Background We aimed to investigate the prevalence of violence against women (VAW) in Panama and its association with social determinants of health (SDH) and to estimate the femicide rates from 2014 to 2017. Methods Data were derived from three cross-sectional population-based studies. Logistic regression models were used to estimate the association between SDH and VAW, expressed as odds ratios (ORs) and 95% confidence intervals (CIs). Age-standardized femicide rates were estimated using data from the Public Ministry. Results Compared to the reference categories, women in the lowest quintile (Q) of income distribution (Q1: OR 4.0 [95% CI 1.4–11.7], Q2: OR 3.0 [95% CI 1.1–7.9]), divorced/separated women (OR 1.5 [95% CI 1.0–2.1]) and those in the age categories 25–33 y and 34–49 y (OR 1.9 [95% CI 1.2–3.1]) were more likely to have experienced violence in the past year. Indigenous ethnicity (OR 2.3 [95% CI 1.3–4.1]), age 15–19 y (OR 1.8 [95% CI 1.1–2.9]) and lowest education levels (very low: OR 4.7 [95% CI 1.4–15.5]; low: OR 4.5 [95% CI 1.4–14.6]) were associated with permissive attitudes towards violence. Indigenous (OR 2.7 [95% CI 1.3–6.1]), Afro-Panamanians (OR 3.1 [95% CI 1.3–7.6]) and education level (low: OR 2.5 [95% CI 1.2–4.9]; medium: OR 3.0 [95% CI 1.4–6.6]) were associated with physical/sexual intimate partner violence. Standardized adjusted femicide rates (×100 000) from 2015 to 2017 were 1.5, 0.9 and 0.8, respectively. Conclusions Our findings highlight the importance of prevention programmes.

RESUMEN Introducción: la sedación profunda con propofol controlada por endoscopista en las diferentes unidades de endoscopia ha sido un tema de continua controversia a lo largo de los últimos años, origen de conflictos de intereses entre las distintas sociedades científicas de Anestesiología y Gastroenterología. Numerosos estudios han demostrado ya la eficacia, eficiencia y escasa aparición de complicaciones en la sedación controlada por un endoscopista formado frente al anestesiólogo. Material y métodos: hemos revisado en nuestra base de datos el porcentaje de complicaciones cardio-respiratorias graves en nuestra unidad, en el periodo comprendido entre 2011 y 2016, en las distintas exploraciones endoscópicas que realizamos (gastroscopia, colonoscopia, colangiopancreatografía retrógrada endoscópica [CPRE] y ecoendoscopia [USE]) y cuya sedación es controlada por un endoscopista. Resultados: se llevó a cabo el análisis de 33.195 exploraciones durante el periodo de estudio. Obtuvimos un 0,13% de complicaciones cardio-respiratorias, la mayor parte de ellas desaturaciones graves (la mayoría respondieron a la apertura de la vía aérea asociada a la interrupción de la infusión del fármaco, precisando la necesidad de ambú en contadas ocasiones). No existieron diferencias estadísticamente significativas entre los diferentes grupos excepto en edad media, riesgo por tipo de exploración y riesgo ASA, donde la CPRE presentó una p < 0,01 frente al resto de exploraciones. Conclusión: con los datos de los que disponemos hasta la actualidad, existen numerosas evidencias en la literatura científica para divulgar que la sedación de las endoscopias controlada por un endoscopista formado es segura, eficaz y eficiente. No obstante, deben realizarse más estudios prospectivos que confirmen estas suposiciones, ya que hasta el momento la mayoría de los estudios son retrospectivos.

Introduction: Over the last few years, novel agents-based combinations have been incorporated into the treatment of MM patients, particularly in relapse setting. However, these novel combinations have been evaluated in clinical trials and patients included represent a selected population. Patients in real life are usually older with comorbidities and disabilities and not allowed to be included in the trials, so, in real setting, is expected worse outcomes and shorter survival. The information about treatment burden in real life is scarce. The aim of our study was to analyze the outcome of MM patients in the real life outside clinical trials, in terms of treatment lines in a single institution setting, and to analyze the influence of comorbidities on the treatment burden. Material and methods: Medical records of MM patients treated at Txagorritxu hospital (Spain) between 2009 and January 2017 were retrospectively evaluated with the aim of mapping the course of patients as well as to investigate the factors that influence treatment-decisions at different stages of the disease. Results: 176 patients with MM were diagnosed from jan-2009 to jan-2017. Baseline patient's characteristics are presented in Table 1. The median age at diagnosis was 71 years (range 33.2-93), main of the patients where non-transplant eligible newly diagnosed MM (NTENDMM): 114 (65%). With a median follow-up of 25 months, 90.6% of newly diagnosed MM patients transplant-eligible (TENDMM) remain alive versus 65% NTENDMM patients (p value: 0.000)(figure 1). Overall, patients received a median of 2 lines of treatment, it should be noted that 86% of patients had received 3 or less lines of treatment and only 14% of the patients could receive more than 3 lines of therapy. To better evaluate treatment burden, we focused on deceased patients. At the time of analysis, 19% of TENDMM (12 patients) and 51.4% of in NTENDMM (57 patients) has died with a median time to death of 29.6 months and 18 months to death, respectively. Median lines of therapy for death patients TENDMM was 3.5 (range 1-8), with a 75 percentile of 5 lines of therapy, by contrast, death NTENDMM patients received a median of 2 lines of therapy (range: 1-6), with an 80 percentile of 3 lines of therapy (figure 2). In order to evaluate the influence of comorbidities in treatment burden for NTENDMM patients, CIRS score was estimated retrospectively. Median CIRS score was 5.5 (1-19). CIRS scale did not predict progression free survival (PFS) among the different groups: CIRS 8: 30.6 months (p: 0.819), however, interestingly CIRS scale predicted overall survival (OS): CIRS 8: 12.3 months, (p: 0.012) (figure 2). Analyzing treatment burden for each CIRS score group 63% of patients with CIRS> 8 received only one line of treatment before death, compared to 39.5% and 37.5% of patients with CIRS4-8 and CIRS Conclusion: Although the impressive progress in the management of relapse/refractory MM patients in recent years, half of the patients, particularly those not suitable to received an autologous transplant, will be able to received only 2 lines of treatment before dying. In fact, an adequate comorbidity assessment could select patients that will only need only one line of treatment. To the best of our known, this is the first study that correlate treatment burden according to comorbidities at diagnostic. This study could guide strategies adapted according to the comorbidity of the patients. Disclosures No relevant conflicts of interest to declare.

There is a lack of prospective studies evaluating the natural history of colonic ischaemia (CI). We performed such a study to evaluate the clinical presentation, outcome, and mortality as well as clinical variables associated with poor prognosis.An open, prospective, and multicentre study was conducted in 24 Spanish hospitals serving a population of 3.5 million people. The study included only patients who met criteria for definitive or probable CI. A website (www.colitisisquemica.org) provided logistical support.A total of 364 patients met criteria for inclusion. CI was suspected clinically in only 24.2% of cases. The distribution of clinical patterns was as follows: reversible colopathy (26.1%), transient colitis (43.7%), gangrenous colitis (9.9%), fulminant pancolitis (2.5%), and chronic segmental colitis (17.9%). A total of 47 patients (12.9%) had an unfavorable outcome as defined by mortality and/or the need for surgery. Multivariate analysis identified the following signs as independent risk factors for an unfavorable outcome: abdominal pain without rectal bleeding [odds ratio (OR) 3.9; 95% confidence interval (CI) = 1.6-9.3], non-bloody diarrhoea (OR 10; 95% CI = 3.7-27.4), and peritoneal signs (OR 7.3; 95% CI = 2.7-19.6). Unfavorable outcomes also were more frequent in isolated right colon ischaemia (IRCI) compared with non-IRCI (40.9 vs. 10.3%, respectively; p0.0001). The overall mortality rate was 7.7%.The clinical presentation of CI is very heterogeneous, perhaps explaining why clinical suspicion of this disease is so low. The presence of IRCI, and occurrence of peritoneal signs or onset of CI as severe abdominal pain without bleeding, should alert the physician to a potentially unfavorable course.

Fundamento y objetivo Debido a la mayor morbimortalidad de la hepatitis aguda por el virus de la hepatitis A (VHA) cuando ocurre en pacientes con infeccion cronica por el virus de la hepatitis C (VHC), interesa conocer la prevalencia de inmunizacion frente al VHA en estos pacientes. Pacientes y metodo Se determinaron los anticuerpos de tipo inmunoglobulina G (IgG) frente al VHA (IgG anti-VHA) en 313 pacientes con infeccion cronica por el VHC (anti-VHC) y en 313 individuos anti-VHC negativo. Resultados La prevalencia de IgG anti-VHA encontrada fue identica en ambos grupos: un 81,2%. Sin embargo, entre los menores de 41 anos esta prevalencia era mayor en los anti-VHC positivo que en los individuos del grupo control. La adiccion a drogas por via parenteral y los tatuajes fueron mas frecuentes en el primer grupo. La presencia de IgG anti-VHA se asocio con la edad y con la procedencia del medio rural en ambos grupos. Conclusiones La prevalencia de IgG anti-VHA aumenta con la edad y es mas elevada entre los individuos de procedencia rural. Tambien podria ser mayor entre los pacientes jovenes anti-VHC positivo, en comparacion con los controles de edad similar. Este hallazgo puede deberse a las peores condiciones higienicas probablemente asociadas con ciertas practicas mas communes entre estos individuos, como la adiccion a drogas por via parenteral y los tatuajes.

Fundamento y objetivo: Es conocido el papel que desempenan los mediadores inmunologicos proinflamatorios en la patogenia de algunas alteraciones de organos distantes en la pancreatitis aguda (PA). El objetivo de este estudio ha sido evaluar la relacion entre dichos mediadores y la afectacion hepatica, renal y pulmonar en pacientes con PA. Pacientes y metodo: En 34 pacientes con PA se valoraron el dia del ingreso, parametros bioquimicos de funcion hepatica, creatinina y presion parcial de oxigeno (PO2) arterial, y se establecieron puntos de corte. Se determinaron el receptor soluble tipo I del factor de necrosis tumoral (sTNFRI), el receptor antagonista de la interleucina (IL) 1 (IL-1Ra), la IL-6, el receptor soluble de la IL-6 (sIL-6R), la IL-18 y la molecula de adherencia intercelular-1 (ICAM-1) los dias 1, 2, 3 y 7, y se compararon entre los pacientes con los parametros en el momento del ingreso (bioquimicos y PO2) superiores o inferiores a un punto de corte establecido. Resultados: Se encontraron valores significativamente mas elevados y mantenidos de sTNFRI, IL-18 e ICAM-1, y de IL-6 solo el primer dia, en los pacientes con algun parametro de funcion hepatica superior al punto de corte. sTNFRI, IL-1Ra e ICAM-1 presentaban al inicio valores significativamente superiores en los pacientes en relacion con la creatinina, y de estos e IL-6 en los que presentaban PO2 inferior a 60 mmHg. Conclusiones: sTNFRI, IL-6, IL-18 e ICAM-1 se comportaron como marcadores tempranos de lesion hepatica; sTNFRI, IL-1Ra e ICAM-1, de disfuncion renal, y estos e IL-6, de afectacion pulmonar.

In this report we describe the results of astudy conducted to better estimate airbornemicroorganisms present in wastewater treatmentplants (WWTP) environments. Air samples weretaken with the Air Sampler MAS 100 impactor(Merck) at each stage of an activated-sludgeWWTP (pre-treatment, primary clarifiers,aeration basins and sludge processors), inorder to determine levels of contaminationaccording to work stations. Culture methodswere used to investigate heterotrophic platecount, moulds and yeasts, total and fecalcoliforms, Pseudomonas aeruginosa andspecies belonging to Mycobacteriumtuberculosis complex, whereas these lastmicroorganisms were also analysed by PolymeraseChain Reaction (PCR) methods. Statisticalanalysis showed that pretreatment and primaryclarifiers were the stages with the highestemission of bioaerosols, and that bioaerosolconcentration at each stage depended on windspeed and daily inflow at the WWTP. Lowconcentrations of P. aeruginosa werefound isolated at pretreatment and primaryclarifiers; however, species of M.tuberculosis complex were not detected at anystage of the treatment plant.

Objetivo: aunque se recomienda vacunar frente al virus de la hepatitis A (VHA) y al virus de la hepatitis B (VHB) a los pacientes con infección crónica por el virus de la hepatitis C (VHC), la estrategia de vacunación más costo-efectiva aún no está establecida. Nuestro objetivo fue comparar la vacunación universal (de todos los individuos) con la selectiva (únicamente de los individuos no inmunizados) frente al VHA y al VHB de los pacientes con infección crónica por VHC en nuestro medio. Pacientes y métodos: comparamos los costes directos de las dos estrategias de vacunación frente a ambos virus en 313 individuos con infección crónica por VHC. Determinamos los marcadores serológicos del VHA (anti-VHA) y del VHB (HBsAg, anti-HBs y anti-HBc) y tuvimos en cuenta los costes de vacunas y reactivos en nuestro ámbito. Resultados: la prevalencia de anti-VHA fue del 81,2% y la de anti-HBc del 24,6%. La prevalencia de anti-VHA aumentaba progresivamente con la edad. La inmunización frente al VHA suponía 19.806,64 € con la estrategia universal y 9.899,62 € con la selectiva. La vacunación frente al VHB ascendía a 18.780 € con la inmunización universal y a 20.385,57 € con la selectiva (mediante el anti-HBc). Se analizaron los costes considerando distintos grupos etarios y diversos factores de riesgo. Conclusiones: en nuestros individuos con infección crónica por VHC la vacunación selectiva frente al VHA es la más costoefectiva. Pero cuando el porcentaje de inmunización frente al VHA desciende por debajo del 20% la mejor opción es la universal. La diferencia en la costoefectividad de ambas estrategias de vacunación frente al VHB es pequeña, a favor de la universal, por lo que en subgrupos con elevada prevalencia de anti-HBc, como adictos a drogas y tatuados, la selectiva podría ser la mejor alternativa.

Because of high fatality rate associated with acute infection by hepatitis A virus (HAV) in chronic hepatitis C patients, it is of interest to know the prevalence of immunization against HAV in these patients.Immunoglobulin G (IgG) IgG HAV antibodies (IgG anti-HAV) were determined in 313 hepatitis C virus antibodies (anti-HCV) positive patients and in 313 anti-HCV negative subjects (control group). Several epidemiological factors were recorded (age, sex, rural vs urban precedence, tattoos, parenteral drugs use, alcohol consumption and surgery).The prevalence of IgG anti-HAV was identical in both groups: 81.2%. However, in those younger than 41 years, this prevalence was greater in those anti-HCV positive than in the control group. Parenteral drugs use and tattoos were more frequent in the first group. The presence of IgG anti-HAV was associated with age and the rural origin in both groups.The prevalence of IgG anti-HAV increases with age, and is more frequent in individuals with rural origin. It was also greater in young anti-HCV positive patients, when compared with controls of the same age. This finding can be due to the poor standards of hygiene probably associated with some practices more common in this population, such as parenteral drugs use, tattoos and others.

6 páginas, 2 tablas.-- et al., [Objetivo]: se trata de comparar prospectivamente el comportamiento durante la primera semana del ingreso de los niveles de interleucina-18 (IL-18), y otros parámetros inmunológicos entre pacientes con pancreatitis aguda con y sin criterios de gravedad, así como entre pacientes con y sin desarrollo ulterior de seudoquiste. [Pacientes y métodos]: se compararon en 36 pacientes con pancreatitis aguda los resultados de sTNF-RI, IL-1Ra, IL-6 e IL-18 los días 1, 2, 3 y 7 desde el ingreso entre pancretitis leve, grave y un grupo control (13 pacientes) con cólico biliar simple, así como entre pacientes con o sin seudoquiste. [Resultados]: al comparar pancreatitis leve con grave, IL-18 fue significativamente superior sólo el primer día en las pancreatitis graves y los otros parámetros a partir del segundo día de forma mantenida. También en pacientes que desarrollaron seudoquiste, IL-18 estuvo significativamente elevada el primer día. [Conclusiones]: IL-18 resultó el marcador más precoz de complicaciones y gravedad de la pancreatitis aguda a nivel sistémico y local (seudoquiste)., [Objective]: our aim was to prospectively compare the behavior of interleukin 18 (IL-18) levels and other immunological parameters during the first week of hospitalization between acute pancreatitis patients with and without severity criteria, as well as between patients with and without late pseudocyst development. [Patients and methods]: in 36 patients with acute pancreatitis we compared sTNF-RI, IL-1Ra, IL-6, and IL-18 levels at days 1, 2, 3 and 7 after hospitalization between mild pancreatitis, severe pancreatitis, and a “control” group (13 patients) with uncomplicated biliary colic, as well as between patients with and without pseudocyst. [Results]: on comparing mild to severe pancreatitis, IL-18 was significantly higher only the first day in severe pancreatitis, while the other parameters were steadily higher after the second day. In patients developing pseudocyst, IL-18 was also noticeably higher the first day. [Conclusions]: IL-18 appears to be the earliest marker of complications and severity in acute pancreatitis at both the systemic and local level (pseudocyst).

There is a role of immunologic proinflammatory mediators in pathogenesis of distant organ disfunction in acute pancreatitis (AP). The aim is to evaluate the relationship between those mediators and liver, kidney and lung disfunction in patients with AP.On the day of admission in 34 patients with AP, biochemical parameters of liver function, creatinine and arterial partial pressure of oxygen (PO2) were determined, and cut points were established. Soluble tumor necrosis factor receptor I (sTNFRI), interleukin (IL) 1 receptor antagonist (IL-1Ra), IL-6, soluble IL-6 receptor (sIL-6R), IL-18, and intercellular adhesion molecule-1 (ICAM-1) were determined on days 1, 2, 3 and 7, and were compared between patients with biochemical parameters and PO2 on admission higher or lower than an established cut point.Levels of sTNFRI, IL-18 and ICAM-1 were significantly higher and sustained, and IL-6 only the first day, in patients with parameters of liver function above the cut point. sTNFRI, IL-1Ra and ICAM-1 early showed significantly higher levels in relation with serum creatinine, and these and also IL-6 in those with PO2 below 60 mmHg.sTNFRI, IL-6, IL-18 and ICAM-1 behaved as early markers of hepatic alteration, sTNFRI, IL-1Ra and ICAM-1 of renal disfunction and these and IL-6 of lung injury.

La práctica de la participación representa un aporte significativo para las y los marginados, al proveerlos de herramientas para que enfrenten su situación con su propio esfuerzo y creen las oportunidades necesarias para su integración social. Estos esfuerzos se han realizado por parte de agencias internacionales, programas de gobierno y organizaciones civiles. Estas últimas, son poco reconocidas debido en parte a que los análisis sobre este tipo de propuestas son escasos, y los realizados en su mayoría sólo describen las experiencias de las organizaciones, pero no el significado de sus acciones, las características de sus integrantes que son quienes desarrollan y contribuyen con los proyectos, el impacto en los sujetos beneficiarios y sus efectos en las comunidades en las cuales trabajan. El presente estudio se encamina en este sentido. El estudio muestra la experiencia diseñada en cuatro comunidades de la Ribera del Lago de Pátzcuaro por la organización civil Jarhuajperakua, grupo que orienta su trabajo a la generación de una auténtica educación popular, que permita a la población, y particularmente a las mujeres, la interpretación de la realidad y la apropiación de diferentes elementos para satisfacer las necesidades que consideran prioritarias. Los alcances e impacto de este proyecto permanecen en el silencio, delineándose como una estrategia de baja intensidad en la lucha contra la pobreza en la región
