und Corbin.Die schrittweise Anpassung des Dokuments hat einen praktikablen und niedrigschwelligen Zugang geschaffen, der es auch der vulnerablen Gruppe der Menschen mit Demenz ermöglicht, sich mit existenziellen Fragen umfassend auseinanderzusetzen und sich darüber auszutauschen. Dieser Fragenkatalog dient als Basisdokument für die anschließende Hauptstudie – hier wird nach der Lebensbindung von Menschen mit Demenz gefragt und somit eine neue Perspektive auf ACP gelegt.

Background When cases of sexual abuse within the Catholic Church became known, the German Bishops' Conference (Deutsche Bischofskonferenz, DBK) commissioned a study by an interdisciplinary consortium to determine the frequency of sexual abuse by Catholic clergy in Germany (the MHG study). Methods Qualitative and quantitative research methods were used and the subject matter of the study was analyzed in seven component projects. To determine the frequency of sexual abuse, 38 156 personnel files of Catholic clergy from the period 1946 to 2014 were studied, and the epidemiologic findings of these evaluations are presented. Results 1670 persons belonging to the Catholic clergy who were accused of sexual abuse of minors were identified from their personnel files, corresponding to 4.4% of the clergy overall. 3677 victims of sexual abuse could be linked to the accused persons; 62.8% of them were male, and 66.7% were under 14 years old when the abuse took place. The mean duration of the abuse in individual cases was 1.3 years. "Hands-on" abuses (i.e., abuses involving bodily contact) occurred in more than 80% of cases. Many of the affected persons suffered serious consequences for their health and social functioning. The ones most commonly reported were anxiety, depression, mistrust, sexual problems, and difficulties with interpersonal contact. Conclusion The figures reported here should be considered a lower bound to the actual frequency of sexual abuse. Asymmetrical power relationships in a closed system such as the Catholic Church can facilitate sexual abuse. Physicians play an important role in the diagnosis and treatment of the victims of sexual abuse, in the diagnosis and treatment of persons inclined to commit abuses and actual abusers, and in the development and implementation of preventive strategies.

Healthcare in inpatient long-term care facilities (nursing homes) should not be limited to medical curative measures, but should also include strengthening social participation, autonomy, self-responsibility and joint responsibility of the residents. Prevention and rehabilitation should therefore be even more integrated into care concepts.This article first introduces various areas of prevention physical activity, nutrition, cognitive competence, psychosocial health, abuse, and freedom-removing measures and then discusses their evidence. Essential for the implementation and the success of such measures is the ability and willingness of people in need of care to engage actively in these therapies; here, appropriate and motivating information plays an important role.Subsequently, geriatric rehabilitation is referred to. In the 2013-2017 empirical study Organization and Rehabilitation for Residents in the Nursing Home to Improve Independence and Participation (ORBIT), 215 people in need of care participated in three-month therapeutic interventions, which were followed by three-months of rehabilitative care. Improvements in mobility and quality of life (Barthel index, QOL-AD) could be demonstrated compared to a control group (n = 28). The results have to be considered against the background of a worsening health and reduced functional capacities in old age. A stronger integration of prevention and rehabilitation services into long-term institutional care is functional for strengthening participation and independence - an important condition for the residents' certainty that their dignity will be respected, competence and strive for self-responsibility and self-determination.
