The patient had hyponatremia, serum cortisol Discussion We identified 9 articles, including case reports, of new-onset adrenal insufficiency and/or adrenal hemorrhage/infarction on CT in COVID-19. Adrenal insufficiency was hormonally diagnosed in 5 cases, but ACTH levels were measured in only 3 cases (high in 1 case and normal/low in other 2 cases). Bilateral adrenal nonhemorrhagic or hemorrhagic infarction was identified in 5 reports (2 had adrenal insufficiency, 2 had normal cortisol levels, and 1 case had no data). Interestingly, the only case with well-characterized new-onset acute primary adrenal insufficiency after COVID-19 had a previous diagnosis of antiphospholipid syndrome. In our case, antiphospholipid syndrome diagnosis was established only after the adrenal infarction triggered by COVID-19. Conclusion Our findings support the association between bilateral adrenal infarction and antiphospholipid syndrome triggered by COVID-19. Therefore, patients with positive antiphospholipid antibodies should be closely monitored for symptoms or signs of acute adrenal insufficiency during COVID-19.

Primary aldosteronism (PA) is the most common cause of endocrine hypertension. PA subtypes include bilateral hyperplasia and unilateral PA, typically aldosterone-producing adenomas. Adrenal venous (AV) sampling (AVS) is a key step to define PA subtype and guide PA management. According current PA guidelines, most PA patients should undergo AVS, which is a challenging procedure, especially in terms of successfully cannulating the right AV. The aim of this study was to report a single tertiary center experience with AVS in PA patients. We retrospectively evaluated 84 AVS from 1984 to 2019. Sequential AVS was performed by an experienced interventional radiologist. AV and inferior vena cava (IVC) samples were obtained under cosyntropin continuous infusion. Successful catheterization was defined by a selectivity index [SI= AV/IVC cortisol (C) concentrations] ≥5. Unilateral disease was defined by a lateralization index [LI= aldosterone (A)/C ratio in the dominant AV divided by A/C in the non-dominant AV] ≥4. Biochemical evaluation revealed normal K levels, aldosterone (A) of 14.7 ng/dl, renin of 2.1 mUI/L (normal, 4.4-46.1) and A/R ratio of 24.8. Confirmatory testing confirmed PA diagnosis. Hypercortisolism investigation revealed a non-suppressible cortisol after an overnight 1 mg low-dose dexamethasone suppression (8.3 μg/dL), abnormal midnight salivary cortisol, and normal urinary free cortisol, plasma DHEAS and ACTH levels. Computed tomography (CT) scan showed a well-limited mass in left adrenal, measuring 5.8 cm with pre-contrast density of 30 HU and absolute wash-out of 72%. Background: Serum thyroglobulin (sTg) evaluation is mandatory on the follow-up of differentiated thyroid cancer (DTC), as it estimates response totherapy and predicts clinical outcomes. The risk of persistent/recurrent disease positively correlates to sTg, commonly measured by immunoassays.Biochemical incomplete response is defined as a nonstimulated sTg level >1ng/mL and negative image in the absence of Tg antibody (TgAb). False negative sTg on immunoassay tests could occur on the presence ofTgAb. On the contrary, false positive sTg is a very rare condition and it may be suspected when sTg increases despite of non-identified disease. Case report: A 39 years-old female patient with DTC was submitted to total thyroidectomy and neck dissection in 2016. sTg on levothyroxine (LT4) was 0.3 ng/mL and raised to 7.5 ng/mL even after radioiodine ablation. There was no evidence of structural disease on post-ablation whole-body scan, neck ultrasound and 18FDG-PET/CT scanning, defining biochemical incomplete response. sTg on LT4 fluctuated from 3.0 to 22 ng/mL, always with negative TgAb. In the meanwhile, she presented polyarthralgia, puffy hands, proximal muscle weakness and alopecia. Esophagopathy and interstitial pneumopathy were also detected. Mixed connective tissue disease was diagnosed with high levels of anti-U1 ribonucleoprotein (RNP) and antinuclear antibody. Immunosuppressive treatment with corticotherapy and azathioprine was introduced and suprisely sTg decreased from 22.7 to 1.6 ng/mL. ContextPrimary aldosteronism (PA) is the most common cause of endocrine hypertension (HT). HT remission (defined as blood pressure ObjectiveTo determine clinical and molecular features associated with HT remission after adrenalectomy in patients with unilateral PA.MethodsWe retrospectively evaluated 100 patients with PA (60 women; median age at diagnosis 48 years with a median follow-up of 26 months). Anatomopathological analysis revealed 90 aldosterone-producing adenomas, 1 carcinoma, and 9 unilateral adrenal hyperplasias. All patients had biochemical cure after unilateral adrenalectomy. KCNJ5 gene was sequenced in 76 cases.ResultsKCNJ5 mutations were identified in 33 of 76 (43.4%) tumors: p.Gly151Arg (n = 17), p.Leu168Arg (n = 15), and p.Glu145Gln (n = 1). HT remission was reported in 37 of 100 (37%) patients. Among patients with HT remission, 73% were women (P = 0.04), 48.6% used more than three antihypertensive medications (P = 0.0001), and 64.9% had HT duration ConclusionThe presence of a KCNJ5 somatic mutation is an independent predictor of HT remission after unilateral adrenalectomy in patients with unilateral PA.

El metabolismo oseo esta influenciado por diferentes factores y la actividad muscular como un estimulador de la plasticidad osea. Condiciones tales como lesiones nerviosas pueden comprometer la fisiologia osea debido a la inactividad muscular. Estudios previos han demostrado que el dano nervioso reduce la sustancia P y el peptido relacionado con el gen de la calcitonina, tambien conocidos como neuropeptidos que pueden tener un papel clave en la cicatrizacion osea. Este estudio evaluo la oseointegracion de los implantes de hidroxiapatita en defectos tibiales de ratas sometidas a la seccion del nervio ciatico unilateralmente. Doce ratas Wistar se dividieron en dos grupos (G1 y G2). En G1, el nervio ciatico se dejo intacto y en el G2 el nervio ciatico izquierdo fue completamente seccionado. Un defecto oseo tibial fue creado experimentalmente en ambos grupos y se relleno con granulos de hidroxiapatita. Los animales se sacrificaron 2 meses despues de la implantacion y las muestras fueron sometidas a inspeccion macroscopica y el analisis histologico. Se observo buena radiopacidad de los granulos de hidroxiapatita y definicion radiografica del defecto oseo. El analisis histologico revelo neoformacion osea adyacente a los granulos de hidroxiapatita en G1 y, en menor medida en G2, donde la proliferacion de tejido conectivo predomino en el sitio de implante. La neoformacion osea estimulada por hidroxiapatita en defectos oseos se puede esperar incluso en animales con paralisis de los miembros producto de una lesion nerviosa; sin embargo, la formacion de hueso se produce a menor velocidad en estos animales y su volumen es menor.

BACKGROUND: notalgia paresthetica is a subdiagnosed sensory neuropathy presenting as a condition of intense itching and hyperchromic macule on the back that interferes with daily habits. OBJECTIVES: To determine the efficacy of treatment of notalgia paresthetica using oral gabapentin, assessing the degree of improvement in itching and influence on quality of life. Moreover, to evaluate the signs and symptoms associated with notalgia paresthetica. METHODS: We conducted an experimental, non-randomized, parallel, non-blinded study including 20 patients with clinical and histopathological diagnosis of notalgia paresthetica. After application of the visual analogue scale of pain adapted for pruritus and of the questionnaire of dermatology life quality index (DLQI), ten patients with visual analogue scale > 5 were given treatment with gabapentin at the dose of 300 mg/day for four weeks. The other ten were treated with topical capsaicin 0.025% daily for four weeks. After the treatment period, patients answered again the scale of itching. RESULTS: The use of gabapentin was responsible for a significant improvement in pruritus (p=0.0020). Besides itching and hyperchromic stain on the back, patients reported paresthesia and back pain. It was observed that the main factor in the worsening of the rash is heat. CONCLUSION: Gabapentin is a good option for the treatment of severe itching caused by nostalgia paresthetica.

Defects associated with bone mass loss are frequently treated by autogenous bone grafting. However, synthetic biomaterials such as calcium phosphate ceramics can substitute autologous grafts as long as they are biocompatible with bone tissue. In addition, low-level laser therapy (LLLT) is used to enhance bone regeneration by stimulating the local microcirculation and increasing the synthesis of collagen by bone cells. However, bone health is fundamental for osseointegration of the graft and bone repair. In this respect, excessive tobacco consumption can compromise expected outcomes because of its deleterious effects on bone metabolism that predispose to the development of osteoporosis. The objective of this study was to evaluate the regeneration of bone defects implanted with biomaterial and stimulated by LLLT in rats submitted to passive cigarette smoking. Porous hydroxyapatite granules were implanted into critical-size defects induced experimentally in the distal epiphysis of the right femur of 20 female Wistar rats submitted to passive smoking for 8 months in a smoking box. The defect site was irradiated with a gallium-arsenide laser at an intensity of 5.0 J/cm2. The animals were divided into four groups: control (non-smoking) rates submitted (G2) or not (G1) to laser irradiation, and smoking rats submitted (G4) or not (G3) to laser irradiation. The animals were sacrificed 8 weeks after biomaterial implantation. The right femurs were removed for photodocumentation, radiographed, and processed for routine histology. The results showed good radiopacity of the implant site and of the hydroxyapatite granules. Histologically, formation of new trabecular bone was observed adjacent to the hydroxyapatite granules in G1 and G2. In G3 and G4, the granules were surrounded mainly by connective tissue. In conclusion, passive smoking compromised bone neoformation in the defects and the LLLT protocol was not adequate to stimulate local osteogenesis.
